Full textHealthcare organizations now consider increased efficiency, reduced costs, improved patient care and quality of services, and safety when they are planning to implement new information and communication technology (ICT) based applications. However, in spite of enormous investment in health information systems (HIS), no convincing evidence of the overall benefits of HISs yet exists. The publishing of studies that capture the effects of the implementation and use of ICT-based applications in healthcare may contribute to the emergence of an evidence-based health informatics which can be used as a platform for decisions made by policy makers, executives, and clinicians. Health informatics needs further studies identifying the factors affecting successful HIS implementation and capturing the effects of HIS implementation. The purpose of the work presented in this thesis is to increase the available knowledge about the impact of the implementation and use of HISs in healthcare organizations. All the studies included in this thesis used qualitative research methods. A case study design and literature review were performed to collect data.
This thesis’s results highlight an increasing need to share knowledge, find methods to evaluate the impact of investments, and formulate indicators for success. It makes suggestions for developing or extending evaluation methods that can be applied to this area with a multi-actor perspective in order to understand the effects, consequences, and prerequisites that have to be achieved for the successful implementation and use of IT in healthcare. The results also propose that HIS, particularly integrated computer-based patient records (ICPR), be introduced to fulfill a high number of organizational, individualbased, and socio-technical goals at different levels. It is therefore necessary to link the goals that HIS systems are to fulfill in relation to short-term, middle-term, and long-term strategic goals. Another suggestion is that implementers and vendors should direct more attention to what has been published in the area to avoid future failures.
This thesis’s findings outline an updated structure for implementation planning. When implementing HISs in hospital and primary-care environments, this thesis suggests that such strategic actions as management involvement and resource allocation, such tactical action as integrating HIS with healthcare workflow, and such operational actions as user involvement, establishing compatibility between software and hardware, and education and training should be taken into consideration.

Full textElderly committing suicide can be in a “preclinical phase” of dementia. Depressive symptoms may indicate a risk to develop a disease of dementia, for example Alzheimer’s Disease. Today almost 10% of the Swedish population older than 65 years suffer from a cognitive impairment diagnosed as dementia. Symptoms of dementia are associated with degenerative changes in the brain caused by a deposition of amyloid, leading among others things to a nerve cell death. A clinical diagnosis can be hard to set, and a definitive diagnose can only be set after a pathological examination, which only is possible after death. For this study we used Congo red staining of brains sections to find amyloid in autopsies from elderly people committing suicide. 35 cases (>60 year) were studied. Of the 35 cases 1/3 showed to be positive for amyloid deposition. This result in addition to other studies suggest that depressive symptoms is a “preclinical phase” of dementia, and therefore the suicide risk for this group must be consider to be elevated. However, more reliable prospective studies most be done to confirm this retrospective study.

Full textElastic fibres ensure blood vessels and other tissues flexibility. Elastic staining of tissue is relevant when there is suspicion of melanoma, temporalis arteritis, venous invasion and after operations on blood vessels. The aim of the study was with the help of different tissue samples optimize and implement Hart´s elastic staining method as a substitute for Verhoeff’s at pathology lab at county hospital Ryhov, Jönköping. Colon, kidney, skin, and umbilical cord cross section got stained with Hart´s automated elastic staining method to evaluate the optimal staining procedure. Same region of the tissues was stained with Verhoeff´s manual elastic staining method and Hart´s method. All cross section were assessed and compared with the help of a pathologist doctor. Optimization of Hart´s method resulted in an incubation period of twelve minutes and optimal staining procedure without xylene. Result of comparison between Hart´s staining method and Verhoeff´s staining method showed that Hart´s staining method had a better contrast and background. Conclusions of the study was that Hart´s staining method was better than Verhoeff´s staining method, further studies could include research about a substitution of the blotting step with an extra ethanol bath as an example and liver tissue instead of the umbilical cord.
Larsson, Patrik. "Inhibiting the IGF-1 receptor with the cyclolignan Picropodophyllin: an in vitro study of ovulation, implantation and receptivity in a mouse model." Thesis, Uppsala University, Department of Medical Biochemistry and Microbiology, 2008. http://urn.kb.se/resolve?urn=urn:nbn:se:uu:diva-89109.
Full textPicropodophyllin (PPP) is an analogue of the anti tumour lignan podophyllotoxin with the unique ability to selectively inhibit the receptor of Insulin like growth factor 1(IGF-1). IGF-1 is believed to play an important part in development of the endometrium facing implantation. With PPP treated mice, studies can be made to measure gene expression from tissue of both treated and untreated mice to compare the role of IGF-1 regarding ovulation, implantation and receptivity. The aim of this study was to analyze gene expression of some steroid hormone receptors and cytokines in ovaries from mice treated with PPP. In this study, seven mice were treated with PPP at different times and tissue was collected. PCR-primers for cDNA sequences of estrogene receptor α, estrogene receptor β, progesterone receptor A, progesterone receptor B, growth hormone receptor, interleukin 1 α, interleukin 1 β, tumour necrosis factor α and androgen receptor were used. Real Time PCR was run with the samples and gene expression was measured. The results of this study showed that the inhibition of IGF-1 receptor interacted with IGF-1 which lead to altered levels of estrogene receptor alpha, progesterone receptor, growth hormone receptor and androgen receptor that can decrease ovulation. The results also showed the differences in gene products between treated and untreated samples, suggesting that IGF-1 plays an important role regarding ovulation.

Thesis, Uppsala University, Department of Medical Biochemistry and Microbiology, 2009. http://urn.kb.se/resolve?urn=urn:nbn:se:uu:diva-114139.
Full textObjectives
The aim of this study was to compare left ventricular ejection fraction (LVEF) results derived from gated single photon emission computed tomography (SPECT) using Cedars-Sinai quantitative gated SPECT (QGS) processing software with results from 2D echocardiography, both obtained in routine clinical diagnostic use.
Methods
Data from previously performed tests were obtained from 73 patients who had undergone both 2D echocardiography and gated SPECT within a time span of 6 months and had not had significant events that could influence LVEF. LVEF from 2D echocardiography was reassessed to obtain discrete values and then the data was compared using Bland-Altman analysis.
Results
The correlation between the tests was shown to be good, but precision lacked. Bland-Altman analysis showed a bias of -0.8 percentage points when gated SPECT compared to mean values and 2 standard deviations (SD) ranged from -20.2 to 18.6.
Conclusions
LVEF values from the two methods can differ quite a bit and comparisons between them should be done with great caution.
Syfte

Full textObjective: The aim of this study was to evaluate a new underfill-function in a POCT-instrument from HemoCue AB (Ängelholm, Sweden). The instrument is in use today among diabetes patients for self-monitoring blood glucose (SMBG). The new function is supposed to guarantee that measuring only will be performed on a sufficient sample volume to assure that the correct glucose value is received.
Methods and results: Blood samples (whole blood) from 12 patients were analysed with the instrument. Measuring were performed using different volumes in the cuvette. Full cuvette, 3µL, 2µL, 1µL and a measuring on an empty cuvette. The instrument performed measurements on all volumes added to the cuvette except for the empty cuvette. The less sample volume that was used the lower glucose values were reported by the instrument.
Conclusions: The new under fill-function did not work satisfactory. If such function would be more reliable it would be beneficial for the patient controlling hers/his bloodglucose provided that the testing procedure is being correctly done. This is very important because the results are often used to treat the patient.
Björklund, Kristofer. "Evaluation and optimization of four real-time PCRs, using TaqMan-probes, for detection of and discrimination between barley, oat, rye and wheat." Thesis, Uppsala University, Department of Medical Biochemistry and Microbiology, 2008. http://urn.kb.se/resolve?urn=urn:nbn:se:uu:diva-9171.
Full textCoeliac disease is a chronic inflammatory disease treated with a gluten-free diet, excluding barley, rye and wheat. Hence, there is a demand for methods able to detect gluten in foods in order to ensure correct labeling of products. According to the Codex Alimentarius Commission, 20ppm gluten is the maximum amount allowed in food labeled gluten-free.
PCR can detect DNA from cereals in food. Four real-time PCR-systems,
using TaqMan®-probes for detection of barley, oat, rye and wheat were optimized and evaluated. Evaluations were carried out using seeds. Primers were targeted to genes coding for prolamines, seed storage proteins. PCR-systems targeted to barley, oat and wheat were shown to be specific for the cereals corresponding to each system. The system targeted to rye showed cross-reactions with durum wheat and spelt wheat. Detection limits were 50pg, corresponding to <10 haploid genome copies for each cereal. All systems were able to detect 250ppm amounts of DNA, most likely even smaller amounts are detectable. All systems showed an amplification efficiency of ≥95%.
Systems for detection of barley, oat and wheat are ready for further evaluation, using food products as samples. The rye system however, needs to be re-designed before further evaluation can take place.

Full textAbstract
Crimean-Congo Hemorrhagic Fever virus (CCHF) and Lassa virus are giving sources illness to humans. In addition to zoonotic transmission, CCHF and Lassa virus can spread from person to person. After a short incubation period, CCHF and Lassa virus infections are characterized by a sudden onset of high fever, chills, headache and cough just like flu. Even some people are vomiting and have diarrhoea. After a few days of illness hemorrhagic manifestations occur. Treatment options for CCHF and Lassa viruses are limited, and there is no vaccine available for use in humans. The purpose of the present study was to produce recombinant nucleocapsid protein of Lassavirus and CCHF virus including an aminoterminal His-tag by a Semliki Forest Virus Replicon (pSFV 4.2). The recombinant proteins are planned to be used in future development of diagnostic methods.

Full textThe obvious relevance of preanalytical errors as a public health problem is clear in both the potential harm to patients and cost to the health system, both unnecessary and avoidable. Some studies indicate that the pre-analytical phase is the most vulnerable to errors, accounting for approximately 60-90% of laboratory errors as a result of lack guidance to patients about the procedures to be performed in the clinical laboratory. Objectives: To systematize the scientific evidence related to preanalytical errors of clinical analysis laboratory. Method: A systematic review was conducted, searching the databases of the Medical Literature Analysis and Retrieval System Online (MEDLINE) , Scopus (which includes MEDLINE and Embase ), ISI Web of Knowledge , SciFinder , Latin American and Caribbean Health Sciences (LILACS) ( which includes the Scientific Electronic Library Online - SciELO) and the Spanish Bibliographic Index of Health Sciences (IBECS) for articles published between January 1990 and June 2012 on laboratory errors that may occur in the preanalytical phase. Studies were included according to the following laboratory tests: complete blood count, biochemical analysis of whole blood or serum, blood coagulation tests, urinalysis and hematological or biochemical analysis of other materials categorized by the type of pre - analytical error and the frequency of incidents. Results: The search in bibliographic databases resulted in the following number of items retrieved: 547 in MEDLINE, Scopus at 229, 110 in the ISI in SciFinder 163, 228 and 64 in the Lilacs IBECS, a total of 1.341 titles. At the end of the systematic review, we obtained a set of 83 articles for reading the full text, of which 14 were included in the review. The studies covered different types of laboratories, technical sectors and source of errors, according to the phase of the laboratory process. Discussion: Seven articles showed errors in the medical requisition, with a high variability in the incidence values. The six articles that studied sample collection errors observed reduction in this outcome. The proportions of reported adverse events and clinical impacts varied, leading to consequences described as: errors resulting from phlebotomy, recollection of samples, repetition of exams, delays in the release of the test results and possible harm to the patient. Conclusions: The laboratory must have written instructions for each test, which describes the type of sample and collection procedure. Identification methods by barcode, robotic and analytical systems, reduce preanalytical errors. The improvement of pre-analytical phase of laboratory tests remains a challenge for many clinical laboratories.

Full textThe Grocott method is the most sensitive staining method for fungal structures in histological tissues in comparison to for example Periodic Acid-Schiff (PAS). Usage of the cancerogenic chromic acid and the time aspect for Grocott are significant problems. The aim of this study was to examine if the automated staining method for Grocott provides equivalent results in comparison to the manual staining method. Twelve tissue sections were stained with both the automated and the manual method. The automated method contains a kit with all solutions. Visual evaluation was conducted with authorized biomedical scientists and a pathologist. Fungi structures in eight of eleven tissue sections and a control were evaluated as more distinct in the automated method in comparison to the manual method. Fungal structures in one of the tissue sections were evaluated more distinct in the manual method. In the remaining two tissue sections, no fungal structures were identified. Results from this study indicates that the automated staining method for Grocott provides equally, alternatively more distinct fungal staining in comparison to the manual method.

The purpose of this study was to fill this gap by examining the relationships between the predictor variables of self-reported history of bacille Calmette-Guérin (BCG) immunization, place of birth, and years of laboratory experience and the outcomes of self-reported lifetime TST positivity, preventive treatment noninitiation, and barriers to treatment adherence for this subgroup. This quantitative, cross-sectional study was guided by the epidemiologic triad model. A researcher-designed self-administered questionnaire including Part A of the Brief Medication Questionnaire was mailed to 4,335 U.S. microbiologist members of the American Society for Clinical Pathology. From the 1,628 eligible respondents, results showed that prevalence of positive TSTs (17.0%) and treatment noninitiation (9.8%) was low. Multivariate analysis identified BCG and foreign birth, as well as age, nonoccupational exposure, history of TB, work in mycobacteriology, and work outside of microbiology as predictors of a positive TST; foreign birth was a predictor of treatment noninitiation. Additional research is needed to identify other laboratorian groups at increased risk for developing TB. These results enhance positive social change by helping to inform recommendations in the global fight to stop the spread of TB, as well as improve allocation of resources among this specific group of HCP.

Full textThe thesis aimed to study the developments, in the area of pharmacoinformatics, of the electronic prescribing and dispensing processes of drugs - in medical praxis, follow-up, and research. For hundreds of years, the written prescription has been the method of choice for physicians to communicate decisions on drug therapy and for pharmacists to dispense medication. Successively the prescription has also become a source of information for the patient about how to use the medication to maximize its benefit. Currently, the medical prescription is at a transitional stage between paper and web, and to adapt a traditional process to the new electronic era offers both opportunities and challenges The studies in the thesis have shown that the exposure of prescribed drugs in the general population has increased considerably over three decades. The risk of receiving potentially interacting drugs was also strongly correlated to the concomitant use of multiple drugs, polypharmacy. The pronounced increase in polypharmacy over time constitutes a growing reason for prescribers and pharmacists to be aware of drug interactions. Still, there were relatively few severe potential drug interactions. Recently established national prescription registers should be evaluated for drug interaction vigilance, both clinically and epidemiologically. The Swedish National Pharmacy Register provides prescription dispensing information for the majority of the population. The medication history in the register may be accessed online to improve drug utilization, by registered individuals, prescribers, and pharmacists in a safe and secure way. Lack of widespread secure digital signatures in healthcare may delay general availability. With a relatively high prevalence of dispensed drugs in the population, the National Pharmacy Register seems justified in evaluating individual medication history. With a majority of prescriptions transferred as ePrescriptions, the detected increased risk for prescription errors warrants quality improvement, if the full potential of ePrescriptions is to be fulfilled. The main conclusion of the studies was that ePrescribing with communication of prescribed drug information, storing and retrieving dispensed drug information, offers new opportunities for clinical and scientific

Full textAim: The aim of this laboratory pilot study was to investigate whether a misfit between an implant and a FDP increase the risk of cracks, chip-off fractures and screw loosening of screw-retained implant-supported FDPs.Material and Method: Twenty screw-retained five-unit implant-supported FDPs were made intitanium with veneering porcelain. The specimens were evenly distributed into a test and control group. In the test group a misfit of 150 μm was created between the implant and FDP at position 5. All specimens underwent artificial aging in a cyclic loading machine for 100 000 cycles. Visible cracks and chip-off fractures were recorded during and after the test and the specimens were controlled for screw loosening. Results: Visible cracks within the porcelain veneer occurred significantly more often in the test group compared to the control group. Nine FDPs in the test group presented visible cracks, compared to one in the control group. Three chip-off fractures were recorded in the test group, none in the control group. This difference was not statistically significant. The retorque values of the screws presented no statistical significant differences neither between the implant positions nor the groups. None of the screws in the groups were loose. Conclusion: Within the limitations of this pilot study the results indicate that the presence of amisfit may increase the risk of cracking and/or chip-off fractures. Further studies are needed to confirm these findings.

Full textIntroduction: Missing teeth can be replaced by dental implants. To achieve good results using dental implants, it is important that there is sufficient bone volume. In cases with bone defects, simultaneous augmentation technique becomes an alternative for bone augmentation. To minimize the contamination with oral bacteria during the bone collection, preoperative rinsing with chlorhexidine is today commonly used as a step in the treatment. The aim of this study is to examine the additional effect of preoperative brushing with chlorhexidine gel on avoiding bacterial contamination during the installation of dental implants.Method: 30 patients participated in a randomized, double blinded study. 15 patients were included in the control group and 15 patients underwent an additional step in the treatment with a preoperative brushing with chlorhexidine gel. Bone samples and paperpoint samples were collected and analyzed in a laboratory and a statistic analysis was performed to compare the number of bacterial colonies in samples from the two groups (CFU/ml) present and the degree of contamination. Results: The mean bacterial count in bone samples from all patients in the control group was calculated at 8 126 CFU/ml and 2 946 CFU/ml in the test group. CFU/ml was different between the groups (P = 0.357) but the difference was not significant (P <0.05).Conclusion: Brushing with chlorhexidine gel preoperatively showed no significant effects in the bacterial contamination of bone chips. No recommendation for adding this additional step in the standard treatment with dental implants can therefore be made today.

Full textIntroduction: The work environment is an important aspect to consider in order to avoid damage. One way to prevent injuries, diseases and poor ergonomics is by using protective equipment that are provided for such things. Another important part of the dental technician profession is that laboratories must take responsibilities for their actions. They need to consider the right use of materials and how to handle them correctly. The laboratory must also focus on the employee’s safety. At last, in order to achieve an optimal result, it is required that laboratories are quality assured.Aim: The aim of this study is to investigate how a dental laboratory looks from a work environment perspectiveMaterial and method: The semi-structed interviews were held in dental laboratories in Malmö. Five dental laboratories participated in the survey in which laboratory manager/safety representative were interviewed. Results: The study showed positive results and awareness among laboratory managers/safety representatives that have knowledge of how to achieve favorable working conditions. Four out of five respondents established that the employees are aware of the risks that may occur in the workplace. Conclusion: Within the scope of the study, the following conclusions can be drawn:• The interviews showed that the respondents are aware of how to achieve an optimal work environment, but they do not always have the resources to do so. • The study showed that measurements of ventilation and suction are performed regularly to ensure that everything is under control.
