Abstract:
The article deals with the analysis of basic studies dedicated to shaping the English language academic communicative competence of the future PhDs in “Healthcare” by distance training methods. The author emphasizes insufficient information coverage of the theme, despite its high urgency. The author defines basic directions of studies: studies of training the future medical PhDs and studies of shaping their academic communicative competence. Another classification of the future medical PhDs training can be classified in the following groups: studies of shaping the scientific research competence in future PhDs, Studies of shapingacademic competence in future PhDs, studies of shaping the pedagogical competence in future PhDs and studies of shaping the professional competence in medical PhDs. The studies of shaping the skills of future research are represented by papers by V.Miniajlo, O.M. Spirin, S. Nikolajeva, who emphasize shaping the research skills of future PhDs as the key training component. The authors stress upon the importance of informational education of future PhDs as the basic components of their training. The article describes the studies of shaping the academic competence within training future PhDs (O. Semenog), dwelling on the modernity of the term. The studies of teaching professional specialized and teaching competences by M. Filonenko and I.Panchenko are dedicated to shaping both the academic and teaching skills during the PhDs training. The significance of academic culture shaping, was studies by several scientists, of which the professional training was covered by V.Lekhan, I.Klishch and L.Lymar. Some Russian scientists studied peculiarities of distance training of the aspirants(postgraduate position in Russia), though they neglect and arrogantly treat this type of education. The article separates basic directions of the studies: studies of teaching the PhDs in “Healthcare” and studies of shaping the academic communicative competence of the last. The author stresses upon the necessity of further qualitative studies of the distance training effectiveness regarding the communicative academic foreign language competence of future medical PhDs.

Objective: In this study, we wanted to check working conditions of public health care doctors(PHDs) working in unbridged island of korea.Methods: We made structured Self-administered questionnaire and sent to all of PHDs working in unbridged island and PHDs working in land as a controlled group.Result: 52 PHDs working in unbridged island and 130 PHDs working in land responded to our survey.PHDs working in unbridged island had difficulty using holiday, sick leave, and offical holiday to participate seminar. especially, difficult to use sick leave was statically significant(p<0.001). Almost all of them were working at night to coping with 24hr medical emergency, thus average overtime working hours was 214.8hr. However average overtime pay was 21.3hr and reward was not fully granted to them. Also, reward for COVID-19 related medical work(extracting COVID-19 specimen, medical examination before vaccination, epidemiological survey,prescription) were not fully payed (48.1%)as a wage guidelines given by the government. They suffered comparative deprivation from the other co-workers at same facility ,because of different working condition and duty. To cope with medical emergency, they required additional human resources with higher-level medical facility hotline(40.4%) and specialized medical emergency education(13.5%).Conclusion: A high percentage of PHDs working in unbridged island were first-year(92.5%),general practitioner or intern(69.2%), But they experienced high responsibility, low support and reward system early at their medical facility. So, policy making about appropriate reward and support system must be considered not only for PHDs working in unbridged island but also for primary health care system in korea.

Abstract:
Background Head pain is a cardinal feature of primary headache disorders (PHDs) and is often accompanied by autonomic and vasomotor symptoms and/or signs. Spontaneous extracranial hemorrhagic phenomena (SEHP), including epistaxis, ecchymosis, and hematohidrosis (a disorder of bleeding through sweat glands), are poorly characterized features of PHDs. Aim To critically appraise the association between SEHP and PHDs by systematically reviewing and pooling all reports of SEHP associated with headaches. Methods Advanced searches using the PubMed/MEDLINE, Web of Science, Cochrane Library, Google Scholar, and ResearchGate databases were carried out for clinical studies by combining the terms “headache AND ecchymosis”, “headache AND epistaxis”, and “headache AND hematohidrosis” spanning all medical literature prior to October 10, 2015. Preferred Reporting Items for Systematic Reviews and Meta-analyses and Meta-analysis of Observational Studies in Epidemiology guidelines were applied. Results A total of 105 cases of SEHP associated with PHDs (83% migraine and 17% trigeminal autonomic cephalgias) were identified (median age 27 years, male to female ratio 1:2.3); 63% had epistaxis, 33% ecchymosis, and 4% hematohidrosis. Eighty-three percent of studies applied the International Classification of Headache Disorders diagnostic criteria. Eighty percent of the reported headaches were episodic and 20% were chronic. Twenty-four percent of studies reported recurrent episodes of SEHP. Conclusions Our results suggest that SEHP may be rare features of PHDs. Future studies would benefit from the systematic characterization of these phenomena.

Abstract:
Objectives: This study aims to understand the current state of regional healthcare services and explore the role of public health doctors of Korean medicine (PHDKMs) in addressing healthcare disparities.Methods: An online survey was conducted among 951 PHDKMs registered with the Korean Public Health Korean Medicine Doctors Association from July 15 to July 19, 2024. A total of 266 responses were collected, and after removing 2 insincere responses, 264 responses were used for analysis. The survey included questions on the provision of healthcare services and the role of PHDKMs in local healthcare crisis.Results: Out of 264 responses, 85 community health centers and branches (32.2%) lacked public health doctors (PHDs). The traveling clinics were the most common alternative in these cases. Centers without PHDs received 46 complaints (54.1%) about healthcare gaps, while centers with PHDs received 70 complaints (39.1%), showing statistically significant differences. The most common complaints involved consultation and management of chronic elderly diseases. PHDKMs reported being capable of providing musculoskeletal and internal medicine treatments, but identified a need for improvement in emergency response, basic and inflammatory treatments, consultation and management of chronic elderly diseases, administration of medications, and vaccinations. They expressed a high willingness and perceived capability to perform minor medical tasks.Conclusion: The decline in PHDs is adversely affecting regional healthcare. PHDKMs can help address these gaps through expanded roles and additional training. Policymakers should consider better integrating PHDKMs into the public healthcare system to ensure continuous and comprehensive healthcare services in underserved areas.

ABSTRACT The activities of defensins HBD-1, HBD-2, and HBD-3 and their C-terminal analogs Phd1, Phd2, and Phd3 against Candida albicans were investigated. Phd1 to Phd3 showed lower-level activities than HBD-1 to HBD-3, although metabolic inhibitors did not render Phd1 to Phd3 inactive. Their activities were also less salt sensitive than those of HBD-1 to HBD-3. Confocal microscope images indicated that the initial site of action was the fungal membrane.

The aim: The paper is aimed to find correlations between English language communicative academic and medical competence components (theoretical, practical, and individual by certain methods), to improve design of the course “Academic English for PhDs in Medicine”, its methods and strategy. Materials and methods: The study sample includes postgraduate groups studying for PhD in Healthcare, aged 21-59 years, in Bukovinian State Medical University (39 respondents), Zaporizhzhia State Medical University (32 respondents), Kharkiv Medical Academy of Postgraduate Education (33 respondents), and Bogomolets National Medical University (318 respondents). The study was held in 2019-2023. We assessed theoretical and practical components using our tests, individual component – psychological methods. The values of three components were transformed into general English communicative academic and medical competence level. The data were treated with SPSS Statistica 18.0, using Spearman correlation significance. Results: We found positive correlation between English communicative competence and communicative tolerance, general communicative skills level, and high or medium communicative control level. Also, positive correlation characterizes interaction as a conflict-resolving strategy and communicative competence. Highly manifested intolerance in communication, predominance of negative mindsets, intolerance to stress negatively affect the level of English communicative academic and professional competence of PhD students. Conclusions: Analysis of the English speaking competence, and its components, showed positive correlation between interaction as a conflict-resolving strategy and English communicative competence of respondents. Regarding the results, the curriculum of the “Academic English for Medical PhDs” should be modified, including interactive methods, case studies, problem solving, and other methods for individual component training.

Abstract:
This pioneering work provides a systemic and comparative analysis of on-the-job training and research in Italy. This is done through an examination of industrial PhDs and apprenticeships for research purposes, which also takes account of the complex relations between businesses, tertiary education, and the labour market. To this end, the paper discusses the relationship between universities and employers, which is usually investigated through a perspective that only considers PhDs’ employability. Although a long-established research strand at both national and supranational level, the author tries to move away from this approach, questioning the effects that these new arrangements might have on productivity and the employers’ innovation capability in Italy. Cooperative research, and industrial and professional doctorates have been extensively covered in international literature. On the contrary, little interest has been shown towards industrial PhDs in Italy, even following the enforcement of Art. 11, par. 2 of Ministerial Decree No. 45 of 8 February 2013. A year since the Decree’s entry into force has elapsed, yet uncertainty still lingers over how to implement these innovative doctoral programmes. Consequently, this analysis sets out to be a first attempt at analyzing industrial PhDs, drawing on a comparison with those countries where they have been in place for a long time. The paper identifies a number of obstacles which act as stumbling blocks to the implementation of on-the -job training in higher education: the absence of placement services in the planning of training schemes and little commitment on the part of industrial relations actors. These shortcomings are the same which hamper the diffusion of apprenticeship schemes –including those for research purposes– which are decisive to fill the void between education and the labour market.

Abstract:
This paper predominantly analyzes the position that women hold in research institutes in Serbia and at the University of Belgrade. The data were collected in May and June, 2006 from the website of the Ministry of science and environment (related to research institutes in Serbia) and directly from 30 faculties of Belgrade University. Among 162 researchers employed in research institutes, women are equally represented with men, not only in the number of employed persons (49% and 51 %, respectively), but also considering the researchers with PhDs (49% vs. 51%), and without PhDs (48% vs. 51%). Also, in both sexes there are the same number of researchers with PhDs (38%: 39%) and without PhDs around 60 % (62%:61%). In spite of a high level of equality between males and females according to the presented data, only 22 females are directors of research institutes, i.e. 0.69% of researchers with PhDs. During the long history of Belgrade University (founded in 1838), only 31.6% out of 34,237 Masters and PhD diplomas were awarded to women (34% of Masters and only 27.7% of PhDs), which indicates the evident difference between males and females. Today, 58% of students are female and women are in a majority at 50% of the faculties of Belgrade University (at some the proportion of female students is even more than 80%). Also, according to data in 2000, more females (60.2%) than males got their diplomas. According to the teaching positions at Belgrade University some 43.3% are held by women, but among professors only 37.4% are female in comparison to 51% among teaching assistants. There are significant differences between the faculties in this respect: the highest proportion of female professors is found in the group of medical sciences (51.3%), the highest proportion of female assistants is found in the group of social sciences (64%). Females are not represented in leading positions at Belgrade University in spite of their qualifications. The University of Belgrade has had only one women elected as a Rector in its long history. During 2000-2004 there were six women Deans while today there are only three (out of 31 faculties). There is more gender equality in research institutes in Serbia than at Belgrade University. Also, women are seriously underrepresented in leading positions at all the analyzed scientific institutions, in spite of their qualifications. Bearing that in mind, it seems it will take another century for women to achieve an equal position to that of their male counterparts.

Individual development plans (IDPs) have been promoted nationally as a tool to help research trainees explore career opportunities and set career goals. Despite the interest in IDPs from a policy perspective, there is little information about how they have been used. The authors examined IDP awareness and use, the benefits of creating an IDP, and ways to facilitate its use by administering a survey to current or former postdoctoral researchers via the National Postdoctoral Association (NPA) and University of Alabama at Birmingham email lists; individuals belonging to Federation of American Societies for Experimental Biology member societies who mentored postdocs; and postdoctoral administrators at member institutions of the Association of American Medical Colleges and the NPA. Although most postdoctoral administrators (>80%) were familiar with IDPs, less than 50% of postdocs and only 20% of mentors were aware of IDPs. For those postdocs and mentors who reported creating an IDP, the process helped postdocs to identify the skills and abilities necessary for career success and facilitated communication between postdocs and their mentors. Despite the fact that creating an IDP benefits postdocs and mentors, IDP use will likely remain low unless institutions and research mentors encourage trainees to engage in this process.

Abstract:
In the era of Consumer Health Informatics, healthcare consumers and patients play an active role because they increasingly explore health related information sources on their own, and they become more responsible for their personal healthcare, trying to find information on the web, consulting decision-support healthcare systems, trying to interpret clinical notes or test results provided by their physician, or filling in parts of their own Personal Health Record (PHR). In spite of the advances in Healthcare Informatics for answering consumer needs, it is still difficult for laypersons who do not have a good level of healthcare literacy to find, understand, and act on health information, due to the communication gap which still persists between consumer and professional language (in terms of lexicon, semantics, and explanation). Significant effort has been devoted to promote access to and the integration of medical information, and many standard terminologies have been developed for this aim, some of which have been formalized into ontologies. Many of these terminological resources are used in healthcare information systems, but one of the most important problems is that these types of terminologies have been developed according to the physicians’perspective, and thus cannot provide sufficient support when integrated into consumer-oriented applications, such as Electronic Health Records, Personal Health Records, etc. This highlights the need for intermediate consumer-understandable terminologies or ontologies being integrated with more technical ones in order to support communication between patient-applications and those designed for experts. The aim of this thesis is to develop a lexical-ontological resource for consumer-oriented healthcare applications which is based on the construction of a Consumer-oriented Medical Vocabulary for Italian, able to reflect the different ways consumers and patients express and think about health topics, helping to bridge the vocabulary gap, integrated with standard medical terminologies/ontologies used by professionals in the general practice for representing the process of care, by means of Semantic Web technologies, in order to have a coherent semantic medical resource useful both for professionals and for consumers. The feasibility of this consumer-oriented resource and of the Integration Framework has been tested by its application to an Italian Personal Health Record in order to help consumers and patients in the process of querying healthcare information, and easily describe their problems, complaints and clinical history.

Abstract:
Il carcinoma ovarico (OC) è la neoplasia ginecologica con il maggiore tasso di mortalità, in parte dovuto all’acquisizione della resistenza al chemioterapico, che si manifesta nel 75% dei casi. In diversi tipi di tumori la sopravvivenza delle cellule tumorali trattate con chemioterapici è stata alla riprogrammazione del metabolismo. Date queste premesse l’obiettivo di questo studio è verificare se trattamenti a base di platino possano indurre un’alterazione del profilo metabolico e come questa riprogrammazione metabolica influisca sul fenotipo cellulare. L’analisi del profilo OCR in due linee cellulari di OC (OV90 e OC314) resistenti al farmaco ha mostrato una riprogrammazione metabolica verso una riduzione del profilo OXPHOS e un aumento del metabolismo glicolitico rispetto alle cellule sensibili . Inoltre, è stato verificato che l’acquisizione della resistenza al farmaco è associata ad un’aumentata espressione dei trasportatori ABCC2 e ABCG2 e ad un rallentamento della proliferazione cellulare. La resistenza è, inoltre, associata all’'acquisizione del fenotipo staminale. È stato dimostrato un arricchimento della popolazione CSC nelle linee resistenti rispetto alle loro controparti sensibili. Infatti, la sovraespressione dei fattori di Yamanaka e di ABCG2 supportano l'acquisizione di un fenotipo stem-like nelle linee resistenti. Pertanto il trattamento cronico con cisplatino ha favorito l’arricchimento delle CSC. Quindi, al fine di investigare il legame meccanicistico fra la riprogrammazione metabolica e l’acquisizione della chemioresistenza e di caratteristiche staminali, è stata effettuata un'analisi bioinformatica: gli elevati livelli di espressione dei marcatori CSC correlano con la sovraespressione dei geni coinvolti nel mantenimento dell’omeostasi del metabolismo lipidico. Questa analisi ha mostrato il coinvolgimento della via PPAR, come via di regolazione, e la β-ossidazione come via metabolica. Le cellule resistenti, con caratteristiche stem-like, costituiscono una sotto-popolazione con un profilo metabolico glicolitico e lipidico. Tali cellule presentano un vantaggio selettivo in un microambiente ricco di lipidi, quale l’omento, e potrebbero pertanto concorrere all’insorgenza della recidiva.<br>Ovarian cancer (OC) is the gynecological malignancy with the highest mortality rate, partly due to the acquisition of resistance to chemotherapy, which occurs in 75% of cases. In several types of tumors the survival of cancer cells treated with chemotherapy has been due to metabolic reprogramming. Given these premises, the objective of this study is to verify whether platinum-based treatments could induce an alteration of metabolic profile and how this metabolic reprogramming affects the cellular phenotype. Analysis of the OCR profile in two drug-resistant OC cell lines (OV90 and OC314) showed metabolic reprogramming towards a reduction in the OXPHOS profile and an increase in glycolytic metabolism compared to sensitive cells. Furthermore, it has been verified that the acquisition of drug resistance is associated with an increased expression of the ABCC2 and ABCG2 transporters and a slowdown in cell proliferation. Resistance is also associated with the acquisition of the stemness phenotype. An enrichment in CSC population in resistant cells was demonstrated compared to their sensitive counterparts. Indeed, the overexpression of Yamanaka factors and ABCG2 support the acquisition of a stem-like phenotype in resistant cell lines. Therefore, chronic treatment with cisplatin has favored the enrichment of CSCs. Then, in order to investigate the mechanistic link between metabolic reprogramming, the acquisition of chemoresistance and stemness characteristics, a bioinformatic analysis was performed: the high levels of expression of the CSC markers correlate with the overexpression of the genes involved in the maintenance of homeostasis of lipid metabolism. This analysis showed the involvement of the PPAR pathway, as a regulatory pathway, and β-oxidation as a metabolic pathway. Resistant cells, with stem-like characteristics, constitute a sub-population with a glycolytic and lipid metabolic profile. These cells have a selective advantage in a lipid-rich microenvironment, such as the omentum, and could therefore contribute to the onset of relapse.

Abstract:
Age determination of dog puppies represents a significant issue of animal welfare and forensic medicine. Despite the legislation in force, the illegal trade of puppies that are too young is expanding. To date, none of the available age estimation methods can be credited with a degree of accuracy sufficient for medical-legal purposes due to the many variability factors affecting the measured biological phenomena, such as teeth and skeletal development, and the lack of standardisation. This work aimed at quantifying the degree of correlation between the chronological age of puppies and the biological age that can be estimated by visual teeth examination and the radiographic examination of limb ossification centres (OCs), and combining the information in a predictive model to obtain greater accuracy in estimating the age. The study included 93 puppies of 10 different breeds, which were examined on a bi-weekly basis from 4 to 20 weeks of age and radiographed from 6 to 16 weeks, when not sold earlier. Teeth eruption and development was affected by a wide degree of variability, in line with the information found in the Literature. On the basis of the number of OCs present, the breeds included in the sample could be divided in “early” and “late” breeds, and no correlation between this variability and size or morphological type was observed. However, the radiographic examination of limb ossification centres allowed to determine, with a fair degree of accuracy, whether a puppy was younger or older than 12 weeks old. Contrarily, at eight weeks, it was not possible to determine the age with a good degree of precision, as various degrees of variability were observed. A model to predict if a puppy was 6 or 8 weeks old was constructed using random forests, which proved to be a powerful tool to be further developed.

Abstract:
The Enterococcus genus consists of natural members of the gastrointestinal tract but they are also opportunistic pathogens. They are a common cause of urinary tract infections but can also cause sepsis and other infections. Enterococcus faecalis and Enterococcus faecium are the most abundant in clinical specimens. Enterococci are a leading cause of nosocomial infections and they have developed resistance against a number of antibiotics e.g. vancomycin. E. faecalis V583 was the first vancomycin resistant isolate reported in the U.S. Movable genetic elements such as pathogenicity islands, PAI, are important for bacterial evolution. PAI:s are large chromosomal fragments mostly seen in pathogenic strains and carry regions such as transposons and insertion elements along with virulence factors and transfer genes. A PAI has been detected in the chromosome of E. faecalis. Excision of PAI:s has been studied for uropathogenic E. coli and frequencies of 10-5 and 10-6 have been reported. In this study the excision rate and excision patterns of E. faecalis V583 was studied using the counterselectable marker pheS*, causing p-Cl-phe sensitivity, and a chloramphenicol resistance gene, cat, inserted at two different positions of the PAI and selecting for excisions by growth on p-Cl-phe. Excision rates of 10-6 and 10-8 were seen based on the p-Cl-phe resistance and chloramphenicol sensitivity. Mutation rate in the pheS* gene was high compared to excision rate which made the method difficult to work with. No obvious excision patterns were detected but the excisions seemed to be limited to the close surroundings of the pheS*/cat insertion.<br>Bakterier finns överallt i vår omgivning och hos oss människor, exempelvis på huden och i vår mag-tarmkanal. Flertalet av dessa är apatogena, d.v.s. orsakar inte sjukdom. Vissa normalt goda bakterier kan dock orsaka sjukdom när de hamnar på fel plats och får tillfälle att orsaka sjukdom s.k. opportunistiska patogener. Ett exempel på detta är bakterien Genetiska egenskaper hos såväl människor som bakterier styrs av arvsmassan, DNA. Hos människor är arvsmassan samlad i 46 kromosomer medan bakterier har en. På senare år har vi lärt oss hur man kan klippa och klistra i exempelvis bakteriers DNA för att introducera egenskaper eller ta bort. Detta används inom forskning för att studera t.ex. bakteriers förmåga att orsaka sjukdom eller anpassning till sin omgivning. Bakterier är mycket duktiga på just anpassning vilket beror på deras förmåga att snabbt förändra sitt DNA ofta genom utbyte med andra bakterier, detta kan bl.a. leda till utveckling av antibiotikaresistens eller nyvunnen förmåga att orsaka sjukdom. Största delen av en bakteries arvsmassa består av konserverade regioner medan andra är mycket föränderliga exempelvis s.k. isertions element, tansposoner och patogenicitetsöar, som har visat sig kunna lämna kromosomen via excision. En patogenicitetsö har hittas hos I den här studien klistras en gen in i patogenicitetsön hos Bakteriekloner där excision förekommit erhölls och excisionsfrekvensen bestämdes till 10 bakterier. Inga kloner där hela patogenicitetsön lämnat kromosomen kunde detekteras, dock visade det sig att områden precis intill området där genen klistrats in hade försvunnit. Inga tydliga excisionsmönster kunde bestämmas. En hög frekvens av mutationer i den insatta genen gjorde metoden svår att arbeta med. Enterococcus faecalis som finns i mag-tarmkanalen hos friska människor men som när den hamnar på fel plats kan orsaka bl.a. urinvägsinfektion, sårinfektioner och i svåra fall blodförgiftning, s.k. sepsis. E. faecalis. Patogenicitetsöar är delar av kromosomen som ofta innehåller virulensfaktorer, som gör bakterien patogen och hos vissa bakterier har man påvisat gener för antibiotikaresistens på sådana öar. E. faecalis som gör att bakterien inte kan växa på ett speciellt selektivt media. Detta gör det möjligt att välja bakterier där ön lämnat kromosomen för vidare studier och en excisionsfrekvens bestämmas. -6 till 10-8. Detta är låga frekvenser jämfört med vad man kommit fram till hos andra 3

Abstract:
This research explores the international evolution of the policy of integration (formalisation) of traditional medicine in health care systems. This concept first arose on the policy agenda of the World Health Organisation in the 1970s and then re-emerged in 2002 (with alternative and complementary medicines). The history of this policy at the global level and its transfer to national levels over this period is analysed, via the content, scope and outcomes of policy and programme documents. This analysis emphasises the roles of context and stakeholders (specifically interest groups). The context in terms of the economic, political and social environment surrounding the development of the policy is considered, and held to offer a potential explanation as to how and why the policy agenda on integration was set and the manner in which programmes were formulated and implemented. Interest group interaction (competition for resources) is concluded to play a key role in explaining the development of this policy on an international level, and its problematic transfer to national levels. A case study of Sri Lanka explores national level implementation in greater detail. An analytical framework to analyse the development and implementation of this policy has been created, from a synthesis of anthropological and political science tools. The combination of several theories into an analytical framework allows this policy issue to be understood as an intrinsically political exercise that has been stimulated by global social and economic forces. The analytical framework developed offers another tool for the analysis and consequent understanding of the health policy process and thus may have relevance beyond the health policy issue of integration.

Abstract:
A prospective intervention study, using clinical practice improvement (CPI) methodology, was undertaken to reduce unnecessary x-ray examinations in the early management of patients presenting to the Emergency Department (ED). This was achieved through raising the awareness of medical and allied health staff to medical radiation by means of clinical education and implementing evidence based diagnostic imaging requisition. The main study was conducted in the ED of a public hospital located in the western Sydney, Australia. A second hospital within the area health service, with similar bed size, activity levels and demographics, was used as the control site. The first phase intervention raised the awareness of the health professionals to medical radiation. The second phase intervention used CPI methodology to attain efficient clinical practices so as to eliminate unnecessary examinations and requests. A multi-disciplinary CPI Project Team involved in the process of imaging examination requisition was empowered to improve the appropriateness of the requested examination utilisation. This it achieved mainly through the implementation of evidence based clinical decision rules and imaging guidelines. An additional method of validating the outcomes was provided through the simultaneous rollout of the interventions at another hospital within the same area health service. At the completion of the study, unnecessary examinations such as Skull, Ribs, Nasal Bone and Kidney Ureter Bladder (KUB) requests at the intervention Hospital site were significantly reduced by 92.6% (p (less than) 0.0001), whereas at the control site Hospital there was minimal reduction which was found to be not statistically significant (p=0.2110). Other frequently requested examinations such as Ankle, Knee and Spine requests were marginally though significantly reduced at the intervention Hospital by 22.7% (p (less than) 0.001), whereas at the control site Hospital the reduction was similarly found to be not significant (p=0.1055). Most importantly, the overall x-ray requisition for every 100 ED presentations at Hospital ‘B’ was reduced by 27%. The results of this study, demonstrated that Radiation Awareness educational programs, targeting medical and allied health staff, will reduce the unnecessary requisition of examinations found not to contribute to the process of the patient’s clinical management. The use of a CPI project approach was found to be important in the process of establishing, implementing and sustaining the achieved improvements, and in particular, the rules and guidelines of evidence based imaging requisition. Importantly, the study also confirmed that the CPI methodology that had been used for the main intervention was adaptable to other organisations when it was found to have been successfully rolled out at another hospital. This confirmed that the main outcomes of the investigation could be generalised to other health facilities. There was an immediate reduction in the requisition of unnecessary examination similar to the results at the main intervention hospital. The implementation and adoption of the CPI intervention across the health care system in general could significantly reduce unnecessary x-ray examinations, saving significant health care resources, and sparing patients from potential cancer risks associated with avoidable exposure to ionising medical radiation.

Abstract:
This research programme investigated the expression of prolyl hydroxylase (PHD) proteins in rodent tissues. The importance of PHD enzymes lies in their ability to render oxygen sensitivity to Hypoxia inducible factor (HIF), the principal mediator of intracellular oxygen homeostasis. The first part of this study focused on developing and validating anti-sera capable of detecting PHD proteins in rodent tissues. With these reagents, it was possible to assess the relative expression of each PHD protein in a number of different rat tissues. PHD2 was the most abundant isoform in all tissues studied. In contrast, an abundance of PHD1 was observed only in testis and skeletal muscle. A number of different tissue species of PHD3 were identified and their abundance was found to vary between different tissues. These observations provide further evidence of the principal role of PHD2 in regulating HIF in vivo, but also point towards additional roles for PHD1 and PHD3 in selected tissues. They highlight the potential for there being a complex interplay between different PHD enzymes which could, in the future, prove potential targets for therapeutic manipulation. This study also provides additional insights into the mechanisms underlying the phenotypes observed in PHD deletional mouse models which appear, in many cases, to be directly related to the abundance of a given PHD isoform. The emerging role of PHD3 as a promoter of sympathetic lineage apoptosis prompted further study of PHD3 expression in rat neuronal tissues. An abundance of PHD3 was demonstrated throughout the rat sympathetic nervous system, a finding which appeared at odds with its known role as a promoter of neuronal apoptosis and resulted in a series of collaborative studies which demonstrated a sympatho-adrenal phenotype in wild type compared to PHD3-/- mice. Further collaborative studies utilising wild type mice and those deleted of specific PHD isoforms, were carried out to assess the significance of the abundance of PHD3 and PHD1 noted here in rat hippocampus and testis respectively. While neither study demonstrated statistically significant phenotypes, these observations remain of interest and areas for future research.

Les symbioses rhizobienne et mycorhizienne à arbuscules sont deux endosymbioses racinaires jouant des rôles importants dans le développement des plantes en améliorant leur nutrition minérale. Les lipo-chitooligosaccharides (LCOs), produits par les bacteries Rhizobia et les champignons mycorhiziens, sont essentiels pour l'établissement de la symbiose rhizobienne et stimulent la mycorhization. Chez la légumineuse Medicago truncatula, trois récepteurs-like kinase à motifs lysin (LysM), LYR3, NFP et LYK3 sont impliqués dans la perception des LCOs. Le travail présenté a eu pour objectif la caractérisation biochimique de ces récepteurs et leurs applications potentielles. Les orthologues de LYR3 de M. truncatula ont été clonés et se sont tous révélés, à l'exception de celui du lupin, capables d'établir une interaction d'affinité élevée avec les LCOs mais pas avec les chitooligosaccharides de structure apparentée. Afin de mieux comprendre les bases moléculaires de la reconnaissance des LCOs, des échanges de domaine entre les protéines LYR3 de lupin et de Medicago ont été effectués et ont révélé l'importance du troisième domaine LysM dans l'interaction. L'exploitation des capacités de reconnaissance des LCOs par LYR3 à des fins biotechnologiques a été évaluée à l'aide de récepteurs chimériques constitués du domaine extracellulaire de LYR3 et du domaine kinase des récepteurs immunitaires AtCERK1 et EFR. Il est apparu que LYR3 peut être utilisé pour élaborer des récepteurs chimériques mais leur mode d'activation reste à optimiser. Enfin l'étude des deux récepteurs symbiotiques NFP et LYK3 suggère qu'ils sont régulés par phosphorylation suite au traitement par les signaux symbiotiques. L'ensemble de ce travail apporte un éclairage nouveau sur les mécanismes de perception des LCOs et sur les modifications associées à leurs récepteurs qui en résultent<br>Arbuscular mycorrhization and rhizobial nodulation are two major root endosymbioses which play important roles in plant development by improving their mineral nutrition. Produced by Rhizobia bacteria and mycorrhizal fungi, lipo-chitooligosaccharides (LCOs) were shown to be essential for the formation of the rhizobial symbiosis and to have stimulatory effects on mycorrhization. In the legume Medicago truncatula three lysin motif (LysM) receptor-like kinases LYR3, NFP and LYK3 have been shown to be involved in LCO perception. Here work is presented aimed at the biochemical characterization and application of these important receptor proteins. Cloned from several legume species orthologs of M. truncatula LYR3, except from lupin, were shown to bind LCOs with high affinity, but not structurally-related chitooligosaccharides (COs). Domain swaps between the lupin and Medicago proteins were used as a tool to decipher the molecular basis of LCO recognition and revealed the importance of the third LysM domain for LCO binding. The possibility of exploiting the LCO-binding capacity of LYR3 in biotechnology, through the composition of chimeric receptors, was investigated by combining together the extracellular domain of LYR3 protein with the kinases of Arabidopsis thaliana immune receptors, AtCERK1 and EFR. The results suggest that LYR3 could be used for constructing biologically active chimeric proteins whose mode of activation needs to be improved. Finally studies on the two LysM symbiotic receptors NFP and LYK3 suggest that they are regulated by changes in their phosphorylation after symbiotic treatments. Together this work brings light on the mechanisms underlying LCO perception and the modifications that receptors undergo after their treatment with LCO

Abstract:
Safe, high-quality health care requires an open, transparent, and just culture where people are willing and have the opportunity to discuss errors and system problems and to do something about them. There is paucity of research in relation to safety issues in health care. Objective: To identify the components of a safe culture and measure staff’s attitudes to those components in an area health service. Method: A mixed mode method comprising qualitative and quantitative measures was used. A 60-item survey comprising a likert response scale and measuring safety attitudes and values was administrated to 3,200 staff in an Area Health Service (AHS) in Western Sydney. Focus groups were conducted with various professional groups to provide greater depth to salient issues identified in the survey. Results: The response rate was 26%. Teamwork, communication, leadership, and performance shaping factors were identified as components of a safe working health service and that staff were committed to safety. The results show that there is a positive safety and teamwork culture. However, the results about organisational culture were not positive. Results also showed that there is a high level of stress factors that influence safety in the organisation. There were a number of attitudes and patterns that warranted improvement and further investigation. However several areas of discontent among staff in terms of leadership and communication were acknowledged. In regard to attitudes suggesting invulnerability to the effects of stress and fatigue, the majority of respondents acknowledged this belief. These findings are more favourable than those earlier studies using the Operating Room Management Attitudes Questionnaire (ORMAQ), although these were conducted in other countries and the respondents were surgeons, anaesthetists, and Intensive Care Unit staff. A number of factors have emerged both from the questionnaire survey and the follow up focus groups that should be considered. The awareness of the need to encourage teamwork, communication, leadership prevailed. Conclusion: Health care organisations can benefit from the valuable information and an understanding of staff’s attitudes to patient safety. It confirms the findings of other studies that attitudes to leadership, teamwork and performance shaping factors such as fatigue and stress have an impact on an organisation’s capacity to develop a culture of safety. Health care organisations need to acquire this kind of information, and build on it if they are considering developing and implementing patient safety programs.

The University of Houston-Downtown supports a STEM program, Scholars Academy (SA) within the College of Sciences and Technology dedicated to enhancing, preparing, and enlightening minority, underrepresented, and first-generation majors seeking entrance into workforce, graduate, and professional programs of preparation. Over the past 18 years the University of Houston-Downtown Scholars Academy has implemented a series of success components supporting the nurturance of post-baccalaureate graduate and professional pursuit yielding a 51% acceptance rate into medical school, over 68 professional degrees (ranging from MD to DO to DDS and DPharm) earned by alumni, over 20 PhD degrees, and over 900 minority/underrepresented undergraduates moving into professional/graduate fields. Since 2006, the GMedTech biomedical research centre in the Galway Mayo Institute of Technology (GMIT) in the west of Ireland has developed capabilities for generating physiological type flow conditions through realistic vascular models based on medical images. The GMedTech centre has designed and developed a number of fully controlled and instrumented synchronised experimental test systems for testing in the abdominal aorta, coronary, venous and intracranial vessels with and without associated disease types. Abstract:
This presentation draws from autoethnographic observations of my experience as a junior PhD researcher undertaking a critical interrogation of the weight-centred health paradigm from within a medical department. I focus on one aspect of my research experience – being asked, and ultimately perplexed by, my attempt to engage in Patient and Public Involvement (PPI). PPI is the term for “working with lay people – especially those with lived experience - as volunteers in influencing and shaping research.” This practice was suggested on the principle that research related to *besity should be co- produced with individuals labelled as *bese. I was signposted to *besity advocacy groups (who were often pharma funded) to find PPI consultants, which led me exclusively to consult individuals with a pre-disposed buy-in in the weight-centred health paradigm. In this presentation I will discuss the limitations of PPI as an effective means to engage the “public” in research, and how my experience sheds light on systemic failures in medical research communities to engage with the politics of fat activism when thinking about weight stigma.

Background Gender-diverse patients (GDP) face barriers in healthcare, including discrimination and limited access to preventive care. Nursing curricula often lack GDP health content, leaving students underprepared to provide equitable care. This QI project aimed to improve RN-BSN students' self-efficacy and comfort in caring for GDP by integrating an educational module into their coursework. Methods A literature review revealed the need for increased education on GDP healthcare needs. An evidence-based educational module including a recorded presentation and an interactive case study was developed and implemented in an online course. Thirty-three RN-BSN students completed the module and pre- and post-intervention surveys, measuring interpersonal comfort (TABS) and self-efficacy (SEST). Changes in attitudes and beliefs were analyzed. Results The results demonstrated statistically significant improvements in students' recognition of non-binary identities and cultural self-awareness. The median SEST score increased significantly from 9.07 to 9.38 (p = .003), and notable gains were observed in students' ability to appreciate and recognize cultural nuances in healthcare. Conclusions The QI project highlights the importance of integrating LGBTQ health education into nursing curricula to address knowledge gaps and improve attitudes toward GDP care. Despite not meeting all objectives, the intervention yielded significant insights and positive shifts in attitudes, suggesting the need for broader implementation and further refinement of educational tools. The 14th Biennial Meeting of the International Human Rights Network of Academies and Scholarly Societies (IHRN) began with an Early Career Scholars Panel, during which up-and-coming scholars deliberated on present-day global human rights issues. The moderator of the panel was Prof Catherine Burns (Associate Professor of Medical History, University of Witwatersrand) and the panel members were Lt Col Dr Esewu Mxolisi Mathebula (South African Association of PhDs), Mr Michael Martin (New Voices in Sciences, Engineering and Medicine, U.S. National Academies), Prof Mzukisi Njotini (Dean of the Faculty of Law, University of Fort Hare, South African Young Academy of Science) and Prof Martha Bradley (Associate Professor in the Department of Public Law, University of Johannesburg, Future Professors Programme). In this session, the Early Career Scholars gave their perspectives on topics related to the theme of the IHRN meeting, ‘The Role of National Academies and Universities in Promoting Human Rights and Enhancing Equality.’ The session had posed a greater number of questions than it had provided answers for. However, some questions stood out, namely how scholars communicate their scientific knowledge in ways that are respectful and dignified, but still critical and engaged across class, gender, hierarchy and region. ASSAf was acknowledged for bringing scholars and colleagues from learned societies together at this event and addressing current and controversial issues. Science can correct itself only through processes such as this.
