Full textThe following study is the result of a case report held at a public pychiatric hospital of a city in the State of Bahia-Brazil. We focus on the development of nurses activities in the institutional routine and on their ways of dealing with situations and social relations with the other health professionals working at that hospital. Being a profesor in the Psychiatric Nursing field at Universidade Estadual de Feira de Santana (UEFS), I have direct contact at that hospital due to nursing students training (as a result of a partnership between the State Health Bureau and UEFS). We have mode a field sampling by taking one male, in-patient ward. We have used, as data-collecting strategy: participative observation, semi-structured questionaires applied to the nurses and other staff members who share daily work with them, and some documents which are important for contextualizing the work of such profesionals. We have presented the nursing tasks by establising their connections among nurses themselves and between nursing and other health profesionals. We have found great concentration of burocratic work among their tasks. Besides, we have found evidence of patients being considered medical problems to be solved, which expresses nurses views of the mentally-ill. In the staff daily routine, nurses considered themselves as being the core. However, this issue was not so evident when we looked upon the other profesionals views. Administrators considered nurses as being great situation managers. This places nurses at a politically-convenient situation for keeping majoritative interests. At this hospital, nurses have various hierarquically estrategic functions, close to the Board of Directors and having direct contact with them, daily. The living conditions of the patients in such institution are extremely poor and humiliating for any human being. The lack of priority given to mental health policies in this State of Bahia greatly contributes for the traditional assistential model, still playing at such hospital.

Full textProfessional nurses, with additional training in mental health, report attitudes and perceptions of mental health nursing that are more positive, whilst those with less training report more negative attitudes and perceptions to mental health nursing. The primary aim of this research study was to describe the attitudes and perceptions of professional nurses towards the mentally ill in a psychiatric hospital in the Cape Metropole. The objectives of the study were: to explore the attitudes and perceptions of professional nurses towards the mentally ill; to identify common factors that influence the professional nurses’ attitudes and perceptions towards the mentally ill; to compare the attitudes and perceptions of professional nurses who have completed the Regulation 425, Regulation 808 and Regulation 212 training in mental health nursing towards the mentally ill. A quantitative, exploratory, descriptive design was employed and cross-sectional survey was carried out. Participants comprised all permanent professional nurses (n=60) at a governmental Associated Psychiatric Hospital in the Cape Metropole. Participants completed a demographic questionnaire and two self-report questionnaires, measuring attitudes to and perceptions of mental health problems. Nurses with a diploma report significantly higher role competency than those nurses with a degree. The ethnicity of nurses played a role in the stereotyping of the mentally ill. No significant differences were evident between those professional nurses who had completed the advanced mental health course and those whom had not. However, the combined effects of learning the appropriate course and experience in the practical field of the mentally ill are necessary for the task of impacting positively on the attitudes of the nurses towards the mentally ill.

Full textAlthough the results of many Brazilians qualitative studies on the learning process in mental health have added knowledge, others are needed using less subjective methods of study and less subject to bias implicit in the academic process. This study aims to evaluate the psychometric properties of the instrument \"Nursing Students\' Attitudes Toward Mental Health Nursing and Consumers\" for use among undergraduate nursing students in Brazil. This is a methodological study of a subjective construct measuring instrument, originally from Australia, which was culturally adapted to the Brazilian context for evaluating its properties through the standardized methodology of cultural adaptation of psychometric instruments, Exploratory and Confirmatory Factor Analysis, by the Statistical Package for The Social Sciences (SPSS) v.21 and AMOS / SPSS, v.22, respectively. The original instrument and Authoritarianism and Minority View fields of the instrument OMI, already validated for the Brazilian reality was applied. Data collection was performed in 5 undergraduate courses in Nursing at public higher education institutions located within and São Paulo and Parana States, from the years 2013 and 2014, totaling 365 subjects. The subjects were divided into 91.3% of females and 8.7% males and their ages ranged from 18 to 58 years, with a mean (M) 21.9 years and a standard deviation ( sd) of 3.88 years, the theoretical component of classes ranged 36-71 hours / class, M = 52 hours; SD = 13.64 and the clinical practice, ranged 76-100 hours / class, M = 91 and SD = 5.22. The initial application time interval and end of the instrument varied depending on the schedule and hours of classes of each higher education institution, ranging between 29 and 136 days, M = 77.63, SD = 39.64. Exploratory factor analysis resulted in a pre-final version of 23 items, with a Cronbach alpha coefficient of between 0.71 to 0.95. While performing the exploratory factor analysis, twelve items were removed due to low commonality and / or internal consistency. This version was not considered valid for convergent validation method and due to major changes in its structure, could not be considered a validated version of the original instrument. This pre-final version was submitted to structural equation modeling, resulting in the presentation of a theoretical model composed of 22 items distributed in five areas, with two areas remained identical to the original one has been modified and two new ones were added, having been validated by the statistical method used and with acceptable reliability (Cronbach\'s alpha 0.72 to 0.95). Although the original instrument has not been validated in Brazil, it is believed that this study contributed to the research and Mental Health Nursing teaching, to present an assessment instrument of mental health learning based on the original instrument, as well as to present an explanatory theoretical model for the process of learning and the student\'s attitude in the mental health area

Full textThe South African health care system shifted the focus of treating psychiatric disorders from institutional care level mental health services to facilitate this process of integration into the Primary Health Care (PHC) settings. All the provinces were thus engaged in improving mental health care services at community level by providing training for professional nurses in mental health at PHC settings. Consequently, mental health nursing has also changed considerably by shifting the focus of mental health care to the primary care level. It is however, suggested that the current revolving door syndrome experienced at psychiatric institutions was partly due to inadequate community-based psychiatric services. It was also suggested that the attitudes and knowledge of health professionals towards mental illness has a major impact on service delivery, treatment and outcome of mental illness. The aim of this research study was to assess the attitude of nursing staff working at a Community Health Centre (CHC) towards the mental health care user. A CHC was chosen that renders 24 hour services. The inclusive sample included all the different categories of nurses permanently employed at this CHC. The Attitude Scale for Mental Illness questionnaire was used to collect the data. Descriptive statistics: means, median and standard deviations were calculated for the following variables: separatism; stereotyping; restrictiveness; benevolence; pessimistic prediction and stigmatization. In conclusion it can be said that the nursing staff with more experience irrespective of category of nurse has less of a stereotyping attitude towards mental illness. The longer the nurse worked at the setting and irrespective of their nursing qualification the more positive their attitude towards the MHCU became.

Full textIt\'s well known that the mental health knowledge field is complex and inter sectorial. Many studies about the mental health patient, in Brazil, have revealed that although there was a change in the psychiatric treatment, still there is patients rights violations, and the feeling in the attention group is of certain hopelessness and lack of knowledge in dealing with the patient. This work aims to explore a masculine and feminine acute unit nursing personnel, in a psychiatry specialized hospital in the inner State of São Paulo, from the point of view of their work. More specifically, it\'s intended to know, from their own perspective their professional pathways and education, the institutional context and their everyday practices concerning work and clients. Eleven nursing professionals were interviewed through \"thematic life history\" interview. Those interviews were taped and integrally transcript and analyzed qualitatively. The analysis showed their life pathways, influences of their personal lives on work, professional education, work routine, support on psychiatric patients and the patient\'s family concepts. It\'s considered that it\'s important to promote reflection, discussion and hearing spaces among mental health professionals, in the work setting, with those who deal directly with the patient and the whole team so it\'s possible to discuss on every day practices, transcend to them relating to personal life repercussions, promoting support for work.

Full textObjetivou-se analisar as competÃncias e habilidades de Enfermagem em SaÃde Mental identificadas pelos acadÃmicos, com base nas Diretrizes Curriculares Nacionais em Enfermagem (DCN/ENF) e na Reforma PsiquiÃtrica. Estudo de carÃter descritivo, exploratÃrio e documental, com abordagem quali-quantitativa, realizado com graduandos do sexto, sÃtimo e oitavo semestres do curso de Enfermagem da Universidade Federal do Cearà (UFC) em Dez/ 2013, em que foi utilizado um questionÃrio semiestruturado e entrevista, com dados sociodemogrÃficos e de competÃncias.Os dados quantitativos foram organizados em tabelas as entrevistas em quadros. As associaÃÃes entre as competÃncias e o grupo foram analisadas por meio dos testes de X2 e de razÃo de verossimilhanÃa. Para todas as anÃlises, o valor de p<0,05 foi considerado estatisticamente significante. A entrevista foi realizada com oito alunas do oitavo semestre e analisada de acordo com a produÃÃo de sentidos de Spink, fundamentada com a literatura pertinente. Os resultados coadunam com outras realidades encontradas em outros estudos sobre o ensino de Enfermagem em SaÃde Mental e Psiquiatria, em que medo e estigma estÃo presentes, influenciando no processo aprendizagem dos alunos. Em relaÃÃo Ãs competÃncias de SaÃde Mental, muitos se sentem deficientes, principalmente naquelas que sÃo bÃsicas para a formaÃÃo do Enfermeiro, como o reconhecimento das diretrizes da polÃtica de SaÃde Mental; aplicar assistÃncia de Enfermagem em SaÃde Mental no contexto familiar e comunitÃrio; caracterizar transtornos mentais; atuar nos diversos nÃveis de atenÃÃo à saÃde mental; e lidar com o paciente em crise de transtorno mental. Consideraram tambÃm o ensino, a didÃtica, a carga horÃria e os campos de estÃgios em SaÃde Mental muitas vezes insuficiente. Nesta perspectiva, faz-se necessÃrio avaliar estas competÃncias que estÃo falhas, assim como outros aspectos para desconstruÃÃo do que à negativo e construÃÃo daquilo que à necessÃrio para o processo de ensino dos docentes que permeiam nas disciplinas, perpetuando a ideia que a SaÃde Mental à peculiar a todos os ciclos de vida e transversal Ãs disciplinas.
This study aimed to analyze the skills and nursing skills in mental health identified by scholars, based on the National Curriculum Guidelines for Nursing (DCN / ENF) and Psychiatric Reform. Descriptive study, exploratory and documentary, with qualitative and quantitative approach, conducted with graduate students from the sixth, seventh and eighth semesters of the nursing course of the Federal University of Cearà (UFC) in Dec / 2013, in which we used a semi-structured questionnaire and interview with sociodemographic and competÃncias.Os quantitative data data were organized in tables interviews in frames. Associations between the skills and the group were analyzed using the X2 test and likelihood ratio. For all analyzes, the p value <0.05 was considered statistically significant. The interview was conducted with eight students of the eighth semester and analyzed according to the production Spink directions, substantiated by the literature. The results are in line with other realities found in other studies on nursing education in Mental Health and Psychiatry, in which fear and stigma are present, influencing the learning process of students. With regard to mental health skills, many are disabled, especially those that are basic to the formation of the nurse, as recognition of the guidelines of the Mental Health policy; apply Mental Health Nursing care in the family and community context; characterize mental disorders; acting at different levels of mental health care; and deal with the patient in a mental illness crisis. Also considered teaching, teaching, workload and field placements in mental health often insufficient. In this perspective, it is necessary to evaluate these skills that are failures, as well as other aspects of deconstruction of what is negative and construction of what is needed for the teaching process of teachers that permeate the subjects, perpetuating the idea that mental health is peculiar to all life cycles and cross the disciplines.

Full textBackground: Medication with antidepressant drugs increases in society. Statisticsfrom the National Board of Health's Drugs database from 2017 show that 632,000people in Sweden are being treated with antidepressants. Studies show thatalthough the patient expects healthcare professionals to inform about drug sideeffects such as sexual side effects, it is not done. Because previous researchshowed that nurses feel uncomfortable even though the side effects profile isknown, an interest was raised to further investigate how and why discussingsexual side effects seems to be less prioritized in nursing.Aim: The aim was to investigate, through interviews, the nurse's experience oftalking with psychiatric patients about sexual side effects in treatment withantidepressant drugs.Method: After an approved ethics application from Malmö University, a semistructuredinterview guide was drafted involving both general nurses andpsychiatric nurses. Ten interviews were conducted and lasted between 15-30minutes and were recorded with mobile phones. The interviews were conducted atthe informants' workplace at a time chosen by the informant. Each interview wastranscribed, and the analysis was made according to a modified version ofBurnard’s method (1991).Result: The following categories were identified in the result: To talk about sexualside effects, to meet the patient and to avoid the question. The result showed thatmost nurses do not talk about sexual side effects. Partly because they consider itembarrassing, but also because they consider themselves possessing limitedknowledge in the area.Discussion: Nurses needed to increase their knowledge and education to makethem feel assured when talking about sexual side effects with patients. Anestablished relationship made the topic easier to talk about, but time shortage wasan obstacle to this. Something that also proved to be problematic was the nurseslack of routines for conversations about sexual side effects.

Full textThe extra-hospital services originated in the proposals of psychiatric care reformulation that resulted from the Brazilian Psychiatric Reform. These services emerged with the purpose to replace the psychiatric care model grounded treatment delivered mainly in hospitals and centered on medication according to the psychosocial care paradigm. Some problems are presented in this process, which hinder the full construction and implementation of the psychosocial care mode, favoring the reproduction of aspects of the care mode that are aimed to be overcome in the replacing services: the available therapeutic actions are centered on medical-clinical treatments while psychosocial rehabilitation actions are considered secondary. Aiming to understand the problem, this study analyzed the organization of therapeutic projects in two extra-hospital services in Ribeirão Preto, SP, Brazil, verified how professionals develop the therapeutic project, and interpreted these professionals\' conceptions in relation to psychosocial rehabilitation. This study was based on a qualitative approach and semi-structured interviews and focal groups were used to collect data. The hermeneutic dialectic philosophy of Jürgen Habermas was the framework used. The thematic axes that emerged from data analysis were based on work organization reflected on extremely technical health actions, centered on medical-clinical technology in relation to other non-medical therapeutic actions, and on the process in which work relations are worsened in these services. In regard to the services organization, we observed that the therapeutic projects are not dialogically developed by the team; teams have not developed interdisciplinary work in their organization and therapeutic actions give priority to treatments that emphasize the pathology and its biological dimension. In relation to the professionals\' conceptions about psychosocial rehabilitation, they maintain an instrumental rationality that defines the health actions in the services to the detriment of a practical rationality that ground psychosocial rehabilitation actions organized according to the psychosocial care paradigm. Based on this analysis, we suggest the implementation of clinical and psychosocial rehabilitation actions in the therapeutic projects that attribute the same value and investment to the both in order to overcome the studied problem. Additionally, participatory evaluation research should be carried out in the services with the purpose to create mechanisms that favor therapeutic projects and improve care delivery based on the psychosocial care mode.

Full textPsychiatric illnesses can sometimes lead to behavioral outbursts that need to be addressed quickly to deescalate potentially explosive situations. Nurses are in a unique position to respond to such outbursts by calling for a rapid response team. Nurses who are part of the rapid response team should be well-informed of their roles and responsibilities in managing aggressive and violent behavior. The purpose of this project was to explore RN's and LPN's knowledge and perceptions of a rapid response team in a psychiatric facility. The Iowa model of evidence-based practice provided the framework to integrate theory into practice to improve care. A quantitative descriptive design was implemented with a convenience sample of nurses using a 4-part questionnaire. Of the 64 surveys distributed on 5 wards, 59 were completed for a response rate of 92%. Descriptive statistics were used to analyze nurse responses to demographic data and background data. A Chi-square statistic was calculated to investigate the relationship between RN and LPN responses to the Likert Agreement Scale; no significant difference in responses was found. Open-ended questions allowed nurses to comment on their role and position during a code. The comments were sorted into categories of reoccurring themes. Results suggested that nurses need to understand signs of behavioral escalation and strategies to deescalate a potentially volatile patient. Nurses commented that knowledge during a code, reasons for calling a code, and good communication skills are essential in code situations. Findings from this project can benefit nurses who work psychiatric emergencies by underscoring the need to development of psychiatric rapid response teams and to update current standards of inpatient care.
