La agenda de gobernabilidad democrática para América Latina debe ser reforzada, en el plano regional, con una integración renovada e inteligente, asentada en la cooperación y en una inserción estratégica común en los foros globales. El proceso requiere la intervención coordinada de un Estado reformado y una sociedad reconciliada y cohesionada que, conjuntamente, impulsen la edificación y el sostenimiento de tres pilares importantes: 1) recuperar la confianza ciudadana en la política y relegitimar las instituciones democráticas; 2) avanzar hacia un nuevo contrato social incluyente y solidario; y 3) poner en marcha unas ambiciosas reformas fiscales progresivas sin precedentes en la región. Este documento se basa en las minutas de los seminarios del proyecto “El estado de la democracia en América Latina”, ejecutado en 2020 por la Fundación Fernando Henrique Cardoso, la Fundación Democracia y Desarrollo e IDEA Internacional; y en las sesiones del Foro Internacional de Santo Domingo, sobre el Panorama socioeconómico y político de América Latina realizado en enero de 2021 conjuntamente con Funglode. El objetivo principal del informe es estimular la movilización de voluntades políticas, capacidades institucionales y recursos humanos y materiales para acelerar el tránsito de América Latina hacia una gobernabilidad democrática plena. Ha sido elaborado por el economista e investigador Jorge Máttar a solicitud de IDEA Internacional, bajo la dirección de Daniel Zovatto, director regional de IDEA Internacional para América Latina. Asimismo, recoge reflexiones, análisis y propuestas de Sergio Bitar, Daniel Zovatto, Fernando Reyes Matta, Sergio Fausto y José Octavio Bordón. Abstract:
This chapter traces boxing's crooked path to respectability in order to gain an understanding the incredible success of TV boxing in the middle of the twentieth century. Boxing was born in the late 1800s, when the adoption of the Queensberry rules ended the old bare-knuckle days of limitless rounds, neck choking, no weight classifications, and muddy deaths. The new rules imposed uniformly sized rings, three-minute rounds, standardized judging, weight categories, and padded leather boxing gloves. The new structure fit in well with Progressive Era concerns regarding social regulation and masculine regeneration. America was changing dramatically in the decades between the end of the Civil War and start of the First World War. Millions of non-English speaking New Immigrants from southern and eastern Europe poured into America's ports. By the 1940s, a single Mafia family managed to dominate the fight racket, led by a hit man named Frankie Carbo. When boxing came to television, the mob did not leave. In fact, it skimmed more money than ever. Home audiences were unaware that Gillette and the mob brought them the Friday Night Fights.

One way of putting powers to work is to use them to ground (at least some) modal truths. One might hold that truths of possibility are true because of the powers of objects. For instance, that it is possible that one more person be in this room is true because of the ambulatory powers of the people in the adjoining rooms. That it is possible that Slow Steve run a fifteen-minute mile is true because of the locomotive powers that Steve has (perhaps along with other powers, such as his respiratory powers). Call the family of stronger or weaker views which hold that possibility claims are true because of powers the ‘Powers Accounts of Possibility,’ or ‘Powers Accounts’ for short. Call a proponent of a Powers Account a ‘Powers Accountant.’ In this paper I present nine objections to Powers Accounts of Possibility and show how a Powers Accountant can respond to them. I begin by providing an exceedingly strong Powers Account and offering three objections to it. The objections will prove useful for forming a more moderate Powers Account. I then subject the more moderate Powers Account to six further objections. In the end, I vindicate a Powers Account of Possibility against all nine objections.

Training in oral and maxillofacial surgery is lengthy and demanding, requiring dual qualification and basic training in dentistry and medicine, followed by completion of a higher surgical training programme. The ‘exit examination’ leading to the awarding of the specialty Fellowship of the Royal College of Surgeons in Oral and Maxillofacial Surgery (FRCS(OMFS)) represents the final academic step of training in the specialty. For part 2 of the FRCS(OMFS) examination, one day is dedicated to ‘vivas’, where the candidate is interviewed by two senior clinicians in their diagnosis and management of conditions in the specialty. As such it encompasses the entire remit of Oral and Maxillofacial Surgery, including orthognathic surgery, craniomaxillofacial trauma, surgical oncology and dermatology, oral medicine, craniofacial surgery, dentoalveolar surgery, temporomandibular joint disorders, salivary gland disease, local flap and free tissue transfer, oral implantology and cleft surgery. This daunting syllabus is covered in four viva sessions of thirty minutes, with six five-minute vivas in each session. The aim of this book is to give candidates a structure as to how to answer such scenarios as well as provide clinical subject matter encompassing the range of the specialty. This includes pertinent scenarios that are generally not covered in other FRCS revision books such as ethics, consent and negligence. As such it will be of interest to clinicians in related specialties, including dermatology, oral surgery, ENT, and plastic surgery.

Abstract:
Beside reduction in tidal volume limiting peak airway pressure minimizes the risk for ventilator-associated-lung-injury in patients with acute respiratory distress syndrome. Pressure-controlled, time-cycled ventilation (PCV) enables the physician to keep airway pressures under strict limits by presetting inspiratory and expiratory pressures, and cycle times. PCV results in a square-waved airway pressure and a decelerating inspiratory gas flow holding the alveoli inflated for the preset time. Preset pressures and cycle times, and respiratory system mechanics affect alveolar and intrinsic positive end-expiratory (PEEPi) pressures, tidal volume, total minute, and alveolar ventilation. When compared with flow-controlled, time-cycled (‘volume-controlled’) ventilation, PCV results in reduced peak airway pressures, but higher mean airway. Homogeneity of regional peak alveolar pressure distribution within the lung is improved. However, no consistent data exist, showing PCV to improve patient outcome. During inverse ratio ventilation (IRV) elongation of inspiratory time increases mean airway pressure and enables full lung inflation, whereas shortening expiratory time causes incomplete lung emptying and increased PEEPi. Both mechanisms increase mean alveolar and transpulmonary pressures, and may thereby improve lung recruitment and gas exchange. However, when compared with conventional mechanical ventilation using an increased external PEEP to reach the same magnitude of total PEEP as that produced intrinsically by IRV, IRV has no advantage. Airway pressure release ventilation (APRV) provides a PCV-like squared pressure pattern by time-cycled switches between two continuous positive airway pressure levels, while allowing unrestricted spontaneous breathing in any ventilatory phase. Maintaining spontaneous breathing with APRV is associated with recruitment and improved ventilation of dependent lung areas, improved ventilation-perfusion matching, cardiac output, oxygenation, and oxygen delivery, whereas need for sedation, vasopressors, and inotropic agents and duration of ventilator support decreases.
