Abstract:
The authors measured the thickness of the medial, central, and lateral bands of the plantar fascia using ultrasonographic techniques in 109 symptomatic patients with 211 painful heels. Plantar fasciitis was diagnosed by the presence of plantar heel pain and tenderness of the plantar fascia on palpation and was correlated with plantar fascia thickness. All of the symptomatic feet had medial band tenderness, with an average thickness of 5.9 mm, 68% had central band tenderness, with an average thickness of 5.3 mm, and 26% had lateral band tenderness, with an average thickness of 4.4 mm. The average thickness of all symptomatic bands was 5.35 mm, which was significantly greater than that for all asymptomatic bands, which was 2.70 mm. There were also significant differences in the thickness of the three plantar fascia bands in symptomatic patients. A plantar fascia index was established consisting of the ratio of the mean thickness of symptomatic medial, central, and lateral plantar fascia bands to that of asymptomatic bands; for this study, the index value is 1.98 (5.35/2.70 mm). Abstract:
Swelling of the leg is a frequent finding in podiatric medicine. Unless the patient draws notice to the condition or the physician is diligent in asking questions and examining the extremity, the condition may be viewed as minor and given little attention. Swelling of the leg can be a clinical sign of certain systemic diseases, lymphatic and venous disorders, or other miscellaneous causes. Dermatologic consequences linked to many of these causes of swelling are well documented. What is not as well chronicled is a dramatic skin alteration associated with chronic lymphedema. This article provides an overview and a case presentation of the unusual and debilitating skin condition related to chronic lymphedema of the lower extremity known as elephantiasis nostras verrucosa. Abstract:
AbstractBackgroundClinically relevant potential drug-drug interactions are considered preventable adverse drug reactions.ObjectiveThe aim of this study was to ascertain the frequency of potential drug-drug interactions in acute ischemic stroke patients and to explore factors associated with occurrence of potentially contraindicated drug-drug interactions.MethodsThis observational retrospective cohort and nested case-control study was carried out among patients treated for acute ischemic stroke at the Neurological Intensive Care Unit in the Clinical Centre Kragujevac, Serbia. The potentially drug-drug interactions for each day of hospitalization were identifi ed using Micromedex® soft ware. Based on the existence or absence of potentially contraindicated drug-drug interactions, the participants were divided into a group of cases (n=111) and the control group (n=444).ResultsA total of 696 patients were analysed. All patients had a minimum of one potential drug-drug interaction during hospitalization. The most common drugs involved in potential drug-drug interactions were aspirin (8.02%), diclofenac (7.49%) and warfarin (7.14%). The number of medications prescribed for simultaneous use during hospitalisation and the use of antipsychotics in therapy signifi cantly increased the likelihood of potentially contraindicated drug-drug interactions aft er adjustment by means of logistic regression for 1.2 and 3 times, respectively.ConclusionsThis study suggests that patients with acute ischemic stroke are frequently exposed to potential drug-drug interactions. It is essential to identify potentially drug-drug interactions in these patients as early as possible in order to prevent adverse drug reactions and ensure safe recovery. Besides, full attention should be paid when adding each new medication in therapy, particularly when a neurologist decides to prescribe antipsychotics, such as risperidone.

Abstract:
AbstractI introduce an organizational model describing the response of the hospital emergency department. The hybrid simulation/analytical model (called a “metamodel”) can estimate a hospital’s capacity and dynamic response in real time and incorporate the influence of damage to structural and nonstructural components on the organizational ones. The waiting time is the main parameter of response and is used to evaluate the disaster resilience of health care facilities. Waiting time behavior is described by using a double exponential function and its parameters are calibrated based on simulated data. The metamodel covers a large range of hospital configurations and takes into account hospital resources in terms of staff and infrastructures, operational efficiency, and the possible existence of an emergency plan; maximum capacity; and behavior both in saturated and overcapacitated conditions. The sensitivity of the model to different arrival rates, hospital configurations, and capacities and the technical and organizational policies applied during and before a disaster were investigated. This model becomes an important tool in the decision process either for the engineering profession or for policy makers.(Disaster Med Public Health Preparedness. 2014;8:436-444)

Abstract:
AbstractDetermining the most appropriate level of care for patients in the prehospital setting during medical emergencies is essential. A large body of literature suggests that, compared with Basic Life Support (BLS) care, Advanced Life Support (ALS) care is not associated with increased patient survival or decreased mortality. The purpose of this special report is to synthesize the literature to identify common study design and analytic challenges in research studies that examine the effect of ALS, compared to BLS, on patient outcomes. The challenges discussed in this report include: (1) choice of outcome measure; (2) logistic regression modeling of common outcomes; (3) baseline differences between study groups (confounding); (4) inappropriate statistical adjustment; and (5) inclusion of patients who are no longer at risk for the outcome. These challenges may affect the results of studies, and thus, conclusions of studies regarding the effect of level of prehospital care on patient outcomes should require cautious interpretation. Specific alternatives for avoiding these challenges are presented.LiT, JonesCMC, ShahMN, CushmanJT, JuskoTA. Abstract:

Abstract:
Aim of the study: To assess whether perfusion changes in one target hepatocellular carcinoma (HCC) lesion, assessed quantitatively by contrast-enhanced ultrasonography (CE-US) at week 2 and 4 of sorafenib therapy, can predict progression of disease at week 8 assessed by contrast enhanced-multidetector computed tomography or magnetic resonance imaging (CE-MDCT or MRI) using RECIST/modified RECIST criteria (response evaluation criteria in solid tumors). Patients and methods: The institutional review board approved the study and all patients provided written informed consent before their enrollment. The study was performed on a sample of subjects with advanced HCC or that was not amenable to curative treatment, in single-agent sorafenib therapy. Tumor response was based on CE-CT or CE-MRI at 2 months using RECIST/modified RECIST criteria. Contrast-enhanced US was performed within 1 week before sorafenib treatment start and during therapy at week 2, 4, 8, 16 and 32. Functional quantitative parameters were assessed using a dedicated software. The changes in dynamic US functional parameters between time 0 and the later time points were compared with tumor response based on RECIST/modified RECIST criteria. Results: The decrease in tumor perfusion parameter values, in particular WiAUC and PE (related to the blood volume), at week 2, 4, predicts the tumor response at 2 months, assessed using RECIST and modified RECIST criteria, that was indicative of stable disease (responders). Conclusion: Dynamic US can be used to quantify changes in tumor vascularity as early as week 2, 4 after sorafenib administration in patients with HCC. These early changes in tumor perfusion may be predictive of tumor response at 2 months and they may have potential in the early evaluation of the effectiveness of antiangiogenic therapy in HCC.

Abstract:
The aim of this work was to study the inhibition of aerobic glycolysis (the main metabolic pathway used by cancer cells to produce energy) achieved by blocking the enzyme lactate dehydrogenase (LDH). My activity has focused on the possibility of using this approach in order to improve the efficacy of anticancer therapy, evaluating its effects on cultured human hepatocellular carcinoma. In order to evaluate the effects of LDH inhibition, in a first set of experiments, we used oxamic acid (OXA). This compound is the only known specific inhibitor of LDH; it is a non-toxic molecule in vivo, but it is active at too high concentrations to allow its therapeutic use. An important result was the demonstration that LDH inhibition achieved by OXA not only triggers cell death signals but, when combined with the administration of sorafenib, also greatly increases the efficacy of this drug, leading to a synergistic effect. This strong potentiating effect of the drug action has been explained with the demonstration that sorafenib has the ability to inhibit the oxygen consumption of the treated cells, making them more dependent on glycolysis for ATP generation. In collaboration with the Department of Pharmaceutical Sciences our research group has identified a compound (galloflavin) that inhibits LDH with much higher efficacy than OXA. To our knowledge, inhibition of LDH is the only biochemical effect described for galloflavin. According to the results obtained on hepatocarcinoma cultured cells, galloflavin might be a promising lead candidate in the field of tumor metabolic inhibitors, deserving a more exhaustive evaluation as a potential anticancer agent.

Abstract:

Abstract:

Abstract:

Abstract:
The interviews and audio recordings comprised in this volume are the outcome of a field research to Maharashtra and Andhra Pradesh which took place in October-November 1985 for the preparation of the Author’s BA thesis on the life and teachings of the Indian saint Sai Baba of Shirdi (d. 1918), discussed at the Ca’ Foscari University of Venice in June 1987 (Un maestro dell’India moderna: il Sāī Bābā di Śirḍī. L’uomo, l’ambiente, gli insegnamenti). The conversations of each day are preceded by excerpts from the Authorʼs diary. These testimonies record the words of various people in the village of Shirdi and other locales, among whom are the last old men who knew Sai Baba: Balaji Pilaji Gurav, Bappaji Lakshman Ratna Parke, Martanda Mhalsapati, Pandharinath Bhagavant Gonkar, Tukaram Raghujiv Borawke, and Uddhavrao Madhavrao Deshpande. While in Shirdi the Author was also able to interview Uttamrao Patil, son of Tatya Kote Patil, and in Mumbai he had the privilege of meeting Swami Ram Baba, who first met the saint in 1914. Taken altogether, these conversations are primary sources for the study of Sai Baba and may help to contextualize Shirdi as a pilgrimage place in the mid-1980s.

Voltage sensitive dye imaging (VSD) can be used to record neural activity in hundreds of locations in preparations ranging from mammalian cortex to invertebrate ganglia. Because fast VSDs respond to membrane potential changes with microsecond temporal resolution, these are better suited than calcium indicators for recording rapid neural signals. Here we describe methods for using a 464- element photodiode array and fast VSDs to record signals ranging from large scale network activity in brain slices and in vivo mammalian preparations, to action potentials in over 100 individual neurons in invertebrate ganglia.

Abstract:
Employees represent a key part of the organization. Their satisfaction has direct effect on co-workers and their productivity. Leader is largely responsible to create general satisfaction in work environment. With organizing, choosing appropriate management concepts and correct approach to his employees, he creates good relationships with colleagues and contributes to the internal culture of the organization. Good interpersonal relationships are thus an important factor in building success in the company and the operation of processes, as they directly affect work performance, organizational climate and organizational culture. Many companies use classic and wellknown leadership styles. Regardless of the chosen style, it is noticed an alienated relationship between the manager and the employee. The issue can be attributed to strict adherence to the set organizational structure and, in connection with this, the expression of the legitimate power of leaders, yet to some extent the problem is in different understanding of leadership, both with individuals and leaders themselves. The newer leadership style, called Leader-member Exchange Theory, focuses on building relationships, mutual respect and trust, and treating employees as co-workers on the same horizontal hierarchical level. The implementation of this style of management can be seen mainly in foreign markets. This approach is not well known or widespread in Slovenia, nevertheless it is noticed in some companies. Based on a case study from the business environment, we studied a company with such leadership in Slovenia.

Abstract:
Security self-organization is a natural right of an individual and a social group. Still, this right is, in conditions of a high degree of institutionalization of security systems, (partially) regulated by the states – both in declarative and legal manners. In Slovenia, this right has been explicitly provided in the resolutions of the national security strategy (1993, 2001, 2010, 2019), as well as in some key legislation from the field of the internal security system, the defence system, and the system of protection against natural and other disasters. Given that there has been no excessive interest in security self-organization in the last three decades in practice, it seems that also the state did not put many interests in the development of this area, neither in terms of support nor in terms of restrictions. However, the sudden appearance of the paramilitary guards (slov. varde) has initiated a vigorous debate in the professional and lay public, showing that a fair share of civil society and security professionals are unwilling to tolerate security self-organization that tries – self-proclaimed and self-assessed – to fill the security deficit of state organizations.

Abstract:
In 2020 the entire world was severely affected by COVID -19 epidemics. Because of preventing of the SARS Cov 2 virus spread there was necessary to introduce considerable restrictions of movement of citizens. This resulted in various negative effects in the field of economy. However, as road traffic is an important source of pollution – particularly of emissions of air pollutants and greenhouse gases it could be expected that these mobility restrictions result in certain positive effects on the environment. The paper will try to estimate the air quality during the period of epidemics when mobility was severely restricted (second half of March and April 2020, second half of October, November and December 2020). The review will be limited to those kinds of pollutants which are particularly related to road traffic (nitrogen oxides, particulate matter, ozone) as well as to carbon dioxide as the most important greenhouse gas.

The paper focuses on the comparison of safety and security of cyberspace users in urban and rural environments. The study involved 1,158 participants from 100 municipalities across Slovenia. Findings showed that residents of urban and rural settings were most frequently victimized with the following threats: 1) pop-up windows that required re-entering of the user name and password to obtain users’ data, 2) receiving a fake email with the infected attachment, and 3) fake online advertising. Residents in both environments highlighted that they feel most vulnerable to receiving a fake e-mail with the infected attachment. Moreover, they expose this threat and extortion with their own sexually explicit materials as those that would cause them the most harm.

Abstract:
The key component of WWER is the Reactor Pressure Vessel (RPV). The evaluation and prognosis of RPV material embrittlement and the allowable period of its safe operation are performed on the basis of impact test results of irradiated surveillance specimens (SS). The main problem is that the SS irradiation conditions (temperature of SS, neutron flux and neutron spectrum) have not been determined yet with the necessary accuracy. These conditions could differ from the actual RPV condition. In particular, the key issue is the possible difference between the irradiation temperature of the SS and the actual RPV temperature. It is recognized that the direct measurement of temperature by thermocouples during reactor operation is the only way for receiving reliable information. In addition, the neutron field’s parameters for surveillance specimens have not been determined yet with the necessary accuracy. The use of state of the art dosimeters can provide high accuracy in the determination of the neutron exposure level. The COBRA project (http://ie.jrc.cec.eu.int/ames/), which started in August 2000 and had a duration of three years, was designed to solve the above-mentioned problems. Surveillance capsules were manufactured which contain state of art dosimeters and temperature monitors (melting alloys). In addition, thermocouples were installed throughout the instrumentation channels of the vessel head to measure directly the irradiation temperature in the surveillance position during the reactor operation. The selected reactor was the Unit 3 of Kola NPP situated in the arctic area of Russia. Irradiation of the capsules and online temperature measurements were performed during one fuel cycle. On the base of statistical processing of thermocouples readings the temperature of irradiated surveillance specimens in WWER-440/213 reactor can be accepted as 269.5±4°C. The results obtained show that there is not need in temperature correction when data of surveillance specimens studies are used for assessment of WWER-440/213 reactor pressure vessels. Maximum neutron flux evaluated using detectors, which were placed in the Charpy specimen simulators, equals ∼2.7·1012 cm−2s−1 with E>0.5 MeV. It is established that depending on the orientation of the capsules with respect to the core, the detectors of the standard surveillance capsules can give both overestimated and underestimated neutron flux values, as compared to the actual flux received by the surveillance specimens. The overestimation or underestimation can reach 10%.

Abstract:
Since 1991 the European Commission has financed a significant number of Technical Assistance Projects to the Commonwealth of Independent States (TACIS) and EURATOM R&D actions addressing the main safety issues on RPV material embrittlement and integrity assessment. Since the VVER 440 reactors of the reference series 213 are made from recognised neutron embrittlement resistant materials and include comprehensive surveillance programmes, a standard plant life management procedure can be applied to address long-term concerns, mostly aiming at reducing uncertainties in the assessment techniques. Therefore, the open issues (flux effect, upgrading of surveillance results, implementation of toughness measurements and relevant acceptance criteria, behaviour of the cladding) are quite universal. The efficiency of late annealing (at fast [E>0.5 MeV] neutron doses over 1020n/cm2) and the re-embrittlement after annealing remain key issues for any final decision for their operational lifetime. The more recently developed VVER 1000 reactors have some well-known features arising from the original design and manufacturing process (high nickel content in the core weld, location of the surveillance specimens), which have to be carefully considered if appropriate mitigation measures are to be implemented during operation. A precise identification of the issues related to the surveillance programme has been achieved thanks to research on dosimetry evaluation, representativeness (temperature and flux effect) of the specimens and optimisation of the evaluation of their results. Nickle, just as copper and phosphorus, is now recognised as having a detrimental effect on neutron embrittlement in synergy with other elements (e.g. manganese). The analyses of available data for CrNiMn steels do not show a significant effect for fast [E>0.5 MeV] neutron doses below 7.1019n/cm2, but their consistency and relevance might be questionable. A way has already been pioneered which shows how valuable results can be obtained using the existing surveillance programmes specimens. A systematic application on the Russian & Ukrainian plants is now planned in order to get updated figures on design end of life (EOL) integrity assessment. This includes updated dosimetry assessment, multiple specimen testing (reconstitution, impact and static toughness tests) and advanced integrity analyses. An optimised database of representative surveillance results (up to the design end of life) is expected, which should provide a sound basis for further understanding and setting up of relevant prediction tools, considering at the same time any other specific R&D results. The global integrity assessment will also provide for preparing and implementing adequate mitigation measures in due time, if necessary. The paper will report about the knowledge on RPV embrittlement effects, providing evidence of recent contributions to solve shortcomings of the VVER 440/213 and 1000 units. The current state-of-the-art and the remaining open issues have been assessed recently by a group of international experts. The planned R&D activities and the detailed scope of the latest TACIS projects are described.

Abstract:
The Indiana Department of Transportation (INDOT) completed a total of 25 high friction surface treatment (HFST) projects across the state in 2018. This research study attempted to investigate the durability and performance of HFST in terms of its HFST-pavement system integrity and surface friction performance. Laboratory tests were conducted to determine the physical and mechanical properties of epoxy-bauxite mortar. Field inspections were carried out to identify site conditions and common early HFST distresses. Cyclic loading test and finite element method (FEM) analysis were performed to evaluate the bonding strength between HFST and existing pavement, in particular chip seal with different pretreatments such as vacuum sweeping, shotblasting, and scarification milling. Both surface friction and texture tests were undertaken periodically (generally once every 6 months) to evaluate the surface friction performance of HFST. Crash records over a 5-year period, i.e., 3 years before installation and 2 years after installation, were examined to determine the safety performance of HFST, crash modification factor (CMF) in particular. It was found that HFST epoxy-bauxite mortar has a coefficient of thermal expansion (CTE) significantly higher than those of hot mix asphalt (HMA) mixtures and Portland cement concrete (PCC), and good cracking resistance. The most common early HFST distresses in Indiana are reflective cracking, surface wrinkling, aggregate loss, and delamination. Vacuum sweeping is the optimal method for pretreating existing pavements, chip seal in particular. Chip seal in good condition is structurally capable of providing a sound base for HFST. On two-lane highway curves, HFST is capable of reducing the total vehicle crash by 30%, injury crash by 50%, and wet weather crash by 44%, and providing a CMF of 0.584 in Indiana. Great variability may arise in the results of friction tests on horizontal curves by the use of locked wheel skid tester (LWST) due both to the nature of vehicle dynamics and to the operation of test vehicle. Texture testing, however, is capable of providing continuous texture measurements that can be used to calculate a texture height parameter, i.e., mean profile depth (MPD), not only for evaluating friction performance but also implementing quality control (QC) and quality assurance (QA) plans for HFST.
