Abstract:
Muscle diseases constitute a large group of hereditary and acquired disorders, collectively referred to as myopathy. Symptoms of myopathy are muscle pain, muscle cramps or spasms, stiff or rigid muscles, and muscle weakness. Muscle pain, or myalgia, is by far the most common presentation. Although most myalgia will be benign and self-limiting, it can be a reflection of underlying serious illnesses which may lead to significant morbidity. It is important to differentiate myalgia from myopathy (muscle disease) and myositis (inflammation of muscle). A muscle is said to be in spasm when it contrac

Abstract:
This chapter discusses the use of fluoroscopy in the performance of psoas and quadratus lumborum muscle injections. These muscles are quite often involved in the genesis of back pain. Unlike other pain syndromes, such as piriformis and thoracic outlet syndromes, that have characteristics signs and symptoms, myofascial pain involving the iliopsoas and quadratus muscles are only characterized by trigger points and/or muscle spasm. One reason for injecting the muscles is to avoid several trigger point injections using large volumes of local anesthetics.

The landmark paper discussed in this chapter is ‘Botulinum toxin type A induces direct analgesic effects in chronic neuropathic pain’, published by Ranoux et al. in 2008. The muscle-relaxant properties of botulinum toxin type A (BTX-A) have long been known and used therapeutically. However, BTX-A actions independent of those affecting neuromuscular function had previously only been alluded to, via the observation that BTX-A injections alleviated pain more than would have been expected through the treatment of muscle spasm alone. This paper was one of the first to specifically examine the effic

An adjuvant is not analgesic but is capable of relieving pain in certain specific pain situations. Selection of an appropriate adjuvant is a key element of a rational and evidence-based approach to management of pain in children. It depends on the diagnosis of the type of pain, considered in this chapter in relation to the selection of a suitable adjuvant, based on recognizing the nature of the pain to be treated. Adjuvants for specific types of pain, including neuropathic pain, bone pain, muscle spasm, and cerebral irritation, and pain syndromes are discussed in detail, with information provi

The name of Charles Bell has been given to a nerve, a facial palsy, a clinical sign, an involuntary muscle spasm, a muscle, and a fundamental law of physiology, making it well known to physicians, surgeons, and medical students alike, even though they generally have little knowledge or appreciation of his actual, very solid accomplishments. He suggested, in fact, new ways to look at—and to make sense of—the nervous system. Bell was revered by some contemporaries for his achievements; to others, however, his name and brilliance were tarnished by charges of intellectual dishonesty and fraud. Thi

Abstract:
Postlaminectomy syndrome is persistent or recurrent back pain after otherwise anatomically successful lumbar spine surgery. A dramatic increase in the number of low back surgeries has been observed since 1997, with an increased incidence of pain after low back surgery in the range of 5% to 74.6%. The mechanisms contributing to back pain are muscle damage during surgery, muscle spasm, and inflammation, with subsequent development of myofascial pain syndrome as well as other typical and atypical back pain generators. Diagnosis is based primarily on history and physical examination, as well as re

Abstract:
Anaerobic organisms of the genus Clostridia (C) can cause significant human disease. Exotoxins secreted by C botulinum and C tetani cause botulism and tetanus, respectively (summarized in Table 156.1). Botulinum neurotoxin causes neuromuscular blockade by interfering with vesicular acetylcholine release, leading to cholinergic blockade at the neuromuscular junctions of skeletal muscle, and consequently, symmetric flaccid paralysis. Tetanus toxin prevents release of inhibitory neurotransmitters at central synapses, leading to overactivity of motor neurons and muscle rigidity and spasms. This ch

Intra-articular facet joint injections commonly refer to the injection of a contrast media and local anesthetic solution, with or without corticosteroids, directly into the facet joint space. The purpose of this procedure is pain relief as well as to establish an etiological diagnosis for surgical interventions such as joint denervation or radiofrequency ablation. Medial branch block, or facet nerve block, refers to injection of local anesthetic and possible corticosteroids along the medial branch nerve supplying the facet joints.
