Abstract:
ZusammenfassungVaskuläre Malformationen sind eine heterogene Gruppe von embryonalen Gefäßfehlbildungen, welche als venöse, arterielle, lymphatische oder kombinierte Anomalien auftreten können 1 2 3 4. Typischerweise sind diese bereits bei Geburt vorhanden. VMF sind äußerst variabel im klinischen Erscheinungsbild, je nachdem welche Gefäße betroffen sind.Bei vorwiegend lymphatischen Malformationen steht die Schwellung der betroffenen Körperregion im Vordergrund 5. Kapilläre Malformationen treten in der Regel als Naevus flammeus in Erscheinung. Die Erweiterung von Kapillargefäßen führt zu einer permanenten lividen Rötung im Hautniveau des betroffenen Areals.Dieser Fall beschreibt einen Patienten mit einer ausgedehnten kombinierten venös-kapillären und lymphatischen Malformation mit Betonung des Gesichts, des Rückens und der unteren Extremität. Aufgrund der auffälligen Schwellung der Unterlippe wurde er mit der Verdachtsdiagnose eines Melkersson-Rosenthal-Syndroms in unserer Ambulanz vorstellig.

Abstract:
ZusammenfassungVaskuläre Anomalien werden nach der Klassifikation der International Society for the Study of Vascular Anomalies in vaskuläre Tumoren, die als häufigste Entitäten kaposiforme Hämangioendotheliome und Hämangiome umfassen, und vaskuläre Malformationen unterteilt. Die Behandlung vaskulärer Anomalien erfolgt interdisziplinär und basiert, abhängig von der Entität auf Resektion, Sklero‑/Kryotherapie, Embolisation und medikamentöser (antiangiogenetischer und/oder zytostatischer) Therapie. Eine komplette Entfernung ist aufgrund von Ausdehnung und meist diffus infiltrativem Wachstum nur in sehr wenigen Fällen möglich. Seit der Erstbeschreibung der Wirksamkeit in dieser Indikation 2008 gilt Propranolol als etablierte Erstlinienbehandlung für komplizierte infantile Hämangiome. Sirolimus ist ein in der Organtransplantation verbreitet eingesetztes Immunsuppressivum, das den Mammalian/mechanistic-target-of-rapamycin(mTOR)-Signalübertragungsweg inhibiert, auch antiangiogenetisch wirksam ist und daher zunehmend für die Behandlung von vaskulären Tumoren und Malformationen eingesetzt wird. Drei Säuglinge mit vaskulären Anomalien (kaposiformes Hämangioendotheliom, gemischt makro‑/mikrozystische lymphatische Malformation, diffuses infantiles Leberhämangiom) erhielten eine Propranolol‑/Sirolimus-basierte medikamentöse Therapie, ohne klinisch relevante Nebenwirkungen entwickelt zu haben. Obwohl die Verträglichkeit von Propranolol und Sirolimus meist ausgezeichnet ist, erfordern schwere, mitunter lebensbedrohliche Nebenwirkungen (Propranolol: kardiovaskulär; Sirolimus: infektiös), die in Einzelfällen berichtet sind, besondere Beachtung.

Abstract:
In einer retrospektiven Studie über den Zeitraum von 1990-2000 wurden in der Klinik für Kinderchirurgie des Universitätsklinikums Charité, Berlin die Kinder untersucht, bei denen eine lymphatische Malformation aufgetreten war. Es ergaben sich insgesamt 18 Patienten. Parallel wurden aus der Klinik für Gynäkologie und Geburtshilfe, Abteilung für Pränatale Diagnostik und Therapie des Universitätsklinikums Charité, Berlin retrospektiv für den oben genannten Zeitraum 31 Patientinnen untersucht, bei deren Feten ein zystisches Nackenhygrom im Verlauf der Schwangerschaft diagnostiziert worden war. Ziel der Arbeit war es, die geeigneteste Therapieform in der Behandlung von lymphatischen Malformationen im Kindesalter in besonderen Hinsicht auf die Prognose, Rezidivfreiheit und Spätergebnisse zu untersuchen. Parallel galt es zu evaluieren, ob in Anbetracht der pränatalen Diagnostizierbarkeit lympahtischer Malformationen und dem postnatal sich daraus ergebenden Krankheitsverlauf eine Schwangerschaftsunterbrechung in Erwägung gezogen werden sollte.<br>A ten year retrospective study of 18 children with lymphatic malformation and 31 pregnant women with fetal cystic hygromas was carried out at the Department of Pediatric Surgery and the Department of Obstetrics and Gynaecology at the University Hospital Charité, Berlin. The objective of this study was to analyze the most effective therapy for lymphatic malformation in children with regard to the children prognosis, the recurrences after treatment and the long-time-follow-up. Another object to analyze was the point of prenatal diagnosis of fetal cystic hygromas and the adverse fetal outcome.
