In the Spring 2001 issue of Journal of Information Systems (JIS), Borthick et al. (2001) presented a case in which students were to query a Microsoft Access database in order to determine the mutual assurance concerning customer service for an automobile manufacturer and its local dealer network. I follow-up this case by presenting an additional teaching note to the case that develops query strategies and solutions for the case using Oracle and its SQL application, SQL-Plus.

Abstract:
PT Mitra Metal Sejati merupakan salah satu perusahaan yang bergerak dalam bidang jasa mekanikal dan machining fabrikasi. Dalam melakukan kegiatan usahanya perusahaan membutuhkan pembelian bahan baku material untuk dapat menjalankan kegiatannya. Pencatatan pembelian kredit merupakan bagian penting dalam suatu perusahaan karena nantinya pencatatan tersebut berkaitan dengan hutang yang merupakan kewajiban sebuah perusahaan. Tujuan penelitian ini menyajikan informasi pembelian dan hutang secara terkomputerisasi yang cepat dan akurat. Metodologi penelitian yang penulis gunakan terdapat beberapa tahapan diawali dengan perencanaan, pengumpulan data, pengolahan data, dan migrasi data ke aplikasi MYOB Accounting Plus V18. Pencatatan pembelian bahan baku material pada PT Mitra Metal Sejati masih menggunakan Microsoft Excel dan belum terkomputerisasi sehingga sering terjadi kesalahan. Kesalahan atau permasalahan yang terjadi pada PT Mitra Metal Sejati seperti: penginputan hutang usaha dan membuat Purchase Order (PO) serta laporan pembelian kredit baik nama supplier, nomor invoice masuk dan nominal masih menggunakan Microsoft Excel sehingga kadang terjadinya kesalahan penginputan data. Adapun dari permasalahan-permasalahan tersebut dan dengan semakin berkembangnya perusahaan membutuhkan Sistem Informasi yang baik untuk menunjang kegiatan-kegiatan usahanya khususnya dalam pembelian kredit yang nantinya berhubungan dengan utang perusahaan. Mengimplementasikan suatu penyajian laporan pembelian kredit dan utang pada PT Mitra Metal Sejati menggunakan aplikasi akuntansi MYOB Accounting Plus V18. Sehingga dapat mempermudah, mempercepat, dalam pencatatan pembelian kredit serta menyajikan laporan pembelian dan utang, dan laporan tersebut dapat digunakan sebagai pengambilan keputusan oleh manajemen.

Penelitian ini dilakukan pada Toko Accurate Jaya Computer di Jl. Yos Sudarso , Teluk Llingga Kota Sangatta. Metode pengumpulan data yang digunakan yaitu studi lapangan dan studi pustaka. Untuk metode pengembangan sistem yang digunakan yaitu waterfall. Untuk metode analisis dan desain sistem penulis menggunakan Data Flow Diagram (DFD), Hierarchy Plus Input Output (HIPO), Entity Relationship Diagram (ERD) serta struktur Database input output system. Perangkat lunak yang digunakan adalah Microsoft Visual Basic 6.0 sebagai bahasa pemrograman, Seagate Crystal Report 8 untuk pembuatan laporan, dan Microsoft Access 2007 untuk pembuatan database. Untuk Ujicoba/Testing Penulis Menggunakan Black Box dan White Box. Dari hasil penelitian ini diperoleh sebuah aplikasi berbasis Microsoft Visual Basic 6.0 yang berbasis client-Server, yang bertujuan untuk mengatasi berbagai kendala dalam hal-hal yang menyangkut servis barang pada Toko Accurate Jaya, baik dalam hal pendataan barang masuk, pendataan pelanggan, inventarisasi biaya kerusakan, proses persetujuan servis dan konfirmasi kerusakan terhadap konsumen, hingga pembuatan laporan servis barang. Tujuan dari fungsi client -server pada aplikasi itu sendiri dimaksudkan agar data dapat diakses oleh semua karyawan, baik administrator maupun teknisi pada satu perangkat server sehingga tidak perlu dilakukan cross-check atau sinkronisasi data.

Abstract:
In this paper, hydrochlorothiazide (HCT) and losartan potassium (LSP) in tablets are simultaneously determined with UV-Vis spectrophotometry and chemometrics without separation. The spectra of standard and sample solutions were recorded in the wavelength from 220 to 300 nm at 1.0 nm intervals. The concentrations of HCT and LSP in the sample solutions were computed with the Kalman filter algorithm written on the Microsoft Excel 2016 and Visual Basic for Applications (VBA) platform. The method validation was determined via the accuracy and repeatability of measurements when analyzing HCT and LSP in the Splozarsin Plus tablet and comparing the mean values of their contents in the sample with those analyzed with HPLC. The proposed method is simple with a low cost compared with the standard HPLC method.

Polyoxymethylene dimethyl ethers (OMEn) are frequently discussed as alternative diesel fuels, with various synthesis routes considered. OME3–5 syntheses demand significant amounts of thermal energy due to the complex separation processes that they entail. Therefore, innovative process designs are needed. An important tool for the development of new processes is process simulation software. To ensure sound process simulations, reliable physico-chemical models and component property data are necessary. Herein we present the implementation of a state-of-the-art thermodynamic model to describe the component systems of formaldehyde-water and formaldehyde-methanol using Microsoft® Excel (2010, Microsoft Corp, Redmond, WA, USA) and Aspen Plus®, (V8.8, Aspen Tech, Bedford, MA, USA) determine the deviation between the calculated results and experimental literature data, and minimize the deviation by means of parameter fitting. To improve the accuracy of the estimation of the missing property data of hemiformals and methylene glycols formed from formaldehyde using group contribution methods, the normal boiling points were estimated based on molecular analogies. The boiling points of OME6-10 are determined through parameter regression in accordance with the vapor pressure equation. As an application example, an optimization of the product separation of the state-of-the-art formaldehyde synthesis is presented that helps decrease the losses of methanol and formaldehyde in flue gas and wastewater.

Electron microscopy has a pivotal role as an analytical tool in pharmaceutical research. However, digital image data have proven to be too large for efficient quantitative analysis. We describe here the development and application of an automated image processing (AIP) program that rapidly quantifies shape measurements of recombinant adenovirus (rAd) obtained from digitized field emission scanning electron microscope (FESEM) images. The program was written using the macro-recording features within Image-Pro® Plus software. The macro program, which is linked to a Microsoft Excel spreadsheet, consists of a series of subroutines designed to automatically measure rAd vector objects from the FESEM images. The application and utility of this macro program has enabled us to rapidly and efficiently analyze very large data sets of rAd samples while minimizing operator time.

Background: General anaesthesia and regional anaesthesia both are used for upper extremity surgeries. Bier’s Blockusing local anaesthetic alone or in combination with other adjuvants provides effective analgesia intraoperatively as wellas postoperatively with fewer complications as compared to general anaesthesia.Objective: This study was designed to compare effectiveness, haemodynamic alterations and total duration of analgesiawith Lidocaine versus Lidocaine plus Verapamil in Bier’s Block.Methods: This is a prospective randomised double blind trial conducted in Kathmandu Medical College TeachingHospital from February 2012 to December 2012 after approval from the ethical committee and informed consent fromthe participants. Total 40 adult patients of age 20 to 50 years, weight 50 to 70 kg, of both gender, belonging to AmericanSociety of Anaesthesiologists Physical Status I and II undergoing elective distal upper extrimity surgery lasting at one toone and half hours with Bier’s Block were included in this study. They were randomly divided into two groups of twentyeach to receive either 40 ml of 0.5% Lidocaine alone (Group A) or 40 ml of 0.5% Lidocaine plus 2.5 mg Verapamil (GroupB). The two groups were compared in terms of onset and recovery from sensory and motor blockade, tourniquet paintolerance time, duration of analgesia, alteration of haemodynamics and major side effects. Data analysis was done byMicrosoft Offi ce Excel 2007 [Polystat, Microsoft Offi ce Excel worksheet.XLS] using student’s two tailed t test. Categoricalparamatres were tested by Fisher Exact test and p-value of <0.05 was considered statistically signifi cant.Result: Onset of sensory blockade was faster in group B (Mean± SD: 3.07± 0.25 minutes) as compared to group A (Mean±SD: 5.59 ± 0.41 minutes). The onset of motor block in group B was 10.4 ± 0.77 minutes versus 13.17 ± 1.45 minutes ingroup A. Recovery of sensory block in group B occurred at 29.95 ± 6.96 minutes versus 11.45 ± 2.16 minutes in groupA. Similarly recovery of motor block in group B occurred in 13.6 ± 1.79 minutes versus 7.65 ± 1.04 minutes in group A.Tolerance of fi rst tourniquet pain in group B was long 41.15 ± 3.82 minutes versus 22.00 ± 2.9) minutes in group A. Secondtourniquet pain tolerance time was again longer (48.25 ± 3.96 minutes) in group B versus 28.05 ± 4.84 minutes in groupA. Total duration of analgesia was more in group B (207.25 ± 21.1 minutes) versus 32.2 ± 5.78 minutes in group A. Totalconsumption of analgesic (Tramadol) in 24 hours in group B was lesser than group A (47.5 ± 38.0 mg versus 112.5 ± 35.8mg). All these differences were signifi cant statistically (p value <0.05). L’objectif de cet article est d’opérer une lecture au-dedans et au-dehors de la fiction mise en place par le cinéaste Brian Flemming dans son documentaire Nothing So Strange (GMD Studios, 2002), ainsi que dans les documents d’archive et les artéfacts web produits en son sillage. L’univers Nothing So Strange s’attache à décrire l’un des événements historiques les plus importants à n’avoir pas eu lieu au xxe siècle : l’assassinat du président de Microsoft Bill Gates. Nous tenterons de voir comment une grande oeuvre participative (au sens de Jenkins) s’est développée à partir du film de Flemming, témoignage hyperréaliste issu d’un univers parallèle ; comment cinéastes, spectateurs et internautes ont détourné la capacité documentaire de l’archive filmique et numérique afin de se réapproprier la figure historique du complot meurtrier et fournir à la fiction un grand déploiement tentaculaire. Nous interrogerons également les enjeux éthiques et esthétiques de cette stratégie, que Flemming lui-même qualifie de piratage de la réalité («reality-hacking»).

Pada penelitian ini telah dibuat Sistem Informasi Pengelolaan Transportasi Pengiriman BBM dengan desain sistem mengunakan Flow Of Document (FOD), Data Flow Diagram (DFD) dan Hirachy Plus Input Process Output (HIPO). Metode pengembangan sistem yang digunakan yaitu SDLC dan metode pengujian menggunakan Black Box. Aplikasi Ini dibangun menggunakan software yaitu Bahasa Pemrograman Visual Basic 6.0, database yang digunakan Microsoft Access 2007. Sistem yang dibangun beroperasi pada Sistem Operasi Windows. Dari hasil implementasi sistem, disimpulkan bahwa dengan penggunaan software dapat membantu dalam proses pengiriman BBM pada PT. Ratah Indah Samarinda. Serta penulis memberikan saran agar PT. Ratah Indah Samarinda membuat koneksi jaringan komputer ke masing-masing bagian untuk memudahkan pengolahan data.

Organic food products are produced without pesticides or genetic engineering. This study analyses the variable product characteristics, website characteristics, Instagram account characteristics, and claims characteristics. This study aimed to analyze the content of claims for organic food on websites and Instagram stores based on the Consumer Protection Law No. 8 of 1999. This study used an exploratory design with content analysis methods. The sample was selected using purposive sampling, namely 100 online shops that sell organic food with details of 50 websites and 50 Instagram accounts. The data collected was categorized and processed using Microsoft Excel, Statistical Package for Social Science (SPSS) 23.0 and Nvivo 12 Plus. The analysis results show that 56% of website and Instagram stores use subjective claims and 40% using objective claims. As much as 47% of online stores, both websites and Instagram, are categorized as not misleading, and 53% are categorized as misleading or deceptive claims such as ambiguous. 22% of online shops violate Article 8 Paragraph 1, and 20% of online stores violate Article 9 of the Consumer Protection Law.

AbstractIn the medical education field, the use of highly sophisticated simulators and extended reality (XR) simulations allow training complex procedures and acquiring new knowledge and attitudes. XR is considered useful for the enhancement of healthcare education; however, several issues need further research.The main aim of this study is to define a comprehensive method to design and optimize every kind of simulator and simulation, integrating all the relevant elements concerning the scenario design and prototype development.A complete framework for the design of any kind of advanced clinical simulation is proposed and it has been applied to realize a mixed reality (MR) prototype for the simulation of the rachicentesis. The purpose of the MR application is to immerse the trainee in a more realistic environment and to put him/her under pressure during the simulation, as in real practice.The application was tested with two different devices: the headset Vox Gear Plus for smartphone and the Microsoft Hololens. Eighteen students of the 6th year of Medicine and Surgery Course were enrolled in the study. Results show the comparison of user experience related to the two different devices and simulation performance using the Hololens.

Abstract Background The specific characteristics of copy number variations (CNVs) require specific methods of detection and characterization. We developed the Easy One-Step Amplification and Labeling procedure for CNV detection (EOSAL-CNV), a new method based on proportional amplification and labeling of amplicons in 1 PCR. Methods We used tailed primers for specific amplification and a pair of labeling probes (only 1 labeled) for amplification and labeling of all amplicons in just 1 reaction. Products were loaded directly onto a capillary DNA sequencer for fragment sizing and quantification. Data obtained could be analyzed by Microsoft Excel spreadsheet or EOSAL-CNV analysis software. We developed the protocol using the LDLR (low density lipoprotein receptor) gene including 23 samples with 8 different CNVs. After optimizing the protocol, it was used for genes in the following multiplexes: BRCA1 (BRCA1 DNA repair associated), BRCA2 (BRCA2 DNA repair associated), CHEK2 (checkpoint kinase 2), MLH1 (mutL homolog 1) plus MSH6 (mutS homolog 6), MSH2 (mutS homolog 2) plus EPCAM (epithelial cell adhesion molecule) and chromosome 17 (especially the TP53 [tumor protein 53] gene). We compared our procedure with multiplex ligation-dependent probe amplification (MLPA). Results The simple procedure for CNV detection required 150 min, with &lt;10 min of handwork. After analyzing &gt;240 samples, EOSAL-CNV excluded the presence of CNVs in all controls, and in all cases, results were identical using MLPA and EOSAL-CNV. Analysis of the 17p region in tumor samples showed 100% similarity between fluorescent in situ hybridization and EOSAL-CNV. Conclusions EOSAL-CNV allowed reliable, fast, easy detection and characterization of CNVs. It provides an alternative to targeted analysis methods such as MLPA.

Objective We sought to identify the percentage of nonmusculoskeletal and musculoskeletal conditions treated by interns in the National University of Health Sciences (NUHS) Student Clinic compared to chiropractic and allopathic health care professionals. Methods The information gathered was taken from the charts of patients treated in the fall trimester, dated September 12, 2011 through December 9, 2011. The data collected included ICD-9 codes for the conditions treated, the number of patient visits, age, and gender, and was evaluated using Microsoft Excel. Results Over half of the 113 eligible patients were women with a mean patient age of 28 years, an average of three treated diagnoses, and a mean of seven treatments. Those treated only for musculoskeletal conditions totaled 52% of the patients; 48% of the patients were treated for nonmusculoskeletal conditions, or musculoskeletal plus nonmusculoskeletal conditions. Conclusion The NUHS Student Clinic interns are treating a greater percentage of nonmusculoskeletal conditions and a lesser percentage of musculoskeletal conditions than practicing chiropractic physicians. The student interns also treat a lesser percentage of nonmusculoskeletal and a greater percentage of musculoskeletal conditions than allopathic practitioners. This comparison would suggest that NUHS is nearing its institutional goal of training its student interns as primary care practitioners.

Abstract:
Purpose: The aim of this work was to analyse all the applied movements when extracting healthy upper and lower jaw premolars for orthodontic purposes. The authors wanted to demonstrate that the different bone densities of the mandible and maxilla are not a significant parameter when related to the extraction force applied. The buccal and palatal rocking movements, plus the twisting movements were also measured in this in-vivo study during premolar extraction for orthodontic purposes. Methods: The physical strains or forces transferred onto the teeth during extraction are the following three movements: gripping, twisting, and traction. A strain measurement gauge was attached onto an ordinary dentistry plier. The strain measurement gauge was constituted with an extensimetric washer with three 45º grids. The system operation was correlated to the variation of electrical resistance. Results: The variations of resistance (∆R) and all the different forces applied to the teeth (∆V) were recorded by a computerized system. Data results were processed through Microsoft Excel. The results underlined the stress distribution on the extracted teeth during gripping, twisting and flexion. Conclusions: The obtained data showed that the strength required to effect teeth extraction is not influenced by the quality of the bone but is instead influenced by the shape of the tooth’s root.

Le développement d’un système d’informations géografiques (SIG) tridimensionnel a besoin d’importer des éléments solides créés en trois dimensions. L’acquisition de ces éléments est généralement effectuée au moyen d’un LIDAR (Light Detection And Ranging). Une autre possibilité est la cartographie des façades de bâtiments, qui en règle générale est réalisée par la prise d'images géocodées au moyen d’un GPS et d’un système inertiel. Ensuite, un laborieux travail d'édition graphique doit être effectué. Evidemment, si le niveau de détail de la représentation des éléments est plus grand, le temps et le coût de l'acquisition de données 3D seront aussi très grands. Cette forme de développement d'unprojet de SIG 3D pour les applications comme la distribution d'énergie électrique est encore peu viable pour la réalité brésilienne en raison des coûts élevés d'acquisition 3D. Ainsi, pour rendre viable l'utilisation de ces outils dans un environnement SIG 3D dans une entreprise d'énergie électrique nationale, nous avons développé une méthodologiealternative à bas coût qui sera présentée dans ce travail. L'alternative est l'utilisation de la restitution photogrammétrique et l'utilisation des données suivantes : des photographies aériennes d’archive à l'échelle 1:8.000; une base cartographique plani-altimétrique permettant l’orientation externe d’images à l'échelle 1:2.000 pour l’obtention du modèle stéréoscopique et la restitution des éléments importants; une image multispectrale à haute résolution du satellite Quick BIRD, utilisée comme toile de fond et pour la texturation du modèle numérique du terrain. Le développement du projet a nécessité l’utilisation de logiciels et bibliothèques Opensource comme l'ensemble VTP (Virtual Terrain Project), qui inclut OGR, GDAL, vtdata et vtlib compatibles avec MFC (Microsoft Foundation Class), GDI+ (Graphics Device Interface Plus) ainsi que la base de données PostgreSQL. La modélisation des données a été réalisée de manière à simplifier l'acquisition des données tridimensionnelles et donc, réduire les coûts, dans le but d’appliquer le SIG 3D aux réseaux dedistribution d'énergie électrique. Des analyses de qualité du positionnement relatif ont ensuite réalisée, les résultats sontprésentés dans cet article.

Abstract Purpose To review the economic evaluation of the guideline implementation in primary care. Data sources Medline and Embase. Study selection Electronic search was conducted on April 1, 2019, focusing on studies published in the previous ten years in developed countries about guidelines of non-communicable diseases of adult (≥18 years) population, the interventions targeting the primary care provider. Data extraction was performed by two independent researchers using a Microsoft Access based form. Results of data synthesis Among the 1338 studies assessed by title or abstract, 212 qualified for full text reading. From the final 39 clinically eligible studies, 14 reported economic evaluation. Cost consequences analysis, presented in four studies, provided limited information. Cost-benefit analysis was reported in five studies. Patient mediated intervention, and outreach visit applied in two studies showed no saving. Audit resulted significant savings in lipid lowering medication. Audit plus financial intervention was estimated to reduce referrals into secondary care. Analysis of incremental cost-effectiveness ratios was applied in four studies. Educational meeting evaluated in a simulated practice was cost-effective. Educational meeting extended with motivational interview showed no improvement; likewise two studies of multifaceted intervention. Cost-utility analysis of educational meeting supported with other educational materials showed unfavourable outcome. Conclusion Only a minor proportion of studies reporting clinical effectiveness of guideline implementation interventions included any type of economic evaluation. Rigorous and standardized cost-effectiveness analysis would be required, supporting decision-making between simple and multifaceted interventions through comparability.

Flow sensors are very essential in many aspects of our daily lives. Many of the industrial processes need a very consistent flow sensor to monitor and check for irregularities in their system. Therefore, flow sensor is an important tool for advanced operation in industrial environment. In this paper, the design and development of a 3D fabricated flow sensor was carried out using SolidWork 3D CAD. SolidWork Flow Simulation was used to model the effect the turbine flow sensor would have on a constant flowing water while MATLAB Simulink flow graph was created to visualize the effect of turbine flow sensor response with voltage input. Afterwards, the design was 3D printed using UP Plus 2 3D printer. The experimentation involved selection of sensors, coding to control the turbine flow sensor and automatic data logging and storage. During the design phase, the sensors and actuators were assembled using locally sourced material. Subsequently, under controlled laboratory environment, the turbine flow sensor was tested using a DC motor which was programmed to control the revolution per minute(rpm) of the turbine flow sensor. The rpm and velocity of the turbine flow meter was measured and stored in a database via Microsoft Excel using Cool Term Software. A total number of 517 readings were analysed to evaluate the performance of the turbine flow sensor. The result shows that the turbine flow meter is responsive to the motor input voltage and yielded accurate measurement of rpm and velocity of turbine flow meter.

BackgroundA rare progressive supranuclear palsy–like syndrome seemingly triggered by aortic surgery was first described in 2004. This largest case series to date describes the features of this syndrome.MethodsWe searched the Mayo Clinic electronic medical records using the advanced cohort explorer search engine for patients evaluated for neurologic symptoms after cardiac-aortic surgery in the past 30 years. Data were extracted to Microsoft Excel from the identified patients and included clinical and neuroimaging features and outcomes.ResultsTwenty-five patients met the inclusion criteria. All surgeries were performed under thoracic aortic bypass and deep hypothermia. Surgery included aortic aneurysm, aortic valve repair, and/or aortic dissection repair. Surgical records were unavailable, although surgery was documented in the Mayo record as uncomplicated in 60% of cases. In the remaining cases, no particular intraoperative or postoperative complications were documented at a high frequency. A typical triad was documented: supranuclear gaze palsy (SNGP; 100%), gait imbalance (80%), and dysarthria (96%). Part or all of the triad was observed before hospital discharge and stabilized over the course of days-weeks. A second phase of symptom worsening plus new symptoms developed up to a year later; this decline continued for up to several years before stabilization. Delayed epileptic seizures occurred in 32% of patients. Brain MRI revealed only nonspecific findings.ConclusionThis syndrome following adult thoracic aortic bypass surgery with deep hypothermia remains unexplained. It follows a biphasic course and is characterized by the triad of SNGP, unsteady gait, and a predominantly ataxic dysarthria.

Introduction : L’objectif principal était de comprendre l’influence de la télévision sur les mécanismes classiques de transmissions des acquis culturels aux jeunes. Méthodes : Nous avons procédé à l’interview de 8 parents, 13 adolescents (dont 9 filles) et 4 spécialistes de l’éducation à Baco-Djicoroni. Nous avons aussi mené des entretiens avec des responsables de programmes et d’émissions de l’ORTM. Une analyse de contenu a été réalisée sur les données qualitatives collectées. Microsoft Excel a été utilisé pour l’analyse quantitative. Résultats : Les personnes interrogées avaient un âge compris entre 30 à 50 ans (parents) et 12 à 15 ans (adolescents) pour la majorité. Tous les parents et adolescents ont affirmé avoir la télévision en famille à l’exception d’un parent qui avait plus de 50 ans. Elle était regardée en famille par la plupart des enquêtés. Les parents pensent que la télévision pérennise les valeurs culturelles tandis que les spécialistes de l’éducation (n=4) la conçoivent comme un outil perturbateur de la transmission des acquis culturels. Quant aux responsables des programmes et émissions de l’ORTM, ils affirment que la chaîne nationale propose des émissions participant à la transmission des valeurs culturelles aux jeunes. Aussi, la moitié d’entre eux pensent que ce médium assure la continuité des mécanismes classiques de transmission des acquis culturels. Conclusion : Les programmes de télévision doivent être revus pour influencer positivement la socialisation de la jeune génération et assurer la transmission des acquis culturels. Background: In Malaysia, breast cancer is the most common cancer in females and also the first most common cancer among population regardless of gender. The percentage of breast cancer detected at stage I and II was 61%, another 27% with locally advanced cancer and 11% with late stage metastatic cancer. From the total, 28% are human epidermal growth factor receptor 2 (HER2) positive and access to targeted therapy (trastuzumab) was very limited; only 19% of eligible patients could be treated. Aim: To determine the incremental cost-effectiveness ratio (ICER) between chemotherapy plus trastuzumab and chemotherapy alone as adjuvant treatment of early breast cancer from Malaysian perspective. Methods: A Markov cohort simulation was developed using Microsoft Excel Workbook 2007 to estimate the cost-utility of adjuvant trastuzumab compared with chemotherapy alone for treatment of early breast cancer with HER2 positive status. Two adjuvant treatment strategies were evaluated: 1) chemotherapy plus trastuzumab and 2) chemotherapy alone as adjuvant treatment. This Markov model was projected to lifetime horizon. All costs and outcomes were discounted at 3% and the cost-effectiveness result was expressed in ICER. One-way sensitivity analysis was performed to address the uncertainty. Results: The base case analysis indicated that 1-year adjuvant trastuzumab treatment generates a deterministic ICER of RM 83,544.59 per QALY gained. Over the lifetime, there is a marginal cost increase of RM 85,659.10 and a marginal benefit of 1.025 QALYs per patient when trastuzumab is added to standard chemotherapy compared with no trastuzumab strategy. Based on one-way sensitivity analysis, these components have shown to be a sensitive parameter for ICER determination: discount rate, disease-free state utility, route of trastuzumab administration and cost of trastuzumab. Conclusion: Addition of 1-year treatment with trastuzumab on top of standard adjuvant chemotherapy is considered as a cost-effective strategy for early breast cancer with HER2 positive, yielding an ICER of RM 83,544.59 per QALY gained, which is within the suggested value of cost-effectiveness threshold by WHO (1-3 times GDP per capita). However, if suggested threshold for Malaysia is taken into consideration, this treatment may not be a cost-effective strategy.

Objetivo: Analisar as concepções dos profissionais de enfermagem em relação à educação permanente e o quanto esse setor contribuiu para a qualificação das suas ações. Metodologia: Pesquisa qualitativa e quantitativa, descritiva e exploratória, realizada com 113 profissionais de enfermagem em um hospital público na região metropolitana de Belo Horizonte. A coleta de dados ocorreu no período de março a julho de 2016, por meio de questionário semiestruturado. Os dados foram tabulados no Microsoft Excel, versão Office Professional Plus 2010 e analisados de acordo com a análise de conteúdo temática. Resultados: 47% dos participantes responderam que a educação permanente tem como função promover aperfeiçoamento e 27% melhorar a assistência prestada aos pacientes. 100% a apontaram como parte fundamental do trabalho. A motivação para participar das atividades de educação permanente, para 98% dos participantes foi a necessidade de aprendizagem e aprimoramento e quanto aos fatores que dificultaram a participação, 56% se referiram a sobrecarga de trabalho. Conclusão: A partir das respostas dos participantes, foi possível perceber que a educação permanente trouxe contribuições quanto aos processos de atualização e capacitação dos profissionais, auxiliando nas atividades que favorecessem o aprendizado, troca de experiências, saberes e reflexão dos processos de trabalho. Constatou-se ainda que a equipe de enfermagem reconhece a importância da educação permanente para a qualificação das suas ações. Em conformidade com a literatura, também foi evidenciado nesse estudo que a sobrecarga de trabalho, cargas horárias exaustivas e falta de recursos materiais dificultaram a participação dos profissionais nas atividades de educação permanente. Absztrakt: Bevezetés: A De Quervain-féle tendinopathia a csukló-kéz régióját érintő betegség. A hüvelykujj mozgatása fájdalmassá válik, a kéz funkciója jelentősen romlik. A betegség a legújabb kutatások szerint inkább degeneratív, semmint gyulladásos eredetű. Első lépésként a kéz sínezése, nemszteroid gyulladáscsökkentő szerek és különböző fizikoterápiás kezelések alkalmazása javasolt. Hatástalanságuk esetén további lehetőség az ínhüvelybe fecskendezett szteroidinjekció és a műtéti úton végzett ínhüvelybemetszés. Célkitűzés: Kutatásunkban megvizsgáltuk, hogy az excentrikus tréninggel kibővített konzervatív kezelés megfelelő alternatíváját nyújthatja-e a jelenleg elfogadott kezelési lehetőségeknek. Módszer: Az excentrikus tréning 8 hétig tartott, melyet indokolt esetben 12 hetesre bővítettünk. A betegek (n = 9) a betanítást követően naponta többször végezték a tréninget, amit a heti találkozók alkalmával kontrolláltunk. Az 1., a 8., valamint a 12. heti találkozó során az inspekciót követően mértük az ízületi mozgástartományt, az izomerőt, a fájdalmas régiók számát, illetve elvégeztük a ’Numeric Pain Rating Scale’, a ’Quick Disabilities of the Arm, Shoulder and Hand’, valamint a ’Patient-Rated Wrist Evaluation’ kérdőívek felvételét. A méréseket páros mintás t-teszttel és ismételt méréses varianciaanalízissel elemeztük. Az elemzéseket IBM SPSS Statistics 25.0 és Microsoft Office Excel Professional Plus 2016 programmal végeztük; p<0,05 esetén tekintettük statisztikailag szignifikánsnak eredményeinket. Abstract:
Goal. To monitor the phytosanitary state of parks and gardens plantations in the Ukrainian Forest-steppe zone and to identify species consistence of the dominant invaders. Methods. Reconnaissances, collection of materials and other attendances were carried out by using generally known ecology and entomology methods using determinants and atlases. Accounting results were subjected to variation-statistical analysis by special application program package according to the statistics and computer graphics: Microsoft Excel 2010, Statgraphics plus. Results. As a result of the phytosanitary state of the parks and gardens plantations in the forest-steppe zone were determined phytophagans species composition including 14 species of invaders among which were dominated Lepidopterans species (70.4%). Less numerous species were expanded forms of Coleoptera — 13.4%, Dipteran — 12.8% and other species which were 3.4%. The most widespread gardens and parks plantations invaders are: chestnut leaf miner (Cameraria ohridella Deschka & Dimic), fall webworm moth (Hyphantria cunea Drury.), linden moth (Phyllonorycter issikii Kumata), acacia lotus borer (Etiella zinckenella Tr.). Among the fastest spreading invaders is boxen lotus borer (Cydalima perspectalis Walker) (common box) from South-East Asia. In Ukraine this species was discovered in 2016 in Transcarpathia region. Conclusion. Over the last years destructive plant feeders such as: horse chestnut leaf miner, fall webworm moth, linden moth and others became more spreading within the parks and gardens plantations of the Ukrainian Forest-steppe zone. The significant injury by adventive species — boxen lotus bore was discovered. The lack of well-timed gardens and parks monitoring in the Ukrainian Forest-steppe zone led to mass distribution of lots of harmful species both quarantine and invasitions.

Abstract:
This paper introduces a novel low-cost solar-powered wearable assistive technology (AT) device, whose aim is to provide continuous, real-time object recognition to ease the finding of the objects for visually impaired (VI) people in daily life. The system consists of three major components: a miniature low-cost camera, a system on module (SoM) computing unit, and an ultrasonic sensor. The first is worn on the user’s eyeglasses and acquires real-time video of the nearby space. The second is worn as a belt and runs deep learning-based methods and spatial algorithms which process the video coming from the camera performing objects’ detection and recognition. The third assists on positioning the objects found in the surrounding space. The developed device provides audible descriptive sentences as feedback to the user involving the objects recognized and their position referenced to the user gaze. After a proper power consumption analysis, a wearable solar harvesting system, integrated with the developed AT device, has been designed and tested to extend the energy autonomy in the different operating modes and scenarios. Experimental results obtained with the developed low-cost AT device have demonstrated an accurate and reliable real-time object identification with an 86% correct recognition rate and 215 ms average time interval (in case of high-speed SoM operating mode) for the image processing. The proposed system is capable of recognizing the 91 objects offered by the Microsoft Common Objects in Context (COCO) dataset plus several custom objects and human faces. In addition, a simple and scalable methodology for using image datasets and training of Convolutional Neural Networks (CNNs) is introduced to add objects to the system and increase its repertory. It is also demonstrated that comprehensive trainings involving 100 images per targeted object achieve 89% recognition rates, while fast trainings with only 12 images achieve acceptable recognition rates of 55%.

SUMMARYThe objective of the current study was to develop, validate and describe a decision support system (DSS) to evaluate cull dairy cow finishing strategies. The DSS was developed within a Microsoft Excel framework. The purpose of a DSS is to assist the process of making accurate and repeatable calculations, assisting the decision on which cull cow finishing strategy is most profitable under individual farm circumstances. The model was based on data from two evaluation experiments including eight finishing strategies in total: ad libitum grass silage (GS); GS+3 kg concentrate (GS+3); GS+6 kg concentrate (GS+6); GS+9 kg concentrate (GS+9); ad libitum grass silage prior to ad libitum spring grass (GS+G); 0·75 grass silage and 0·25 straw prior to ad libitum spring grass (GS+S) and finally grass silage plus 6 kg concentrate dry matter (DM)/cow/day and milked twice daily prior to ad libitum spring grass (EXTLAC). Stochastic budgeting was included in the model to account for variability in key input and output variables on the overall profitability of various finishing strategies. The stochastic input and output variables included in the model were initial carcass value, feed strategy, concentrate cost and final carcass value. Net profit per cow was selected as the output distribution. The mean net profit per cow with the GS, GS+3, GS+6, GS+9, GS+G, GS+S and EXTLAC was €85·3, €73·7, €95·6, €58·5, €158·8 and €186·8 and €283·0, respectively. Profitability for the EXTLAC strategy was stochastically dominant to all other strategies evaluated meaning a higher level of profit and a lower level of risk is associated with the EXTLAC strategy. The optimal strategy of cull cow beef production depends greatly on the prevailing economic environment, purchase and sale price, milk price, feed costs, housing and labour.

To analyze the phytosanitary state of vegetable agrocenoses of the Right-Bank Forest-Steppe of Ukraine, to establish the species composition of the dominant pests and to study the natural saturation with beneficial organisms. Methods. The collection of material and entomological observations were carried out in accordance with generally accepted methods. Variation-statistical analysis was carried out using special software packages for statistics and computer graphics: Microsoft Excel 2010, Statgraphics plus. Results. By monitoring the phytosanitary state of vegetable agrocenosis in open ground, the species composition of the complex of harmful and beneficial organisms has been established. It has been proved that phytophages dominate on the plantings of white cabbage (variety Dymerskaya 7): black cruciferous flea beetle, numbering 8.2—10.8 ind./plant; cabbage spring fly — 9.0—11.6 eggs / plant; hidden proboscis — 2.6—3.1 persons/plant; onion thrips — 2.3—6.1 ind./ plant; cabbage aphid — up to 32 ind./plant and cabbage whitefly, the number of which was 2.5—6.7 ind./pl. The most common in the agrocenoses of white cabbage among lepidoptera were cabbage moth, numbering 2.8—4.3 gus./ plant, cabbage scoop — 1.6—1.9 gus./plant, cabbage and rape whites — 0.6—3.1 gus./ plant. Common spider mites dominated on cucumbers, with a number of 3.6—4.2 specimens/plant. and melon aphids — 14.8—16.6 ind./plant. A complex of entomophages belonging to the families Coccinellidae, Staphilinidae, Ichneumonidae, Trichogrammatidae, Braconidae, Tachinidae, Sirphidae was noted. Conclusions. The species composition of the complex of harmful and beneficial organisms in the vegetable agrocenoses of the open ground of the Right-Bank Forest-Steppe of Ukraine includes 10 species of phytophages of white cabbage, 2 species of pests of cucumbers and 7 species of entomophages.

Relevance Obesity has a major impact on the health and well-being of the population, and the prevention of overweight and obesity is a public health priority in many countries around the world.Purpose: was to study the prevalence of obesity among the adult population of the Voronezh region for the period 1996 – 2017, a comparative assessment of the structure of eating behavior in people with normal weight and overweight/obesity.Methods. The research material was data the annual form No. 12 of Rosstat for 1996 through 2017. In order to study eating disorders, 150 people aged from 20 to 61 years were surveyed using the Dutch questionnaire. The main types of eating behavior among the respondents were determined. Ideal weight was calculated by Brock's field formula (1891), the upper limit of normal body weight was considered ideal weight plus 15% ideal weight. Statistical data processing was performed using Microsoft Excel 2007.Results. The study on the prevalence of obesity was conducted among the adult population of the Voronezh region for the period 1996-2017. The Prevalence of obesity over the period studied has increased by 8 times, the average annual growth rate over the last 4 years was 30,04%. The study of the types of food behavior in respondents with normal and overweight / obesity showed that in most cases the respondents observed pathological types (most often mixed and external). A comparative assessment of the types of eating behavior did not show any significant differences between the groups.Conclusions. The increase in the prevalence of obesity, the high average annual growth rate indicates the increased impact of risk factors on the formation of obesity and the ineffectiveness of the strategy and tactics of dealing with this pathology. The absence of significant differences in the structure of eating behavior does not allow us to consider it a risk factor for obesity.

Abstract:
The aim of this study was to evaluate myocardial changes in a short-haired guinea pig (Cavia porcellus L) after everyday use of energy drinks for 60 days. The experiments were carried out on 11 male gui- nea pigs, age of 2 months, with a mean weight of 220±0.2 g. The guinea pigs were randomly grouped into a control group of 5 guinea pigs and experimen- tal group of 6. Experimental group guinea pigs were orally administered energy drinks 4 ml per day as a single oral dose. After the 60 days experiment, the quantitatively evaluated structures of myocardial in- terstitial collagen in the left ventricle using automa- ted image analysis system, which was connected to Olympus BXx51 microscope. Assessment was per- formed with Image-Pro Plus 5.1 analysis system. We calculated volume and perimeter of the interstitial fibrillar collagen in the left ventricular myocardium, the number of separate fibers in the field of view were counted. In total, there was evaluated 100 fields of view in the control group and 120 fields of view in the experimental group. Statistical analysis was per- formed using statistical package SPSS Statistics 20 and Microsoft Excel. We used bifactorial analysis of variance (ANOVA) to compare histomorphometric parameters between the two groups. We assessed that the difference between parameters was up to 1% and the individual specific effect was counting for 50% (p &lt; 0.05). Variation of the featu- res was much larger within the experimental group, when compared to the control group. Maximum ave- rage values of the fibrillar collagen volume, perimeter and the number of separate fibers in the field of view were 1.2 times larger in the experimental group than in the control group. Repeated energy drink consumption acts as external factor stimulating adaptive remodeling processes. Quantative structural changes of fibrillar collagen in left ventricle of experimental group animals are reaction to a stress factor. Unbalanced changes of fi- brillar collagen may determine inadequate remodeling and become a reason for pathogenesis.

Objective – To provide an updated categorization of Library and Information Science (LIS) publications and to identify trends in LIS research. Design – Bibliometric study. Setting – The Library and Information Science Abstracts (LISA) database via the CSA Illumina interface. Subjects – 11,273 item records published from 2004-2005 and indexed in LISA. Methods – First, a search was set up to retrieve all records from 2004-2005, limited to peer review items (called “arbitrated works” by the authors (150)) and excluding book reviews. Second, thematic descriptor terms used for the records were identified. Frequency counts for descriptor term occurrence were compiled using Microsoft Access and Pajek software programs. From the results of this search, the top terms were analyzed using the Kamada-Kawai algorithm in order to eliminate descriptor term co-occurrence frequencies under 30. A cluster analysis was used to depict thematic foci for the remaining records, providing a co-word network that visually identified topic areas of most frequent publication. Conclusions were drawn from these findings, and recommendations for further research were provided. Main Results – The authors identified 18 “thematic research core fields” (152) clustered around three large categories, “World Wide Web”, “Education”, and “Libraries”, plus 12 additional peripheral categories, and provided a schematic of field interrelationships. Conclusion – Domains of greatest focus for research “continue to be of practical and applied nature,” (153) but include increased emphasis on the World Wide Web and communications technologies, as well as on user studies. A table of the most frequently occurring areas of research along with their top three descriptor terms is provided (Table 1, 152) (e.g., “World Wide Web” as the top area of research, with “online information retrieval” (268 occurrences), “searching” (132 occurrences), and “web sites” (115 occurrences)).

he protection of ecosystem and preservation of biodiversity through the approach of geospatial technology was the aim of this research. The channel was monitoring the spatial transformation of the Federal University of Technology, Akure, Nigeria between year 2002 and year 2018 using Satellite Remote Sensing and Geographical Information System techniques. Landsat 7 Enhanced Thematic Mapper (ETM) plus of year 2002, Landsat 8 Operational Land Imager (OLI) and Thermal Infrared Sensor (TIRS) of year 2014 and year 2018 all of 32m resolution were the satellite images obtained for the study. These images were processed with supervised maximum likelihood classification algorithm using ArcGIS 10.3 software. To validate the classification and ensure high accuracy, an accuracy assessment was performed using training samples from 60 points on each of the satellite imagery on a reference image from google earth combined with ground data collected on actual visitation to the study area to verify the true land-cover type existing on the site. The resultant images deemed fit for analyses were classified into built-up, thick vegetation, light vegetation and bare land, land cover classes. Microsoft Excel spreadsheet was used to perform land cover area calculations through which the land cover dynamics and the spatial expansion were identified. The result showed built-up (13.58%, 14.59%, 20.75%); thick vegetation (33.78%, 26.26%, 12.18%); Light vegetation (24.57%, 32.29%, 30.51%); Bare land (28.08%, 26.26%, 36.56%) for the three years respectively. A special focus was put on the general depletion of the (thick and light) vegetation of which trees are a major actor. These depletion were adduced to the positive transformation of other land cover classes through the underlining landuse. The study concluded that alteration, depletion and consequent disappearance of trees in the green ecosystem is a threat to environment’s sustainability and the protection of ecosystem and preservation of biodiversity. The study recommended the research as a tool to controlling the removal of trees and thick forest, growing more trees and plants among other factors to protect ecosystem and preserve biodiversity.

Background: The Nigeria Centre for Disease Control (NCDC) has categorized dengue fever as a priority epidemic-prone disease. Kano metropolis is a hub for international trade and has seen rapid population growth with unplanned urbanisation. This provides the right environment for dengue virus transmission and spread. Most fevers in Nigeria and Kano in particular are treated as malaria or typhoid. This study aimed to determine the prevalence and factors associated with dengue fever and dengue-malaria co-infection among febrile patients in Kano metropolis. Methodology: We conducted a hospital-based cross-sectional study among febrile patients attending secondary health facilities in Kano metropolis. We used multistage sampling technique to recruit 440 participants into the study. An interviewer administered structured questionnaire was used to collect participants’ information, while blood was screened for malaria using rapid diagnostic test (RDT) kit and tested for dengue fever using an ELISA kit for dengue IgM. Data was cleaned and analysed using Microsoft Excel 2016 and Epi Info version 7.2 to calculate frequencies, proportions and odds ratios. Results: The median age of participants was 24 years (IQR= 13-36 years) while the age-group most represented was the 10-19 years’ age group with 97 (22.9%) participants. Males constituted 257 (60.6%) while most were single 238 (56.1%). Dengue IgM was positive for 332 (78.3%), while malaria RDT was positive for 81 (19.1%) and 67 (15.8%) were positive for both. Those aged above 25 years were less likely to present with dengue fever (OR=0.6; 95% CI=0.33-0.97). Dengue fever contributes a high percentage to febrile illnesses seen in Kano metropolis. Significant proportions of febrile patients have both dengue and malaria co-infection. None of the participants had dengue as a presumptive diagnosis. Conclusion: These findings imply dengue fever should feature as a prominent differential for febrile illnesses and dengue screening tests should be made available for all cases seen in Kano. Résultats: L'âge médian des participants était de 24 ans (IQR=13-36 ans) tandis que le groupe d'âge le plus représenté était le groupe d'âge 10-19 ans avec 97 (22,9%) participants. Les hommes en constituaient 257 (60,6%) tandis que la plupart étaient célibataires 238 (56,1%). Les IgM de la dengue étaient positives pour 332 (78,3%), tandis que les TDR du paludisme étaient positifs pour 81 (19,1%) et 67 (15,8%) étaient positifs pour les deux. Les personnes âgées de plus de 25 ans étaient moins susceptibles de présenter une fièvre dengue (OR=0,6; IC à 95%=0,33-0,97). La dengue contribue à un pourcentage élevé des maladies fébriles observées dans la métropole de Kano. Des proportions significatives de patients fébriles présentent une co-infection à la fois par la dengue et le paludisme. Aucun des participants n'avait de dengue comme diagnostic présumé. Introduction : La simulation apparaît aujourd’hui comme un outil indispensable dans l’apprentissage de compétences médicales techniques et non techniques, tout en respectant le principe éthique suivant : « Jamais la première fois sur le patient ». Actuellement, l’intégration de la simulation dans les programmes de formation des internes de médecine d’urgence est disparate : son cadre demeure à définir devant l’ouverture récente du diplôme d’études spécialisée (DES) de médecine d’urgence à l’automne 2017. Objectif de l’étude : Cette étude a évalué l’utilisation de la simulation en France dans l’enseignement actuel de la médecine d’urgence, afin d’initier une réflexion sur la place que pourrait avoir ce type d’enseignement pour le DES. Méthode : Il s’agissait d’une étude observationnelle multicentrique. Un questionnaire a été envoyé par e-mail à l’ensemble des responsables universitaires de France impliqués dans l’enseignement de la médecine d’urgence. Les données ont été colligées dans Excel (Microsoft). Elles étaient à la fois quantitatives et qualitatives et concernaient le cadre, la structuration et le contenu de la formation par la simulation. Résultats : Vingt-cinq facultés de médecine sur 29 ont répondu au questionnaire. Parmi elles, 23 utilisaient la simulation (soit 92 %), dont 22 la simulation haute fidélité et 21 la simulation procédurale. Arrêt cardiorespiratoire (22), état de choc (21), douleur thoracique et tachycardies (20), traumatisé crânien et traumatisé grave (17) étaient les thématiques majoritairement abordées. Les gestes techniques les plus enseignés étaient : intubation (22), intubation difficile (21), ventilation assistée-contrôlée et voie intraosseuse (18). En médiane, la formation comptait entre deux et trois jours de simulation par année de formation. Par ailleurs, dans 12 centres (soit 52 %), les formateurs avaient un temps professionnel dédié, et 15 centres (soit 65 %) bénéficiaient de personnel salarié. Enfin, seulement 12 centres (52 %) estimaient « plutôt » respecter l’adage « Jamais la première fois sur le patient ». Discussion : L’utilisation de la simulation dans l’enseignement de la médecine d’urgence est inégale. Les responsables pédagogiques interrogés semblent tomber d’accord quant aux principales thématiques et techniques à aborder. Le développement de la simulation semble souhaité de tous, mais demeure complexe en raison notamment de l’investissement humain et matériel que cela représente. Ces réponses pourraient fournir un axe de réflexion afin d’établir un programme de simulation commun à l’ensemble des facultés.

Effective management of fertiliser nitrogen (N) inputs by farmers will generally have beneficial productivity, economic and environmental consequences. The reality is that farmers may be unsure of plant-available N levels in cropping soils at sowing and make decisions about how much fertiliser N to apply with limited information about existing soil N supply. NBudget is a Microsoft (Armonk, NY, USA) Excel-based decision support tool developed primarily to assist farmers and/or advisors in Australia’s northern grains region manage N. NBudget estimates plant-available (nitrate) N at sowing; it also estimates sowing soil water, grain yields, fertiliser N requirements for cereals and oilseed crops and N2 fixation by legumes. NBudget does not rely on soil testing for nitrate-N, organic carbon or soil water content. Rather, the tool relies on precrop (fallow) rainfall data plus basic descriptions of soil texture and fertility, tillage practice and information about paddock use in the previous 2 years. Use is made of rule-of-thumb values and stand-alone or linked algorithms describing, among other things, rates of mineralisation of background soil organic N and fresh residue N. Winter and summer versions of NBudget cover the 10 major crops of the region: bread wheat, durum, barley, canola, chickpea and faba bean in the winter crop version; sorghum, sunflower, soybean and mung bean in the summer crop version. Validating the winter crop version of NBudget estimates of sowing soil nitrate-N against three independent datasets (n=65) indicated generally close agreement between measured and predicted values (y=0.91x+16.8; r2=0.78). A limitation of the tool is that it does not account for losses of N from waterlogged or flooded soils. Although NBudget also predicts grain yields and fertiliser N requirements for the coming season, potential users may simply factor predicted soil N supply into their fertiliser decisions, rather than rely on the output of the tool. Decisions about fertiliser N inputs are often complex and are based on several criteria, including attitudes to risk, history of fertiliser use and costs. The usefulness and likely longevity of NBudget would be enhanced by transforming the current Excel-based tool, currently available on request from the author, to a stand-alone app or web-based tool.

Arterial hypertension is one of the most common causes of atherosclerotic cardiovascular disease, which is still the reason of mortality for a lot of persons. Assessment of 10-year risk of atherosclerotic cardiovascular disease in hypertensive patients is very important for further treatment improvement. The aim is the absolute 10-year risk assessment of atherosclerotic cardiovascular disease in hypertensive patients for further correction of treatment. We included 61 patients with stage 1 to 2 arterial hypertension into our study. Most of them are women (70%). The patients were (56.84 ± 8.1) years old. The total cholesterol was (4.32 ± 1.0) mmol/l, high and low density lipoprotein cholesterol – (1.35 ± 0.2) mmol/l and (2.44 ± 0.7) mmol/l, respectively. Online calculator «ASCVD Risk Estimator Plus» was used. It included the assessment of age, sex, race, systolic and diastolic blood pressure, total cholesterol, high and low density lipoprotein cholesterol, presence of diabetes, smoking status, hypertension treatment, consumption of statins or aspirin. The risk was classified as low (< 5%), borderline (5% to < 7.5%), intermediate (≥ 7.5% to < 20%), or high (≥ 20%). The results were analyzed statistically using Microsoft Excel. For five persons it was not possible to calculate this risk using the online calculator because of too low values of total cholesterol and low density lipoprotein cholesterol. Low 10-year risk of atherosclerotic cardiovascular disease was confirmed in 30 (53.6%) people, borderline – in 2 (3.6 %), intermediate – in 11 (19.6%), high – in 13 (23.2%). Low absolute 10-year risk of atherosclerotic cardiovascular disease was determined in more than half (53.6%) of patients, high – in about quarter (23.2%), intermediate – in the fifth part (19.6%) of patients with arterial hypertension. For patients with borderline and intermediate risks, it is reasonable to continue therapy with moderate-intensity statins. For hypertensive patients with high risk, high-intensity statin therapy can be recommended and low-dose aspirin might be considered if bleeding risk is not increased. If the absolute 10-year risk of atherosclerotic cardiovascular disease is low, it is reasonable to assess additionally total cardiovascular risk categories and continue moderate-intensity statin therapy in the case of absence of low-density lipoprotein cholesterol goal levels with the next lipid profile assessment in 8 (± 4) weeks.

AimsTo ascertain the compliance of the Mental Health Service of Older People (MHSOP) memory clinics in obtaining ECGs, based on the agreed criteria.BackgroundCholinesterase inhibitors are a main pharmacological treatment for Alzheimer's dementia (AD). These drugs may worsen pre-existing cardiac conditions or cause significant cardiac side effects. A baseline ECG can be beneficial before starting patients on these medications. Previously in Leicestershire, all memory clinic patients were receiving routine ECGs. However, new standards were set based on the NICE guidelines and criteria outlined in other regions, to reduce the use of this time consuming and expensive investigation for patients who may not require it.MethodA total of 120 patients attending memory clinics in Leicestershire over a 6 month period (April to September 2019), were randomly selected and their electronic records retrospectively reviewed. The data collection tool was designed to encompass the key aspects of the criteria for obtaining an ECG for those attending the memory clinic. The information was analysed using Microsoft Excel.ResultOf the 120 patients, 23 (19.2%) were diagnosed with AD, 10 (8.33%) with mixed and 19 (15.8%) with vascular dementia. 68 (56.7%) had a diagnosis of “other” which included mild cognitive impairment or diagnosis still under investigation. 0 patients were diagnosed with Lewy Body Dementia or Parkinson's dementia. Of the total number of patients, only 10 had an ECG done, 2 with a diagnosis of AD, 1 with mixed dementia, 1 with vascular dementia and 6 “other”. The 10 ECGs done were all requested by nursing staff.Although 27 (22.5%) patients were identified to have a diagnosis of AD or mixed dementia, plus at least one of the criteria for an ECG, only 6 (22.2%) were discussed with the Multi-Disciplinary Team (MDT) following which only 3 of the 27 patients (11.1%) had an ECGConclusionDespite having clear criteria for requesting an ECG for those attending the memory clinic, compliance over the 6 month period was low. The following recommendations may be useful in improving compliance:Displaying the ECG algorithm in the memory service clinic rooms.Raise awareness amongst memory service clinicians of the criteria for requesting ECGs.

This comprised three health states: (1) progression-free survival (PFS), (2) progressed disease and (3) death.ResultsThree randomised controlled trials (RCTs), RADIANT-3 [RAD001 in Advanced Neuroendocrine Tumors, Third Trial; pancreatic NETs (pNETs): everolimus vs. best supportive care (BSC)], A6181111 (pNETs: sunitinib vs. BSC) and RADIANT-4 (RAD001 in Advanced Neuroendocrine Tumors, Fourth Trial; GI and lung NETs: everolimus vs. BSC), met the inclusion criteria for the clinical effectiveness systematic review. The risk of bias was low. Although the NETTER-1 (Neuroendocrine Tumors Therapy) RCT, of 177Lu-DOTATATE plus 30 mg of octreotide (Sandostatin®, Novartis) compared with 60 mg of octreotide, was excluded from the review, we nonetheless present the results of this trial, as it informs our estimate of the cost-effectiveness of 177Lu-DOTATATE. The pNETs trials consistently found that the interventions improved PFS and overall survival (OS) compared with BSC. Our indirect comparison found no significant difference in PFS between everolimus and sunitinib. Estimates of OS gain were confounded because of high rates of treatment switching. After adjustment, our indirect comparison suggested a lower, but non-significant, hazard of death for sunitinib compared with everolimus. In GI and lung NETs, everolimus significantly improved PFS compared with BSC and showed a non-significant trend towards improved OS compared with BSC. Adverse events were more commonly reported following treatment with targeted interventions than after treatment with BSC. In the base case for pNETs, assuming list prices, we estimated incremental cost-effectiveness ratios (ICERs) for everolimus compared with BSC of £45,493 per quality-adjusted life-year (QALY) and for sunitinib compared with BSC of £20,717 per QALY. These ICERs increased substantially without the adjustment for treatment switching. For GI and lung NETs, we estimated an ICER for everolimus compared with BSC of £44,557 per QALY. For GI (midgut) NETs, the ICERs were £199,233 per QALY for everolimus compared with BSC and £62,158 per QALY for a scenario analysis comparing 177Lu-DOTATATE with BSC. We judge that no treatment meets the National Institute for Health and Care Excellence’s (NICE) end-of-life criteria, although we cannot rule out that sunitinib in the A6181111 trial does.LimitationsA RCT with included comparators was not identified for 177Lu-DOTATATE. The indirect treatment comparison that our economic analysis was based on was of a simple Bucher type, unadjusted for any differences in the baseline characteristics across the two trials.ConclusionsGiven NICE’s current stated range of £20,000–30,000 per QALY for the cost-effectiveness threshold, based on list prices, only sunitinib might be considered good value for money in England and Wales.Future workFurther analysis of individual patient data from RADIANT-3 would allow assessment of the robustness of our findings. Defects in the functioning of neutrophilic granulocytes (NG) are accompanied by various purulent-inflammatory diseases that have a persistent-relapsing course. The bactericidal activity of NG, the processes of intracellular and extracellular killing of bacteria, is directly dependent on their phenotypic features: the expression level of functionally significant membrane receptors CD64, CD16, CD32, CD11b triggering effector processes. It was shown that there are subsets of NG with different phenotypic characteristics and the course and outcome of infectious and inflammatory diseases directly depends on their usefulness. Despite the large amount of accumulated information, the question of the transformation of NG subpopulations phenotype during NG response formation in children with local purulent processes of soft tissues - a small purulent infection (SPI) remains poorly understood. Purpose: to evaluate the local and systemic effector function of neutrophilic granulocytes (phagocytosis, NET formation) with clarification of the phenotype of functionally significant CD64-CD16+CD32+CD11b+, CD64+CD16+CD32+CD11b+ and CD64-CD16+CD32-CD11b+ NG subpopulations in peripheral blood (PB) in children with SPI. Materials and methods. We evaluated the phagocytic function of NG in 60 PB samples of children 5-9 years old, on the 2-3rd day of acute purulent process, before surgery, in smears - prints taken from the local focus of purulent inflammation and 17 PB samples of conditionally healthy children. Also the ability to form NET was additionally determined in smears-prints in children with SPI. In parallel, in PB we evaluated:% NG simultaneously carrying CD64, CD16, CD32, CD11b receptors and their expression density (MFI) by flow cytometry (CYTOMICS FC500, Beckman Coulter, USA). Statistical data processing was performed in the Microsoft Exel 2016 and Stat Plus 2010 software packages. Nonparametric criteria were used: median (Me) and interquartile range (Q1; Q3), Mann-Whitney U-test and Wilcoxon test. Results . We found a significant increase in the content of the NG subpopulation with a negatively transformed phenotype CD64-CD16+CD32-CD11b+ with defects in the expression of CD64 and CD32 receptors, which is the cause of inadequate inclusion of NG in inflammatory processes, disruption of phagocytic activity at the systemic and local levels and the occurrence of SPI in children. The formation of NET in the first stage is necessary to supplement the effector function of NG in order to compensate for disorders of phagocytosis.

Background: HIV co-infection with hepatitis B (HBV) and/or hepatitis C virus (HCV) is common, largely due to shared routes of transmission, but paucity of data exists for long term treatment outcomes of HIV infected patients, and those co-infected with HBV and HCV despite the high burden in Nigeria. The aim of study was to describe the longterm treatment outcomes in HIV infected Nigerians and to assess the effect of HBV and HCV co-infections on longterm response to antiretroviral therapy (ART).Methodology: This was a retrospective study of HIV infected adults (> 18 years old) consecutively initiating ART between July 2004 and December 2007, who were followed up for 7 years (2011 and 2014). HBV and HCV infections were diagnosed by detection of serum hepatitis B surface antigen (HBsAg) and HCV antibody (HCVAb) respectively. HIV viral load and CD4 count were monitored 3-monthly after initiating ART, and treatment outcomes based on these were compared between patients with HIV mono-infection, HIV/HBV, HIV/HCV and HIV/HBV/HCV co-infections. Clinical and laboratory data of the patients were abstracted from the medical databases, FileMaker Pro, v 10, entered into Microsoft Excel, and analyzed using SPSS version 20.0.Results: A total of 2,800 adults were evaluated (median age of 35.5 years; 64.2% female), of whom 197 (7.0%) were co-infected with HBV, 53 (1.9%) with HCV, and 15 (0.5%) with HBV and HCV. During the 7-year period, 369 (13.2%) patients were lost to follow up. Immune reconstitution, measured by CD4 recovery, was lower in both HBV and HCV co-infections compared to HIV mono-infection, but this was not statistically significant (p>0.05). Median baseline HIV viral load was 4.63 log copies/ml for all groups, which decreased to undetectable level at a median time of 6 months and remained so for the study duration.Conclusion: This study revealed a higher virologic failure among HIV/HCV co-infected group compared to other groups. No immunological difference in ART treatment outcomes between HIV mono-infected and those co-infected with HBV and HCV after 7-year follow-up. Gradual rise in CD4 was found to be an immunological evidence of the body’s recovery from HIV, buttressed by the drop in viral load over the 7-year period. Les données cliniques et de laboratoire des patients ont été extraites des bases de données médicales, FileMaker Pro, v 10, saisies dans Microsoft Excel et analysées à l'aide de SPSS version 20.0.Résultats: Un total de 2800 adultes ont été évalués (âge médian de 35,5 ans; 64,2% de femmes), dont 197 (7,0%) étaient co-infectés par le VHB, 53 (1,9%) par le VHC et 15 (0,5%) par le VHB et VHC. Background: Staphylococcus aureus is a ubiquitous bacterium present in the environment and one of the leading causes of superficial and deep infections. In the food industry, it is acclaimed to be globally responsible for several food-borne diseases. This study was designed to isolate methicillin-resistant S. aureus (MRSA) and determine the effect of rinsing on MRSA load in frozen meat and fish obtained from open market in Benin City.Methodology: Forty frozen meat samples (15 beef, 10 fish and 15 chickens) were randomly obtained from five markets in Benin City. The samples were analysed before and after rinsing using standard culture-based techniques to determine heterotrophic bacterial count, isolation of S. aureus, MRSA, and antibiotic susceptibility testing. Data were analysed using SPSS version 21 and Microsoft excel 2016, and association between variables were measured using Student’s t-test with a probability level of < 0.05.Results: The natural logarithm (LN) of heterotrophic bacterial count (CFU/g) before rinsing were 11.53±1.25 (beef), 11.16±0.95 (fish) and 11.42±1.58 (chicken), while the counts after rinsing were 2.70±0.45 (beef), 2.68±0.25 (fish) and 2.79±0.49 (chicken) (p<0.05). Sixteen of the 40 (40%) were positive for S. aureus, of which 4 (10%) were MRSA. Amongst the frozen meat evaluated in the study, beef had the highest frequency of S. aureus contamination (46.7%) followed by chicken (40.0%) and fish (30.0%). The profile of antibiotic resistance of S.aureus showed that they were least resistant to ciprofloxacin (6%) but showed high resistance to erythromycin (94%), amoxicillin/clavulanic acid (87.5%) and trimethoprim-sulfamethoxazole (81%). Dans l'industrie alimentaire, il est reconnu pour être globalement responsable de plusieurs maladies d'origine alimentaire. Cette étude a été conçue pour isoler S. aureus résistant à la méthicilline (SARM) et déterminer l'effet du rinçage sur la charge de SARM dans la viande et le poisson congelés obtenus sur le marché libre de Benin City.Méthodologie: Quarante échantillons de viande congelée (15 bœuf, 10 poissons et 15 poulets) ont été obtenus au hasard sur cinq marchés de Benin City. Les échantillons ont été analysés avant et après le rinçage en utilisant des techniques de culture standard pour déterminer le nombre de bactéries hétérotrophes, l'isolement de S. aureus, le SARM et les tests de sensibilité aux antibiotiques. Les données ont été analysées à l'aide de SPSS version 21 et de Microsoft Excel 2016, et l'association entre les variables a été mesurée à l'aide du test t de Student avec un niveau de probabilité <0,05.Résultats: Le logarithme naturel (LN) du nombre de bactéries hétérotrophes (UFC/g) avant rinçage était de 11,53±1,25 (bœuf), 11,16±0,95 (poisson) et 11,42±1,58 (poulet), tandis que les comptages après rinçage étaient de 2,70±0,45 (bœuf), 2,68±0,25 (poisson) et 2,79±0,49 (poulet) (p<0,05). Seize des 40 (40%) étaient positifs pour S. aureus, dont 4 (10%) étaient SARM. Parmi les viandes congelées évaluées dans l'étude, le bœuf présentait la fréquence la plus élevée de contamination par S. aureus (46,7%), suivi du poulet (40,0%) et du poisson (30,0%). Le profil de résistance aux antibiotiques de S. aureus a montré qu'ils étaient les moins résistants à la ciprofloxacine (6%) mais présentaient une résistance élevée à l'érythromycine (94%), à l'amoxicilline/acide clavulanique (87,5%) et au triméthoprime-sulfaméthoxazole (81%). L'indice de résistance aux antibiotiques multiples de S. aureus a été calculé à 0,63.Conclusion: Les résultats de cette étude ont révélé que les aliments surgelés pourraient servir de vecteur de dissémination de bactéries résistantes aux antibiotiques (ARA) et de risques potentiels pour la santé des consommateurs. Abstract:
Cochrane produces independent research to improve healthcare decisions. It translates its research summaries into different languages to enable wider access, relying largely on volunteers. Machine translation (MT) could facilitate efficiency in Cochrane’s low-resource environment. We compared three off-the-shelf machine translation engines (MTEs)—DeepL, Google Translate and Microsoft Translator—for Russian translations of Cochrane plain language summaries (PLSs) by assessing the quantitative human post-editing effort within an established translation workflow and quality assurance process. 30 PLSs each were pre-translated with one of the three MTEs. Ten volunteer translators post-edited nine randomly assigned PLSs each—three per MTE—in their usual translation system, Memsource. Two editors performed a second editing step. Memsource’s Machine Translation Quality Estimation (MTQE) feature provided an artificial intelligence (AI)-powered estimate of how much editing would be required for each PLS, and the analysis feature calculated the amount of human editing after each editing step. Google Translate performed the best with highest average quality estimates for its initial MT output, and the lowest amount of human post-editing. DeepL performed slightly worse, and Microsoft Translator worst. Future developments in MT research and the associated industry may change our results.
