Abstract:
Be to, jis mokė mokinius kaip tapti laisvais ir nepriklausomais piliečiais, gebančiais svarstyti problemas, remiantis savo įsitikinimais.

Abstract:

Abstract:

Abstract:

Abstract:
The article deals with a historical-pedagogical analysis concerning the previous and following years the publication of Lettera a una professoressa, by observing the context and some phenomena with essential elements. Specifically, detailed references are reminded in order to clarify the thought and the work of the prior of Barbiana. The social, economic and political contradictions of the post-war period that reflect in the Italian school education emerge: the class-oriented structure of society, the marks used for selective and justicialist purposes, early school dispersal. Only from the 70s, after the teaching of Latin was removed from secondary school, 1968 and free access to University, education becomes a common good, by bringing about the principles of the Constitution. The close analysis of Don Milani still catches the motivation to give due considerations for education in general, for integration processes and of “I Care” of which the current social context is greatly necessary with its various “emergences”: the importance of language, the dedication to foster self-esteem processes, the choice for the tutoring technique and of cooperative-learning. Based on a visionary ideal of cooperative and tolerant society, the Don Milani method proposes a new humanism by turning into a leading school model. The article includes significant conclusions from the Lettera for educational topicality, for the issues it calls and for the solutions it requires, first of all the pedagogical-social demand of a school for all and everyone which avoids “to make equal parts among unequal”.

Abstract:

Abstract:
Background:IgG4-related disease (IgG4-RD) is generally considered a chronic fibro-inflammatory condition with insidious presentation and subclinical course. Our clinical experience, however, suggests that a sizable proportion of patients experience multiple accesses to the emergency department (ED), either at disease onset or during the disease course. Objectives:In the present study we aimed (i) to assess the prevalence of acute manifestations of IgG4-RD at disease onset requiring referral to the ED, and (ii) to calculate the diagnostic delay from the initial acute presentation. Methods:We revised our database and identified patients admitted to the ED because of symptoms lately attributed to IgG4-RD onset (Group 1) and those that were referred to our outpatient clinic without previous urgent manifestations (Group 2). Acute manifestations were clustered based on the anatomical district affected by IgG4-RD. Epidemiological, clinical, and serological features of Group 1 and Group 2 were compared. Results:The study included 141 patients with IgG4-RD. 76 (54%) presented to the ED at disease onset. The most common clinical manifestations requiring admission to the ED were jaundice (53%), abdominal pain (41%), and fever (10%). Gastrointestinal involvement was the most frequent cause of referral to the ED (71% of cases), followed by involvement of the retroperitoneum (14.5%), and of the nervous system (6.6%). Pancreato-biliary involvement was significantly more frequent in Group 1. Head, neck, salivary and lacrimal gland involvement was more frequent in Group 2. The diagnostic delay was significantly shorter in Group 1 than in Group 2.Conclusion:Clinical manifestations associated with IgG4-RD onset require referral to the ED in the majority of cases. This finding contrasts with the general view of IgG4-RD as a condition with non-acute presentation.References:[1]Bledsoe JR, Della-Torre E, Rovati L, Deshpande V. Abstract:
This study of the complex of San Lorenzo in Milan draws on archaeological and archaeometric evidence. It presents the results of a recent investigation into late antique masonry techniques at San Lorenzo, which looked at stone and brick masonry techniques, the various type of foundation systems and the building techniques responsible for the structure as a whole. The archaeological element of the paper is based on a stratigraphical analysis of the entire complex, including its stone foundations, pavements, brickwork, vaulting and plasterwork; research on bricklaying, stone-cutting and masonry techniques; and the dating of its windows, doors, and frames. At the same time, the archaeometric aspect of the paper comprises the typological categorisation of the building materials identified at the site. These include stones, bricks, and the ingredients of bonding sections, mortar work and plastering. The origins and dating of these materials will also be an important part of the paper. Dating was carried out using TL and 14C methods.

Abstract:
Background:The staging of interstitial lung disease (ILD) is important to monitor disease progression and for prognostication. A disease severity scale of Systemic Sclerosis (SSc)-related lung disease has long been proposed (i.e. Medsger’s severity scale). This scale was mostly developed by discussion and consensus and stage thresholds were not computed by a data-driven approach. Hidden Markov models (HMM) are methods to estimate population quantities for chronic diseases with a staged interpretation which are diagnosed by markers measured at irregular intervals. Objectives:To build a SSc-ILD specific disease severity scale with prognostic relevance via HMM modeling. Methods:A total of 358 SSc patients at risk for or with ILD were enrolled in a discovery (207 cases, Milan1) and in a validation (151 cases, Milan2, Pavia and Rome) cohort. Patients were included if satisfied the following criteria: 1) Diagnosis of SSc according to the EULAR/ACR 2013 criteria, 2) absence of anticentromere antibodies, 3) dcSSc subset or 4) other subsets with either 4a) ILD-related antibodies (Scl70, PmScl, Ku) or 4b) evidence of ILD on HRCT, 5) disease duration < 5 years at the time of the first pulmonary function test (PFT). Serial PFTs were retrieved and the time up to the last available visit -if the patient alive-, or to death due to pulmonary complications, was recorded. HMM were used to estimate the threshold of a 3-stage model (SL3SI, Scleroderma Lung 3-Stage Index) based on PFT functional values (normal/mild, moderate, severe involvement) in the discovery cohort. Survival estimates of the SL3SI model were compared to Medsger’s severity classes estimates and their predictive capability evaluated via the explained residual variation (R2) of prediction errors (the higher the better). One-hundred random replicates were generated to simulate the prediction effort in patients with different disease duration and lung severity. Results:Patients characteristics are summarized in the Table. Fifteen-years survival estimates for Mesdger’s classes in the discovery set were: normal=0.88, mild=0.86, moderate=0.84 and severe=0.71. The SL3SI was defined by the following thresholds: normal/mild, FVC and DLco >=75%; moderate FVC or DLco 74-55%; severe, FVC or DLco <55%. SL3SI 15-yrs survival estimates were: normal/mild=0.89, moderate=0.82 and severe=0.63. Prediction analysis showed a higher R2values at 15 yrs for the SL3SI compared to Medsger’s classes, providing evidence for a better predictive capability of the former (discovery: 0.31 vs 0.25; validation: 0.28 vs 0.19).Conclusion:The SL3SI, a simplified 3-stage functional model of SSc-ILD, yields better survival estimates and long-term prognostic information than Medsger’s classes. Its reproducibility and ease of use make it a useful tool for the functional and prognostic evaluation of SSc patients at risk for or with ILD.Table:VariablesDiscovery (n=207)Replication (n=151)DcSSc62 (30%)98 (64%)Age at first PFR48.6±1249.1±14.4Disease duration at first PFR1.7±1.61.3±2.4FVC90.5±18.191.1±20.2DLco70.7±19.861.3±20.1ILD on HRCT179 (86%)125 (80%)Scl70157 (76%)153 (78%)SSA63 (30%)32 (21%)n of visits38571473Follow-up time, yrs11±5.610.6±5.7Deaths27 (13%)23 (15%)Disclosure of Interests:Alessandro Santaniello: None declared, Chiara Bellocchi: None declared, Luca Bettolini: None declared, Marcello Cassavia: None declared, Gaia Montanelli: None declared, Adriana Severino: None declared, Monica Caronni: None declared, Corrado Campochiaro Speakers bureau: Novartis, Pfizer, Roche, GSK, SOBI, Enrico De Lorenzis: None declared, Gerlando Natalello: None declared, Paolo Delvino: None declared, Claudio Tirelli: None declared, Lorenzo Cavagna: None declared, Giacomo De Luca Speakers bureau: SOBI, Novartis, Celgene, Pfizer, MSD, Silvia Laura Bosello: None declared, Lorenzo Beretta Grant/research support from: Pfizer

Abstract:
Background:the prognosis of Systemic lupus Erythematosus (SLE) patients has significantly improved over time, raising the need for more data about disease activity and damage accrual in the long term. Objectives:to investigate the risk of long term disease activity and to identify viable prognostic markers for disease flares in SLE patients with long standing diseaseMethods:data on SLE patients regularly followed at ASST PINI-CTO, Fondazione Ca’ Granda Policlinico and Ospedale San Raffaele, Milan (Milan Systemic Lupus Erythematosus Consortium, SMiLE, cohort) with disease duration ≥ 20 years, were retrospectively analyzed. Organ involvement as per the British Isles Lupus Assessment Group (BILAG) definitions was recorded along with achievement of clinical and complete remission (CR and CCR: clinical SLEDAI =0, PGA <0.5 and no prednisone or immunosuppression ± negative serology) and lupus low disease activity state (LLDAS) at 15 (T15) and 20 (T20) years of follow up. Damage accrual was estimated according to the SLE International Collaborating Clinics/American College of Rheumatology damage index (SDI).Results:data from 168 patients (table 1) were available for analysis. Remission (CR+CCR) and LLDAS were achieved in 22% and 61% at T15 and 25% and 71% at T20. LLDAS was not associated with a history of involvement in any BILAG domain, but it was inversely associated with treatment with mycophenolate at any time (50 vs 23% treated vs not treated; p=0.02). SDI>0 was found in 49% patients at T15 and in 71% at T20. LLDAS at T15 was associated with lower flare rates in the following five years (HR= 0.395, 95%, CI=0.239-0.653; Figure, left panel; p<0.001). The risk of flaring for LLDAS was largely comparable to CCR and CR (Figure, middle panel). In the T15-T20 timeframe, 37% of patients had a flare. Patients with both low complement and anti-dsDNA positivity at T15 had an increased risk of flaring compared to serologically inactive patients (HR=2.86, 95%, CI=1.572-5.19; Figure right panel). Flaring patients were more likely to show an increase in SDI from T15 to T20 (37% vs 9% in patients with stable SDI; p<0.001)Table 1.Demographic, laboratory and clinical characteristics of patients with SLECharacteristics(n=168)Demographic characteristicsFemale, n (%)150 (89.3)Age at diagnosis years, median (IQR)24 (18-32)Clinical and serological features during 15 years follow up, n (%)Musculoskeletal140 (83)Mucocutaneous135 (80)Constitutional114 (68)Haematological103 (61)Nephritis81 (48)Cardiopulmonary49 (29)NPSLE31 (18)Positive anti-dsDNA136 (81)Hypocomplementemia136 (81)Positive antiphospholipid73 (44)CR / CCR at T1513 (8) / 23 (14)CR / CCR at T2017 (10) / 25 (15)Conclusion:LLDAS is common in SLE patients with long disease duration although up to 37% of patients with 15-year disease duration may experience a flare during the following 5 years. The flare risk increases with failure to attain LLDAS at T15 and with active serology. Late flares associate with damage accrual.References:[1]Aringer M, Ann Rheum Dis. 2019; Franklin K, Ann Rheum Dis 2019Table 2.therapy during the 20 year follow upTherapy (ever)%Prednisone83Hydroxychloroquine91Mycophenolate mofetil33Azathioprine50Methotrexate23Cyclophosphamide36Fig 1:risk of flare according to disease activityDisclosure of Interests:Maria Gerosa: None declared, Giuseppe Alvise Ramirez: None declared, Chiara Bellocchi: None declared, Lorenza Maria Argolini: None declared, Luca Moroni: None declared, Martina Cornalba: None declared, Nicola Farina: None declared, Lorenzo Dagna Grant/research support from: Abbvie, BMS, Celgene, Janssen, MSD, Mundipharma Pharmaceuticals, Novartis, Pfizer, Roche, SG, SOBI, Consultant of: Abbvie, Amgen, Biogen, BMS, Celltrion, Novartis, Pfizer, Roche, SG, and SOBI, Roberto Caporali Consultant of: AbbVie; Gilead Sciences, Inc.; Lilly; Merck Sharp & Dohme; Celgene; Bristol-Myers Squibb; Pfizer; UCB, Speakers bureau: Abbvie; Bristol-Myers Squibb; Celgene; Lilly; Gilead Sciences, Inc; MSD; Pfizer; Roche; UCB, Enrica Bozzolo: None declared, Lorenzo Beretta Grant/research support from: Pfizer

Abstract:

Abstract:
Thirteen architectural drawings by four architects-Bramante, Baldassare Peruzzi, Giuliano da Sangallo, and Antonio da Sangallo the Younger-all dating from the period between early 1505 to 18 April 1506, all except one in the Gabinetto dei Disegni e Stampe of the Uffizi, and all connected with the earliest proposals for the new St. Peter's, are examined to establish their authorship and date and the exact sequence in which they were executed. Beyond that, the chronological alignment of the drawings enables us to follow the process of visualizing and creating a building of an unprecedented type and an extraordinary scale. The ground plans of several small-scale prototypes-such as the Audience Hall of the Piazza d'Oro at Hadrian's Villa, the Oratorio of Santa Croce (a tiny 2nd- or 5th-century structure that stood near the Lateran Baptistery until the end of the 16th century), and the 9th-century San Satiro in Milan-are combined with elements of larger-scale prototypes such as San Lorenzo in Milan and the cathedrals of Milan, Pavia, and Florence in the search for a plan and elevation that are both spacious and structurally sound, that sum up both Roman architectural achievement and the heightened unities of Renaissance church design. The main concern in most of these drawings is the delineation of the crossing, the baldacchino formed by the great piers and the dome they support, protecting the tomb of St. Peter and the altar of the Early Christian church. Although in nearly every drawing some attention is paid to the outer perimeters of the building and its internal spatial divisions, many of those decisions are left in suspense, particularly the question of whether the building is to be centralized or longitudinal. Bramante's main concern was to establish the scale of the crossing, the size and shape of the piers and their distance from each other. This nucleus, constructed up through the pendentive level during his lifetime, set the scale for everything that was to follow. In the absence of a definitive plan attributable to the Bramante/Peruzzi team, the pier designs of Uff. 529 A verso and of f. 1466 verso of the Rothschild drawing book, and the interior of the crossing as it appears in the perspective drawing Uff. 2 A, are the best evidence of Bramante's permanent contribution. The drawings considered here trace the experiments with shape and scale that led to the establishment of these elements.

Abstract:
e20042 Background: Light-chain deposition disease (LCDD) is part of a wide spectrum of plasma cell proliferation disorders. LCDD predominantly affects kidneys by systemic deposition of congo red negative non-fibrillar light chains. Due to the rarity of disease, no prospective data is available, and clear guidelines don’t exist for the treatment. We reviewed data to summarize effective treatment strategies. Methods: A literature search was conducted on PubMed, Cochrane, and ClinicalTrials.gov with final update in November 2020 using terms ‘light chain deposition disease’, ‘therapy,’ and ‘disease management’. Library was manually screened to remove articles on LCDD in transplant kidneys. We identified 63 case reports/series and 11 retrospective studies n=365. Results: Retrospective Studies: 9 studies (n=283) identified various combinations of LCDD and plasma cell dyscrasias given the rarity of idiopathic LCDD. Lorenz et al. reported 6 patients (pts) that received ASCT with MEL preconditioning with 92% improved proteinuria and 95% improved eGFR. Pozzi et al. reported 63 pts (32% idiopathic LCDD, 65% concurrent MM and 3% concurrent CLL) where all pts received a combination of vincristine, doxorubicin, dexamethasone/methylprednisolone. 5/63 patients received ASCT. The median survival was 4.1 years. Bortezomib (V) based regimens: Most experts favor V based regimens to treat LCDD, but no dedicated trials exist. 26 case reports/case series (37 pts) included; 23 pts with idiopathic LCDD and rest with a concurrent disorder. Daratumumab: Milani et al. treated 8 pts with LCDD and concurrent MM with single agent daratumumab who failed at least one regimen. Autologous stem cell transplant (ASCT): 13 case reports/case series (20 pts) identified who received ASCT with melphalan (MEL) conditioning with/without various combinations of chemotherapies before and after ASCT. 14 pts had LCDD, and 6 pts had concurrent clonal cell dyscrasia. Miscellaneous regimens: Various other regimens showed promising results with improved renal functions but not well studied e.g., thalidomide and dexamethasone (Haruyuki et al.). Conclusions: Dedicated clinical trials are difficult given rarity of LCDD but needed. Bortezomib based regimens seem to be good options, ASCT with MEL conditioning is a suitable choice for younger and fit pts.[Table: see text]
