Abstract:
This study investigated the mental health of Portuguese children in Canada. Preliminary work involved a survey of professionals serving the Portuguese community and the translation and assessment of a standardized child behaviour checklist. Forty-five Portuguese children and 45 non Portuguese children referred to a children's mental health centre were compared on demographic and family indicators and their referral source. There were similar proportions of boys and girls in the two groups, similar types of services were requested, and they had similar treatment histories. The Portuguese children were older at the time of referral and were more likely to be referred by educational agencies than the non Portuguese children. Portuguese families appeared to experience different stresses than non Portuguese families. Implications of these findings for the provision of culturally sensitive interventions for Portuguese children and their families are discussed.

The Healthy Minds/Healthy Children Outreach Service is part of Alberta's strategy to increase the capacity of primary care providers to identify and treat children and youth with mental health issues. Program development, implementation, and interim results are presented. Particular challenges connecting with Aboriginal communities, developing resources, and providing clinical consultation and online professional development are discussed. Evaluation data suggest that this form of collaborative, educational, interprofessional service is a promising approach to narrowing the care gap for children with mental health issues.

Mental health problems are the leading health problems that Canadian children currently face after infancy. At any given time, 14% of children aged 4 to 17 years (over 800 000 in Canada) experience mental disorders that cause significant distress and impairment at home, at school, and in the community. Fewer than 25% of these children receive specialized treatment services. Without effective prevention or treatment, childhood problems often lead to distress and impairment throughout adulthood, with significant costs for society. Children's mental health has not received the public policy attention that is warranted by recent epidemiologic data. To address the neglect of children's mental health, a new national strategy is urgently needed. Here, we review the research evidence and suggest the following 4 public policy goals: promote healthy development for all children, prevent mental disorders to reduce the number of children affected, treat mental disorders more effectively to reduce distress and impairment, and monitor outcomes to ensure the effective and efficient use of public resources. Taken together, these goals constitute a public health strategy to improve the mental health of Canadian children.

The children who experienced the genocide against the Tutsi in Rwanda are now in their mid to late 20s. It is almost impossible to comprehend the scale of the terror and destruction of Rwanda's societal infrastructure between 6 April and 16 July 1994. While the world remained inactive, Rwanda, a small impoverished central African state, experienced the murder of about 1 million of its citizens; it also saw the terrorising, humiliation and rape of countless thousands. Although women and children were directly targeted, some actively engaged in atrocities. About 300000 children were murdered, a significant number at the hands of other children. The level of terror differed across the country and escape was frequently by luck alone. A UNICEF (2004) study of 3000 children revealed that 80% had experienced death in the family, 70% had witnessed a killing or injury, 35% saw other children killing or injuring other children, 61% were threatened with being killed and 90% believed they would die (Human Rights Watch, 2003). Of the 250000 women raped, 30% were between 13 and 35 years of age, 67% developed HIV/AIDS and 20 000 births resulted (Donovan, 2002).

Children and adolescents with learning disabilities have high rates of mental health problems and behavioural difficulties. Comorbid disorders such as epilepsy, autism and attention-deficit hyperactivity disorder are common. Despite this, many areas in the UK are failing to provide a psychiatric service for these young people and their families. The children suffer as a result and may have to move away from home unnecessarily, at enormous emotional and financial cost. Each area should have a specialised multidisciplinary health team working closely with colleagues from education and social services to assist these complex children and give them the best chance to fulfil their potential.

In 2015, the United Nations International Children’s Emergency Fund (UNICEF) named Syria as the most dangerous place on earth to be a child (UNICEF, 2). Since the onset of civil war in 2011, nearly 4.8 million Syrians are refugees outside of Syria and approximately 6 million are internally displaced (United Nations Office for the Coordination of Humanitarian Affairs, 2016). While some refugees have successfully resettled in North American and European nations, many remain in limbo in refugee camps. What is most staggering about the population of affected persons is that nearly half, approximately 6 million, are children (UNICEF, 2016). Nearly all of these children have been subjected to trauma that has manifested in a variety of ways. They have often been subjected to or witnessed violence and have experienced the loss of one or more of their caregivers. Refugees face difficulty accessing psychological and health services and are met with the stigma surrounding mental health in countries including Lebanon and Turkey, regions that many refugee children have fled to. In the absence of these supports, the mental trauma a child is experience can impact learning and development and have disastrous impacts on their future.
