Abstract:
Thomas Heywood’s 1607 play, A Woman Killed with Kindness, ends with the protagonist, Frankford, discovering the lute of Anne, the wife he has just banished for adultery. Grieved by the sight of the instrument that he conflates with his marriage and with Anne herself, Frankford exiles the lute along with his wife. When she receives the instrument, Anne plays a lament, then directs her coachman to “go break this lute upon my coach’s wheel, / As the last music that I e’er shall make” (16.69–70). Shortly following the destruction of the lute, Anne dies. Anne’s body and memory, clearly, are inextricably linked to the lute: in the drama, her body is a musical instrument that she can play, that can be played upon, and that can be destroyed. The lute as body metaphor is a common image in early modern English literature, and Heywood both uses and complicates the metaphor. The lute, first, demonstrates Anne’s impossible and paradoxical identity as a chaste wife, noblewoman, and possible prostitute. Moreover, the lute emphasizes Anne’s powerlessness over her own body, particularly her humours. Like other characters in the play, Anne had let her bodily passions control her, but when she breaks the lute, she breaks also her passions’ power over herself and others. Yet when she destroys the lute, she does not abandon music altogether, for music can bring about powerful social harmony. Instead, she plays her own body as a musical instrument, which makes her self-slaughter instructive rather than destructive. Her death is didactic for the audience—both onstage and in the theatre—that gathers around her deathbed, and suggests a variety of means of controlling the passions, some of them more deadly than others. In A Woman Killed with Kindness, Anne’s music is an exemplar of the extraordinary efforts necessary to quell the unruly passions that cause so much of the conflict in the play. Comme d’autres personnages de la pièce, Anne a perdu la maîtrise de ses passions charnelles, mais en détruisant le luth elle détruit aussi l’emprise de ses passions sur elle-même et sur les autres. Cependant, lorsqu’elle détruit le luth, elle ne renonce pas totalement à la musique, car la musique peut engendrer une forte harmonie sociale. Elle joue plutôt de son propre corps comme d’un instrument de musique, faisant de son suicide davantage une instruction qu’une destruction. Sa mort a une valeur didactique pour ceux qui le regardent (tant de la scène que des gradins du théâtre), assemblés autour de son lit de mort; il laisse entendre que plusieurs méthodes permettent de maîtriser les passions, certaines étant plus mortelles que d’autres. Dans A Woman Killed with Kindness, la musique d’Anne symbolise la peine extraordinaires pour imposér une discipline aux passions sans règle, à la source de tant de conflits dans la pièce.

Adapun perspektif yang digunakan dalam tulisan ini yaitu perspektif hukum yang hukum Islam akan menindak tegas para pelaku yang melanggar ketentuan dan peraturan yang telah ditetapkan berdasarkan nash Alquran dan Hadis. Hasil tulisan ini menjelaskan bahwa fenomena homoseksual dalam perjalanan sejarah keislaman sebenarnya telah terjadi pada kaum Nabi Luth yang disebut kaum Sadum. Salah satu bukti pengulangan kisah kaum Nabi Luth yaitu Pompeii, yang merupakan simbol dari degradasi akhlaq yang dialami kekaisaran Romawi, adalah pusat perzinaan dan homoseks. Selain itu, fenomena kaum homoseksual kembali muncul di New York dengan disahkannya undang-undang pernikahan sejenis. Ini menunjukkan. Dengan kata lain, rekapitulasi sejarah perilaku homoseksual yang dilakukan kaum Nabi Luth telah terjadi pada masa yang lampau, kini juga dilakukan oleh generasi masa sekarang dan secara implisit ada kemungkinan terjadi di masa yang akan datang.Islam sangat melarang keras perilaku homoseksual karena memiliki dampak negatif dan merupakan dosa besar. Allah telah menjelaskan dalam nash Alquran dan hadis tentang kisah azab bagi kaum Nabi Luth yang melakukan perilaku homoseksual.

Objectives: During pregnancy, the fetus requires iron for blood formation so does the mother, this increases the iron requirement which is higher than that of non-pregnant women. This study assessed the level of awareness and prevention of anemia among pregnant women attending the antenatal clinic at Lagos University Teaching Hospital (LUTH), Lagos, Nigeria. Material and Methods: A descriptive cross-sectional study was conducted among 182 pregnant women attending the antenatal clinic at LUTH in Lagos, Nigeria. A balloting system was done to select the respondents in the clinic that runs 4 times a week, with an estimated number of 70 pregnant women per clinic. Data were collected using a structured self-administered questionnaire and analyzed with the Statistical Package of the Social Sciences version 22. Descriptive and inferential statistics were used for analysis, Chi-square test was done for the association between the variables at P < 0.05 level of significance. Ethical approval was obtained from the Human Research Ethical Committee of LUTH with approval number (ADM/DCST/HREC/APP/2589). Informed consent was taken and respondents were reassured of the privacy and confidentiality of the information obtained. Results: The highest percentage (33.3%) of the respondents was within the range of 26–30 years with a mean age of 28.18 ± 0.84 years. Majority of the women had a good level of awareness of anemia (68.89%) and good overall practices (73.89%) of prevention of anemia in pregnancy. There was no significant relationship between the respondents’ level of awareness of anemia and its prevention (χ2 = 1.533, P = 0.216). Conclusion: The study has shown that even though awareness and prevention practices were good, there is a need to create more awareness among pregnant women and also to give adequate health education on prevention of anemia to produce favorable outcome in pregnancy for both the child and mother.

Abstract Objectives Intracranial neoplasms have distinct diagnostic histologic features and some are common in certain gender and age groups. The most common intracranial neoplasm worldwide is meningioma, followed by gliomas, most especially astrocytic tumors, and then pituitary adenomas. There are geographical differences in the pattern of occurrence of intracranial neoplasms. Few studies have been done in Nigeria to demonstrate the pattern of occurrence, age, and sex distributions of these neoplasms. The aim for this study is to establish the pattern of occurrence with age and sex distribution of different histologic types of intracranial neoplasm in our environment. This study may help portray the health burden of these tumors and aid in epidemiological studies. Methods A total of 296 patients (165 females, 131 males) diagnosed with intracranial neoplasms between January 2008 and December 2017 at Lagos University Teaching Hospital (LUTH), Idi-Araba, Lagos, Nigeria, were retrospectively analyzed. Patients’ data were retrieved from the archives of the Department of Anatomic and Molecular Pathology, LUTH, Idi-Araba, Lagos. Histologic patterns with age and gender distribution were noted. The data obtained were analyzed with SPSS version 23. Results Majority of the patients diagnosed with intracranial neoplasm were between 41 and 50 years of age. The most frequently diagnosed intracranial neoplasm at LUTH within the study period was meningioma (105 cases, median age of 42 years, male to female ratio of 3:7), followed by pituitary adenoma (78 cases, median age of 47 years, male to female ratio of 3:2), and then gliomas (66 case), most especially the astrocytic and oligodendroglial tumors (median age of 37 years, male to female ratio of 2:3). Conclusion The result of the study shows that the pattern of occurrence of primary intracranial neoplasms in our environment is different from that in Caucasians, with meningiomas being the most common, followed by pituitary adenomas and then gliomas.

Tot op heden is nog geen onderzoek gedaan naar de manier waarop het begrip “troost” wordt gebruikt in de cantates van Johann Sebastian Bach (1685-1750). Dit artikel is de neerslag van een verkennend onderzoek naar de betekenis van “troost” in samenhang met dood en sterven. Omdat Bach in een traditie stond waarin de tekst met muzikale middelen werd uitgelegd, wordt ook onderzocht op welke manier hij dit woord muzikaal behandelt. Het woord “troost” komt vaak in vocaal-instrumentale werken van Bach voor, maar meestal niet in samenhang met de dood. Daarnaast zijn er cantates die troost willen bieden, zonder dat dit woord wordt gebruikt. Specifiek in verband met de dood komt het woord “troost” in negentien cantates voor. Troost in verband met dood en sterven heeft verschillende betekenissen en bestaat in ieder geval uit het geloof en de zekerheid dat de gelovige het hemelse paradijs zal bereiken, waar hij in de armen van Jezus is. Daarnaast bestaat troost uit het weten dat de dood een slaap is en dat God in dood en sterven van het verderf redt. We zien hier motieven die ook voorkomen in verschillende werken die deel uitmaakten van Bachs theologische bibliotheek. Een voorbeeld zijn de publicaties van de theoloog Heinrich Müller (1631-1675), waarin een sterk verlangen naar voren komt om bij Jezus te zijn, zo sterk, dat het sterven zelf als troost wordt gezien.

Background: Categorization of endometrial carcinomas as type I and II provides useful insights into their different risk factors, pathogenesis and biologic behaviours.Aim: To determine the immunohistochemical classifications of endometrial carcinomas in Nigerian women.Design: A retrospective review of histopathologic slides of cases of endometrial carcinomas seen at the Lagos University Teaching Hospital (LUTH) over a 5-year period. The slides were reviewed, and the diagnoses made according to the WHO nomenclature. The classification of endometrial carcinomas into Type I and II was made by immunohistochemistry using antibodies to ER, PR, p53 and Ki-67.Results: Eight cases of endometrial adenocarcinoma were reported accounting for 53.3% of all endometrial malignancies. Of these, only 1 case showed the classic type I immunophenotype while type II staining pattern was seen in 4 cases. The remaining 3 cases had equivocal immunophenotypes: one was p53+ but showed ER+, PR+ and high Ki-67 index; the second was p53-, ER+, PR+ but had a high Ki-67 expression; while the last was p53-, but ER-, PR- and had high Ki-67 expression.Conclusion: Endometrial carcinomas in Nigerian women are more likely to be type II carcinomas. A reasonable proportion of the cases were equivocal thus requiring further categorization with molecular studies.Keywords: Endometrial carcinomas, immunohistochemistry, LUTH, Nigerian.

Abstract:
Objectives. This study was aimed at determining the knowledge and acceptability of HPV vaccine among women attending the gynaecology clinics of the Lagos University Teaching Hospital (LUTH). Methods. This was a descriptive cross-sectional study involving 148 consecutively selected women attending the gynaecology clinic of LUTH. Relevant information was obtained from these women using an interviewer-administered questionnaire. The data was analysed and then presented by simple descriptive statistics using tables and charts. Chi-square statistics were used to test the association between the sociodemographical variables and acceptance of HPV vaccination. All significance values were reported at P<0.05. Results. The mean age of the respondents was 35.7±9.7 years. The study showed that 36.5% of the respondents had heard about HPV infection while only 18.9% had knowledge about the existence of HPV vaccines. Overall, 81.8% of the respondents accepted that the vaccines could be administered to their teenage girls with the level of education of the mothers being the major determinant of their acceptability (P=0.013). Conclusions. Awareness of HPV infections and existence of HPV vaccines is low. However, the acceptance of HPV vaccines is generally high. Efforts should be made to increase the awareness about cervical cancer, its aetiologies, and prevention via HPV vaccination.

Indonesia mengalami degradasi karakter yang ditandai dengan maraknya penyimpangan moral, baik sosial maupun agama. Penyimpangan tersebut jauh dari nilai-nilai keislaman namun ramai digandrungi generasi muda seperti budaya pacaran, seks bebas hingga homoseksual. Kasus aborsi di Indonesia terjadi sekitar 2,5 juta jiwa setiap tahunnya yang 30% diantaranya adalah remaja. Selain itu hasil survei lembaga independen menyebutkan 3% atau sederhananya 3 dari 100 penduduk Indonesia adalah pelaku homoseksual. Fenomena tersebut tidak lain disebabkan karena adanya dorongan perkembangan media dalam memfasilitasi free sex dengan tawaran akses yang semakin lengkap dan jauh meninggalkan nilai agama dan moral. Oleh sebab itu, pendidikan seks diharapkan menjadi jalan tengah yang perlu digalakkan dengan keyakinan bahwa pendidikan ini mampu membentengi dan mengarahkan mereka kepada pemahaman seks yang baik dalam pandangan agama maupun negara. Maka tujuan dari penelitian ini adalah untuk mengetahui bagaimana materi dalam melakukan pembinaan pendidikan seks menurut pandangan Islam. Desain penelitian menggunakan analisis konten dengan pendekatan kualitatif. Hasil studi ini menunjukkan bahwa sex education masih tabu dan kental akan pro dan kontra, namun substansi perdebatan tersebut hanya berputar pada materi dalam pendidikan seks. Oleh kebab itu, pembinaan sex education hendaknya melibatkan orang tua dan pendidik yang disertai dengan pengawasan berdasarkan Al-Quran dan As-Sunnah, yakni memberikan pemahaman terkait fitrah manusia, sopan dan memahami waktu yang tidak diperkenankan berkunjung, menahan diri dan menjaga pandangan, dan menjauhi zina.

Background: Seminal fluid analysis is essential in the evaluation of the male partner of infertile couple. Available data suggest an increase in male-related factors among infertile couples seeking fertility treatment. Seminal fluid analysis performed using standardized methods can identify several causes of male-factor infertility. The latest World Health Organization (2010) guidelines for the laboratory examination and processing of human semen allows for standardization and identification of the possible causes of male-factor infertility. This study aimed to determine the pattern of seminal fluid analysis of male partners of infertile couple presenting to the fertility (gynaecological and urological) clinics of the Lagos University Teaching Hospital (LUTH).Methods: This was a retrospective study of the semen parameters of male partners of infertile couple that were evaluated at LUTH, from January 2018 to December 2019. The medical records of all male partners of infertile couples who had seminal fluid analysis were reviewed and the relevant seminal fluid parameters were collected using a standardized questionnaire.Results: A total of 604 male partners of couples with infertility sought fertility treatment during the study period. Out of the 604, 502 (83.1%) of the participants had complete seminal fluid result and these data were included in final analysis. The mean (SD) age of subjects was 42.1 (7.7) years. About three-quarter (73.1%) of the subjects have abnormal semen parameters. The most frequent semen abnormality was asthenozoospermia (55.8%) followed by oligozoospermia (39.4%).Conclusions: Male factor is still common cause of infertility among infertile couple seeking treatment in our environment.

Three patients admitted to the Accident and Emergency Unit of Lagos University Teaching Hospital (LUTH) after eating a cassava based meal 'Gari' died shortly after admission. The patients vomited and complained of abdominal pain immediately after the meal. They were unconscious with renal failure and died of cardiopulmonary arrest. The cyanide levels in the blood and urine averaged 1.12 and 0.54 mg 1-1, respectively. Cassava contains cyanogenic glycosides which slowly release cyanide and this may have been responsible for the death of these patients. There is an urgent need to establish maximum tolerable levels of cyanide in 'Gari' and other cassava food products.

Abstract Introduction Prostate cancer is the second most frequent cancer worldwide. It is the commonest cancer among men in Nigeria. Gleason grading and scoring system constitute the most useful prognostic indicator in prostate cancer diagnosis and management. Over the years, it has gone through important modifications. The most recent of these changes was in 2014 by the International Society for Urological Pathology (ISUP). The WHO adopted the new grading system in 2016. It was not adopted as part of routine reporting of prostate cancer biopsies in Lagos University Teaching Hospital (LUTH) until 2017. The aim of this study is to categorize prostate cancers diagnosed in LUTH based on the grade group system and determine the pattern of prognostic distribution of prostate cancer cases since its adoption. Methods This is a descriptive study; data were retrieved from the histopathologic records of the Department of Anatomic Pathology. Prostate cancers cases diagnosed between 2017 and 2018 were stratified using the WHO-adopted 2014 ISUP grading system. Results During the 2 years, 93 cases of prostate cancers were diagnosed by core needle biopsies. Overall, mean age of cases was 69.85 years (SD 8.49, median = 70 years). Nearly 39% of the 93 cases were ISUP grade group 5 and about 24% were ISUP grade group 4. Together ISUP grade groups 3, 4, and 5 constituted 76.35% of all cases of prostate cancers. There was no statistically significant association between ISUP grade group and age (P = .266). Perineural invasion was present in 35.82% of cases. Sixty-seven percent of these were ISUP grade group 5. Conclusion The data from this study suggest that high-grade cancers, particularly those of the ISUP grade group 5, are the most frequently diagnosed prostate cancers in our institution. Further studies are required to document biochemical recurrence-free progression and survival among our patients.
