Abstract:
♦ The pelvis acts as a fulcrum for the forces transmitted between the lower limb and trunk especially on twisting and turning movements while running, and in the reverse direction when kicking. Sports injuries around the pelvis are therefore common in weight-bearing sports, such as running, football, rugby, and basketball♦ Injury can occur to the various structures around the pelvis. Bone stress injuries affect the symphysis pubis, pubic rami, femoral neck, and sacrum. Stress fractures are more common in women and may occur as part of the female athlete triad (Box 7.15.1) where there is hypo-oestrogenaemia and low bone density♦ Tendon injuries, including enthesopathies, most commonly affect the adductors, lower abdominals, glutei and hamstrings. Hip injuries can occur as a result of labral tears and femoroacetabular impingement. Sacroiliac joint instability may also cause symptoms especially in the buttock region. Synovitis of either joint may suggest an inflammatory arthritis♦ Pain is the most common symptom. However it may be referred from elsewhere, especially the lumbar spine. Pain may also originate from other systems including the reproductive organs and the gastrointestinal and urinary tracts.

Abstract:
The cervical facet joints are well-documented sources of chronic neck pain and headache. Ultrasound may offer the advantage of visualizing the actual target nerves, which is not possible with fluoroscopy. The relevant structures are located much more superficially than in the lumbar spine, hence visibility of the potential targets with ultrasound is expected to be better than in the lumbar region. Besides the ability to perform diagnostic nerve blocks, ultrasound imaging is expected to increase precision of radiofrequency neurotomy due to the ability to localize the exact course of a facet joint supplying nerve. For practitioners with only little experience in cervical sonoanatomy, we recommend performing ultrasound-guided cervical medial branch blocks with parallel fluoroscopic control first till one gains more experience. Correct level determination with ultrasound as described may be difficult for beginners, and the parallel use of fluoroscopy will help developing a “feel” for the procedure.

Abstract:
Ultrasound (US) guidance has gained recognition in the field of regional anesthesia mainly because of its definite advantage of visually localizing the desired target and also due to perceived benefits of safety, accuracy, and efficiency when peripheral nerve blocks are performed. On the contrary, ultrasonography of the spinal structures may be challenging because of depth, bony acoustic shadowing, and complex three-dimensional anatomy. Nevertheless, US allows satisfactory imaging of the posterior elements of spine column and paraspinal soft tissues. This makes US applicable and practical in the outpatient clinical setting, and thus ultrasonography has been increasingly penetrating into chronic spinal pain management. Perhaps the major advantage of ultrasound-guided spine interventions is the lack of radiation exposure. Lumbar transforaminal injections are frequently performed for managing acute and chronic radicular pain, and US guidance may reduce overall radioactive contamination.

Over the past couple of decades there has been increased awareness that opioid use for postoperative pain in neonates may not result in the best outcomes for these patients. Concurrently, there has been an increased use of regional techniques for postoperative pain in the neonate, in particular epidural anesthesia. The most common technique has been an epidural block via a caudal catheter. Caudal catheters can be used for lumbar and thoracic epidural blocks. The caudal catheter technique allows for a theoretically safer means of placement than the classic, at-level, loss-of-resistance technique.
