Cubbon, John Edward. "Case registers for mentally handicapped people." Thesis, Sheffield Hallam University, 1985. http://shura.shu.ac.uk/20651/.

Full text
Abstract:
This thesis is an evaluation of mental handicap registers as planning tools and is based on a national postal survey and semi-structured interviews with register-operators. Registers have emerged from pressure for greater rationality in planning the health and social services, a co-ordinated approach to priority groups, and a radical change in attitude to mental handicap. Mental handicap registers are found to be organisationally, financially and morally feasible. Moreover they can develop naturally as by-products of Community Mental Handicap Teams, thereby significantly reducing the costs of data-collection. Registers provide a basis for service-planning which is more objective and takes account of more of the users of the service than the opinions of experienced professionals. Names, addresses, dates of birth and details of services used can make a considerable contribution to planning. In addition, some indicator of the services that subjects need is required. Register-operators tend to regard the widely-used Wessex Behaviour Rating System as a crude measure and hanker after a more direct assessment of need such as might be made at a multi-disciplinary review. Most registers hold identifying and service-details of their subjects; however there are a number of registers holding in addition a large amount of information for which there is little demand in planning. The data which registers have held have not been fully exploited partly because they have not been sufficiently closely linked to the planning process and the methods of planning have not been sufficiently receptive to quantitative data. Other statistical client-based data-bases for service-planning run the risk of similar neglect by decision-makers unless vigorous efforts are made to promote their use.

Abstract:
Learning - the process by which new skills and behaviours are acquired - is a central concept in the understanding of mental handicap. Discrimination learning Is specifically identified as an essential pre-requlslte for the acquisition of a variety of more complex behaviours and skills. Many studies have reported that severely mentally handicapped persons show a relative inability to profit from unstructured learning experiences, and frequently fail to learn discriminations trained through simple differential reinforcement (trial-and-error training). Programmed training procedures are identified as important vehicles to facilitate the discrimination learning of severely mentally handicapped children. An initial study is reported that confirms the superiority of programmed over trial-and-error discrimination training, and demonstrates this superiority is maintained over a series of similar discrimination tasks. The results also demonstrate that stimulus fading procedures can be successfully implemented using micro-computer technology, and that 'errorless' learning can be effected through the sequencing and fading of trainer prompts. However, programmed procedures are not invariably effective in teaching discriminations to developmentally impaired persons, and failures to learn following programmed training are not infrequently reported. In a series of studies, some of the basic procedural parameters underlying diverse programmed techniques were evaluated as determinants of the development (or lack of development) of appropriate stimulus controL Increasing the salience of S+, whether through manipulation of S+ or S-, was shown to be more effective than increasing the salience of S-. Further, congruence between prompt and training cue dimensions was shown to be a significant variable affecting the outcome of training. The effectiveness of programmed procedures was also shown to interact with task difficulty. Finally, wide individual differences in learning within the population of severely mentally handicapped persons have frequently been reported, and were also observed in the present research. In a final study, individual differences in some basic cognitive abilities and aspects of classroom behaviour were assessed and shown to be significantly correlated with discrimination performance. The results are related to the literature on compound conditioning (e.g., Kamin, 1968) and theories of discrimination learning (Terrace, 1966; Rescorla & Wagner, 1972; Zeaman and House, 1963, 1979), and suggest the importance of attentlonal processes in discrimination learning. Lastly, some educational implications of the results are discussed.

Abstract:
The horticulture industry offers numerous unskilled and semi-Skilled job opportunities for qualified handicapped individuals. A mail survey of 557 private Virginia ornamental horticultural businesses was conducted to document the employment of handicapped persons and to investigate employers' perceptions of mentally handicapped workers. A response rate of 60% was obtained. Forty-two percent of the respondents reported employing mentally, physically or emotionally handicapped persons. Primary businesses which have employed mentally handicapped workers were associated with grounds maintenance, nursery/garden centers, and golf courses. Overall the employers' perceptions were favorable of the general work habits and entry-level horticultural skill competencies of mentally handicapped persons. This indicated a potential for employment

Abstract:
Food choices, food purchases and convenience food preparation by a group of mildly intellectually handicapped students are investigated. These students attend John Grant High School, in Lachine, Quebec, for continued academic intervention and vocational training. Results indicate most of these young adults make food choices based on a desire for good nutrition. There is a significant correlation (p $<$ 0.05) between nutrition knowledge and choosing nutritious foods in this sample. Yet the knowledge needed to make sound food choices is insufficient, as is the knowledge of food marketing. Families are the major influence on food selection and preparation, but as a source of information are found to be lacking. At school, home economics contributed most to students' knowledge of foods. Students found food preparation instructions particularly difficult to follow; students who had repeatedly prepared foods had greater success in producing an acceptable outcome. For this population practice is needed in making optimal food choices, menu planning and food preparation.

Full text
Abstract:
This thesis is based on multiple observations that paint a picture of a collection of restrictions on social participation for people with handicaps related to mental illness. In the absence of a legal context which encourages the social participation of these individuals, documentary evidence shows that there are roadblocks in various domains, such as domestic, social, economic, professional, political, cultural and artistic domains. In this context, and in line with international terminologies, our thesis aims to define the conditions which enable support groups for individuals with mental handicaps (known in France as Groupes d’Entraide Mutuelle, or GEM for short) to strengthen the social participation of their members. We aim to measure the strength of this involvement both on an individual and a collective level. After an overview of the concepts of participation and social participation, we will propose a theoretical model that covers the process and results of social participation, which we will compare and contrast to what we observe in the context of our research. This research is comprised of open-ended questionnaires administered face-to-face with members of GEMs, fragmented ethnographic observations in twenty GEMs, and extensive participant observation in two GEMs. The analysis of the data, which was in large part qualitative, points to different forms of participation on the individual and collective level. With these forms in mind, we identify various conditions necessary for the emergence of social participation, linked both with the individual characteristics of members and the specific contexts of each GEM, whether they be spatial, organisational, relational and/or professional. Our analysis leads us to understand the individual process of social participation as an experience, whereas at the collective level, the process is a publicization of a set of problems and points of view which transforms each of the actors involved. In terms of results, we identify three levels of social participation as well as the emergence of different types of expertise. In conclusion, we modify our original theoretical model and propose a new transactional model of the process of social participation and its results

Abstract:
In a competitive society where the culture of productivity and profit prevails, how do the mentally handicapped fit in? It is our main goal, in this dissertation, to provide answers to this question. The contribution of Jean Vanier in refinding the value, the gift, the treasure of those who are marginalized by society and that, unlike the social paradigmas, through their wounded, suffered and desfigurated body may reveal to mankind the true human dimension. Wounded body that by being reached can transform either the one who reaches as well as the recheable. To present the wounded and vulnerable body of those who are mentally handicapped as a physical and theological place opens our minds to others and revals us the hidden face of God amongst the most fragile and vulnerable human beings.

Abstract:
The purpose of this study was to investigate the possibility of implementing community based care for moderately mentally retarded persons in a specific centre in the Port Elizabeth area. The objectives of the study were to identify • the needs of the moderately mentally retarded children. • the physical, psychosocial and vocational rehabilitation means of meeting these needs. • how involved the communities, parents and government were in the care of moderately mentally retarded children. A quantitative, exploratmy and descriptive design was used. A sample of 50 moderately mentally retarded children was utilised. Data were collected by means of questionnaires and semi-structured interviews. The major research findings indicate that community care for MMRPs could only be feasible with sufficient resources, expertise and community involvement. All these aspects appeared to be lacking in the centre where this research was conducted raising questions as to benefits which the mentally retarded children and their parents and the community could derive from these services. However recommendations were made on identified shortcomings, problems and needs.

Background: Oral health condition of the handicapped is usually poor, and their oral hygiene needs to be improved. While this is largely due to their own physical limitations, care givers who lack proper oral health knowledge may also be attributable for this unfavorable condition. Oral health education is commonly promoted in kindergartens and elementary schools in Taiwan. However, we lack proper oral health education for the care givers of the handicapped. Study objective: The aim of this study is to intervene on the oral health education for the care givers in one handicapped institution. An intervention study is then investigated based on the changes of knowledge, attitude and behavior of the care givers, as well as on the changes of oral cleaning effect of the handicapped. The contributing factors of these changes are also investigated. Method: The objects of this study are the 21 care givers and 61 handicapped residents in the “Home for the Disabled, Social Affairs Bureau, Kaohsiung City Government”. All care givers will receive 4 hours of training in oral hygiene instruction and 13 hours of teaching regarding clinical tooth brushing techniques by expertise in Special Care Dentistry. Research protocols include surveys on: lifestyles of the residents, oral examination charts, and the knowledge, attitude and behavior of care givers on oral health education. Duration of the research lasts about 5 months. Surveys were collected from the care givers before the education, immediately after the education and 3 months after the education. Oral cleaning conditions were also investigated in the first, seventh and fourteenth week. Statistical methodology includes the t-Test, Chi-Square test, and Multi-Regression Analysis. Results: In surveys collected immediately after and 14 weeks after the oral hygiene intervention, statistically significant differences (p<0.0001) were demonstrated in care givers’ score in oral hygiene knowledge and attitude; and in changes in the oral cleaning effect of the residents. The tooth brushing technique and frequency of tooth brushing of the care givers also improved significantly. After Multiple Regression Analysis was performed, only gender of the care givers were related to the changes of the care givers’ oral hygiene knowledge. On the other hands, changes in attitude of the care givers and changes in the oral cleaning effects of the residents were mainly caused by oral hygiene intervention, and were not related to their backgrounds. Conclusion: Statistical analysis demonstrates that oral hygiene intervention was effective, and that this education program can be a reference to the promotion of Special Care Dentistry in the future. Preventive Dentistry includes both home care and routine oral examination; therefore, care givers and dental professionals are equally important. Cooperation will be necessary to help the handicapped to maintain a good oral hygiene condition.
