Abstract:
Computer networks and especially the Internet have allowed some common activities such as shopping or gambling to become remote (e-shopping and e-gambling). The poker game played over a network is known as mental poker. The problem with mental poker is the difficulty of keeping it practical while guaranteeing the same standards of security, fairness and auditability offered by standard casinos for physical poker. The important aspects to take into account when designing mental poker protocols are: functionality, security, and computational and communication cost. Proposals in the literature usually focus on the first two items only. This makes comparisons difficult. This thesis starts with a formal cost analysis of the main proposals in the literature. The analysis is not limited to costs, though; security is also analyzed and, in fact, our study detected a fundamental weakness in one of the compared mental poker protocols. The attack is presented in a separate chapter after the global comparative analysis. The three following chapters of this thesis present three new protocols that enhance the proposals in the literature in different ways. The first proposal belongs to the family of TTP-free protocols and does not preserve the confidentiality of player strategies; it reduces the computational cost by avoiding the use of zeroknowledge proofs. The second proposal is TTP-free, preserves the confidentiality of player strategies and reduces the computational cost by requiring players to perform less mathematical operations. The third proposal addresses a novel functionality usually not offered in the literature, namely player dropout tolerance, i.e. the ability to continue the game even if some players leave it.

Abstract:

The aim of this work is to identify, in the discourse of Diogo Mainardi, patterns that might allow us to recognize the construction of a specific mental model of representation of the President of the Federal Republic of Brazil, Luis Inácio Lula da Silva, and of members of the Workers Party (PT). The corpus contains three chronicles published in the VEJA magazine, between June 2006 and January 2007. We based our studies on the principles of the Social Cognitive Theory and the Critical Discourse Analysis, as defined in the writings of Teun A. van Dijk (1983; 1991; 2004; 2005). Through our research, we come to the conclusion that Mainardi created a negative mental model of President Lula and of the members of PT, by using contextual manipulation and unfavorable social characteristics, such as corruption, incompetence, critical incapacity and attachment to the notion of people/popular.

Abstract:
Human beings are defined as a product of the culture where they are born and develop. Assimilation of the culture restricts the freedom of subjects in favour of social institutions. Mental health is one of these institutions that, in the rehabilitation or biomedical model, attempts to control all behaviour that departs from the norm by making individuals responsible for the limitations and barriers they face. Thus, mental health sidelines the participation of people in the processes and objectification of what is understood as a problem to be treated. The discipline of social work aims to promote a paradigma shift in the care of people who are diagnosed with a mental disorder from the perspective of the social model, which considers the social system to be responsible for the inequalities and exclusion that subjects undergo, while recognising their capabilities. This axiom identifies the ideal value of one’s life experience as a necessary tool in the process of dignifying mental health in the community, supporting the emergence and transformation of an environment that accepts mental diversity as a sign of social enrichment. The main goals of this research are to reflect on and frame the role that social work has historically played in the field of mental health and examine how this discipline achieves its task of improving people’s quality of life. Therefore, qualitative methodology is used to capture the experiences of different agents, such as people disgnosed with a mental disorder, from “first person” informants; family members and social workers. Six participant observations, twenty in-depth interviews and six focus groups were conducted. Analysis of extracted data allows deeper understanding and diffusion of the strategies and methods of social work to be attained, in which the importance of the emancipation of the discipline from the spheres of power is reconsidered to avoid the reestablishment of power structures and promote diversity in society. It emphasises the role of the discipline in identifying and humanising difference as a key element at the base of social structure. It is stated that social work is grounded in models of a critical nature with institutional and structural order to avoid a focus on pre-set responses and to consider the Other as a subject with the capacity to act. It has been determined that people diagnosed with a mental disorder frequently accept the label and the role assigned to them due to the benefits they receive as a consequence. These statements are based on the fact that political lines of action take advantage of the fallacy of participation or inclusion to homogenise and normalise differences. In this situation, social work has a continuing responsibility to ensure the diversity of the connotations of the concept to avoid binarisms, such as health and disease.

Abstract:
There is a consistent research base that shows that class and inequality is associated with poorer mental health and experiences of distress. Various explanations for this link have been proposed, including psychological, social, structural, material and political factors. Experiences of powerlessness and oppression have also been implicated. Nevertheless psychology focuses predominately on explanations and interventions at the individual level. To explore this incongruence, a Foucauldian Discourse Analysis was completed of texts produced by Improving Access to Psychological Therapies (IAPT), related to the development and practice within IAPT and supervision sessions of high-intensity therapists practicing in IAPT. The emerging discourses are discussed.

Made available in DSpace on 2013-12-06T00:08:35Z (GMT). No. of bitstreams: 1 320330.pdf: 2292712 bytes, checksum: 3903bb3a4832de72dd7865fc3b3981a4 (MD5) Previous issue date: 2013

The aim of this study was to identify the main factors of mental workload, which are induded in the (NASA-TLX) instrument, incidents in the Magistrates Court in Santa Catarina, in terms of their activities. The pressure of society, the main characteristic inherent to the profession of the Magistrate requires promptness and insight in the interpretation of laws and causes beyond the concern with productivity, great effort to the duties with neutrality. Research techniques used were the instrument of NASA-TLX subjective evaluation and measurement of physiological indicators such as heart rate, breathing and blood pressure. The qualitative research was conducted with fifteen (15) Judges of the Court of Santa Catarina to determine which factors of mental workload bring consequences for the health of Magistrates and confront them with the results of the application of NASA -TLX and the physiological indicators. Questionnaires with closed questions and focused exclusively on socioeconomic aspects were completed by the subjects, as well as the forms of the method of subjective evaluation of NASA-TLX, so that it be applied in its entirety. Before starting the trial session, physiological parameters were measured and after the completion of the session, another measurement was performed. The data were gathered in statement tables in order to get a visual representation of all the information, and consequently an overview of the data. We used the technique of triangulation, which allows the interaction of different sources of evidence to support the constructs, in order to detect convergences or divergences in information. Subsequently, we performed a detailed analysis of the data collected in order to explain the evidence that must be embasadas with the existing theory. Limitations of the study were significant, referring to the small number of studies on the subject and the data system of the Medical Officer of the Judiciary by Catarina does not appear in its records the distances to health care Magistrates, since it could to make a bridge between the clearances for psychosomatic disorders category in the study. The sample size does not represent that the statistical significance, was another limitation found, besides the non-clinical testing, the object of the literature review, was a point not raised in the study, as well as conducting the evaluation of magistrates in one opportunity, as well as the non-extension of the study to other servers. In addition to the many factors that may have affected the results of the research, the physiological measures may have been influenced by lifestyle, disease history, medication use and adherence to treatment. From a qualitative standpoint, the results of this study with the Judges during the trial session, are representative and show the mental demand is affected to a great extent. The burden of mental work can resonate so expressive and striking the disease process category that is usually associated with the load variations and results in modifications or changes to the overall work performance. In the measurement of physiological indicators was high in the results of most of the Magistrates, especially blood pressure, which showed higher sensitivity in that activity. Knowing that Hypertension - HTN is one of the most important public health problems in Brazil and one modifiable risk factor, concerted efforts will be needed, as well as implement measures that encourage prevention and health promotion of the Magistrates and that incorporate some changes in habits. It can be inferred then, that the similarity of the results suggest a trend of correlation between overload and increased blood pressure results may have been affected by innumerable factors and lifestyle habits such as sedentary lifestyle, stress, smoking, previous disease, overweight, food , among others, which demand a deeper scientific investigation.

This dissertation deals with the power negotiation between psychiatrists and patients, or users, of a mental health public service. Based on Michel Foucault analytic of power we investigate how the power surrounds the therapeutic practices of the interviewed psychiatrics, as well as on the relation of the hospital with other services of mental health care network. The study was made in a psychiatric hospital of the public mental health care network of Minas Gerais state, Brazil. This particular type of service was chosen since historical data and literature testify that the psychiatric hospitals were the major target of criticism in a time of disciplinary psychiatric practice. Further, there is the important confirmation that this kind of service is still present in the whole country, notwithstanding all the efforts made to its replacement. The psychiatric reform movement, a historical phenomenon which generates new theories and practices, arises as a political and epistemic response to that psychiatric and its most evident dispositive, namely the hospital. One of the most fundamental proposals of the psychiatric reform is to subvert the psychiatric disciplinary relations in regard to madness. This qualitative research, made through semi-structured interviews with psychiatrists, was ultimately focused on the permanence of violent acts — analyzed in the light of Hannah Arendt’s concept of violence — and on the importance of the hospital dispositive in a psychiatric practice that insists on being an asylum one. Two fundamental transformations were found: first, the passage from the hospital gaol to an antinomy of the network functioning; second, the exercise of psychiatric power as a producer of normative subjectification instead of systematic exclusions. It is concluded that madness is still confined in medical discourse, and the necessity of psychiatric reform in reinventing new ways of subjectification is verified.

Abstract:
Embedded in the field of Information and Communication Sciences, making use of Michel Foucault’s influential work on Discourse and “Power/Knowledge”, this PhD research calls into question the political, social and media treatment of mental disorders which has been renewed by the switch from Psychiatry to Mental Health. The advent of the “mental-health user – citizen”, which can only occur through a new discursive path, needs to take into account the legacy of social representations that affect people living with a mental health condition. This PhD research has elaborated an a posteriori pluri-methodology for the study of social representations of the “Mad”, the “Mentally-ill” and the “Depressed”. Its aim is to examine the hypothesis of a new intelligibility of “madness” through a constructive comparison between the results of a public health survey called “Mental Health in the General Population : images and realities” and the findings of a quantitative and qualitative analysis of three hundred press articles published in the French newspaper Le Monde in either 2005 or 2015. Even if the public health survey results conclude that the “Mad” is confined in the Otherness held back by the Psychiatric “Power/Knowledge” – de facto establishing a close relation with the “Mentally-ill”, the media analysis reveals that “madness” can be an accepted even claimed identity insofar as the medical control and the Psychiatric “Power/Knowledge” ascendency fade away. Thereby, Mental Health can be considered as a new area to which this PhD research gives the name of The Great Landing as to underline that it should be analysed as the antithesis of Michel Foucault’s historical and well-known “Great Confinement”

Abstract:
Power issues in the relationship between service users and clinical psychologists have received little attention from a postmodern perspective. The recovery approach and the scientist-practitioner model as recommended in best practice guidelines creates an argument for investigating power dynamics in academic and practical disseminations. This study aimed to investigate the social construction of service users and clinical psychologists in articles. Twelve articles and opinion pieces written by clinical psychologists and service users were sampled from publications of the Clinical Psychology Forum. A Foucauldian Discourse Analytic method was used to identify dominant discourses and counter-discourses. The discourses were linked to the power dynamics in play between relevant institutions. The analysis identified an economic discourse, a technical-rational discourse and an expert discourse as constructing service users and clinical psychologists. Clinical psychologists were found to have more discourse availability than service users, and in a position to make choices, whereas service users were found to have availability to a limited number of discourses with fewer options of subjectivity. A need for clinical psychologists to make conscious choices in practice was implied.

For this purpose, a brief historical review of social assistance policies was produced in order to understand the current scenario, marked by neoliberal reforms. The study, from an ethnographic inspiration, there was basically through three stages. At first, the researcher followed meetings with the CREAS team. For each meeting it was developed a Field Diary. In the second, the researcher conducted reflexive interviews with four workers. The third and last, was developed the interpretation, analysis and discussion of the collected material in the previous steps. This process revealed the constant changes that the work in public service suffers according to Social Assistance policies in the municipality and its relationship with neoliberal processes in the broadest sense. If, on the one hand, despite this context, the workers have the capacity to also turn on and organize their activities, giving new meanings, collectively, on the other, it was observed that the mental strain is present in their daily lives. It seems to be associated to the numerous impediments to the realization of the work activity raised by management, as well as the workload related to the nature of services to families who have suffered violations of their rights. To handle these situations, the workers take more individual tactics rather than a collective organization.
Abstract:
Mental suffering related to labour activities is a relatively new tleld of study which has gained increasing importance due to the serious implications that work-processed mental ilInesses bring to societies. In the specitlc case of the Judiciary workers, this condition becomes increasingly more and more important once the pathologies associated to the psychic system are the main cause of work leaves in this group of workers. This research aimed at identifying the effects of technology on mental illnesses in the workers of the Judiciary in the State of Paraná. It has been investigated to which extent the work, as an analytical category, accounts for psychiatric disorders in these workers. The methodological choices consisted of two stages, being the tirst a documentary research in the statistical reports produced by the Occupational HeaIth Service of the target institution. These reports show the total number of workers who asked for a work leave by disease groups organised according to the World Health Organization's Intemational Classitlcation of Diseases (ICD- IO). Reports of medicaI leaves motivated by Mental and Behavioral Disorders (Chapter V of ICD-IO) have been analyzed. The compiled data were: "total number of people affected", "total of days off work", and "specitlc mental and behavioral disorder". With such data, an accurate analysis of the medicaI history of magistrates and public servants chosen by pre-detined criteria was carried ou!. Thus, it was possible to identify individuaIs who did not have previous history of psychiatric pathologies prior to joining the institution. Subsequently, a semi-structured interview was applied to previously selected individuais. This consisted of objective and open questions, whose aim was to identify the possible relationships between the work performed and the development of psychic disorders. The collected responses were analyzed on the lenses of the Discourse Analysis of the Collective Subject methodology. The epistemological path of this research wilI be the Psychodynamics of Work interfaced with labor relations in the present capitalist society as proposed by Christophe Dejours. The research provided elements of analysis of the processes of mental suffering related to work in this category of professionals, pointing to the adoption of new technologies and the consequent changes in work processes that might be related to the mental illnesses of the public servants of the analysed institution. Therefore, it points out to the need of proposing actions with a view to minimizing potential damages to the mental health ofmagistrates and public servants.

Grech, Paulann. "The therapeutic alliance in mental health services : a politico-critical analysis of knowledge and power." Thesis, University of Sheffield, 2014. http://etheses.whiterose.ac.uk/7781/.

Full text
Abstract:
The therapeutic alliance in mental health services has been researched extensively especially with regards to care provider and care receiver characteristics that may affect therapeutic outcomes. In the literature, it seems to be commonly assumed that the alliance has an inherent power imbalance in favour of the professional. Beyond this assumption, research on the knowledge-power balance in the alliance is limited. The aim of the study was to explore the dominant discourses related to the balance of knowledge and power in the therapeutic alliance in mental health services. An additional interest was the role that these identified discourses may play in the positions occupied by care providers and care receivers in the alliance. A final objective was the identification of care provider and care receiver knowledge needs in relation to the management of the knowledge-power balance in the alliance in a manner that may enhance therapeutic effects. Ten semi-structured interviews were conducted involving individuals who were receiving psychiatric care in the inpatient setting at the state psychiatric hospital in Malta. The second phase of this study involved the analysis of the Medical and Nursing records that pertained to the ten participants who had been interviewed. A Foucauldian Discourse Analysis framework was used to guide the study and Foucault’s concepts of knowledge and power served as the theoretical underpinning of the study. The care receivers’ contribution to the discourses pertaining to the knowledge-power balance in the alliance led to the identification of four different types of alliances with distinguishable knowledge-power characteristics. Three discursive themes were identified from the Medical and Nursing records and these were perceived as being complementary to the findings that emerged from the care receivers’ interviews. Finally information related to the potential knowledge required by care receivers and care providers in order to manage the knowledge-power balance in an effective manner was extracted from the data and presented together with recommendations.

Abstract:
Mom Power (MP) is a trauma-informed parenting intervention which aims to break the cycle of intergenerational risk transmission by promoting enhanced parent-child attachment, increased social support, connection to community services, and utilization of self-care skills. MP was developed to engage vulnerable families (e.g., mothers with trauma histories, low income, single mothers) and seeks to mitigate common treatment accessibility barriers for underserved populations through provision of childcare, transportation, and a trauma-informed/culturally sensitive milieu. Results from a community-based randomized control trial (RCT) in Michigan (MI) have shown that MP is effective at reducing maternal psychopathology, lowering parenting stress, decreasing parental hopelessness, and promoting reflective parenting; however, no studies have examined the feasibility of implementing MP outside of MI, nor the effectiveness of MP with additional samples (Rosenblum et al., 2017). The present study is a hybrid implementation/effectiveness design that is two-fold: (1) Implementation design to assess the feasibility of training rural, Appalachian community-based providers in the MP model, recruiting high risk mothers, retaining mothers in this 10-week intervention, and implementing MP groups with community-based partners (2) An effectiveness oriented, pre-post, within subjects design with mothers of young children who are attending MP groups in the community [n = 33 mothers; M maternal age = 26 (SD = 5) years; M child age = 12 months (SD = 15)]. Regarding implementation results, the MP training involved n = 31 community-based providers from 5 agencies (e.g., community mental health, foster care agency) and 3 university-based mental health training programs. In the 18 months since training, three 10-week MP groups have been completed (n = 25 mother-child dyads), and one group is currently being held (n = 8 dyads). Regarding recruitment, we have had great success reaching high risk families, having more family referrals than available group slots. Of the families served in TN thus far, 60% had DCS involvement, 46% were in substance abuse treatment, 68% endorsed ≤ 4 ACEs, 77% had clinically significant depression, 58% had clinically significant anxiety, 78% were single/had no co-parent, 81% had low educational attainment, and 100% were below the federal poverty level. Regarding effectiveness, following completion of the current group, we will present pre-/post- differences in maternal PTSD symptoms, emotion regulation, and parenting behavior, as well as examine the relationship between attendance and change scores.

Lee, Jessica Nalani Oi Jun. "Too Much Information: Agency and Disruptions of Power in Personal Narratives of Mental Illness and Suffering." Diss., The University of Arizona, 2014. http://hdl.handle.net/10150/323465.

Full text
Abstract:
Healing in the mental health system of the 21st century is difficult as the credibility of mental health users is constantly called into question, their experiences and perceptions of their "illness" undervalued or even completely ignored. This attitude towards mental health users must be changed in order to work towards truly alleviating mental illness and suffering. Careful analysis of the rhetoric of published personal narratives written by women describing their experiences with mental healthcare reveals the ways in which medical knowledge is created, owned, and disseminated only by the “authoritative expert,” defined as healthcare professionals who categorize, taxonomize, and pathologize in order to treat both physical and mental illness. I argue the authoritative expert marginalizes the "everyday expert," exemplified through the perceptions of women who, in their narratives, record realities that do not always match the diagnoses and prognoses assigned to them by their healthcare providers. My project's central question asks: In what ways do personal narratives of mental illness and suffering illuminate the ways in which language constructs reality? My research illuminates the ways in which narratives of mental illness and suffering are healing, and thus serves as an advocate for patient rights, both by empowering patients and by furthering discussion among medical professionals regarding problematizing "standard" treatment. My work advances the connection between politics and language as it takes a commonly undervalued form of language and lived experience--narrative--and researches the ways in which it has been and can continue to be used as a powerful political agent to empower mental health users by giving them a voice. Specifically, I demonstrate how patients' personal experiences should and can be valued as a way to illuminate their own understanding of their disease as well as to inform their treatment. This project lays the foundation for future research examining ways treatment for mental illness should be differentiated from treatment for physical illness. I am interested in ways to further combat the stigma of mental illness by looking at ways providers can honor and respect the opinions and values of mental health patients in non-pejorative ways.

Abstract:
This study explores the issues around the separation from society of people who have mental health problems. It reconstructs the model of power provided by Foucault in his later work on governmentality. A careful examination of Foucault's theory of power is undertaken in comparison to others within the sociological tradition to demonstrate how this lends itself most appropriately to the kind of analysis undertaken here. The focus is on Foucault's later work where he addresses the power/knowledge couplet and what subsequently came to be coined 'structures in governmentality'. In this sense the study is located as a post Foucauldian analysis and develops some important elements of his theorizing about power. The emphasis demonstrates the significance of power and power relationships in an attempt to make sense of the experiences of people with mental health problems and in particular the impact that mental health policies have on them. Of particular interest is the way that Foucauldian sOciologists such as Rose and Dean have re-constructed Foucault's theory of power. It is this emphasis on governmentality as forms of regulation, social control and management in relation to people who have mental health problems that is at the centre of this study.

Abstract:
Since the earliest published American narratives, writers and subsequent Western clinicians alike have often mislabeled Indigenous behaviors, especially the behaviors of Indigenous women, as insanity. And yet, as Pemina Yellow Bird (Three Affiliated Tribes) explains, "Native peoples generally do not have a notion of "insane" or "mentally ill." (4). Instead, Indigenous peoples often discuss mental health in their communities through storytelling. As but one example of the ways that cultural narratives work to reclaim Indigenous understandings of mental health, this paper analyzes how the writings of Chickasaw author Linda Hogan challenge non-Indigenous understandings of mental health as a gendered phenomenon within tribal communities. Hogan does this in ways that destigmatize behaviors including hallucinations or prophetic dreams that Western medicine considers abnormal, and reintroduces community-specific understandings of these behaviors as either a supernatural phenomenon or a gift of foreknowledge. Hogan's novel Solar Storms (1995), in particular, reframes stereotypical images of tribal women as insane with images of Indigenous women as cultural, political, and spiritual leaders in their communities. While she addresses community-specific understandings of actual mental illness, Hogan also characterizes what many might mistake for mental illness as the essential foresight of Indigenous women and thereby offers a healing corrective to the prevailing narrative of Indigenous women's presumed insanity. A central discussion in this paper is how Hogan defines knowledge-making and Indigenous women's rights and responsibilities in Solar Storms. The term "rights and responsibilities" refers to a sense of stewardship Indigenous women in the novel experience to protect land and community: this charge may include giving life through childbirth, communicating with animals and the dead, dreaming of medicinal plants, intuitively remembering traditional song and dance, "seeing" creatures without one's eyesight, and healing abilities, among others. Female knowledge-making, then, refers to insights about oneself, community, and the material and immaterial world in enacting these behaviors. By expressing the possibilities of Indigenous women's relationship with the natural and supernatural world instead of either exoticizing or dismissing them, Solar Storms works to legitimize Indigenous modes of female knowledge-making in the face of ongoing colonial assumptions about Indigenous insanity.

Abstract:
In this work, we present a set of methods aimed at improving the discriminative power of time-varying features of signals that contain noise. These methods use properties of noise signals as well as information theoretic techniques to factor types of noise and support signal inference for electroencephalographic (EEG) based brain-machine interfaces (BMI). EEG data were collected over two studies aimed at addressing Psychophysiological issues involving symmetry and mental rotation processing. The Psychophysiological data gathered in the mental rotation study also tested the feasibility of using dissociations of mental rotation tasks correlated with rotation angle in a BMI. We show the feasibility of mental rotation for BMI by showing comparable bitrates and recognition accuracy to state-of-the-art BMIs. The conclusion is that by using the feature selection methods introduced in this work to dissociate mental rotation tasks, we produce bitrates and recognition rates comparable to current BMIs.

The present work intended to investigate the concept, the strategies and the devices of encouragement to empowerment recognized by the main actors involved in mental health care, in two day-care psychosocial services, as a way of contributing to the assessment of how the identification, characterization and the appropriation of devices of empowerment in the process of the Psychiatric Reform in the country have been taking place. As a field of research, we selected a day-care psychosocial Center and a Center for Coexistence, both in the municipality of Campinas SP. We had as subjects of the research seven users, three family members and twenty-four multidisciplinary team professionals. The case study took the form of a descriptive and qualitative research, and used two instruments for data collection: the interview and the participant observation. The semi-structured interview was used both for individualized interviews, conducted with users and their families, as well as press conferences, made in the wheels of conversation with the professionals. The participant observation, aided by a previously prepared sequence, subsidized the participation in various and different spaces in the research field. The interviews were recorded and transcribed, being analyzed by means of identifying major themes, from which it was sought to make a descriptive analysis of the views of each of the sets of actors. As a result we obtained that the empowerment concept presented by users, families and professionals envisions autonomy of the users and family, seeping through the power of choice, by the power of decision and by the power of their subjects with their diversity and similarities. And these concepts are presented in groups and workshops taking as points in common the search for the role and by the co-responsibility of users in its therapeutic project in the services and in its life project. It is expected that this work will contribute to the understanding of the importance of empowerment in the reinsertion of people suffering mental disability, serving as a tool, as a possible way to modify the various realities of caring developed in the CAPS all over Brazil.

Abstract:
Introduction Workers, interlocutors in this study, are characterized by the simultaneous sum of two professions, police and military. Both require extreme subjective engagement and include violent experiences. Job peculiarities and job demands entails that these professionals are not usually seen as workers and they frequently stay on the sidelines of discussions and relevant actions in the Occupational Health field. This study aims to contribute to the expansion of studies on mental health and work, specifically considering gender relations and the elements related to balance or imbalance between personal and professional lives. Aim To apprehend, from the gender frame, mediations between subjectivities, work, violence and psychic suffering of men and women workers in the condition of military police officers of a corporation located in Brasilia, Federal District of Brazil. Methodological procedures Interviews using a guiding script were conducted individually with 7 military policewomen and 17 military policemen from different positions in the organization hierarchy, who worked or had worked in ostensive policing. A thematic content analysis, comprehensive and inductive, was performed. This type of analysis has a descriptive and an interpretative dimension supported by theoretical and analytical concepts. In the analysis of in-depth interviews the following theoretical concepts were used as analytical categories: subjectivity, gender relations, work-related violence, and mental health. The project was approved by the Ethics Committee of School of Public Health/University of São Paulo. Results Work organization was identified by informants as more harmful to the mental health than the work content (to deal with urban violence). Militarization shapes the subjectivity of police workers, hindering them to face the abusive situations and expressing emotions, resulting in psychological distress. At the same time, gender relations may adversely affect or have a protective effect on mental health of military police, depending on the quality of relationships at work and with family. Conclusion Preventive actions and mental health promotion in the military police organizations are required and urgent in order to improve the quality of life and psychological well-being of workers and their families, with implications, including, on the intra and interpersonal violence, either police, domestic or social violence.

The main objective of this doctoral thesis was to analyze how the primary teachers hospitalized in the Juliano Moreira Hospital, in Pará, were conceptualized as insane from relations knowledge-power instituted in that space institutionalized for madness in our state. The specific purpose is directed to reflect on the theoretical and methodological perspectives that involve the production of truth about the theme of Madness and its constitution as epistemological area of scientific production in Brazil; to problematize relations of power-knowledge that guided the production of Madness from the institutionalization of HJM hospital model in the years 1964-1984;to analyze how the official discourse of Para governments engendered by the power mechanisms of discipline and biopolitics constituted the madness treatment practices in this hospital; and analyze how primary teachers hospitalized in this psychiatric space were conceptualized as insane based on the relations of power-knowledge prescribed in their medical records. What devices is under, the thematic related to madness has been constituted as epistemological area of scientific production in Brazil based on the relations of power-knowledge? How relations know-power produced the ways of subjectivity from the Madness of speeches in Juliano Moreira Hospital, in Pará, between the years 1964-1984?How the mechanisms of power of discipline and biopolitics was constituted in official speeches of Pará governments in this period for through the analysis of Madness treatment practices in this hospital? How the primary teachers hospitalized in this clinical space were conceptualized as insane based on the relations of power-knowledge prescribed in their medical records? It is a historical and educational research under theoretical and methodological ballast Foucault's genealogy with documentary contribution in the medical records of HJM in Pará governorsmessagesof the period in question, newspapers, SESPA reports and two important journals begotten by professionals of psychiatry from Paráthe Bulletins of the Juliano Moreira Hospital Research Centre that circulated between the years 1967-1971 and Psychiatry Para Magazine edited in 1984. The formulated thesis consists of the assertion according to which the field of education the madnessconstitutes themed of little epistemological interest, however, studies of Michel Foucault about the analytical categories knowledge, power, discipline, biopolitics, speech and subjectivity are important theoretical tools able to helpin the understanding of mental health biopolitics engendered by the Pará Stateduring the functioning of Juliano Moreira Hospital, because within this institution had confrontation treatment paradigms of hospitalized subjects andtheir speeches produced for framing women as primary teachers as insane emerged from relations know-power coming from the areas of medicine and its related, as well as the operating force of the disciplinary society traversed for government official politics, whose capillarity extends of prescriptive way to family cycles that also contributed to the legitimacy of the subjective process of these teachers like mad. Sustenance starting from Foucault analytical anchors, which these primary teachers resisted to subjective process they have suffered, and the various manifestations of these invisibilized teachers, including their knowledge, were characterized as transgression mechanisms against the abduction of their bodies. I reiterate also that the forms of resistance of these teachers have subverted the discursive rationality centered on freedom and humanization of relations in the HJM from 1964 and all its prescriptive apparatus. It was concluded that primary teachers hospitalized in the Hospital Juliano Moreira and their various formsof manifestations that produced them like insane, had their knowledge also guided by a technical-scientific rationality, however, fully deprecated versus relations know-power psychiatrist, as well as the manifestations of resistance of these women-teachers about their subjectivation process, emerged as claim evidence of their madness and not as resistance strategies to control their bodies, without giving the crossing of gender and sexuality issues, permeated by normativities and power of families.

Abstract:
This submission for the award of PhD by Published Work includes a range of single, joint and multiple-authored publications that were published between 2005 and 2016. The publications cover a range of issues relevant to social work with a particular emphasis on mental health and health inequalities. The statement provides an underpinning conceptual framework that demonstrates interwoven themes of power, partnership and marginality. These are explored in relation to the published work, demonstrating an original and coherent contribution to the social work knowledge and practice base. The discussion draws on a reflexive journey through social work practice, education and research. The conclusion proposes that considerations of power and partnership are crucial elements of the potential for creative work ‘on the margins’.

Abstract:

The lives of humans and animals have been intertwined through time immemorial, and in many instances the relationship between humans and animals has been thought to be good for human well-being. As such, it is not surprising that treatments for a wide range of ailments, from physical to psychological, have developed that capitalize upon the relationship between humans and animals. While animal-assisted interventions have become popular in practice, the research-base of evidence to demonstrate the effectiveness of these approaches is limited. Research is needed to clarify the effectiveness of animal-assisted approaches using well-controlled and randomized samples, and to help shed light on the mechanisms of effectiveness for these techniques. While a variety of animals are used for psychotherapeutic intervention, horses are of particular interest because of the qualities that distinguish them from traditional household companion animals. The size, power, and sensitive nature of horses are thought by many to serve as a foundation for personal growth and psychological healing for humans. The current study evaluated the impact of equine-assisted psychotherapy (EAP) versus traditional contact with horses on the depressive symptoms of five at-risk adolescents. Results suggest that structured contact with horses in either a horsemanship or EAP context may be both enjoyable, engaging, and beneficial for at-risk youth within the areas of depressive symptoms, social skills, and coping skills. Additional skills, such as persistence and problem-solving, may be uniquely impacted by EAP.

Abstract:

Abstract:
Expansive and constrictive body postures serve a primary communicative function in humans and other animals by signalling power and dominance. Whether adopting such “power postures” influences the agent’s own perception and behaviour is currently a subject of debate. In this PhD thesis, I explored effects of adopting power postures on behaviours closely related to the postures’ primary function of social signalling by focusing on responses to faces as particularly salient social signals. In a series of experiments, I utilized reverse correlation methods to visualize mental representations of preferred facial traits. Mental representations of implicitly as well as explicitly preferred faces evoked an affiliative and slightly dominant impression, but revealed no replicable effects of power postures. Two further separate experiments investigated posture effects on the perception of threatening facial expressions, and approach vs. avoidance actions in response to such social signals. While postures did not influence explicit recognition of threatening facial expressions, they affected approach and avoidance actions in response to them. Specifically, adopting a constrictive posture increased the tendency to avoid individuals expressing anger. Finally, an attempt to replicate posture effects on levels of testosterone and cortisol demonstrated that even repeatedly adopting a power posture in a social context does not elicit hormonal changes. Altogether, these findings suggest that our body posture does not influence our mental representations and perception of other people’s faces per se, but could influence our actions in response to social signals

The aim of this study is to describe and analyze how the therapeutic theatre workshop, guided by the Theatre of the Oppressed's theory, influences on users' contractual power and autonomy from a CAPS I. We have carried out, initially, a literature review, which contextualizes the theoretical and practical bases of the Brazilian psychiatric reform, where we present the first experiments with art and the current form as this work more specifically the theatre, has been used in several Mental Health devices in the country. Then, we have outlined the Theatre of the Oppressed's methodology, Augusto Boal and his contributions to the Mental health field, to later perform a dialogue between this theory and the Pedagogy of the oppressed by Paulo Freire. Then, we present the work of drama therapy workshop, held in CAPS I of Puerto Real, in the State of Rio de Janeiro. We carry out filming in this workshop for seven meetings and a focus group. From there we conducted a thematic analysis of the ideas that we have seen of this material. This analysis revealed that the experience with the theatre workshop has helped in the treatment, providing reflections and actions that could be related to an increase of autonomy and power.

Abstract:
This thesis critically examines a national programme in mental health which has been driven by the implementation of National Institute of Health and Clinical Excellence (NICE) guidance. Assumptions which underpin research method, drawn from the natural sciences, are critiqued in terms of their adequacy in accounting for human relating and expert therapeutic practice. The work of Dreyfus and Dreyfus (1986) is problematized in how they account for proficiency and expertise as intuition and the leap that they make from calculative to deliberative rationality. An alternative source of understanding, based on non-linear causality and complex responsive processes, is developed, building on the work of Stacey (2001, 2005, 2007). The ineffability of expert practice (or clinical judgement) is contrasted with competence based, rule governed practice, which necessarily underpins the early stages of learning. It is argued that because research practices undertaken in randomised controlled trials (RCTs) must be describable, measurable and focussed on predictable outcomes, then these cannot account for expert practice, therefore the assertion that the Improving Access to Psychological Therapies programme (IAPT) is wholly based on research based, evidence based therapies, cannot be substantiated. The work explores professionalism and specifically considers the role of psychiatrists, psychologists and psychological therapists in mental health and in increasing access to psychological therapies. The role of managers and managerialism are explored, specifically how the NHS has sought to manage 3 professional staff and multi-disciplinary teams in adopting corporate and new ways of working (NWW). This includes the importance of and difficulty in countering professional identity using competence based approaches. The performance management processes in the NHS are recognised as an equally relevant source of evidence (to that of NICE), despite there being a poor (traditional) evidence base for it (Stacey, 2010; Seddon, 2008). Power relating in human relationships is identified as immanent, using the context of a management group, and it is argued that Foucault’s concept of disciplinary power (1994) can account for what is considered to be knowledge and truth, drawing on specialist expertise based on science and research, with a forceful potential for rendering others silent as well as pervasively self-silencing, in processes of inclusion and exclusion (Elias, 1978). It is argued that these on-going processes of relating influence policy decisions at national and local levels and how these policies are implemented in practice. The inevitability of unpredictable outcomes is highlighted, despite strong centralised programme management along with the provision of an explicit blueprint for implementation.

Abstract:
Abstract This paper will attempt to analyze how similar forms of power play are manifested towards the protagonists in both The Bell Jar and “The Yellow Wallpaper”. The aim of the essay is to investigate how power play affects the protagonists’ relations with their caregivers and how it affects their treatments. Thus, the hypothesis is that it is the power play that prevents the protagonists in “The Yellow Wallpaper” and The Bell Jar from recovering from their mental illness, which is confirmed by my analysis. Therefore, the concept of power play will be used in the essay as an instrument of analysis. The hypothesis will be discussed from five main points: obstacles to recovery, caregivers’ role in recovery, patients’ response to treatment, the role of power play, and the negative impact which power play has on recovery. Keywords: Power play, mental illness, treatment, recovery, patient-caregiver relationship, Charlotte Perkins Gilman, Sylvia Plath, Reet Sjögren.

This research analyzes the processes and practices of compulsory hospitalization in the Short Stay Unit at Hospital Adauto Botelho (from May 10th, 2010, Clinical Attention State Hospital - HEAC), located in Cariacica - municipality of Greater Vitória Metropolitan Region Espírito Santo/ Brazil. This study was developed from a work experience outline, from 2007 to 2010. It discusses the modes of operation that insist on the production of imprisoned lives and focally analyzes the periodic admissions, their effects and coping strategies to use this legal mechanism that judicializes life under the logic of biopower. The research still discusses the current challenges of Mental Health National Politics, their progress and retreats, Anti Asylum Fight progresses and the Psychiatric Reform. The cartography is used as methodological perspective. In the course of this work a cartographic posture was built, that pursued, above all, to live the experiences of know-doing not established beforehand, but the knowledge that appears, that emerges from doing. The research results in the building of five insights/causes that focus on the mixture of people, from different nations that resist the passing of time, that does not succumb to the chronicity and update the power of a collective. Thus, it is noted that this research/intervention could inaugurate dialogs, potentiate new and insistent talk meetings, providing different degrees of visibility about the madman, the madness of today. It tensions the field of mental health via the compulsory hospitalization and its interface with the justice. It inaugurates the articulation of compulsory hospitalization with the judicialization of health, and it extends this debate for the judicialization of life. It emphasizes the use/abuse of the mechanism of compulsory hospitalization in its facets of a counterpsychiatric reform, exclusion and perpetual confinement of people.

Abstract:
Over the past decade, there has been a growing interest in the devel- opment of integrated circuits (ICs) for wearable or implantable biosensors, aiming at providing personalized healthcare services and reducing the health-care expenses. In biosensor ICs, the analog-to-digital converter (ADC) is a key building block that acts as a bridge between analog signals and digital processors. Since most of the biosensors are attached to or implanted in hu- man bodies and powered by either portable batteries or harvested energy, ultra-low-power operation is often required. The stringent power budget im- poses challenges in designing power-efficient ADCs, especially when targeting high-resolution. Among different ADC architectures, the Sigma-Delta (Σ∆) ADC has emerged as the most suitable for low-power, high-resolution appli- cations. This thesis aims to enhance the power efficiency of continuous-time (CT) incremental Σ∆ (IΣ∆) ADCs by exploring design techniques at both architectural and circuit levels. The impact of feedback DACs in CT IΣ∆ ADCs is investigated, so as to provide power-efficient feedback DAC solutions, suitable for biosensor ap- plications. Different DAC schemes are examined analytically considering the trade-off between timing error sensitivity and power consumption. The an- alytical results are verified through behavioral simulations covering both the conventional and incremental Σ∆ modes. Additionally, by considering a typi- cal biosensor application, different feedback DACs are further compared, aim- ing to offer a reference for selecting a power-efficient DAC scheme. A two-step CT IΣ∆ ADC is proposed, analyzed, implemented and tested, with the objective of offering flexible and power-efficient A/D conversion in neural recording systems. By pipelining two CT IΣ∆ ADCs, the pro- posed ADC can achieve high-resolution without sacrificing the conversion rate. Power-efficient circuits are proposed to implement the active blocks of the proposed ADC. The feasibility and power efficiency of the two-step CT IΣ∆ ADC are validated by measurement results. Furthermore, enhancement techniques from both the architecture and circuit perspectives are discussed and implemented, which are validated by post-layout simulations. A comparative study of several CT IΣ∆ ADC architectures is presented, aiming to boost the power efficiency by reducing the number of cycles per con- version while benefiting from the advantage of CT implementation. Five CT IΣ∆ ADC architectures are analyzed and simulated to evaluate their effective- ness under ideal conditions. Based on the theoretical results, a second-order CT IΣ∆ ADC and an extended-range CT IΣ∆ ADC are selected as implemen- tation case studies together with the proposed two-step CT IΣ∆ ADC. The impact of critical circuit non-idealities is investigated. The three ADCs are then implemented and fabricated on a single chip. Experimental results reveal that the three prototype ADCs improve considerably the power efficiency of existing CT IΣ∆ ADCs while being very competitive when compared to all types of the state-of-the-art IΣ∆ ADCs.

QC 20150422

Abstract:
Standardisation of professional work is a major policy concern to ensure quality and efficiency of services and a number of hospitals are now focusing on the use of clinical pathways as an important tool to standardise their work. This study sheds light on the processes set in motion when notions of standardisation meet local practice. In order to gain insight into what clinical pathways mean for professional work in mental health care, the focus of the study was to explore the contexts in which standardisation by “rule production” takes place. Two empirical cases from Norwegian mental health care show how dedicated professionals are in charge of carrying out the standardisation work, strongly influenced by a steering framework of defined governmental policies where employee involvement and responsibility ensured loyalty to the idea. Along with a “package” of ideas, new bodies and techniques, clinical pathways contribute to the institutionalisation of prima facie knowledge in demonstrating that evidence basing is linked to steering and control of employees. Thus, professional autonomy is threatened in an insidious way: through the institutionalisation of evidence-based knowledge as ‘prima facie’ knowledge in combination with professionals who standardise and control their own work. The thesis therefore concludes that the control of professional work has now become a complex and sophisticated process where professional work is “controlled by knowledge”.

Minett, Roger. "An evaluation of a 'user-led' training course for mental health nurses and the effect of user involvement upon professional power, autonomy and practice." Thesis, University of Leicester, 2004. http://hdl.handle.net/2381/30866.

Abstract:
This study explores how 'user led' workshops for mental health workers were evaluated and subsequent effects upon professional power, practice and partnership working. Research methods involved observation of workshops, semi-structured interviews with course participants and analysis of evaluation forms. A content analysis of the evaluations, interviews and observation notes were interpreted in relation to literature on user involvement, professional power, practice and partnership working, although sub themes emerged during the process including safety and accountability, Care Programme Approach, resources in practice and resistance to change.;The findings reveal that the reaction of nurses to user involvement has been mixed and two groups have emerged. One group are more accepting of user involvement, the user perspective and seek more equality and partnerships with users. They espouse the rhetoric and implement the policy on user involvement and the study reveals a number of changes to their professional practice as a result of the workshop. However even in this group user involvement and partnership may be negated by the need to defer to others or legal and professional imperatives.;In contrast there is a second group who, may also espouse the rhetoric of user involvement and partnership working but nevertheless feel threatened by it. These nurses have been resistant to policy developments, are tokenistic in their acceptance of the user perspective and reluctant to change practice. They do not regard users as equal partners or engage in partnership working. This research has demonstrated therefore that user involvement in education and training can have a beneficial effect upon the working practices of some professionals but there remains a group for whom education alone is less effective.

Abstract:

How does a social welfare officer know when a parent with mental illness or behavioral symptoms is harmful to the children? As a social you are confronted with different ethical dilemmas on a daily basis. The social welfare officers have to follow their own authorities, laws, guidelines and regulations while at the same time do everything to satisfy their clients’ needs. This inconsistency can sometimes be difficult to handle. The purpose of this study was to gain knowledge and a better understanding of how social welfare officers handles their elbow room in the field of actions to support the children of parents with mental illness. We have conducted a qualitative study where we interviewed six social welfare officers using the “vinjett” method. All social welfare officers included in the study are working with children of parents with mental illness. By using Michael Lipsky’s theory of Street-Level Bureaucrats we intended to get an understanding of how social welfare officers would act in specific situations. Our study shows that social welfare officers’ first contact is with the parents for a judgment of their capacity as parents. The results shows that the social welfare officers, who we interviewed, felt that they had a wide elbow room with resources to determine and shape different actions for the children of parents with mental illness but at the same time they experienced different barriers which may limit the alternatives in their decision-making. The majority of the social welfare officers in our study felt that they were aware of the power and control they possesses, which also are defined in Lipsky’s theory.

Abstract:
Objectives The objective of the present study was to investigate the usefulness of time-frequency analysis (TFA) of olfactory-induced EEG change with a low-cost, portable olfactometer in the clinical investigation of smell function. Materials & methods A total of 78 volunteers participated. The study was composed of three parts where olfactory stimuli were presented using a custom-built olfactometer. Part I was designed to optimize the stimulus as well as the recording conditions. In part II EEG-power changes after olfactory/trigeminal stimulation were compared between healthy participants and patients with olfactory impairment. In Part III the test-retest reliability of the method was evaluated in healthy subjects. Results Part I indicated that the most effective paradigm for stimulus presentation was cued stimulus, with an interstimulus interval of 18-20s at a stimulus duration of 1000ms with each stimulus quality presented 60 times in blocks of 20 stimuli each. In Part II we found that central processing of olfactory stimuli analyzed by TFA differed significantly between healthy controls and patients even when controlling for age. It was possible to reliably distinguish patients with olfactory impairment from healthy individuals at a high degree of accuracy (healthy controls vs anosmic patients: sensitivity 75%; specificity 89%). In addition we could show a good test-retest reliability of TFA of chemosensory induced EEG-power changes in Part III. Conclusions Central processing of olfactory stimuli analyzed by TFA reliably distinguishes patients with olfactory impairment from healthy individuals at a high degree of accuracy. Importantly this can be achieved with a simple olfactometer.

Rosenblum, Katherine L., Maria Musik, Diana M. Morelen, et al. "Community-Based Randomized Controlled Trial of Mom Power Parenting Intervention for Mothers with Interpersonal Trauma Histories and their Young Children." Digital Commons @ East Tennessee State University, 2017. https://dc.etsu.edu/etsu-works/7705.

Full text
Abstract:
We conducted a study to evaluate the effectiveness of Mom Power, a multifamily parenting intervention to improve mental health and parenting among high-risk mothers with young children in a community-based randomized controlled trial (CB-RCT) design. Participants (N = 122) were high-risk mothers (e.g., interpersonal trauma histories, mental health problems, poverty) and their young children (age <6 years), randomized either to Mom Power, a parenting intervention (treatment condition), or weekly mailings of parenting information (control condition). In this study, the 13-session intervention was delivered by community clinicians trained to fidelity. Pre- and post-trial assessments included mothers’ mental health symptoms, parenting stress and helplessness, and connection to care. Mom Power was delivered in the community with fidelity and had good uptake (>65%) despite the risk nature of the sample. Overall, we found improvements in mental health and parenting stress for Mom Power participants but not for controls; in contrast, control mothers increased in parent-child role reversal across the trial period. The benefits of Mom Power treatment (vs. control) were accentuated for mothers with interpersonal trauma histories. Results of this CB-RCT confirm the effectiveness of Mom Power for improving mental health and parenting outcomes for high-risk, trauma-exposed women with young children.
