Abstract:

Abstract:
Purpose. To study the morphological features of the palatine tonsils of experimental animals in normal conditions and during the modeling of chronic tonsillitis. Material and methods. A model of experimental chronic tonsillitis was created on 15 rabbits, 5 animals were included in the control group. Chronic tonsillitis was simulated under general anesthesia. Staphylococcus aureus was injected into the palatine tonsils of animals in a dose of 3 million microbial bodies. Morphological studies of the tissues of the palatine tonsils of animals in the control group and the experimental group were carried out one month after modeling. Results. The results of a morphological study of the tonsils of the experimental group of rabbits clearly demonstrated manifestations of chronic tonsillitis (proliferation of connective tissue in the subepithelial layer, weakening of the muscle tone of the vessels with a simultaneous change in the permeability of the vascular wall, developing hyalinosis). Conclusion. Morphological studies of the tonsils have confirmed the similarity of the modeled chronic pathology with the disease of a similar human organ. Abstract:
Abstract Introduction: The palatine tonsil play a key role in initiating immune responses against the antigenic material entering the mouth. Materials and Methods: Tissue pieces of oropharynx for the palatine tonsilwere collected from different prenatal age groups of sheep. These tissue pieces were fixed and processed for routine paraffin embedding technique to get 3-5pm thick sections. The paraffin sections were subjected to routine haematoxylin and eosin and some other special staining methods. Results: In three months foetal age of sheep, the palatine tonsils appeared as diffused lymphocytic infiltration in the propria submucosa overlaid by the mucosa of the oropharynx. The surface epithelium of the mucosa of the oropharynx formed primary crypts. The primary crypts in turn invaginated to form secondary crypts. The collagen fibres from the submucosa appeared to enter at the base of the developing tonsil. In the fourth month of foetus, the tonsillar surface epithelium became thicker and many crypts arose from that of primary crypts. Organized lymphocytic infiltration as isolated units was noticed near or around the secondary crypts. In five months of foetal age, the crypt epithelium was found to be infiltrated with lymphocytes, macrophages, plasma cells and mesenchymal cells. The connective tissue capsule and septa were completely formed. The lymphocytic tissue was characterized by an increased population of lymphocytes in the primary follicles without germinal centres. Conclusion: The results were compared with the literature and the palatine tonsil showed the gradual changes of histoarchitecture from third to fifth month of foetal age.

Aim.To find out microbial landscape characteristics of the palatine tonsils in healthy students, depending on the season. Materials and methods. It were examined 40 students without any oropharyngeal pathology aged 17-30 (23 ± 3.5). There were no signs of chronic tonsillitis at pha-ryngoscopy, no signs of acute respiratory infection; they did not take antibiotics for one last month prior to the study and had no tonsilitis for the last 5 years. Examination was made three times, once in a season: autumn from October 16 till November 2; winter from January 27 till March 6 ; spring from April 2 till April 19. Results. The normal microlandscape in healthy individuals aged 17-30 years permanently had Streptococcus viridans, Streptococcus pyogenes, Staphylococcus aureus, Staphylococcus epidermidis, Candida albicans, Neisseria, Micrococcus, Bacillus, and Pseudomonas aeruginosa. Presence of S. pyogenes andP. aeruginosa in the structure of microlandscape of the palatine tonsils did not cause any clinical symptoms in their carriers. We also revealed a seasonal dynamics expressed by the frequency of the increase of conditionally pathogenic flora on palatine tonsils in spring and the decrease in all microorganisms growth in winter.

Abstract:
Összefoglaló. A lateralis cysticus nyaki terimék két leggyakoribb oka a branchiogen cysta és a cysticus nyaki áttét. Az átfedő lokalizáció (a leggyakrabban a IIA nyaki régióban), a betegek életkora és az esetenként hirtelen kezdet alapján a két leggyakoribb ok differenciáldiagnózisa nagy kihívást jelenthet. Egy hirtelen fellépő fájdalmas, bal oldali nyaki duzzanattal, dysphagiával és lázzal jelentkező 72 éves férfi esetét ismertetjük. A nyak komputertomográfiás vizsgálata egy 6 cm legnagyobb átmérőjű, vastag falú, többrekeszes cysticus terimét igazolt. Infektív branchiogen cysta lehetőségére gondolva az elváltozást eltávolítottuk. A szövettan azonban p16-pozitív laphámrákot igazolt. Orv Hetil. 2020; 162(15): 595–600. Summary. Branchial cleft cysts and cystic neck metastases are the two most common causes of cystic lateral neck masses. Based on the overlapping location (neck level IIA), patient age at onset and the occasionally sudden onset, their differential diagnosis is challenging. We present a 72-year-old male presenting with a suddenly emerging painful, left-sided neck swelling, dysphagia and fever. Computed tomography showed a 6 cm thick-walled multicystic mass. With the suspected diagnosis of an infected branchial cleft cyst, the lesion was removed. Histology confirmed p16 positive squamous cell carcinoma. Primary tumor was identified in the ipsilateral palatine tonsil. Definive radiotherapy was performed and the patient is free of disease at the 18-month follow-up. Beyond pure and infected branchial cleft cysts and pure cystic metastases, infected cystic or necrotic metastasis of human papillomavirus associated oropharyngeal squamous cell carcinoma should be included in the differential diagnosis of cystic lateral neck lesions. A nine-month-old female entire Rottweiler was presented for further investigation of progressive, symmetric facial swelling over the maxilla of four weeks duration. Biochemistry revealed severe azotaemia with hyperphosphataemia. CT revealed poor mineralisation of the facial and calvarial bones. The maxillary and palatine bones were replaced by proliferative, heterogeneous tissue of mixed soft tissue and granular mineral attenuating material. On ultrasound, parathyroid glands were enlarged and renal lesions suggestive of juvenile nephropathy were observed. Due to poor long-term prognosis, the owner elected euthanasia. Necropsy confirmed generalised osteopenia and chronic kidney disease. Renal secondary hyperparathyroidism was diagnosed and should be considered a differential for bilateral facial swelling in young dogs.

Abstract:
Topicality: There are many conflicting opinions in the literature regarding the best surgical technique for the treatment of OSAS related to palatine hypertrophy – partial (tonsillotomy) or complete (tonsillectomy) removal of lymphoid tissue of the tonsils. Also, there are many opinions about the use of a variety of traditional and modern surgical instruments to reduce the volume of the tonsils, including using a laser, a radiofrequency knife, an ultrasonic knife, a shaver (microdebrider) and a coblator. Aim of the study: The aim of the study was to compare the surgical methods effectiveness of tonsils hypertrophy treatment which the based on the restoration of the mucous membrane's local immunity function. Materials and methods: Children who undergoing surgical treatment for palatine tonsils hypertrophy 2-3 degrees according to the Brodsky classification and the presence of OSAS took part in the study. The patients were randomized and divided into four comparison groups using different surgical techniques. The first group, which used the surgical technique of classical tonsillotomy (TT) included 28 patients, the second group, which used the surgical technique of intracapsular shavertonsillotomy(STT) included 18 patients, the third group, which used surgical technique of intracapsular coblationtonsillotomy (CTT), and groups which using surgical technique of tonsilloplasty included 12 patients (TP). Determined the state of local immunity before surgery and 1 month after surgery by concentration levels of sIgA, α-IFN, IL-1β, TGF-1β and general-purpose immune complexes. Results: Recovery of the concentration of the major marker of local immunity sIgA occurs significantly (P<0.05) faster when using the surgical technique of CTT and STT. Recovery of the concentration of the major marker of antiviral immunity α-IFN occurs significantly (P <0.05) faster when using the surgical technique of CTT, STT and TP. Recovery of the concentration of the anti-inflammatory cytokine IL-1β occurs significantly (P<0.05) faster with the use of the surgical technique of STT and TP. Indicators of the regeneration factor TGF-1β are significantly the highest when using the surgical techniques of STT, TT and TP. The concentration of immune complexes in the oropharyngeal secretions of patients with tonsil hypertrophy at 1 month after surgery significantly decreased with the use of surgical techniques TT and TP. Significant (P <0.05) increase in IgG level was de- termined in all comparison groups except the group where tonsilloplasty was used. Conclusions: The safety surgical technique of palatine tonsils volume reduction – TP and STT promotes significantly (P<0,05) faster restoration of concentration of the main mucous membrane's local immunity markers.

Abstract:

Abstract:
Cosmography can be considered as a sort of a model-independent approach to tackle the dark energy/modified gravity problem. In this review, the success and the shortcomings of the [Formula: see text]CDM model, based on General Relativity (GR) and standard model of particles, are discussed in view of the most recent observational constraints. The motivations for considering extensions and modifications of GR are taken into account, with particular attention to [Formula: see text] and [Formula: see text] theories of gravity where dynamics is represented by curvature or torsion field, respectively. The features of [Formula: see text] models are explored in metric and Palatini formalisms. We discuss the connection between [Formula: see text] gravity and scalar–tensor theories highlighting the role of conformal transformations in the Einstein and Jordan frames. Cosmological dynamics of [Formula: see text] models is investigated through the corresponding viability criteria. Afterwards, the equivalent formulation of GR (Teleparallel Equivalent General Relativity (TEGR)) in terms of torsion and its extension to [Formula: see text] gravity is considered. Finally, the cosmographic method is adopted to break the degeneracy among dark energy models. A novel approach, built upon rational Padé and Chebyshev polynomials, is proposed to overcome limits of standard cosmography based on Taylor expansion. The approach provides accurate model-independent approximations of the Hubble flow. Numerical analyses, based on Monte Carlo Markov Chain integration of cosmic data, are presented to bound coefficients of the cosmographic series. These techniques are thus applied to reconstruct [Formula: see text] and [Formula: see text] functions and to frame the late-time expansion history of the universe with no a priori assumptions on its equation-of-state. A comparison between the [Formula: see text]CDM cosmological model with [Formula: see text] and [Formula: see text] models is reported.

Abstract:
Background: Endoscopic endonasal transpterygoid (EET) approach is well suited for trigeminal schwannomas. Case Description: A 25-year-old female presented with the right hemifacial pain for 1 month. Examination revealed mild sensory loss to both touch and pain in the right V2 and V3 dermatomal distribution. On magnetic resonance imaging, a well-defined extra-axial lesion was seen in the right infratemporal fossa extending intracranially, widening the foramen ovale. Erosion of the lateral pterygoid plate was evident on computed tomography. Diagnosis of a trigeminal schwannoma was made and excision through endoscopic transnasal transpterygoid approach was planned. After adequate nasal decongestion, lateralization of the inferior turbinate followed by medialization of middle turbinate done to expose the posterior part of the lateral nasal wall. Sphenopalatine foramen was visualized and the branches of the sphenopalatine artery to turbinate were coagulated followed by partial inferior and middle turbinectomy. Next, antegrade uncinectomy, bullectomy, and middle meatal antrostomy performed. Anterior and posterior ethmoidectomy was done to gain access to the sphenoid sinus. Medial maxillectomy done, posterior maxillary sinus wall exposed and drilled to reach infratemporal fossa harboring the tumor. The lateral recess was further exposed to visualize the vidian and maxillary nerves. Palatine bone and pterygoid body along with the medial pterygoid plate were completely drilled to expose the tumor capsule, which was gently dissected off. Complete tumor excision was performed uneventfully. Conclusion: EET provides a good oncological and functional outcome. With a lesser risk of surgical morbidities, it is an excellent alternative to conventional skull base approaches to trigeminal schwannomas in the modern era.

Abstract:
Case summary An 8-month-old neutered male domestic shorthair kitten was examined for anorexia, lethargy and palatine ulcers. Systemic lupus erythematosus (SLE) was suspected based on a positive antinuclear antibody (ANA) titer and six manifestations of autoimmunity: fever, paronychia, oral ulcers, proteinuria, thrombocytopenia and leukopenia. Mastocytemia was observed on the blood smear. Although the clinical presentation of this case meets the classification criteria for SLE in humans, tick-borne disease and histopathology evaluation of the oral and cutaneous lesions would have been necessary to support a definite diagnosis of SLE. Baseline ANA titration was performed in two laboratories with conflicting results, which may reflect substrate differences used for the titration, but a false-positive result cannot be excluded. The cat received prednisolone and all clinical and laboratory abnormalities resolved after two months of treatment. Subsequent ANA titers remained positive and were not correlated to the patient’s clinical progression. Relevance and novel information This report describes new findings associated with a presumptive diagnosis of SLE in a kitten, highlighting that SLE may not be ruled out even in young cats and may be associated with mastocytemia. ANA titration is part of the initial diagnostic work-up of SLE but is a non-specific test and discrepancies can be observed between laboratories. The titration of more specific antibodies such as those used in humans would be helpful to diagnose SLE. ANA titration may not correlate with clinical activity of SLE; hence, the interest of an ANA titer follow-up to establish disease control warrants further investigation.

Abstract:
SUMMARY Objective: This randomized clinical trial evaluated the effect of 35% hydrogen peroxide in comparison with 37% carbamide peroxide in a nonvital bleaching technique of “walking bleaching” (four sessions of treatment) on periodontal markers: nuclear factor kappa B-ligand (RANK-L—process of root resorption marker) and interleukin 1β (IL-1β—inflammatory response marker). Methods and Materials: Fifty volunteers presenting with discoloration of nonvital teeth and endodontic treatment in good condition participated. Fifty teeth were randomly divided into two study groups according to bleaching gel: HP = 35% hydrogen peroxide (n=25) and 37% carbamide peroxide (n=25). Nonvital bleaching was performed with a walking bleaching technique consisting of four sessions of bleach application. Gingival crevicular fluid samples were taken in order to quantify the RANK-L and IL-1β levels by enzyme-linked immunosorbent assay. Samples were obtained from six periodontal sites for each bleached tooth: three vestibular and three palatine (mesial, middle, and distal) at seven time periods: baseline, after each of the four sessions of nonvital bleaching, at one week, and at one month after nonvital bleaching. Tooth color variations were analyzed in each session by VITA Bleachedguide 3D-MASTER (ΔSGU). Results: Significant increments in the RANK-L and IL-1β levels were detected in each evaluated time compared with baseline (p<0.05); however, no differences were detected between hydrogen peroxide and carbamide peroxide on increments of the biomarkers studied. The change of color was effective for both nonvital bleaching therapies (p<0.05). Conclusions: Nonvital bleaching induced a significant increment in the RANK-L and IL-1β levels in periodontal tissues around bleached, nonvital teeth.

Abstract:

Abstract:
Problem Palatine tonsils are secondary lymphoid organs active in sampling antigens entering the upper respiratory tract and in producing immunoglobulin (Ig) both locally and distally through migrating tonsillar B cells. Their size has been found to be directly proportional to the number of B and T cells. The aim of this study is to compare the change in serum and saliva Ig levels after total and partial tonsillectomy. Methods Children with obstructive tonsils and/or adenoid undergoing total or partial tonsillectomy were recruited in a pilot study. Patients with history of recurrent infections or immunodeficiency were excluded. Blood and saliva samples were obtained immediately preoperatively and at the 1st follow-up visit. Serum Ig's (G,M&A) concentrations were determined using radial immunodiffusion. Saliva secretory IgA was measured using an Enzyme Immuoassay (EIA). Results Twenty five patients (13 partial and 12 total) were included. There were no statistically significant changes in serum Ig's levels after total or partial tonsillectomy, except for IgM which increased (1.317 to 1.632 g/L) after partial tonsillectomy, more significantly in males, in those aged 5 years and older, and when the postoperative samples were taken within a 2 month-period. Moreover, there was statistically non-significant decrease in secretory IgA level after both total and partial tonsillectomy. Conclusion The effect of partial or total tonsillectomy on immunoglobulin level seems reassuring. The increase in IgM and the non-significant decrease in SIgA may represent a kind of immunological readjustment after tonsillar surgery. Significance This is the first study (to our knowledge) that compares the changes in immunoglobulin levels (in serum & saliva) after partial vs. total tonsillectomy. The obtained results are preliminary and are reassuring to the parents and physicians who have concerns regarding the immunological sequalae of tonsillectomy in children. A longer follow up will be done in a larger future study. Support This study was supported by Medtronic.

Abstract:
Focal palatine erosion (FPE) is a misleading term that is used in the literature to describe inflammatory lesions associated with depressions of the palatal mucosa in cheetah. Cheetahs have large cheek teeth and these depressions are formed to accommodate them. Previously FPE was only described as a mandibular molar tooth malocclusion on the hard palate due to suspected rotation and super eruption of the mandibular molar teeth of cheetahs aged 18 months and older. Two hundred and fifty six cheetahs (135 male, 121 female), originating from two independent facilities, had their oral cavities evaluated as part of an annual health visit over a decade. Ninety-nine cheetahs were seen once, 59 cheetahs were seen twice, 33 were seen three times, 43 on four occasions, 16 on five occasions, 5 on six occasions, and 1 cheetah was seen seven times. Apart from these clinical cases a prospective study on 5 cheetah cubs (3 male and 2 female) was conducted to document their skull development and mandibular molar tooth eruption over a period of 25 months. Of the 261 cheetahs observed none developed rotation or super eruption of their mandibular molar teeth. The term FPE is a misnomer as these inflammatory lesions were found in palatal depressions opposing any of the cusps of all of the cheetah mandibular cheek teeth. It consisted mainly of deep ulcerations, inflammation and oedema and also micro abscess formation. In severe cases oro-nasal fistulas were present. Of all the depressions present on the cheetah's palate, the large one palatal to the 4th maxillary premolar tooth was most commonly affected. In the five cubs evaluated prospectively, focal palatitis was evident from the 7 month evaluation, before all the permanent teeth erupted. Conservative treatment of the inflamed depressions by removing the foreign material through curettage and copious flushing reduced the grade of the inflammation when observed on follow-up. Focal palatine erosion is an incorrect term used to describe focal palatitis that occurs randomly in cheetahs. This focal palatitis is often associated with foreign material trapped in the palatal depressions. Conservative management is sufficient to treat these animals without odontoplasties.
