Chapter 8 discusses how Fragile X syndrome (FXS) is caused by the absence of the RNA-binding protein fragile X mental retardation protein (FMRP). FMRP is highly expressed in the brain and gonads, the two organs mainly affected in patients with the syndrome. The solution of Schrodinger’s equation is discussed for a model in which atoms are represented by potential wells, from which the band structure follows. Three further models are discussed, the Ziman model (which is based on the effect of Bragg reflection upon the wave functions), and the Feynman model (based on coupled equations), and the tight binding model (based on a more realistic solution of the Schrödinger equation). The concept of effective mass is introduced, followed by the effective number of electrons. The difference between metals and insulators based on their band structure is discussed. The concept of holes is introduced. The band structure of divalent metals is explained. For finite temperatures the Fermi–Dirac function is combined with band theory whence the distinction between insulators and semiconductors is derived.

Abstract:
This article discusses the modulation design of plasmonics for diagnosis and drug screening applications. It begins with an overview of the advances made in terms of theoretical insights, focusing on the origins of surface plasmon wave and manipulation, admittance loci design method, and surface plasmon grating coupled emission. It then considers how prism coupler, Ge-doped silica waveguide, nanograting and active plasmonics can trigger the excitation of surface plasmon resonance (SPR). It also examines the metallic effect of long-range surface plasmon resonance and conducting metal oxide as adhesive layer before describing three SPR waveguide biosensors that were developed for the realization of a hand-held SPR system. In particular, it presents a lateral-flow microfluidic channel based on a nitrocellulose membrane and integrated with a SPR waveguide biosensor to achieve dynamic detection. Finally, the article evaluates the biomolecular layer effect, with emphasis on kinetics analysis of antibody binding.

This chapter continues consideration of reductive intentionalism without embracing the doctrine, framing it in the context of cognitive science. Cognition, including perception, is representation. An agent’s cognitive, perhaps perceptual, state is a relation binding the agent to a proposition by means of her mental representation. Intentionalism would explicate the phenomenal character of a perceiver's experience in terms of the content of her prevailing perceptual representation. While minimal intentionalism maintains that the phenomenal character of the perceiver's experience merely supervenes on her representation's content, maximal intentionalism would reduce character to content. For maximal intentionalism maintains that phenomenal character is simply what introspection finds. Yet, according to maximal intentionalism, introspection, when tuned to conscious perception, detects only the content of experience. Hence, the maximalist identifies phenomenal character with the content carried by perceptual representation.

Anaphors such as English herself, French elle-même, and Mandarin ziji are usually claimed to obey locality requirements stated by Condition A of Binding Theory. But we observe that in various languages, the same anaphors can be exempt from these locality requirements under certain conditions. The goal of this book is to describe and explain this widespread dual behavior of anaphors on the basis of French, English, Mandarin, Korean, and Icelandic. First, several strategies are proposed for distinguishing between the two possible behaviors of anaphors. Plain instances of anaphors require local and exhaustive binding, as well as sloppy readings in ellipsis. Exempt instances of anaphors, however, only require a logophorical interpretation, that is, to occur in phrases expressing the first-personal, mental perspective of their antecedent. Second, a new theory of exempt anaphora is proposed, which consists in deriving all properties distinguishing exempt from plain anaphors to one: the presence of a silent, syntactically represented logophoric operator introducing a local, perspectival binder for superficially exempt anaphors. This hypothesis parsimoniously reduces exempt to plain anaphors obeying Condition A, thus directly accounting for the cross-linguistically widespread morphological identity of plain and exempt anaphors. Under this proposal, the reason why exempt anaphors appear to escape locality requirements is that their binder is implicit, and their mandatory logophoric interpretation derives from the nature of this binder. Finally, several diagnostics are provided for testing the hypothesis that so-called long-distance anaphors can be analyzed just like exempt instances of anaphors.

Abstract:
This chapter discusses six diseases caused by inborn errors of metabolism affecting the biosynthesis of haem. Haem is a tetracyclic metal-binding compound involved in oxygen transport (in haemoglobin and myoglobin) and redox reactions (e.g. in the cytochrome P450 system). Each of these conditions is caused by a single gene defect in one of the enzymes involved in the biosynthesis of haem. Inheritance is usually autosomal dominant with incomplete penetrance. The enzyme defect results in disease, not as a result of deficiency of the reaction product, but as a result of accumulation of precursors. Early, soluble precursors, 5-aminolaevulinic acid, and porphobilinogen (not porphyrins as such) are neurotoxic and, when present in great excess, as occurs when flux through the haem synthetic pathway is increased in response to particular medications or hormones, lead to acute neurovisceral crises. Later cyclical precursors (porphyrins) in the pathway are also water soluble and excreted in urine, but are susceptible to activation by electromagnetic radiation in the visible spectrum and are converted to free-radical metabolites that cause pain, inflammation, and tissue damage in the skin. The final haem precursors (also porphyrins) are hydrophobic and excreted in the bile and faeces and are also activated by light to toxic metabolites.

Abstract:
Attention to the human rights of the elderly has gained increased international attention. This chapter provides an overview of some international agencies’ efforts to address human rights of elders, as well as related efforts in different regions of the world. The UN does not have a specific treaty or convention regarding human rights of the elderly; only in 1995 was comprehensive legal analysis of the rights of the elderly addressed. The 2002 Madrid International Action Plan on Aging was a non-legally binding declaration by participating governments. Article 14 addresses equal access to healthcare and services, including physical and mental health services. The Commission on Human Rights 2011 Special Rapporteur emphasized human rights issues in primary healthcare and chronic illness; long-term care; palliative care; and informed consent as related to older persons. The Alzheimer’s Disease International Kyoto Declaration provides countries with a framework of action to address the needs and quality of life of those with dementia and their caregivers. Similarly, the United States has developed a national plan to address Alzheimer’s disease.

A debate on public goods is urgently needed in health care. Care must be recognized as a social function, as an occupation and, at the same time, as a human right—which imposes binding obligations to comply with precise standards of quality, quantity, suitability, adaptability, and accessibility, among others. It is a complex and invisible task, that may be done as part of a medical treatment, post-surgical recovery process, or permanent support in cases of chronic illness, disability, or mental health conditions. And it tends to be provided mainly in the home, by women, without remuneration. In Latin America, care has not been included in a coordinated and specific public health policy agenda but has been advanced through isolated actions—in many cases highly fragmented and heterogeneous—without a clear awareness of the public nature of care and the associated responsibility of the State. Accordingly, this document takes a gender and rights-based approach. It starts with an analysis of the main definitions of unpaid work in the health sector, and then focuses on initiatives in three Latin American countries (Colombia, Costa Rica, and Uruguay) with regard to measurement, valuation, integration, and recognition in national health systems or policies, in care models, and in time-use surveys. The conclusions propose recommendations aimed at addressing unpaid care as an essential element of social policies in general, and health policies in particular, from a gender and rights-based perspective.
