Full textHome health care is heavily utilized to assist homebound clientele with a variety of medical needs. Home health services greatest population is the baby boomers. This population has multiple chronic medical problems that requires surgical intervention, disabilities that limit mobility, many suffer from a loss of mental capacities, or they suffer from depression which affects their physical and mental well-being. Because this population will continue to grow over the next several years, there will continue to be a growing need for home health care. RTK Home Health Services proposes to improve the home care delivered to this population by introducing a new psychological concept that is currently helpful in Psychiatry. The goal is to introduce this service during therapy sessions to decrease the emotional perception of pain by meditating. The full name of this service is Mindfulness Based Stress Reduction Techniques. It is accomplished by instituting meditation prior to therapy to convince the client to control and manipulate their perception to pain during therapy.

Full textSyftet med detta projekt var att undersöka inställningen till eHälsa inom äldrevården. Projektet initierades på grund av att det fanns ett behov av nya tillvägagångssätt för att ta hand om äldre människor. Målet var att analysera inställningen till eHälsa, och om möjligt, finna faktorer som kan påverka implementeringen av eHälsotjänster. För att lokalisera inställningen gjordes enkätundersökningar med personal inom hemtjänsten samt med allmänheten. Även intervjuer med kommuner utfördes. En påbyggnad av Technology Acceptance Model användes för att lokalisera upp- fattningen kring sju kategorier relaterade till eHälsa. Enkätundersökningarnas resultat analyserades statistiskt, samt så utfördes en faktoranalys för att hitta grupper som hade liknande uppfattning gällande eHälsa. Analyserna visade att den allmänna uppfattningen gällande nyttan med, och användarvänligheten av eHälsa är positiv. Majoriteten anser att samhället behöver investera mer pengar i området. Faktoranalysen resulterade i sex olika grupper av inställningar inom personalen samt tre grupper för allmänheten. Intervjuer med kommuner påvisade att en klar och tydlig definition av eHälsa saknas. Trots det, har de i stort en positiv inställning till eHälsa och dess utveckling. En slutsats är att tydligare riktlinjer rörande arbete med eHälsa skulle kunna möjliggöra en smidigare implementering.

Full textThe municipality is a large employer and the activities within the municipal home care servicealso affect many people who encounter employees within the home care service in the role ofcare recipient. To fulfill the organization's goal that good nursing should be conducted; researchhas previously mostly focused on the importance of a successful leader. In recent times, theimportance of followers for both organization and leadership has also been highlighted. The theory of Leader-Member Exchange is about the relationship between leader and followerand the exchange that takes place there. The leader values the follower's contribution to theexchange in the relationship based on his general notions of followers, also called ImplicitFollowership Theories. Previous research has shown that if these general perceptions of theleader are of a positive nature, it increases the leader's expectations of the follower and canimprove the relationship. The purpose of our survey is to investigate how leaders in themunicipal home care service view followers and whether they feel that the followers affecttheir ability to lead. Semi-structured interviews and thematic analysis form the basis for this study, which wasconducted in a management group within municipal home care. The results initially describethe participants' perception of the concept of following and what characteristics and behaviorsare included in their general perception of followers. Furthermore, the results of the surveydescribe how the participants perceive their actual followers and that their general expectationsof followers do not fully correspond to the actual followership. Finally, the survey clearlyshows that following has an impact on the conditions for leading.

Full textThe demand from care homes on NHS services continues to rise, with little evidence of ambulance service contribution in this area. The Yorkshire Ambulance Service provides an advanced practitioner model to support care homes in Sheffield, as an alternative to calling 999. This study investigated the experiences and needs of the care home staff who use the ambulance service advanced practitioner model. This qualitative study conducted semi-structured, face-to-face interviews with 19 staff members from 10 different care home settings. Thematic analysis using a combination of NVivo and manual coding was undertaken. The three key themes from the interviews were variations in service demand, the service user’s expectations and experience, and benefits to residents. Participants reported that good community services reduced the need to call 999, empowering carers to support residents to remain in the community. Care homes require comprehensive services that meet their needs. The advanced practitioner model provided by the ambulance service supports this, preventing unnecessary 999 calls and fitting with other community service provision.

Full textENGLISH ABSTRACT: Legislation encourages local government! municipalitiesl local authorities in South Africa, to fulfil a development role. One of the main objectives of municipalities performing a development role is to empower communities, especially previously disadvantaged communities. This study argues that the services delivered by municipalities are an essential component of a development orientation. In view of this fact, the study proposes an operational framework, whereby community empowerment can be achieved through municipal service delivery. The operational framework suggest that for community empowerment to be achieved through municipal service delivery, requires that the empowerment enabler (municipalities or departments within municipalities) should assure that: disadvantaged communities have access to services, services must be delivered in a non-discriminatory manner, the community should understand the rationale as to why the service is delivered, opportunity should be given for community participation in the delivery process, there should be a constant information channel between the giver (enabler) and receiver of services, and human resources from the local community should be utilised where possible in the delivery process. Lastly, a descriptive evaluation is undertaken of the health department's approach (at the Stellenbosch Municipality) to the delivery of primary healthcare services and service infrastructure. The purpose of the evaluation is to ascertain whether the principles as proposed in the operational framework are present in the health department's approach to service delivery. The evaluation reveals that most of the proposed principles of the operational framework features in the health department's approach to the delivery of primary healthcare services and services infrastructure. Thereby, concluding that the health department follows to a certain extent an approach to service delivery that could ultimately lead to community empowerment.
AFRIKAANSE OPSOMMING: Wetgewing vereis dat plaaslike regering/ plaaslike owerhede/ munisipaliteite in Suid-Afrika, 'n ontwikkelingsrol moet vervul. Een van die doelstellings van 'n ontwikkelingsrol vir munisipaliteite, is om gemeenskappe te bemagtig, spesifiek gemik op agtergeblewe gemeenskappe. Full textIn many developing countries including South Africa, there is an overwhelming number of people infected with HIV/AIDS in the hospitals, which the health care workers are unable to cope with. Therefore, an urgent need to develop alternative community based activities such as support groups, home based care programs and placement of 
orphans, for the provision of social health care services to the HIV client, arises. However, a deeper understanding of the challenges facing community organizations when 
providing home based care is crucial and necessary in order to formulate effective and relevant care services. The aim of the study was to explore the challenges faced by Umtha 
elanga Community Organization (UWCO) in the provision of HIV/AIDS home based care. The objectives of the study were to assess the current home based care of the UWCO and to explore the challenges that community based organizations have to face when providing care services to people being infected and affected by HIV/AIDS. The research design undertaken was the case study of UWCO. The qualitative approach with in-depth interviews with the 
management staff and home based caregivers was utilized. Data analysis began immediately after the interviews by transcribing and translating the audio-taped data. Thematic analysis was used in which data was coded and categorized. The findings of the research 
indicated that the home based care programme assists PLWHA clients in their natural home setting. All home based carers work voluntary and get supported by the organization as 
ell as by the department of social development and health such as the social workers. Finances appear to be, both for the organization and the home based carers, a huge 
hindrance. Research indicated that the organization experiences various challenges to provide the service to their community. It is recommended that Government must ensure that 
home based care workers get recognition, credit and support for their valuable contribution. Training for HBC employers needs to be extended to expose and add more skills. This 
will ensure that they are equipped for their tasks. Home based care workers must become registered and paid better salaries by the government.

“Social exclusion” is a hot issue in social policy. Researches focus on the impact of “social exclusion” in institutional failure recently. Social exclusion has expanded into home care service to highlight the disadvantages experienced by service users, and focused on how to achieve "social inclusion". On the other hand, the demands for long-term care will undergo rapid growth in Taiwan. Currently the government is already engaged in planning the “long-term care insurance system” to respond the demand of caring service. However, the past researches can not provide a comprehensive perspective to understand the ways in which policies and practices create and sustain difficulties for people receiving care. This research is a pilot qualitative study to describe the impact of “social exclusion” in home care service. The results revealed different types of social exclusion in Taiwan’s home care service, “self exclusion” especially. This research suggests initiate inclusive policy-making and service delivery, and empower service users not only minimal maintenance at home but also their full participation in social and community life to achieve “successful aging”.

In Taiwan, the home care service has been developed over 26 years since 1983. However, due to the lack of coordination and integration of the government, the elderly care is conducted by three systems, namely, Social system, Health system, and Veteran system, respectively. To reflect the problem of population aging, the government implemented a ten-year project of long-term care to integrate the resources of related departments for developing comprehensive care, local aging, and diverse services. The government also will start the insurance of long-term care in 2011. Therefore, the home care service industry is predicted to be prosperous. In this thesis, we propose a Home Care-service Management System (HCMS) to improve the operating efficiency of home care service dispatch organizations. The functions of HCMS include the service contents of home care and health care established by the Ministry of Domestic Affairs and local governments. Besides, due to the urgent requirement of the elderly health care, HCMS also has the function of vital sign measurement of the patient. The functional modules of HCMS can store and show the vital sign data and curve of the patient to control chronic diseases of the patient. HCMS records complete service data to be able to input data, output data, and produce reports requested by variant government units. In the further, these service data also may satisfy the requirement of the insurance of long-term care to supply more perfect home care service. Evaluated or used by two manpower-dispatch companies of home care service, HCMS is considered to be able to improve the service quality and operation efficiency of dispatch organizations.

Background: To implement the policies of the deinstitutionalization, developing an effective community-based service model is an essential part of a modern mental health system. However, the costs and care burden for both the caregivers and official medical care system are still higher than we expected. Purpose: The study was trying to explore the effectiveness of the hospital-based psychiatric home care service in a medical center of southern Taiwan, for the purpose of improving the current care model among the patients with schizophrenia. Methods: We applied a retrospective design had study during the period from January 2006 to December 2015. The participants who had received home care services for more than one year were enrolled. The primary outcomes were the numbers of acute ward admissions and the duration of each admission. The comparison of the main outcomes was to analyze before and after the enrolled date within a 1-year period. Results: The mean age of participants was 32.68. The most common treatment strategy for the participants receiving the home care service was long-acting injection of antipsychotics (83.3% of all participants). After adjusting the possible confounding factors such as the medication use, the major result of the current study was statistically significant decreases in both the numbers of acute admissions and the duration of each admission among the patients with schizophrenia who received home care service for the first period. Conclusion: The study revealed that even short-term home care service would be helpful to improve the re-hospitalized rate and the duration needed for admission among the patients with schizophrenia. However, due to the shortcomings of the pre-post study, further randomized, controlled trials are necessary.

Even though work is only part of one’s career and life, but it takes up a large amount of time and can help one reach self-actualization. Thus, a tight connection between the organization and an individual is formed at the very beginning of direct practice. An individual working in an organization no longer works to simply satisfy their basic living needs; instead, they become focused on gaining self-worth and recognition. This dissertation is based on a supervisor’s narrative on her social work career, from her learning experience to gaining basic trainings and finally to her entrance into direct social work, listing her experiences with the environment and interactions. Her life story is gradually formed through the constant process of “experiencing”, “reflectioning”, “contemplating” and “taking action”, which in turn formed into the complete sequence of events in her supervising career. The delivery of home care services depends on promotion from social welfare and health care workers and the social workers and nurse aides from partnering organizations. The organization’s supervisor acts as an agent in this kind of inter-professional collaboration by linking social welfare workers, health care workers, social workers, nurse aides, clients and their family members, in order to bring efficiency and effectiveness to the government programs and, at the same time, reaching organizational work tasks and goals. The chapters of this research are organized in the following order: how a supervisor recognizes and fulfills her roles in a home care service program; how to reach her full potential in the functions of her role under the expectations of the system; how to continue the process of self-reflection and learning when facing problems and dilemmas. The topics of the study include the dangers of the working environments of home care services, interaction experiences with related personnel of the program, the interaction and relationship between organizations, organizational development and change, supervising style of the supervisor, power and abilities.

Long term caring service industry operation and management – starting from the localization experience Abstract Recent years with the accumulation of the aging population and changing family structure, disabled elderly people has become the most significant problem among the greater society. The government has acknowledged this issue and became one of the most important tasks in the agenda, and various projects are developing henceforth. Apart from the “Caring service industry development project” and “ten-year long-term caring project”, the latest government action is to start long-term caring insurance plan at the year of 2011, and it is anticipated that via insurance mechanism, to solve the long-term service issues raised by the general public. This study mainly adopts qualitative methodology, and with the aid of small amount of questionnaires to conduct quantitative analysis, it is expected to draw different dynamic results via both qualitative and quantitative methods, in order to generate useful outcome in terms of long term caring service industry. Formal depth interviews will be conducted with key personnel including the National long term caring industry planning group. The interview will be formal, structured, and opened to interviewee for their crucial input towards long term caring service industry. In addition, questionnaires will be given to front line care-workers and to those families receiving the service. With questionnaires, it is anticipated to collect their thoughts towards the long term caring service industry. Consequently, by combining the interview and questionnaire data for analysis, it is expected to gain beneficial and deeper understanding towards future planning of service oriented industry. This study found that in order to conduct proper service-oriented industry operation and management, certain key issues are required. Firstly, organizations should have their employees well-trained in order to increase the managing capability, and secondly, they should develop care management system to service the disabled elderly people. Thirdly, to take significant consideration on the quality aspect of service work (ServQ), in order to sustain and maintain the service quality level.The conclusion of the study is provided for future planning of long term caring industry as a reference, practical skills, and theoretical discussion only. Background: Taiwan has entered the aging society. According to the Executive Yuan calculate that in 2025, more than 20% people will into the super-aged society. The statistics of Ministry of Health and Welfare showed there were 41,547 people received the "Taiwan Home Care Agency services" in 2104. It shows that home nursing service needs becomes an important issue. Purpose: The purpose of this study was to understand the nursing experience in providing home care service for disabled elderly. Method: A qualitative design was used in this study. In-depth interviewing and purposive sampling was conducted with the home care nurses working in home care agencies in Taipei were recruited. Data were collected from January 27, 2016 to March 31, 2016. Nine participants completed the interviews. The researchers continued to collect data until she was assured that saturation was achieved. Content analsysis was used to analyze the data. Result: Analysis of the interviews revealed six themes: assess needs and provide care ,challenging care situations and objects ,qualified for the position conditions ,facing multiple pressures, job in predicament and shock, and professional mission and expectations. The care services and connotation of conflict could be understood by the participants description through the interviewing. The reason of the participants’ keep working is that they could feel bitter and sweet sense of accomplishment through providing professional service. Conclusion/Implications for practice: Home care nurses provide experience in care of disabled elderly. Regardless of different educational, the home care services provided are the same. By accumulation and correction these care experience to find out suitable for home care. The elders are often facing the problem of deteriorating functional capacity which will worsen with age and may even lead to physically disability. The proportion of elderly in Taiwan is more than 14 percent as of April 2018. It is estimated that the number of disabled in Taiwan will reach 820,000 in 2018. Trying to free the disabled from the hassle of visiting the hospital, the government launched the National Health Insurance Integrated Home Care Project in 2016. However, the progress of providing home health care service (HHC) is hindered by the shortage of eligible staff. While there are some studies in Europe on dealing with this shortage problem with better staff scheduling, these studies consider nurse or caregiver only. However, the HHC in Taiwan is different from the existing studies in that it requires the service of doctors as well. This policy of including doctor visit for HHC is fairly new and thus there is a lack of study on this issue. This thesis is aimed to fill the gap between current study and HHC practice in Taiwan by formulating a mathematical model of staff and route scheduling for HHC in Taiwan. It is hoped that, by better scheduling, practitioners can provide HHC service more efficiently and assist more people in need with the limited resources. To this end, we will gather information by reviewing the policy of HHC in Taiwan and by consulting practitioners of a local hospital to better understand the current practice and the problems of HHC in Taiwan.

The Researcher tried to start from the home care workers’ point of views, explored the driving forces of continuous service, and retention or turnover, organizational commitment, job satisfaction, supervisory functions, human resource management, care relationship and social support networks as the basis of seven theories, a view can be the future home care in human resource management direction on efforts to strengthen their incentive to work and continued willingness to serve. The research has two purposes: 1. To explore the home care service experience in the course of continuous service; 2. To look forward to future studies as home care workers in the practice and the relevant policy reference amended. The research used qualitative case study research as the main method, and the in-depth interview to collect data for the research from Taipei County, two home care units in line with the fiduciary service: 1. The work of more than three years of service; 2. A week into hours of work over 30 hours; 3. Be willing to share, express himself/herself and provide information and so on the three indicators, such as home care workers a total of six. The results of the research show that: With regard to home care workers’ driving forces of continuous service can be broadly divided into the individual characteristics, the work itself, the organization, formal social support networks, informal social support networks, family relations with the clients, the supervisory functions and job satisfaction and so on the eight aspects. According to the results of the research, the researcher proposes suggestions for the practice, home care workers, policies and suggestions for future researches for reference.

The purposes of this study were to identify home care service users' and providers' perception of home care service quality, to explore its influential factors, and to analyze the gap between them. The samples include 112 home care workers and 218 service users from a home care service unit in Nantou-County. A structured questionnaire is used to collect data. Among service users, 200 valid questionnaires are collected, and the response rate is 91.7%. As to home care workers, the valid questionnaires received are 109, and the response rate is 97.3%. According to the results, the service users' perception of the five dimensions of home care service quality can be ranked from the highest to the lowest as assurance, responsiveness, reliability, empathy and tangibility, that is to say, "assurance" is the most powerful dimension of service quality. The mean value for the services was between 4.06-4.72. The item with the highest mean is "I have confidence in the home care services provided by the home care workers." On the other hand, the one with the lowest mean is "The home care workers can take my best interests into consideration."In addition, the service providers' perception of the five dimensions of home care service quality can be sorted from the highest to the lowest as assurance, empathy, responsiveness, reliability and tangibility. The results indicate that "assurance" is the most influential dimension of service quality. The mean value for the services was between 4.08-4.54. The item with the highest mean is "The service users have confidence in the home care services I provide." On the contrary, the one with the lowest mean is "The home care workers can perform the services accurately from the first time." Home care service users' and providers' attributes will lead to significant differences in the perception of home care service quality. It is found that there is no difference in the perception of home care service quality between these two parties. The findings of this study can serve as references for home care service units to improve their service quality, to increase in-service education opportunities for home care workers and to promote benefits for home care service users.

The study uses direct scaled measurement to survey Care takers’preference of the attributes of home care service. The content of the attributes of home care service were collected , analyzed , and integrated with the three levels of product . Questionnaires were designed accordingly . 114 effective questionnaires were collected from Care takers’.The response rate was 100﹪. Data collected were analyzed with the SPSS software. Main findings include: 1. With the Product Benefits :On average, the most important benefits were ‘convenience’ , saving time , and ‘ saving money . 2. With the Carrier Product : the most needed carrier product were ‘assessment of the needs of physiological care’ for the assessment of the demand of care , ‘Instruction of tube care technique’ for the instruction of care technique , ‘oral instruction’ for the method of care instruction ’ , ‘emergency handling’ for health eduction , ‘nursing personnel’for the service attendant , ‘psychological support’ for spiritual care , ‘listening’ for listening and sharing. 3. With the Augmented Product : the most important augmented product were‘quality of service’ and ‘image of organization’. The research also found that individual characteristics, such as sex , age , education , marriage , household income , cargivers replaceability , without attending of faoreign laborer , ability of self care , taking servuce of time ,are significantly related with preference . Final , suggestions based on research findings for developing new product concepts and implications for providing home care service were discussed.

Ethical issues in home care service has been neglected but not forgotten in the research. This thesis intends to identify them by interviewing the home care workers, their supervisors and administrators in the nonprofit organization in Taipei area. Five nonprofit organizations are chosen to interview ten home care workers, four supervisors and four administrators, respectively. Our findings show: I. The majority of respondents recognize the working rules as the code of ethics serving the “bilateral rights and interests” and “designated scope of work” functions. While administrators emphasis more on “job duties”, “behavior management” and “formal policy in dealing with ethical issues”, the home care workers and the supervisors focus more on “peer exchanging”. II. Supervisors usually use the "relationship" and "hierarchy" approach to get along with others. The home care workers and the clients intend to develop “friendship” or “family relationship”. The operation of home service is heavily dependent on “human factors” whose influence sometimes beyond the code of professional ethics. III. There are some unavoidable ethical issues in home care services: the latent period, repetitive violators, repeatability and interrelated issues, which can be divided into several types: (1) The Administration: Responsibilities inappropriate behavior, Attendance, Privacy; (2) Interaction: personal promise with clients, the dilemma of expecting multiple job duties. IV. Possible arising issues: (1) Different characteristics of home care workers and clients; (2) Cognitive improper service; (3) Human factors; (4) Interaction and Experience of the workers; (5) Insufficient training; (6) blurred services boundaries; (7) uncontrollable condition; (8) The needs of the client; (9) The best interest; (10) The effect of institutional; (11) Operating characteristics of home service; (12) Economic pressure. V. In responses, supervisors, administrators and home care workers express the anger, dismay and frustration emotions. Supervisors and administrators felt surprised because of home care workers’ behaviors and the home care workers felt contradiction about the standard of the institutions. VI. The treatment of these issues depends on the severity of the subject and the interaction between the home care workers, supervisor and client. They are (1) Institutions: use of progressive manners to re-educate the home care workers; (2) Home care workers: They act according to circumstances, except for the unique circumstances. In the end, several suggestions are proposed to the Institutions, home care workers and the government, respectively.

Taiwan has begun 10 years program for Long-Term Care since 2007, where home care service have gradually became a main generally accepted service. Self-paid home care service is grown since the aged society influences. The aim of this research is to explore the market of self-paid home care, researcher roles as a service provider experiences and observation on field, object studied from who accepted the self-paid home care service provided by researcher, and the home care service provider who in employment, the purpose of the research: First, experience the labor situation of home care service providers; Second, exploring the link factors of supply and demand of self-paid home care service; Third, review of researcher experience while providing home care service. This research takes the qualitative research method, collects the information on field by the researcher roles as the service provider, through analyze the researcher's field notes, the service objects and interview home care service providers. Through the provision of home care service practically and interview service objects to understand self-paid home care service aspects, and review the value from interview to home care service providers, and proposed self-paid home care service development proposals, the research found as follows: First, before researcher provide home care service, has accumulated practical experience of care service, the care services related abilities been trained, and according to elder care service foundation from working experiences, these become the source of self-paid home care service objects. Second, self-paid home care service market service provider needs following requirements: (a) Ability of market development; (b) Adequate service technical skills; (c) Ability of open-minded communication; (d) Adequate professional competitiveness Third, self-paid home care service providers still face the low-level stereotypes of society, and it will occurs phenomenon of labor alienation. With the growth of medical technology, the life span of human beings has been prolonged, and the elderly population in countries around the world has also increased year by year. In 2018, Taiwan officially entered the aged society, and the government implemented LTC 2.0 in January to improve the shortcoming of LTC 1.0. Expand service coverage and projects to try to alleviate the care needs of the elderly. However, since the implementation, the manpower is still insufficient, and some of the care needs can be improved improve the quality of life and care. Therefore, this research will be based on the case of home care in the Taiwan's LTC system. Conduct a preliminary analysis through literature collection to understand the development and current status of home care. Using in-depth interviews and field observations to collect data, through interview records to depict the patient and carer persona to define the segmentation and needs. Establish a care journey map based on on-site observations and define the pain points that make both sides feel bad. Finally, based on the pain point needs, put forward a hi-tech. solution and draw a service blueprint.

Ucarer is a Taiwanese-based startup looking into home care business, seeks to connect the caregivers and the patients with a multi-sided platform. The target audience of one side of the platform is the frail and sick patients or family members who need an extra pair of helping hand; the other side of the platform is the caregivers who are on idle and looking for alternative source of income. Foreign market expansion brings in additional revenue stream but it is also a high risk business activity. Therefore, we use CAGE (culture, administrative, geographical and economical) framework to evaluate the external market environment and select the most feasible market to enter. After assessing the above four dimensions, we decide the go-to market is Singapore. We then use Porter’s five forces to analyze Singapore market attractiveness and profitability. Our analysis shows that Singapore is medium attractive and profitable. The internal analysis on Ucarer shows that the highly integrated platform allowing direct and self-service booking system has a leading advantage. In terms of implementation, we recommend a holistic market entry plan including the timing to enter, entry mode, execution plan and financial plan. The final point relates some of the points for considerations on the next step after a year of entering

To address of the trend of population aging and declining birth rates, as well as the emergence of cloud services, this study proposed an event-driven architecture-based model to support collaborative distributed services. The proposed model can accommodate loosely coupled distributed services aimed at providing home care services for elderly adults who live alone through the development of an interactive home care service model; thus, the proposed model facilitates providing immediate safety and health management. The event messages of the proposed system architecture were based on the Health Level 7 (HL7) protocol. Thus, the system can provide rapid support for medical treatment and is compatible with all HL7-based medical information systems, thus enabling medical staff to provide immediate assistance to elderly adults in the event of an emergency. In recent years, population aging has been an ongoing concern. As societies gradually transition to an aging society, providing care for the elderly population becomes a crucial issue. Therefore, this paper also presents a mechanism for providing long-term care through a body sensor network (BSN) to ensure the mobile security and emergency monitoring of elderly adults. Moreover, the proposed mechanism can further facilitate providing welfare services for elderly adults as well as necessary information and assistance for medical personnel. This study focused on investigating health care by analyzing BSN data. The concept and learning mechanism are introduced to demonstrate how the proposed system can fulfill follow-up processing and applications. Moreover, because the BSN is powered by batteries, it effectively uses 45% less power than full-speed transmission networks operating under normal conditions. Thus, effective long-term monitoring is demonstrated in this paper.

Bodywork is defined as the direct work on others’ bodies, and involves interactions of bodies and emotions. This study adopts the concept of bodywork as the analytical approach to investigate the homecare practices in Taiwan. Through inter-corporeal aspects, the study describes the daily activities of homecare workers, and the care relationships between them and care recipients. I also demonstrate how professionalism, institutional and organizational arrangement, specific cultural practices shape the practices of homecare services. Relying upon ethnographic fieldwork and interviews in two homecare organizations located in urban area and a mountainous indigenous township respectively, the study finds that bodywork is the core activity of carework. Through developing multiple strategies, homecare workers and their care recipients negotiate the boundaries of bodies and establish the mutual relationships. In addition, the care workers need to rearrange the practices of carework based on the varieties of care recipients’ gender, aging, and disabilities. Through the comparison of these two homecare organizations, the study discusses how the salary systems, organizational management, resource allocation, and interpersonal network shapes care workers’ professional identities and their daily work. The ‘extra work’ performed by care workers is created by the inflexible regulations, the characteristics of carework, and care ethic. Based on the findings, the study argues that the government and society should value the bodywork in homecare services. Additionally, the practices of carework need to consider the specific social and cultural context where care recipients are situated.

Since Taiwan has been an aging country, the Home-Care Services have become critical in helping the elderly in this island. Moreover, services delivered by Home-care Workers(HCWs) directly influence the quality of the lives of the elderly. The study investigated the current training on home-care workers in Taiwan and explored the advantage and disadvantage of the system. Data were collected from three sources, including deep interview, participant observation, and secondary data such as official publication and information. The quality research method is also used to identify the relationship among HCWs’ competency, service quality and users’ satisfaction. The research found that HCWs pre-job training has been integrated since 2003. It became the first bar of qualification and the base of career ladder for HCWs. The universal training is designed for caregivers of hospitals, institutions, communities, or home-care. Previous research has shown that training can promote service quality and increase users’ satisfaction. The results revealed that:(1)The resources of attending pre-job training courses on Home-care Workers are diverse and open. However the selection mechanisms of the training recruit lack.(2)The course of the training focus on cure and care skills but not housekeeping and communicating skills which are critical to home-care services for the elderly. (3)The training courses are designed for general caregivers who working in hospitals, care institutions, communities, and home-care, but not aim at Home-care Workers. These results indicate that the current training system is not enough for the Home-care Workers providing services for the elderly. (4)The better a Home-care Worker’s competency is, the higher service quality he/she delivered. The most influencial part of competency is attitude.(5)Through service quality, users’ satisfaction is influenced by Home-care Workers’ competency.(6)After adjusting the competency and positive/negative characteristic of Home-care Workers, the effect of service quality is not significant. There are gaps between supply and demand of training. According to previous discussion, this study has suggested improving clinical practice and creating medical consulting & supporting network for Home-care Workers, to increase service quality, and improve life quality of older users’. This would be a win-win situation for individual and our country.

Chiayi Christian Hospital has provided home care services for elderly persons in Chiayi County since 1999. Due to different regions of the geographical environment and cultural characteristics, different management approaches have been developed. This study explores the service management model of the home care service team in Chiayi Christian Hospital, which was started in 2013. The research data were collected through a number of methods including relevant secondary information, the home attendant questionnaire survey, the management interview and the participant observation. The care service teams were based upon village level of administrative district and were responsible for the management of workforce to provide home care services. The development of the working model could be divided into the expected period, grass period, run-in period and breakthrough period. The effect of the model could be observed from improvement in work efficiency and solidarity among managers and care workers. Moreover, the spatial variations of the geographical environment and humanistic characteristics also affect the operation of the care team. The mountain area is constrained by the terrain limitation, hence the distance of the traffic distance and the operation effect is limited. The service delivery process should be based on regional differences with adapting service supply. The study suggests that to use technology, innovation management to promote the efficiency of service provision with integrated service planning.

The purpose of this study was to discuss the relationship between service quality and disabled elders’ quality of life. Questionnaire investigation was adopted and 133 frail elders who have home care service and live in Taipei City and New Taipei City were the study subjects. Self-arranged service quality questionnaire, WHO questionnaire for quality of life, basic information of the elders, and the hours that elders have home care service were the study tools. The collected data was analyzed through descriptive statistics, t-test, one-way ANOVA, Pearson correlation analysis, and hierarchical regression. The comprehensive analytical study results and findings are as follows: 1. The disabled elders’ satisfaction towards service quality was relatively high, where the scores of reliability and assurance were higher. For quality of life, the elders’ feeling towards their quality of life was averagely satisfied, but they were not satisfied with their health, where the scores of social relationship were the highest and the scores of psychological condition were the lowest. 2. For basic attributes of the disabled elders, significant difference was found regarding the elders’ feelings towards home service quality, as in relevant variables including age, education level, and average monthly expense. Significant difference was found regarding the elders’ feelings towards quality of life, as in relevant variables including age, education level, living condition, have main carer or not, degree of disability, and weekly home care service hours. Other variables including gender, religion, and have chronic diseases or not did not influence the elders’ feelings towards home care service quality or quality of life significantly. 3. The reliability and assurance of service quality was significantly related with the environmental factors of quality of life. 4. After finishing hierarchical regression, 22.7% of variance of elders- society relationship was explained by variables including age, education level, average monthly expense, living condition, have main carer or not, degree of disability, weekly home care service hours. After combining age, education level, average monthly expense, living condition, have main carer or not, degree of disability, weekly home care service hours, tangibility, reliability, and care with two interaction factors, service quality and degree of disability, 6.9% of variance of elders’ environmental factors could be explained. Suggestions were provided for the government and related management and service organizations in accordance with the study result as the reference of future policy plan and service. If the service organizations provided good home care service quality, then the quality of life of elders could be promoted so that they could live happily in the community.

Background: In Taiwan, the home care service hours allocated by care managers, which was based on standard characteristics such as cases’ disability level and the cognitive function. Additionally, nonstandard characteristics included cases’ and their family caregivers’ characteristics, the workload of care managers, and the region resources may also affect the decision of the care managers in allocating home care service hours. How these standard and nonstandard characteristics affect the allocation of home care service hours warrants investigation. Objective: To determine the effect of standard and nonstandard characteristics on home care service hours allocated by care managers to care for disabled elderly. Method: A cross-sectional design was applied with the ten-year plan for long-term care database. Subjects were disabled elderly who received home care services in 2013, and we estimated monthly home care service hours by the initial assessment and reassessment. Disability level and cognitive function were used as standard characteristics, whereas the characteristics of home care cases and their family caregivers, the workload of care managers, and the region resources served as nonstandard variables. Data analyses were conducted using a generalized estimating equation (GEE) because each subject was repeatedly observed. Results: The average of home care service hours for disabled elderly in 2013 was 29.12. Subjects with mild disabilities had 16.06 hours; those with moderate disabilities had 28.8 hours; and those with severe disabilities had 39.09 hours. Allocating home care service hours was significantly higher in the subjects who were more severely disabled, older age, spouse, had medically proven physical or mental disabilities or both, whose caregivers over workload, workload for care managers, and who lived in a region that is high number of care managers. The initial model included disability level and cognitive function and found the explanatory power was 20.66%. After controlling for the characteristics of the home care cases and their family caregivers, the number of home care cases assigned to cater managers, the characteristics of the regions served and the counties or cites in which the cases where located had an increased explanatory power of 24.64%, 24.74%, 25.88%, and 31.38%, respectively. Conclusion: The home care service hours allocated by care managers for the disabled elderly depends on standard characteristics, workload of care managers, and the region resources. The government could consider care managers’ workload and intention of stay.
