Abstract:
Culturally sensitive care is thought to take into account a person's specific cultural values and preferences when providing mental health care services. Latinos currently comprise 17% of the total U.S. population at 50.5 million and persons of Mexican heritage constitute over 66% of all Latinos in the United States. Persons of Mexican heritage experience higher rates of mental health issues and illness with 30% lifetime incidence versus 20% incidence for Anglos. Few studies have focused on the mental health treatment preferences for persons of Mexican heritage. Treatment preferences could reflect personal characteristics, acculturation perspective about mental health issues and illness, and experience with treatment. Mass media may also influence treatment preferences and mental health information-seeking.The purpose of this study was to describe preferences for mental health treatment services for persons of Mexican heritage living in the Southwest along the United States-Mexico border. Twenty-one participants were interviewed individually and their responses analyzed using Atlas-ti qualitative analysis software. The participants reported twenty-five mental health treatment preferences. The top six preferences- medication, going to the doctor, social and family support, counseling and herbal medicines, were consistent throughout demographic categories of age, gender, income, generational status, insurance status, education, and acculturation. Self-management interventions and integrative medicine were also reported as treatment preferences. Participants reported media use of television, internet, books and magazines, in-person interaction, and radio as primary mental health information sources. Media influences on mental health included education/information, hope, normalization, and a catalyst for conversation. Ascribed meanings for anxiety, depression, substance abuse, schizophrenia, and bipolar disorder included cognitive, behavioral, and interactional reports. Mental health services for persons of Mexican heritage should include varying holistic mental health treatment practices, recognizing the need for understanding of potential meanings for mental health issues and illness. Persons of Mexican heritage report the desire for the same types of allopathic care including medications and counseling as Anglos in the US. Additionally, self-management interventions and integrative medicine therapies, as well as innovative media outreach methods were reported as integral to the holistic treatment process of obtaining help for mental health issues and illness.

Abstract:
Forensic psychiatric nursing is a specialty at the junction of two well-researched intersecting systems with two different mandates: criminal justice (public protection) and health care (public good). Nurses’ involvement at one of the systems’ points of juncture, review board (RB) hearings, has largely been left unexplored. At RB hearings, a panel of legal and health care professionals determines if persons unfit to stand trial (UST) or not criminally responsible on account of mental disorder (NCRMD) represent significant threats to the safety of the public, and orders conditions aimed at keeping the community safe. The aim of this research project was to explore how psychiatric and public safety discourses construct the identity of persons UST or NCRMD during RB hearings, and nurses’ contribution to such identity construction. Critical ethnography methodology was employed, mobilizing three data sources: interviews with forensic psychiatric nurses, observations of RB hearings, and RB documentary artifacts. A poststructuralist lens was used to discern how RB culture produces truths about persons UST or NCRMD that sustain the hegemony of public safety and psychiatric discourses. The main finding was that the forensic psychiatric structure leverages therapeutic nursing interventions and documentation as evidence of deviancy, so that persons UST or NCRMD can be objectified and produced as dangerous, prior to socially rehabilitating them. Discursive structures sustaining the forensic psychiatric system inscribe nursing care within a disciplinary scheme, rendering it coercive and punitive. Thus, a care-and-custody dichotomy is insufficient to explain the complex processes at play in forensic psychiatry. These findings have implications for research, practice, and education in forensic psychiatric nursing, nursing ethics, and other nursing specialties on the medico-legal borderland.

Abstract:
The purpose of this study was to describe the perceived barriers to obtaining psychiatric treatment at the Johnson City Community Health Center. The context of the study was a rural area in Eastern Tennessee. Five patients with confirmed DSM-IV mental health diagnoses were recruited during treatment and interviewed at the Johnson City Community Health Center after their scheduled appointments with a Mental Health Nurse Practitioner (MHNP). The semi-structured interview focused on perceived barriers to obtaining treatment, perceptions of treatment received, and perceived availability of treatment. From those interviews, two themes were identified and each of which had two sub-themes identified: Realities of Treatment with the sub-themes of Therapy-Related Realities and Logistics Realities, The Way It Is with the sub-themes of Take Care of It Myself and Don’t Want People to Know. The findings indicate that there is a duality of positive and negative aspects of treatment at Johnson City Community Health Center. Understanding the needs and perceptions of those with psychiatric diagnoses will assist all staff and mental health providers in developing programs that are better suited for those with psychiatric diagnoses receiving treatment from Johnson City Community Health Center.

Este trabalho é resultado de uma pesquisa do tipo estudo de caso, realizada em um macro-hospital público psiquiátrico de uma cidade do interior do Estado da Bahia. Enfoca o desenvolvimento das atividades dos enfermeiros no cotidiano institucional, suas formas de lidarem com as situações e as relações sociais estabelecidas com os demais profissionais de saúde que trabalham no hospital. Como docente da disciplina Enfermagem Psiquiátrica, tenho relação direta com o local, acompanhando estágio de estudantes de enfermagem, haja vista o convênio firmado entre a Secretaria de Saúde do Estado e a Universidade Estadual de Feira de Santana, a qual pertenço. Procedi a um recorte do campo, delimitando o espaço de um Pavilhão de internamento masculino. Como estratégia de levantamento dos dados utilizei a observação participante, entrevista semi-estruturada com os enfermeiros e demais membros que com ele convivem no dia-a-dia de trabalho, além de alguns documentos importantes na contextualização do trabalho desse profissional. Procedi a apresentação das atividades dos enfermeiros, estabelecendo a trama entre eles e deles com os demais profissionais da saúde. Observei uma grande concentração de suas tarefas em torno dos fazeres burocráticos, além de ver evidenciada a figura do paciente como intercorrências a serem resolvidas, denotando a visão que têm do doente mental. Na convivência com os outros profissionais, vêm-se como centro, apesar de essa visão nem sempre ser compartilhada por esses. Pelos dirigentes, o enfermeiro é visto como o grande contornador de situações, o que o coloca em uma posição politicamente conveniente na manutenção dos interesses majoritários. Nesse hospital, o profissional enfermeiro ocupa diversas funções hierarquicamente estratégicas junto à Diretoria, com a qual mantém contato direto e diário. As condições em que vivem os pacientes da instituição são extremamente precárias e degradantes para qualquer ser humano. A falta de prioridade dada à política de saúde mental no Estado contribui fortemente para a manutenção do modelo assistencial tradicional, ainda implementado nesse hospital.
The following study is the result of a case report held at a public pychiatric hospital of a city in the State of Bahia-Brazil. We focus on the development of nurses’ activities in the institutional routine and on their ways of dealing with situations and social relations with the other health professionals working at that hospital. Being a profesor in the Psychiatric Nursing field at Universidade Estadual de Feira de Santana (UEFS), I have direct contact at that hospital due to nursing students’ training (as a result of a partnership between the State Health Bureau and UEFS). We have mode a field sampling by taking one male, in-patient ward. We have used, as data-collecting strategy: participative observation, semi-structured questionaires applied to the nurses and other staff members who share daily work with them, and some documents which are important for contextualizing the work of such profesionals. We have presented the nursing tasks by establising their connections among nurses themselves and between nursing and other health profesionals. We have found great concentration of burocratic work among their tasks. Besides, we have found evidence of patients being considered medical problems to be solved, which expresses nurses’ views of the mentally-ill. In the staff daily routine, nurses considered themselves as being the core. However, this issue was not so evident when we looked upon the other profesionals’ views. Administrators considered nurses as being great situation managers. This places nurses at a politically-convenient situation for keeping majoritative interests. At this hospital, nurses have various hierarquically estrategic functions, close to the Board of Directors and having direct contact with them, daily. The living conditions of the patients in such institution are extremely poor and humiliating for any human being. The lack of priority given to mental health policies in this State of Bahia greatly contributes for the traditional assistential model, still playing at such hospital.

A saúde mental e mais concretamente o campo das doenças mentais e psiquiátricas é com toda a certeza uma das áreas da saúde que mais alterações tem produzido ao longo das últimas décadas. Assim, e se é comummente aceite que a saúde mental é um bem a preservar, sendo considerada uma prioridade da saúde pública dado o seu inestimável valor, as doenças são contingentes da condição humana, com pesadas implicações para os indivíduos e para a sociedade (OMS, 2001). O presente relatório visa responder às exigências regulamentares do Curso de Mestrado em Enfermagem – Ramo de Enfermagem de Saúde Mental e Psiquiátrica relatando as incidências da realização de um Estágio Final. O referido Estágio foi desenvolvido no Departamento de Psiquiatria e Saúde Mental da Unidade Local de Saúde do Norte Alentejano, EPE e teve como temática avaliar o nível de literacia em saúde mental dos enfermeiros cuidados gerais. O referido projeto propunha-se partir de uma avaliação tomando como instrumento de colheita o questionário QualisMental (Questionário de Avaliação da Literacia em Saúde Mental, versão adaptada e autorizada para Portugal do National Survey of Mental Health Literacy in Young People –Interview version) e mediante os resultados obtidos estruturar uma proposta de consultoria em Enfermagem de Saúde Mental e Psiquiátrica de ligação numa Unidade Local de Saúde, na esfera de competências do Enfermeiro Especialista. Não foi possível concretizar o referido projecto na sua totalidade já que as autorizações do Conselho de Administração da ULSNA não foram despachadas até à data da conclusão do Estágio. No entanto não quisemos deixar de apresentar a referida proposta. Este documento pretende, ainda, analisar o processo de aquisição de competências de Mestre e de desenvolvimento de competências de Enfermeiro Especialista em Enfermagem de Saúde Mental e Psiquiátrica; ABSTRACT: Mental Health literacy of General Care Nurses The mental health, and the field of mental diseases is, without a doubt, one of health areas that had more changes in the last decades. Therefore, if its well known that the mental health is a value to preserve, being considered one priority in the public health due to its priceless value, the diseases are contingents in the human condition, with heavy implications to the individuals and the society (OMS, 2001). This report aims to respond to the regulatory requirements of the Master Course in Nursing - Mental and Psychiatric Nursing Health reporting the incidences of the completion of a Final Stage. This internship was developed in the department of psychiatrics and mental health of the Local health of North Alentejo and had as thematic, to evaluate the level of the literacy in mental health of the nurses of global care. This project proposed starting from an evaluation using the “QualisMental” questionnaire (Questionnaire for Assessment of Mental Health Literacy, adapted and authorized for Portugal from the National Survey of Mental Health Literacy in Young People - Interview version) and using the results obtained structuring a proposal of Consulting in Mental Health and Psychiatric Nursing in a Local Health Unit, within the competence of the Specialist Nurse. This project could not be fully implemented since the authorizations of the Board of Directors were not dispatched until the public discussion of this Report. This document intends to analyze the process of skills acquisition of master and development of skills of the specialist nurse, in mental health and psychiatrics nursing.

Abstract:
Psychiatric inpatient services provide important care for individuals with serious mental health problems. Studies show that passivity and social disengagement prevail in inpatient settings, and the transition to outpatient care is associated with increased suicide risk. Behavioural Activation is an intervention that targets depression by increasing personally meaningful activities. Preliminary research shows that Behavioural Activation can be used in inpatient settings. The overall aim of this thesis was to evaluate the feasibility and effectiveness of Behavioural Activation for individuals in psychiatric inpatient settings, and in the transition between inpatient and outpatient care. Study I investigated inpatient activities and associated experiences. Study II was a pilot single-case experimental study of the feasibility and effectiveness of inpatient Behavioural Activation. Study III, was an interrupted time series evaluation of nursing-adapted Behavioural Activation across three wards. In Study IV Behavioural Activation in the transition from inpatient to outpatient care was compared to Supportive Therapy in a randomised controlled trial with 64 participants. The primary outcome was that of self-reported depressive symptoms and participants were followed up 12 months after treatment completion. Doing nothing was the most common inpatient activity, along with meal related activities. Passive and solitary activities were associated with negative distress and reward profiles. The preliminary evaluation of Behavioural Activation found high patient and staff satisfaction, and four of six participants showed improvement in depressive symptoms and functioning. After nursing-adapted Behavioural Activation was implemented on three wards, engagement increased. Avoidance decreased but later returned to baseline levels. Depressive symptoms and global clinical severity did not improve after nursing-adapted Behavioural Activations was introduced. The randomised controlled trial found that adding Behavioural Activation in the transition to outpatient care had a small, short-term, advantage over Supportive Therapy for self-reported depression. In conclusion, inpatient disengagement is associated with distress, and Behavioural Activation is a feasible intervention in inpatient settings that can be used by both trained therapists and nursing staff to increase patients’ treatment engagement. Behavioural Activation seems useful in targeting depressive symptoms in the transition from inpatient to outpatient care, a period associated with increased risk of suicide and clinical deterioration.

Abstract:
The prevalence of mental health disorders in the United States is estimated at 1 in 5 persons in any given year, with a lifetime prevalence of approximately 50% (National Institute of Mental Health, 2016a, 2016b). Despite scientific progress towards effective behavioral and psychopharmacological treatment, nearly 50% of Americans with mental illness do not receive treatment (Substance Abuse and Mental Health Services Administration [SAMHSA], 2014). This project will identify key geographical and logistical factors restricting recruitment and retention of psychiatric providers (MD or NP) for an underserved region in California. The results of the study will be used to develop a model for effective partnerships aimed at a “grow your own” approach to addressing the shortage of psychiatric providers. In California, the Mental Health Services Act (MHSA) was approved by the voters as Proposition 63 in 2004, funding expansion of mental health resources and specifically, workforce development (California Department of Health Care Services [DHCS], 2017). National funding also supports the expansion of PMHNP training and capacity through Health Resources and Service Administration (HRSA) and SAMHSA. Despite administrative and funding resources from MHSA and HRSA, the shortage of psychiatric providers continues in rural and remote areas. While few national studies have included nurse practitioners in the findings, a growing body of evidence suggests that nurse practitioners can serve similar if not the same function as physician colleagues (DiCicco-Bloom & Cunningham, 2014; National Governors Association, 2012; Newhouse et al, 2011). Barriers to mental health services in rural communities include challenges around a) availability, b) accessibility, c) affordability, and d) acceptability (Wilson, Bangs, & Hatting, 2015). Using a descriptive design, this project explores these factors necessary for patient access to psychiatric services in rural areas. A logic model is used to create a summary of the findings, which will be used to propose a clinical training partnership between an urban university-based PMHNP education and a rural clinic-based training to be completed upon conclusion of the project.

Professional nurses, with additional training in mental health, report attitudes and perceptions of mental health nursing that are more positive, whilst those with less training report more negative attitudes and perceptions to mental health nursing. The primary aim of this research study was to describe the attitudes and perceptions of professional nurses towards the mentally ill in a psychiatric hospital in the Cape Metropole. The objectives of the study were: to explore the attitudes and perceptions of professional nurses towards the mentally ill; to identify common factors that influence the professional nurses’ attitudes and perceptions towards the mentally ill; to compare the attitudes and perceptions of professional nurses who have completed the Regulation 425, Regulation 808 and Regulation 212 training in mental health nursing towards the mentally ill. A quantitative, exploratory, descriptive design was employed and cross-sectional survey was carried out. Participants comprised all permanent professional nurses (n=60) at a governmental Associated Psychiatric Hospital in the Cape Metropole. Participants completed a demographic questionnaire and two self-report questionnaires, measuring attitudes to and perceptions of mental health problems. Nurses with a diploma report significantly higher role competency than those nurses with a degree. The ethnicity of nurses played a role in the stereotyping of the mentally ill. No significant differences were evident between those professional nurses who had completed the advanced mental health course and those whom had not. However, the combined effects of learning the appropriate course and experience in the practical field of the mentally ill are necessary for the task of impacting positively on the attitudes of the nurses towards the mentally ill.

Abstract:
Background: High-fidelity simulation recreates real-life situations in a safe learning environment and encourages critical thinking in students. Published research in simulation in psychiatric/mental health nursing is sparse. Methods: Four scenarios exemplifying drug or alcohol abuse utilizing the computerized, mannequin SimMan® were implemented. Students evaluated their learning experience following completion of the simulation via a 20-item, Likert-scale survey which included open-ended questions. Results: Results were positive. Students rated all items on the survey as “agree” or “strongly agree” (Mean 4.77, SD=0.55). Conclusions: High fidelity clinical education simulations are an effective means of facilitating student learning of psychiatric and mental health clinical experiences. Students found simulation to be a useful and engaging means by which to learn to care for clients with drug or alcohol abuse disorders.

Abstract:
Muito embora os resultados de muitos estudos qualitativos brasileiros sobre o processo de aprendizagem em saúde mental tenham agregado conhecimento, outros são necessários utilizando métodos de estudo menos subjetivos e menos sujeitos aos vieses implícitos no processo de formação acadêmica. Este estudo visa avaliar as propriedades psicométricas do instrumento\"Nursing Students\' Attitudes Toward Mental Health Nursing and Consumers\" para uso entre graduandos de Enfermagem no Brasil. Trata-se de um estudo metodológico de um instrumento de medida de construto subjetivo, oriundo da Austrália, o qual foi adaptado culturalmente ao contexto brasileiro para avaliação de suas propriedades, por meio da metodologia padronizada de adaptação cultural de instrumentos psicométricos, análise fatorial exploratória e confirmatória, pelo pacote estatístico Statistical Package for The Social Sciences (SPSS), v.21 e AMOS/SPSS, v.22, respectivamente. Foi aplicado o instrumento original e os domínios Autoritarismo e Visão Minoritária do instrumento ODM, já validado para a realidade brasileira. A coleta de dados foi realizada em 5 cursos de graduação em Enfermagem de 5 instituições públicas de ensino localizadas no interior e capital paulista e paranaense, dentre os anos de 2013 e 2014, totalizando 365 sujeitos. Os sujeitos foram distribuídos em 91,3% do sexo feminino e 8,7% do sexo masculino e sua faixa etária variou de 18 a 58 anos, com uma média(M) de 21,9 anos e um desvio-padrão (dp) de 3,88 anos, a carga horária teórica de aulas variou de 36 a 71 horas/aula, M=52 hs; dp=13,64 e a carga horária prática, variou de 76 a 100 horas/aula, M=91 e dp=5,22. O intervalo de tempo de aplicação inicial e final do instrumento variou em função do cronograma e carga horária de aulas de cada Instituição de Ensino Superior, oscilando entre 29 e 136 dias, M=77,63, dp=39,64. A análise fatorial exploratória resultou numa versão pré-final de 23 itens, com um coeficiente Alfa de Cronbach entre 0,71 a 0,95. Durante a realização da análise fatorial exploratória, doze itens foram retirados, devido a baixa comunalidade e/ou consistência interna. Esta versão não foi considerada válida pelo método de validação convergente e devido às grandes alterações em sua estrutura, não pôde ser considerada a versão validada do instrumento original. Esta versão pré-final foi submetida à modelagem de equações estruturais, resultando na apresentação de um modelo teórico composto por 22 itens distribuídos em 5 domínios, sendo que dois domínios permaneceram idênticos aos originais, um foi modificado e outros dois novos foram acrescentados, tendo sido validados pelo método estatístico usado e com confiabilidade aceitável (Alfa de Cronbach de 0,72 a 0,95). Embora o instrumento original não tenha sido validado no Brasil, acredita-se que o presente estudo contribuiu para a pesquisa e o ensino de Enfermagem em Saúde Mental, por apresentar um instrumento de avaliação da aprendizagem em saúde mental baseado no instrumento original, assim como por apresentar um modelo teórico explicativo para o processo de aprendizagem e da atitude do estudante na área de saúde mental
Although the results of many Brazilians qualitative studies on the learning process in mental health have added knowledge, others are needed using less subjective methods of study and less subject to bias implicit in the academic process. This study aims to evaluate the psychometric properties of the instrument \"Nursing Students\' Attitudes Toward Mental Health Nursing and Consumers\" for use among undergraduate nursing students in Brazil. This is a methodological study of a subjective construct measuring instrument, originally from Australia, which was culturally adapted to the Brazilian context for evaluating its properties through the standardized methodology of cultural adaptation of psychometric instruments, Exploratory and Confirmatory Factor Analysis, by the Statistical Package for The Social Sciences (SPSS) v.21 and AMOS / SPSS, v.22, respectively. The original instrument and Authoritarianism and Minority View fields of the instrument OMI, already validated for the Brazilian reality was applied. Data collection was performed in 5 undergraduate courses in Nursing at public higher education institutions located within and São Paulo and Parana States, from the years 2013 and 2014, totaling 365 subjects. The subjects were divided into 91.3% of females and 8.7% males and their ages ranged from 18 to 58 years, with a mean (M) 21.9 years and a standard deviation ( sd) of 3.88 years, the theoretical component of classes ranged 36-71 hours / class, M = 52 hours; SD = 13.64 and the clinical practice, ranged 76-100 hours / class, M = 91 and SD = 5.22. The initial application time interval and end of the instrument varied depending on the schedule and hours of classes of each higher education institution, ranging between 29 and 136 days, M = 77.63, SD = 39.64. Exploratory factor analysis resulted in a pre-final version of 23 items, with a Cronbach alpha coefficient of between 0.71 to 0.95. While performing the exploratory factor analysis, twelve items were removed due to low commonality and / or internal consistency. This version was not considered valid for convergent validation method and due to major changes in its structure, could not be considered a validated version of the original instrument. This pre-final version was submitted to structural equation modeling, resulting in the presentation of a theoretical model composed of 22 items distributed in five areas, with two areas remained identical to the original one has been modified and two new ones were added, having been validated by the statistical method used and with acceptable reliability (Cronbach\'s alpha 0.72 to 0.95). Although the original instrument has not been validated in Brazil, it is believed that this study contributed to the research and Mental Health Nursing teaching, to present an assessment instrument of mental health learning based on the original instrument, as well as to present an explanatory theoretical model for the process of learning and the student\'s attitude in the mental health area

The incidence of smoking among mentally ill people is very high. Smokers have a 50%, rate of mental illness diagnosis compared with 23% rate for general population. To address this problem, the purpose of this project was to plan a smoking cessation program for patients in a mental health facility. The theoretical foundation for this project was based on the theory of planned behavior, which identifies the predictive nature of smoking and the benefits that can be derived from implementing a systematic approach for change. The project question examined the effectiveness of smoking cessation program using educational support, pharmacological strategies, and bi-weekly meetings to help patients in a mental health hospital to decrease smoking behavior. The project design was based on use of smoking questionnaires, the Hooked on Nicotine Checklist (HONC), effective pharmacological strategies, educational support, and counseling treatments to evaluate symptoms of dependency. The key results of this project included the creation of a plan that could foster reduction in illness, improved quality of life, and reduced costs related to the onset of major illness in this vulnerable population. This data collection process focused on a qualitative design in which selected professionals were asked to review the materials and answer questions. This project could increase awareness of the issue of smoking; in addition, this project could equip nurses with the tools to deliver evidence based interventions for tobacco dependence that may significantly reduce tobacco use. This project has the implications for positive social change through its potential to improve the health of people with mental illnesses. It also creates a safe and healthy environment in mental health facilities for patients who do not smoke.

The South African health care system shifted the focus of treating psychiatric disorders from institutional care level mental health services to facilitate this process of integration into the Primary Health Care (PHC) settings. All the provinces were thus engaged in improving mental health care services at community level by providing training for professional nurses in mental health at PHC settings. Consequently, mental health nursing has also changed considerably by shifting the focus of mental health care to the primary care level. It is however, suggested that the current revolving door syndrome experienced at psychiatric institutions was partly due to inadequate community-based psychiatric services. It was also suggested that the attitudes and knowledge of health professionals towards mental illness has a major impact on service delivery, treatment and outcome of mental illness. The aim of this research study was to assess the attitude of nursing staff working at a Community Health Centre (CHC) towards the mental health care user. A CHC was chosen that renders 24 hour services. The inclusive sample included all the different categories of nurses permanently employed at this CHC. The Attitude Scale for Mental Illness questionnaire was used to collect the data. Descriptive statistics: means, median and standard deviations were calculated for the following variables: separatism; stereotyping; restrictiveness; benevolence; pessimistic prediction and stigmatization. In conclusion it can be said that the nursing staff with more experience irrespective of category of nurse has less of a stereotyping attitude towards mental illness. The longer the nurse worked at the setting and irrespective of their nursing qualification the more positive their attitude towards the MHCU became.

Este trabalho teve como objetivo buscar conhecera visão de auxiliares de enfermagem de um setor de agudos masculino e feminino de um hospital especializado em psiquiatria do interior do estado de são paulo a respeito de seu trabalho. mais especificamente, pretende-se conhecer, a partir de sua própria perspectiva, a trajetória e formação profissional dessas pessoas, o contexto institucional e as práticas cotidianas envolvidas em seu trabalho e suas concepções a respeito da clientela atendida. Foram entrevistados através da \"história de vida temática\" onze auxiliares de enfermagem. Essas entrevistas foram gravadas e transcritas na íntegra, e analisadas qualitativamente. A análise permitiu apontar os seguintes temas: suas trajetórias de vida, influências recíprocas entre a vida pessoal e do trabalho, a formação dos profissionais,o perfil do auxiliar, a rotina de trabalho e seus apoios a visão sobre o paciente psiquiátrico, sua família. Os relatos convergiram para a apresentação da maneira como se configura o trabalho do auxiliar de enfermagem, quanto às suas visões e condições para o trabalho cotidiano Considera-se que é importante promover a criação de espaços de reflexão, discussão e escuta entre os profissionais de saúde mental, dentro dos setting de trabalho, integrando principalmente os que lidam diretamente com o paciente, de modo que nesses espaços se possa conversar sobre questões da prática diária específicas, mas também transcender a elas; relacionando essa prática com repercussões da vida pessoal no trabalho, promovendo apoio ao trabalhador.
It\'s well known that the mental health knowledge field is complex and inter sectorial. Many studies about the mental health patient, in Brazil, have revealed that although there was a change in the psychiatric treatment, still there is patients rights violations, and the feeling in the attention group is of certain hopelessness and lack of knowledge in dealing with the patient. This work aims to explore a masculine and feminine acute unit nursing personnel, in a psychiatry specialized hospital in the inner State of São Paulo, from the point of view of their work. More specifically, it\'s intended to know, from their own perspective their professional pathways and education, the institutional context and their everyday practices concerning work and clients. Eleven nursing professionals were interviewed through \"thematic life history\" interview. Those interviews were taped and integrally transcript and analyzed qualitatively. The analysis showed their life pathways, influences of their personal lives on work, professional education, work routine, support on psychiatric patients and the patient\'s family concepts. It\'s considered that it\'s important to promote reflection, discussion and hearing spaces among mental health professionals, in the work setting, with those who deal directly with the patient and the whole team so it\'s possible to discuss on every day practices, transcend to them relating to personal life repercussions, promoting support for work.

Abstract:
Violence in mental health care continues to be a problem. The incidence of violent episodes in healthcare settings with aggressive behavior of patients aimed at staff members or other patients is almost four times greater in healthcare than in other industries. Reducing violent episodes enhances the quality of care and improves safety for staff members and for patients. The project focused on development of a staff education program exploring the practice-focused question: Will this program effectively guide staff member approaches to mental health patients with challenging behaviors? The purpose of this project was to address the identified gap in practice in one mental health unit at a Florida correctional facility. A comprehensive literature review was completed using 30 sources from 2012-2017 and included peer-reviewed research and government resources to guide the development of this program, called TAPOUT. Sources of evidence emerged from a systematic review of the literature and an expert panel in mental health. Using the Delphi technique, all 5 panel members came to consensus after 2 rounds, agreeing to implement the TAPOUT program. The findings demonstrated the TAPOUT program may effectively guide staff member approaches to mental health patients with challenging behaviors and showed the benefits of using the TAPOUT program for reduction of violence. The DNP project has demonstrated TAPOUT can address the identified practice gap. The educational program's goal was to reduce violence and positively impact social change by providing staff members with tools to prevent and to deescalate emerging violent behaviors and episodes, preventing injury among staff members and patients alike.

A descriptive qualitative research method was used in this study. In this study, five women were asked to describe their perceptions of how effective the Psychiatric Wellness Clinic interventions were in controlling their depression symptoms. With the decreased availability of state funding, the standard form of treatment was no longer available. This Psychiatric Wellness Clinic concept was developed as a mechanism to provide this patient population with skills that would empower them to take care of themselves and decreased dependency on the system. This researcher wanted to see if the clients felt that the program had make them feel less depressed. If so, it would be inferred that the clients were participating in controlling their own mental health well being. The findings in this study indicate that clients do perceive the Psychiatric Wellness Clinic as effective in decreasing depression symptoms.

There are many negative consequences of childhood sexual abuse, (Browne Finkelhor, 1990). These effects do not vanish with adulthood and some adults stay scarred for life. However, it has been noted that some victims of childhood sexual abuse recover better than others regardless of the particular type of abuse. Could this adaptation be related to, or represent "hardiness" of the adult? The purpose of this report is to explore the hardiness of adult victims of child sexual abuse and the adaptation strategies practiced. To achieve this goal, The Health Related Hardiness Scale, created by Susan Pollock, PhD. was adopted. Data was collected from fifteen adult females who, by their own definition were sexually abused as children. They completed the HRHS questionnaire and a demographic questionnaire. Data analysis revealed a significant p-value of 0.0002 indicating that support is an important component to "buffer" the negative effects of stress. However this study revealed a high level of hardiness overall in the participating individuals

People living with a mental health issue is elevated both nationally and internationally, and the likelihood of a person interacting with someone who has a mental health issue is high. It is even higher for those who work in the health care profession, so understanding their attitudes on this matter is important. As a review of current literature reveals, there is not only stigma in the health care system, but there is also stigma among health care professionals and students. One of the groups of students that has not been researched to a great extent is nursing students and the degree of stigma they have for caring for those with mental health issues. A cross-sectional survey was conducted to assess the degree of stigma among these individuals. A voluntary online survey was done was administered to undergraduate nursing students at a university in the southeastern part of the United States using the twenty item Opening Minds Scale for Health Care Providers (OMS-HC). The results indicated that mental illness stigma did exist within the student nurse community with no association between gender, classification, and mental health nursing course completion and total score on the OMS-HC scale.

It is widely supported that there are significant, positive relationships between the occurrence of some mental health symptoms and physical illnesses. Research indicates that the burden experienced by those with a physical and mental illness are magnified compared to individuals who do not experience an illness. More specifically, one of the burdens experienced by individuals is the monetary burden of affording the necessary health services to properly manage their illness. This study attempts to reveal a difference between mental health symptom count for individuals who do and do not experience difficulty affording health care for their physical problems. The first hypothesis states that the indication of medical problems will be associated with greater mental health symptoms. The second hypothesis states that the relationship between medical concerns and mental health symptoms will be moderated by difficulty affording health services. Lastly, the third hypothesis states that the indication of unhealthy behaviors, such as smoking and irregular exercise, will be associated with greater mental health symptoms. All data used in this study is archived data that was gathered by the Institute for Rural Health’s Mobile Health Units during free community health fairs from September 2012 to February 2014. The first hypothesis was supported because the incidence rate of mental health symptom count was increased with the presence of some physical problems. These findings supported previous research that indicated that the presence of physical illness increases the chance of developing a mental illness. The study results revealed that the second hypothesis was not supported since difficulty affording health services did not have a significant effect on the relationship between indicated physical illnesses and mental health symptom count. Previous research reports that there is monetary burden for individuals who experience a physical or mental illness when accessing appropriate health services. This research aimed to explore if that burden would significantly affect the relationship of those illnesses. Lastly, the third hypothesis was supported because mental health symptom count incidence rate was found to increase for individuals partaking in negative health behaviors, such as smoking, and decrease for individuals partaking in positive health behaviors, such as exercising.

The aim of this study was to determine the ability and proficiency of the newly qualified professional nurses in utilising psychiatric nursing skills at Chris Hani District Psychiatric Health Care Services in the Eastern Cape, South Africa. The research method was a qualitative, phenomenological approach. A purposive sample of newly qualified professional nurses and supervisors participated in the study. The data were collected through interviews and focus group discussions, using semi – structured interview guides. Interviews responses were recorded on the interview guide. Data were analysed using the computer software Atlas ti and manually. Positive and negative themes were identified. Ethical considerations were ensured by means of privacy, anonymity and confidentiality. Ethical clearance was obtained from the University of Fort Hare and permission from the Provincial Department of Health and Chris Hani District Health and Mental Health Institution at Queenstown was sought. The findings revealed that newly qualified professional nurses performed well, according to the perception of their supervisors. Newly qualified professional nurses were faced with challenges such as shortage of resources, mental illness not considered as a priority and lack of skills development. The study also revealed that newly qualified professional nurses focussed on the curative aspect of ailments, rather than preventive care. However community health workers (CHW) focussed on the preventive aspect of care. Limitations and recommendations based on the findings of the study are presented.

Psychiatric illnesses can sometimes lead to behavioral outbursts that need to be addressed quickly to deescalate potentially explosive situations. Nurses are in a unique position to respond to such outbursts by calling for a rapid response team. Nurses who are part of the rapid response team should be well-informed of their roles and responsibilities in managing aggressive and violent behavior. The purpose of this project was to explore RN's and LPN's knowledge and perceptions of a rapid response team in a psychiatric facility. The Iowa model of evidence-based practice provided the framework to integrate theory into practice to improve care. A quantitative descriptive design was implemented with a convenience sample of nurses using a 4-part questionnaire. Of the 64 surveys distributed on 5 wards, 59 were completed for a response rate of 92%. Descriptive statistics were used to analyze nurse responses to demographic data and background data. A Chi-square statistic was calculated to investigate the relationship between RN and LPN responses to the Likert Agreement Scale; no significant difference in responses was found. Open-ended questions allowed nurses to comment on their role and position during a code. The comments were sorted into categories of reoccurring themes. Results suggested that nurses need to understand signs of behavioral escalation and strategies to deescalate a potentially volatile patient. Nurses commented that knowledge during a code, reasons for calling a code, and good communication skills are essential in code situations. Findings from this project can benefit nurses who work psychiatric emergencies by underscoring the need to development of psychiatric rapid response teams and to update current standards of inpatient care.

Length of hospital stay (LOS) is a major indicator for measuring efficient care. Intellectually disabled psychiatric patients (IDPP) with chronic medical problems have longer LOS due to challenges faced by providers in jointly managing both psychiatric and medical problems. The purposes of this study were to understand the significance of LOS for IDPP, create an intervention toolkit to reduce LOS, establish the content validity of the toolkit, and recommend its implementation. The items of the toolkit are pharmacology, somatic, rehabilitation, psychosocial, and monitoring patients' psychiatric and medical symptoms across care domains. The toolkit was created from the constructs of the psychiatric rehabilitation process model to jointly mange psychiatric and medical issues. The project question asked if a universal agreement rating will be achieved to establish content validity of the toolkit. Orem's self-care deficit theory was used to guide this study. Ten experts with experience in the clinical, financial, legal, and psycho-social aspects of IDPP care, were recruited from 5 county facilities and asked to participate in the study. The inclusion criteria focused on the experts' leadership roles in those facilities. The experts answered two online quantitative surveys. Survey 1 asked 9 questions and elicited opinions on LOS issues for IDPP. Survey 2 asked the experts to rate the efficacy of the toolkit to reduce LOS for IDPP. Survey 1 finding showed that 8 of 10 experts agreed that LOS for IDPP needed to be reduced. Survey 2 finding showed a universal agreement toolkit rating of 0.84, indicating the experts' readiness to adopt the toolkit to reduce LOS for IDPP. This study has the potential to promote social change by enhancing interdisciplinary and collaborative use of best care processes in psychiatry to reduce LOS and jointly manage psychiatric and medical problems affecting IDPP.

This study sought to explore the perceptions of mental health care practitioners’ perceptions on mental illness within the isiXhosa cultural context. A qualitative exploratory descriptive and contextual design was used for the study. A non-probability purposive sampling method was used to select eight participants from Komani Hospital in Queenstown. Data was collected through semi-structured interviews. The services of an Independent Interviewer were used to avoid any bias as interviews took place where the researcher is employed. All interviews were transcribed verbatim and the data collected was analyzed according to Tesch’s eight steps of data analysis as described in Cresswell (1994:155). The researcher utilized services of an Independent Coder who verified the identified major themes. Four major themes emerged from the analysis of the interview: Mental health care practitioner’s perceptions of mental illness, perception of the causes of mental illness within the isiXhosa cultural context, mental health care practitioners’ views in the management and treatment of mental illness and suggestions put forward to improve the services to mental health care users. The major findings of this study were the lack of knowledge of culture of mental health care users. It is recommended that it would be proper if there could be co-operation between mental health care practitioners and traditional healers by working together as a team.

Recent publicity has focused on the problems created by the usage of illicit drugs in the community. The growing use of illicit drugs throughout the Grampians region and the lack of resources and professional services available to regional and rural areas raise many questions as to treatment options and the accessibility and appropriateness of drug and alcohol and mental health services. Despite the fact that mental health professionals in rural/regional areas are expected to deliver the most appropriate care to individuals with a comorbid drug and alcohol and psychiatric disorder, a number of these rural/regional mental health professionals have limited preparation and experience in dealing with dual diagnosis issues. This phenomenological study focuses on the area of dual diagnosis, specifically the experiences of health professionals who care for clients diagnosed with a serious mental illness and a coexisting drug and alcohol disorder. Results are described in the form of four themes, which emerged from data collected during in-depth interviews with 13 mental health professionals who care for clients with a dual diagnosis. The themes captured in this research will be described using metaphors as headings. The first theme Sink or swim represents mental health professionals’ initial preparation to care for this group of complex clientele. Treading water symbolises mental health professionals’ endeavours to keep their head above water and reflects on their feelings while endeavouring to do so. Rowing against the tide describes mental health professionals’ understanding of clients’ drug misuse, which impacts greatly on the level of care.

Estudo de carÃter descritivo, exploratÃrio e documental, com abordagem quali-quantitativa, realizado com graduandos do sexto, sÃtimo e oitavo semestres do curso de Enfermagem da Universidade Federal do Cearà (UFC) em Dez/ 2013, em que foi utilizado um questionÃrio semiestruturado e entrevista, com dados sociodemogrÃficos e de competÃncias.Os dados quantitativos foram organizados em tabelas as entrevistas em quadros. As associaÃÃes entre as competÃncias e o grupo foram analisadas por meio dos testes de X2 e de razÃo de verossimilhanÃa. Para todas as anÃlises, o valor de p<0,05 foi considerado estatisticamente significante. A entrevista foi realizada com oito alunas do oitavo semestre e analisada de acordo com a produÃÃo de sentidos de Spink, fundamentada com a literatura pertinente. Os resultados coadunam com outras realidades encontradas em outros estudos sobre o ensino de Enfermagem em SaÃde Mental e Psiquiatria, em que medo e estigma estÃo presentes, influenciando no processo aprendizagem dos alunos. Em relaÃÃo Ãs competÃncias de SaÃde Mental, muitos se sentem deficientes, principalmente naquelas que sÃo bÃsicas para a formaÃÃo do Enfermeiro, como o reconhecimento das diretrizes da polÃtica de SaÃde Mental; aplicar assistÃncia de Enfermagem em SaÃde Mental no contexto familiar e comunitÃrio; caracterizar transtornos mentais; atuar nos diversos nÃveis de atenÃÃo à saÃde mental; e lidar com o paciente em crise de transtorno mental. Consideraram tambÃm o ensino, a didÃtica, a carga horÃria e os campos de estÃgios em SaÃde Mental muitas vezes insuficiente. Nesta perspectiva, faz-se necessÃrio avaliar estas competÃncias que estÃo falhas, assim como outros aspectos para desconstruÃÃo do que à negativo e construÃÃo daquilo que à necessÃrio para o processo de ensino dos docentes que permeiam nas disciplinas, perpetuando a ideia que a SaÃde Mental à peculiar a todos os ciclos de vida e transversal Ãs disciplinas.
This study aimed to analyze the skills and nursing skills in mental health identified by scholars, based on the National Curriculum Guidelines for Nursing (DCN / ENF) and Psychiatric Reform. Descriptive study, exploratory and documentary, with qualitative and quantitative approach, conducted with graduate students from the sixth, seventh and eighth semesters of the nursing course of the Federal University of Cearà (UFC) in Dec / 2013, in which we used a semi-structured questionnaire and interview with sociodemographic and competÃncias.Os quantitative data data were organized in tables interviews in frames. Associations between the skills and the group were analyzed using the X2 test and likelihood ratio. For all analyzes, the p value <0.05 was considered statistically significant. The interview was conducted with eight students of the eighth semester and analyzed according to the production Spink directions, substantiated by the literature. The results are in line with other realities found in other studies on nursing education in Mental Health and Psychiatry, in which fear and stigma are present, influencing the learning process of students. With regard to mental health skills, many are disabled, especially those that are basic to the formation of the nurse, as recognition of the guidelines of the Mental Health policy; apply Mental Health Nursing care in the family and community context; characterize mental disorders; acting at different levels of mental health care; and deal with the patient in a mental illness crisis. Also considered teaching, teaching, workload and field placements in mental health often insufficient. In this perspective, it is necessary to evaluate these skills that are failures, as well as other aspects of deconstruction of what is negative and construction of what is needed for the teaching process of teachers that permeate the subjects, perpetuating the idea that mental health is peculiar to all life cycles and cross the disciplines.

Genom att tidigare forskning visade attsjuksköterskor känner sig obekväma trots att biverkningsprofilen är känd, väcktesett intresse att undersöka vidare hur och varför samtal om sexuella biverkningar iomvårdnaden verkar hamna längre ner i prioriteringen.Syfte: Syftet var att genom intervjuer undersöka sjuksköterskors erfarenheter avatt samtala med psykiatriska patienter om sexuella biverkningar vid behandlingmed antidepressiva läkemedel.Metod: Efter godkänd etikansökan från Malmö Universitet utformandes ensemistrukturerad intervjuguide, både allmänsjuksköterskor samt psykiatrisjuksköterskordeltog. Tio intervjuer utfördes på informanternas arbetsplats.Intervjuerna spelades in digitalt och varade mellan 15 och 30 minuter.Transkribering gjordes efter varje intervju. Texten analyserades enligt enmodifierad version av Burnard (1991).Resultat: Följande kategorier identifierades i resultatet: Att samtala om sexuellabiverkningar, att möta patienten och att undvika frågan. Resultatet påvisade att deflesta sjuksköterskor inte samtalar om sexuella biverkningar. Delvis på grund avde anser det vara pinsamt, men även på grund av de anser sig besitta begränsadkunskap inom området.Diskussion: Sjuksköterskorna var i behov av ökad kunskap och utbildning för attde skulle känna sig trygga att samtala om sexuella biverkningar med patienterna.En etablerad relation visade sig göra ämnet enklare att samtala om, dock fanns detofta ett hinder för detta, vilket var tidsbrist. Statisticsfrom the National Board of Health's Drugs database from 2017 show that 632,000people in Sweden are being treated with antidepressants. Studies show thatalthough the patient expects healthcare professionals to inform about drug sideeffects such as sexual side effects, it is not done. Because previous researchshowed that nurses feel uncomfortable even though the side effects profile isknown, an interest was raised to further investigate how and why discussingsexual side effects seems to be less prioritized in nursing.Aim: The aim was to investigate, through interviews, the nurse's experience oftalking with psychiatric patients about sexual side effects in treatment withantidepressant drugs.Method: After an approved ethics application from Malmö University, a semistructuredinterview guide was drafted involving both general nurses andpsychiatric nurses. Ten interviews were conducted and lasted between 15-30minutes and were recorded with mobile phones. The interviews were conducted atthe informants' workplace at a time chosen by the informant. Each interview wastranscribed, and the analysis was made according to a modified version ofBurnard’s method (1991).Result: The following categories were identified in the result: To talk about sexualside effects, to meet the patient and to avoid the question. The result showed thatmost nurses do not talk about sexual side effects. Partly because they consider itembarrassing, but also because they consider themselves possessing limitedknowledge in the area.Discussion: Nurses needed to increase their knowledge and education to makethem feel assured when talking about sexual side effects with patients. Anestablished relationship made the topic easier to talk about, but time shortage wasan obstacle to this. Something that also proved to be problematic was the nurseslack of routines for conversations about sexual side effects.

Os serviços extra-hospitalares de saúde mental têm sua origem a partir das iniciativas advindas das propostas de reformulação da atenção psiquiátrica decorrentes dos Movimentos de Reforma Psiquiátrica brasileira. Esses serviços nascem com o propósito de substituírem o modelo que fundamenta um modo de atenção psiquiátrica apoiado no tratamento prioritariamente hospitalar e medicocentrado pelo paradigma da atenção psicossocial. Nesse percurso alguns problemas se apresentam, dificultando a plena construção e implementação do modo de atenção psicossocial e favorecendo a reprodução de aspectos da forma de atenção que se pretende superar nos serviços substitutivos ao manterem as ações terapêuticas disponibilizadas centradas nos tratamentos médico-clínicos, ao mesmo tempo em que secundarizam ações de reabilitação psicossocial. Com a proposta de buscar uma aproximação à problemática exposta, este estudo teve como objetivos analisar a organização dos projetos terapêuticos em dois serviços extra-hospitalares de saúde mental do município de Ribeirão Preto e como se localizam nesses projetos as ações de reabilitação psicossocial, conhecer a forma pela qual os profissionais elaboram o projeto terapêutico e interpretar as concepções dos mesmos em relação à reabilitação psicossocial. O estudo foi efetuado com base em pesquisa cuja metodologia pautou-se em abordagem qualitativa e as técnicas de coleta de dados escolhidas foram entrevistas semiestruturadas e grupos focais. O referencial teórico utilizado foi a filosofia hermenêutica dialética de Jürgen Habermas. Os eixos temáticos obtidos a partir da análise dos dados se dispuseram com base nos tópicos da organização do trabalho refletida na extrema tecnificação das ações de saúde, na centralidade da tecnologia médico-clínica em relação a outras ações terapêuticas não médicas e no processo de precarização das relações de trabalho presentes nesses serviços. Quanto ao tópico da organização dos serviços, observa-se que os projetos terapêuticos não têm sido elaborados dialogicamente pela equipe de profissionais; as equipes não desenvolveram a interdisciplinaridade em sua forma de organização e as ações terapêuticas têm priorizado os tratamentos com ênfase na patologia e na dimensão biológica. No que diz respeito às concepções sobre a reabilitação psicossocial os profissionais mantêm uma racionalidade instrumental definindo as ações de saúde nos serviços em detrimento de uma racionalidade prática como base das ações de reabilitação psicossocial, organizadas segundo o paradigma da atenção psicossocial.Com base nessa análise propõe-se como caminho de superação da problemática apontada a possibilidade de reunir ações clínicas e de reabilitação psicossocial em projetos terapêuticos que confiram o mesmo valor e investimento a ambas as ações e a realização de pesquisa avaliativa participativa nos serviços buscando criar mecanismos que favoreçam, além da melhoria da assistência prestada, projetos terapêuticos com base no modo de atenção psicossocial.
The extra-hospital services originated in the proposals of psychiatric care reformulation that resulted from the Brazilian Psychiatric Reform. These services emerged with the purpose to replace the psychiatric care model grounded treatment delivered mainly in hospitals and centered on medication according to the psychosocial care paradigm. Some problems are presented in this process, which hinder the full construction and implementation of the psychosocial care mode, favoring the reproduction of aspects of the care mode that are aimed to be overcome in the replacing services: the available therapeutic actions are centered on medical-clinical treatments while psychosocial rehabilitation actions are considered secondary. Aiming to understand the problem, this study analyzed the organization of therapeutic projects in two extra-hospital services in Ribeirão Preto, SP, Brazil, verified how professionals develop the therapeutic project, and interpreted these professionals\' conceptions in relation to psychosocial rehabilitation. This study was based on a qualitative approach and semi-structured interviews and focal groups were used to collect data. The hermeneutic dialectic philosophy of Jürgen Habermas was the framework used. The thematic axes that emerged from data analysis were based on work organization reflected on extremely technical health actions, centered on medical-clinical technology in relation to other non-medical therapeutic actions, and on the process in which work relations are worsened in these services. In regard to the services organization, we observed that the therapeutic projects are not dialogically developed by the team; teams have not developed interdisciplinary work in their organization and therapeutic actions give priority to treatments that emphasize the pathology and its biological dimension. In relation to the professionals\' conceptions about psychosocial rehabilitation, they maintain an instrumental rationality that defines the health actions in the services to the detriment of a practical rationality that ground psychosocial rehabilitation actions organized according to the psychosocial care paradigm. Based on this analysis, we suggest the implementation of clinical and psychosocial rehabilitation actions in the therapeutic projects that attribute the same value and investment to the both in order to overcome the studied problem. Additionally, participatory evaluation research should be carried out in the services with the purpose to create mechanisms that favor therapeutic projects and improve care delivery based on the psychosocial care mode.

Psychiatric illnesses can sometimes lead to behavioral outbursts that need to be addressed quickly to deescalate potentially explosive situations. Nurses are in a unique position to respond to such outbursts by calling for a rapid response team. Nurses who are part of the rapid response team should be well-informed of their roles and responsibilities in managing aggressive and violent behavior. The purpose of this project was to explore RN's and LPN's knowledge and perceptions of a rapid response team in a psychiatric facility. The Iowa model of evidence-based practice provided the framework to integrate theory into practice to improve care. A quantitative descriptive design was implemented with a convenience sample of nurses using a 4-part questionnaire. Of the 64 surveys distributed on 5 wards, 59 were completed for a response rate of 92%. Descriptive statistics were used to analyze nurse responses to demographic data and background data. A Chi-square statistic was calculated to investigate the relationship between RN and LPN responses to the Likert Agreement Scale; no significant difference in responses was found. Open-ended questions allowed nurses to comment on their role and position during a code. The comments were sorted into categories of reoccurring themes. Results suggested that nurses need to understand signs of behavioral escalation and strategies to deescalate a potentially volatile patient. Nurses commented that knowledge during a code, reasons for calling a code, and good communication skills are essential in code situations. Findings from this project can benefit nurses who work psychiatric emergencies by underscoring the need to development of psychiatric rapid response teams and to update current standards of inpatient care.

Engaging in physical activity can help older adults to take part in community activities, maintain relationships, and initiate new friendships, thus preventing loneliness and depression. The purpose of this quality improvement (QI) project was to evaluate whether participation in physical activity improved depressive symptoms in 15 older adults at a local senior center. Pender’s health promotion model was used to determine nursing and behavioral science views on components that affect health behavior. The Exercise Benefit/Barrier Scale (EBBS) survey was evaluated before implementation of the walking program to measure the benefits of and barriers to exercise. The EBBS results showed that participants perceived physical activity as beneficial with high percentages in the dimensions of physical performance (90%) and psychosocial aspects (76%). The leading barrier to exercise was fatigue (50%). The 15 elderly participants tracked physical activity by counting the number of steps a day over a 2-month period with a mean number of steps of 3,788/day. The Patient Health Questionnaire-2 (PHQ-2) was administered before and after implementation of physical activity tracking to measure changes in mood and sadness over the past 2 weeks. Forty-seven percent of the participants had depressive symptoms before the program, compared to 13% after the program. Using descriptive statistics via percent difference, results revealed a 34% decrease in depressive symptoms in the program participants. Therefore, this QI project was successful in improving depressive symptoms among older adults. This project paves the way for positive social change through improved quality of life and improved physical/mental health outcomes for older adults who participate in a physical walking activity by decreasing depressive symptoms.

The purpose of this study was to investigate the Hardiness level of battered women residing at a South Florida shelter and to investigate if there was a major difference in the hardiness level of women who were first time in a shelter as opposed to those who had been previously in shelters. Using a descriptive design, the Health-Related Hardiness Scale was given to 50 battered women residing at a shelter. The results confirmed our hypothesis that proposed that hardier women would leave their abusive environment. The total hardiness of these women was (M= 148.86 SD= 22.64). Furthermore, no major differences were found among the two groups, the total hardiness for the women who been in shelters before was (M=150.17 SD= 26.06) and for those who were first time users was (M=148.45 SD= 21.81). The results provide a baseline to begin to understand the role that hardiness play in the lives of battered women. These findings and future studies may have implications for breaking the cycle of domestic violence.

Método: Pesquisa de artigos científicos na base de dados Ebsco e recurso à literatura. Análise sistemática e reflexiva de vários indicadores mediante entrevistas, observação e instrumentos de avaliação, tais como: Inventário de Depressão de Beck e Escala de Ansiedade de Hamilton. Recurso a técnicas de relaxamento para reduzir a ansiedade. Resultados: Foram desenvolvidas as competências e as mesmas permitiram a estabilidade do humor e redução da ansiedade da população alvo. Conclusão: O desenvolvimento de competências especializadas promove uma maior qualidade dos cuidados de saúde, diminuindo significativamente o grau de incapacidade que estas perturbações mentais originam; ABSTRACT:Problem: The clinical manifestations of depressive mood in association with anxiety have significant consequences in terms of human suffering, personal costs, family, social and economic. Conclusion: The development of expertise promotes a higher quality of care, reducing significantly the degree of disability that these mental disorders originate.

Abstract:
Acute inpatient mental health nursing staff provide mental health care for individuals when they are most vulnerable and unwell. The therapeutic relationship can facilitate positive changes and recovery for individuals. Therefore, understanding nursing staff’s attitudes is paramount. In this thesis, the attitudes of nursing staff towards those experiencing mental health difficulties was explored. In the systematic review, the attitudes of European nursing staff supporting those experiencing severe mental health difficulties were synthesised and evaluated. A total of 14 cross-sectional studies met the inclusion criteria. The review identified that the role of personal experiences of mental health difficulties through friends or family members was shown to elicit more positive attitudes. Overall attitudes amongst nursing staff were varied, and factors that influenced these attitudes were less clear and consistent. The limitation of using cross-sectional questionnaires to explore attitudes was also discussed. Moreover, recommendations regarding further research priorities as well as clinical implications were identified. Nursing staff’s attitudes towards adults who were either ‘informal’ voluntary clients or those who had been admitted under the Mental Health Act (1983) to an inpatient ward were explored in the empirical paper. The attitudes of nursing staff have the potential to impact on the development of therapeutic relationships and therefore upon treatment outcomes. Repertory grid interviews were completed with twelve nursing staff. All staff made critical judgements about some of their clients; however, staff who used more dimensions to construe clients made less clear distinctions between clients and non-clients. The findings highlight the need for support mechanisms that enable staff to formulate clients’ difficulties and explore the complexity of interactions. The implications of these results are discussed, as well as future research directions. The final paper consists of a critical reflection of the research and the research process. This includes an evaluation of the decision making processes and discussion of the strengths and weaknesses of this research.

The conceptual framework was resiliency, as these participants continued to survive and thrive after the adverse assaultive events. Data were collected using in-depth interviews from purposeful sampling. Registered nurses working in the high risk areas of inpatient psychiatry and emergency departments provided detailed descriptions about the phenomena. Data management was an inductive, iterative analysis completed and facilitated by the use of NVivo 10 software program. The study found that participants had a brief emotional response post assault mitigated by the community of nursing personal from their immediate surroundings and that assault is "part of the job." Providing a true culture of safety would include enhancements to the internal community of bedside nursing practice. In addition, research is needed on interventions that can effectively enhance the internal community after assault by patients. This study contributes to positive social change by providing registered nurses, an oppressed group, and a "voice" to mitigate negative consequences associated with assault in the hospital setting.
