Abstract:
Background Despite the importance of their perspectives, end users (eg, patients, caregivers) are not typically engaged by academic researchers in the development of mobile health (mHealth) apps for perioperative cardiac surgery settings. Objective The aim of this study was to describe a process for and the impact of patient engagement in the development of an mHealth app that supports patient and caregiver involvement with enhanced recovery protocols during the perioperative period of cardiac surgery. Methods Engagement occurred at the level of consultation and took the form of an advisory panel. Patients who underwent cardiac surgery (2017-2018) at St. Boniface Hospital (Winnipeg, Manitoba) and their caregivers were approached for participation. A qualitative exploration determined the impact of patient engagement on the development (ie, design and content) of the mHealth app. This included a description of (1) the key messages generated by the advisory panel, (2) how key messages were incorporated into the development of the mHealth app, and (3) feedback from the developers of the mHealth app about the key messages generated by the advisory panel. Results The advisory panel (N=10) generated 23 key messages to guide the development of the mHealth app. Key design-specific messages (n=7) centered around access, tracking, synchronization, and reminders. Key content-specific messages (n=16) centered around medical terms, professional roles, cardiac surgery procedures and recovery, educational videos, travel, nutrition, medications, resources, and physical activity. This information was directly incorporated into the design of the mHealth app as long as it was supported by the existing functionalities of the underlying platform. For example, the platform did not support the scheduling of reminders by users, identifying drug interactions, or synchronizing with other devices. The developers of the mHealth app noted that key messages resulted in the integration of a vast range and volume of information and resources instead of ones primarily focused on surgical information, content geared toward expectations management, and an expanded focus to include caregivers and other family members, so that these stakeholders may be directly included in the provision of information, allowing them to be better informed, prepare along with the patient, and be involved in recovery planning. Conclusions Patient engagement may facilitate the development of a detail-oriented and patient-centered mHealth app whose design and content are driven by the lived experiences of end users.

Abstract:
CancerCare Manitoba (CCMB) introduced virtual visits at the beginning of the COVID-19 pandemic to replace many in-person visits. This study examines the impact of virtual visits for cancer care on travel distance, travel time, and carbon dioxide (CO2) emissions. We included all visits to CCMB for invasive and in situ cancers from 1 April 2020 to 31 December 2022. Data were extracted from CCMB’s electronic health record. The percentage of visits done virtually by month was reported by age, gender, cancer diagnosis, and regional health authority of residence. Postal codes for patients’ residences and clinic locations were converted into latitude and longitude values. Travel distance, travel time, and CO2 emissions associated with travel were estimated. The percentage of virtual visits was highest during the months when COVID-19 restrictions were present in Manitoba and represent more than 50% of such monthly visits. Virtual visits increased with age, were highest among men with urogenital cancer, and were lowest among northern Manitoba residents. The median travel time per visit ranged from 30 min in Winnipeg to 15 h in the Northern Region. The estimated travel distance saved varied from 420,000 to 750,000 km per month. Estimated travel time saved varied from 5500 to 9600 h per month. Estimated CO2 emissions prevented varied from 87 to 155 metric tons per month. Virtual care is an important tool for better supporting those living with cancer by substantially decreasing travel distance and time. Virtual care also contributes to reducing greenhouse gas emissions.

Abstract:
In 1989 a new record was established for the number of fires (1147) and area burned (3.28 million ha) in Manitoba. These fires resulted in the unprecedented evacuation of 24,500 people from 32 different communities and cost over $68 million (CDN) to suppress. The first major outbreak of fires occurred in central Manitoba in mid-May and was followed by a second peak of fire activity in northern Manitoba between July 16 and August 2. Both periods were characterized by multiple fire ignitions, severe fire weather and fire danger conditions, and extreme fire behavior. A general description of the events that took place during this historic fire season are presented in this paper.

Abstract:
Alvars are rare in Canada and uncommon globally. This study represents the first formal attempt to describe and delineate the extent of alvars in Manitoba. A systematic examination of the Interlake region’s edaphic and biological characteristics, using a geographic information system, resulted in the identification of 67 sites warranting further field study. Of these sites, 61 were surveyed and information was collected on vegetation composition and structure, soil characteristics, land ownership, and land use. Alvar was confirmed at 28 sites, extending over approximately 3930 ha in five geographically distinct clusters. Four putative types of alvar communities are described: grassland, shrubland, savannah, and wetland. Livestock grazing is the dominant land use and occurs across more than three-quarters of Manitoba alvar. Approximately 12% coincide with mining claims or quarry leases. Two-thirds occur on publicly owned land.

Abstract:
Abstract: DESCRIPTION OF BACTERIAL CONTAMINTANT IN TOOTHBRUSH. Brushing activity is an effort to keep oral hygiene from pathogenic microorganisms that can cause infection. Oral infection has been an increasingly common risk factor for systemic disease. Oral infections, especially periodontitis, can affect travel and pathogenesis of a number of systemic diseases, such as cardiovascular disease, bacterial pneumonia, diabetes mellitus, and low birth weight. Keeping your teeth and mouth clean is not enough just brushing your teeth, but replace the toothbrush for a maximum of three months. This study aims to identify the contaminant bacteria found on toothbrushes that have been used for more than 3 months. The method of identification by planting the isolation result of 20 toothbrushes has been used for more than three months on selective media ie blood agar (AD), McConkey Agar (MCA), Eosin Methylen Blue (EMB), Manitol Salt Agar (MSA) . The results showed that all samples of toothbrush were contaminated with Streptococcus and Staphylococcus, 16 samples were Klebsiella (80%), four samples (20%) were Proteus, and one sample (5%) was contaminated with Escherichia. All of the toothbrush samples showed to have been contaminated by bacteria. Bacterial contamination of the toothbrush can be caused by the storage of a toothbrush that is too close to the toilet and toilet, possibly exposed through aerosols containing intestinal bacteria. This toothbrush contamination plays an important role in both oral and systemic diseases, including septicemia and gastrointestinal, respiratory cardiovascularmy, and kidney problems.

Abstract:
ABSTRACTThe Aging in Manitoba (AIM) project includes nearly 9,000 seniors who have been interviewed. Three independent cross-sectional samples (i.e. 1971, 1976, 1983) were subsequently followed (1983–1984, 1990), producing cross-sectional and longitudinal data. A major goal of this paper was to examine non-response over time and representativeness of the panel of AIM survivors. This involved analysis of non-response, calculations of completion/response rates, and comparisons of demographic variables across the AIM sample and the broader Manitoba and Canadian populations. Non-response was very low (4.6% in 1983–1984, 5.0% in 1990) and did not appear to be dependent on such factors as location, gender, education level, or nationality descent. The 95.0 per cent completion rate compares favourably with other longitudinal studies, demonstrating the value of employing rigorous tracking procedures. These findings suggest that attempts to minimize bias and selective attrition in the AIM study have been successful.

Abstract:
Goal: This study aimed to examine provincial differences in patient spending for cancer care and reductions in household spending including decisions to forego care in Canada. Methods: Nine-hundred and one patients with cancer, from twenty cancer centers across Canada, completed a self-administered questionnaire (P-SAFE version 7.2.4) (344 breast, 183 colorectal, 158 lung, and 216 prostate) measuring direct and indirect costs and spending changes. Results: Provincial variations showed a high mean out-of-pocket cost (OOPC) of CAD 938 (Alberta) and a low of CAD 280 (Manitoba). Differences were influenced by age and income. Income loss was highest for Alberta (CAD 2399) and lowest for Manitoba (CAD 1126). Travel costs were highest for Alberta (CAD 294) and lowest for British Columbia (CAD 67). Parking costs were highest for Ontario (CAD 103) and lowest for Manitoba (CAD 53). A total of 41% of patients reported reducing spending, but this increased to 52% for families earning <CAD 50,000 per year. The highest national rates of decisions to forego care were in relation to vitamins/supplements, the selection made by 21.3% of those who indicated spending reductions. Reductions for complementary and alternative medicine (CAM) were made by 16.3%, and for drugs, by 12.8%. Most cost categories had higher proportions of individuals who decided to forego care when family income was <CAD 50,000 per year and for patients under 65 years of age. Conclusions: Levels of financial burden for patients with cancer in Canada vary provincially, including for OOPC, travel and parking costs, and lost income. Decisions to forego cancer care are highest in relation to vitamins/supplements, CAM, and drugs. Provincial differences suggest that regional health policies and demographics may impact patients’ overall financial burden.

Abstract A discrete choice travel cost model was developed for backcountry forest recreation using 2 yr of backcountry canoe registrations from Nopiming Park, Manitoba. A statistically significant variable explaining choice of route was the amount of forest along the route that was burned in the last 10 yr; virtually all of which occurred in two particularly severe fires during 1983. The current economic welfare loss from these fires was estimated using a discrete choice travel cost model combined with a count model of trip demand. Since the forest grows back following fire, these losses will eventually decrease to zero. A linear intertemporal damage function for one of the fires is estimated, and the present value of damages under alternative discount rate assumptions is presented. For. Sci. 42(4):450-455.

Abstract:
AbstractNumerous scholars have sought to locate the origins of social scientific research in the late-sixteenth-century ars apodemica, the northern European body of literature dedicated to methodizing educational travel. Little attention has been paid, however, to the earlier model of educational travel that emerged from sixteenth-century Venetian diplomatic culture. For many Venetian citizens and patricians, accompanying an ambassador on a foreign mission served as a cornerstone of their political education. Diplomatic travelers were encouraged to keep written accounts of their voyage. Numerous examples of these journals survive from the sixteenth century, largely following a standard formula and marked by an emphasis on the description of customs. This article examines the educational function of diplomatic travel in Venice and the practices of cultural description that emerged from diplomatic travel, arguing that Venetian diplomatic travel offers an earlier model for the methodization of travel—one with its own distinctive norms of observation.

Abstract. The theory of travel time and residence time distributions is reworked from the point of view of the hydrological storages and fluxes involved. The forward and backward travel time distribution functions are defined in terms of conditional probabilities. Previous approaches that used fixed travel time distributions are not consistent with our new derivation. We explain Niemi's formula and show how it can be interpreted as an expression of the Bayes theorem. Some connections between this theory and population theory are identified by introducing an expression which connects life expectancy with travel times. The theory can be applied to conservative solutes, including a method of estimating the storage selection functions. An example, based on the Nash hydrograph, illustrates some key aspects of the theory. Generalization to an arbitrary number of reservoirs is presented.

Abstract:
Episodic memory (memories of the personal past) and prospecting the future (anticipating events) are often described as mental time travel (MTT). While most use this description metaphorically, we argue that episodic memory may allow for MTT in at least some robust sense. While episodic memory experiences may not allow us to literally travel through time, they do afford genuine awareness of past-perceived events. This is in contrast to an alternative view on which episodic memory experiences present past-perceived events as mere intentional contents. Hence, episodic memory is a way of coming into experiential contact with, or being again aware of, what happened in the past. We argue that episodic memory experiences depend on a causal-informational link with the past events being remembered, and that, assuming direct realism about episodic memory experiences, this link suffices for genuine awareness. Since there is no such link in future prospection, a similar argument cannot be used to show that it also affords genuine awareness of future events. Constructivist views of memory might challenge the idea of memory as genuine awareness of remembered events. We explain how our view is consistent with both constructivist and anti-causalist conceptions of memory. There is still room for an interpretation of episodic memory as enabling genuine awareness of past events, even if it involves reconstruction.

Abstract:
Background and objectivesThe prevalence of ESKD is increasing worldwide. Treating ESKD is disproportionately costly in comparison with its prevalence, mostly due to the direct cost of dialysis therapy. Here, we aim to provide a contemporary cost description of dialysis modalities, including facility-based hemodialysis, peritoneal dialysis, and home hemodialysis, provided with conventional dialysis machines and the NxStage System One.Design, setting, participants, & measurementsWe constructed a cost-minimization model from the perspective of the Canadian single-payer health care system including all costs related to dialysis care. The labor component of costs consisted of a breakdown of activity-based per patient direct labor requirements. Other costs were taken from statements of operations for the kidney program at Seven Oaks General Hospital (Winnipeg, Canada). All costs are reported in Canadian dollars.ResultsAnnual maintenance expenses were estimated as $64,214 for in-center facility hemodialysis, $43,816 for home hemodialysis with the NxStage System One, $39,236 for home hemodialysis with conventional dialysis machines, and $38,658 for peritoneal dialysis. Training costs for in-center facility hemodialysis, home hemodialysis with the NxStage System One, home hemodialysis with conventional dialysis machines, and peritoneal dialysis are estimated as $0, $16,143, $24,379, and $7157, respectively. The threshold point to achieve cost neutrality was determined to be 9.7 months from in-center hemodialysis to home hemodialysis with the NxStage System One, 12.6 months from in-center hemodialysis to home hemodialysis with conventional dialysis machines, and 3.2 months from in-center hemodialysis to peritoneal dialysis.ConclusionsHome modalities have lower maintenance costs, and beyond a short time horizon, they are most cost efficient when considering their incremental training expenses.PodcastThis article contains a podcast at https://www.asn-online.org/media/podcast/CJASN/2018_07_18_CJASNPodcast_18_8_F.mp3

Abstract:
This article considers the ways in which eighteenth-century womens travel narratives function as autobiographical texts, examining the process by which a travellers dislocation from home can enable exploration of the self through the observation and description of place. It also, however, highlights the complexity of the relationship between two forms of writing which a contemporary readership viewed as in many ways distinctly different. The travel accounts considered, composed (at least initially) in manuscript form, in many ways contest the assumption that manuscript travelogues will somehow be more self-revelatory than printed accounts. Focusing upon the travel writing of Lady Mary Wortley Montagu, Katherine Plymley, Caroline Lybbe Powys and Dorothy Richardson, the article argues for a more historically nuanced approach to the reading of womens travel writing and demonstrates that the narration of travel does not always equate to a desired or successful narration of the self.
