Digital technologies are transforming European societies, politics, and markets. These efforts have targeted the dual missions of pan-European market-making, as well as market correction. Rural Massachusetts is delightful at the end of summer. The classic New England architecture blends with the lawns, gardens, and green forests into a picture of perfect harmony. It is sunny, and the right time in the afternoon for a stroll around the college town of Amherst. However, after a few blocks a distinct crack appears in this idyll: a traditional white wooden house is being renovated and a skull and crossbones sign on the lawn is telling us to keep out due to danger of lead poisoning. It turns out that the customary white colour of the houses around here was oft en due to lead-based pigments. The use of lead in paints was phased out in 1978, but it is still an issue judging from the 16-page pamphlet available in six languages from the US Environmental Protection Agency, and the criminal cases brought against real estate companies and landlords failing to inform tenants and buyers of the lead status of their homes. Marcus Vitruvius Pollio would probably have agreed with this pamphlet and legislation, and so most certainly would Alice Hamilton. Although almost two millennia separate the Roman engineer from the first woman on the faculty of Harvard Medical School, they are united in the fight against the dangers of lead to the workforce and to the public. We do not know much about the life of the first century BC architect and engineer Marcus Vitruvius Pollio, otherwise known as Vitruvius, except what can be inferred from his famous work The Ten Books on Architecture. This magnum opus, written in the days of the Emperor Augustus, probably represents the summary of the professional experience of an old man. The title is slightly misleading, as architecture in Roman times would cover a much broader area than today. So Vitruvius tells us a great deal about engineering in general, about the chemistry of pigments and, to the benefit of this story, about aqueducts and the proper treatment of water. He is also clearly a conservative man, lashing out against ‘decadent frescos’ and ‘these days of bad taste’.

For more than a century, catatonia has been considered a marker of the Kraepelin/Bleuler concept of schizophrenia. However, over the past half-century, it has been increasingly recognized as a separate entity, independently diagnosable and treatable. This chapter explores the diagnosis, treatment, and biological underpinnings of catatonia. Initially, it lists certain motor behaviours whose presentation might indicate catatonia, according to the Catatonia Rating Scale (CRS) such as mutism, delirium, and repetitive rhythmic acts. Through the intravenous administration of certain drugs, such as benzodiazepines, barbiturates, or gamma-aminobutyric acid (GABA) agonists, the effect on the patient’s CRS score is used to confirm the diagnosis. The treatment history of catatonia prior to the discovery of the efficacy of benzodiazepines and induced grand mal seizures (electroconvulsive therapy) is broached, such as chemically induced seizures and amobarbital. Finally, its different presentations are discussed, along with its consideration as a biological fear response.

Tube shunts have become an important part of the surgical armamentarium for controlling intraocular pressure (IOP). Once reserved for use only in refractory cases, they are rapidly gaining popularity as an effective alternative to trabeculectomy and cyclodestruction for lowering IOP in patients who are not satisfactorily controlled on medications. Although trabeculectomy remains the most commonly performed glaucoma surgery, tube shunts offer some significant advantages. One of these advantages is a theoretically reduced risk of one of the most feared complications of any penetrating ocular surgery— endophthalmitis. The incidence of late-onset infections such as blebitis and endophthalmitis after trabeculectomy with antifibrotic agents is alarmingly high compared to other penetrating intraocular surgeries, perhaps as high as 5% over 3–5 years. Trabeculectomies produce a perilimbal bleb that may be made thinner and more ischemic by the concomitant use of antifibrotics such as 5-fluorouracil and mitomycin-C (MMC). These anteriorly placed blebs are more exposed, more fragile, and more prone to become infected than the robust, thicker, and more posteriorly located blebs overlying a tube shunt plate. Gedde and associates reported a 1% (1 of 107 eyes) incidence of endophthalmitis after Baerveldt® Glaucoma Implant (BGI) placement compared to a 5% (5 out of 105 eyes) incidence after trabeculectomy with MMC after 5 years of follow-up. However, despite this potential advantage, endophthalmitis still does occur in association with shunts, and it is essential to understand the risk factors for its occurrence and the proper steps to take to prevent, recognize, and treat this potentially devastating complication. Distinguishing sterile from infectious endophthalmitis can be difficult. Signs suggestive of an infectious endophthalmitis include marked inflammation, hypopyon, fibrinoid anterior chamber reaction, corneal edema, marked conjunctival congestion, eyelid edema, vitritis, and retinal periphlebitis. Signs suggestive of a noninfectious etiology are a gradual onset of symptoms, such as pain, redness, and inflammation, in the absence of tube or plate erosion. Sterile endophthalmitis one month after discontinuation of corticosteroid therapy postoperatively has been reported. Possible causes for noninfectious (sterile) endophthalmitis include exacerbated preexisting uveitis, iris trauma, and toxicity from foreign substances introduced during surgery.

As discussed so far, race, place, and music cannot be taken for granted. What they can be taken for arises from processes that happen along two axes. Along the first axis, the figures of blackness, the Pacific, and música del Pacifico are bounded, and bound together, to give them unity of form and stability of meaning. We have seen this process—we might call it stabilization—in the narrowing racial ascription of Pacific sounded practices in the colonial-era process that resulted both in the creative world-making from which black life in the Pacific arose and the heuristics of black atavism that marked the Pacific as unfit for modernity. The second axis both is occluded by stabilization and works to undo it. This is the axis of transformation, of history, by which stabilized reifications bifurcate into new forms as they are instrumentalized under changing political conditions or as they bleed into, feed back from, or abrade against one another and against the trails of associations and the latent traces of older reifications that recur much later. This is also apparent in the transformation of regional folklore into Afro-Colombian culture. The axes of stabilization and transformation produce the commonsensical forms into which meaning solidifies. While making meaning is always a power play, neither the axis of stabilization nor that of transformation is reserved for either the powerful or the abject. Out of the stabilizing and transformational power plays unfurls the story of black Pacific sounded practice traced in this book.
