Full textThe purpose of this study was to look at the experiences of midwives in maternalhealth care encounters with non-european-born women and men, and to determine ifmidwives deem any special competence necessary to handle these encounters well.We gathered information by means of qualitative interviews and semi structuredquestions with eight midwives all of whom matched the inclusion criterias and gavetheir personal consent. Midwives from four district health care centers in Uppsalawere included. The interviews were transcribed and analyzed by means of qualitativecontent analysis. The results reveal the experiences from encounters with non-european patients to betwofold. On one hand the encounter is a positive, exiting experience with anopportunity to learn more about a foreign culture and exchange experiences. On theother hand complications can occur as patients may have unexpected expectationsregarding the health care, have great difficulties with the language or haveexperienced traumatizing incidents, all on top of coming to Sweden alone withoutrelatives. In the encounter with non-european-born patients the midwives consider it importantto have special competence in form of knowledge of other cultures and religions asthis provides a greater understanding of the reasoning behind the patients’ decisions.Cultural competence is also important as it helps avoid inadvertently insulting thepatient during the encounter. The special competence held by the midwives has been attained from their basic andspecialist education as well as from self acquired experiences and interests. A lifelongeducation is required to uphold this competence as well as a dialog betweenmidwives at the health care clinic.

The aim of this study was to find out what new parents think about parental leave, parental education, the child welfare center and the prenatal clinic. The main focus was on what role the care system played and to what extent the father was involved. Eleven parents were interviewed and the findings were analysed and put into different themes. The result showed that most of the parents were satisfied with the care given at the child welfare center and the prenatal clinic. The participants also thought they were well treated by the nurses and midwives during the encounters. Both parents felt equaly involved while talking to the midwife. All of the parents wanted to have parental leave to some extent and the main reason was that they wanted to spend time with their child. In half of the cases mothers used the greater part of the parental leave. Both breast-feeding and economical aspects were mentioned to be reasons for this. The participants felt that the child welfare center and the prenatal clinic strongly recommended breast-feeding which contributes to the fact that breast-feeding still is a key aspect in the decision on which parent who is going to stay at home with the child during the fist period. All of the parents thought that the visit to the delivery department was the best part during the parental education, they also appreciated meeting other people in the same situation. The general information given during the regular meetings were things that many of the participants already knew and could for that reason have been more detailed.

Full textThe last decade has brought great changes for homosexuals in laws and regulations. Among other things, the right to be parent is now legally accepted. Many gay couples have therefore decided to create a family. The purpose of this study was to describe lesbian women´s experiences in connection with prenatal care and childbirth. The compilation resulted in five main categories: Communication, Acknowledgment, Defense, Knowledge and Openness, which was worked up from Imogene King´s theory of caring. The results showed that several women had experiences of positive character, but that there also were many who talked about negative experiences. Heteronormativity was a constantly recurring theme that permeated all categories. Caregivers often assumed that the lesbian women were heterosexual without regard to other possibilities. This study shows that health professionals should be aware of their own way to communicate and to ensure the uniqueness of each individual. Positive experiences emerged when the midwife is clearly confirmed the lesbian women as prospective parents. Good care requires that health professionals are aware of heteronormativity, and that creates obstacles to the care of lesbian women.

Full textIn contemporary Sweden it is a matter of course that prospective fathers should be involved in pregnancy, childbirth and care of the newborn. Most fathers are present at the visits to the antenatal clinic, including the ultrasound examination; they participate in parental education classes; and they are present at the birth of their child. Yet many fathers express dissatisfaction with prenatal care, delivery care and postnatal care: they frequently feel excluded, invisible and irrelevant. This study is an attempt to better understand those experiences of exclusion, invisibility and irrelevancy. The study is based on interviews with ten Swedish fathers. Their narratives about their encounter with prenatal, delivery and postnatal care were analysed with a phenomenological--hermeneutic narrative approach. Special attention was paid to the way the major characters were positioned in the father's narratives. The results can be summarised in three points: (1) The strong focus in health care on the physical aspects of pregnancy and childbirth may make it difficult for prospective and new fathers to find a meaningful position in the encounter with prenatal, delivery and postnatal care. (2) The narratives of the interviewed fathers can be roughly divided into "we"-narratives where the father found an untroubled position and "I"-narratives where he didn't. I take this to mean that the fathers sought a "we"-position where pregnancy and childbirth could become a joint concern for the mother and the father. (3) The difficulty of finding an untroubled position in the encounter with prenatal, delivery and postnatal care is first and foremost a consequence of the fact that the prospective/new father is not pregnant, not of the fact that he is a man. It is whether the body is pregnant or not that matters, not the sex category to which the body belongs. The desired "we"-position can therefore be characterised as gender irrelevant. One conclusion is that for prenatal, delivery, and postnatal care to truly involve the expectant mother's partner in pregnancy, childbirth and care of the newborn, they need to seriously consider the question of how they can help the prospective mother's partner to find a position that better includes him/her in this encounter. One possibility is the "we"-position that this study emphasizes. This is a position in which pregnancy, childbirth and care of the newborn is a joint concern for both parents. Making way for a "we"-position also means widening the time perspective from the here-and-now of pregnancy, beyond the approaching childbirth to what comes next: parenthood -- no matter who was pregnant.
