Full textThe work presented in this thesis is the result of two research strategies. - On the one hand, a structured critical review evamines several major psychosomatic theories ; - On the other hand, the clinical approach that was used concerned a long-term (3 years) psychological investigation of patients with Crohn's disease. From the theoretical perspective, we have shown that the "psychosomatic" model itself has been constructed in priority with respect to the medical model. These various theoretical constructions investigated designate, broadly speaking, the psychological dysfunction as the "causal event" of the somatic disorder. The results of the clinical analysis yield two main points. At the outset, one may first describe an initiation of psychological activity (corresponding to "work of mourning") by the patient, which is specific for the somatic symptom involved. A comparison with a control group furthermore reveals that the appearance of such psychological activity, in those patients who do so, is accompanied by specific behaviour patterns related to the management of that symptom, resulting in a reduction of the number of days of hospitalisation per year. This investigation, as a whole, raises the issue of the role of psychology in the medical sphere. Key words : psychosomatic theories, Crohn's disease, clinical approach, clinical psychology

Full textThe renal insufficiency is a chronic disease, which, reached certain stage requires to the patient who is affected, to resort to a treatment of substitution allowing to mitigate the failures of the function of the kidney. One of these techniques, the hemodialysis, includes three important stages in the route of the sick person: the announcement of the disease, the creation of a vascular access, the passage in the treatment. The announcement of the renal insufficiency often takes place some years before the intervention of the artério-venous fistula and thus good upstream to the stake in dialysis. A longitudinal study would have allowed to proceed to the complete follow-up of the renal insufficient person from the discovery of the disease but the constraints of time drove me to limit me to the effects afterward of the announcement and to concentrate on the echoes of the trauma at the time of the first burglary (creation of the artério-venous fistula) and of second burglary (the stake in dialysis). Towards the psychoanalytical theories of the trauma, I tried to bring to light the impact of the physical burglaries on the psychosomatic unity of the person affected by renal insufficiency which differs from that of the psychic burglary of the trauma excluding a physical wound. I took back the hypothesis of Freud concerning the war neurosises which stipulates that the physical achievement protects from the traumatic neurosis, but I suggested modifying the content of its comments by adapting them to the situation of serious illness. Consequently, the aim of this research is to study the cours) of insufficient renal in three time traumatic of the disease in its at once psychic and somatic dimensions. To feel my hypotheses, a clinical consultation is proposed during the first interview, then projective tests (TAT and Rorschach) accompanied with a clinical consultation are proposed during the second meeting. My population consists of eight patients from 22 to 60 years old that I met first time during the creation of the fistula venous artério, one moment in the course of which I was able to raise that the announcement of the renal insufficiency had not played the role of an anxiety alarm in the psychic economy. Because indeed, upon the arrival of this new foreign body, an impulsive overflowing appears updating a fantastical of traumatic chap. The second phase is situated at the beginning of the sessions of dialysis when the sensoriality and the sensations are put in the service of a "somato-psychic distress " a feeling of disturbing strangeness and a difficulty of demarcation of the physical borders. On one hand, the individual analysis of the results reveals that the preventive value of the announcement of the disease some years before the intervention was decreased by the violence of the physical burglaries. On the other hand, the analysis of the general tendencies brought to light an embrittlement of the narcissistic investment and the objectals marks at three levels: sensory, sensual, of the representation. Several answers or couples of answers in front of the traumatic event which can belong in regredient and\or progredient dimensions joining a singular story and expressing himself through a psychic functioning appropriatefor each, were able to be brought out as: the inhibition of the thought, the rationalization and the intellectualization, the masochism (mortiferous or guard of life), the denial and the not structural split(cleavage) of the Me, the neurotic operating processes, the solutions ( emotional repression) and the unsuitable solutions.Finally, the examination of the psychic functioning in the projective tests does not allow to establish of direct link between the mental organization and the potentialities of reorganization further to the impact of the trauma understanding a physical achievement

Full textOur interest is on the first place the determination of the status of auto-immune diseases. This opens the question of the relationships between the biological individuality and the subjectivity. Are auto-immune diseases the manifestation of an unrecognizing-destructive SELF? Or are they just the result of a genetically determined dysfunction of integrative and communicational organic systems? The Self metaphor shows a certain historical utility, an epistemological tool. There is still a third possible reasoning: disease is always, in part, subjective; human nature is cultural. But the modern medicine's scientific quest of a diagnostic and therapeutic purity denies the subjective component of disease. Though an historical and epistemological approach, we established a splitting of individual aspects during the 2nd half of 19th century, moment of separation between "substantial" aspects that will become immunology, and "subjective" aspects, i. E. Psychoanalysis. The end of the 20th century will see a shift in the immunological speech. It will no longer be a matter of self organisation, protection against foreign, but a communication system within an integrated organism. However, this view won't be an answer to the auto-immune disease paradigm. Based on a clinical experience, we argue that a technological approach of modern medicine underestimates the disease concept. Full textThe concept of somatization is a difficult one. Different definitions for somatization and for somatoform disorders account for differences in prevalence, comorbidity, and risk factors. This study evaluates several classifications (dsm-iii-r, dsm-iv, icd-10) and operational definitions for these disorders. Moreover, their prevalence in a group of patients presenting with multiple unexplained somatic symptoms, their comorbidity with other mental disorders and their vulnerability factors are studied. Somatizers are known for the frequent consultations and medical interventions they require, and for their general insatisfaction with medical diagnoses or with the absence of an organic diagnosis. The evolution of their symptoms and disorders is difficult to assess. It is not clear which criteria should be used to determine whether a patient is improving. Although our patients were treated extensively and received somatic as well as psychiatric treatment in many cases, their symptoms often persisted and their attitude towards these symptoms didn't change

Full textThe different trends which study the psychosomatic question all work on the principle that isolates a determining factor (biological, psychological and/or social factors) which is at the root of the psychosomatic disease. However, the reversal occuring in the process of causality made by the freudian discovery shows the existence of an "epistemosomatic lack". Our work questions the function of the psychosomatic phenomenon in the subjective structure basing ourselves on the hypothesis of the unconscious. It implies that we consider the very nature of the link between psyche and soma. We show that in the neurosis the psychosomatic phenomenon (P. P. S. ) comes under a particular configuration of the process of appropriation of the body (a defficieny of the subjective and fatherly metaphor) and that it is different from the symptom of hysteric conversion. The P. P. S. Which implies a noticeable lesion appears when the subject is confronted to an unbearable event which goes beyond the defensive capacities of the ego. As a 'narcissistic crossing', it strengthens the ego where it fails and keeps the other at a distance with an identification of second type. The P. P. S. Corresponds to the erogenousness of an organ, as a specific pleasure, in the form of self-eroticism. It is at the crossroads of the real, the symbolism and the imagination, three topics that the Œdipus complex has for goal to make stand together. In this way, we think that the psychosomatic phenomenon serves as a substitute for the name-of-the-father. In the psychosis, the psychosomatic phenomenon acts as a substitute linking the organism and the body together. Our methodology is based upon the study of a special case and the primacy of the word. The interviews done with patients carrying a dermatosis have revealed the unconscious dimension of the studied phenomenons and the interface which is necessary between medicine and clinical psychology

Full textAccording to World Health Organization, headaches are one of the first twenty causes of disability in the world and affect 15 % of the world population. A lot of psycho-medical researches have raised the links between headaches and depression, anxiety or stress (Lantéri-Minet et al., 2007). Psychosomatic theory considers migraine as a somatization which must be thought according to the general psychosomatic functioning of each person (Debray, 1997; Marty, 1951). In addition, some promising but general work on attachment theory in chronic pain and migraine show a correlation between attachement and perception/expression of pain doctor/patient relationship and the subject's commitment to care (Maunder, Hunter, 2008 ; Mistycki, Guédeney, 2007; Wearden, Lamberton, Crook, Walsh, 2005).Two researches with migraine patients at the Besançon Hospital were conducted: a quantitative study (N=101) and a quantitative-qualitative longidunal study (N=32).Our quantitative results highlight that migraine patients have a specific psychological profile, different from the general population on all indicators measured (anxiety, depression, stress, social support, psychological distress and alexithymia), and that is the same results that the symptomatology is chronic or episodic. A majority of patients have an insecure attachment profile and uncertain mentalization’s abilities. The case studies help to refine these results and to appreciate the place of migraine in the psychosomatic functioning of each person.The results lead us to propose a modeling of the evaluation of the place of migraine in the global psychosomatic functioning. This must be related to the inter-subjective, groupal and societal context in which the subject fits. This modeling will make it possible to propose the best therapeutic option to each patient, to arrive at an individualized care in case by case

Full textSpite of scientific progress, physicians and psychotherapists still have to face substantial hindrance to prevent and to heal breast diseases - mainty breast cancers - and also to confront their emotional outcomes. Besides physiological, cellular and genetic dysfunctions, breast diseases also threaten an organ that plays an essential role in human's early psycho-emotional experiences and development. The real breast illness, or its phantasies, disturbs women's representations of their own femininity and maturity, and those of their maternal and erogenous competence. The permanent spitting between the conceptions of an anatomical physiological breast - as considered by medical practice and research - and those of an erogenous-imaginary breast - as conceived by psychoanalysis - is a tough difficulty for a medical and a psychotherapeutic work with women having breast diseases, or a familial back-ground of this pathologies. The consideration of the dynamic links between the real breast and the imaginary breast enables the development of specific knowledge and clinical strategies to overcome those difficulties and also to prevent their disturbing effects on breast cancer early detection and on breast diseases' follow-up

Full textThe present work is based on a theoretico-historical, psychological, and psychosomatic study. The first theoretico-historical part which has 4 chapters (forming the first and second volumes) constitutes the basis of the whole study. Referring to Freud's (chap. 1), m. Klein's (chap. 2),H. Deutsch's (chap. 3) and m. Bonaparte (chap. 4) work, the study traces and reveals the place the breast is given in the psychoanalytic theory. In these 4 chapters we have particularly focused on two main issues, namely seduction, because the breast could be considered as the first agent of seduction, and precocious identifications, due to the presumed effects of the identification with breasts on girls' psychosexual development. The second part, consisting of 2 chapters collected in the third volume, presents a psychological study based on 39 interviews with women showing how girls wait for and react to the growth of their breasts (chap. 5) together with a study on graphic and verbal messages (chap. 6) enabling one to reveal the eternal character of the representations of the breast not only as a symbol of seduction but also as a symbol of protection, separation, and loss. The third part which constitutes the fourth volume (chap. 7) presents and discusses 10 clinical observations of women having major or minor mammary disorders. In both cases, the observation of a coincidence between a certain type of loss resulting in a narcissic injury and a somatic reaction suggests that the breast is sensitive to situations which elicit feelings of helplessness, loss of love and loss of self-esteem

Full textCancer patients experience extreme conditions of existence requiring a specific form of psychotherapy. The cancer and essential anticancer treatments together form the cancerous situation, invaded by the cancer’s violent, processual logic. This situation can constitute a sea change in the subject’s life, sweeping away their most firmly held convictions and the most deeply rooted foundations of their narcissistic identity and character. The subject is at major risk of suffering a breakdown. In addition, the ‘actual’ trauma draws strength from the revival of primary traumas up until now extinguished. Their return takes the form of hallucinatory sensations producing states of body and perception of the external world psychologically unrepresentable. The subject sees and experiences themself passively, without really being capable of feeling affected. Deadly dullness, inertia and pain dominate or, on the contrary, the subject feels vividly alive, hypersensitive, invaded and persecuted, at the risk of bursting. The world essentially revolves around this bipolarity, which the radical operational defence system covers in whiteness. It throws its mantle over the world, relationships, objects, affects and the subject’s body. Sometimes nothing can touch the subject at all, and their psyche is reduced to its archaic core, at the edge of psyche-soma. The cancer eradicates subjectivity, steeling away the active principle of primal logic. This sometimes lasting state of white melancholy is but one phase in a polymorphic melancholic processuality. This force dominates the subject’s life, with a tendency either to proto-melancholy or proto-mania, which replace the affective world by a propensity to inertia or psychomotricity. The aim of melancholy work is to revive the deadlocked capacity for identification. We will examine the specific use of transference and, in particular, the key implementation of a basic formal transference. This works based on the dual primal activation of formal signifiers shared by the therapist and their patient. The therapist may be subject to motions of lethargy and psychocorporal excitations, plunging the therapeutic relationship into crepuscular states, relative to the cancer and to the return of primary traumas. We will look at how the principle aim of psychotherapy is the psychological use, and not eradication, of the cancer object, as a means of forging the conditions rendering the therapeutic context feasible: allow the representation of states of body, restore symbolisation, imagination and recourse to unconscious fantasy. Together, these form the essential basis of psychotherapy for cancer patients

Clinic examination of this hypothesis may be understandable by a psycho-analysis psychosomatic theory. Meanwhile, the economic aspect of P. Marty psychosomatic theory (operational life, bad mentalization, essential depression) does not explain how some nevrotic patients, with good mentalization, have organic diseases after addiction stoppage. The ego function have to be reconsidered in this relation with the self, primitive form of psychics processes built with narcissic cathexis. Early traumatism during the maternal nursing cause a structural splitting on a (somatic) perception. The author sollicits being a psychic “pre-psychic” traumatism, or in other words, a narcissic traumatism - excitation in excess – disorganizing psychics process, prevent those ones to be organized: a part of this psychics don't leave a chimic or somatic form. This psychic ”pre-psychic” trauma realised a psychic crash: the ego doesn't settle enough self space, and primary masochism failing

Hence the importance of pinpointing the place of the body in psychoanalytical studies, more exactly that of the body entity in allergic responses. Thus our thesis explores the connections between the "body" and the motives of the unconscious that derive from an allergic alternative. As a result, an approach of the immune system designed as an "identity card" is put forward. Such an identity implies that the "allergic alternative" is a kind of subjective act of aggression against the system. In the light of this hypothesis, our metapsychological interpretation method of investigation proves to be basic and enables us to consider more closely the incumbent role of the unconscious in the building up of the connection between body and psyche and, similarly to analyse the issue of the somatic response through the metapsychological method

Full textThis research aims to question the place of the "somatopsychic expression" in its "proto defensive" value within the psychosomatic behaviour of adult subjects suffering from a persistent non-controlled moderate to critical asthmatic symptomatology. The first part provides a theorical review medical and educational point of views. It also offers theories in the psychosomatic psychoanalytic field. It concludes with an epistemological reading. The second part details the research methodology wich is themes transversal, longitudinal, evaluative and therapeutic, and supports by various research tools (psychometric assessments, projective tests and therapeutic consultation). To understand somatopsychic phenomenon, four hypotheses and their operationalization are presented : the capabilities of symbolization, the defensive mechanisms, the weight of the trauma, and the evolutionary process within the subjectification process of the subject suffering of unstabilized asthma. Also, the clinical analysis focuses on identifying the capabilities of the archaïc, primary and secondary mentalisation of each subject, including the levels of somatopsychic, characterial and mental connections. The synthesis presents the observation of three ways of mentalisation (discontinuous, unstable, irregular) that allow for a more in-depth examination of the understanding hypothesis of this same phenomenon. Finally, a third part presents the analysis of two monographic publications in order to develop the theoretical-practical thoughts, especially on defensive mechanisms implemented in this situation of chronic disease. Full textThe aim of this research is to examine the failures ofself-perception regarding the somatic signs. One such failure is, for instance, the relatively well known fact that some people take much more time than others to become aware of their own bodily signs. We have chosen to interpret this negative phenomenon in the relationship of an individual and his body as a kind of silence, a silence spread over the normal metabolism's sounds. Looking forward to understanding this apparent absence of self-perception in a continuum of phenomena regarding the bodily self-perception, we decided to examine it by comparison with another phenomenon that could in fact be considered as its opposite pole: the curious power of dreams to amplify minimal bodily changes. This power was qualified as "hypochondriac" by Freud. Our next step was thus to examine two types of clinically observable facts: the excess of signs perceived by hypochondriacs and the complete absence of perception in some organically ill patients. The opposition of these two phenomena represents the main line of our theoretical discussion. As a tentative hypothesis, we supposed that the bodily cathexis observable in hypochondriac patients could be considered a necessary condition for their amplified self-perception. A thorough discussion of the freudian notions of auto-erotism and narcissism is then required to understand how could they explain the hypochondriac self-perception. Full textThis thesis is orientated by the suffering felt by the subjects with a psychosomatic phenomenon (PSP). My clinical practice at the surgery unit of the public hospital is basis toward PSP on a somatic disease. This enigmatic character of the phenomena evokes well the abutting of medical and psychological discourses. Usually, these discourses bump into PSP, and these patients are rapidly qualified, like psychosomatic. This diagnosis avoids the caring, and patients are disqualified (deficit of mentation or maladapted emotion), without care’ solution. More this reality is a problem for Public Health. Some theories considered psychosis unable to have some somatic troubles. But how explain they died ?To discuss theses theoretical and descriptive positions, this thesis has decided to turn back to the practice, with some patients suffering from PSP. Usually about PSP, the interpretations are focusing on the signifier and the object “a”, which is an object of Pulsion. Something is writing unconsciously, in the repetition in number.Problematic leads us to posit an hypothesis: “With a subject of psychopathologic structure, psychosis or neurosis, the clinical practice of PSP at the hospital, is a clinical practice of “trait unaire” which can moved in clinical practice of letter”.Methodology adopted is praxeology, an articulation between practice and theory. Studies of cases illustrate this way. The paradigm is psychoanalysis.Hypothesis is corroborated by the clinical practice. An other interpretation is efficiente with PSP, “trait unaire”. The repetition in number stop and PSP disappears. This psychotherapeuty preserv the subjective answer of PSP, limit to the Jouissance and authorize an other subjective position, with the different psychopathologic structures

Full textSudden infant death syndrome (SIDS) is still one of the major causes of post-neonatal death in industrialised countries. Apparent life-threatening events (ALTE) also often occur during the post-neonatal period and we may establish a relation between these two disorders. Although there may be many hypotheses about causes of these tragedies, they often remain unexplained. These disorders have hitherto been the domain of organicist medicine and has been little explored from the viewpoint of the family psycho-affective environment. This thesis is based on this question. The theoritical framework of the thesis is psychoanalysis, the psychosomatic theory of the Paris School and haptonomy or the science of affectivity. We tested five ethiological hypotheses. They refer to the organicist medical approach, the "explosive cycle" concept, transgenerational transmissions, the lack of a basic state of security and the contingencies of motherhood, respectively, without a priori giving greater importance to any one of them. We met three groups of ten subjects (families that had lost a baby to SIDS, families with a child who have suffered from ALTE and control families). We took down their testimonies during clinical interviews, guided by specific indicators. Our analysis of the contents of these interviews clearly shows that the four hypothese concerning the psychoaffective dimension are confirmed. There are however hierarchical differences that explain the different causes of the pathologies observed. We were able to highlight some factors of risk and protection, showing certain distinctive features for each of the two pathological groups. However, the differences were more pronounced in the two cases with the control group. The preventive measures proposed at the end of this study are based on this observation

Full textPsychosomatic patients and those suffering from panic disorder have a history of medical treatments for their body ailments and they have also an intense psychological distress. This research is divided in three phases:Phase 1A retrospectiveA control group was selected from 417 asthmatic patients of the Hospital de Clinicas J. de San Martin (Buenos Aires, Argentine). It was established an association between asthma degree and type of compliance (X 2 = 53.42 significant at 1%). It was found that in all kinds of asthma, lower probability of improvement corresponds to patients who discontinued ≥ 3 days and/or or changed the dose of medication. Patients with good compliance have 87% of chance to improve and those with poor compliance only 38%.Prospective Phase 1B asthmatic word groups33 patients were evaluated with severe and almost fatal asthma for 1 year in a program that included word group. At the beginning of the therapeutic word group all patients had a bad compliance. At the end of the year, the proportion of patients who did not fulfil their treatment correctly was 10%. Nocturnal asthma is associated with compliance variables and is statistically significant (p <0.001).The direct association between improvement in spirometer and compliance variables is significant (p <0.001).Phase II prospective asthmatics patients in Photolanguage® groups.Psychological approach in Photolanguage® groups for asthma treatment is a variable able to influence favourably the evolution and prognosis of these patients as it improves compliance and favours free association, expanding preconscious and symbolization.Also they could turn a more complex psychic apparatus, and showed no emergencies or hospitalizations. The psychosomatic patient's psychic suffering began in archaic time and his mother´s continence was abruptly changing. Patients could built a more autonomous identity after many ambivalence battles.Phase III prospective. Patients with panic attack in Photolanguage® groups.The psychological approach to patients with panic disorder in Photolanguage® groups is a useful tool for the evolution and better prognosis, in addition to the necessary pharmacological indications. These patients have a history of punishment, humiliation and psychic pain, which occurred when their psychic apparatus had some level of symbolization. They showed no subsequent panic attacks without medication, unbearable anguish decreased. They could differentiate characteristics about deadly situations from those that are not and also between time before and now. A Photolanguage® group creates the necessary distance needed to create links favouring preconscious thinking ability and transcription processes. Psychic work isdone simultaneously over the group level and over their effects on the individual level.Number of preconscious representations increased, patients could build autonomy and certain otherness levels, with vital and possible identifications. Photolanguage ®produces personal work awareness (foundation sense) allowing each think and connect with their own history

Full textThe link between personality, health and disease has been a central question for centuries, even if it remains controversial in France. Much research around the world has been carried out in this field particularly in the study of cancer, which is the basis of the present approach. This kind of research immediately raises the question of measurement, which is quite complex in relation to personality traits. The present research selected five self-evaluation tests, which are among the most widely used and which have already provided promising results in health Psychology. Although these tools emerge from different theoretical backgrounds, they were examined together through various statistical procedures with the aim of determining the relationships among them. A main objective was to categorise subjects, since health or disease primarily concerns individuaIs who tend to be overlooked in dimensional approaches focusing on variables. The main results uncover an underlying construct composed of two central dimensions in the theory of attachment: self-and other-representation. This structure is recurrent throughout the analyses of our sample. It accounts for the coherence of previous results obtained with these different scales and constitutes a solid basis for categorising subjects. A complementary set of results provides finer analyses and the assessment of other theoretical factors relevant to clinical or health Psychology. Globally, this research contributes an evaluation tool which is more reliable than each of the separate tests composing it. As such, it constitutes a useful way to advance research in many areas concerned with assessing personality traits

Full textThis thesis is an interdisciplinary study about the representation of the psychosomatic disease in Denis Diderot’s work put in an open dialectical relationship with Vitalistic medical theories from Montpellier medical school, around the mid-18th century. The notion of the organism as an animata anatome where soul is made flesh through sensibility is the precondition for the study of a disease that affects both the body and the mind. This is also the main finding proposed by the vitalists in their epistemological and methodological renewal of medicine. The treatises taken into consideration in our corpus following Diderot’s notes in Éléments de physiologie, as well as the articles of the Encyclopédie concerning medical matters, present attempts to theorize this new type of disease. By transcribing their experiences, doctors use poetic solutions that open the medical treatise to the forms and modes of the novel. The same figurative and poetic approaches can be found in Éléments de physiologie. In this essay, Diderot thus formulates his “science de l’homme”, he reflects on the consequences of physiological determinism, and he finally envisions the human being as a complex convolution of needs and relations. Diderot's novels and tales are the laboratory where he questions the complexity of the relationship between the individual and reality and he provides the psychosomatic disease with narrative and aesthetic functions. By representing sick physiological manifestations in a realistic way, Diderot appropriates the clinical observation specific to physicians, he involves his reader in the narration, and he questions classical moral philosophy

Can we define it as an expression of psychic defense or as a substitute physiological defense for an impossible psychic approach of the environment ? The psychodynamic orientation given by the psychoanalytic concept of repression is an essential contribution to the problem of resistance through-out the treatment. One question remains concerning the persistance of non organic fonctional disorders in children when their perception is not yet co-ordinated with a mental représentation but is a condition of psychic development as well as an activation of signal of a biological rythm disorder. This point justifies the investigation of the relation between selective fonctional disorders and relational experiences where the environment reactions amplify the intensity of the baby’s emotional tensions, reinforces the relief of these tensions into muscular activity and finally increases reflex organic mechanisms against pain. Among the factors that provoque lasting fonctional disorders, we can take into consideration the repeated miss-matching of the baby’s physiological need of tension regulation with his mother’s emotional reactions. We can also mention the obstruction effect of non matching interaction to the transition between perceptive experiences and the primary psychic defense that is memory

Full textOur thesis aims to define the influence of aesthetic emotion in the process of visual art therapy, on subjects suffering from psychotic disorders or boundary issues. We shall observe the way it relates to psycho-corporal boundaries, in a context where leadership of a group is assumed in hospital institution. Our study thereby details a peculiarity specific to the situation of the artist in art-therapeutic relationship, which involves his or her aesthetic sensibility, creating a mirror-effect with the aesthetic questions of the subject expressing their creativity. Our overall hypothesis is that of a link between aesthetic emotion and the bonding process (Stern, 1989) between the art-therapist and the subjects within the art-therapy group, and that this triggers perceptions and representations of boundaries among these subjects. The research took place in two stages, on four groups of adolescent subjects and two groups of adult subjects. The first phase enabled the collection of results produced prior to our initial research which focused on three groups, and during which we held art-therapy sessions in the presence of an observer and a carer. Data was collected for two goals : to understand the role of aesthetic emotion in transferential movements within the group and the therapeutic effects of these processes, in relation to the creative processes and on the formal representations in the artistic work (observation sheets, self-administered questionnaires, clinical notes) ; and to enrich the objectification of the therapeutic effect of this approach, with tests before/after (projective tests for the six groups, and, for one of them, aesthetic perception tests with eye-tracking, the first results of which are presented under development). An element of mirroring became apparent between the art-therapist (a strangeness in the aesthetic emotion) and the subjects (well-being in the aesthetic emotion, often in relation to gestures), as well as a potential connection between this mirroring and the appearance of boundaries within artistic works, at pivotal moments during sessions. We have paid special attention to this aspect in our thesis, by considering drive aspect in accordance with our examination of aesthetic emotion, at the meeting point between the scopic drive, the aesthetic gaze and its mirror effect

Based upon the psychoanalytic theory of Sigmund Freud and Jacques Lacan and the political philosophy of Michel Foucault and Giorgio Agamben, the present work proposes a psychoanalytic anthropology of bio-political society, in other words it draws a parallel between the pathogenesis that characterises modem society (clinique du social) and the pathology of the subject as expressed nowadays through the body and its vicissitudes (clinique du cas). One of the main issues of this work has been that of a critical approach of the explanation of bodily phenomena (primarily psychosomatic ones) by means of the post-modem theory of symbolic collapse. Indeed, the interest of this research into psychosomatic-dermatological phenomena consists in the exploration of this type of phenomena in relation to the processes of desymbolization (homo sacer, to be a body, formless) and bio-symbolisation (bio-political subject, to have a body, forms-of-life) which are characteristic of the apparatus (dispositifs) of subjectivization in late modernity

Full textMy PhD thesis aims to analyse the fibromyalgia syndrome understood as an entity defined by contemporary and located biomedical knowledge and practices. While a transnational biomedical vision of the syndrome is analysed, the ethnographic research field is located within the Italian context. The first chapter is focussed on biomedical theories which analyse chronic pain without organic lesions, also framed here by the concept of pain as a disease; it ends by analysing how complex chronic painful states are considered at one legislative and political level in the Italian national context. The second chapter analyses fibromyalgia syndrome seen as a biomedical category. A first section examines how the syndrome is framed by transnational biomedical scientific knowledge; the second section focuses on data collected during the ethnographic research. The third chapter is dedicated to an investigation of the medical reasons that outline fibromyalgia as a female syndrome. It also proposes, in its second section, the testimony of Marzia, a woman affected by fibromyalgia; beyond a purely biomedical interpretation of the disease, through Marzia’s story the syndrome emerges in contrastive, opposite and unpredictable ways
La mia tesi di dottorato analizza la sindrome fibromialgica intesa come entità presa in carico e trattata da saperi e pratiche biomediche contemporanee e localizzate. Pur focalizzandomi sulla visione biomedica transnazionale della sindrome, il mio terreno di ricerca è situato all’interno del contesto nazionale italiano. La prima parte della tesi è centrata sull’analisi della visione biomedica del dolore cronico senza lesione, inquadrato qui anche nei termini di “dolore-malattia”. Essa propone anche una osservazione di come sono inquadrate nella contemporaneità forme dolorose croniche complesse a un livello più propriamente legislativo e politico all’interno del contesto italiano. La seconda parte della tesi si concentra sulla fibromialgia intesa nei termini di categoria medica. Un primo capitolo analizza come attualmente la sindrome è inquadrata attraverso un sapere scientifico transazionale; un secondo capitolo è focalizzato sui dati raccolti durante la ricerca etnografica. La terza parte della tesi esamina la costruzione medica della fibromialgia nei termini di una sindrome femminile. Propone infine la testimonianza di Marzia, una donna colpita da fibromialgia che, attraverso la sua storia e opponendosi a un’interpretazione puramente biomedica della malattia, definisce la sindrome in modo contrastante, mutevole e imprevedibile

Full textThe aim of this research was to better understand the links between emotional labor, burnout and somatic complaints. The first chapter constitutes a literary review that exposes the concept, its history, and its links with burnout and somatic complains: Hochschild’s sociological analysis of emotional labor is developed and its relevance to the hospital context is exposed. The evolution of the concept is then presented and we propose to focus on the intra psychic perspective of emotional labor which pays attention to three components of the concept: emotional dissonance, surface acting and deep acting. Based on this approach, we realized a literature review that exposed the associations between these three components and burnout, somatic complaints and job performance. Diverse problems associated with the comprehension of the impact of emotional labor on health outcomes are developed. The second chapter exposes the problems associated with the limitations of the measure used to evaluate the concept of emotional labor. An alternative measure is proposed: this one distinguishes the two emotion regulation processes included in the conceptualization of deep acting, which are, re-evaluation and attentional deployment. It also distinguishes the two expressive regulation strategies included in the conceptualization of surface acting, that are expressive suppression and expressive amplification. Finally, these emotion regulation processes are separate from the emotional dissonance state. Our results present several issues: First, they indicate that emotional dissonance is positively associated with burnout and somatic complains and that the variance part of these two variables are broadly explained by the emotional dissonance state. Second, they reveal that surface acting and deep acting measures include different processes with opposite effects on burnout: while expressive suppression is positively linked with burnout, expressive amplification is negatively related to it. Moreover, re-evaluation presents a positive impact on health outcomes while attentional deployment has a negative impact on them. The aim of the third chapter is to determine the added value of emotional labor to the job demands-resources model: links between emotional labor, burnout and somatic complaints were analyzed by taking into account different organizational demands and resources (emotional demands, workload, organizational justice…). Based on the job-demands-resources principles, our result show that emotional labor components explain a great part of burnout and somatic complaints beyond the parts of variance explained by demands and lack of resources. As emotional dissonance plays a determinant role in the burnout process, the fourth chapter examines to what extent reflexivity among work teams moderates its negative effects. Results indicate that social reflexivity moderates burnout induced by emotional dissonance. Finally, chapter five explores the links between emotional labor, burnout and mistreatments toward patients. Results confirmed the link between emotional dissonance and burnout on mistreatments. Indeed, burnout and emotional dissonance mediate the effects of stressors linked with the work context on mistreatment
