Abstract:
Heart diseases are among the nation’s leading couse of mortality and moribidity. Data mining teqniques can predict the likelihood of patients getting a heart disease. The purpose of this study is comparison of different data mining algorithm on prediction of heart diseases. This work applied and compared data mining techniques to predict the risk of heart diseases. After feature analysis, models by five algorithms including decision tree (C5.0), neural network, support vector machine (SVM), logistic regression and k-nearest neighborhood (KNN) were developed and validated. C5.0 Decision tree has been able to build a model with greatest accuracy 93.02%, KNN, SVM, Neural network have been 88.37%, 86.05% and 80.23% respectively. Produced results of decision tree can be simply interpretable and applicable; their rules can be understood easily by different clinical practitioner.

Abstract:
Wound healing is a well-coordinated process that restores skin integrity upon injury. However, some wound treatment poses harmful effects on the skin, which delay the normal wound healing process. Marphysa moribidii, a marine baitworm or polychaete, represents unique ability to regenerate posterior segment after injury, which may be beneficial in the wound healing treatment. The effectiveness of the polychaete as wound healing treatment was discovered through skin irritation, microbial testing, animal wound model, and chemical identifications. Three polychaete extracts (PE) emulsifying ointment (0.1%, 0.5%, and 1.0%) were topically applied to the full thickness wound model once daily for 14 days. Interestingly, PE 1.0% revealed the most rapid wound healing effects as compared to other treatments, including gamat (sea cucumber) oil (15% w/v) and acriflavine (0.1% w/v). Histopathological analysis using Masson’s trichrome staining further confirms that PE treated wound exhibited minimal scar, high collagen deposition, and the emergence of neovascularisation. The extract also displayed a minimum inhibitory concentration (MIC) of 0.4 g/ml against Escherichia coli and absence of skin irritation, infectious bacteria, and heavy metals from the extract. Moreover, chemical compounds such as alkaloid, flavonoid, amino acids, and organic acid were detected in M. moribidii extracts, which could contribute to wound healing activity. In conclusion, this study further justifies the beneficial use of polychaete in treating wound healing and could be developed as a novel bioactive agent in nutraceuticals and pharmaceutical drugs.

Thoracic sympathectomy is an important option in the treatment of palmar hyperhidrosis and pain disorders. Earlier surgical procedures were highly invasive with known morbidity, acceptable outcome, and established recurrence rates that were the limitations to considering surgical treatment. Thoracoscopic sympathectomy is a minimally invasive procedure that allows detailed visualization of the sympathetic ganglia and minimal postoperative morbidity; however, outcome studies of this technique have been limited. The authors treated 39 patients with 60 thoracoscopic procedures, and the outcomes in this small series were equivalent to previously established open surgical techniques; however, operative moribidity rates, hospital stay, and time of return to normal activity were substantially reduced. Complications and recurrence of symptoms were also comparable to previous reports. Overall patient satisfaction and willingness to repeat the operative procedure ranged from 66 to 96% in all patients. Patients and physicians can consider minimally invasive thoracoscopic sympathectomy procedures as an option to treat sympathetically mediated disorders because of the procedure's reduced morbidity and at least equivalent outcome rates in comparison to other treatments.

SUMMARYLack of access to safe water and sanitation contributes to diarrhoea moribidity and mortality in developing countries. We evaluated the impact of household water treatment, latrines, shallow wells, and rainwater harvesting on diarrhoea incidence in rural Kenyan children. We compared diarrhoea rates in 960 children aged <5 years in 556 households in 12 randomly selected intervention villages and six randomly selected comparison villages during weekly home visits over an 8-week period. On multivariate analysis, chlorinating stored water [relative risk (RR) 0·44, 95% confidence interval (CI) 0·28–0·69], latrine presence (RR 0·71, 95% CI 0·54–0·92), rainwater use (RR 0·70, 95% CI 0·52–0·95), and living in an intervention village (RR 0·31, 95% CI 0·23–0·41), were independently associated with lower diarrhoea risk. Diarrhoea risk was higher among shallow well users (RR 1·78, 95% CI 1·12–2·83). Chlorinating stored water, latrines, and rainwater use all decreased diarrhoea risk; combined interventions may have increased health impact.

Abstract:
Abstract Variables important in predicting the final postsurgical outcome of 435 patients who sustained missile head wounds during the Iran-Iraq War were evaluated over a 99-month period. The type of projectile, site of injury, and presence or absence of foreign material did not seem to have a significant effect on the final outcome. Of the patients with a perforating type of injury, 48.8% had a poor surgical outcome as compared with 19.9% with a penetrating type and 15.6% with a tangential type. This difference is statistically significant (x2 = 14.7 and 17.1, respectively; p &lt; 0.001). The most important factor in predicting overall outcome was the Glasgow Coma Scale (GCS) score at the time of admission. Mortality and morbidity contributing to a poor surgical outcome were noted in only 6% of patients with a GCS score at admission of 13 to 15, in 24.6% of those with a GCS score of 9 to 12, in 57% of those with a GCS score of 6 to 8, and in 65% of those with a GCS score of 3 to 5. Of the 71 patients who died, 75% had a score of 3 to 8. Perforating projectiles or those traversing two or more dural compartments were statistically significant in contributing to mortality and moribidity (x2 = 17.2; p &lt; 0.001). The incidence of focal neurological deficit was 100, 90.6, 88, and 52.2% in patients with GCS scores of 3 to 5, 6 to 8, 9 to 12, and 13 to 15, respectively, The two best predictors of mortality in this group of patients were a low GCS score and infection.

Abstract— Covid -19, a virus strain and a member of family with known member as Severe Acute Respiratory Disease (SARS) or Middle East Respiratory Syndrome (MERS), but having a potentially lethal differences in not readily recognized by our immunity. Classified as betacoronavirus, with incubation period up to 14 days, the sign and symptoms can be highly variable, from asymptomatic to mild to criticalin the form of severe pneumonia. Almost half the persons infected did not show any symptoms, but in cases of people with comorbidities such as advanced age, pregnancy, obesity or other certain diseases, could increase the likelihood of morbidity and even potentially fatal. Covid-19 has spread globally, including Indonesia, with a potentially increased number of cases, prevention in covid-19 protocol is a necessity. Abstract:
L'amélioration de la prise en charge thérapeutique des patients infectés par le VIH, en diminuant les causes de morbi-mortalité dues à l'infection et en prolongeant la durée de vie des patients, a fait émerger la problématique du tabagisme. Nous avons mené nos travaux de recherche autour de deux axes dont les principaux résultats sont les suivants. 1) Importance du tabagisme (fréquence et complications). La prévalence du tabagisme est plus élevée chez les patients infectés par le VIH (51%), en comparaison à la population générale (27%), et la consommation de tabac se poursuit à des âges plus avancés, augmentant le risque de complications liées au tabac. Nous montrons aussi que l’arrêt du tabagisme est bénéfique pour prévenir les pneumopathies bactériennes, quel que soit le niveau d'immunodéficience. 2) Prévention du tabagisme. Si 40% des patients infectés par le VIH sont motivés pour arrêter de fumer, ils sont confrontés à des obstacles majeurs tels qu'une dépendance élevée avec, fréquemment, des co-dépendances et des symptômes dépressifs. Près de 90% des fumeurs infectés par le VIH nécessitent une prise en charge médicale spécialisée pour arrêter de fumer. Nous montrons également que les tentatives d'arrêt du tabagisme sont fréquentes et que celles-ci ne sont pas liées à la sévérité de l'infection à VIH. Ces résultats nous ont permis de définir les mesures de prévention les mieux adaptées et de mettre en place un essai clinique randomisé, en double insu, dont l'objectif est d'évaluer l'efficacité et la tolérance de la varénicline dans l'aide au sevrage tabagique chez les patients infectés par le VIH, avec une prise en charge systématique des épisodes dépressifs
The Improvement of the therapeutic management of HIV-infected patients, reducing the causes of morbidity and mortality due to HIV and increasing the life expectancy of patients, yielded to an emergence of the issue of tobacco smoking. We have conducted our research around two axes; the main results are as follows. 1) Burden of tobacco smoking (frequency and complications). Smoking prevalence is higher among HIV-infected patients (51), compared to the general population (27%), and tobacco consumption continues to older ages, increasing the risk of tobacco-related complications. We also show that smoking cessation is beneficial in preventing bacterial pneumonia whatever the level of immunodeficiency. 2) Prevention of tobacco smoking. If 40% of HIV-infected patients are motivated to quit smoking, they face major obstacles such as high dependency with frequent co-dependencies and depressive symptoms. Nearly 90% of HIV-infected smokers motivated need a specialized medical management to quit smoking. We also show that smoking cessation attempts are frequent in HIV-infected patients and that they are not related to the severity of infection. These results enabled us to identify the most adapted preventive measures and to implement a randomized double-blind clinical trial aiming at evaluating the efficacy and safety of varenicline for smoking cessation in HIV-infected patients, with a systematic management of depressive symptoms
