Abstract:
The aim of this chapter is to argue against the idea that there needs to be a polar opposition between "biological" and "humanistic" psychiatry. The basis for this idea lies in the philosophical tradition-the view that "mind" and "brain" must be conceived either as two separate "substances" or as one and the same. It is argued that this ontological conception of the problem should be replaced with a phenomenological description of what is actually meant by talk of human mental life. In particular, Merleau-Ponty's account of human beings as "body-subjects" enables us to explain some aspects of some mental disorders as manifestations of neurological dysfunction, others as human responses to traumatic situations, and others as both.

How exactly do mental disorders affect one’s agency? How might therapeutic interventions help patients regain or improve their autonomy? Do only some disorders excuse morally inappropriate behavior, such as theft or child neglect? Or is there nothing about having a disorder, as such, that affects whether we ought to praise or blame someone for their moral success or failure? Our volume gathers together empirically informed philosophers who are well equipped to tackle such questions. Contributors specialize in free will, agency, and responsibility, but they are informed by current scientific and clinical approaches to a wide range of psychopathologies, including autism, addiction, Tourette syndrome, personality disorders, depression, dementia, phobias, schizophrenia, and obsessive-compulsive disorder. These conditions exhibit a diverse array of symptoms that can contribute quite differently to being blameworthy or praiseworthy.

Abstract:
Throughout history and in all the world’s major faith traditions, it has been noted that conversion and other important spiritual and religious experiences may share or have features that overlap with signs and symptoms of psychiatric illness. William James, especially in The Varieties of Religious Experience, contributed significantly to the understanding of this overlap. The aim of this chapter is to explore the ethical and clinical dilemmas that arise when clinicians attempt to negotiate the seemingly conflicting imperatives of diagnosing on the one hand and of being open to the transformational significance of mental illness on the other. This is achieved by presenting three clinical cases from the author’s community psychiatric practice and analyzing them through the lens of Jonsen’s Four Quadrants Model for ethical case analysis.
