Day care of mentally handicapped people fulfils the need of both the person and the community. The transmission of culture which occurs through nurturing, education and social integration in the case of ordinary people is also necessary in the case of a mentally handicapped person. This alone can eventually lead the mentally handicapped person to prepare himself for the role of an adult in the community. Day care plays an important part in facilitating this transmission because, contrary to common belief, mentally handicapped people can learn better during their early adult life. Yet factors within the community itself make this provision less effective. The aim of this article is to review day care in the context of its need and place in social organization and the problems in the community at present.

AbstractObjective: To study the relationship between epilepsy and aggression in a population of severely mentally handicapped people. Methods: Comparing epilepsy and aggression variables in people with epilepsy without aggression, people with aggression without epilepsy, and people with both. The epilepsy variables were: seizure frequency, classification, anticonvulsant drugs, therapeutic drug monitoring, and neuroleptic drugs. Aggression variables were: frequency, direction, type, and neuroleptic drugs. Results: Prevalence of aggressive behaviour was similar in people with and without epilepsy (36%). Partial seizures were less prevalent in people with epilepsy and aggression. People with epilepsy were more likely to manifest unprovoked aggression directed against property. Conclusion: Some episodes of aggressive behaviour in people with epilepsy may be ictal in origin.

In spite of all the publicity in recent years about child abuse, very little mention has been made of another group of people who are also very vulnerable to abuse, namely adults with a mental handicap. Although chronologically adults, and therefore not susceptible to the laws designed to protect children, they may be functioning intellectually and emotionally at a much lower level. Because they are often dependent on other people for many aspects of their care, this makes them vulnerable to physical and sexual abuse and neglect. Difficulties with speech and communication may render them unable to inform anyone of their plight or ask for help, therefore many problems remain hidden. Personal involvement in one such case made me aware of this problem and the current lack of legal protection for this group of people. I therefore undertook a national survey in order to gain more information on the prevalence of abuse, factors associated with it, and possible legal solutions.

With mentally handicapped people now being moved into the community, information on mentally handicapped criminal offenders is valuable. A group of such offenders was examined by reviewing the case-notes of 92 patients referred on hospital order to two mental-handicap hospitals over 10 years. Compared with the general criminal population, these offenders' ages were higher, the ratio of male to female offenders was similar, and the proportion of married people was lower. The offences committed were for the most part serious, with a greater number of offences against property and public order in the subgroup whose intelligences were in the mentally handicapped range. Their tested intelligences fell almost entirely into the normal and mild mental-handicap range, suggesting that factors other than intelligence testing, such as social skills, were considered in their admission to the hospitals.

The management of mentally handicapped offenders has long been problematic. Three misconceptions of the relationship between mental handicap and criminality have been widely held since the turn of the century, despite there being no conclusive evidence in their support (Jackson, 1983). These are: that mentally handicapped people are more likely than others to commit antisocial acts in general; that they have a particular predisposition to commit serious crime, especially sexual crime (Robertson, 1981); and that they are unlikely to be deterred by normal sanctions. Misunderstanding breeds misapprehension. Sadly, in the past, many mentally handicapped people were admitted to hospital after committing only trivial offences. Moreover, one Special Hospital study (Parker, 1974) found that most “severely subnormal and subnormal” detained patients actually had IQs above the category to which they had been assigned.

SynopsisThis paper examines the quality of care in four types of residence for mentally handicapped adults in Britain. Data were gathered from 175 residents of 150 living units – mental handicap hospital wards, voluntary and private homes, local authority hostels and parental homes. Differences were apparent between the types of residence in terms of both residents' characteristics and care practices and routines. The findings indicate that some of the differences in care practices are not associated with differences in the residents' functioning. Additionally, alternatives to hospital care for mentally handicapped people are not providing demonstrably improved quality of care in several aspects of residential practice. A discussion of those aspects of care calling for more improvement is presented.

A prototype to improve learning tasks for mentally handicapped people is shown in this research paper using ambient intelligence techniques and based on cyber-physical systems. The whole system is composed of a worktable, a cyber-glove (both with several RFID and NFC detection zones), and an AmI software application for modeling and workflow guidance. A case study was carried out by the authors where sixteen mentally handicapped people and 3 trainers were involved in the experiment. The experiment consisted in the execution of several memorization tasks of movements of objects using the approach presented in this paper. The results obtained were very interesting, indicating that this kind of solutions are feasible and allow the learning of complex tasks to some types of mentally handicapped people. In addition, at the end of the paper are presented some lessons learned after performing the experimentation.

The prevalence of psychiatric illness was studied in 150 epileptic mentally handicapped people (both hospital in-patients and living in the community) and a matched group of 150 non-epileptic controls. The Profile of Abilities and Adjustment (PAA) scale was used for the initial screening of psychiatric illness. Mildly to moderately handicapped individuals who had good communication skills and scored positively on the PAA schedule for psychiatric illness were interviewed using the PSE interview schedule. Severely mentally handicapped individuals who scored positively on the PAA's psychiatric illness subscale were observed and information was gathered from their medical notes and carers. A psychiatric diagnosis was made using DSM–III–R criteria. The non-epileptic group showed significantly more psychiatric illness than the epileptic group. Psychiatric illness was diagnosed in 25% of the cohort.

One hundred and fifty mentally handicapped people (100 from hospital and 50 from the community) with epilepsy were studied along with an individually matched control group of 150 (100 from hospital and 50 from the community) non-epileptic mentally handicapped people. Behaviour was studied using the Profile of Abilities and Adjustment Schedule. Of the total population, 55.3% showed some type of severe behaviour problem. Although the epileptics showed slightly more severe behaviour problems than the non-epileptic group, there was no statistically significant difference between the two groups. Some differences emerged between the groups when subgroups of epileptics were studied.
