Abstract:

False aneurysms of the internal mammary artery are extremely rare. A case of false aneurysm of a branch of the right internal mammary artery after median sternotomy is reported. A large right-sided mediastinal mass was seen on the thoracic radiogram. A false aneurysm was suspected on CT-scan and confirmed by angiography. In the same setting percutaneous embolization was performed.

Abstract:

Absztrakt: A tünetes koszorúér-betegség kezelésében ma is kiemelkedő szerepet játszanak a revascularisatiós műtétek (coronary artery bypass grafting – CABG). Az első beavatkozások elvégzése óta eltelt több mint ötven év alatt számos klinikai megfigyelés igazolja, hogy a koszorúér-áthidalásra az arteria mammaria interna (IMA) használható a legkedvezőbb eredménnyel. Az IMA szövettani struktúrája és élettani tulajdonságai ellenállóvá teszik az eret az érelmeszesedéssel szemben. Cikkünkben a Magyarországon IMA-graft felhasználásával elsőként végzett CABG-műtétről emlékezünk meg, egyúttal az IMA kedvező tulajdonságait az irodalomban eddig közölt leghosszabb (harmincöt éves) utánkövetéses coronarographia eredményeivel illusztráljuk. Az eset alátámaszthatja azt a feltételezést is, hogy a mammariagraft prosztaciklintermelése megvédi a kiáramlási pályát az atherosclerosis progressziójától. Ennek igazolására nagy esetszámú vizsgálat javasolható, amely sztentimplantációval hasonlítaná össze az IMA-graft feltételezhető védőhatását a kiáramlási pályán. A hosszú távú követés során a bal coronaria elülső leszálló ágának (LAD) sztentelését versus az arteria mammaria interna graftnak a felhelyezését követően a LAD kiáramlási pályájának alakulását kellene értékelni. Die perkutane transluminale Angioplastie ist im Vergleich zur offenen chirurgischen Revaskularisation ein alternatives Therapieverfahren bei kurzstreckigen Stenosen oder Verschlüssen im Abgangsbereich der Arteria subclavia. Besteht der Verdacht auf eine Obstruktion der Arteria subclavia (z.B. Blutdruck-Differenz an beiden Armen), sollte vor geplanter Anlage von Hämodialyseshunts am Arm und vor operativer Myokardrevaskularisation unter Verwendung der A. mammaria interna eine DSA der oberen Extremität einschließlich des gesamten Aortenbogens erfolgen. Sekundär symptomatische proximale Subclaviastenosen, z.B. nach Anlage eines Hämodialyseshuntes am Arm mit Ischämiesymptomatik der Hand, können erfolgreich mittels PTA und Stentimplantation behandelt werden.

Abstract:

The aim of our study was to compare a systemic and a local aprotinin application in patients during coronary artery bypass graft (CABG) surgery. The advantage of a topical aprotinin application is seen in the fact that this may not lead to systemic side effects. A prospective, randomized study comprising 97 patients was conducted. A dose of 5 106 KIU aprotinin was given systemically to 49 patients and four doses of 1.25 106 KIU aprotinin were applied topically to 48 patients by spraying the substance on the target area (A. mammaria interna region and pericardium). We determined markers for the inflammatory response, coagulation system, standard haematological markers and postoperative complications. Exclusion criteria were defined as surgical bleeding, redo operations, neurological, haematological, liver and kidney disorders. Sex, age, perfusion times, mortality, renal failure and strokes were identical in both groups. Biochemical markers and clinical outcome demonstrated no significant differences between the systemic and local applications. Interleukin 6 and elastase were tendentially higher ( p= 0.1) in the local group, but with a high standard deviation in each patient. Our results suggest that there is no difference between the perioperative application of 5 106 KIU systemically given aprotinin and 1.25 106 KIU locally applied aprotinin.

Abstract:

Ultrasonic Aspirators (USA) or Ultrasonic Dissectors, respectively, are used in a variety of surgical disciplines . The functional principle is to fragment and aspi rate tissue components using a piezoelectric PZT (lead, zirconium, tantalum) ceramic electrostrictive transducer tip, which "vibrates" in Tts long axTs at about 23-24 kHz producing longitudinal waves.This study deals with the preparation of the ATI for CABG and compares the effects of conventional preparation of ATI by forceps, scalpel and cautery vs. USA-Application (SONOCORR; SOERING Ltd, Quickborn/FRG), whereby two different energy levels (5%, 25% of the maximal available energy (100W) ) were tested. The aim of the study was to check the effects of the USA on endothelial morphology. Two experiments were done: first, the distal ends of ATIs from four patients immediately prior to CABG were prepared either (two of them) conventionally (cf. above) or by use of USA (one at 5%, one at 25% of max. available energy).
