Pengendalian peralatan listrik rumah tangga secara manual menyebabkan penggunaan tidak terkontrol dengan baik, apalagi jika pengontrolan yang harus dilakukan harus pada ruang yang berbeda dan lampu listrik yang dikontrol secara manual. Untuk menghemat waktu dan mengurangi resiko lupa mematikan lampu maka dibuatlah sistem pengontrolan menggunakan smartphone Operating Sistem Android karena mudah digunakan dan dapat dibawa kemana-mana. Operating Sistem open source milik google, yang diberi nama ANDROID sebagai alat pengontrolan dari jarak jauh (remote) dengan memanfaatkan jaringan internet nirkabel (wireless internet). Untuk mengontrol sebuah perangkat listrik rumah tangga diperlukan sebuah IP sebagai identitas dari perangkat tersebut, untuk itu perlu ditanamkan sebuah TCP/IP sebagai media penyimpanan IP. Selain itu TCP/IP Starter Kit juga berfungsi sebagai penghubung antara smartphone dengan perangkat yang akan menerjemahkan perintah yang dikirimkan oleh smartphone untuk dieksekusi mikrokontroler ATMega328. Mikrokontroller berperan sebagai otak bagi device hardware. Antara smartphone dan perangkat, masing – masing diberikan alamat identitas berupa IP Address, Subnet Mask dan Gateway sehingga keduanya dapat saling berkomunikasi. Pada penelitian menggunakan metode perancangan yang digambarkan dalam sebuah flowchart, kemudian pada metode pengujian menggunakan metode pengujian black box. Sistem untuk mengontrol lampu hanya menekan button yang ada pada layer android dan ketika berhasil maka akan tampil ON/OFF pada layer tersebut. Kata Kunci: android, mikrokontroller, wireless, TCP/IP

Abstract:

Thrombotic thrombocytopenic purpura (TTP) is a potentially fatal condition that can be challenging for clinicians to identify in the setting of autoimmune diseases such as systemic lupus erythematosus (SLE). This difficulty is compounded when a patient presents with all of the clinical signs of a TTP-like microangiopathy, however, with near normal ADAMTS13. This case report describes a 44-year-old female with a history of SLE who was hospitalized with acute on chronic anemia, thrombocytopenia, and altered mental status. The patient’s ADAMTS13 was mildly low; hence, she was initially treated for SLE-associated immune thrombocytopenic purpura without any clinical response. The patient then underwent plasmapheresis (plasma exchange [PLEX]) for treatment of a suspected TTP-like microangiopathy. She responded to PLEX with improvement in her platelet count and mental status. This case illustrates the importance of considering TTP-like microangiopathic hemolytic anemia in the differential for patients with a history of SLE presenting with clinical signs of TTP even in the setting of near-normal ADAMTS13, thus warranting prompt treatment with PLEX.

Abstract:

With Xi Jinping’s consolidation of political power in China, a personality cult has increasingly emerged. In this article, we analyze online documents and state news media to argue that this phenomenon is driven in part by local government officials and traditional media but most significantly by individual Chinese ‘netizens’. The current personality cult phenomenon is thus primarily society-driven and bottom-up rather than state-driven and top-down. We argue that the rise of this personality cult around Xi has its roots in national anxiety in an important transitional period in China. While some worry about a possible return to the politics of the Cultural Revolution by encouraging this personality cult, others are responding to economic anxieties and to the social anxieties created by social injustice greatly due to official corruption. We conclude that the possibility of society-driven personality cults will increase over time, as a paradoxical corollary of the potential of new media to allow for the democratization and opening up of politics and culture to new voices.

Penggunaan sistem pengontrolan jarak jauh semakin meningkat sejalan dengan era globalisasi di mana perpindahan dan pergerakan manusia semakin luas dan cepat. Pada saat ini sudah dikenal masyarakat luas bahwa untuk melakukan pengontrolan terhadap suatu electronic home appliances dari jarak jauh dapat menggunakan remote kontrol. Permasalahannya penggunaan remote kontrol terkendala oleh keterbatasan jarak antara sinyal yang dipancarkan oleh remote dan sinyal yang diterima oleh electronic home appliances, apabila jarak antara electronic home appliances yang dikontrol dengan remote/pengontrol melewati batas toleransinya, maka electronic home appliances tersebut tidak dapat berfungsi sesuai yang diinginkan. Untuk memecahkan permasalahan tersebut maka dirancang sebuah sistem yang dapat melakukan pengontrolan electronic home appliances dari jarak jauh sehingga tidak terkendala lagi oleh jarak, tempat dan waktu. Perancangan sistem ini dengan memanfaatkan teknologi jaringan internet yang dapat diakses di mana saja dan kapan saja selama tersedianya jaringan. Jaringan internet dimanfaatkan TCP/IP starter kit berbasis modul jaringan NM7010A-LF sebagai jembatan antara mikrokontroler AVR low cost micro system dengan jaringan komputer untuk melakukan pengontrolan terhadap peralatan elektkronika, AVR low cost micro system berfungsi sebagai sebuah web server. Hasilnya berupa sebuah prototype pengendalian electronic home appliances yang dapat mengendalikan electronic home applances dari jarak jauh yang tidak terkendala oleh jarak, tempat dan waktu sehingga pada akhirnya meningkatkan efektifitas, efisiensi dan kenyamanan dalam pengendalian.

Dari puluhan ribu penduduk miskin, banyak diantaranya tinggal di Rumah Tidak Layak Huni (RTLH). Seiring terbitnya Peraturan Daerah (Perda) nomor 22 Tahun 2012 Tentang Tanggung jawab Sosial Perusahaan (TSP), maka kalangan swasta atau pengusaha memiliki tanggung jawab dalam pengentasan kemiskinan di Kabupaten Kulon Progo. Untuk itu, dalam penelitian ini akan mengkaji sejauh mana peran CSR atau keterlibatan kalangan pengusaha dalam upaya pengentasan kemiskinan, khususnya melalui program bedah rumah. Background and Purpose Thrombotic thrombocytopenic purpura (TTP) is a heterogeneous disease primarily characterized by thrombocytopenia and microangiopathic hemolytic anemia. Therapeutic plasma exchange has dramatically improved mortality, allowing for emergence of refractory, relapsing, and atypical presentations. in this case series we aim to present our institutional data for Apheresis in Sheikh khalifa medical City in AbuDhabi. We will also present patient demographic and clinical presentation and treatment protocol we use Methodology -Case series with Retrospective review. -Routine laboratory tests such as peripheral blood cell counts, reticulocyte count, coagulation profile, serum lactate dehydrogenase (LDH), bilirubin, serum creatinine, cardiac enzymes, and urinalysis, were performed. -ADAMTS13 levels and inhibitor titer were determined for all patient in outside lab -Baseline demographic characteristics were calculated in frequencies and percentages. (include age ,Gender , clinical manifestations and treatment strategy) Results and Discussions thrombotic thrombocytopenic purpura (TTP) pentad consisting of fever, thrombocytopenia, microangiopathic hemolytic anemia (MAHA), neurological abnormalities, and renal failure. less than 5 % of patient reported in literature have all associated clinical features. -Total of 10 patients M:F 4:2 , Median Age 44yr 50% presented with Neurological manifestations and renal disease , 30% presented with Fever only 20% had cardiac manifestation on admission . None of the patient presented with all 5 pentad. -All patients received TPE , steroid . -90 % of the patients received Rituximab except for 1 because of Allergy. -All patients has low ADAMTS 13 , except one has normal ADAMTS13 but came with relapse and on first admission had low ADAMTS13 -All patient presented with MAHA and TCP except 2 patient whom had normal Hb but significant schistocytes on peripheral blood with TCP both patient where relapsed cases. -3 patient were relapsed 7 de novo , the 3 relapsed cases all did not receive Rituximab in first remission . One of them relapsed twice but did not received Rituximab due to allergy -Although some publication include large number of TTP patients, but only few case reports have evaluated the clinical feature, laboratory parameters and therapeutic outcome of TTP. Without treatment, TTP is almost uniformly fatal with a mortality rate approaching 90%. With the timely institution of therapeutic plasma exchange (TPE) mortality decreases to about 10%-20%. A disintegrin and metalloprotease with thrombospondin Type 1 motif, Member 13 (ADAMTS13) levels less than 5% are a hallmark of TTP. We do ADAMTS 13 Activity and inhibitor titre levels in outside facility TAWAM hospital with turn-around time of 7 days which is helpful in planning Rituximab treatment. with availability of Rituximab our relapse rates are low but not zero Conclusions -Thrombotic thrombocytopenic purpura (TTP) pentad consisting of fever, thrombocytopenia, microangiopathic hemolytic anemia (MAHA), neurological abnormalities, and renal failure. -5 % of patient reported in literature have all associated clinical features. -We found that majority of patient presented with evidence of thrombocytopenia and MAHA only. -Without treatment, TTP is almost uniformly fatal with a mortality rate approaching 90%. With the timely institution of therapeutic plasma exchange (TPE) mortality decreases to about 10%-20%. -TPE ,steroid and rituximab was very effective in achieving sustain remission in 100% of ours patients with median follow up 8 month -More awareness is needed for early diagnosis and early referral to centers with appropriate tertiary care facilities. We have isolated and characterized the manA gene and physically located it between 3.5 and 5.5 kb centromere distal of the riboB locus on chromosome VIII. The manA gene contains four introns and encodes a 50.6-kDa protein which has significant sequence identity to type I phosphomannose isomerase proteins from other eukaryotes. We have constructed by integrative transformation a null mutation in the manA gene which can only be maintained in a heterokaryotic strain with wild-type manA+ nuclei. Thus, a manA null mutation is lethal in A. nidulans. The phenotype of the mutation was analyzed in germinating conidia. Such conidia are able to commence germination but swell abnormally, sometimes producing a misshapen germ tube, before growth ceases. The reason for the lethality is probably the lack of synthesis of mannose-containing cell wall polymers that must be required for normal cell wall development in growing hyphae.

Abstract:

In frequency assignment problems (FAPs), separation of the frequencies assigned to the transmitters is necessary to avoid the interference. However, unnecessary separation causes an excess requirement of spectrum, the cost of which may be very high. Since FAPs are closely related to T-coloring problems (TCP), multiagent systems and evolutionary algorithms are combined to form a new algorithm for minimum span FAPs on the basis of the model of TCP, which is named as Multiagent Evolutionary Algorithm for Minimum Span FAPs (MAEA-MSFAPs). The objective of MAEA-MSFAPs is to minimize the frequency spectrum required for a given level of reception quality over the network. In MAEA-MSFAPs, all agents live in a latticelike environment. Making use of the designed behaviors, MAEA-MSFAPs realizes the ability of agents to sense and act on the environment in which they live. During the process of interacting with the environment and other agents, each agent increases the energy as much as possible so that MAEA-MSFAPs can find the optima. Experimental results on TCP with different sizes and Philadelphia benchmark for FAPs show that MAEA-MSFAPs have a good performance and outperform the compared methods.

The benefits if teenagers can have the ability to manage good anger emotions are to achieve mental health and be able to resolve social conflicts that result from emotional situations. Management of anger emotions is the ability of individuals to express adaptive responses when faced with situations that cause anger. The research design used for this study is quantitative descriptive to explain the phenomenon or picture of anger emotional management ability in high school students, amounting to 67 people. The results of this study are the level of anger management ability of class XI students of SMA Brawijaya Smart School has a medium category. The results of this study can be used as a reference for counselors to provide appropriate interventions.</p><p class="Abstract"><strong>Abstrak:</strong> Kemampuan kelola emosi marah perlu dimiliki oleh remaja sebagai individu. Manfaat apabila remaja dapat memiliki kemampuan kelola emosi marah yang baik adalah mencapai kesehatan mental dan mampu menyelesaikan konflik sosial yang diakibatkan dari situasi emosional. Kemampuan kelola emosi marah adalah kemampuan individu untuk mengemukakan respon yang adaptif ketika dihadapkan pada situasi yang menimbulkan kemarahan. Abstract:

The first independent work of the Viennese architect Konstantin A. Jovanović (1849-1923) in Belgrade, the house of solicitor Marko Stojanović at 53-55 Knez Mihailova Street, built in 1885, signifies the Europeanising surge of the secular architecture in the Kingdom of Serbia, which conclusively suppressed the Oriental architectural practices. Jovanović's father Anastas helped him develop friendship alongside the business relationship with the patron of the house. This allowed Jovanović to construct a notable privately-owned building in the city thoroughfare, thus recommending himself to the favour of wealthy investors. Often linked in historiography to Jovanović's cult of the Italian Renaissance, Stojanović's house also contains elements of Mannerism and Baroque, harmoniously combined into an eclectic whole. Adapted to the Belgrade setting as a two-storey structure built at the top of the Sava River Slope, the house is a direct interpolation of the Viennese eclectic experiences.

The clinical presentation of myelodysplastic syndrome (MDS) is not specific. Many patients can be asymptomatic and can be detected only due to an abnormal complete blood cell count (CBC) on routine exam or for other reasons while others can be symptomatic as a consequence of underlying cytopenias. Thrombotic thrombocytopenic purpura (TTP) usually is suspected under the evidence of microangiopathic hemolytic anemia (MAHA) and thrombocytopenia and because it is a life-threatening condition (medical emergency) immediate initiation of plasmapheresis could be life-saving. The following case illustrates an unusual presentation of MDS in a patient who came in to the emergency room with the classic TTP “pentad” of fever, renal involvement, MAHA, mental status changes, and thrombocytopenia. We will focus our discussion in the clinical presentation of this case.

Abstract Acquired thrombotic thrombocytopenic purpura (TTP) is characterized by thrombocytopenia and microangiopathic hemolytic anemia (MAHA) without an obvious cause, and may include fever, mild renal failure, and neurologic deficits. It is characterized by a deficiency of the von Willebrand factor (VWF) cleaving enzyme, ADAMTS13 (a disintegrin and metalloproteinase, with a thrombospondin type 1 motif, member 13), resulting in formation of microthrombi in the high sheer environment of the microvasculature. This causes microvascular occlusion, MAHA, and organ ischemia. Diagnosis is based on the presence of clinical symptoms, laboratory aberrations consistent with MAHA, decreased ADAMTS13 activity, and possibly presence of anti-ADAMTS13 autoantibodies. Upfront treatment of acute TTP includes plasma exchange and corticosteroids. A significant number of patients are refractory to this treatment and will require further interventions. There are limited data and consensus on the management of the refractory TTP patient. Management involves simultaneously ruling out other causes of thrombocytopenia and MAHA, while also considering other treatments. In this article, we describe our management of the patient with refractory TTP, and discuss use of rituximab, increased plasma exchange, splenectomy, and immunosuppressive options, including cyclophosphamide, vincristine, and cyclosporine. We also review recent evidence for the potential roles of bortezomib and N-acetylcysteine, and explore new therapeutic approaches, including recombinant ADAMTS13 and anti-VWF therapy.

Abstract:

Microangiopathic hemolytic anemia (MAHA) can be observed as a paraneoplastic syndrome (PS) in certain tumors. MAHA-related signet ring cell carcinoma (SRCC) of an unknown origin is very infrequent. Herein we present a SRCC case presented with refractory acquired thrombotic thrombocytopenic purpura (TTP). A 35-year-old man applied to the emergency service with fatigue and headache. His laboratory tests resulted as white blood cell 9,020/µL, hemoglobin 3.5 g/dL, platelet 18,000/µL. Schistocytes, micro-spherocytes, and thrombocytopenia were observed in his blood smear. MAHA was present and he was considered as having TTP. Plasma exchange treatment was initiated; however, he was refractory to this treatment. Thorax and abdomen computerized tomography revealed thickening of minor curvature in stomach corpus with hepatogastric and paraceliac lymphadenopathy. Bone marrow (BM) investigation by our clinic resulted as the metastasis of adenocarcinoma. Ulceration and necrosis were observed by gastric endoscopy procedure. Biopsy was taken during endoscopic intervention, which resulted as SRCC. MAHA may be seen as a PS in some tumors, especially gastric cancers. Tumor-related MAHA is generally accompanied by BM metastases. As a result, BM investigation may be used as the main diagnostic method to find the underlying cancer. The clinical course of cases with tumor-related MAHA is usually poor, and these cases are usually refractory to plasma exchange treatment. In conclusion, physicians should suspect a malignancy and BM involvement when faced with a case of refractory TTP.

It is known that the Herpes simplex virus type 1 (HSV-1)-encoded thymidine kinase (TK) co-purifies with an associated thymidylate kinase (TMPK) activity and that thymidylate (TMP) inhibits the phosphorylation of thymidine by the HSV-1 TK. Here we demonstrate that: (i) TMP phosphorylation catalysed by the viral TMPK is competitively inhibited by thymidine (TdR) with a Ki equal to its Km as substrate for the viral TK; (ii) L-thymidine (L-TdR), the enantiomer of the naturally occurring D-TdR and a substrate for the HSV-1 TK [Spadari, Maga, Focher, Ciarrocchi, Manservigi, Arcamone, Capobianco, Caruso, Colonna, Iotti and Garbesi (1992) J. Med. Chem. 35, 4214-4220], is a powerful inhibitor of the HSV-1 TMPK activity with a Ki value identical with its Km as a substrate for the viral TK; (iii) both viral TK and TMPK activities are inhibited, in a competitive way and with identical Ki values, by novel, non-substrate inhibitors of HSV-1 TK, N2-phenylguanines; (iv) L-TdR is phosphorylated to L-TMP by the viral TK, but L-TMP is not phosphorylated to L-TDP by the viral TMPK activity; and (v) L-TMP inhibits competitively and with identical potencies the phosphorylation of TdR and TMP catalysed respectively by the HSV-1 TK and TMPK activities. Mixed connective tissue disease (MCTD) is not a specific disorder and identified serologically by high titers of of antibodies to RNP. Coexistence of TTP with asymptomatic MCTD is rare encounter. We describe here a rare case of secondary TTP in a 42 year, old female presenting as menorrhagia for seven days, severe headache and one episode of seizure. This case emphasizes the early diagnosis of TTP and its association with underlying condition for proper management and to avoid fatal outcome. Metode streaming dapat diaplikasikan di kehidupan sehari-sehari untuk memantau aktivitas yang sedang berlangsung salah satu contohnya adalah aktivitas di luar pintu. Aktivitas ini dapat bersifat negatif maupun positif. Berdasarkan permasalahan tersebut, penulis merancang sistem pendeteksian aktivitas dengan menggunakan Passive Infrared Sensor (PIR) yang akan mendeteksi pergerakan dari tubuh manusia. Dengan begitu, sensor PIR dapat menjadi bel rumah otomatis apabila ada orang yang berkunjung. Setelah sensor PIR mendeteksi, maka kamera Android (client) akan aktif dan mengirim pergerakan yang sedang berlangsung ke Android (user) agar pemilik rumah dapat memantau secara langsung. Hasil dari penelitian ini menyimpukan layanan video streaming dengan menggunakan protokol TCP telah memenuhi standar dari Quality of Services (QoS) TIPHON dengan menggunakan parameter delay 3,95745 ms dengan kategori sangat memuaskan, parameter throughput 327637,144 bit per second (bps) dengan kategori sangat memuaskan, serta parameter packet loss 0,952% dengan kategori memuaskan. Berdasarkan penelitian yang telah dilakukan, penulis berharap alat ini dapat membantu pemilik rumah dalam memantau aktivitas yang ada di luar pintu dari mana saja.

Abstract When a pregnant or postpartum woman presents with sudden and severe microangiopathic hemolytic anemia (MAHA) and thrombocytopenia, three syndromes that require urgent care must be considered: (1) preeclampsia with severe features/hemolysis, elevated liver function tests, low platelets (PE/HELLP) syndrome; (2) thrombotic thrombocytopenic purpura (TTP); and (3) complement-mediated thrombotic microangiopathy (C-TMA; also referred to as atypical hemolytic–uremic syndrome). The distinction among these three syndromes is often unclear because they share multiple clinical features. Overlap between PE/HELLP syndrome and the other two syndromes is also apparent from the fact that pregnancy can be a trigger for both TTP and C-TMA both before and after delivery and also the increased frequency of PE/HELLP syndrome in women who have recovered from TTP. When diagnostic criteria for PE/HELLP syndrome are present, management of hypertension and delivery is curative. Absence of improvement or actual progression of MAHA, thrombocytopenia, and kidney function abnormalities after delivery requires consideration of TTP and C-TMA. Minimal kidney involvement with severe thrombocytopenia suggests TTP and the need for treatment with plasma exchange; progressive kidney injury (in the absence of a cause for acute tubular necrosis) suggests C-TMA and the need for anti-complement treatment. We describe how we use these criteria to evaluate and manage pregnant/postpartum women with MAHA and thrombocytopenia.

Abstract Introduction Thrombotic thrombocytopenic purpura (TTP) is a prothrombotic disorder characterized by microangiopathic hemolytic anemia (MAHA) and thrombocytopenia. It has been associated with a deficiency or dysfunction of ADAMTS13, an enzyme responsible for cleaving ultra-large von Willebrand factor multimers, thus preventing spontaneous platelet adhesion and aggregation. In its absence, platelet aggregates accumulate in the microvasculature causing neurological symptoms, cardiac ischemia and renal dysfunction. Most cases in adults are idiopathic, and associated with severe ADAMTS13 deficiency (<10%) and anti-ADAMTS13 antibodies. Acquired TTP due to various conditions, such as HIV, drugs, malignancy, collagen disease, bone marrow transplant and acute pancreatitis (AP), have also been described. Some cases of these secondary TTP may be associated with no to moderate ADAMTS13 deficiency and are referred to as thrombotic microangiopathies (TMAs). If plasma exchange (PLEX) has been the treatment of choice for acquired TTP, it has not been shown to be very effective for TMAs. AP-associated MAHA and thrombocytopenia (MAHA-T) might be one exception. Methods To better understand the relationship between AP and MAHA-T and to evaluate its response to PLEX, we retrospectively reviewed all cases of suspected TTP/ MAHA-T treated by PLEX in the context of AP in our institution from 2005 to 2015. For the purpose of this report, we preferred to use the term MAHA-T to describe this phenomenon since TMA usually refers to a pathologic diagnosis. Finally, a literature review of published cases was also performed. Results Five patients were treated with PLEX for suspicion of TTP in the context of AP between January 1st, 2005 and December 31st, 2015 at our centre. Median age was 36 (31-60). Etiologies for pancreatitis were alcohol (3/5) or idiopathic (2/5). Peak lipase level ranged between 426 and 5853 U/L. One patient had 3 episodes of MAHA-T following AP, always in the context of alcohol abuse. Two patients had evidence of MAHA-T on admission, while the remaining 3 patients had an unremarkable CBC at presentation followed by an abrupt drop in platelet count (nadir, 9-23 x 109/L). All patients showed simultaneously signs of MAHA (Hb nadir between 47 and 93 g/L) with fragmentation on the blood smear and abnormal hemolytic parameters: LD level 1104-8097 U/L, bilirubin 36-176 umol/L, reticulocyte count 130-541 x109/L and undetectable haptoglobin. The median time from AP presentation to the development of laboratory or clinical features of MAHA-T in these cases was 2 days (1-3). Acute kidney injury was present in all cases with creatinine levels ranging between 118-906 umol/L. One patient required hemodialysis. Other observed end organ damage included neurologic symptoms (3/5 patients), elevated troponin (5/5 patients) and transaminitis (4/5). One patient experienced a cardiac arrest. All patients had ADAMTS13 activity measured. One patient had severe deficiency on two separate episodes (ADAMTS13 was <1% and 6% respectively). Three other patients had normal ADAMTS13 activity (average 69%) and one had a moderate deficiency by a qualitative assay. In 4 patients, where tests were performed, there was significant heterogeneity in complement protein and function studies between patients ranging from normal to depressed C3 and C4 and normal to elevated CH50. Complement genetics screen was performed in 3 cases and was normal in all 3 cases. PLEX was initiated in all patients and remission (platelet count over 150 x 109/L) was seen for all after a median of 10 daily treatments (2-12). High dose steroids were used in 4 patients. All patients survived to discharge and completely recovered their kidney function. Conclusion TTP/MAHA-T precipitated by AP is increasingly recognized and more than 40 cases have been described in the literature. Similar to our findings, time to MAHA-T occurrence is generally reported within the first 4 days after admission for AP, often when markers for pancreatitis are improving, and most patients do not have severe ADAMTS13 deficiency. While many have looked at ADAMTS13 level, to our knowledge, none have yet reported thorough complement studies and genetics. The 3 cases described herein are then the first cases suggesting this is not a complement mediated disorder. Finally, our review and cases confirm that PLEX is effective and prognosis is good in almost all cases. We present a patient with a history of rectal cancer treated with mitomycin-C who developed MAHA, acute kidney injury, and thrombocytopenia 6 months after completing therapy and to did not respond the plasmapheresis or steroids. She was treated with four weekly doses of rituximab with full recovery.

Abstract:

One of the great challenges of the Romanian ethnography after the union of Transylvania with Romania was - in parallel with the delimitation of the main ethnographic areas - the formation of a typology of the Romanian folk costume. Due to the specific history of this great province, this approach proved to be extremely complicated: compared to the conservative and eminently Romanian areas as the "cultural costumes" of the Apuseni (L. Apolzan), Năsăud, Ţara Oltului, Maramureş, Haţeg etc. , there were identified regions where, at least at first sight, the traditional costume worn by the Romanian population was far from being considered, according to the classic patterns in our ethnography, a specific Romanian identity mark. Such a case is the " villagers traditional costume" in Șiria region, Arad county, seen as a mixture of foreign styles (Hungarian, German and Slovak), the only remaining pieces of the old costume being the shirt, the lap and the winter coat. Marcel Olinescu's suggestion to study the costume in Șiria from the perspective of the Sociological School of Bucharest, therefore analysing it as a "social fact" leads to the conclusion that we are facing a specific phenomenon of creating a Romanian identity mark with a precise motivation meant to emphasize the privileged economic and social status of the Arad winegrowers. Compared to the white clothes worn by the mass of seasonal workers attracted to this El Dorado, the costume from Șiria, made of expensive fabrics, such as silk and velvet fabrics, brought the inhabitants of this area closer to the models at the top of the Arad society, where the clerks and the owners of the former large landed properties belonged to the cohabiting ethnic groups.

Introduction: Anthocyanins of maize in microcapsules (MC) can be added in maize dough to modify color and antioxidants in tortillas and derivatives. Objective: To evaluate the addition of anthocyanins of maize in MC or extract (EL) to blue-purple maize dough on color, texture, total soluble phenols (TSP), total anthocyanins content (TAC) and antioxidant capacity (AC) of maize tortillas. Methodology: Different concentrations (0.5, 0.75 and 1.0 %) of MC or EL anthocyanins were added to bluepurple maize dough (Bolita). Blue tortilla made with Chalqueño maize (traditional) was used as reference for color and AC. Results: The tortilla with 0.75 and 1.0 % MC had red-purple hue, and with EL the tortilla showed red-orange hues. When cooking the tortilla, it lost 39 to 49.1 % of anthocyanins, regardless of how anthocyanins were added to the dough (MC or EL). The hardness of the tortilla increased with addition of anthocyanins in EL, but not in MC. The addition of anthocyanins increased TAC and TSP. AC of the tortilla in the pigment treatments was higher (P ≤ 0.05) compared to the control and reference. Study limitations: Results may change depending on the blue-purple maize grain used. Originality: There are no reports of similar studies. Conclusions: The addition of low amounts of anthocyanins (< 2 mg) to the blue-purple maize dough produces tortillas similar in color to traditional blue maize tortillas, and with higher AC.

Abstract:

Mammaglobin-A (MamA) is overexpressed in 40–80% of all human breast cancers. Recent phase I clinical trials of the MamA DNA vaccine showed encouraging safety outcomes. However, this vaccine elicited only a modest increase in MamA specific CD8+T lymphocyte (CTL) activation. As vaccine adjuvants play a critical role in enhancing the immunotherapeutic efficiency of vaccines, we tested the potential role of three synthetic CpG oligodeoxynucleotides (ODN2216—class A ODN, ODN2006—class B ODN, and ODN M362—class C ODN) to further enhance MamA specific CTL responses. Towards this, naïve CD8+T cells were obtained from healthy HLA-A2+ human donors. The HLA-A2 specific immunodominant epitope of MamA, MamA2.1 (LIYDSSLCDL), was utilized to activate naïve CD8+T cells. The THP-1 (HLA-A2+) cells were used as antigen presenting cells to stimulate naïve CD8+T cells along with (or without) co-treatment of various ODNs mentioned above. Activation of naïve CD8+T cells with the MamA2.1 peptide along with ODNs demonstrated enhanced MamA specific CTL mediated cytotoxicity on AU565 (HLA-A+/MamA+) breast cancer cells following co-treatment with ODN2006 and M362 compared to ODN2216 or MamA2.1 peptide alone. However, no significant cytotoxicity was noted upon treatment of MamA2.1 activated CTLs on MCF7 (HLA-A+/MamA−) cells, suggesting that the activation of CTLs is specific to the MamA antigen. Functional characterization studies demonstrated specific IL-12 mediated cross-talk between TLR-6 and -9 in THP-1 cells following stimulation with ODN2006 and M362, which was critical for the final cytotoxic activation of CD8+T lymphocytes. Based on these data, we conclude that ODN2006 and ODN M362 exerted a strong adjuvant effect through induction of the initial innate immune response through TLR9 upregulation followed by enhanced MamA specific CTL dependent adaptive immune responses. Our current data provide evidence for the application of Class-B/-C-CpG-ODNs as potential vaccine adjuvants towards enhancing the success of MamA based breast cancer vaccination.

Teknik pengumpulan data primer diperoleh dengan wawancara dan data sekunder melalui sumber buku, artikel , dan jurnal. Hasil dari pembahasan adalah model komunikasi yang terjadi adalah top-down sehingga masyarakat desa hanya menerima jadi kebijakan kelurahan, di mana pendapat atau kebutuhan warga Dusun Jambewangi Indah I kurang didengarkan atau diapresiasi.

Patients diagnosed with thrombotic thrombocytopenic purpura (TTP) typically present with microangiopathic hemolytic anemia (MAHA) and thrombocytopenia; these two clinical manifestations were often believed to be essential indicators of TTP. However, such indicators are not always present in every case. Here, we present a patient affected by TTP but showing no distinctive schistocytes on blood smear review. TTP was diagnosed through a critically low level of a disintegrin and metalloproteinase with thrombospondin type 1 motif, member 13 (ADAMTS13) activity. Awareness of such an atypical presentation of TTP is essential for timely treatment to prevent serious and even fatal outcomes for patients.

Abstract:

Abstract Introduction The diagnosis of TTP requires thrombocytopenia without an underlying cause and microangiopathic hemolytic anemia (MAHA)1. TTP is usually confirmed with a low ADAMTS level. However, the turnaround time for an ADAMTS level is several days and treatment for this rare disease cannot be delayed, since it is uniformly fatal if left untreated but highly responsive to plasma exchange therapy. A rapid diagnosis is required and still must be made on clinical findings and peripheral smear morphology. The degree of MAHA that is seen with TTP, though, has not been well defined, or correlated with ADAMTS results. Automated fragmented red cell analyzers are not yet widely available or accepted, so the determination of MAHA still relies on manual smear reviews. Presenting smears from pts with TTP over a 1 ½ year period were reviewed, with manual counting of schistocytes, and were compared to smears from in-patient non-TTP cases of thrombocytopenia where hematology was consulted to rule out TTP. Methods 29 cases were identified and peripheral smears from date of presentation were reviewed, initially by a hematologist and experienced special hematology technician, then by 2 medicine residents after they were given a 1 hour tutorial on smear interpretation. All smear reviewers were instructed on schistocyte identification as defined by the International Council for Standardization in Hematology (ICSH)2. The number of schistocytes per 1000 red cells was counted at 1000 power magnification. The blood smears were coded/de-identified and then read "blindly" by the 4 observers. Degree of agreement between the 1st 2 observers was measured (using Pearson correlation coefficient), as was the their average as compared to the average #schistocytes counted between the 2 med residents. Time taken to review smears was also measured. Results Ave age 54 (19-83); 59% female, 41% male; 62% Caucasian; 35% AA; 3% Hispanic. 9 had TTP and 20 did not. All cases of TTP had a low ADAMTS level (med <3%, range <3-13%) and all but one had a +inhibitor (one had congenital TTP, confirmed by ADAMTS mutation analysis). The non-TTP cases included sepsis (8/20), DIC (3/20), metastatic cancer (2/20), scleroderma crisis (2/20) and single cases of HIT, EtOH, gestational, MPN/MDS and unknown causes for thrombocytopenia. For TTP cases: med plt count was 22k (4-52k); LDH 760 (309-1175); hgb 9.4 (7.1-10.9); creat 1.59mg/dL (0.6-4.85). For non-TTP cases: med plt count was 44k (3-126k); LDH 300 (155-1111); hgb 8.1 (5.9-14.7); creat 1.36mg/dL (0.9-9.1). 3 non-TTP pts had ADAMTS levels sent (med= 69%, 57-100%). The med #schistocytes in TTP cases was 6% (ave 6.3, 1.85-11.95%) compared to 0.625% (ave 1.52%, 0.25-7.5%) in the non-TTP group (p= 0.00002). The R-coefficient for the 1st 2 observers was 0.93 (95% CI 0.85-0.96) and for their average compared to the 2 medicine residents it was 0.82. The average time to read smears overall was 10.5 minutes. Conclusions: This retrospective study showed that manual quantification of schistocytes is feasible, may be done in a timely fashion and with excellent interobserver reproducibility. All pts had some degree of MAHA but the TTP cases, not surprisingly, had a significantly higher percentage of schistocytes/1000 RBCs than non-TTP thrombocytopenic in-pt consult cases (6% vs 0.625%, respectively, p= 0.00002). All cases of low ADAMTS, positive inhibitor confirmed TTP had >2.5% schistocytes, compared to 4/20 non-TTP cases. A larger cohort of acquired TTP should be studied to determine if there is a cut-off % schistocytes which, in the appropriate clinical setting, should warrant urgent plasma exchange while waiting for ADAMTS levels to return. 1George JN and Al-Noun ZL. Objetivo: Determinar el efecto de la maca fresca sobre el rendimiento físico de deportistas en la altura. Diseño: Estudio experimental, prospectivo y comparativo. Materiales y Métodos: La muestra estuvo constituida por 10 deportistas de altura, quienes fueron sometidos a actividad física para medir el consumo máximo de oxígeno (VMO2 ) basal; luego, se les administró durante 60 días 1500 mg/día de maca fresca en presentación micropulverizado y se determinó el consumo máximo de oxígeno. En cada evaluación se realizó dosaje de transaminasa glutámico oxalacética (TGO), transaminasa glutámico pirúvica (TGP) y creatinina sérica. Resultados: En la evaluación basal, se encontró que VMO2 2 mL/kg/min. Al realizar la evaluación luego de 60 días del consumo de la maca fresca, se encontró que la media del VMO2 fue 57,75 mL/kg/min (p=0,001). Se demostró un incremento del rendimiento físico de los deportistas en promedio de 10,3%. En todos los casos, los valores de TGO, TGP y creatinina sérica se mantuvieron en el rango de normalidad y no se presentaron reacciones adversas. Conclusiones: La maca fresca es un producto con actividad energética, que permite el incremento del rendimiento físico de deportistas en la altura a una dosis de 1500 mg/día, durante un período de administración de 60 días.

ABSTRACT The MICs of ciprofloxacin for 33 clinical isolates of K. pneumoniae resistant to extended-spectrum cephalosporins from three hospitals in Singapore ranged from 0.25 to >128 μg/ml. Nineteen of the isolates were fluoroquinolone resistant according to the NCCLS guidelines. Strains for which the ciprofloxacin MIC was ≥0.5 μg/ml harbored a mutation in DNA gyrase A (Ser83→Tyr, Leu, or IIe), and some had a secondary Asp87→Asn mutation. Isolates for which the MIC was 16 μg/ml possessed an additional alteration in ParC (Ser80→IIe, Trp, or Arg). Tolerance of the organic solvent cyclohexane was observed in 10 of the 19 fluoroquinolone-resistant strains; 3 of these were also pentane tolerant. Five of the 10 organic solvent-tolerant isolates overexpressed AcrA and also showed deletions within the acrR gene. Complementation of the mutated acrR gene with the wild-type gene decreased AcrA levels and produced a two- to fourfold reduction in the fluoroquinolone MICs. None of the organic solvent-tolerant clinical isolates overexpressed another efflux-related gene, acrE. While marA and soxS were not overexpressed, another marA homologue, ramA, was overexpressed in 3 of 10 organic solvent-tolerant isolates. These findings indicate that multiple target and nontarget gene changes contribute to fluoroquinolone resistance in K. pneumoniae. Besides AcrR mutations, ramA overexpression (but not marA or soxS overexpression) was related to increased AcrAB efflux pump expression in this collection of isolates.

Gojek adalah perusahaan layanan berupa tempat bertemunya penyedia jasa dan/atau barang dengan konsumen. Salah satu layanannya adalah Gopay. Gopay (e-wallet) adalah layanan dalam pembayaran secara online untuk bertransaksi dalam Gojek. Dalam memandang gopay, terdapat perbedaan pendapat dari sudut pandang yang berbeda pula. Dalam perdata, tidak terdapat suatu masalah. Dalam Syariah, terbagi menjadi dua pendapat, yakni yang mengatakan bahwa gopay ini halal, dan yang mengatakan haram. Yang mengatakan halal karena tidak mengandung hal-hal yang diharamkan. Sedangkan yang mengharamkan mengatakan gopay mengandung unsur riba. Letaknya adalah saat kita top-up sejumlah uang kepada gojek adalah akad utang piutang, sementara nantinya setelah kita top-up, kita akan membeli barang yang ada diskonnya, disinilah terjadinya riba (akad top-up (utang piutang) bertemu dengan diskon. Maka dari itu, berdasar dewan fatwa al-irsyad, dan fatwa MUI salah satu hal yang menjadi solusi bagi konsumen untuk menghindari riba tersebut adalah menghindari diskon jika ingin menggunakan gopay (utang tidak bertemu dengan diskon), atau membeli barang tanpa menggunakan gopay (diskon tidak bertemu dengan utang). Dan juga solusi dari pihak Gojek menyikapi uang saldo dari konsumen sebagai amanah, sehingga tidak dipakai untuk kepentingan pribadi, maka akad akan berubah menjadi titipan, sehingga tidak masalah untuk mengambil diskon.
