Abstract:

Meningoencephalocele adalah defek kongenital yang sangat jarang, tapi insidennya tinggi di Asia Tenggara, termasuk di Indonesia. Penderita dengan nasofrontal meningoencephalocele memerlukan koreksi pembedahan sedini mungkin karena adanya kelainan bentuk wajah, gangguan pandangan, bertambahnya ukuran meningoencephalocele disebabkan bertambahnya prolaps cerebri dan risiko infeksi. Dalam laporan ini kami presentasikan kasus seorang bayi 9 bulan dengan meningoencephalocele naso-orbital dan hydrocephalus non communican yang menjalani operasi VP shunt dan eksisi cele. Adanya massa tersebut, baik meningoencephalocele nasofrontal atau frontoethmoidal maupun occipital, menimbulkan kesulitan bagi ahli anestesi mulai saat induksi, pemeliharaan anestesi dan pasca operasinya. Menjadi tantangan bagi ahli anestesi dalam pengelolaan meningoencephalocele, dimana sebagian besar penderitanya adalah anak-anak yang mempunyai kesulitan tersendiri, termasuk mengamankan jalan nafas dengan intubasi dan adanya massa yang akan mempersulit ventilasi saat induksi, adanya massa pada nasofrontal serta nasoethmoidal yang berhubungan dengan komplikasinya dan penilaian yang tepat terhadap perdarahan dan hipotermia.Anesthesia Management for A child with Nasofrontal Meningoencephalocele and Hydrocephalus Non CommunicantMeningoencephaloceles are very rare congenital malformations in the world that have a high incidence in the population of Southeast Asia, include in Indonesia. Children with nasofrontal meningoencephaloceles should have surgical correction as early as possible because of the facial dysmorphia, impairment of binocular vision, increasing size of the meningoencephalocele caused by increasing brain prolapse, and risk of infection of the central nervous system. In this report, we presented a case of a 9 months-old baby girl with nasofrontal meningoencephalocele and hydrocephalus non communican, posted for VP shunt (ventriculo-peritoneal shunt) and cele excision. Because of the mass, nasofrontal or frontoethmoidal and occipital meningoencephalocele leads the anaesthetist to problems since the preoperative visit, time of induction, maintenance of anaesthesia during the operation until post operative care. Anaesthetic challenges in management of meningoencephalocele, which most of the patients are children, include ventilation, intubation and securing the airway with intubation with the mass in nasofrontal and nasoethmoidal with its associated complications and accurate assessment of bloodloss and prevention of hypothermia.

Abstract:

Introduction Orbital meningoencephalocele formation is primarily a result of congenital defects in the pediatric population and trauma of the anterior cranial fossa in adults. We present a unique case of nontraumatic nasal and orbital meningoencephaloceles presenting as bilateral proptosis with exotropia secondary to chronic hydrocephalus. Clinical presentation A 20-year-old male with a history of tuberous sclerosis, X-linked intellectual disability, and epilepsy presented to the emergency department with two days of nausea, emesis, seizures, and two months of progressive proptosis. Initial radiographs of the skull showed a “copper beaten” appearance, indicating chronically elevated intracranial pressure. Computed tomography imaging of the head demonstrated bilateral defects in the cribriform plate and anterior cranial fossa. Magnetic resonance imaging of the brain revealed triventricular hydrocephalus with meningoencephalocele extension into the nasal cavity and frontal horn herniation into the extraconal space of the orbits. The hydrocephalus was managed with ventriculoperitoneal shunt placement with rapid and complete resolution of the proptosis. Conclusion No reports have described bilateral proptosis as the presenting finding of orbital and nasal meningoencephaloceles in the absence of trauma or congenital defect. This case study demonstrates the management of meningoencephalocele formation secondary to chronic hydrocephalus.

Background Pediatric patients presenting with a nasal mass is uncommon and a high index of suspicion for it to be a meningoencephalocele is essential. Majority of these are congenital and require early intervention owing to the risk of meningitis. Surgery in these cases is very challenging because of the risk of anesthesia and limited space. Objective This study aimed to describe our technique and experience in managing pediatric meningoencephaloceles. Methods A retrospective study of 19 pediatric patients (age ranging from 40-day-old infants to 11.5-year-old children) was conducted at our tertiary care center from January 2012 to February 2017. Patients presenting with an intranasal meningoencephalocele were treated by endoscopic approach, using otological microinstruments. After detailed imaging and clinical evaluation, a tailored repair, using fat/fascia/cartilage, a multilayer closure, was carried out in all cases. Results All patients had a successful repair and recovery, except for one death following an episode of convulsion. There was a minor circumferential narrowing of the nasal vestibule and synechia formation in 2 of our cases. Conclusion Transnasal endoscopic repair of meningoencephalocele is minimally invasive. It avoids permanent anosmia and is cosmetic. A combination of miniaturized instruments and a 4-mm 0° nasal endoscope gives excellent visual field and an adequate working space. Owing to the larger defects in congenital meningoencephaloceles, a multilayer repair provides excellent outcomes.

Abstract:

Objective: Meningoencephaloceles of the temporal bone are rare entities. There are two main categories, congenital and acquired. Acquired meningoencephaloceles are more common due to iatrogenic injury to the tegmen tympani which is a common complication during mastoidectomy. Case Presentation: We present a case of an idiopathic meningoencephalocele that was diagnosed and treated successfully during cochlear implantation via transmastoid approach. Fascia lata graft was used to reconstruct the deficit of tegmen tympani. Audiological outcome has been improved. Conclusions: Idiopathic Meningoencephaloceles present without specific clinical symptomatology and as a result diagnosis delay significant. In literature, there are various surgical approaches with mixed results.

Abstract:

Background The management and surgical approach to cerebrospinal fluid (CSF) leaks and meningoencephaloceles have undergone transformation throughout the last 10 years. It is our interest to examine the long-term surgical outcome and reoccurrence rates of CSF leaks or meningoencephaloceles in patients having endoscopic surgical repair. Methods We performed a retrospective evaluation of 50 patients that underwent endoscopic surgical repair of a CSF leak, meningoencephalocele, or both, between September 1985 and October 2003. Results Cumulatively, reoccurrence rates were 15% (7/47) among the CSF leak patients with an average time frame for reoccurrence ranging from 1 to 25 months (average, 7 months). Patients with meningoencephaloceles had an overall reoccurrence rate of 8% (1/13). In addition, a Medline search on CSF leaks and meningoencephaloceles provided the names of 32 authors that have studied outcomes of endoscopic surgical repair. Of the 151 patients still followed in the 5- to 10-year postoperative group, there were 37 recurrences of CSF leaks and 5 reoccurrences of the meningoencephaloceles with a total recurrence rate of 27% (37 + 5/151). Of the 19 patients still followed in the >10-year postoperative group, there were three reoccurrences of CSF leaks and no reoccurrences of meningoencephaloceles, giving a reoccurrence rate of 16% (3 + 0/19). Conclusion Based on our cumulative results, a reoccurrence of a CSF leak or meningoencephalocele after endoscopic repair will occur within the first 2 years postoperatively. Once patients pass these postoperative time frames they are relatively free of reoccurrence from this very effective surgical management. Endoscopic repair results are better than craniotomy with much less morbidity.

Abstract:

Object Spontaneous meningoencephaloceles of the lateral sphenoid sinus are rare lesions that are hypothesized to result from persistence of the lateral craniopharyngeal canal. Prior reports of the management of this lesion have been limited by its relative rarity. The objective of this paper is to report the theoretical etiology, surgical technique, and outcomes in patients undergoing endoscopic repair of spontaneous meningoencephalocele of the sphenoid sinus. Methods The authors conducted a retrospective review of a multiinstitutional series of 13 cases involving patients who underwent endoscopic repair of spontaneous meningoencephalocele of the lateral sphenoid sinus. The surgical technique and pathophysiological considerations are discussed. Results The clinical manifestations included CSF rhinorrhea (85%), chronic headache (77%), and a history of meningitis (15%). The endoscopic approaches to the lateral sphenoid sinus were transnasal (39%), transpterygoid (23%), and transethmoid (39%). Two patients (8%) had postoperative CSF leaks, one of which closed spontaneously and one of which required revision endoscopic closure. All patients were free of leak at most recent follow-up. One patient experienced postoperative meningitis in the early postoperative period. Conclusions Endoscopic endonasal closure is an effective modality in the treatment of spontaneous meningoencephaloceles of the lateral sphenoid sinus. If the sphenoid sinus has extensive lateral pneumatization, adequate exposure may require a transpterygoid approach.

Abstract:

Introduction Indications of surgical intervention in congenital nasal meningoencephaloceles includes presence of cerebro spinal fluid rhinorrhea having a risk of causing meningitis, episodes of prior meningitis and bilateral nasal obstruction causing respiratory difficulty in these obligate nasal breathers. Many authors would like to wait till the patient attains the age of 2 to 3 years for repair of the defect due to surgical feasibility. However, early intervention prevents further episodes of meningitis in the future. We present the youngest patient of nasal meningoencephalocele successfully repaired via endoscopic approach. Case Report A 21 days old neonate was referred to us with a nasal meningoencephalocele with active cerebrospinal fluid rhinorrhoea. Radiological investigation showed a cribriform plate defect on the right side. Repair was done by endoscopic route by multi-layered closure of the defect which was augmented with a mucoperichondrial flap from the septum. Patient was asymptomatic in the post-operative follow up period and did not have any episode of meningitis till date. Conclusion Early repair by transnasal endoscopic route is a feasible surgical option for congenital anterior skull base defects with meningoencephaloceles to prevent further episodes of meningitis. This is feasible even in the neonatal period due to improved technique and instrumentation now available for endoscopic nasal surgeries.

AbstractNasal meningoencephaloceles are rare findings, represented by protrusions of intracranial contents into the nasal cavity. They present as unilateral masses, and commonly determine unilateral nasal obstruction, rhinorrhea and non-characteristic headaches.We present the case of a 34-year-old patient diagnosed with a posttraumatic transethmoidal meningoencephalocele. The patient presented with unilateral nasal obstruction, mild headache and episodic watery rhinorrhea. The treatment was endoscopic endonasal surgical excision and repair of the skull base defect, in a mixt ENT-neurosurgical team. Patient follow-up showed no remaining mass or symptoms and normal closure of the skull base defect.

Intranasal meningoencephaloceles have historically been managed by neurosurgeons, although their main clinical manifestations are rhinological. Recent advances in endoscopic skull base surgery has significantly improved the treatment of these lesions and consequently diminished appreciable surgical morbidity. We report an ethmoidal meningoencephalocele case operated on by endonasal endoscopic approach for removal of the lesion and reconstructing the associated skull base. From this experience, we conclude that removal of the lesion and watertight closure of the skull base irrespective of the size of the mass and anterior skull base defect are the operation’s most important aspects.

Abstract:

OBJECTIVEAlthough rare, frontoethmoidal meningoencephaloceles continue to pose a challenge to neurosurgeons and plastic reconstructive surgeons. Especially when faced with limited infrastructure and resources, establishing reliable and safe surgical techniques is of paramount importance. The authors present a case series in order to evaluate a previously proposed concise approach for meningoencephalocele repair, with a focus on sustainability of internationally driven surgical efforts.METHODSBetween 2001 and 2016, a total of 246 patients with frontoethmoidal meningoencephaloceles were treated using a 1-stage extracranial approach by a single surgeon in the Department of Neurosurgery of the Yangon General Hospital in Yangon, Myanmar, initially assisted by European surgeons. Outcomes and complications were evaluated.RESULTSA total of 246 patients (138 male and 108 female) were treated. Their ages ranged from 75 days to 32 years (median 8 years). The duration of follow-up ranged between 4 weeks and 16 years (median 4 months). Eighteen patients (7.3%) showed signs of increased intracranial pressure postoperatively, and early CSF rhinorrhea was observed in 27 patients (11%), with 5 (2%) of them requiring operative dural repair. In 8 patients, a decompressive lumbar puncture was performed. There were 8 postoperative deaths (3.3%) due to meningitis. In 15 patients (6.1%), recurrent herniation of brain tissue was observed; this herniation led to blindness in 1 case. The remaining patients all showed good to very good aesthetic and functional results.CONCLUSIONSA minimally invasive, purely extracranial approach to frontoethmoidal meningoencephalocele repair may serve well, especially in middle- and low-income countries. This case series points out how the frequently critiqued lack of sustainability in the field of humanitarian surgical missions, as well as the often-cited missing aftercare and dependence on foreign supporters, can be circumvented by meticulous training of local surgeons.

Abstract:

Meningoencephalocele is a rare condition that usually occurs in children and is treated by neurosurgeons with occasional help from ENT doctors. The symptoms of meningoencephalocele might not develop until adulthood, but usually they are apparent immediately after birth. The case of small anterior basal transethmoidal meningoencephalocele in a 24-year-old patient who had headaches and runny nose since childhood is presented. Endonasal endoscopic approach for meningoencephalocele removal and skull base defect reconstruction was used. It was concluded that endoscopic endonasal approach is less invasive and provides an acceptable operative outcome with short recovery time and less postoperative complications in comparison with other external microsurgical approaches.

✓ A case of posttraumatic intradiploic meningoencephalocele is reported. The patient presented with a progressively enlarging lump in the left parietal area approximately 8 months after a blunt trauma. Magnetic resonance imaging showed an intradiploic cyst containing cerebrospinal fluid and brain tissue. These findings were confirmed at surgery. In addition, the cyst was found to have a lining of arachnoid membrane.

Abstract:

✓ The authors present the case of a 6-year-old girl who developed a right frontal intradiploic meningoencephalocele following an accidental dural tear produced during surgery for craniosynostosis. Although rare, growing skull fractures have been described in at least eight cases following the accidental laceration of the dura mater in the course of craniosynostosis repair. These cases closely resemble those produced by accidental trauma to the growing skull. However, the intradiploic location of a meningoencephalocele following surgery for craniosynostosis has not been documented previously. This patient presented with headache and a frontal tumor of bonelike consistency. Radiographs and computerized tomography scans of the skull revealed an intraosseous cyst, whereas magnetic resonance imaging demonstrated cerebral tissue herniation within the intradiploic tumor. Surgical treatment consisted of duraplasty and cranioplasty, which achieved good functional and cosmetic results. The pathogenesis of this unusual lesion is discussed and compared with the hypotheses advanced for explaining posttraumatic intradiploic cysts.

Abstract:

The article represents a clinical case of secondary bacterial meningitis in a patient with meningoencephalocele, craniopathy rarely found in adults. The features of the course of the disease, diagnosis and treatment are discussed. Meningoencephalocele is a prolapse of the arachnoid membrane with brain substance due to craniopathy. Most cases of this disease are related to malformations detected in childhood. In adults, meningoencephalocele can be a consequence of injury, iatrogenesis, or some disease of unknown etiology. In this case, we can speak of spontaneous meningoencephalocele. The cause of this pathology is currently considered the effect of increased intracranial pressure on the “vulnerable” cranial areas: the Turkish saddle diaphragm, the walls of the sphenoid sinus, the ethmoid plate. Surgical treatment of intracranial hypertension is another factor likely to influence the occurrence of meningoencephalocele. Nasoliquorrhea is the most common symptom that causes patients with encephalocele health encounter. 33% of those are diagnosed with meningitis. Neuroimaging plays a key role in encephalocele diagnosis. Computed tomography of the brain can detect craniopathy. Leptomeningeal accumulation of contrast agent on T1-weighted images on magnetic resonance imaging indicates inflammation of the meninges in a patient with a clinical picture of meningitis. Encephalocele treatment is surgical. In the clinical case presented in this article, the patient with noncommunicating hydrocephalus and a history of surgical treatment of a colloid cyst was diagnosed with bacterial meningitis. Computed tomography of the head revealed a defect of the ethmoid bone and meningoencephalocele. The radiological signs of meningoencephalocele and meningitis are presented according to the data of magnetic resonance imaging in this patient. Abstract:

The article represents a clinical case of secondary bacterial meningitis in a patient with meningoencephalocele, craniopathy rarely found in adults. The features of the course of the disease, diagnosis and treatment are discussed. Meningoencephalocele is a prolapse of the arachnoid membrane with brain substance due to craniopathy. Most cases of this disease are related to malformations detected in childhood. In adults, meningoencephalocele can be a consequence of injury, iatrogenesis, or some disease of unknown etiology. In this case, we can speak of spontaneous meningoencephalocele. The cause of this pathology is currently considered the effect of increased intracranial pressure on the “vulnerable” cranial areas: the Turkish saddle diaphragm, the walls of the sphenoid sinus, the ethmoid plate. Surgical treatment of intracranial hypertension is another factor likely to influence the occurrence of meningoencephalocele. Nasoliquorrhea is the most common symptom that causes patients with encephalocele health encounter. 33% of those are diagnosed with meningitis. Neuroimaging plays a key role in encephalocele diagnosis. Computed tomography of the brain can detect craniopathy. Leptomeningeal accumulation of contrast agent on T1-weighted images on magnetic resonance imaging indicates inflammation of the meninges in a patient with a clinical picture of meningitis. Encephalocele treatment is surgical. In the clinical case presented in this article, the patient with noncommunicating hydrocephalus and a history of surgical treatment of a colloid cyst was diagnosed with bacterial meningitis. Computed tomography of the head revealed a defect of the ethmoid bone and meningoencephalocele. The radiological signs of meningoencephalocele and meningitis are presented according to the data of magnetic resonance imaging in this patient. The patient underwent a two-stage surgical treatment with the use of endoscopic techniques and craniotomy. Four years after the operation, the radiological picture and the outcome of the disease were assessed.

Background: The clinical presentation of sphenoid sinus meningoencephaloceles (MEC) may have insidious onset and evolution. Contemporary treatment incorporates endoscopic resection via the endonasal route.Case description: We present a case of 3 year old girl who had been complaining of permanent nasal discharge, impeded nasal breathing and difficulty with feeding since she was 5 months old. There was no history of rhinoliquorrhea. Pre-operative magnetic resonance imaging demonstrated MEC that extended from the sellar region through the non-pneumatized sphenoid sinus to the nasopharynx. The lesion was resected via endoscopic endonasal approach. Follow-up rhinoscopy confirmed the absence of post-operative cerebrospinal fluid leak.Conclusion: Endoscopic endonasal approach can be an effective and a safe treatment option for resection of congenital transsphenoidal MEC in early childhood.Keywords: Sphenoid sinus; endoscopic endonasal approach; meningoencephalocele; CSF leak; congenital.

Complications of Le Fort III midfacial advancement include CSF rhinorrhea, meningitis, and ocular and cerebral injury. This report reviews the anatomy of the Le Fort III osteotomies and their relevance to the unusual complication of meningoencephalocele. In this report, a young male patient with Crouzon syndrome underwent subcranial midfacial advancement at the age of 10 years for obstructive sleep apnea and ocular exposure. He presented 4 years later complaining of nasal obstruction. On physical examination, a mucous-covered mass was noted in the left upper nasal vault medial to the turbinates. Computed tomography scanning and MR imaging confirmed the diagnosis of frontoethmoidal meningoencephalocele. Repair of the meningoencephalocele was accomplished using a combined neurosurgery and plastic surgery approach. Meningoencephalocele is a rare complication of subcranial midfacial advancement. The abnormal anatomy of the anterior cranial base in patients with syndromic craniosynostosis places them at greater risk for fracture of the cribriform plate and dural tears during this procedure. Unrecognized dural injury is the etiology of this complication in this young patient; however, elevated intracranial pressure may have been a confounding factor. Attention to the anatomy of the anterior cranial base, as seen on sagittal CT images, will aid in preventing this complication.

It usually presents as nasal broadening and/or as a blue, pulsatile, compressible mass near the nasal bridge which transilluminates, enlarges with crying or with bilateral compression of the internal jugular veins (Furstenberg test), or as an intranasal mass arising from the cribriform plate. We are presenting case of a 7-month-old boy who presented with CSF rhinorrhea and an attack of meningitis after which CT scan was done and diagnosis made of meningoencephalocele. Abstract:

Case summary A 10-year-old castrated male domestic shorthair cat was referred for surgical treatment of a left-sided frontal lobe meningioma diagnosed by CT. Clinically, the cat had generalised tonic–clonic seizures, which reduced in frequency after treatment was started with prednisolone. After definition of the anatomical landmarks of the feline skull, a bilateral transfrontal craniotomy allowed en bloc removal of the meningioma. While postoperative recovery was uneventful, right-sided proprioceptive deficits were still present 6 months after surgery. MRI detected a probable meningoencephalocele herniating through the surgical bone defect in the frontal sinus. Because of the mild neurological deficits and good quality of life, the meningoencephalocele was not treated. Thirty-one months after meningioma removal the cat was alive without further neurological progression. Relevance and novel information To our knowledge, this is the first report to describe, in detail, the technique of transfrontal craniotomy in cats. Iatrogenic meningoencephalocele is a complication that has not previously been described after meningioma removal in cats, and should be considered as a potential complication after craniotomy.
