Abstract:
2500 Background: People with HIV have been excluded from immuno-oncology (IO) studies. Anti- PD-1/PD-L1 therapies are approved for a growing number of cancers. We evaluated pembrolizumab (pembro) in people with HIV and cancer. Methods: CITN-12 is a multicenter phase 1 trial. Key eligibility: advanced cancer; ECOG ≤1; CD4 ≥100 cells/μL; ≥4 weeks antiretroviral therapy (ART), HIV viral load (VL) < 200 copies/mL. Exclusion: uncontrolled HBV/HCV, autoimmune disease. Participants (pts) accrued into CD4 based cohorts (C): C1 100-199; C2 200-350; C3 > 350 CD4 cells/μL. Pembro 200 mg IV administered Q3W for up to 35 doses. Adverse events (AE) evaluated by CTCAE. Immune related AE ≥ grade (Gr) 2 were events of clinical interest (irECI). Clinical benefit (tumor shrinkage or stable disease [SD] ≥24 weeks) was estimated. Data were locked for safety analysis and publication once C2 and C3 completed accrual and all pts completed ≥2 cycles. Accrual continued for 6 C1 pts and a new phase 1b Kaposi sarcoma (KS) cohort (C4). Results: 30 pts, characteristics: C1 (6), C2 and 3 (12 each), median (med) age 57 years (range 39-77), 28 men, 2 women, 60% White, 30% Black, 10% Hispanic. Med CD4 285 cells/μL (132 - 966). Cancers: KS (6), non-Hodgkin lymphoma (NHL) (5), non-AIDS defining (19) – most common: anal (6) and squamous cell skin (3). Prior radiation (19), med prior systemic therapies 2 (0-8). Safety observed over 183 cycles, med 5 (1-32). Treatment emergent AE ≥ possibly attributed to pembro mostly Gr 1-2, with 20% of pts having Gr 3. irECI: hypothyroidism (6), elevated AST/ALT (1), pneumonitis (3), rash (2), musculoskeletal (1).1 KS pt developed KSHV-associated multicentric Castleman disease (KSHV-MCD) and died of the AE. HIV was controlled and increasing CD4 counts were observed. Best response: complete (lung, 1), partial (NHL, 2), SD ≥24 weeks (KS, 2), SD < 24 weeks (13), progressive disease (10), not evaluable (2). Conclusions: Pembro has acceptable safety in cancer pts with HIV on ART and > 100 CD4 cells/µL, similar to patients without HIV. Anti-PD-1 may unmask KSHV-MCD and such KSHV-viremic patients should be excluded. Clinical benefit was noted in several tumor types. Anti-PD1 is appropriate for FDA-approved indications in this population. Patients with HIV meeting appropriate eligibility criteria should be included in IO studies. Clinical trial information: NCT02595866.

Skin ulcers can be very painful and detrimental in patients with systemic sclerosis, or systemic scleroderma. A brief review of scleroderma skin ulcers is presented, as well as a case study that demonstrates the effectiveness of becaplermin gel supplemented by oral immunosuppressive agents in the treatment of ulcers resulting from systemic sclerosis. The time to healing (approximately 3 months) was comparable to that associated with the oral agents and surgical interventions specifically designed to help heal scleroderma ulcers. Except for incisional biopsy, no surgical procedures were performed. (J Am Podiatr Med Assoc 92(6): 350-354, 2002)

Abstract:
<p>Background</p><p>There are different organizational structures and control and it has been proved difficult to find a combination between these that leads to goal congruence and improved efficiency. An organization can be structured according to hierarchical levels and the decision making can be either centralized or decentralized. Organizational control can be classified according to the amount of rules and balanced scorecard can be used to weld together the structure with the control.</p><p>Purpose</p><p>The purpose is to create an understanding of how two types of structures and control in an organization appears and also to investigate how these differ from one another concerning the balanced scorecard as a control system. We will also investigate how the variables goal congruence and efficiency are affected by the implementation of the balanced scorecard.</p><p>Method</p><p>To answer the purpose of this thesis we have mainly used the qualitative method since we have studied two organizations thoroughly through interviews on different levels of the organization. The study also has some quantitative elements since we wanted to get a more truthful picture of the organizations. Therefore we have used questionnaires to include the employees of the lower level in the hierarchy.</p><p>Conclusion</p><p>The result of the study shows that one of the organisations, which we assumed to be bureaucratic, fulfilled the assumption for the most part and the other organisation was professional which we also had assumed prior to the study. The control in the two organisations was different, in the bureaucratic the control was also bureaucratic while in the professional the control was divided into bureaucratic and clan control. The result also show that the use of the balanced scorecard on the lower level has not spread in the way the management of the organisations had expected. The goals presented in the scorecard have not lead to any goal congruence since the employees do not find the scorecard important.

Abstract:
This work proposes the computational modeling of two-dimensional media mechanical behavior with a continuous approach, related to the Finite Element Method (FEM) associated with Cohesive Fracture Mechanics (CFM) models, and a discrete approach, using the Discrete Element Method (DEM). The FEM consists in a numerical tool widely used to achieve approximate solutions of Continuum Mechanics problems, involving physical and geometrical nonlinearities phenomena with quasi-static or dynamic behaviors, having already established practical applications in many areas of science and industry. On the other hand, DEM has more recent development and has been increasingly used to model discrete nature problems involving contact, impact and fragmentation phenomena and flow of particulate systems. Focused on analysis of problems with interactions between these natures, a FEMDEM coupling code is developed to solve the problem by a sub-region scheme where the FEM is used on modeling of nucleation process and crack propagation in continuous media, and DEM is employed to model granular media, whether due its nature or its conception, in a transient behavior. The possibility of opening and propagation of cracks is considered by using CFM models, intrinsically incorporated into the FEM formulation through interfaces inserted into the inner edges of the finite element mesh.

Evaluation of carotid artery intima-media thickness in patients affected by psoriasis Psoriasis is associated with an increased risk of atherosclerosis. This study compared subclinical atherosclerosis, evaluating intima-media thickness the of the carotid in psoriasis vulgaris patients and healthy controls using high-resolution ultrasonography and the correlation of this parameter with other cardiovascular risk factors, like insulin resistance and dyslipidemia, METHODS: We will study 40 psoriasis patients, asymptomatic for cardiovascular diseases, and 40 healthy controls matched for age and sex. Intima-media thickness of the common carotid arteries will be measured ultrasonographically. Diabetes mellitus, hypertension, renal failure, a history of cardiovascular or cerebrovascular disease will be exclusion criteria. Subjects who are receiving lipid-lowering therapy, antihypertensive or anti-aggregant drugs, nitrates or long-term systemic steroids will be also excluded. Objective of this study is the evaluation of carotid artery intima-media thickness and its correlation with other blood cardiovascular risk factors in patients affected by psoriasis but asinptomatic for coronary comparing this data with the healthy control subjects. Considering that the presence of psoriasis is an independent risk factor for subclinical atherosclerosis, we want to consider this method of evaluation of cardiovascular risk and to control this risk to prevent IMA.

The objective of the present project was to develop and evaluate a solution forstorage of textiles in humid indoor environments, in this case churches. The proposed solution is based on creating a microclimate in the storage cabinets separated from the rest of the building. The cabinets were made air tight and insulated. With the use of a simple control system for conservation heating, the relative humidity can be kept below dangerous levels. The project comprises development and evaluation of textile cabinets in three churches on Gotland, Sweden. Measurements over one year show that cabinets with climate control are a simple and reliable method to prevent mould growth. Overall, the control systems have worked according to expectations. The relative humidity and temperature in the cabinets was maintained within a safe interval.

<p>Body image in women with breastcancer</p><p>Breast cancer is the second most common form of cancer in Sweden today. To get a breast cancer diagnose can be a traumatic experience for many women. It´s difficult to forsee how these women are going to control their situation. When a breast has to be removed because of cancer, the women has to adjust to changes in both physical and phychological aspects. The aim of this study was to elucidate body image in women with breast cancer after a mastectomy. Six scientific articles, which quality was critically checked, composed the basis of this study. The findings of the study were classified into three categories: The feeling of loss, ability to adjust to a changed body and the feeling of womanhood. In the findings the women describe a changed body image that affects their ordinary lives. They also brought up the feeling of not beeing able to feel like a woman and feelings of unattractiveness, a changed relationship to both partners and relatives and difficulties in accepting themselves. It is important to help these women to face their changed body image, related to their illness.</p>

Abstract:
This study aimed to understandwhetherinfection control nurses in Denmark considerthe national guidelines on infection control asuseful and relevant regarding multi-drug resistant microorganisms. We also aimedto identify potential needs and wishes to optimizeor reconsider the general and supplementary guidelines on infection controlas applied to preventing multi-drug resistant microorganisms. Methods: This qualitative study collected data from 24 Danish infection control nursesin 5 focus group discussions. Data was analyzedusing systematic text condensation. The theoretical and conceptual frame included Antonovsky’s theory of salutogenesis, communication, and health literacy. Results: Participants experienced varying levels of support from the guidelines, but mainly perceived them as a theoretical foundation and framework. They saw a great challenge in making the guidelines understandable in practice. Due to language difficulties and lake of helth literacy, some health care professionals lacked elementary knowledge of hygiene because they were unable to read and understand the text. Accordingly, the guidelines required translation and follow up in practice. The health care professionals’ knowledge and ability to access the guidelines also varied. Combined these challenges with hospitals’ arrangements and economic priorities, the guidelines’practicability was problematic. Furthermore, participants distrusted the guidelines’development processand called a new perspective that focuses less focus on making more supplementary guidelines and instead focus more on the general guidelines. Conclusions: Denmark should reconsider its level of focus on preventing multi-drug resistant microorganisms. A national collaboration could optimizethe contents, layout, and communication of the guidelines to make them understandable, useful, and relevant in preventing infections caused by multi-drug resistant microorganisms<br><p>ISBN 978-91-86739-84-3</p>

Abstract:
For a long time, the construction industry has lagged behind in the field of digitalization. It is about time for the industry to find new tools to be able to deliver a higher standard of quality assurance. This report is concerned with the application BIM 360 Field, which is a cloud based IT-tool and can be used by anyone on the construction site. The application has multiple areas of usage, including deviation handling, self-monitoring, QR-code, inspection and completion plans. The purpose of the study is to examine how effective BIM 360 Field is compared to the industry’s traditional way of handling deviations. This degree project is written in cooperation with Skanska Sverige AB and the project “J-huset” in Uppsala. The purpose is further to examine if BIM 360 Field can be applied and used for the inspection of “J-huset”.The study has been carried out through interviews with the production management on various projects. Interviews, as well as an introduction to the application, have been carried out with proficient staff from Skanska. A field study has been undertaken on the “J-huset” in Uppsala for the purpose of gaining a personal opinion as well as knowledge of the application. The result gained from the study is that BIM 360 Field has great potential for saving time during the construction process. From the interviews with production management, who used BIM 360 Field during production, some issues have been identified with the application. The conclusion in this study is that the BIM 360 Field is applicable to the “J-Huset” in Uppsala. The program is user friendly and has great potential to streamline work on quality assurance in production. In order to achieve a successful result, it is recommended to use the program at an early stage and throughout the production

Abstract:
Sudden cardiac death (SCD) is defined as unexpected death due to a cardiac disease, in a patient with or without known cardiac disease and which occurs within 1 hour from the appearance of the first clinical symptoms. The sudden cessation of cardiac activity leads to haemodynamic collapse, typically due to sustained ventricular tachyarrhythmias. The event is described as an aborted SCD (or sudden cardiac arrest) when an intervention (e.g. defibrillation) or spontaneous reversion restores circulation. The lack of uniformity with this definition complicates SCD statistics. By convention, the use of SCD to describe both fatal and non-fatal cardiac arrests persists. SCD continues to be a leading cause of death in Western countries, and accounts for 15%–20% of all natural deaths in adults in the US and Western Europe, and up to 50% of all cardiovascular deaths. In the US, estimates of SCDs from retrospective death certificate analyses range from 300 000 to 350 000 annually, giving an incidence of 0.1%–0.2% per year amongst the population above the age of 35 years. Event rates are said to be similar in Europe, although worldwide incidence is difficult to estimate and varies in accordance to the prevalence of CHD. The incidence of SCD increases with age and underlying cardiac disease. There is also a male preponderance, with men 2–3 times more likely to experience SCD than women, and this reflects the higher incidence of CHD in men.

Abstract:
Religious Dissent in Late Antiquity reconsiders the religious history of the late Roman Empire, focusing on the shifting position of dissenting religious groups. The groups under consideration are non-Christians (‘pagans’) and deviant Christians (‘heretics’). The period from the mid-fourth century until the mid-fifth century CE witnessed a significant transformation of late Roman society and a gradual shift from the world of polytheistic religions into the Christian Empire. This book demonstrates that the narrative is much more nuanced than the simple Christian triumph over the classical world. It looks at everyday life, economic aspects, day-to-day practices, and conflicts of interest in the relations of religious groups. The book addresses two aspects: rhetoric and realities, and consequently delves into the interplay between the manifest ideologies and daily life found in late antique sources. We perceive constant flux between moderation and coercion that marked the relations of religious groups, both majorities and minorities, as well as the imperial government and religious communities. Religious Dissent in Late Antiquity is a detailed analysis of selected themes and a close reading of selected texts, tracing key elements and developments in the treatment of dissident religious groups. The book focuses on specific themes, such as the limits of imperial legislation and ecclesiastical control, the end of sacrifices, and the label of magic. It also examines the ways in which dissident religious groups were construed as religious outsiders in late Roman society.

Abstract:
Guidewires for peripheral interventions come in a variety of lengths for the 0.035/0.038-, 0.018-, and 0.014-in. platforms. Common lengths include 75/80, 145, 180, 200, 260, 300, and 330 cm. Whereas a navigation wire needs only to be slightly longer than the catheter in use, guidewires over which exchanges are necessary for intervention need to be considerably longer. As a general rule, a guidewire should be long enough to be passed sufficiently beyond the treatment zone (e.g., occlusion) to facilitate adequate support with enough wire external to the patient to allow for any desired catheter or balloon use/exchange (e.g., 150 cm for a typical tibial balloon). This chapter elaborates on techniques to overcome a short wire obstacle during procedures.

Abstract:
The specificity of serine protease inhibitors belonging to the serpin superfamily depends on the nature of the reactive center amino acid residues. For example, Met→Arg mutation at the reactive center P1 residue (position 358) alters the specificity of α1-antitrypsin (AT) from the Met-specific enzyme neutrophil elastase to the Arg-specific proteases thrombin, plasma kallikrein (K) and activated Factor XII fragment (XIIf). To obtain an inhibitor species which would inhibit K and XIIf but not thrombin, we now have produced by site-directed mutagenesis of cloned AT cDNA an AT variant having Arg at P1 and Ala at P2. This modification at P2 was made because C1-inhibitor, the major inhibitor of K and XIIf, also has Ala at P2. In purified systems, AT Ala-357 Arg-358 inactivated thrombin, K and XIIf with 2nd-order rate constants of 1.1, 21.8 and 0.6 μM-1 min-1 whereas values of 8.5, 4.2 and 2.1 μM-1 min-1 were found with AT Arg-358. Thus, when compared to AT Arg-358, AT Ala-357 Arg-358 was 5.2 times more efficient for inhibiting K but 7.7 times less efficient for inhibiting thrombin. In vivo, AT Ala-357 Arg-358 (0.7 mg i.v.) did not modify the thrombin time of male Wistar rats while a 2-fold prolongation was seen with 0.7 mg AT Arg-358. However, AT Ala-357 Arg-358 (0.7 mg i.v.) partially prevented the kinin-mediated circulatory collapse induced by XIIf (0.1 μg i.v.) since rats (n=4) treated with this double mutant had a blood pressure fall of 14 ±3 (meaniSD) mmHg while control animals (n = 8) receiving saline or AT Val-358 (0.7 mg i.v.) had a decrease of 27 ± 3 mmHg (p&lt;0.01 by t test). AT Ala-357 Arg-358 has therapeutic potential for disease states with activation of the plasma kinin-forming system such as angioedema attacks or septic shock.

Abstract:
In the final step of protein C pathway, activated protein C (APC) is neutralized with a plasma inhibitor, termed protein C inhibitor (PCI). PCI was first described by Marlar and Griffin (1980) and then isolated from human plasma as a homogeneous form and characterized by the authors (1983). PCI is a single chain glycoprotein with M 57,000 and a plasma concentration of 5 ug/ml. Analysis of a cDNA nucleotide sequence has clarified that a precursor of human PCI consists of a mature protein of 387 amino acid residues (M 43,759) and a signal peptide of 19 amino acid residues. Only one cysteine residue is present in the entire protein as in α1antitrypsin (α1AT) and α1antichymotrypsin (α1ACT). Three Asn-X-Ser/Thr sequences and two Ser/Thr-X-X-Pro sequences are present as potential attachment sites of carbohydrate chains. Based on the amino acid sequence of the carboxyl-terminal peptide released from the inhibitor by APC digestion, the reactive site peptide bond of PCI was found to be Arg(354)-Ser(355). It is similar to the reactive sites of the other serine protease inhibitors which are located to their carboxyl-terminal Arg(393)-Ser (394), Met(358)-Ser(359) and Leu(358)-Ser(359) in antithrombin III, α1AT and α1ACT, respectively. The alignment of the amino acid sequence of PCI with heparin cofactor II, α1plasmin inhibitor, ovalbumin, angiotensinogen and the above noted plasma inhibitors showed that PCI is a member of serine protease inhibitor superfamily. PCI inhibits APC noncompetitively in a 1:1 stoichiometry and forms a covalent acyl-bond with a Ser residue in the active center of APC. The half life of APC in plasma approximately 30 min, which is rather slow compared with the other protease inhibitors. However, optimal concentrations of heparin, dextran sulfate and its derivatives potentiate the rate of inhibition 30-60 fold. PCI has Ki of 10-8m for APC, and can inhibit thrombin, Factor Xa, urokinase and tissue plasminogen activator as well in the presence of heparin or dextran sulfate, though the Ki for these enzymes is slightly higher. During the complex formation with APC, PCI is cleaved by the complexed APC to form a modified form with M 54,000. PCI is synthesized in several hepatoma cell lines and decreased in plasma of patients with liver cirrhosis. It is also decreased in patients with DIC or those during cardiopulmonary bypass in parallel with the decrease in protein C, suggesting that PCI participates in regulation of the protein C pathway in intravascular coagulation. Recently, we have obtained the recombinant PCI from COS-1 cells which were transfected with expression vector pSV2 containing the cDNA of PCI. The recombinant PCI had the same Mr and specific activity as the protein purified from plasma. It also had an affinity for heparin and dextran sulfate. Moreover, we have predicted a three dimentional structure of the proteolytically modified PCI with computer graphics based on its amino acid sequence homology with the modified α1AT whose structure had been elucidated with X-ray crystallography. All potential carbohydrate attachment sites were estimated to exist on the surface of the protein. Succesively we have constructed the interaction model between the intact PCI predicted from the modified form and the active center of APC which was simulated from that of trypsin. From the model, it was observed that the amino-group of Arg (354, PI site) of PCI could strongly interact with the carboxy1-group of Asp (88, SI site) of the heavy chain of APC at the base of the active center pocket of the enzyme.

Abstract:
The median age at first marriage in the United States has increased steadily since the mid-20th century. In the mid-1950s, the median age was at a record low of just over 20 for women and 22 for men, but by 2020, the median age was 28 for women and 30 for men (see Figure 1). The median age at first marriage has increased similarly for both men and women. Consequently, the gender gap in the median age at first marriage has persisted, fluctuating between 1.6 and 2.7 years. This profile uses data from the 2019 American Community Survey (ACS), 1-year estimates to track the trends in women’s and men’s median ages at first marriage. The ACS is ideal because it provides the best annual data on marital status and demographic characteristics allowing for direct estimation of the median age at first marriage (Simmons &amp; Dye, 2004). This is an update to our previous profiles on the topic for the years 2017 (FP-19-06), 2014 (FP-16-07), 2013 (FP-15-05), 2010 (FP-12-07), and 2008 (FP-09-03).
