Abstract:
We systematically investigated the effect of tube angulation on angular distortion of the anteroposterior radiograph of the foot. Three-dimensional data from the metatarsals originating from computed tomographic scans of ten healthy volunteers were projected onto the supporting surface at various tube angulations to simulate radiography. The distortion of the intermetatarsal angles decreased from 1.2° to 3.5° at 20° tube angulation to 0.4° to 2.7° at 0° tube angulation. The relatively small improvement in angular measurement using 0° instead of 15° tube angulation would not outweigh the adverse effects of changing the standard radiographic technique. Abstract Abstract 1651 Poster Board I-677 Background KSHV-associated multicentric Castleman's disease (MCD) is a rare lymphoproliferative disorder characterized by fever, splenomegaly, adenopathy, hypoalbuminemia, hyponatremia, cytopenias, elevated inflammatory markers, and a waxing and waning course. Most MCD arising in HIV-infected patients is KSHV-associated. Historically, prognosis has been poor. There is no standard therapy, although benefit has been reported with cytotoxic chemotherapy, interferon-á, retinoic acid and ganciclovir. Rituximab has reported activity in KSHV-MCD, but may not be sufficient as monotherapy in patients with severe disease, and can be associated with worsening of Kaposi's sarcoma (KS). Within a natural history study of KSHV-MCD, we evaluated the treatment effects of R-Dox on correlates of disease activity in patients with severe MCD or MCD with concurrent KS. Methods Patients with biopsy confirmed MCD that was severe or accompanied by severe KS were treated with liposomal doxorubicin 20mg/m2 plus rituximab 375 mg/m2 every 21 days until substantial clinical improvement or disease progression. Post R-Dox therapy, discussed below, was used to consolidate or maintain responses. Clinical, biochemical and radiographic response were evaluated individually using protocol-defined criteria. Overall complete responses (CR) required normalization of all clinical, laboratory or radiographic abnormalities attributed to MCD lasting at least 3 weeks. Results Twelve patients (1 woman, 11 men) have been treated with R-Dox to date. Patient characteristics: median (med) age 43 (range 34-55); all were on HAART, med CD4 331 cells/μL (21-1598), HIV viral load <50 copies/mL in 10 patients. Med number of prior therapies 2 (0-8); concurrent KS (5); dependent on steroids (3); patients hospitalized during first cycle (6). Med baseline values for biochemical response parameters: C-reactive protein 9.7 mg/dL (0.4-21.0), albumin 2.7 mg/dL (1.5-3.4), sodium 133 mEq/L (126-140), platelets 70 K/uL (10-377), hemoglobin 9.4 g/dL (6.8-12.0). 11 had diffuse adenopathy and all had splenomegaly, med spleen 18.5 cm (12.5-28 cm). Patients received med 4 cycles (3-9) of R-Dox. All patients met criteria for clinical CR after a med 2 cycles (range 1-5). Best biochemical response was CR in 9 (75%) and partial response (PR) in 1 (8%); 2 (17%) had stable biochemical parameters. Best radiographic response was CR in 6 (50%) and PR in 6 (50%) with a med 5 cm (+0.5 cm, -10 cm) decrease in spleen size. Best biochemical response was achieved after med 3 cycles (1-7), and best radiographic response after med 3 cycles (2-5). 2 of 12 had an overall CR at completion of R-Dox. Post R-Dox therapy included: IFNá (8), high-dose AZT + valganciclovir (2), additional liposomal doxorubicin (1). To date, another 6 patients achieved overall CR with additional therapy. Concurrent KS improved in 4 of the 5 patients affected. With a median potential follow-up of 25.5 months (actual follow-up range from 5.5+ to 41+ months), 9 of 12 patients have had no MCD relapse after starting R-Dox, 2 patients had recurrent MCD flares (months 7 and 17) that responded to additional therapy and 1 patient had progressive MCD during cycle 6 associated with worsening KS, and died at month 6 of central pontine myelinolysis. He was found to have primary effusion lymphoma at autopsy. An additional patient died of pneumonia (month 17). Toxicity was minimal. 9 patients had infusion reactions (Gr. 1 = 3, Gr. 2 = 4, Gr. 3 = 2) with the first dose of rituximab. 9/55 cycles were complicated by neutropenia (Gr. 2 = 7, Gr. 3-4 = 2). There were no infectious complications. Conclusions R-dox is highly effective in heavily pretreated patients with severe KSHV-MCD or MCD with concurrent severe KS. Further evaluation of R-Dox in patients with severe KSHV-MCD is ongoing. Disclosures Off Label Use: Rituximab and Liposomal Doxorubicin are being explored in the treatment KSHV-MCD.

A masterful teacher and a captivating lecturer, Charcot created the foundations of neurology as an independent discipline, and transformed the Salpêtrière hospital, in Paris, into one of the world’s greatest teaching centers for clinical neurologic research. His name is attached to the distinct pathologic entity, Charcot’s joint disease, that he so meticulously described. AbstractOur commentary is focused on three studies that used microRNA overexpression methods for directed differentiation of stem cells into insulin-producing cells. Islet transplantation is the only cell-based therapy used to treat type 1 diabetes mellitus. However, due to the scarcity of cadaveric donors and limited availability of good quality and quantity of islets for transplant, alternate sources of insulin-producing cells are being studied and used by researchers. This commentary provides an overview of distinct studies focused on manipulating microRNA expression to optimize differentiation of embryonic stem cells or induced pluripotent stem cells into insulin-producing cells. These studies have used different approaches to overexpress microRNAs that are highly abundant in human islets (such as miR-375 and miR-7) in their differentiation protocol to achieve better differentiation into functional islet beta (β)-cells.

Much of high medieval culture was deeply influenced by the reception of classical literature, as best represented by the genre of the romans antiques, the Roman de Thèbes, the Roman d’Enéas, and the Roman de Troie. These were based, in turn, on the Thebaid of Statius (92 C.E.), Vergil’s Aeneid (after 19 B.C.E.), and the story of Troy as retold by Dares Phrygias and Dictys Cretensis (in Greek, first century C.E., lost today; in Latin, fourth century C.E. [Dictys] and sixth century C.E. respectively [Dares]). Two of the most respected medieval French scholars, Joan M. Ferrante and Robert W. Hanning, now provide new access to the Roman de Thèbe through their English translation, which they have based on the personal copy owned by Henry Despenser (1370–1406), Bishop of Norwich, well known especially for his ruthless suppression of the Peasant Revolt in 1381. This manuscript is today housed in the British Library, London, under Add. 34114, fol. 164a-226d, and it was critically edited by Francine Mora-Lebrun with a facing page modern French translation in 1995. Ms. A (Paris, BnF, fr. 375) was recently edited by Luca di Sabatino (2016), which could not be consulted here for obvious reasons. Ms. C (Paris, BnF, fr. 784) was edited by Guy Reynaud de Lage in 1966, 1968, then re-edited along with a facing-page modern French translation by Aimé Petit in 2008).

INTRODUCTION: Airplane headache (AH) is unique to plane travel and looks like a short duration migraine attack or paroxysmal hemicrania (PH) attack without any autonomic symptoms. Until now, there has been no documented association between AH and PH.CASE REPORT: We report a 50-yr-old healthy woman with a very severe and sudden pulsating headache located in the left frontal region with radiation into the left eye during takeoff which diminished within 10-15 min during her airplane journeys.DISCUSSION: The patient was diagnosed with AH and she had good response to indomethacin. The pain was unique to plane travel but looked like PH. We discuss the association between AH and PH attack in the light of diagnostic criteria and therapeutic approach in the report.Koçer A. This article reviews the extent of health-care students’ computer literacy and presents the results of a survey of podiatric medical students’ computer literacy. The results of this survey indicate that podiatric medical students are more likely than other health-care students to rate their computer literacy as good or very good. There was no gender difference in this self-reported computer knowledge. Current cloning efficiencies are relatively low and there is evidence to suggest that a less differentiated cell type can increase these (Jaenisch et al. 2002 Cloning and Stem Cells 4, 389-396). As part of an ongoing study that aims to develop cloning as a breeding tool for the pig industry, we examined whether mesenchymal stem cells (MSCs) isolated from bone marrow (Experiment 1) and blood (Experiment 2) could increase cloning efficiencies compared with fibroblasts isolated from ear tissue of live animals. MSCs were isolated from the femurs and blood of two pigs over a Histopaque-1077 density gradient. Cells were seeded and cultured in DMEM supplemented with 10% fetal bovine serum (FBS) and antibiotics/antimycotics. Blood MSCs were plated onto fibronectin-coated dishes (Faast and Nottle, Proc. Australian Health Med. Res. Congress 2004, abst. 1272). Cells from passages 2-4 were used for nuclear transfer by means of a fusion before being subjected to the activation protocol described previously (Harrison et al. 2004 Cloning and Stem Cells 6, 327-331). Embryos were cultured in a modified NCSU23 for 7 days. In Experiment 1 there was no difference in the number of fused embryos that cleaved in the bone marrow MSC and fibroblast groups (374/447; 84% vs. 370/446; 83%, respectively; 9 replicates). The number of embryos that developed to the blastocyst stage by day 7 was significantly higher in the bone marrow MSC group compared with the fibroblast group (136/447; 30% vs. 79/446; 18%, respectively, Chi square test; P < 0.01). In Experiment 2 there was no difference in the percentage of embryos that cleaved in the blood MSC and fibroblast groups (287/375; 77% vs. 275/347; 79%, respectively; 8 replicates). The number of embryos that developed to the blastocyst stage by day 7 was also similar between the blood MSC and fibroblast groups (67/375; 18% vs. 63/347; 18%, respectively). In conclusion, our results suggest that bone marrow MSCs may be a more efficient cell type, compared with fibroblasts, for the cloning of live animals.

Rhabdomyosarcoma is the most common soft tissue sarcoma of childhood. The aim of this study was to identify molecular events involved in rhabdomyosarcoma onset for the development of new therapeutic approaches against specific molecular targets. BALB-p53neu mice develop pelvic rhabdomyosarcoma and combines the activation of HER-2/neu oncogene with the inactivation of an allele of p53 oncosuppressor gene. Gene expression profiling led to the identification of genes potentially involved in rhabdomyosarcoma genesis and therefore of candidate targets. The pattern of expression of p53, HER-2/neu, CDKN2A/p19ARF and IGF-2 suggested that these alterations might be involved in gender-, site- and strain-specific development of rhabdomyosarcoma. Other genes such as CDKN1A/p21 might be involved. The role of IGF-2, CDKN2A/p19ARF and CDKN1A/p21 in tumor growth was investigated with siRNA in murine rhabdomyosarcoma cells. Silencing of p19ARF and p21 induced inhibition of growth and of migration ability, indicating a possible pro-tumor and pro-metastatic role in rhabdomyosarcoma in absence of p53. In addition the autocrine IGF-2/IGF-1R loop found in early phases of cancer progression strengthens its key role in sustaining rhabdomyosarcoma growth. As rhabdomyosarcoma displays defective myogenic differentiation, a therapeutic approach aimed at enhancing myogenic differentiation of rhabdomyosarcoma cells. Forced expression of myogenin was able to restore myogenic differentiation, significantly reduced cell motility and impaired tumor growth and metastatic spread. IL-4 treatment increased rhabdomyosarcoma cell growth, decreased myogenin expression and promoted migration of cells lacking myogenin. Another approach was based on small kinase inhibitors. Agents specifically targeting members of the HER family (Lapatinib), of the IGF system (NVP-AEW541) or downstream signal transducers (NVP-BEZ235) were investigated in vitro in human rhabdomyosarcoma cell lines as therapeutic anti-tumor and anti-metastatic tools. The major effects were obtained with NVP-BEZ235 treatment that was able to strongly inhibit cell growth in vitro and showed anti-metastatic effects in vivo.

The objective of the present project was to develop and evaluate a solution forstorage of textiles in humid indoor environments, in this case churches. The proposed solution is based on creating a microclimate in the storage cabinets separated from the rest of the building. The cabinets were made air tight and insulated. With the use of a simple control system for conservation heating, the relative humidity can be kept below dangerous levels. The project comprises development and evaluation of textile cabinets in three churches on Gotland, Sweden. Measurements over one year show that cabinets with climate control are a simple and reliable method to prevent mould growth. Overall, the control systems have worked according to expectations. The relative humidity and temperature in the cabinets was maintained within a safe interval.

Kläder och identitet är starkt förknippade eftersom människan mer eller mindre är beroende av kläder, åtminstone på våra breddgrader. Med kläder visar vi också vilka vi är och vilken grupp vi tillhör. Vår studie syftar till att undersöka hur lärare förhåller sig till ett införande av obligatorisk skoluniform. Fokus ligger på hur ett införande av obligatorisk skoluniform påverkar eleverna i relation till mobbning. Metoden som användes var kvalitativ och datainsamlingen skedde genom tio intervjuer. Intervjuerna berörde frågor med anknytning till teorier kring mobbningens orsaker samt även påverkan på skolans elever vid ett införande av obligatorisk skoluniform. I resultatet framkom det ett flertal faktorer som ett införande av obligatorisk skoluniform för med sig. Slutsatsen av vår studie blev att det fanns både för och nackdelar för skolan och dess elever med skoluniform. Nackdelar som framkom var bland annat att eleven fråntas sin identitet då den inte kan framhävas genom klädsel. Fördelarna med skoluniform är att ingen avviker genom klädseln och därmed kan mobbningen minskas.

Den relativt nya företeelsen BrandME – I am the brand är en eTjänst som fungerar som ett virtuellt provrum och därmed gör det möjligt att kunna prova på kläder virtuellt. Till sin hjälp har man bland annat använt sig av 3D-grafik samt databaser. BrandME är en produkt av det kanadensiska företaget My Virtual Model, ett företag som har arbetat med att sätta standarder för virtuell identitet sedan år 2000.Då denna företeelse är relativ ny, skapar detta utrymme för undersökning och bedömning om huruvida välutvecklad och välanpassad BrandME är. Undersökningens syfte är att utvärdera BrandME för att sedan kunna fastställa nyttan med denna eTjänst. Under utvärderingen koncentrerar vi oss dels på användbarheten hos BrandME och dels på utformningen av BrandME.

Uppsatsens syfte är att undersöka hur Sveriges Lärarförbund arbetade med införandet av en ny läroplan, Lgr 80. De frågor som jag har valt att inrikta mig på är Hur Sveriges Lärarförbund arbetade med Lgr 80 och vilka förändringar som Sveriges Lärarförbund ville göra i Lgr 80. Sveriges Lärarförbund var med under hela arbetet med att ta fram den nya läroplanen genom att både arbeta med läroplanen inom förbundet och att vara med i de arbetsgrupper som skol-överstyrelsen tillsatt. Källor som jag främst har använt mig av till undersökningen är Sveriges Lärarförbunds remissyttrande, Läroplansförslaget och sammanträdesprotokoll från Sveriges Lärarförbund.Nyheter som Lgr 80 innebar var bland annat en revidering av betygssystemet, decentraliserad skola och ett nytt tillvalssystem. Sveriges Lärarförbund kunde påverka arbetet med läroplanen både genom sin medverkan i arbetsgrupperna samt genom sitt remissyttrande.

På bara ett par år har elektronisk handel potential att radikalt förändra de ekonomiska och sociala förhållanden i samhället. Internets snabba tillväxt kopplar samman individer och organisationer till ett stort globalt nätverk som representerar enorma möjligheter. Det talas om ett nätverkssamhälle och en nätverksekonomi.

Elektronisk handel driver företag mot att bedriva affärer på ett fundamentalt nytt sätt. Behovet av att dela information i realtid ökar tillsammans med förändringarna i värdekedjan.

Syftet med studien är att kartlägga elektronisk handel i allmänhet. Studien fokuseras på hur elektronisk handel används för att länka samman företag, stora som små, från råmaterial till konsumenten. Arbetet belyser hinder och krav i samband med elektronisk handel och det sätt de hanteras på.

Företagen i undersökningen visar ingen tydlig strävan mot att uppnå en totalt integrerad värdekedja. Grundliga affärsprocesser flyttas mot Internet för att utnyttja nätverkets fördelar. Främst används EDI och online-tjänster som Internetbutiker för elektronisk handel.

Abstract:

Denna studie syftade till att undersöka relationsbaserad och kompetensbaserad självkänsla hos personer med fibromyalgi. Detta gjordes genom att jämföra enkätresultat hos 26 personer som fått diagnosen fibromyalgi med 39 friska personer i en kontrollgrupp. Resultaten kunde inte påvisa någon skillnad i kompetensbaserad självkänsla mellan grupperna men där fanns en tendens till högre relationsbaserad självkänsla i patientgruppen.

In many institutional settings in today’s globalized job market, people have to deal with different role asymmetries in the co-construction of meaning. In this study, institutional, cultural and linguistic asymmetries are focused on in interviews at an employment agency in Sweden. Interviews between a recruiter and fourteen female job candidates with an academic background from other countries were video taped. Three sequences on personality were analysed: What do you consider to be your strengths? What personal characteristics do you want to improve? and What has made an impact on you? The general aim of the study was to gain knowledge of the processes whereby self-presentations are co-constructed and how participants try to reach common understanding when they do not share common linguistic and cultural resources. Theoretically, the study has a dialogical framework. Discourse is seen as the place where society, culture, situation, individual and language meet and where meaning is constructed through social action. Within an interactional sociolinguistics framework, an holistic approach to methods combines ethnography of communication with ethnomethodology and conversation analysis. The results show that the meaning-making project in this institutional situation is institutionally framed, culturally hidden, socially constrained by face-work and interactionally embedded. The recruiter orients to the institutional frame by embedding reformulations of the candidates’ answers in her uptake, often an adjective, which is filled in on a form and later transferred to a data base. The recruiter also takes on the face-work of the communicative dilemmas that the questions exhibit, for example by using explanations when candidates admit to low self-confidence. It is also shown that for some candidates the hidden agenda of the situation is concealed and that their communicative styles clash with the recruiter’s expectations. The asymmetrical situation can for the candidates be seen as both a resource and a constraint. The linguistic asymmetry is not foregrounded. Instead, the negotiation of meaning concerns the institutional and cultural frame rather than linguistic meanings. On the other hand, the recruiter shows a tendency to normalize the candidates according to her own institutional and cultural knowledge. This dynamic interplay between heterogenization and homogenization tendencies is an important feature in the interviews.

Introduction: Pain is a subjective experience. People subjected to pain often experience a reducedquality of life. Chronic pain is pain that has been going on for at least three

months.

Problem area: People with chronic pain are subjected to unnecessary suffering and disability. Therefore it is crucial that the nursing staff takes their situation seriously.

Purpose: The purpose of the study was to describe the patient's’ experiences and apprehension

of living with chronic non-malignant pain, and how this affected their everydaylife.

Method: Systematic analysis of 23 depicting articles. The databases used were Medline via PubMed and Sience Direct. The text in the chosen articles was categorized on the

basis of content.

Result: The study showed that people subjected to chronic pain feel that their everyday life

is very limited. They dealt with pain differently, according to their life situation. Many of them had a feeling of loss and they thought had lost something in their life.

Discussion/conclusion: Living with chronic pain brings significant changes to the sufferers everyday life. It is crucial that they receive sufficient support by people around them and their nursing staff to function in everyday life.

The goal of this study is to evaluate and compare the thermodynamic performance of three feasible hybrid solar power tower-desalination plants for co-generation of power and fresh water. In these hybrid configurations, either multi effect desalination (MED) or thermal vapor compression (TVC)-MED unit is integrated to the Rankine cycle power block. The particular focus is on comparison between single plant and hybrid plants in terms of energy efficiency and penalty in power production to determine the more efficient configuration. The achieved results showed that integration of MED unit to the power cycle is thermodynamically more efficient, due to less reduction in power production and efficiency than the TVC-MED configurations. Also, for hybrid solar tower-MED plat, the average penalty in power production was between 9.27% and 12.88% when fresh production increased from 10000 m3/day to 31,665 m3/day. Another important finding showed the specific power consumption (specific power penalty) of the hybrid plant decreases with increasing the fresh water production. Especially at higher fresh water production, this specific power consumption was competitive to other desalination technologies such as reverse osmosis. The proposed hybrid solar tower-MED plant offers different benefits such as possibility of eliminating the cooling system requirement of the cycle as it can be replaced by the MED unit.

Abstract Here we perform a steady state thermodynamic analysis of a cogeneration cycle capable of producing power through a supercritical Carbon Dioxide (sCO2) recompression Brayton cycle and water through a Multi Effect Distillation (MED) plant. The integration considers a demineralized water loop heated by rejected sCO2. Thus, the performance of the power block is independent of the desalination block. Each block of the system is independently analyzed to identify the operational or design parameters that influence the thermodynamic efficiency and water and power production. For a power block producing 112.6 MW, with a fixed turbine inlet temperature, the pressure ratio (PR) is the most influential variable in order to maximize net power production and thermal efficiency. For the desalination block, producing 2659 m3/d of water, the last effect temperature (TN) is the most influential variable for water production. In the integrated cogeneration system, increasing PR beyond the power block optimized value (From PR = 3.5 to 3.96) reduces power (from 112.6 to 112.4 MW), but increases water production (from 35 to 40 kg/s), and decreases the specific energy consumption from 29.4 kWh/m3 of water to 26.5 kWh/m3 of water.

Alcohol has several effects on hemostasis that might predispose for thromboembolism. The association between alcohol consumption and venous thrombosis is sparesly documented and we have therefore conducted a prospective study.Methods: 395 consecutive patients with suspected DVT entered the study. The patients were questioned about consumption of beer, wine and liquor and the average weekly consumption of ethanol was estimated. The total weekly consumption of tobacco was also estimated. 302 of the patients (130 men and 172 women) underwent phlebography, which was classified either as normal or as fresh DVT or/and postthrombotic changes.Results: 31 of the 130 men reported a weekly consumption of 100 g ethanol or more. In men below 65 years of age (62 men, see table) there was a significant relationship between ethanol consumption and the presence of fresh DVT or/and postthrombotic changes at phlebographyIn men above 65 years of age the incidence of DVT/postthrombotic changes was not higher among those who consumed more than 100 g ethanol per week (9 of 14) than among them who consumed less than 100 g per week (35 of 54). Only 5 of the women admitted a consumption of at least 100 g ethanol per week, one of them had a DVT.Tobacco consumption was not related to the occurrence of DVTConclusion: Ethanol seems to be a risk factor for DVT in young and middle-aged men
