Abstract:
This article deals with the complexity of health behaviour from a self-efficacy perspective, and shows the naivety in assuming knowledge as the main guide to better protection against HIV. The authors accentuate the importance of local knowledge when developing health strategies as in the case of protection against HIV, in this case for female university students in Malawi. Being part of a transition period, these students have to handle complex and at times opposing expectations. This makes HIV protection into a complex social- and health issue. However, the close association between universities and rational thinking has for long made public health see self-efficacy as one of the main determinants in general health behaviour. By seeing health behaviour as complex, this study explores into how female university students perceive their own self-efficacy in protecting themselves against HIV in Malawi with a HIV score of approximately 12%. The study is based on data from Chancellor College in Zomba, Malawi. The authors point to the potential of a closer collaboration between social work and public health in issues of both a social and a health nature, as in the case of HIV protection.

Abstract:
This article discusses professional discretion in relation to placing a child outside the family, as understood by Malawian social workers. The article is a product of an exploratory study covering different aspects of social work practice with children and families in Malawi. It is based on focus group discussions with practicing social workers that were conducted using a vignette. This article describes how social workers handle child protection cases, in which a child has to be placed outside the home or family. The article points out different solutions and the reasoning behind certain decisions on placing children outside their home. The study explores issues of patriarchy, intervention methods into families and the cooperation between social workers, community members and other professionals when helping families. The study found that a number of different factors affect the decision of placing a child outside the home. Social workers in this study put an emphasis on the importance of helping children within the immediate- and extended family to help cope with the lack of financial resources that would provide alternative options.

RÉSUMÉNous avons validé sept algorithmes d’évaluation de maladie chronique pour l’usage dans L’Étude longitudinale canadienne (ÉLCV) sur le vieillissement. Les algorithmes ont concerné le diabète type 2, parkinsonisme, obstruction chronique de flux d’air, ostéoarthrite de main, ostéoarthrite de hanche, ostéoarthrite de genou, et la maladie cardiaque ischémique. Notre recrutement de cible était 20 cas et contrôles par chaque maladie. Quelques cas ont été utilisés comme contrôles avec certaines maladies. Tous les participants ont répondu à des questionnaires au sujet des symptômes de la maladie et d’utilisation de médicaments. Les cas et les contrôles de diabète ont subi le test de jeûne de glucose et les cas et les contrôles de l’obstruction chronique de flux d’air ont subi le test de spirométrie. Pour chaque maladie, nous avons utilisé l’algorithme adapté pour classifier si les participants étaient positifs ou négatif pour la maladie. Nous avons également calculé la sensibilité et la spécificité utilisant le diagnostic de médecin comme norme. L’échantillon final a fait participer 176 participants, qui ont été recrutés dans trois villes canadiennes entre 2009 et 2011. La plupart des sensibilités et spécificités étaient 80% ou plus, indiquant que les sept algorithmes peuvent correctement identifier des personnes avec les maladies.

THIS article aims to clarify the national political culture of Tanzania through an analysis of one important institution — the Cabinet. Although attention to the norms and rules that regulate political behaviour in formal and informal ways can reveal insights that are not disclosed by other approaches, political culture has been a neglected field in the study of African politics.By way of contrast, class analysis has been a popular approach to African politics in past decades.1 It has been shown, for instance, that those in government positions siphon off surplus from the peasantry through the marketing of agricultural produce. This has happended in countries as diverse as Ghana, Côte d'Ivoire, Malawi, and Tanzania, 2 where there are enormous differences in economic performance, as well as with respect to the way in which political conflicts are handled. People n similar class positions can act in a variety of ways – this is a matter of culture – because there is more than one possible logic with which to perceive and defend interests.

While learning this piece, I experienced a slight discomfort in my right upper arm. A more severe pain developed later, over several weeks, and my right hand became partially paralyzed. Full recovery took almost 2 months. … My reversible injury was caused by playing just six bars, and it may owe something to the way I “handled” this brief section, or more likely, “footed” it.

Over the past 30 years, information technology has gradually transformed the way health care is provisioned for patients. Chronic Obstructive Pulmonary Disease (COPD) is an incurable malady that threatens the lives of millions around the world. The huge amount of medical information in terms of complex interdependence between progression of health problems and various other factors makes the representation of data more challenging. This study investigated how formal semantic standards could be used for building an ontology knowledge repository to provide ubiquitous healthcare and medical recommendations for COPD patient to reduce preventable harm. The novel contribution of the suggested framework resides in the patient-centered monitoring approach, as we work to create dynamic adaptive protection services according to the current context of patient. This work executes a sequential modular approach consisting of patient, disease, location, devices, activities, environment and services to deliver personalized real-time medical care for COPD patients. The main benefits of this project are: (1) adhering to dynamic safe boundaries for the vital signs, which may vary depending on multiple factors; (2) assessing environmental risk factors; and (3) evaluating the patient’s daily activities through scheduled events to avoid potentially dangerous situations. This solution implements an interrelated set of ontologies with a logical base of Semantic Web Rule Language (SWRL) rules derived from the medical guidelines and expert pneumologists to handle all contextual situations.

The family is an institution of God Himself (Genesis 2:18-25) aside from the church (Matthew 16:18) obtaining a glorious mandate through God's family to want the birth of Divine offspring (Malachi 2:15), which is a God-fearing and living in its prescribed streets. To achieve the above goal, a Christian Religious Education of faith became something very important. But unfortunately there are Christian families who are unaware of this and are shifting this glorious mandate to the church through sunday school teachers or transferring it to school (through Christian religious teachers). The writer assesses this distraction on the one hand as a parent's misunderstanding of the mandate or on the other hand because of the parents' inability to handle it. The method the writer uses is a descriptive qualitative with a library approach. The writer tapped relevant resources from the bible, books and journals. Starting with a general understanding and juridis about the family, the Biblical basis of the family and its calling, the family's responsibility for Christian Religious Education and the danger of displacing the function of Christisn Religious Education on the third hand and the writer will eventually conclude that it is important to restore the family's function as a base of Christian Religious Education as well as to accord with scriptural values to be so effective in reaching the goal of bearing Divine offspring.

L’utilisation d’analgésiques par voie orale est un élément central de la gestion des symptômes. L’utilisation de médicaments par cette population, cependant, est complexe et la nécessité de contrôler les symptômes doivent être mis en balance avec les préoccupations concernant la sécurité des médicaments. Notre étude s’est concentrée à illustrer et à explorer les variations entre divers médicaments différents utilisés pour gérer les symptômes liés à l’ostéoarthrite. Nous avons analysé les données provenant d’un échantillon de personnes âgées de 55 ans et plus, qui vivent dans les communautés, et qui souffrent d’arthrite de la hanche ou du genou pour examiner les facteurs sociaux et médicaux associés à la variation dans les médicaments rapporté. Une conclusion principale est que les types de médicaments utilisés par les patients atteints d’ostéoarthrite varient selon l’âge et le sexe, indépendamment de la maladie et du contexte médical et social. Les explications possibles ont été considérés comme relatives aux préférences des patients et des professionnels.

Abstract:
Cette étude est une analyse à long terme d'un groupe de patients porteurs d'une prothèse de la hanche ou du genou infectée. Nous avons sélectionné 28 patients avec une infection de prothèse (22 hanches, 6 genoux) documentée par l'identification d'une bactérie. Au moment du diagnostic et à la revue (en moyenne 46 mois après la fin du traitement), nous avons évalué le tableau clinique et l'aspect radiologique de l'articulation atteinte, ainsi que les paramètres biologiques. 19/28 patients avaient un tableau clinique sans équivoque, tandis que 9 présentaient un tableau clinique douteux. Les traitements se résument en 14 changements de prothèse en deux-temps, 7 ablations simples de prothèse, 5 traitements conservateurs et 2 changements de prothèse en un-temps. A la revue, 25 patients sont considérés comme guéris de leur infection et 3 comme échec. Sur le plan fonctionnel, 9 patients ne présentent pas de limitation, tandis que 19 sont limités dans leur activité quotidienne. La moitié des patients ne souffre d'aucune douleur. Du point de vue radiologique, 20/26 patients évalués ne montrent pas de signe de récidive. Les examens paracliniques sont importants pour diagnostiquer les infections à bas-bruits, en particulier la mise en évidence d'une bactérie par des prélèvements pré-opératoires (ponction articulaire, frottis de fistule). Malgré la guérison de l'infection, les séquelles fonctionnelles et douloureuses sont souvent non négligeables. En fonction de l'expérience acquise, nous préconisons une prise en charge chirurgicale en deux-temps. Ce n'est que lorsque l'état général du malade l'interdit ou que l'infection n'est pas sous contrôle que nous envisageons un traitement alternatif (arthrodèse, girdelstone, consevateur).

Abstract Background We sought to identify perceptions of neurorehabilitation challenges for paediatric cerebral malaria (CM) survivors post-hospital discharge at Queen Elizabeth Central Hospital (QECH) in Blantyre, Malawi. Methods An exploratory approach was used to qualitatively investigate the perceived neurorehabilitation challenges for paediatric CM survivors. Data were collected through semi-structured in-depth interviews (IDIs) and focus group discussions (FGDs). Eighteen data-gathering sessions were conducted with 38 total participants, including 3 FGDs with 23 primary caregivers, 11 IDIs with healthcare workers at QECH, and 4 IDIs with community-based rehabilitation workers (CRWs). Results FGDs revealed that caregivers lack important knowledge about CM and fear recurrence of CM in their children. Post-CM children and families experience substantial stigma and sociocultural barriers to integrating into their community and accessing neurorehabilitative care. At a community-level, rehabilitation infrastructure, including trained staff, equipment, and programmes, is extremely limited. Rehabilitation services are inequitably accessible, and community-based rehabilitation remains largely unavailable. Conclusions There is an urgent need to establish further training of rehabilitation personnel at all levels and to build accessible rehabilitation infrastructure in Malawi for post-CM patients. Additional work is required to expand this study across multiple regions for a holistic understanding of neurorehabilitation needs.

Abstract Background and objectives Research has shown an increased prevalence of substance use among adolescents in countries in Sub-Saharan Africa. Peer affiliation, bullying, and psychological stress are significantly associated with substance use. This study empirically tested theoretical frameworks linking peer affiliation, bullying victimization, loneliness and substance use (specifically tobacco or alcohol use) among adolescents. Materials and methods Data were obtained from the 2009 Malawi Global School-based Health Survey (GSHS) with a representative sample of 2359 students aged 13–17 years in Malawi, Sub-Saharan Africa. Missing data were handled using multiple imputation. The study conducted path analyses using Mplus to test the conceptual models of tobacco use and alcohol use. Results The results showed that loneliness partially mediated the association between bullying victimization and tobacco use in the first model, as well as the relationship between bullying victimization and alcohol use in the second model. Results indicated statistically significant indirect paths from bullying victimization to tobacco use and alcohol use through the mediation of loneliness. However, peer affiliation did not directly predict bullying victimization in the two path models; it directly predicted tobacco use only. Conclusions These findings have important implications in early intervention for health practitioners in school and mental health settings to prevent feelings of loneliness and substance use among adolescents who have experience with bullying victimization but no experience with depression, alcohol use or tobacco use.
