AbstractThere are undoubtedly many factors that contribute to inter-examiner variability relevant to the use of medical practitioners in justiciable matters. One source of variability with regard to claims relating to hearing disorders could well be the training and ‘calibration’ of medical examiners. A tentative analysis of the examination papers and of the declared roles of the specialties that provide these examiners lends support to such a thesis. One solution would be to train special specialists for medicolegal work, as envisaged by Boyarsky for forensic urology (Boyarsky, 1996). At the same time there is the need to change the role-perception of many examiners. There is also the need for medical examiners to express honest, unbiased opinions. There are also problems inherent in the litigation process which does not promote the interactive and adaptive processes between experts that characterise scientific discussions and enquiry.

Resumo: A Violência Familiar representa uma das faces mais cruéis da violência, pois atinge pessoas indefesas, geralmente mulheres e crianças, nos seus próprios lares, ambiente que Ihes deveria garantir proteção e bem-estar. A despeito das leis que visam garantir a defesa dos direitos das crianças e adolescentes, diversas formas de violência continuam a acometer diariamente estes seres em formação, produzindo pessoas psicologicamente desajustadas e propensas à continuidade do comportamento violento em suas relações. O presente trabalho analisa o perfil epidemiolágico das crianças e adolescentes vítimas de violência familiar na Região Metropolitana da Grande Vitória, nos anos de 2002 e 2003, examinadas no Departamento Médico Legal da citada região. Foi realizado um estudo retrospectivo dos laudos de Exame de Lesões Corporais dos arquivos daquele Departamento, sendo detectados 895 casos de crianças e adolescentes vítimas de agressões não-fatais impostas pelos seus familiares. Ao contrário de outros trabalhos, a maior parte das vítimas era do sexo feminino (567 - 63,35%), contra 328 (36,64%) do sexo masculino. Na maioria dos casos (482 - 53,85%) os principais agressores foram os pais biológicos, seguidos pelo padrasto (111 - 12,40%). A maior incidência de agressões se deu nos fins de semana e aconteceram por meio de trauma direto, com as próprias mãos, mas o uso de cinto e outros objetos caseiros foi relatado em 129 casos (14,41%). Em 89 casos (9,94%) as vítimas não apresentavam nenhuma lesão corporal visível. As lesões mais freqüentes eram equimoses (479 - 59,42%), escoriações (400 - 49,62%) e hematomas (184 - 22,82%). As regiões corporais mais atingidas foram os membros superiores (390 - 25,74%), a cabeça (351 - 23,30%), os membros inferiores (269 17,75%) e a face posterior do tórax (180 - 11 ,88%). Na face, as regiões mais atingidas foram as orbitárias (79 -15,22%), a frontal (74 - 14,25%) e a oral (69 - 13,29%). Em 54 circunstâncias houve várias vítimas da mesma família, totalizando 117 crianças ou adolescentes agredidos, em sua maioria, pelos pais biológicos. Foram observados 8 casos (14,81%) em que as vítimas eram irmãos gêmeos. De acordo com os critérios do Código Penal Brasileiro, a maior parte das crianças e adolescentes deste estudo sofreu um dano físico classificado como Lesão Corporal de Natureza Leve, uma vez que não apresentaram um dano maior para a integridade física ou funcional
Abstract: Family Violence represents one of the cruelest faces of violence, because it affects defenseless people, women and children, mostly in their own homes, where protection and well-being are expected. Despite the laws focusing on the rights for children and teenagers, several forms of violence continue to psychologically affect children in such age group, promoting violent behavior in their relationship. The present work analyzed the epidemiological profile of children and teenagers victims of family violence in the Metropolitan Region of Grande Vitoria, between 2002 and 2003. Individuais were examined at the Legal Medicine Department in that region. A retrospective study of forensic examination records of that Department was conducted. A total of 895 cases of children and teenagers victims of non-Iethal aggressions by their family members were detected. However, previous studies verified that most of the victims were female (567 female - 63.35% and 328 male - 36.64%). Besides, the highest incidence of cases was verified for teenagers, aged 15-18 years. There was no significant difference between teenagers (450 - 50.27%) and children (445 - 49.72%). In most cases (482 53.85%), the main aggressors were the biological parents, followed by stepfather (111 - 12.40%). The highest incidence of aggressions occurred on weekends by means of direct trauma, with bare hands; the use of a belt and other household objects was related in 129 cases (14.41%). In 89 cases (9.94%), the victims presented no corporal lesions. The most frequent lesions were bruise (479 59.42%), scratch (400 - 49.62%) and hematoma (184 -22.82%). The most affected body regions were the upper limbs (390 - 25.74%), head (351 - 23.30%), lower limbs (269 - 17.75%), and posterior thorax (180 - 11.88%); as for the face, the orbits (79 - 15.22%), the forehead (74 -14.25%) and the mouth (69 - 13.29%) were the most affected regions. In 54 cases, there were several victims of the same family, totaling 117 children or teenagers, most of whom were violently punished by their parents. In 8 cases (14.81%), the victims were twin brothers. According to criteria described in the Brazilian Penal Code, most children and teenagers in this study suffered damage classified as Minor Corporal Lesion, since their physical or functional integrity was not affected.

Abstract:
Opioids are scheduled by the propensity for misuse and abuse with a high rate of dependency and risk of fatal overdose. Opioids can be divided into different classes, including, natural, synthetic, and semi-synthetic. Opiates are naturally occurring and come directly from the opium poppy plant; whereas the semi synthetics opioids are chemical modifications of the poppy plant. Synthetic opioids attach to the opioid receptor but contain no part of the poppy plant. The increased variety and frequency in opioid prescriptions contributed to an opioid epidemic in the United States which is still on going. According to the CDC, the opioid epidemic has occurred in three waves. The first wave of the epidemic began in the 1990’s with the increase in opioid prescription pain medication overdoses. The second wave began around 2010 when heroin overdoses became more prevalent. This was followed by a sharp uptick in fentanyl deaths beginning around the year 2013, indicating the start of the third wave. The opioid epidemic has had a huge cost to society, not just due to deaths but also because of lost productivity, medical expenses and judicial system costs (Florence, Zhou, Luo, & Xu, 2016). To best design and implement strategies to combat this issue, an understanding of the population effected is needed. Since many public health policies are implemented at the regional level, knowing the characteristics and demographics of the epidemic at the local level is important. This study evaluates trends in drug related death cases in the Florida District 6 Medical Examiner Office (MEO) from the calendar years 2011 through 2016. Specifically, it focuses on opioids and the role of fentanyl in overdose related mortality. Additional attention is given to fentanyl and fentanyl analog related deaths. Fentanyl analogs present challenges from an analytical toxicology perspective. Fentanyl analogs can be difficult to detect. Two sets of data from each calendar year were obtained from the MEO. This data was collated, standardized and then statistically analyzed. It was determined that there was not a significant difference in month of the year or the day of the week that drug related fatalities occurred. The time of day was statistically significant with more drug related mortalities occurring during the hours of 8:00am and 4:00pm. When assessing mortality rates, Pinellas and Pasco county demonstrated differences. Pasco county has higher overall mortality for opioid related deaths. Pinellas county has almost twice the number of the opioid, fentanyl, related overdose fatalities. Racial demographics, divided into White, Black, and Asian populations, demonstrated that the White population is disproportionally affected by fentanyl drug related mortality. Binary logistic regression showed that fentanyl and heroin tend to co-occur, and that ethanol, hydrocodone, methadone, morphine, and oxycodone do not usually co-occur with fentanyl in drug related fatalities. These data help elucidate trends in the opioid epidemic at a regional level. There are differences between Pinellas and Pasco county; with the former having more fentanyl related drug deaths and the latter having more opioid related drug deaths over the six years analyzed. An interesting result is derived from the binary logistic regression. It is shown here that fentanyl and heroin tend to co-occur together. It is also shown that ethanol, hydrocodone, morphine, oxycodone, and methadone do not co-occur with fentanyl related overdose cases. Notably, methadone has the strongest negative association with fentanyl related overdoses.

Muscle morphological and mechanical properties play a crucial role in explaining the mechanisms underlying the development and progression of muscle weakness, joint stiffness, muscle contraction, and the resultant loss of motor function in children. Information in the literature about how muscle architecture correlate with muscle force production in passive and active conditions in children is very limited. Therefore, new information regarding muscle mechanics and morphology has the possibility to contribute to the improvement of more targeted and more effective treatments for children. The goal of this project is to develop a feasible experimental method to examine passive and active muscle force production capacity in the lower limbs in healthy children and to analyse the correlations of muscle morphological parameters obtained from diffusion tensor images(DTI) and force generation capacity in passive and active conditions. For this project, 10 healthy children were recruited and tested in Astrid Lindgren Children’s Hospital, Karolinska University Hospital. The chosen muscles to examine was medial gastrocnemius, soleus, and tibialis anterior. Neuroflexor device was used for the passive force measurements. A fixed version of a hand-held dynamometer was utilized for the active measurements. In order to capture the muscle activities during the movement, surface electromyography was collected simultaneously. The findings from both measurements gave consistent results. In terms of the passive resistance force measured by NF, the characteristic force peaks can be further analyzed to separate different contributions for more informative results. Regarding the correlations, stable and high correlations were determined between the volume(v) and both force measurements except the medial posterior SOL for MVC. Fascicle length (FL) correlations showed more of a variety since high correlation was observed for PF and FL while negligible correlation was found between P3 and FL. Further research with more parameters is needed to obtain more reliable results. Overall, not only healthy subjects but also children who suffer from muscle weakness and disabilities should be investigated for further examination.

Bakgrund: Intensivvård innebär att förebygga och behandla hotande eller manifest svikt i ett eller flera organsystem hos svårt sjuka patienter. När den nyutexaminerade intensivvårdssjuksköterskan påbörjar sitt arbete på en intensivvårdsavdelning inleds en övergångsperiod. Denna kan pågå upp till två år och är en kritisk period där den nyanställda sjuksköterskan kan känna sig stressad och överväldigad. Mentorskap är en form av handledning där mentorn bland annat agerar som guide och förebild och tillhandahåller kunskap och råd inför den nya rollen. För drygt två år sedan påbörjades ett mentorprogram på en intensivvårdsavdelning i en storstadsregion i Sverige. Syfte: Studiens syfte är att undersöka nyutexaminerade intensivvårdssjuksköterskors erfarenheter av att ha deltagit i ett mentorprogram. Metod: Fokusgruppintervjuer genomfördes och intervjumaterialet analyserades med kvalitativ innehållsanalys. Resultat: Studiens resultat presenteras under fyra kategorier. Mentorprogrammet – dess syfte och struktur belyser generella aspekter om programmet. Övergången – från student till en ny roll som intensivvårdssjuksköterska presenterar resultat angående den första tiden i den utökade yrkesrollen. Mentorskapet – adeptens och mentorns relation belyser olika aspekter av relationen och Mentorsamtalet – att prata av sig beskriver adeptens och mentorns samtal. Slutsats: Mentorprogrammet behövs och fyller en viktig funktion men behöver struktureras och individualiseras. Det är betydelsefullt hur adepter och mentorer matchas och det faktum att de även är kollegor bör beaktas om problem dem emellan uppkommer. Klinisk betydelse: För att underlätta övergången för nyutexaminerade intensivvårdssjuksköterskor kan mentorprogram införas och vissa aspekter bör beaktas innan uppstarten men även efter dess införande.
Background: Intensive care means treat and prevent threatening or manifest failure in one or several organ systems in very sick patients. The transition period, about two years, is critical for the newly hired intensive care nurse that often experience stress and a feeling of being overwhelmed. Mentorship is a sort of guidance where the mentor guide and act as a role-model as well as provide knowledge and advice. About two years ago a mentoring program started in an intensive care unit in a large city area in Sweden. Aim: The purpose of this study is to examine newly graduated intensive care nurses´ experiences after participation in a mentoring program. Method: Focus group interviews were completed and the data were analyzed using qualitative content analysis. Result: The result from the study is presented in four categories. The Mentoring program – aim and structure present general aspects of the program. The transition – from student to a new role as an intensive care nurse present results concerning the first period in the new profession. The Mentorship – the mentee and mentor relationship highlights different aspects of the relationship and The Mentorship Dialogue – to unburden describes the dialogue. Conclusion: The mentoring program is needed and performs an important function but should be structured and individualized. The matching between mentees and mentors are important and the fact that they are colleagues should be borne in mind if problems occur. Clinical matter: A mentoring program can ease the transition but certain circumstances should be taken into consideration.

Abstract:
The Medical Examiner of Virginia (ME) dataset and the Virginia Department of Forensic Science Driving Under the Influence of Drugs (DUI) datasets were used to determine whether people have the potential to develop tolerances to diphenhydramine, cocaine, oxycodone, hydrocodone, methadone, and morphine. These datasets included the years 2000-2004 and were used to compare the concentrations of these six drugs between people who died from a drug-related cause of death (of the drug of interest) and people who were pulled over for driving under the influence. Three drug pattern groups were created to divide each of the six drug-specific datasets in order to compare concentrations between individuals with the drug alone, the drug and ethanol, or a poly pharmacy of drugs (multiple drugs). An ANOVA model was used to determine if there was an interaction effect between the source dataset (ME or DUI) and the drug pattern groups. For diphenhydramine and cocaine, an interaction was statistically significant, but for the other drugs, it was not significant. The other four drug-specific datasets showed that the DUI and ME were statistically significantly different from each other, and all of those datasets except for methadone showed that there was a statistically significant difference between at least two drug pattern groups. Showing that all of these datasets showed differences between the ME and DUI datasets did not provide sufficient evidence to suggest the development of tolerances to each of the six drugs. One exception was with methadone because there were 14 individuals that had what is defined as a "clinical 'lethal' blood concentration". These individuals provide some evidence for the possibility of developing tolerances.The main outcomes of this study include suggesting changes to make to the ME datasets and the DUI datasets with regard to the way data is kept and collected. Several problems with the fields of these datasets arose before beginning the analysis and had to be corrected. Some of the changes suggested are currently being considered at the Virginia Office of the Chief Medical Examiner as they are beginning to restructure their database.

Remote control and management functions are widely utilized in multiple industries.The remote control and management functions has allowed for peopleto connect and interact to solve technical problems more efficiently. However,the healthcare organizations have not utilized the remote controlling and managementfunctions to a degree similar to other industries. Telephoning ande-mailing are still two mainstream ways of work when it comes to solvingtechnical support issues in-house. In order to understand what the technicalpersonnel and the clinical users at a hospital desires in new solutions, thismaster thesis project aimed at finding the existing needs in terms of remotecontrolling and management functions. To find these needs, Q-methodologywas applied for collection of subjective data from healthcare personnel abouta software device that aims at providing remote controlling and managementfunctions. In addition to finding and defining the needs, this thesis also aimedat examining how well such systems can address these needs. Performing this methodology three factors where found representing three differentattitudes regarding the needs for remote functions. The three factorsare "Technical Communication is Significant", "Functionality Appreciativeand Experienced" and "Do if fast!". These factors and their interpretationhelps to be aware of and to evaluate remote support solutions in a systematicway.
Tillämpningen av tekniska distanslösningar används i flertalet industrier i olika syften. Dessa tillämpningar har möjliggjort att människor kan ansluta och interagera med varandra för att på ett effektivt sätt lösa tekniska problem. Trots detta har inte vårdgivarorganisationer tillämpat dessa typer av lösningar i en liknande utsträckning jämfört med många andra industrier. Kommunikationskanaler som telefonsamtal och e-post är fortfarande vanliga när vårdpersonal bemöter tekniska problem som ska lösas internt. Syftet med denna uppsats är att hitta och definiera de befintliga behov som teknisk personal och kliniska användare upplever på sjukhus gällande tekniska distansl ösningar. För att hitta och definiera dessa behov har Q-methodology tillämpats för att systematiskt samla subjektiv data från vårdpersonal gällande ett nytt verktyg som ämnar till att leverera tekniska distanslösningar till sjukvården. Utöver detta har denna uppsats också undersökt hur väl denna typ av nya verktyg tillfredsställer de adresserade behov som beskrivits ovan.   Applicerandet av denna metodik resulterade i tre åsiktsgrupper som representerar tre olika attityder gällande behov för tekniska distanslösningar. Dessa tre åsiktsgrupper är "Technical Communication is Signficant", "Functionality Appreciative and Experienced" och "Do it fast! ". Vetskapen om dessa existerande_åsiktsgrupper bidrar till en större förståelse och en större förmåga att utvärdera tekniska distanslösningar.

The paper examines the issue arising when delivering healthcare in the modern information society. Throughout the past decade, the Internet has seen a significant rise of the "Web 2.0" trend, which carried on its wings a health industry trend often referred to as "Health 2.0" or "Medicine 2.0". More recently, we have also witnessed the crowning of concepts such as Health Social Media, eHealth and mHealth. European Union as well as the national states develop strategies implementing new technologies for personal and medical data sharing, including the prescription of medicals as well as their validation through websites. Healthcare data privacy and security is one of the top challenges, healthcare providers face. The huge amount of data the medical care generates holds potential for researchers, providers, pharmaceutical companies as well as for doctors, who can use it to improve care or find new treatments and insights into disease. The key issue to examine is how to balance the competing interests of privacy and data-sharing and not exclude the patient as a holder and owner of the information. The paper addresses the issue of privacy protection in digitized healthcare, using the analysis of the legislation and case-law of the Czech Republic, stressing the demands for human rights and privacy protection of a member state of the European Union. The paper introduces several proposals for providers on how to re-design digital healthcare with respect to laws and patients´ rights. The paper concludes that even modern and digitized medicine is based not only on evidence and modern technologies but also on human interaction and face-to-face approach and trust between the doctor and patient.

"The purpose of this study was to examine the factors that influence an individual’s choice to share personal information online. Specifically, the role of age, gender, personality, overall media exposure, and perceived risks and benefits were examined in relation to a willingness to share personal information that differed in sensitivity (high school grades, medical records, income) and differed in target audience (social media, online store, general public). A total of 202 individuals participated in this survey study. The majority were young (M age = 22.46 years, SD = 5.77), single (83.7%), women (80.7 %), with at least some post-secondary education (90.1%). A series of hierarchical regression analyses were conducted. The results indicated that willingness to share personal information on social media was predicted by having higher scores on the personality traits of extraversion, agreeableness, and negative emotionality. Higher scores on perceived purchase benefits and total media exposure also predicted willingness to share personal information on social media. In terms of willingness to share personal information with an online store, total media exposure was a significant predictor along with higher extraversion and lower conscientiousness scores. Finally, willingness to share personal information with the general public was predicted by overall media exposure. Participants generally believed that there were risks involved in sharing personal information, but these risks were considered to be slight. As well, they only slightly disagreed when asked if the internet could be trusted, and were neutral on whether there were purchase benefits to providing personal information."

Abstract Congenital heart disease – structural abnormalities of the heart that are present at and persistent beyond birth – affects about one in every 100 live births every year in the United States [1]. One such disease is that of the aberrant coronary artery, which describes abnormalities of coronary artery morphology. In our case, we examine a patient exhibiting complex aberrant right coronary artery – a rare congenital heart defect. Pre- and post-procedural imaging was acquired from this case, and subsequent three-dimensional (3D) anatomical models were created. These models were used postoperatively to identify and describe the patient’s disease presentation for the purposes of research and education, and were incorporated into virtual reality (VR) scenes in order to further educational insights.

The National Institute of Justice (NIJ) and its Forensic Technology Center of Excellence (FTCoE) hosted the National Opioid and Emerging Drug Threats Policy and Practice Forum on July 18–19, 2019, in Washington, DC. The forum explored ways in which government agencies and programs, law enforcement officials, forensic laboratory personnel, medical examiners and coroners, researchers, and other experts can cooperate to respond to problems associated with drug abuse and misuse. Panelists from these stakeholder groups discussed ways to address concerns such as rapidly expanding crime laboratory caseloads; workforce shortages and resiliency programs; analytical challenges associated with fentanyl analogs and drug mixtures; laboratory quality control; surveillance systems to inform response; and policy related to stakeholder, research, and resource constraints. The NIJ Policy and Practice Forum built off the momentum of previous stakeholder meetings convened by NIJ and other agencies to discuss the consequences of this national epidemic, including the impact it has had on public safety, public health, and the criminal justice response. The forum discussed topics at a policy level and addressed best practices used across the forensic community.

The relationship between health and employment status continually shows that individuals who work have lower levels of illness and higher self-reported health. This study examines how self-reported health and objective measures of health (multimorbidity and mental health problems) differ across employment status and occupations among adults of working age (25-65 years). In addition, the study examines how public health coverage – medical card and GP visit card (GPVC) – and private health coverage (PHI), and lack thereof, differ across occupations. Overall, individuals not in employment have much lower rates of self-reported health and higher rates of illness. In particular, mental health problems are three times higher among unemployed individuals across all age groups. Examining workers separately, differences in health status across occupations are small. However, rates of health coverage differ considerably across occupations. In general, occupations associated with poorer health status tend to have the highest percentages of workers without a medical card/GPVC or PHI. This affects workers’ ability to access lower cost or free healthcare, including for the purpose of certified sick leave.

The surge in armed conflicts involving terrorism has brought to the fore the general question of medical care in armed conflict and the particular legal protections afforded to those providing such care to terrorists. Against this backdrop, we evaluate international humanitarian law (IHL) protections for wartime medical assistance concerning terrorists. Through that lens, we expose gaps and weaknesses in IHL. We also examine tensions between IHL and state responses to terrorism more broadly. In studying the IHL regime applicable to medical care, substantive fragmentation and gaps in legal protection between states and across types of conflict emerge. These ruptures are not new. But they are increasingly noticeable as terrorism is more frequently conceptualized as forming part of armed conflicts and as more states undertake aggressive responses to terrorist threats. The U.N. Security Council has been a key driver of these responses, requiring member states to take more and broader steps to obviate terrorist threats. Yet so far the Council has not required that, in doing so, states fully exempt impartial wartime medical care, even in circumstances that would render such care protected under IHL. Rather, the Council seems to consider providing medical assistance and supplies to al-Qaeda and its associates as at least a partial ground for designating those who facilitate such care as terrorists themselves. The overall result today is unsatisfactory. By prosecuting physicians for supporting terrorists through medical care in armed conflicts, some states are likely violating their IHL treaty obligations. But in certain other instances where states intentionally curtail impartial medical care there is no clear IHL violation. Both those actual IHL violations and the lack of clear IHL violations, we think, are cause for concern. The former represent failures to implement the legal regime. And the latter highlight the non-comprehensiveness - or, at least, the indeterminateness and variability - of the normative framework.

The share of couples who cohabit prior to marriage has increased over time. Among marriages that took place between 2015 and 2019, three quarters were preceded by cohabitation, up from only one-third in the mid-to-late 70s (FP-21-04). This shift in behavior has been accompanied by a shift in attitudes toward cohabitation. Using Monitoring the Future data, this profile examines change in high school seniors’ attitudes toward cohabitation as a testing ground for marriage from 1976 to 2020. This measure is based on agreement or disagreement (neutral responses are not shown) with the statement “It is usually a good idea for a couple to live together before getting married in order to find out whether they really get along.” This profile updates previous profiles on high school seniors’ attitudes toward cohabitation using the most recent available data (FP-19-10; FP-16-13), and is a companion profile to High School Seniors’ Expectations to Marry, 2020 (FP-22-04).

Regional Air Mobility (RAM) focuses on building upon existing airport infrastructure to transport people and goods using innovative aircraft that offer a huge improvement in efficiency, affordability, and community-friendly integration over existing regional transportation options. These aircraft, which typically carry less than 20 passengers or an equivalent weight in cargo, are flexible in terms of where they can take off and land, even using existing runways and infrastructure to maximize compatibility with today’s airports. This project examines the feasibility of RAM supporting high-speed transportation for high-priority passenger and cargo movement within Fresno County and connection to coastal urban centers. Some examples of high-priority passengers and cargo could include, but would not be limited to, medical patients needing specialized and/or emergency treatment, organ transport, and critical medical supply deliveries. Electrification of aviation is happening, and Fresno County has the potential to combine our existing closely spaced underutilized airport infrastructure, early demonstration, and experience with electric aircraft, renewable energy opportunities, central location within the state, and the need to open the door for new industry opportunities for youth to take advantage of this “Third Revolution” in aviation.

Snomed CT is a widely used medical ontology which is formally expressed in a fragment of the Description Logic EL++. The underlying logics allow for expressive querying, yet make it costly to maintain and extend the ontology. Existing approaches for ontology generation mostly focus on learning superclass or subclass relations and therefore fail to be used to generate Snomed CT definitions. In this paper, we present an approach for the extraction of Snomed CT definitions from natural language texts, based on the distance relation extraction approach. By benefiting from a relatively large amount of textual data for the medical domain and the rich content of Snomed CT, such an approach comes with the benefit that no manually labelled corpus is required. We also show that the type information for Snomed CT concept is an important feature to be examined for such a system. We test and evaluate the approach using two types of texts. Experimental results show that the proposed approach is promising to assist Snomed CT development.

This paper comparatively examines the ruling religious populist governments in Turkey and Pakistan through a theoretical framework that focuses on populists’ promises, their compromises, and their attacks on democratic institutions. Through our three-legged framework, we examine how these religious populists behave in power and how strategic necessities, the realities of governing, and structural constraints shape their policies. Similar to the other populists in other parts of the world, before coming to power, Islamist populists make sweeping promises to the people and quick fixes to major problems of the country—most famously, quick and substantial economic development. While they may want to retain their uncompromising style and lofty goals, the realities of governing force populists to make serious compromises to their designated ‘enemies’ and on their values once they are in power. Finally, like other authoritarian politicians, Islamist populists attack formal institutions of democracy such as the judiciary, the media, and civil society; they politicize them, evacuate them, and eventually capture them from within.
