Full textAlzheimer’s disease (AD) is the most common cause of dementia in the elderly. A cure has not yet been found, but studies show that the combination of pharmacological and non-pharmacological therapies (NPT) can slow down the progression of the symptoms of the disease. Among the NPT, active music therapy (AMT) has been widely used to help relieve the symptoms of dementia. However,most AMT require constant supervision or guidance so that the person with AD can participate in the therapy session and perform it correctly. The use of technologies specifically designed to meet the needs of this population could allow them to autonomously participate in AMT sessions.This master’s thesis is organized into two articles, the first is a literature review regarding the application of AMT in AD. The second article presents the co-design process of an oversized musical keyboard that can guide users with mild AD through AMT sessions that integrate musical, cognitive and motor tasks. Further studies should evaluate the effects of using this device on different symptoms of Alzheimer’s disease. However, the results presented in the first article of the thesis and the observations from the interactions between the participants (with mild AD) and the prototypes provided during the co-design process indicate that the use of this keyboard has the potential to alleviate symptoms related to AD and, thus, improve the quality of life.

Full textThe purpose of this study was to investigate the particular role played by the executive functioning in the driving activity. The objectives of this research were indeed to study how the executive functions would evolve in normal cognitive aging and in the early stages of Alzheimer's Disease (AD), and then to study how these evolutions can be evaluated in driving situations. This study took place in the theoretical frame suggested by Miyake et al. (2000). Indeed, our methodology consisted in a neuropsychological evaluation of three executive components, mental flexibility, updating and inhibition. This neuropsychological evaluation was combined with an experiment in a driving simulator. We developed four driving scenarios Each one evaluated one of the three executive components and the fourth one was a natural driving situation, with left turning intersections. However, in reality, this situation of left turning is a situation which is particularly dangerous for older drivers. The results indicated that the three executive components did not change in a similar way in normal aging, which would preserve for a certain time the mental flexibility. In AD, the three executive components were impaired. In addition, the driving experiments indicated similar and correlated results to those obtained with the neuropsychological evaluations. The driving situations developed in the simulator are thus relevant for the evaluation of the three executive components in an ecological manner. Lastly, the results of the fourth driving experiment showed that the executive components are essential to drive safely. Finally, this study offers future research possibilities concerning the improvement of our knowledge on driving cognitive functioning. Interesting prospects are also offered concerning the possibilities, not only for evaluation, but also for driving readaptation

Full textPragmatic analysis of communication in patients suffering from alzheimer's disease within a communication situation where several themes are introduced in different forms of conversational exchanges shows the influence of a certain number of factors on these patients' communicative capacities : these factors may be personal, psychosocial, cognitive, linguistic or contextuel. In addition, the influence of these different factors brings about a modification of means of communication - both verbal and non verbal-acts-in particular by simplifying them and making them more automatic. The influence on communicative capacities of different factors related at the same time to normal and to pathological aging, with both its'organic and psychological consequences, makes it necessary to exercice caution as to the diagnosis and prognosis of alzheimer's disease, even more so as the problems of communication engendered may increase even further intellectual decline. However, consideration of these factors and the resulting modifications in communication methods may allows us to envisage a therapeutic approach to communication problems shown by patients suffering from alzheimer's disease

Full textIntroduction : Group cognitive-behavioural therapies (GCBT) became, last decade, the intervention of choice in terms of efficacy, cost/benefit balance and feasibility in Alzheimer disease's (AD) family caregivers. They are mainly based on psycho-education and problem solving sessions. However, clinically, the benefit of CBT remains weak. Losada et al (2004, 2006) showed that programs including also an exploration and modification of dysfunctional thoughts about caregiving sessions (DT) improve the CBT efficacy, particularly in decreasing the level of depressive symptoms. Method : The aim of this study was to evaluate the efficacy of two combined spouse-patient interventions compared to a classical neurological follow up without any intervention in caregivers: 1) Group intervention program PRISMA (cognitive-behavioral therapy in spouses, focused on dysfunctional thoughts about caregiving, associated to a cognitive stimulation in patients) 2) Group intervention program ETNA (psycho-education in spouses, associated to a cognitive stimulation in patients), compared to a 3) Classical neurological follow up in patients without any intervention in caregivers. A secondary purpose was to explore the differences between spouse and child caregivers. 50 patients, 50 spouses and 10 adult children were included in this study. Spouse and patients evaluations were realized at the inclusion, at 6 months and 12 months later. Results: The results of this study show the benefit of a cognitive-behavioral intervention in spouses, focused on dysfunctional thoughts about caregiving. Associated to a cognitive stimulation in patients, both for spouses and patients emotional state. Depression is significantly reduced in spouses. Depressive and anxious symptoms also decrease in patients. Furthermore, spouse caregivers report significantly more burden and perceived stress at the inclusion. Dysfunctional thoughts about caregiving are also significantly more present in spouses. Conclusion: This study shows the efficacy of a combined spouse-patient intervention program (cognitive-behavioral intervention in spouses, focused on dysfunctional thoughts about caregiving, associated to a cognitive stimulation in patients) to improve their emotional state
