Abstract:
During a median follow-up period of 9 years (ranging from 9 months to 25 years), 24-hour ambulatory electrocardiographic studies were undertaken in 155 patients after repair of tetralogy of Fallot. The patients were divided into two groups. Group A consisted of 76 patients in whom the right ventricular approach was used and group B comprised 79 patients whose repair was through the right atrium. A transannular patch was employed in all patients in group A and in none of the patients in group B. Age at surgery was between 1 and 37 years (median age 4.8 years). During follow-up, 37 patients (48.6%) in group A had significant ventricular arrhythmias (Lown grade 2 or higher) and 13 patients (15.4%) in group B had significant ventricular arrhythmias. A close relationship was observed between age at surgery and Lown grade (R2 = 0.374, p < 0.001) and between follow-up duration and Lown grade (R2 = 0.514, p < 0.001), especially when the two groups were analyzed separately (R2 = 0.502, 0.476, p < 0.001). In contrast, no significant relationship was observed between the ratio of right ventricular to left ventricular pressure and Lown grade or between right ventricular systolic pressure and Lown grade. Discriminant analysis revealed risk factors associated with postoperative ventricular arrhythmias are follow-up duration (partial F = 3.22, p < 0.01), right ventricular to pulmonary artery pressure gradient (partial F = 3.35, p < 0.01), and operative method (partial F = 2.4, p < 0.05). Despite antiarrhythmic therapy, 11 of 22 late postoperative deaths occurred suddenly, presumably from ventricular arrhythmias. In this series of patients, the right atrial and pulmonary artery approach significantly reduced the risk of life-threatening ventricular arrhythmias after repair of tetralogy of Fallot.

Abstract:
Purpose: Tenascin-C (TN-C) and amino-terminal fragment of the B-type natriuretic peptide (NT-proBNP) are the important predictors in prognosis of heart failure (HF). The aim of this study was to analyze the relationship of TN-C and NT-proBNP levels with the frequency and severity of ventricular arrhythmia. Materials and Methods: Our study included 107 HF patients with EF < 45%. According to Holter analysis, the patients were divided into two groups as malignant arrhythmia group (n=29) with Lown Class 4a and 4b arrhythmia and benign arrhythmia group(n=78) with Lown Class 0-3b arrhythmia. The groups were compared with respect to levels of TN-C and NT-proBNP. The relationship of TN-C and NT-proBNP levels with frequency of ventricular premature beat (VPB) was also analyzed. Findings: NT-proBNP (5042.1±1626 versus 1417.1±1711.6 pg/ml) and TN-C (1089±348.6 versus 758.5±423.9 ng/ml) levels were significantly higher in the malignant arrhythmia group than that of the benign arrhythmia group (p

Abstract:
Sudden cardiac death and the heaviest arrhythmic events are connected to ventricular arrhythmias (VA). The issue of optimization of drug therapy for VA remains relevant. Given the possible multidirectional effects of antiarrhythmic drugs on the functional state, in order to determine the effectiveness of pharmacotherapy in patients with VA it is advisable to use a stepwise integrated approach. Aim. Compare the effects of bisoprolol, nebivolol and sotalol on the functional state of patients with VA. Materials and methods. 120 patients with VA of grade IIV based on the В. Lown grading system, III groups based on J. Bigger grading system against the background of essential hypertension or its combination with coronary heart disease took part in the research, they were randomized into three groups for treatment with bisoprolol, nebivolol and sotalol. As part of combination therapy, patients were administered lisinopril, and if indicated, acetylsalicylic acid, atorvastatin. Initially and after 24 weeks of therapy the following was done: quantitative assessment of regulatory-adaptive status by cardio-respiratory synchronism test, treadmill test, six-minute walk test, subjective assessment of quality of life, all-day monitoring of blood pressure and electrocardiogram. Results. With comparable hypotensive and antiarrhythmic effects, therapy with nebivolol had a positive effect on the regulatory-adaptive status, had a better effect on exercise tolerance in comparison with bisoprolol and sotalol, more improved the quality of life, in comparison with bisoprolol and sotalol. Conclusion. In patients with VA against the background of essential hypertension or its combination with coronary heart disease as part of combination therapy the use of nebivolol may be preferable to bisoprolol or sotalol due to a more pronounced positive effect on the functional state.

Abstract:
Aim To develop a method for prediction of high-grade ventricular extrasystole (VE) in patients with chronic heart failure with preserved ejection fraction (CHF-PEF) based on results of an echocardiography (EchoCG) study.Material and methods At the first step, the study included 121 patients of the Cardiology Department, Municipal Clinical Hospital #31, St. Petersburg (calculation group) with symptoms and clinical signs of CHF-PEF (median age, 62 years). For testing accuracy of the developed formula, a control group was formed, which consisted of 42 patients with CHF-PEF (median age, 59 years). EchoCG at rest and ECG Holter monitoring were performed for all patient. The VE classification according to B. Lown and M. Wolf (1971) in the M. Ryan (1975) modification was used. Results of the evaluation were determined by the most significant recorded grade. Grade III or higher VE were considered as high-grade VE.Results Using logistic regression analysis of data for patients of the calculation group, a statistical model was constructed and a respective formula was developed to predict a probability of high-grade VE in CHF-PEF patients depending on the presence of risk factors (EchoCG criteria). According to the obtained data the following factors primarily contributed to the model: interventricular septal (IVS) thickness (p=0.007; Wald=7.44), end-diastolic volume index (EDVI) (p=0.044; Wald=4.13), and the degree of diastolic dysfunction (DD) (p<0.0001; Wald=19.90). For testing the formula accuracy, the analysis was performed in the control group. Based on data of both stages, the following values were obtained: for the calculation group, the method sensitivity was 77.8 %, the specificity was 82.4 %, the accuracy was 81.0 %; for the control group, 81.8 %, 70 %, and 76.2 %, respectively; for both groups together, 79.3 %, 80.0 %, and 79.8 %, respectively. In ROC-analysis of this prognostic model, the area under the ROC-curve (AUC) was 0.852 (95 % CI: 0.776–0.910; p<0.0001) for the calculation group; 0.818 (95 % CI: 0.669–0.920; p<0.0001) for the control group; and 0.855 (95 % CI: 0.792–0.905; p<0.0001) for both groups together, which indicated a good quality of the prognostic model.Conclusion The EchoCG predictors of high-grade VE in patients with CHF-PEF included degree of DD, EDVI, and IVS thickness. The developed method with the constructed formula for prediction of high-grade VE in CHF-PEF patients showed high sensitivity, specificity and accuracy.

Abstract:
Abstract Background: CD27 antibody stimulation of T cells has been shown to activate and promote myeloid cell infiltration leading to enhanced antibody-dependent cellular phagocytosis (ADCP) by anti-CD20 in lymphoma preclinical models (Turaj et al Cancer Cell 2017). In this phase IIa study (RiVa NCT03307746), the safety and efficacy of rituximab (ritux) and varlilumab (varli, anti-CD27) was tested in relapsed/refractory B-cell lymphoma. Transcriptional effects of therapy were investigated by RNA sequencing of pre- and post-treatment tumour biopsies and single cell RNA sequencing of in vitro peripheral blood mononuclear cells (PBMC). Methods: Eligible patients with relapsed/refractory CD20 + B-cell lymphoma were randomized 1:1 to arms A: cycle 1 day 1 ritux, day 2 varli; or B: cycle 1 day 1 ritux, day 8 varli. Cycles 2-6 are identical in both arms (day 1 ritux (cycles 2-6); day 2 varli (cycles 3 and 5); 2-weekly cycles. The primary endpoints were overall response and safety. RiVa was funded by CRUK (CRUKD/17/008) and Celldex Therapeutics Inc. Intratumoral biopsies were taken pre-treatment and on cycle 1 day 7/8 post treatment, i.e. post- ritux and varli in arm A, and post-ritux alone in arm B, and subjected to RNA sequencing, deconvolution by CIBERSORTx and Gene Set Enrichment Analysis (GSEA). To gain further insight into how anti-CD27 stimulated T cells activate myeloid cells, PBMC from healthy donors were treated with varli or an isotype control for 48 h and analysed by CITE-seq (10x Genomics). Results: Twenty-seven patients were randomised; 15 indolent B-cell non-Hodgkin lymphoma cases (NHL) (1 mantle cell lymphoma and 14 follicular lymphoma (FL) grade 1, 2 or 3a) and 12 aggressive B-NHL (9 diffuse large B-cell lymphoma, 2 FL grade 3b and one transformed FL). Median age was 71 (range 49-87), median lines of previous treatment were 4 (range 1-13) and 22% were refractory to the last line of treatment. Thirteen patients completed all 6 cycles of treatment, 1 patient was withdrawn after 1 cycle due to a new cancer diagnosis, 3 patient/investigator withdrawals occurred and 10 progressed on treatment. The overall response rate at the end of treatment was 26.9% (7/26; 95% CI, 13.4-44.7) (indolent B-NHL 26.7% (4/15); aggressive B-NHL 27.3% (3/11)). Within responders, 4 had partial response (PR) (3 aggressive B-NHL, 1 indolent B-NHL) and 3 had stable disease (SD) (all indolent B-NHL), with a duration of response between 2 months to &gt;1 year. Amongst the responders with aggressive B-NHL, one had stage III disease, 3 previous lines of treatment and remained in remission &gt;1 year. Another had stage IV disease, 6 previous lines of treatment including CAR-T cells, and was in remission at last follow up (&gt;16 days). Thirty-three percent (9/27) of patients experienced at least one adverse event graded ≥3 with the commonest being infection (11%, 3 cases). GSEA of post- vs pre-treatment biopsies in ritux/varli-treated (arm A) patients showed pathways enriched in T-cell signalling (normalised enrichment score (NES) 2.47, q-value=0.001) and Fc gamma receptor-dependent phagocytosis (2.05, q=0.001), which were absent in ritux-treated (arm B) patients. CIBERSORTx analysis showed &gt;100-fold increased CD4 T cell infiltration in partial responders compared to those with progressive disease (p=0.027) and a strong correlation between intratumoral B-cell depletion and macrophage infiltration (Fig 1). Responders were enriched in T-cell signalling and Th1 signatures, pre-treatment (NES 2.52, q=0.02). PBMC cultures treated with varli induced CD4 + and CD8 + T-cell effector and memory activation (IFNG, TNFA, NFKB), and monocyte differentiation from classical (CD14 ++CD16 -) into intermediate (CD14 ++CD16 +) and dendritic cell-like (CD14 -CD16 +CD83 +CD86 +) phenotypes. In comparison to isotype control data, receptor-ligand interaction analysis predicted that varli-treated T cells may activate monocytes through the MIF signalling pathway (CD74, CD44, CXCR4). Conclusion: Combined rituximab and varlilumab administration is safe in relapsed/refractory B-cell lymphomas and demonstrates efficacy in patients with T-cell activated tumours. Transcriptomic analysis of pre- and post-treatment samples confirms that varlilumab has in vivo agonistic activity. B-cell depletion by rituximab is dependent on intratumoral macrophages and varlilumab induces myeloid cell activation via a T-cell dependent, MIF signalling pathway. Figure 1 Figure 1. Введение. Несмотря на значительные достижения современной кардиологии в совершенствовании лечения пациентов c ишемической болезнью сердца, ее клиническая форма – острый инфаркт миокарда – является потенциально фатальным событием и причиной смерти среди взрослого населения. Иммунная система реагирует на острый инфаркт миокарда и развитие его осложнений. Считается, что первопричиной аритмий могут быть иммуновоспалительные реакции. Однако механизмы развития этих реакций при образовании желудочковых аритмий остаются малоизученными. Представляется целесообразным изучить состояние системного воспалительного ответа у пациентов с острым инфарктом миокарда с желудочковыми экстрасистолами. Это позволит оценить роль воспаления в развитии желудочковых аритмий, а также определить прогностическое значение маркеров системного воспалительного ответа у данной группы пациентов.Цель. Проанализировать уровни маркеров системного воспалительного ответа у пациентов с острым инфарктом миокарда со стойкими желудочковыми экстрасистолами.Материалы и методы. В исследовании принял участие 351 пациент с ИБС: 185 человек было с острым инфарктом миокарда со стабильным подъемом сегмента ST и 91 пациент с острым инфарктом миокарда без подъема сегмента ST, контрольную группу составили 75 пациентов со стабильной стенокардией. Выборка пациентов проводилась в период с 2015 г. по февраль 2018 г. В исследовании группа STEMI включала 96 (51,9%) жителей города и 89 (48,1%) жителей из сельской местности, группа NSTEMI состояла из 42 (46,2%) жителей города и 49 (53,8%) жителей из сельской местности. Группы были сопоставимы по доле пациентов из города и из сельской местности. Все обследованные лица были сопоставимы по возрасту, социальному положению и полу (соотношение мужчин и женщин составило 4 к 1).Уровни биомаркеров острого инфаркта миокарда определяли при первом контакте. Для определения биомаркеров воспаления кровь брали через 24 часа после начала клинических проявлений острого инфаркта миокарда.Желудочковые экстрасистолы были выявлены при помощи холтеровского мониторирования ЭКГ в течение 24 часов. Выявление желудочковых экстрасистол проводили через 5 дней послеSTEMI. Критерием включения в исследование было наличие желудочковых экстрасистол 1–2 градаций по B. Lown и турбулентность сердечного ритма.Результаты. Самый высокий уровень высокочувствительного С-реактивного белка был в группе пациентов STEMI и составил 10,63 [9,20–13,44] мг/л и достоверно превышал уровень этого показателя в группе пациентов NSTEMI – 8,47 [7,05–10,61] мг/л (р<0,05). По сравнению с группой пациентов со стабильной ИБС, где уровень высокочувствительного С-реактивного белка составлял 4,78 [3,60–6,45] мг/л, в группе пациентов STEMI его уровень был в 2,3 раза выше (р<0,05). Уровень высокочувствительного С-реактивного белка у пациентов STEMI с желудочковой экстрасистолией составил 11,82 [9,86; 14,00] мг/л и был достоверно выше – 9,67 [7,43; 12,00] мг/л – в подгруппе STEMI без желудочковой экстрасистолией (р<0,05). Наибольшую площадь под кривой ROC (AUC=0,695, 95% ДИ 0,624–0,761) среди анализируемых маркеров иммуновоспалительного ответа имел высокочувствительный С-реактивный белок. В точке отсечения >10,05 мг/л чувствительность составила 70,25%, а специфичность – 64,06%. Относительный риск возникновения желудочковой экстрасистолии у пациентов со STEMI при уровне высокочувствительного С-реактивного белка >10,05 мг/л составил 1,646, 95% ДИ 1,273–2,129.Выводы. Системная воспалительная реакция, лежащая в основе значений уровней высокочувствительного С-реактивного белка, фактора некроза опухоли-α и интерлейкина-6, более выражена у пациентов со STEMI, чем у пациентов с NSTEMI. Относительный риск возникновения желудочковой экстрасистолии увеличивается в 1,646 раза среди пациентов с острым инфарктом миокарда при повышении уровня высокочувствительного С-реактивного белка выше 10,05 мг/л. Introduction. Despite significant advances in modern cardiology in improving the treatment of patients with coronary heart disease, its clinical form called acute myocardial infarction is a potentially fatal event and cause of death in the adult population. The immune system reacts to acute myocardial infarction and the development of its complications. It is believed that immuno- inflammatory reactions may be the root cause of arrhythmias. However, the mechanisms of these reactions in the development of ventricular arrhythmias remain poorly understood. It seems appropriate to study the state of the systemic inflammatory response in patients with acute myocardial infarction with ventricular arrhythmias. It will allow to assess the role of inflammation in the development of ventricular arrhythmias, as well as to determine the predictor value of the markers of systemic inflammatory response in this group of patients.Purpose. To analyze the levels of markers of systemic inflammatory response among patients with acute myocardial infarction and permanent premature ventricular contractions.Materials and methods. The study involved 351 patients with coronary artery disease (CHD): 185 patients with acute myocardial infarction with stable ST segment elevation and 91 ones with acute myocardial infarction without ST segment elevation; the control group consisted of 75 patients with angina pectoris. The sample of patients was carried out in the period from 2015 to February 2018. In the study, the ST segment elevation myocardial infarction (STEMI) group included 96 (51.9%) residents of the city and 89 (48.1%) ones of the village; the non-ST segment elevation myocardial infarction (NSTEMI) group consisted of 42 (46.2%) residents of the city and 49 (53.8%) ones of the village. The groups were comparable in the proportion of patients from the city and village. All the surveyed persons were comparable in age, social status and sex (the ratio of men and women was 4 to 1).The levels of the acute myocardial infarction biomarkers were determined at the first contact. The blood was taken 24 hours after the onset of clinical manifestations of AMI to determine the biomarkers of inflammation.The premature ventricular contractions (PVCs) were identified with the help of Holter ECG monitoring that lasted for 24 hours. Identification of the PVCs was conducted in 5 days after STEMI. The PVCs of the 1–2 gradations by B. Lown and heart rate turbulence were the criteria for inclusion in the study. Results. The highest level of high-sensitivity C-reactive protein (hs-CRP) was in the group of STEMI patients – 10.63 [9.20–13.44] mg/l and noticeably exceeded the level of this indicator in the group of NSTEMI patients – 8.47 [7.05–10.61] mg/l (p<0.05). In comparison with the group of patients with stable CHD, where the level of hs-CRP was 4.78 [3.60–6.45] mg/l, in the group of patients with STEMI, its level was 2.3 times higher (p<0.05). The hs-CRP level in patients with STEMI with premature ventricular contractions was 11.82 [9.86; 14.00] mg/l, and it was significantly higher than 9,67 [7.43; 12.00] mg/l in the group STEMI without PVCs (p<0.05). The hs-CRP had the largest area under the ROC curve (AUC=0.695, 95% CI 0.624–0.761) among the analyzed markers of immuno-inflammatory response. At the cut-off point >10.05 mg/l, the sensitivity was 70.25% and specificity was 64.06%. The relative risk of occurrence of PVCs in patients with STEMI for hs-CRP >10.05 mg/l was 1.646, 95% CI 1.273–2.129.Conclusions. The systemic inflammatory response behind the values of hs-CRP, tumor necrosisfactor-α, and interleukin-6 levels are more pronounced in patients with STEMI than in those with NSTEMI. The relative risk of premature ventricular contractions increases by 1.646 times in patients with acute myocardial infarction with the increase of the level of hs-CRP higher than 10.05 mg/l.

Abstract Rapid assessment of pork quality by packers necessitates using early postmortem (∼1 d) traits as an indication of aged pork quality (∼14 d). Efforts have been made to develop a grading system based on color and marbling of the ventral side of boneless loins. In order for this system to be successful, there must be a correlation between early postmortem quality traits observed by packers and the same traits observed by consumers after aging. However, the strength and direction of those correlations are unclear. It is also unknown if the correlations between early and aged postmortem quality differ between barrows (B) and gilts (G). Therefore, the objectives were to determine correlations between early postmortem loin quality characteristics and aged loin quality characteristics, and determine if those correlations differed between barrows and gilts. Early postmortem (∼1 d) quality traits included: instrumental and subjective color, marbling and firmness, and loin pH on the ventral surface of the loin. Loins were aged until 14 d postmortem in vacuum packages. Aged quality traits included traits evaluated early as well as shear force and cook loss. Correlations were compared between barrows and gilts using a Fisher's z test. Overall, early subjective firmness scores of barrows were greater (P &lt; 0.001) than those of gilts. No other early quality traits differed between sexes. Early pH was correlated with aged pH (r = 0.80 B; 0.75 G), ventral lightness (r = –0.57 B; –0.54 G), ventral yellowness (r = –0.55 B; –0.55 G), subjective ventral color (r = 0.55, B; 0.41 G), and subjective chop color (r = 0.42 B; 0.44 G). Correlations of early pH and aged quality did not differ between sexes. Early lightness was correlated with aged ventral pH (r = –0.56) and subjective color (r = –0.39) in barrows but not gilts (P ≤ 0.04). Early lightness was correlated with aged lightness (r = 0.60 B; 0.51 G) and yellowness (r = 0.49 B; 0.55 G), but was not correlated with to any aged chop quality traits. Early marbling was correlated with ventral color (r = 0.42) in barrows and ventral marbling (r = 0.67 B; 0.66 G) and chop marbling (r = 0.57 B; 0.59 G) in barrows and gilts. In summary, early pH and lightness were correlated with aged quality characteristics and correlations rarely differed between barrows and gilts. Sex does not need to be accounted for when relating early and aged quality characteristics.

Using the correlation analysis method to select the key texture extraction in the first step. Then,the classification is realized based on the BP neural network. The train set and test set are randomly chosen from 135 cases. Tests performed show that the proposed method result in a high classification accuracy, which will provide the researcher a valuable opinion on the pork fat content detection.

Defaults arising from illiquidity can lead to private workouts, formal bankruptcy proceedings or even liquidation. All these outcomes can result in deadweight losses. Corporate illiquidity in the presence of realistic capital market frictions can be managed by (a) equity dilution, (b) carrying positive cash balances, or (c) entering into loan commitments with a syndicate of lenders. An efficient way to manage illiquidity is to rely on mechanisms that transfer cash from "good states" into "bad states" (i.e., financial distress) without wasting liquidity in the process. In this paper, we first investigate the impact of costly equity dilution as a method to deal with illiquidity, and characterize its effects on corporate debt prices and optimal capital structure. We show that equity dilution produces lower firm value in general. Next, we consider two alternative mechanisms: cash balances and loan commitments. Abstracting from future investment opportunities and share re-purchases, which are strong reasons for corporate cash holdings, we show that carrying positive cash balances for managing illiquidity is in general inefficient relative to entering into loan commitments, since cash balances (a) may have agency costs, (b) reduce the riskiness of the firm thereby lowering the option value to default, (c) postpone or reduce dividends in good states, and (d) tend to inject liquidity in both good and bad states. Loan commitments, on the other hand, (a) reduce agency costs, and (b) permit injection of liquidity in bad states as and when needed. Then, we study the trade-offs between these alternative approaches to managing corporate illiquidity. We show that loan commitments can lead to an improvement in overall welfare and reduction in spreads on existing debt for a broad range of parameter values. We derive explicit pricing formulas for debt and equity prices. In addition, we characterize the optimal draw down strategy for loan commitments, and study its impact on optimal capital structure.

AbstractWith student loan borrowing becoming an increasingly common experience in U.S. households, it is crucial to understand the interpersonal manifestation of education debt within family systems. This study sought to understand how accruing and repaying student loan debt for one’s own higher education relates to family dynamics and communication within families. Leveraging Family Communication Patterns Theory, this study asked: How do student loan borrowers describe loan-related family communication patterns prior to loan accrual and during the repayment period? Utilizing qualitative and quantitative data collected through a concurrent nested mixed methods study design, findings from this study profile family communication typologies leading up to, and during, student loan repayment. Study findings suggest that the ways in which families communicate about student loans prior to loan accrual and during repayment (a) relate to family financial socialization processes and (b) play at least a partial role in how they experience student loans as part of their overall family dynamics. This study proposes a model of loan-related family communication dynamics and offers implications for future scholarship and practice.

Tujuan penelitian adalah menganalisis kelayakan usaha pengembangan industri ikan tuna loin di Kabupaten Kaur Provinsi Bengkulu. Penelitian dilaksanakan pada bulan Juli-Agustus 2018, dengan metode survei. Pengumpulan data dilakukan dengan metode observasi dan wawancara. Analisis data dilakukan dengan metode deskriptif. Penilaian kelayakan pengembangan usaha industri tuna loin menggunaka 4 kriteria investasi yaitu B/C Ratio, NPV (Net Present Value), Internal Rate of Return (IRR), dan Payback Period (PP). Hasil perhitungan dari 4 kriteria investasi pengembangan industri perikanan tuna loin di Desa Pasar Lama Kabupaten Kaur Provinsi Bengkulu, yaitu Net B/C ratio= 1,13, NPV= 594.414.281, IRR= 16,20 dan PP= 2,9. Berdasarkan nilai criteria investasi tersebut, dengan dukungan ketersediaan bahan baku dan aspek pemasaran, maka pembangunan industri tuna loin di Desa Pasar Lama Kabupaten Kaur Provinsi Bengkulu layak untuk dikembangkan.

The aim of this article is to measure the economic impact of Rural Social Services (RSS) microcredit programme on its participants. Multistage random sampling method was applied to select total 1,008 beneficiary respondents (households) and 504 control respondents (households) from Bangladesh. The propensity score matching (PSM) technique was applied to analyse the outcome of RSS micro-credit interventions. The results show that there were statistically significant differences in income, food expenditure, and non-food expenditure between the programme and non-programme participants. The result of this survey also reveal that after taking RSS loan, 4.34 per cent graduated from category A to B, and 8.41 per cent graduated from category A to C. On the other hand, 30.43 per cent loan takers graduated from category B to C. This result proves that RSS loan helps a certain number of families uplift their income position. The study suggests to further focus on knowledge and skills development of the poor people.

Data were collected using an online course management system. The aim for this paper is a) to identify the difficulties and problems faced by UKM in obtaining venture capital loans. b) Determining the strategy of development of UKM through increased ease of working capital credit facilities through credit channeling institution. Design/methodology/approach : This type of research is exploratory. Types of data used are data collected by primary and save a list of questions. Findings : The results are as follows: More than 50% of UKM have difficulty capital. UKM that utilize external capital sources of credit channeling institution only at 17.50%. Reason UKM have not utilized most of the credit is a factor of the policies, terms and loan interest rates are quite high.. Research limitations/implications : Location of the research done on UMKM in the district of Nganjuk. Practical implications : there are several strategies that can be done are (1) Optimizing the role KKMB in nurturing and mentoring the UKM prospect will apply for business loans, (2) promoting financing for results or venture capital, (3) enhance the role of credit guarantee institutions for UKM prospects will hit their collateral requirements. It is expected that with the implementation of the above strategies, the prospects of UKM no longer have difficulty in obtaining loans from the venture capital Credit Channeling Institutions. Originality/value : UKM provide a significant contribution to employment, amounting to 99.6 percent. Meanwhile, the contribution of UKM to Gross Domestic Product (PDB) by 56.7 percent. This study was conducted to test the effects of a commercial enzyme (with beta-glucanase and pentosanase activities) supplemented into low-protein low-energy barley and wheat based broiler diets on broiler performance. The enzyme was added at 500 g/ton into broiler grower and finisher diets consisting of mainly wheat at 76%, 85% or barley 67%, 75%, respectively. Four dietary treatments were wheat, wheat + enzyme, barley, barley + enzyme. Each treatment had six replications. This experiment was planned according to a completely randomised design by placing ten 14-day-old mixed male and female chicks into one experimental cage unit with wire floor. Cobb broiler chicks were used in this study. Experimental grower and finisher diets were fed to chicks between 14&ndash;28 and 28&ndash;42 days of age, respectively. One-day-old chicks were fed a standard starter diet (23% protein; 12.77 MJ ME/kg) according to NRC (1994) recommendations. Grower diet and finisher diets were formulated to be 10% lower than NRC (1994) with respect to the protein and metabolisable energy content. Body weight, average weight gain (14&ndash;42 days period), feed intake and feed efficiency ratio were measured at 42 days of age. The results of this study demonstrated that the enzyme with beta-glucanase and pentosanase activities supplemented into barley-based broiler diets significantly (P &lt; 0.05) improved body weight by 10%, from 1 779 to 1 958 g, and gain by 12%, from 1 485 to 1 657&nbsp;g, respectively. However, when the same enzyme was supplemented into wheat-based diets, no improvement (P &lt; 0.05) was obtained in body weight and feed efficiency, being 1 723 and 1 677 g and 1 973 and 1 957, respectively for wheat and wheat + enzyme groups. The feed efficiency ratio was also significantly (P &lt; 0.05) improved in barley-based diet from 1.898 to 1.845 by enzyme addition during the 14&ndash;42 days experimental period. &nbsp;

Background : This is a hospital based prospective, cross sectional and interventional case control study conducted in department of urology, Chittagong Medical College, Bangladesh. Materials and methods : Total 120 patient of upper ureteric stone were included in this study divided by Group-A (60) and Group-B (60). Group-A 60 patient under went in situ ESWL and group-B 60 patient under went push back, DJ stenting and ESWL. Number of Shock wave session, energy used (KV) stone clearance, complications of ESWL like loin pain, haematuria, fever and Lower Urinary Tract Symptom (LUTS) and cost were compared between the two groups were recorded and analyzed. Results : In Group A total stone clearance were 96.7% (58/60) and group-B were 98.3% (59/60). But it was not statistically significant (p=0.559). Mean hospital stay of in situ group (Group A) was 1.03 ± 0.181 days compared to 2.57 ± 0.722 day in push back ESWL group (Group B). It was statistically highly significant (p=0.000). In group-A there was loin pain in 20 (33.33%). Haematuria 12(20%), fever in 06 (10%) and LUTS in 16 (26.7%) patients. In group B loin pain was in 35 (58.3%) (p-0.006). Haematuria 26(4.33%) (p=0.06), fever 14 (23.7%) (p=0.05) LUTS was 30 (50%) (p=0.009). More energy of ESWL (Kilo Volt-KV) required in group A than in group B. But is was not statistically significant (p=0.190). Conclusion : Complications between the groups were statistically highly significant. In situ ESWL is very good option for the management of upper ureteric stone. It is non-invasive, effective, safe, cost savings, less hospital stay and usually does not need anaesthesia. Al-Munawwir is the largest Arabic-Indonesian dictionary and the most widely used in Indonesian. And in education world, methods plays a very important role to help learning process. This study aimed at determining the Arabic loan words in Al-Munawwir Dictionary (from letter B to H) and the proposed method to learn Arabic by using those words for beginner students. The research used descriptive analysis method. The result of this study were: In Al-Munawwir Dictionary (from letter B to H), there were some Arabic loan words as many as 206 words. This study also illustrated a proposed method for learning Arabic using those words. The method describes four elements of the curriculum and applied them in dialogues, texts and exercises.

Abstract:
The goal of the paper is to disclose the links between the dynamics of macroeconomic indicators and the level of bank loan rates based on international and Ukrainian practice. On the basis of the previous analysis, the paper also aims to identify the key trends in the formation of loan prices in the long run and identify problematic issues related to bank loan rates. The main characteristics of bank lending rates in Ukraine are: a) their high rates; b) sharp changes in the weighted average bank loan rates from year to year; c) higher loan rates for households compared to the cost of bank loans for businesses; d) higher bank loan rates for short- and medium-term loans versus long-term ones; e) lower rates on loans in foreign currency compared to the loans in hryvnia; and f) high share of non-performing loans to households and businesses in bank portfolios. In the context of world and Ukrainian practice, the paper demonstrates the reverse effect between macroeconomic indicators such as GDP per capita, the ratio of loans to GDP, the ease of doing business index and bank loan rates. The article also demonstrates a direct relationship between the dynamics of inflation rate in the country, the dynamics of non-performing bank loans and their rates.

Tujuan penelitian adalah untuk mendapatkan informasi tingkat kesukaan panelis terhadap warna, aroma, rasa dan teksur dari produk roti berbahan baku subtitusi tepung pisang lowe. Penelitian ini menggunakan metode Rancangan Acak Lengkap (RAL) masing-masing perlakuan yang diulang sebanyak 3 kali menjadi 12 unit perlakuan (4 perlakuan diulang 3 kali hasilnya 12). Adapun formulasi penggunaan tepung pisang lowe dan tepung terigu, yaitu : A = Tepung Pisang Lowe 100g tanpa tepung terigu; B = Tepung Pisang Lowe 75g + tepung tepung terigu 25g; C = Tepung Pisang Lowe 50g + tepung tepung terigu 50g; D = Tepung Pisang Lowe 25g + tepung tepung terigu 75g. Penelitian ini berlangsung dengan 2 tahap, yaitu tahapan pertama pembuatan tepung pisang lowe dan tahapan kedua membuat roti menggunakan kombinasi tepung pisang lowe dengan tepung terigu. Metode analisis penelitian ini dilakukan dengan menggunakan “Skala Hedonik”, yaitu tingkat kesukaan terhadap warna, aroma, rasa dan tekstur. Sampel disajikan secara acak, kepada panelis diminta untuk memberikan nilai menurut tingkat kesukaan. Jumlah skala yang digunakan terdiri dari 5 skala yaitu : 1. Sangat tidak suka 2. Tidak suka 3. Agak suka 4. Suka 5. Sangat Suka. Uji organoleptik yaitu uji hedonik dengan melibatkan 26 orang panelis tidak terlatih. Dari hasil penelitian dapat ditarik menjadi kesimpulan bahwa : Produk roti yang terbuat dari komposit tepung pisang lowe dengan tepung terigu memberikan pengaruh sangat nyata terhadap seluruh parameter yang diamati. Sesuai dengan hasil penilianan tingkat kesukaan panelis baik warna, aroma, rasa, dan tekstur roti komposit dari tepung pisang lowe dengan tepung terigu (50 g : 50g) diterima dan di sukai panelis dengan skala agak suka-suka.

Twelve individually housed West African Dwarf (WAD) Bucks averaging 7.3 ±0.91kg were used to study the effect of varying levels of Bambara nut seed meal on the performance and carcass characteristics of West African Dwarf (WAD) Bucks. Four experimental diets designated A, B, C and D were formulated to contain 0, 10, 20 and 3096 Bambara nut meal (BNM) respectively. The twelve WAD bucks were divided into four (4) groups of three animals each. Each group was assigned to one of the four experimental diets in a completely randomized experiment. Each animal in a group represented a replicate. The experiment lasted 56 days, Data on Feed and dry matter intake, Weight gain, concentrate/gain ratio, dressing percentage and carcass quality was collected. Investigations revealed that incorporating BNM in concentrate diets did not significantly (P>0,05) enhance dry matter intake (405, 416, 395 and 407g/day for diets A, B, C and D respectively), daily weight gain (27, 60, 23, and 42g/day for diets A, B, C and D respectively) concentrate /gain ratio (9.36, 5.62, 10.60 and 8.37 for diets A, B, C and D respectively). Dressing percentage differed significantly (P<0.05) among treatment groups and was higher for goats fed the BNM- based diets compared to the control and the values were 40.56, 43.28, 41.85 and 41.34%for diets A, B, C and D respectively. Meat cut values showed that leg. loin and shoulder of all the Jointed parts (Shoulder, leg. loin, sets and ends) were significantly different (P<0.05) among treatment groups. The values were 13.20, 13.56, 12.12 and 11.44 %; 12.38, 11.30, 13.19, 11.97 %; 13.60, 12.37, 10.91 and 11.31 % for goats on diets A, B, C and D for leg, loin and shoulder respectively. There was significant differences (P<0.05) among treatment means for all parts that constitute the offal (head, skin, feet, full gut and empty gut) except for empty gut. On the average Diet B comparatively, yielded lower offal weight. It can be concluded that goats fed Diet B (10% BNM performed better comparatively in terms of feed conversion ratio, dressing percentage and bone to lean ratio.

This paper attempts a periodization and dialectal attribution of Iranian loan words found in Tocharian A and B, two Indo-European languages attested in c. 10,000 fragments unearthed in Chinese Turkestan since 1892. More than 100 loan words are scritinized and classified in eight sections, according to their origin: Old Iranian (probably issued from the common ancestry of the ‘Sakan’ languages, Khotanese, Tumshuqese and Waxi), three different stages of Khotanese, ‘Śaka’, (the language of the Iranian invaders of northern India), Parthian, Bactrian and Sogdian. Tocharians had dealings with all neighbouring Iranian peoples, but Khotanese and its ancestors clearly exerted the most durable influence. No loan word from more remote dialects (e.g. Persian and Ossetic) can be evidenced. The predominance of war-related and political vocabulary among the loan words and the direction of borrowing, overwhelmingly from Iranian to Tocharian, both point to a political ascendancy of Sakan-speaking tribes, and later of Bactria, on Tocharians.

Mutation of the inositol polyphosphate 5-phosphatase OCRL1 results in two disorders in humans, namely Lowe syndrome (characterized by ocular, nervous system, and renal defects) and type 2 Dent disease (in which only the renal symptoms are evident). The disease mechanisms of these syndromes are poorly understood. Here we identify two novel OCRL1-binding proteins, termed inositol polyphosphate phosphatase interacting protein of 27 kDa (IPIP27)A and B (also known as Ses1 and 2), that also bind the related 5-phosphatase Inpp5b. The IPIPs bind to the C-terminal region of these phosphatases via a conserved motif similar to that found in the signaling protein APPL1. IPIP27A and B, which form homo- and heterodimers, localize to early and recycling endosomes and the trans-Golgi network (TGN). The IPIPs are required for receptor recycling from endosomes, both to the TGN and to the plasma membrane. Our results identify IPIP27A and B as key players in endocytic trafficking and strongly suggest that defects in this process are responsible for the pathology of Lowe syndrome and Dent disease.

Background: Pertussis continues to be an important health issue in Vietnam despite infant vaccination programs. In Vietnam, the incidence rates of pertussis per 100,000 population rose from 0.09 in 2014 to 0.33 in 2015 and to 0.58 in 2017. Macrolides, especially erythromycin, are the treatment of choice. However, erythromycin-resistant cases, caused by transition at A2047G position in 23S rRNA, have been reported in the region. Few data are available on antimicrobial resistance in Bordetella pertussis to guide treatment in Vietnam. We report antimicrobial susceptibility of the circulating strains in southern Vietnam during 2015–2017. Methods: Tracheal aspirates from 263 suspected pertussis cases were subject to multiplex real-time PCR to identify B. pertussis and Bordetella spp. Samples were cultured on Regan Lowe agar with 10% sheep blood containing cephalexin (40 µg/mL) and incubated at 37°C for 10 days. The antimicrobial susceptibilities to erythromycin, azithromycin, clarithromycin, and trimethoprim/sulfamethoxazole were determined using the disc diffusion method (CLSI-2017) on Regan Lowe and Mueller Hinton agar. Erythromycin minimum inhibitory concentrations (MICs) were determined using an E-test. The results were recorded after days 3 and 7 of incubation. Sequencing of the 23S rRNA gene was performed to detect mutations conferring macrolide resistance. Results: Of 263 cases, 119 were positive for B. pertussis (45.2%) by real-time PCR, and 15 of 263 strains (5.7%) were successfully cultured. All 15 isolates were susceptible to macrolides and no heterogeneous phenotype was recorded after 7 days; erythromycin MICs were ≤0.094 µg/mL (Fig. 1). We observed no difference in results generated on Regan Lowe and Mueller Hinton media. However, for testing trimethoprim/sulfamethoxazole, results on were superior, as those on Regan Lowe media were unclear. Sequencing of 23S rRNA identified no mutations known to confer macrolide resistance. Conclusions: None of 15 B. pertussis isolates tested were nonsusceptible to erythromycin and macrolides. Similarly, no mutation at the erythromycin-binding site in the 23S rRNA gene was identified. The low isolation rate of B. pertussis by culture means that few positive specimens were tested for antimicrobial susceptibility. Abstract Multiple tools are currently available to effectively evaluate gilt and sow body condition. Both A-mode and B-mode ultrasound technology can provide accurate measures of backfat while B-mode ultrasound can deliver precise measures of loin eye area or loin depth. More recently, Knauer invented and developed the sow body condition caliper. The sow caliper technology has been implemented in 39 countries across the world. Quantifying the angularity from the spinous process to the transverse process of a sow’s back, the sow body condition caliper has moderate correlations with weight, backfat and muscling. Recommendations for the “ideal” sow caliper score in relation to reproductive performance come from multiple, large scale studies conducted on commercial farms across an array of modern genetic lines. Results suggest a sow caliper angle of 127.5 to 132.5° maximizes subsequent reproduction. Going forward, tools should be developed to automate the estimation of sow body condition. While the sow caliper provides a low-cost phenotype, automating A-mode and B-mode ultrasound also warrant investigation. Perhaps scientists should collaborate with industry experts to speed the development of automated sow body condition tools.

The gamma-radiation resistance of five enterotoxic and one emetic isolate of Bacillus cereus vegetative cells and endospores was tested in mechanically deboned chicken meat (MDCM), ground turkey breast, ground beef round, ground pork loin and beef gravy. The D10 values for B. cereus ATCC 33018 were 0.184, 0.431 and 2.56 kGy for logarithmic-phase cells, stationary-phase cells, and endospores at 5°C on MDCM, respectively. Neither the presence nor absence of air during irradiation significantly affected radiation resistance of vegetative cells or endospores of B. cereus ATCC 33018 when present on MDCM. Irradiation temperature (−20 to +20°C) did affect the radiation resistance of stationary-phase vegetative cells and to a limited extent that of spores on MDCM. Impedance studies indicated that surviving vegetative cells were severely injured by radiation. A dose of 7.5 kGy at 5°C was required to eliminate a challenge of 4.6 × 103 B. cereus ATCC 33018 from temperature-abused MDCM (24 h at 30°C). The radiation resistance of a mixture of endospores of six strains to gamma radiation was 2.78 kGy in ground beef round, ground pork loin and beef gravy, but 1.91 kGy in turkey and MDCM. The results indicate that irradiation of meat or poultry can provide significant protection from vegetative cells but not from endospores of B. cereus.

Abstract:
The aim was to assess the quality and safety of traditional smoked meats produced from the meat of Puławska breed pigs. The research material consisted of two skeletal muscles, namely, semimembranosus and longissimus dorsi, taken from 10 carcasses of Puławska breed fatteners. Smoked meats were produced with a 3% addition of salt, spices and acid milk whey (2%). The products were assessed in terms of their basic chemical components (water, protein, fat and salt), colour parameters in the CIE L*a*b* colour space and the content of contaminants, i.e. PAHs [chrysen, benzo(a)anthracene, benzo(b)fluoranthene, benzo(a)pyrene] and residues of nitrates (III) and (IV). The muscle type had no significant effect on the chemical composition of the products, although a higher content of fat and a lower content of salt were found in loin (by 2.79 and 0.38 percentage points, respectively). Smoked meat from longissimus dorsi was significantly lighter in colour (L*; 60.85 vs. 69.28), whereas smoked meats from ham showed a significantly greater degree of redness (a*; 13.24 vs. 8.75). The degree of yellowness of both products was similar (b*; 8.56 vs. 8.85). The content of 4 PHAs and B(a)P in smoked meats from ham was below the permissible limits for traditional products, which are 30 μg/kg and 5 μg/kg, respectively. In the case of smoked meats from loin, the threshold value (30 μg/kg) for the total of 4 PHAs was exceeded in two products, which affected their relatively high average content in smoked meat from this muscle (48.28 μg/kg). Nitrates III and IV were present only in one smoked meat from ham at the levels of 12 ± 2.7 mg/kg (expressed as NaNO2) and 48 ± 11 mg/kg (expressed as NaNO3), respectively. For the other products (from both ham and loin), the results were below the detection limit (results not presented in the table). From the nutritional point of view, the chemical composition of these smoked meats was beneficial: the protein content was high, the fat content was optimal, the content of table salt did not exceed the permissible limit, and there were only trace quantities of nitrates III and V. As far as PHAs are concerned, some measures should be taken to reduce their level, especially in smoked meats from loin.

A part of fish flesh denaturation was caused by the formation of metmyoglobin, which resulted brownish color. Carbon monoxide (CO) and filtered smoke (FS) will react myoglobine to establish carboximyoglobine the stable form of red figment of tuna flesh. The purpose of this study was to determine the myoglobin changes of tuna flesh by treated CO and FS during frozen storage. The research method was an experimental where the tuna (Thunnus albacores) loin was sprayed by CO and FS, then kept at frozen storage during 4 weeks. Observations carried out at 0, 1, 2, 3 and 4 weeks. The surveillance parameters were: myoglobin content, color and protein profile of tuna loin. The result showed the myoglobin content of tuna loin during storage at 0, 1,2, 3, and 4 week were 31.34 mg%, 29.05 mg%, 31.25 mg%, 36.28mg% and 41.01 mg% respectively, while FS were 18.42 mg%, 20.82 mg%, 24.32 mg%, 34.72 mg% and 52.48mg% respectively. According to the color analysis of tuna loin treated by CO and FS, it indicated that the brightness value L* of the tuna loin treated by CO during storage at 0, 1, 2, 3, and 4 week were 26.33; 43.67; 36; 24.33; and 20.33 resvectively and treated by FS there were 30; 42; 37; 27, 25,and 67 resvectively. The red color based on a * and b * values calculated by oHue, of the tuna loin treated by CO during storage at 0, 1, 2, 3, and 4 week were 47.26; 55.41; 54.42; 46.52; and 42.09 respectively and the tuna loin treated by FS were 50.57; 55.82; 55.61; 50,81 and 40,47 respectively. It concluded that both tuna loin treated by CO and FS classified as red color. Profile of tuna loin protein treated by CO and FS obtained 2 bands indicated by molecular weights of 100.92 kD and 52.05 kD.The myoglobin content of tuna treaded by CO and FS during storage increased so it categorized as a good quality.

Commercial banks have a pivotal role in an economy as they provide easy access for firms to fulfill financing needs and help stimulate economic development. This study aims to analyze the impact of key bank-specific determinants on bank value in Nepalese commercial banks, covering 133 observations from 19 commercial banks over the period 2012/13 to 2018/19. Bank value is measured through M/B and Tobin’s Q. Size, profitability, credit risk, loan, deposit and capital are used as explanatory variables. Panel data regression models have been used for analysis purpose. The results of this paper show that profitability, deposit and loans are major determinants of bank value. Moreover, return on assets and bank deposit have positive effect on bank value whereas loan has negative explaining power on bank value. Thus, this paper concludes that Nepalese commercial banks have to pay special attention for the efficient and effective utilization of assets to increase profits and try to increase the size of deposits to increase loan portfolio. Steps and enforcement actions need to be taken by policy level authorities for effective loan management to minimize credit risk and increase bank value.
