Abstract:
Every day, people struggle with mental health challenges; one in five people will experience a mental illness in their lifetime. Innovative approaches to strengthen the public mental health strategy warrant careful deliberation. This article reintroduces and explores the conceptual framework of mental hygiene. The concept of mental hygiene was originally introduced in the early 20th century, with the aim of preventing and treating mental illness and milder mental disorders. The movement lost its momentum shortly thereafter and the concept went largely ignored since then. Mental hygiene is a form of preventive maintenance that can be likened to other hygienic practices. Through the plasticity of the brain, mental training activities can foster healthy cognitive patterns that are conducive to well-being. The article offers a brief overview of some of the mental hygiene practices one can engage in, on a daily basis, to support well-being and assist in preventing mental health issues. Such mental training behaviors may potentially reduce ubiquitous human tendencies to ruminate and mind-wander without awareness, which when in excess correlate with increased activity of the default mode network and susceptibility to the pathogenesis of mental illness, along with impeding human flourishing. The article advocates for the routine engagement in healthy mental hygiene to become a global recommendation.

A century has passed since Klifford Beers published his work "A mind that found Itself". This paper has inspired the shift of mental hygiene towards the prevention of mental disorders and health improvements. At the beginning of this millenium, with much improved scientific knowledge, experience and possibilities, but confronted with new challenges we are observing the tempestuous historical features which have influenced the development of mental health care. Mental health is crucial for the welfare of a society and its individuals, since such disorders are not only the cause of emotional suffering, but they also deteriorate the quality of life, cause alienation and discrimination of an individual. At the same time, they are a great economic burden to the society as they require long-term therapy and often result in poor productivity. In order to decrease the burden of mental disorders it is required to stimulate prevention and improvement of mental health of the population within the framework of national policies, legislation and financing.

Abstract Introduction Typically, college students practice unhealthy sleep hygiene behaviors, obtain too little sleep, and experience poor sleep quality. Sleep hygiene includes the routines or practices that prepare a person for the best possible night of sleep. Good sleep hygiene habits, like creating a sleep-friendly environment and making time for sleep, promote healthy duration and quality of sleep. Stress is also an important factor to consider during the college experience. Sleep and mental health are tightly connected, and stress can negatively impact the sleep and mental health of individuals. The focus of the current study was to examine habitual sleep habits in college students, in association with sleep quality and psychological health. Methods Participants included 51 undergraduate students (18 men, average age M=20.25 years, SD=1.78) who wore wrist actigraphs to measure their typical sleep habits for one week. After one week, participants completed questionnaires about sleep quality (Pittsburgh Sleep Quality Index, PSQI) and sleep hygiene practices (Sleep Hygiene Index, SHI). Higher scores on PSQI represent poorer quality; higher scores on SHI represent unhealthy sleep hygiene behaviors. Mental health symptoms were measured by the Depression, Anxiety, and Stress Scale (DASS-21). Results Overall sleep duration was 6.59 hours and sleep efficiency was 82.55% as measured by actigraphy. PSQI scores (M=6.86) demonstrated poor sleep quality and SHI scores (M= 24.80) indicated overall poor sleep hygiene practices. SHI scores predicted higher PSQI scores (F(1, 50) = 18.05, p<.001), but did not predict sleep duration or efficiency. Depression, anxiety, and stress scores on the DASS predicted poorer sleep hygiene (F(1, 50) = 18.05, p<.001; F(1, 50) = 5.82, p=.020; F(1, 50) = 13.42, p<.001; respectively). Conclusion As expected, college students’ sleep was short in duration, poor in efficiency, and poor in quality. Additionally, poor sleep hygiene practices predicted poorer sleep quality. Interestingly, scores that indicated worse depression, anxiety, and stress predicted poorer sleep hygiene practices, suggesting that mental health may contribute to healthy sleep practices. In the article there is considered the content of the terms of such branches of science as "psycho-hygiene" and "psycho-prophylaxis", the history of the incurrence, tasks, structure, interrelationship and significance in the activity of specialists taking part in the elimination of consequences of radiation accidents and incidents. Psycho-hygiene, presenting a part of general hygiene, is an interdisciplinary field of scientific knowledge referred to general and social hygiene, social and medical psychology, and psychiatry also. It deals with the influence of various environmental factors and labor activity on the mental health of a person, as well as the development and implementation of measures to preserve people's mental health at the public, group and individual levels. The main subject of psycho-hygiene is mentally healthy people, and the most important task is the creation and provision of conditions for the formation of a harmoniously developed personality. In psycho-hygiene, like any other science, there are several areas of activity. These are: fundamental, applied and practical. In the form of the implementation of measures, it is divided into preventive, restorative and curative. Psycho-prophylaxis is a system of state, social, psychological, hygienic and medical measures aimed at ensuring a high level of mental health and preventing the occurrence of mental disorders. Psycho-prophylaxis has such main subject as subclinical, painful manifestations in the psyche of people, in the conditions of occurrence and impact of unfavorable and stressful factors on the person. It develops and implements measures to prevent the occurrence of mental illness, and also facilitates the rehabilitation of people with mental disorders. Considerable attention is paid to the role and importance of psycho-hygiene and psycho-prophylaxis in the activity of emergency rescue teams. In the course of the elimination of consequences of radiation accidents and incidents, in this category there is possible the appearance of psychological trauma with a complex, specific and multifactorial stress effect that underlies a whole spectrum of mental disorders. The implementation of a complex of timely psycho-preventive measures will allow reveal initial signs of mental disadaptation, psychosomatic pathology, emotional burnout and determine effective measures for restoring and preserving mental health in emergency rescue specialists of teams of Federal Medical Biophysical Centre of the the Federal Medical Biological Agency of Russia.

Mental disorders have the potential to affect an individual’s capacity to perform household daily activities such as water, sanitation, and hygiene (food hygiene inclusive) that require effort, time, and strong internal motivation. However, there is limited detailed assessment about the influence of mental health on food hygiene behaviors at household level. We conducted a follow-up study to detect the effects of mental health on food hygiene behaviors after food hygiene intervention delivery to child caregivers in rural Malawi. Face-to-face interviews, based on the Risk, Attitude, Norms, Ability, and Self-regulations (RANAS) model, were conducted with 819 participants (control and intervention group) to assess their handwashing and food hygiene-related behaviors. Mental health was assessed using the validated Self-Reporting Questionnaire. Study results showed a significant negative relationship between mental health and handwashing with soap behavior (r = −0.135) and keeping utensils in an elevated place (r = −0.093). Further, a significant difference was found between people with good versus poor mental health on handwashing with soap behavior (p = 0.050) among the intervention group. The results showed that the influence of the intervention on handwashing with soap behavior was mediated by mental health. Thus, integration of mental health in food hygiene interventions can result in improved outcomes for caregivers with poor mental health.

Sleep is an integral human need. A significant public health issue is getting little sleep, which harms health. University students are one of the groups most at risk for poor sleep quality and chronic sleep loss, which could influence both physical and mental health. Students frequently report having persistent sleep issues and may exhibit poor sleep hygiene habits. However, little research has investigated the role of good sleep habits concerning sleep quality and mental health. The present study explores the relationship between sleep hygiene, sleep quality, and mental health among university students. The research design was cross-sectional, and the sample was chosen using a non-probability convenience sampling strategy. Sample comprised of university students (N= 210). The scale of positive mental health, sleep hygiene index, and general sleep scale were used for data collection. Results revealed a significant correlation between sleep quality, sleep hygiene, and mental health among university students. Both sleep hygiene and quality were found to be significant predictors of positive mental health. A significant gender difference was found between sleep quality, sleep hygiene, and positive mental health among male and female university students. These findings can provide information about the link between the study variables and interventions for developing good sleep hygiene habits and quality of sleep amongst university students. It could help improve mental health among this at-risk population.

The increasing flow of information affects health. The prevalence of information as an environmental factor is considered. With the growth of traditional information-dependent morbidity according to ICD-10, a new pathology appears as follows: computer syndrome, television addiction, depression from social networks, Internet addiction, ludomania, nomophobia, Internet suicides. There is presented a conceptual framework of prevention trends: information hygiene, ecology, ethics, digital-, cyber-, internet-hygiene. Information hygiene is a branch of medical science that studies the regularities of the impact of information on the mental, physical and social well-being of a person, his working capacity, life expectancy, public health of society, developing standards and measures to improve the information environment and optimize intellectual activity. There was analyzed the existing hygienic regulation of physical signals and perspectives of hygienic assessment activities with pronounced information loads. The shortcomings of the existing dosimetry, modern diagnostic devices for the dose-effect methodology are considered. There is discussed the importance of informational hygiene for content limits, targeting boundaries and specialization in the dissemination of information for the prevention of infopandemics, accompanied by virusophobia, quarantinomania, radiophobia, mysophobia. There are provided the scientific trends of multi-disciplinary mega-projects of the study of intelligence for the new hygienic approaches to optimize mental health. Taking into account the strategies of the national project “Education”, the digital development of the information society in Russia, as well as prevention of info-epidemics, the development of a textbook on information hygiene can be relied upon the principles outlined in the axiomatics of information ecology. There is proposed expediency of adequate responses from hygienists, preventive medicine and the state sanitary service for consideration. In social and communal hygiene - the study of mental statistics, public health, prediction, prevention infopandemics. In the hygiene of children and adolescents there is considered the development of recommendations and standards for gadgets and the time of their use, the assessment of the phenomena of clip-thinking, the transition to electronic textbooks, “blinkers” on the quality and volume of information in the search for knowledge. In occupational health - determination of the limits of intelligence from neuroscience, intensification of brain-computer interfaces, “dose-effects” and maximum permissible levels of carrier signals, optimization of information cooperation.

Introduction: Smoking is a major risk factor for development of serious disease and smoking cessation greatly reduces this risk. The association between smoking, smoking cessation and mental health however, is less clear-cut, therefore this thesis aimed to further investigate this association. Methods: The first part of the thesis reports a systematic review and meta-analysis of longitudinal studies to determine the difference in change in mental health between quitters and continuing smokers. The second part of the thesis reports three prospective analyses of individual level-patient data from five trials for smoking reduction treatment. The first analysis examined the association between cessation and change in mental health using propensity score matching (PSM). The second analysis examined the association between cessation and risk of psychiatric disorder using PSM. The final analysis examined the association between change in mental health after quitting and odds of relapse. Results and interpretations: Cessation was associated with improvements in mental health compared with continuing smoking; there was no association between cessation and risk of psychiatric disorder, and no association between change in mental health after cessation and future relapse. Results support the misattribution hypothesis, and have implications for future research, smoking cessation treatment and public health policy.

Abstract:
The purpose of this study is to examine the dynamics between professional knowledge and the power to construct social realities. The focus is on the institutions which contributed to mental hygiene as a protocol for public policies directed toward children. The social history of the child in the twentieth century is juxtaposed with shifts in the configurations of private and public institutions in a sociology of mental illness. The mental hygiene movement created one of the twentieth century's major paradigms. Mental hygiene was conceptualized as the development of a science of promoting mental health and preventing mental illness. The' working premise of the movement was that early life experiences determined adult competence and constituted the root cause of major social problems from crime and dependency to labour unrest and war. The National Committee for Mental Hygiene was established in the United States in 1909 and a second National Committee was established in Canada in 1918. Mental hygienists developed an ideology of child oriented prevention in public health, welfare and educational policies which legitimated public intervention into the private spheres of family relations and child rearing. The idea of mental hygiene was based on a medical model and as such it was part of the new psychiatry and public health movements of the Progressive Era. As a paradigm mental hygiene fostered the identification of children according to scientific standards. Mental hygiene contributed to the transformation of juvenile delinquency into a psychiatry of maladjustment in childhood. As a positivistic approach to public health, mental hygiene research elaborated criteria to determine age related stages of normal psychological and biological progress. Mental hygiene was a product of professional researchers and policy makers. The knowledge base of mental hygiene grew with the expansion of higher education in the United States especially in regard to scientific medicine. The medical model was subsequently applied to research in the behavioural and social sciences. Scientific philanthropy provided funds for research, professional education, and the distribution of knowledge. The accumulation of monetary resources by nineteenth century entrepreneurial capitalists, who applied these funds to further the growth of scientific models, were a sustaining factor in twentieth century mental hygiene. The agents of power described as part of the mental hygiene movement include: 1) the National Committees for Mental Hygiene in the United States and Canada; and, 2) general purpose foundations in Rockefeller related philanthropy and the Commonwealth Fund. By mid-century, the federal, state/provincial and local governments of the United States and Canada had assumed major aspects of the former role of the National Committees and philanthropy in mental health advocacy. The theoretical foundation of mental hygiene evolved in conjunction with the development of the scientific method as applied to preventive medicine, especially in fields related to psychiatry. Mental hygiene was a primary carrier of the medical model into applied disciplines in the social and health sciences. The professionalization of education, social welfare and psychology, as imbued with mental hygiene, translated technological change into revised concepts of public and private spheres in relationship to family and child life. The medicalization of human differences limited the potential for radical revisions in social organization. It justified unequal access to political and economic power on the basis of psychological and biological characteristics. The mental hygiene paradigm served to maintain established social configurations in the face of social change. The function of justifying inequalities was especially important in the United States but less so in Canada for reasons of the timing of nation-building, national history, character, and culture.

Perinatal mental illness is a global health issue with detrimental outcomes for women and their families if left untreated. Unfortunately, many women do not get the treatment they need for many reasons, one often acknowledged reason is that the stigma some women experience inhibits disclosure of their needs to healthcare providers. This thesis looked at Internet resources for women with perinatal mental illness, in particular online forums. It examined how forums might affect stigma and thus disclosure behaviour. This thesis is by prospective publication. Article 1 aimed to describe and interpret qualitative studies regarding forum use and perinatal mental illness stigma. A metasynthesis of five studies identified four key themes: a safe place to talk; virtual support; stigma and identity; and repair of the mother identity (Moore, Ayers & Drey, under review). Article 2 aimed to identify what websites about postnatal mental illness were available and assess them for content and quality. A systematic review of 114 websites evaluated accuracy of information, resources and website quality. Results showed information was largely incomplete and difficult to read; resources were limited and website quality was variable (Moore & Ayers, 2011). Article 3 aimed to determine how women with perinatal mental illness use web based resources. A qualitative interview study (n= 15) found that the anonymity and non-judgemental social support on forums may have made it an acceptable way to challenge internal stigma and that most women described forums as providing a space to discuss stigma and test out disclosing about their illness to others (Moore & Ayers, 2016). Article 4 therefore aimed to identify whether forums for perinatal mental illness reduce stigma and facilitate disclosure. Thematic analysis of 1546 posts over six months on a forum for postnatal mental illness suggested that forum discourse reconstructed ideology of motherhood as compatible with perinatal mental illness. Many women overcame stigma and posted that they had taken advice and disclosed to a healthcare provider (Moore, Ayers, & Drey, 2016). The final article developed and tested a hypothesised model of the relationship between stigma and disclosure about perinatal mental illness. Study 5 developed a questionnaire measure of stigma for perinatal mental illness in order to test the model. Questionnaire items were completed online by women with perinatal mental illness (n=279). Psychometric testing suggested it was a valid scale with three subscales: external, internal and disclosure stigma (Moore, Ayers, & Drey, 2017). Study 6 tested a hypothesised model that stigma would mediate the relationship between forum use and disclosure to healthcare providers. An online survey of women with perinatal mental illness (n=200) who had used forums provided partial support for this hypothesis, with internal stigma mediating the relationship between length of forum use and disclosure (Moore, Drey, & Ayers, 2017). This research highlights the importance of considering the stigma associated with perinatal mental illness and its role in online forum use and disclosure. Overall, findings suggest that forums may facilitate recognition of stigma, which may in turn lead to greater disclosure of symptoms to healthcare providers. However, the relationship between forum use, stigma and disclosure may be more complex than our initial model proposed. Similarly, most participants in these studies were white, well-educated and actively participated in the forums. Future studies would benefit from testing these relationships using longitudinal designs with more representative samples.

There are gaps in the UK knowledge base for understanding the implementation and evaluation of public health interventions which aim to improve the mental health and wellbeing of participants. In this thesis I examine the measurement of mental wellbeing and the implementation of health improvement interventions in a community setting and investigate the practicalities of their evaluation using a measure of mental wellbeing -- WEMWBS. Methods: Using a mixed methods approach I collected and analysed i) three cross sectional surveys of Coventry residents, ii) quasi-experimental before and after outcome evaluations of three CHIP projects, and iii) undertook semi-structured interviews with CHIP stakeholders. Data were integrated using a matrix technique. Results: A total of 8188 individuals (~40% response rate) completed valid survey questionnaires in 2010-2012, while 590 individuals (~88% response rate) completed valid before-after mental wellbeing outcome evaluations in 2011 and 2012 from three CHIP projects. Fifteen one-on-one interviews were completed. I found that health and lifestyle variables ‘sleep quality’ ‘physical activity’ and ‘fruit and vegetable consumption’ showed the strongest and most consistent patterns of association with levels of mental wellbeing measured using WEMWBS. CHIP projects demonstrated associations between the intervention and increases in mental wellbeing, some of which were both statistically significant and clinically meaningful. Some were sustained at three months. Interview findings showed that the difference between the plans and the observed implementation practices resulted in some of the projects struggling to cope with the evolving and changing needs of the programme, for example moving from outputs to outcomes, introducing mental wellbeing and changing concepts of health, and the work required to achieve partnership with the local authority). The effect on programme level outcomes and outcome measurement of these struggles was a reduction in the number and quality of valid evaluation returns from some of the projects in the programme and reduced staff capacity to deliver project objectives. The introduction of mental wellbeing as an outcome measure created a momentum of change for understanding complex health interventions and outcomes among stakeholders; it assisted those delivering the CHIP programme to understand the underlying health improvement rationale for their programme better. Through integrating quantitative datasets I provided a benchmark from which to make comparisons between population estimates of WEMWBS and observed evaluation findings. Integrating quantitative evaluation process challenges and qualitative insights from stakeholder interviews allowed for complex issues to be ‘untangled’. Interrelated mechanisms affected facilitators and barriers of programme planning, implementation, evaluation and sustainability. Integrating my quantitative and qualitative findings highlighted some clear health benefits from the projects but also highlighted a lack of congruence between the documented linear, unidirectional and unrealistic operational planning which I found in CHIP at a programme level, compared to practical implementation on the ground, which was nonlinear, complex and dynamic. Conclusion: Iterative, transitional stages of programme development could benefit implementation processes and potentially health outcomes, including mental wellbeing, in future public health practice. Further research in this area should explore the extent to which complex, collective, and adaptive operational planning can result in more successful public health improvement programmes.

This thesis examines how mental hygiene principles were adopted for a student audience through the elementary-level health textbooks series, Health and Personal Development, used in Ontario schools from 1952 until 1963. In particular, I explore the didactic messages pertaining to mental hygiene as they related to girls. The results of this analysis demonstrate that healthy mental hygiene and personal development for girls, according to the textbooks, meant becoming wives, mothers, and homemakers, as their own mothers model. While these roles required many skills and responsibilities, and provided women with a certain amount of agency in the female-dominated sphere, girls were represented in the textbooks as having a limited set of options in life: to emulate their mothers’ feminine domesticity, or to risk a life marred by poor mental hygiene.

Background: The burden of mental disorders in post-conflict areas is higher than in countries where there is no conflict. Post-conflict areas lack the resources to respond to such high burden, resulting in a treatment gap. This study explored the potential for mental health reform in Gaza. Design: Mixed methods study. Methods: The WHO-AIMS questionnaire was used to assess mental health services in Gaza and documentary analysis was conducted to evaluate the Mental Health Policy and Plan. Healthcare professionals‟ mental health training needs were identified and policy makers, healthcare professionals, service users and carers took part in focus groups to elicit their perspectives on mental health reform. Framework analysis was used to identify recurrent themes. Results: Psychotherapy, recovery, family intervention and crisis management were rated priority training areas. Poor coordination between governmental and non-governmental organisations and short-term funding were identified as main barriers to mental health reform. Service users and carers felt excluded from service delivery and development and suggested possible models of partnership working with service providers to address exclusion. Conclusion: Meaningful mental health reform in post-conflict areas requires inclusive policy development, targeted staff training, improved coordination between different service providers, and partnership working between service providers and service users and carers.

Background: The HIV/AIDS epidemic has contributed to a drastic increase in the number of orphans and vulnerable children in sub-Saharan Africa. However, little is known about the mental health of these children in low prevalence areas such as Ghana. The thesis investigated the relationship between orphanhood, parental HIV/AIDS status and mental health. It further examined the mediating effects of identified risk and protective factors on the relationship between orphanhood/parental HIV/AIDS status and psychological difficulties. Finally, the thesis identified pathways through which HIV/AIDS impacts children by exploring the interactive and cumulative effects of the various risk and protective factors on psychological difficulties. Method: The thesis employed cross-sectional, quantitative interviews that involved 291 children aged 10-18 years and their caregivers that compared children who have lost their parents to AIDS, those who have lost their parents to other causes, those who are living with HIV/AIDS-infected caregivers and children from intact families in the Manya Krobo district in Ghana. ANOVAs, T-tests, General Linear Models, Log-linear Analyses, Chi-Squares and Bivariate Correlations were used to analyze the data that were obtained from both the children and their caregivers. Results After controlling for relevant socio-demographic factors, both children and informants’ reports showed that children orphaned by AIDS and those living with infected parents showed higher delinquency (p <.01), peer problems (p <.001), hyperactivity (p <.01) and lower self esteem (p <.001) than other orphans and children from intact families. AIDS orphans, other orphans and those living with HIV/AIDS-infected parents all reported significantly more depression (p <.001) and relationship problems (p <.001) than those for intact families. Conduct problems as indicated by informants’ reports were generally, significantly higher for orphans and vulnerable children compared to children from intact families. Over 70% of both AIDS orphans and children living with infected parents showed internalising symptoms that were above clinical cut-offs for abnormality. AIDS orphans and children living with infected parents reported more stigma, abuse, child labour and lower levels of SES and lower perceived social support. These factors independently, strongly mediated the relationship between orphanhood, parental HIV/AIDS status and mental health. The interactive and cumulative effect of engagement in child labour and being physically abused heightened the risks for depressive symptoms from 38% to 66%. Neglect and psychological abuse increased the risks for symptoms of Reactive Attachment Disorder from 26.6% to 67.3%. The cumulative effect of stigma and either child labour or physical abuse substantially increased the likelihood of delinquency symptoms to approximately 67%. Conclusion: The findings demonstrated that both AIDS orphans and children living with HIV/AIDS-infected parents showed heightened psychosocial symptoms. The present evidence also highlighted the interactive, cumulative, co-occurrence of contextual factors and HIV/AIDS unique exposures to create heightened vulnerabilities for psychological difficulties among children. The findings call for a comprehensive intervention programme that addresses factors specific to HIV/AIDS and contextual variables.

Motivational based approaches have been shown to be effective in reducing problematic behaviours in the general population. The present study reviewed 31 studies that aimed to reduce a number of problematic behaviours amongst individuals with severe mental health problems. Evidence supporting the efficacy of motivational based approaches in increasing adherence to medication was inconsistent. In the minority of studies where an increase in medication adherence was reported, the duration of effect appeared to decline over time. The evidence from the studies that aimed to reduce substance use was more consistent. A number of other positive outcomes were reported reductions in: dependence, general functioning and mental health symptomatology. The empirical paper presents a quantitative study that aimed to look at the reasons for cannabis use amongst individuals with and without severe mental health problems. It was found that the motivations for using cannabis did not differ between the two groups; both using cannabis to cope with negative affect, for pleasure and for social reasons. However, individuals with severe mental health problems differed in that they expected cannabis to be more ‘socially and sexually facilitative’. Irrespective of mental health status, participants who used cannabis more problematically endorsed more coping and pleasure motives.

This study explores the defining moments in six men’s lives. The empirical dimension of the research is built around the personal narratives these men tell of their lives across a series of four interviews. The central research theme is the notion of the defining moment as a key element in the processes of establishing how men understand and interpret the events and incidents that have shaped their lives. In the context of this study, the defining moment is seen as the moment or period in time when an individual gives definition to a specific event or experience, as a transition point with (potentially) life-altering consequences. Some of the thematic structures presented include relationships with significant adults (parents, teachers), masculinity, self-harm, schooling, mental illness, isolation, loneliness, stress and relationships with peers. In my pursuit of a methodology that could accommodate the aims of this study, I explored the process of meaning through the qualitative paradigm. Drawing on the principles of qualitative research, as applied through narrative inquiry, I deployed a semi-structured interview format to collect the lived experiences of participants. By privileging the stories that individuals tell of their experiences, the narrative method recognises that data are inexorably located in the contextual and contingent. The experiences and narratives that are presented in this thesis are built around the authentic voices of participants. The study presents a warrant for working with men’s defining moments to disrupt, alter and redefine their attitudes and behaviours in order to improve their lives. Based on the insights gleaned through this study, I argue that there are defining times/points in people’s lives where their experiences can be life altering. When these experiences involve uncertainty, anxiety, stress and other pernicious effects, their longer-term consequences can be devastating. The study confirms existing research, that men are reluctant to seek help or reveal their insecurities during such times, therefore making them particularly vulnerable to defining moments. The conclusion of this thesis establishes some broad recommendations pertaining to working effectively with men and their defining moments. I focus particular attention on the place of schooling and education in helping individuals recognise and respond to the early symptoms of what is potentially a life-altering experience. Schools and, by association, teachers need to be actively and strategically involved in this process. To this end, I argue the need for targeted interventions that are both sensitive and timely. In their engagements with young males, parents, teachers, coaches and mentors need to be particularly attuned to their silent screams for help.

Contemporary neoliberal reconfigurations of statutory mental health services involve significant organisational changes. Based on findings from twelve months fieldwork within a community mental health team, the thesis examines the effects of this new service landscape on the way conceptualisations of mental distress are utilised and articulated. The thesis combines critical realist epistemology and reflexive ethnographic method to produce a contextually situated understanding of the field capturing the dynamic relationships between concepts, agents and the context of action. This draws on and extends Rhodes’ ‘pentimento’ (1993) as a conceptual framework for understanding mental health practice. It argues the mental health team is a ‘differentially sedimented structural institution’ in which practitioners and service users navigate a field of contradictions defined by four strata: the custodial system of the asylum; the biomedical treatment system of the hospital; community care within the Keynesian welfare state; and neoliberal welfare reconfigurations. These are conceptualised as ideological positions that coexist within practitioners as alternative modes of thinking and operate in a relationship of mutual tension. Practice should be understood as a process shaped by mechanisms at different levels of scale from micro to macro, and involving movement between these overlapping and co-existing strata of historically sedimented meaning.

AbstractAlthough the social structure of Turkish society has changed from a broad family order to a nuclear family, family relations still hold an important place, where traditional elements dominate. Still, elderly people are cared for by their family in their home environment. Thus, the role of family members is crucial in taking care of elderly individuals. In Turkey, the responsibility of care is largely on women; the elderly’s wife, daughter, or daughter-in-law most often provides the care. Family members who provide care need support so that they can maintain their physical, psychological and mental health. At this point, Antonovsky’s salutogenic health model represents a positive and holistic approach to support individual’s health and coping. The salutogenic understanding of health emphasizes both physical, psychological, social, spiritual and cultural resources which can be utilized not only to avoid illness, but to promote health.With the rapidly increasing ageing population globally, health expenditures and the need for care are increasing accordingly. This increase reveals the importance of health-promoting practices in elderly care, which are important for the well-being and quality of life of older individuals and their families, as well as cost effectiveness. In Turkey, the emphasis on health-promoting practices is mostly focused in home-care services including examination, treatment, nursing care, medical care, medical equipment and device services, psychological support, physiotherapy, follow-up, rehabilitation services, housework (laundry, shopping, cleaning, food), personal care (dressing, bathroom, and personal hygiene help), 24-h emergency service, transportation, financial advice and training services within the scope of the social state policy for the elderly 65 years and older, whereas medical management of diseases serves elderly over the age of 85. In the Turkish health care system, salutogenesis can be used in principle for two aims: to guide health-promotion interventions in health care practice, and to (re)orient health care practice and research. The salutogenic orientation encompasses all elderly people independently of their position on the ease-/dis-ease continuum. This chapter presents health-promotion practices in the care of elderly home-dwelling people living in Turkey.

AbstractThis paper reviews three competing ways of organizing health care delivery—professionalism, consumerism and statism—and explores how Germany’s exclusively statist model facilitated the ascendency of an alternative Nazi medical ethics predicated on eugenic conceptions of national “race hygiene.” The primary obligation of health care personnel became using their skills and knowledge to achieve the aims of the Nazi state, which justified forcible eugenic sterilization programs, and then the killing of children and adults with mental or physical disabilities and, eventually, the medicalized mass murder of other groups seen to pose a genetic threat to the health of the state, such as homosexuals, Jews, and Roma. The evolving international response to these medical crimes would come to affect medical professional approaches to virtually every issue in contemporary Bioethics, from abortion to xenotransplantation. In the early post-war years, news of German health care professionals’ participation in these actions shocked fellow health care professionals. Many denied these accounts, some defended German researchers, others dismissed the Germans’ justifications of their actions as madness parading as medicine or medical ethics. Ultimately, however, reformers seeking to remedy or prevent actions reminiscent of Nazi medicine created the foundational documents of modern health care professional ethics and the new field of Bioethics. These are the Nuremberg Code, the Declaration of Geneva, and the Belmont Report. In firmly rejecting Nazi medical ethics, these documents emphasize the rights of autonomous individuals, with health professionals serving as their agents, thus cementing modern ideals of health care professionalism.

Abstract The objective of this paper is to advocate the practice of mental hygiene as an occupational health and safety solution to safeguard mental health and enhance psychological resilience of the workforce in the oil and gas industry. The oil and gas industry is a highly-demanding industry with its inherent environmental and psychological challenges posing a certain risk to employees' mental health. In addition, varied responses to psychological factors of the workforce may also increase the risk to mental health. A comparative study has shown that anxiety and depression is more prevalent among workers in the oil and gas industry in comparison to the general population(1). The reason behind this is particular pressure to avoid mistakes in the industry. There are serious consequences of mistakes in this industry including loss of lives and harm to the environment. In addition, offshore workers also experience isolation, loneliness, frustration and dissatisfaction stemming from working remotely, away from family. Unum Group conducted a claim analysis in the oil & gas extraction industry that showed that 26 percent of incapacity claims between 2014 and 2018 are caused by mental and psychological issues(2). The growing acknowledgment of the role of stressors in the workplace on mental health accentuates the importance of preventive psychological strategies. Mental hygiene is a preventive measure for sustaining good emotional health by developing and maintaining certain behavioral, social, and emotional skills. This paper highlights some of the mental hygiene practices considering the pertinence and aptness to the unique work environment of the oil and gas industry.

Depression and anxiety in general medical practice lead to maladaptive conditions that reduce performance and contribute to the manifestation of psychosomatic disorders. Timely prevention and diagnosis of these disorders becomes an important condition for the success of medical care and the return of the patient to work. The purpose of the work is to study the nature of the distribution and the severity of secondary anxiety and depression, and the state of the problem of mental health and mental hygiene from the standpoint of occupational medicine. Research methods. Patients (n=260) undergoing hospitalization in the clinic of occupational diseases were examined. The survey was conducted with the HADS and Beck tests (n=241, mean age 55.7±11.3 years) and included men (n=120, mean age 55.3±12.9 years), women (n=121, mean age 60.4±10.5 years). Results. In the risk group n=94 with severe anxiety (10.9±2.84) and depression (10.2±1.9), 67% were women. The distribution of indicators of anxiety and depression showed the predominance of values on the anxiety scale for occupational (10.5±2.4) and work-related (11.8±3.2) diseases in the group of women. Conclusion. Based on the results obtained, the question arises of the need for an in-depth study of these conditions in the clinic and the improvement of psychohygienic measures in the workplace.

Evidence suggests that the mental health and psycho-psychopathological consequences of the COVID-19 pandemic in the general population include an increase in symptoms of anxiety and depression, addictive behaviors, and psychological distress (Osváth, 2021; Galea, Marchant & Lurie, 2020). The prevention and mitigation of negative effects require the development and application of general and specific methods that support mental hygiene. Psychological distress due to the epidemic affects the risk of burnout in helping professionals, as it significantly increases the level of emotional exhaustion and depersonalization and reduces the feeling of personal effectiveness (Mion et al, 2021). At the 2021 International Conference on Education and New Developments, we presented our research, in which we involved the staff of the Somogy County Pedagogical Service, mainly special education teachers. The 116-person sample was conducted with a version of the Maslach Burnout Inventory developed for educators (Maslach Burnout Inventory-Educator's Survey) in the pre-COVID-19 pandemic period. In our presentation, we present the results of a survey conducted in December 2021 with a large proportion of the study population. Based on our knowledge from the literature, we expected that the level of burnout would increase in the study population, despite the institution's mental hygiene and burnout prevention strategy, but our hypothesis was not confirmed. Subjects achieved the highest scores on the three subscales of the questionnaire in the emotional exhaustion subscale and the lowest scores on the depersonalization subscale in the present study. However, based on their answers to our question, 81% of respondents felt that the COVID-19 pandemic had a negative impact on their mental health.

The article examines the results of the study of the socio-psychological consequences of the COVID-19 pandemic, which are felt by participants in the educational process, conducted as part of the research «Overcoming the effects of the COVID-19 pandemic in the psychological service of education.» The rating list of social and psychological problems of the participants of the educational process, which are formed by the participants of the online focused interview from Chernivtsi region, is presented. An anonymous online survey of teachers has identified four main features that create a high level of personal anxiety and worsen the state of mental health, and hence the quality of professional teaching: fear of infection; difficulties with the organization of time in terms of distance learning; inability to perform professional tasks in full; emotional exhaustion. The consequences of the pandemic, quarantine, COVID-19 disease on the emotional burnout of teachers are analyzed. The article specifies the tasks of professional activity of specialists of the psychological service of the education system to overcome the consequences of the pandemic. Strategies strategies and means of psychological assistance to pedagogical workers, parents, students are offered for the purpose of prevention of emotional burnout and overcoming of psych emotional exhaustion after illness in the conditions of distance learning: practical-orientation strategy of preservation of mental health, formation of skills of psych hygiene; practical and constructive strategy of achieving emotional balance and mastering new trajectories of emotional response; personality-facilitative as an emotional support of the teacher in the personal-professional transformation to online learning, self-development. Abstract: Proper amount of sleep has a critical role in biological processes and overall health and is linked to negative health outcomes, such as diabetes, heart disease, brain stroke, weight changes, pain, mood swings, Alzheimer's disease, cognitive health, chromo-some changes etc. Despite dedicating one-third of our lives to sleep, a substantial por-tion of the world's population faces sleep deprivation. Insufficient sleep goes beyond simple tiredness, and it is affecting humans metabolic, cardiovascular, cognitive, and emotional dimensions. When it comes to physical activity and athletic performance, sleep deprivation manifests in compromised reaction time, accuracy, vigour, sub-maximal strength, and endurance. These challenges also affect athlete’s cognitive functions such as judgment and decision-making. In the context of athletic perfor-mance, even partial sleep deprivation can lead to significant impairments. Relation-ship between sleep and physical activity was found as emphasizing the importance of adequate sleep for optimal performance and shows that good sleep hygiene and rec-ommended amount of physical activity really go hand in hand contributing to overall well-being including physiological and mental health. As part of CLARISSA’s qualitative, thematic research agenda, Geographic Information System (GIS) journey mapping and ethnographic observation was conducted to gain insights into the daily lives, experiences, journeys, and feelings of children involved in the Adult Entertainment Sector (AES). Twenty children documented their daily activities using a mobile app, with support and accompaniment from CLARISSA researchers. Subsequently, a collaborative analysis was conducted with participants and the CLARISSA research team, culminating in the formation of a number of GIS-based Action Research Groups (ARGs) to work on predominant issues or themes. The formation of Nepal ARG 5 was prompted by recurring issues around children’s mental health and overall wellbeing that emerged through the research. The children involved are exposed to various forms of exploitation, abuse, trauma, and violence, all of which have a profound and adverse impact on their mental health. As such, this group was formed to work on the theme/issue ‘mental health and wellbeing’.
