False aneurysms of the internal mammary artery are extremely rare. A case of false aneurysm of a branch of the right internal mammary artery after median sternotomy is reported. A large right-sided mediastinal mass was seen on the thoracic radiogram. A false aneurysm was suspected on CT-scan and confirmed by angiography. In the same setting percutaneous embolization was performed.

Absztrakt: A tünetes koszorúér-betegség kezelésében ma is kiemelkedő szerepet játszanak a revascularisatiós műtétek (coronary artery bypass grafting – CABG). Az első beavatkozások elvégzése óta eltelt több mint ötven év alatt számos klinikai megfigyelés igazolja, hogy a koszorúér-áthidalásra az arteria mammaria interna (IMA) használható a legkedvezőbb eredménnyel. Az IMA szövettani struktúrája és élettani tulajdonságai ellenállóvá teszik az eret az érelmeszesedéssel szemben. Cikkünkben a Magyarországon IMA-graft felhasználásával elsőként végzett CABG-műtétről emlékezünk meg, egyúttal az IMA kedvező tulajdonságait az irodalomban eddig közölt leghosszabb (harmincöt éves) utánkövetéses coronarographia eredményeivel illusztráljuk. Az eset alátámaszthatja azt a feltételezést is, hogy a mammariagraft prosztaciklintermelése megvédi a kiáramlási pályát az atherosclerosis progressziójától. Ennek igazolására nagy esetszámú vizsgálat javasolható, amely sztentimplantációval hasonlítaná össze az IMA-graft feltételezhető védőhatását a kiáramlási pályán. A hosszú távú követés során a bal coronaria elülső leszálló ágának (LAD) sztentelését versus az arteria mammaria interna graftnak a felhelyezését követően a LAD kiáramlási pályájának alakulását kellene értékelni. Sekundär symptomatische proximale Subclaviastenosen, z.B. nach Anlage eines Hämodialyseshuntes am Arm mit Ischämiesymptomatik der Hand, können erfolgreich mittels PTA und Stentimplantation behandelt werden.

D’abord, nous avons visé à établir un modèle ex vivo de la dysfonction endothéliale induite par le glucose élevé (HG) dans les artères isolées de rat Wistar mâle et porc. Notre but était de clarifier le rôle des SGLT1/2 dans les cellules endothéliales dans les conditions HG pour évaluer l'effet protecteur des gliflozines sur la fonction endothéliale. Cependant, HG n'a pas affecté la relaxation dépendant de l'endothélium. L'absence d'effet d’HG pourrait être liée aux facteurs, tels que les conditions d'incubation, le genre, l'âge, la souche, l’espèce d'animal et les conditions de logement. Enfin, nous avons caractérisé les artères mammaires internes humaines (AMI) de 58 patients ayant subi un pontage coronarien au Nouvel Hôpital Civil de Strasbourg. Nous avons trouvé l'association du diabète et de l'hypertension au niveau accru du stress oxydatif dans les AMI humaines. Les niveaux d'expression de la eNOS, des SGLT et du système d’angiotensine local doivent être étudiés plus en détail
Firstly, we aimed to establish an ex vivo model of high glucose (HG)-induced endothelial dysfunction in isolated arteries from male Wistar rat and porc. Then, our goal was to clarify the contribution of SGLT1/2 in endothelial cells under HG conditions to evaluate the protective effect of gliflozins on the endothelial function. However, HG did not affect the endothelium-dependent relaxation response in all tested types of artery. The lack of effect of HG might be related to certain factors, such as the incubation conditions, gender, age, strain, species of studied animals, and conditions of housing of animals. Secondly, we characterized human internal mammary arteries (IMA) of 58 patients underwent coronary artery bypass grafting in the New Civil Hospital of Strasbourg. We found the association of diabetes and hypertension in the enhanced level of oxidative stress in human IMA. The expression levels of eNOS, SGLTs and the components of angiotensin system need to be further investigated

This chapter provides a summary of a landmark historical study in cardiac surgery related to internal mammary artery ligation versus sham sternotomy for angina pectoris. It describes the history of the procedure and a summary of the study including study design and results, and relates the study to a modern-day principle of evidence-based medicine: blinding and sham surgery. Whether or not sham surgery is ethical remains under debate. Proponents for sham surgery agree that it should be used only when a question cannot be answered adequately by other methods. Cobb and his colleagues were among the first to demonstrate the value of sham studies in addressing important clinical questions.

Coronary angiography, injecting radiopaque contrast into the coronary arteries through a catheter, transformed the care of millions of patients and the careers of tens of thousands of cardiologists. As was typical of new diagnostic techniques, coronary angiography was the result of a series of technological innovations combined with contributions that many individuals had made over several years. But Mason Sones Jr. of the Cleveland Clinic was the main innovator and promoter of coronary angiography, which he described in a 1960 publication. At the time, there was no accepted surgical treatment for coronary heart disease, so very few cardiologists saw the need to refer their patients for this invasive catheter-based procedure. Sones and his colleagues at the Cleveland Clinic became tireless promoters of coronary angiography and of the Vineberg internal mammary artery implant operation as a treatment strategy for patients with coronary artery disease and angina pectoris.

Current standard surgical technique in patients undergoing coronary bypass grafting involves the use of an internal mammary artery (also known as an internal thoracic artery) bypass graft, in general to the left anterior descending coronary artery, and in many centres also the use of the right internal mammary artery to the right coronary artery. Several clinical studies have shown the superiority of mammary artery bypass grafts for survival of patients compared to the use of venous bypass grafts alone. Indeed, the internal mammary artery has several biological features, discussed in this chapter, that are remarkable and of great interest for biologists, cardiologists, and surgeons alike.

Upper extremity artery disease due to atherosclerosis is mostly situated at the level of the brachio-cephalic trunk, and the subclavian and axillary arteries. When clinically suspected, it can be assessed by DUS, CTA or MRA. In most asymptomatic patients, medical treatment is the option of choice. Revascularization can be proposed when: severe/disabling symptoms, bilateral stenosis, stenosis with ipsilateral arteriovenous fistula for dialysis, patients planned for coronary artery bypass graft surgery or those already operated on with ipsilateral internal mammary artery grafted to coronary arteries with evidence of myocardial ischaemia. When revascularization is considered, both endovascular and open surgical options can be proposed, according to lesion characteristics and patient’s risk.

The current incidence of coronary artery bypass graft spasm has been estimated at 0.43%. The first report of early postoperative graft spasm as a clinical entity was in the left internal thoracic (mammary) artery graft in 1987. Subsequently it was reported that spasm could be a localized phenomenon or a diffuse process, referred to as a ‘string sign’. These clinical reports promoted intense physiological and pharmacological studies on the biological characteristics of the internal thoracic artery to understand the nature of spasm.

Breast tissue develops from ectoderm, the primary mammary buds being noted during the fifth week of gestation. Glandular epithelium, stroma, and fat receive blood from the internal mammary and posterior intercostal arteries. In females, estrogen mediates ductal development. In males, androgen leads to destruction of the epithelial component of the breast bud. Most breast complaints are due to a mass, nipple discharge, or pain. Ultrasonography is useful in young women and as an adjunct to mammography. Wide local excision, mastectomy, sentinel lymph node biopsy, and axillary dissection can be useful in men and women undergoing breast surgery. Lymphedema may occur after axillary lymph node dissection or radiation therapy.

While coronary artery bypass graft (CABG) remains the gold standard in the treatment of multivessel coronary artery disease, it remains quite invasive. Techniques and evolving technologies of minimally invasive (including hybrid and robotic) approaches to coronary revascularization are reviewed in this chapter with a summary of the available supporting literature. Minimally invasive direct coronary bypass (MIDCAB) surgery uses an anterior, medially placed, mini-thoracotomy incision for both direct-vision left internal mammary (LIMA) harvest and creation of an anastomosis of the LIMA to a coronary artery in off-pump fashion. As in standard CABG, all patients should undergo a complete preoperative work-up, and body mass index and body habitus are to be noted. Obesity is considered a relative contraindication for MIDCAB as it may predispose to wound infection; this concern is primarily due to tissue necrosis caused by pressure on the wound edges by the retractor during LIMA harvest.
