Urinary incontinence is not an inevitable consequence of ageing and its impact on social, psychological, and physical well-being is comparable to that of other chronic conditions such as diabetes and dementia. Meanwhile, constipation, including symptoms of evacuation difficulty and/or fewer bowel movements, is a common problem as people age. Risk factors include problems in cognition, mobility, gastrointestinal motility, dysautonomia, anorectal dysfunction, and disabling neurologic disorders. Faecal incontinence is more common in frail individuals but is often assessed inadequately. The cause is often multifactorial. Treatment depends on the cause: a combination of approaches may be necessary, including avoidance of faecal impaction, instigation of a structured bowel care plan including regular prompted toileting, dietary modification, and (in some cases) use of loperamide or similar medications.

Abstract:
The small bowel chapter explores the diseases and conditions of the small bowel. These conditions are listed and the possible presenting symptoms defined. The occurrence rates of the conditions within the small bowel are explored and the potential causes detailed. The investigations that are frequently used to diagnose these conditions are identified. Treatment options or preventative measures are explained with description of any potential complications. Diverse conditions and diseases can affect the small bowel; these include coeliac disease, lactose intolerance, and Meckel’s diverticulum. There is a more in-depth focus on intestinal failure. Intestinal failure is rare but can result in permanent, life-altering changes to the small bowel, potentially the result of treatment for Crohn’s disease or a small bowel infarct. Succinct descriptions of these conditions and diseases can assist the nurse in clinical care.

This chapter reviews palliative considerations for bowel management in a patient with advanced disease such as cancer, liver failure, AIDS, or general debility. In particular, it discusses the definition, prevalence, pathophysiology, assessment, and management of gastrointestinal symptoms common in the palliative population, including constipation and diarrhea. It also reviews malignant bowel obstruction and ascites and how these may impact quality of life for patients with life-limiting illness. The discussion of management includes both pharmacologic approaches and supportive care options. This information is designed to help guide nursing assessment and treatment of bowel management, as well as provide useful insight to any clinical provider encountering these conditions.
