Full textWhipple’s disease (WD) is a chronic systemic infection with the ubiquitous bacterium Tropheryma whipplei. Whereas self-limiting infections with T. whipplei occur frequently and lead to a protective immunity, a chronic manifestation of WD is rare and seems to be associated with predisposing immunological factors. These underlying factors have not yet been sufficiently understood. The present work analysed T cell reactivity against T. whipplei antigens and interaction of the pathogen with peripheral blood mononuclear cells in vitro. Upon stimulation with bacterial lysate, recombinant GrpE or Hsp70 of T. whipplei, the proportions of activated CD4+ and CD8+ T cells were significantly lower in WD-patients as compared to healthy controls. Productive presentation of GrpE antigens via the MHC-I complex resulted in an increase of perforin+ CD8+ T-cells in WD-patients. The proportion of interferon-gamma+ CD8+ T cells was comparable to healthy controls, thus demonstrating that the reduced T cell response in WD-patients is not due to a T cell defect, but to a dysfunction in antigen presentation. T. whipplei infection of in vitro differentiated macrophages significantly reduced the expression of HLA-DR and CC-18 in WD-patients and controls, whereas bacterial lysate and bacterial supernatant did not induce phenotypic changes. These findings supported the hypothesis of an active immunomodulation by T. whipplei, resulting in reduced antigen presentation and enabling intracellular persistence. Microarray based transcriptome analysis identified 105 candidate genes that may be related to the chronic manifestation of WD. Clinical relevance of these genes has to be examined in further studies, as well as the importance of CD14+ depleted cells for the pathogenesis of WD. The detection of bacterial DNA in CD14+ depleted cells in the course of a prospective pilot study indicated that, in addition to monocytes, other cells are involved in the systemic distribution of T. whipplei.
