Abstract:
Objective: To compare desvenlafaxine with its parent drug, venlafaxine, to determine the usefulness of this new medication. Data Sources: Information was obtained through a MEDLINE search (1966-June 2008) and from published abstracts. Search terms included desvenlafaxine, O-desmethyIvenlafaxine, Pristiq, major depressive disorder, and venlafaxine. STUDY selection and data extraction: All English-language studies and abstracts pertaining to desvenlafaxine and venlafaxine were considered for inclusion. Preference was given to human data. Data Synthesis: Desvenlafaxine is a serotonin-norepinephrine reuptake inhibitor and is the active metabolite of the antidepressant venlafaxine. The recommended dose is 50 mg daily, based on the efficacy and safety data of 50, 100,150,200, and 400 mg of desvenlafaxine. The response and remission rates of depression at 8 weeks for the 50-mg dose are 51 -63% and 31-45%, respectively. These rates are comparable with those seen with venlafaxine (58% and 45%, respectively). Adverse effects are also similar to those of venlafaxine, with the most common being insomnia, somnolence, dizziness, and nausea. The decreased potential of CYP2D6 activity with desvenlafaxine compared with the parent drug may be a potential advantage in patients on other medications metabolized via this enzymatic pathway. Also, desvenlafaxine tablets are less expensive than extended-release (XR) venlafaxine, which may decrease healthcare costs in the short term. However, venlafaxine XR is expected to go off patent in 2010. Conclusions: With the overall similarity between these 2 drugs and the potential lack of cost savings, the need for desvenlafaxine and its ultimate utility in treating major depressive disorder appears to be insignificant.

Abstract Beckett’s writing—in prose and theatre—is commonly imagined as a series of exercises in decay and diminution. His protagonists exist in a world in which all systems decay, and where, because of this endemic system failure, they are progressively unable to express the reality of their situation through systems of discourse. This article argues that, although this may be true for Beckett’s prose pieces, it is not true of his theatrical works. Beckett’s plays rely for their effects on the fact that theatre itself can be thought of as a coercive system—one that does not decay, but which persists, even as the protagonists trapped within it are worn down to nothing.

While persons may differ on the identified start of what evolved into the “Me Too” movement of 2017, the media focus makes the Anita Hill and Clarence Thomas saga of October 1991 a starting point for a slow cruise to a season of reckoning. This article explores the circumstances that led to a cataclysm where women have been believed and the alleged perpetrators have experienced consequences. These elements are a grassroots movement against sexual harassment across sectors; high-profile celebrity cases that attracted public attention; the use of a social media venue, the # MeToo, that facilitated the victims speaking publicly, from a safe distance from the harasser or abuser, no longer feeling compelled to silence for personal or career reasons; the election of a President (Trump) who was recorded jesting about engaging in sexual harassment; courageous investigative journalism in the face of threats from powerful persons; and President Obama’s Title IX enhancements that put sexual assaults on college and universities in the news (and Betsy Devos’s reversal of some of these initiatives). A final ingredient is the initial mistrial of Bill Cosby in 2017 (he has since been convicted of sexual assault in 2018).
