Full textBackground: Good quality care means nursing with compassion and experiencing inner satisfaction of doing so. Nurses and patients are in agreement on that. Nurses who fail to experience compassion satisfaction can end up in a state called compassion fatigue which is a condition that is due to the high stresses that arise at work. Aim: To describe nurses' experiences of achieving a sense of power through care. Method: A general literature review was conducted on two quantitative articles and eleven qualitative articles. The analysis was conducted according to Friberg Result: The results showed different factors that bring forth a sense of joy and motivation in the nursing profession. Four different categories were revealed: interpersonal confirmation, moral satisfaction, existential satisfaction, professional recognition and development. The categories describe different sources of satisfaction and the empowering elements that supports retention in the profession. Examples of factors are: a chance to make a difference for the patient, being able to be autonomous and getting confirmation from colleagues and the management. Conclusions: Nurses can find power in caring, however, there are also nurses who fail to experience compassion satisfaction. More knowledge about compassion satisfaction can benefit nurses in the future.

Full textAbstract The purpose of this study was to describe patients' experiences of receiving physical activity prescribed by the district nurse, and the possibility of adhering to the prescription. The study had a descriptive design and a qualitative method was used. Ten women and ten men who received physical activity on prescription from two different medical centers in central Sweden participated in the study. Data was collected through semi-structured interviews. The interviews were analyses with a qualitative content analysis. The analysis resulted in one theme: Positive and simple, yet difficult to adhere to the prescription and change their lifestyles. The analysis resulted in four categories: To see physical activity and changes as an opportunity, To see physical activity and changes as an obstacle, To become aware of the need for physical activity and To take control or not over their physical activity. Most of the participants in the study were in favor of the prescription. Many experienced the physical activity prescribed as a motivation to start exercising, some had even changed their lifestyle habits and diets. Various obstacles and circumstances contributed to why prescriptions were not adhered to. The conclusion is that the patients in general proved positive to physical activity on prescription, however it may sometimes be difficult to adhere to the prescription and change lifestyles due to lack of motivation. Keywords: District nurse, physical activity on prescription, motivation, patient, experience

Full textSuffering is an unavoidable threat to the individual as a whole. Suffering of patient and relatives affects the nurse, and creates a personal suffering when incapable of relieving the patients suffering, or inflicting even more. The purpose of this study was to describe how ICU nurses experienced caring for suffering patients. A descriptive, semi-structured interview study was conducted on ICU nurses and interpreted by content analysis. Result: The theme Suffering permeates the nurse’s world was identified and described how the suffering of patient, relatives and self had to be handled by the nurse. The theme consisted of three categories. The manifestation of suffering described how suffering in the ICU is perceived. The interaction of suffering described how cooperating with suffering patients, relatives and colleagues caused negative and positive emotions. Integrating suffering of the other described often negative personal emotions of the nurse evoked when encountering suffering.

Full textIn the work at an emergency department, nurses are regularly exposed to traumatic events. This can adversely affect both mental and physical health and affect the quality of care. Psychosocial work environment loads occur to a large extent, and nurses in emergency health care are highly represented in the sick leave statistics in Sweden. Working close to severely ill patients who undergo severe psychological trauma can adversely affect the nurse and an acute secondary stress can occur. Secondary traumatic stress (STS) and post-traumatic stress disorder (PTSD) are two forms of anxiety disorder that can occur after individuals experience, participate or witness an emotional traumatic event. Research is shown on the presence of both STS and PTSD symptoms in nurses working at emergency clinics which could be related to the prevalence of traumatic events. Defusing and psychological debriefing are examples of two forms of structured conversation that’s been proved effective as support for traumatic events for nurses. The aim of the study was to investigate the emergency department nurses' psychological reactions and needs after a traumatic event. This with a focus on defusing and psychological debriefing and what barriers that may exist for implementation in the working environment. The method used was of qualitative design. The data was collected through physical, semi-structured interviews with nine nurses who were working at two emergency departments in central Sweden. The data was analyzed by latent content analysis. The result was presented in three themes with eight sub-themes. The emerged themes were: The Individual, Needs and Organization. The personal and professional experience felt to be of importance for how nurses react to and handle traumatic events. This affected the perceived need for support. Also, social support such as colleagues and friends were components that influenced the experienced need. Most of the nurses experienced a need for structured conversations such as defusing after a traumatic event, mainly focusing on facts and confirmation of the event. Organizational barriers obstructed the possibility of defusing and psychological debriefing. The conclusion was that the need for support such as defusing and psychological debriefing after traumatic event in work at emergency department was an individual experience impacted by numerous various factors. The nurse as individual has various needs of support conversations after a traumatic event based on earlier private and professional experiences. Facts and confirmations about the event appeared to be important in order to be able to handle and process traumatic events more easily. Most of the nurses experienced that the general need for conversations after a traumatic event was substantial. The structure of defusing was more appealing to the informants compared to the structure of psychological debriefing, as it was regarded as too complex. The study illustrates a need of the nurses that the organization fail to meet. Deficiencies in time, resources, leadership and routines emerge.

Full textBackground: According to the recommendations of the National Board of Health, pain estimation should be made in the end of life. Lack of pain relief at the end of life is no one wants to experience. If the district nurse has a good routine for pain assessment instruments, it provides a quality assurance for the palliative patients. Aim: To describe district nurses' experiences of pain estimation in palliative patients. Method: A qualitative empirical study with descriptive design. Result: Three categories emerged from the analysis; the organization's influence, methods for pain estimation of palliative patients, the organization's importance for quality improvement with underlying subcategories. The study showed that district nurses lacked routines for pain estimation. District nurses pain assessment based on their clinical gaze, experience and pain assessment tools. The district nurses were aware of that pain should be assessed according to pain estimation instruments, but the instruments and their documentation were sometimes forgotten even though it had been used. District nurses believe that pain estimation with instruments can be improved through training in the instruments and routines of pain estimation. Conclusion: For district nurses, it would be easier with clear routines on which pain assessment instruments should be used in specific situations. Education is demanded in pain and the assessment tools to bring about frequent use of pain assessment instruments.

Full textDespite the vast amount of research on work motivation, theorists still lack consensus regarding what truly motivates employees; yet employee motivation is widely recognized as critical to organizational survival in the global marketplace. In the knowledge economy, knowledge workers are the key to knowledge creation and, thus, organizational survival and success. Thus, organizations that wish to survive in the current economy must find ways to capitalize on the strengths of knowledge workers by developing an understanding of the motivating forces driving knowledge workers. Research suggests differences in employee motivation across age groups and generations. As Traditionalists and Baby Boomers approach retirement and exit the workforce, Generation Y, born between 1981 and 1997, is becoming a major part of the workforce. Thus, organizational leaders must develop an understanding of what motivates knowledge workers from Generation Y to contribute to the organization's goals and objectives, thereby contributing to higher levels of organizational performance; however, scholarly research has yet to address the work motivation experiences of Generation Y knowledge workers. This grounded theory study explored the work motivation experiences of Generation Y knowledge workers through a review of key motivation theories and exploratory, in-depth interviews with Generation Y knowledge workers in the healthcare industry, toward an understanding of Generation Y's intrinsic and extrinsic motivators, demotivators, and process of and approach to work motivation. The data revealed that Generation Y knowledge workers in the healthcare industry have a strong desire to contribute, both to their organizations and to their communities. The desire for purpose, which can be achieved through the attainment of social acceptance and self-worth, served as the primary motivator for the research participants.

Full textSubstance abuse is the source of significant financial cost to our society and of more indirect interpersonal costs. It is estimated that only about 18% of people classified with either substance abuse or dependence in 2012 received treatment (SAMHSA, 2013). Drawing from the existing theories of motivation to change, theories regarding the contribution of effective coping skills, and various explanations of what motivates substance abuse recovery, this study investigated the phenomenon of natural recovery. Klingemann, Sobell, and Sobell (2010) described natural recovery as being when a patient interrupts problematic behaviors or experiences improvement in his or her condition without formal intervention. Despite the existing body of research on many substance-abusing populations, there remains little research focusing on motivation to change for adolescent methamphetamine users who recover naturally.
This study utilized a grounded theory design, as outlined by Maykut and Morehouse (1994), to retrospectively examine the experiences of emerging adults (18–28) who recovered without the use of formal treatment in order to identify factors related to motivation to change. Participants were required to have been formally diagnosed or met criteria for methamphetamine abuse or dependence between the ages of 12–18. This study included 10 participants. By attempting to understand the experiences of emerging adults, this study hoped to expand upon the understanding of motivations for those who recover naturally from methamphetamine abuse.
Participants needed to have at least 12 months of sobriety from methamphetamine use. The researcher conducted semi-structured interviews with participants in order to highlight the factors motivating change in recovery and discover overarching themes as they relate to motivation to change and change behaviors. These themes were integrated to illustrate the complex interaction between participants’ motivation to change. The results were also compared with current relevant substance abuse treatment literature. This study will help to expand existing knowledge regarding motivation to change and methamphetamine abuse, as well as adolescent substance abuse treatment. Future investigations could attempt to apply these characteristics to adolescents who are at risk of or actively involved in a substance abuse treatment program.

Full textBackground: Sweden is changing and becoming a more diverse society. Nurses need to work holistically in order to provide a patient-centered care. Challenges may arise when there are communication difficulties due to language barriers and different cultural traditions, which may cause different values about what health-care involves. Aim: To illuminate nurses' experiences of cultural encounters with patients Method: A literature review with grounding in qualitative approach. Results: A cultural difference was something that brought a reaction of the nurses in the encounter with the patient. Something that was partly due to different values, traditions and attitudes. The most prominent was the language barrier and also relatives' participation in their care; one aspect that many nurses felt that it hindered the possibility of an individualized care. Other differences were food culture, religion, and emotional expression and behavior. Conclusion: These results were consistent with previous research undertaken in the chosen phenomenon. The nurses who rarely met patients from other cultures usually experienced major challenges in nursing, while those nurses who had more frequent cultural encounters and more experience tended patients with a different cultural background showed a higher level of cultural competence. The need for further knowledge and education about other cultures is something that could be implemented during nursing education in order to prepare nurses to cultural care encounters. A more diverse set of staff with different cultural backgrounds could enrich and facilitate nursing care. Clinical significance: There is both a lack of knowledge and communication difficulties in cultural encounters in health care. Professional interpreter should convene when necessary and develop cultural awareness through clinical training of nurses and other healthcare professionals.

Full textTitle: Lifestyle change in type 2 Diabetes - an interview study from the patient´s perspective Course: Caring science, Independent project (Master) in Primary Health Care Specialist Nursing, 15 credits Pages: 33 pages Authors: Johanna Rosell & Michaela Sorttanen Supervisor: Cecilia Fagerström Examiner: Ulla Peterson Background: Type 2 diabetes is a growing and common public disease that is linked to lifestyle. Lifestyle changes such as healthy diet combined with physical activity are the basic components of diabetes treatment. Lifestyle changes prevent complications and give great health benefits. Objective: The purpose of the study was to describe experiences of lifestyle change in patients with type 2 Diabetes. Method: Qualitative interview with six patients with type 2 Diabetes were conducted, between ages 48 to 70 years. The interview were analyzed with a qualitative manifest content analysis. Results: Three main categories and seven subcategories were identified from the analysis. Main categories were; The nursing relationship with the diabetic nurse, which represents the patient´s experience that the diabetes nurse has a significant role in motivational work on lifestyle changes. Based on the experience of patients, it emerged that active involvement and person-centered work from the diabetic nurse motivated and maintained motivation for lifestyle change. Furthermore it was motivated to be informed of the measured values of the diabetic nurse. Patient´s attitude to lifestyle change category found that patient´s attitude played a significant role in how lifestyle changes are followed. The motivation for lifestyle change varied depending on the patient's disease insight, responsibility as well as habit patterns and past behaviors. External factors in terms of social influence from family and friends were experienced to have both a positive and negative impact on the motivation for lifestyle change in patients. Likewise, everyday circumstances such as weather and work affected the patient's motivation to implement recommended lifestyle changes. Conclusion: A person-centered and committed work by the diabetes nurse creates and maintains motivation for lifestyle change in patients. A good medical relationship with mediated knowledge enables patient insight and responsibility. Good and committed cooperation between diabetic nurse and patients motivates lifestyle changes and preserves a healthy lifestyle.

Full textBackground: In primary care physicians and district nurses faces on a daily basis patients seeking help for symptoms that are directly related to smoking. Working disease prevention through lifestyle changes is an important part of the district nurse's responsibility. According to the National Board of Health (2011) National guidelines there are clear recommendations for district nurses to proceed with patients seeking help to quit smoking. Objective: The objective was to highlight the district nurse practice and experience of working with smoking cessation for patients in primary care. Method: A qualitative method was used, and 14 semi-structured interviews were conducted with district nurses in Southern Sweden. The data was analyzed according to the method manifest qualitative content analysis. Results: The analysis of the data resulted in two categories: The primary nurse approaches and factors affecting smoking cessationwith five sub-categories in each category. The results of the study show that the district nurses working methods consists of Motivational Interviewing, MI and qualified advisory conversations with patients and smoking cessation. District nurses common experience that these two methods are the two most effective to use to find out how determined and how motivated the patient is to quit smoking and how they together can plan the way to quit smoking. District nurses experience from smoking cessation is that it gives them positive feedback when they manage to get the patient non-smoking and it gives positive energy to continue with the motivational work. An identified general weakness are shortcomings in monitoring whether patients' abstinence has been sustained after the treatment is finished. Conclusion: Motivational interviewing and nicotine replacement medicine was described by the district nurses as important in the quest for supporting patients to quit smoking. Support of colleagues, training in communication technique in the form of network meetings and work were important parts of the districts nurses and facilitating them in their work is often very difficult and important task to try to support patients to smoke.

Full textBackground: Nursing profession has undergone major changes in recent decades. The nurse is expected to be a link between all parts in the care chain in an organization where reorganization takes place and working conditions are continuously changing. Stress is an internal phenomenon in care work and is just as the concept of health of highly individual and complex. The nurse needs to take care of her/his own health in order to the care of patients. While the nurse has a responsibility the prerequisite for being capable to provide good care is that the organization, management and workplace is designed to make it possible for the nurse to maintain health. Aim: The aim was to describe nurses' experiences of their work situation in order to understand how it affects their own health. Method: The thesis is a literature review. The result is based on nine pieces of scientific articles. The articels were read several times and the similarities and differences were identified. Results: The following main themes emerged; nurse's experience of the organization, the nurse's experience of management, the nurse's experience of stress and nurses' experience of meeting the patient. The second main theme has three sub-themes; not to be listened to, not to get support and to get support. The main theme of three has two sub-themes; negative stress and positive stress that develops. Discussions: The results were discussed using transition as a theoretical base. The nurses had rather similar views about their work situation as showed also in other studies. They experienced a high degree of stress and dissatisfaction with the management and organization in their workplace that highlights the deficiencies in management, and organization that have strong impact on health.

The analysis of the results shows that the pupils have a good understanding for what influences their motivation. They give us many examples where physical and psychical wellbeing plus the teacher`s pedagogical work in the classroom are of importance for the motivations process. The results from the inquiry further shows, within the frame for this limited study, that pupils in class three and five are more negatively disposed to mathematics compared to pupils in class one. To illuminate the subject, theoretical aspects from pedagogical research on the subject motivation, are also accounted for. These are discussed in the last chapter related to the results of the study.

Full textBackground: The Liverpool Care Pathway is a guidance of care for the dying patient. It was created to transfer the hospice model of care into other hospital settings. LCP provides a pathway in communication within the multidisciplinary team, with relatives and the dying patient. LCP gives guidance for doctors to prescribe medication for the most common symptoms that can appear in the dying phase to improve symptom management. LCP is today used in several places in Sweden within different frameworks of medical facilities. Aim: The aim was to illustrate the use of Liverpool Care Pathway in an acute hospital setting from a nurse perspective. Methods: This essay is a systematic literature review and the data was based on nine articles of published research. The published articles were found in four different databases. These were: Pubmed, Chinhal, Medline and ProQuest Nursing & Allied Health Source. The data was then analyzed and structured into different main themes, and was then compiled into a result. The articles were based on studies made in England, New Zealand and Italy. Results: When analyzing the articles six different main themes emerged. These were: communication, documentation, increased self-confidents, medical guidelines and care, improved end-of-life care and improved continuity in the care of the dying. Discussion: The studies relevance to Sweden was discussed because none of them had been made in Sweden. The main themes were related to previous research and to Katie Eriksson's concept of suffering and care.

Full textNurses’ knowledge of health education can always be improved. A method to conduct health education can be the use of motivational interviewing (MI). The purpose of this study was to describe nurses’ experiences’ of the motivational interviewing during patient counseling. 13 articles were reviewed, quality controlled and used in the result, which formed four categories. The result showed that health is an important part in nurses' communication with patients and covers a large part of the nursing care. It appeared, however, that knowledge and education to some extent were lacking, to in a good way keep a patient counseling. MI is proved to be a good method to carry out health advice and was able to increase nurses' educational insight. Individualized information and open-ended questions facilitated the patient's reflection on their behavior. It appeared to be difficult for the nurse to resist the feeling to assume full responsibility for patient behavior change but could by MI-training be improved. Lack of time was something that made it difficult for nurses when they would use MI. The perceived lack of time also affected the adherence and continuity, which was significant in behavioral change. Many nurses felt that the MI was something all health professionals might have use for. In-depth training, school and business, is desirable in order to improve the communication skills of nurses.

The Health Care Nurse plays an important role in motivating patients to implement lifestyle changes. Unhealthiness causes risks of developing long-lasting diseases and other complications. Living conditions and living habits are of great significance in terms of human health. Lifestyle changes, in contrast to drugs, often have an impact on both risk markers and states of diseases. The purpose of this study was to describe district nurses’ experiences when motivating patients to take part in health-promoting lifestyle changes. The study is based on a qualitative method with an inductive approach. Through interviews, data was collected from six health care nurses and analyzed using a qualitative content analysis. The results are based on two categories, quality of the professional support together with evidence and experiences important to the changing process. These categories formed the theme: Health Care Nurses´, sensitivity and knowledge are the guiding principles in terms of motivating patients to pursue a healthier lifestyle. The results of our study show that it is important to listen and to acknowledge the patient to be able to support them emotionally. Undergoing lifestyle changes require explanation and information. The changing process requires tools, the informants must have a reflective attitude to be able to work based on evidence. The patient needs to realize the importance of making a lifestyle change and also be prepared to take the losses. When doing so the patients will learn that they gain more than they lose.

The study aims to explore the motivation and experience of Tzu Chi couples volunteer joining social services. And to further understand the impact of the joint participation of the couple in volunteer services on work, career and family. In the face of challenges, how do they overcome and persist? This research adopts qualitative research methods, mainly through the collection of relevant data, personal participation in observation and in-depth interviews. The interviewees were three generations of old, middle, and young; four pairs of couple ; and eight Tzu Chi volunteers. The results of the study show that respondents’ motives for participating in volunteer work are attributed to charitable personalities, as well as being inspired by the moral integrity of the Master Dharma Mater Cheng Yen, the selfless dedication of the Tzu Chi people, and the simplicity of the Hualien Jing Si Abode. After participating in the Charity and Disaster Relief Activities in 921, most people realized the truth of “see suffering and recognize blessings” and “life is impermanent,” and then they changed their bad habits or improved the family-child relationship, and in the circumstance of “ more and more happy”, they encouraged their spouses to join the volunteer service. The benefits of joint participation of the couple in the volunteer include:There is more time to share life stories with each other. At the same time, they can complement each other in work, family, and volunteer work to support and learn from each other. Although the long-term volunteer activities will inevitably lead to frustrations in personnel management and communication, or due to physical load problems, which may generate the idea of displacing volunteer activities. However, Tzu Chi’s efforts to create a volunteer environment for the development of true, good, and beautiful volunteers, the mutual support and care of Tzu Chi’s selflessness, and the more intelligent expression of the interviewees’ more and more happy are the key factors in the joint and continuous input of Tzu Chi’s couple volunteers. This study looks forward to providing objective references and suggestions to the couple volunteers.
