The Momconnect project was launched in 2014 by its champion, the minister of health, Hon. Dr. Aaron Motsoaledi. The system is an m-health application targeted at maternal health patients in South Africa. Execution of the project was carried out by a consortium headed by the National Department of Health. Project execution began in 2012, and the launch was two years later in 2014. In this research, a health systems framework for evaluation of implementations of m-health systems in developing countries was used to review the system. The sources of data for the study were face-to-face interviews of the Momconnect Task Team, Ministerial Advisory Committee on e-Health, and a sample of health care providers and consumers from the health facility level in Johannesburg Metro City. These interviews were supplemented by a document review. The findings of the study are reported according to the four headings of the study framework.

Although a constellation of factors would seem to have predisposed South Africa to make major new commitments to expand access to healthcare after the fall of apartheid, embrace of National Health Insurance has taken place in name only more than 20 years later. The chapter suggests that this sad tragedy owes its fate paradoxically to dynamics of political competition that left the African National Congress unrivalled and unchallenged, with a strong mandate to rule. Torn between a desire for radical reform – that would destroy the medical schemes that serve a privileged few – and a more incremental and measured response that would leave them in place, amid a lack of political competition, the ruling party has opted for the status quo. And the entreaties of a professional movement have gone unanswered.

The chapter explores and describes community participation in the National Development Plan through Primary Health Care by using case studies of LGBT organizations in South Africa. Post-Apartheid and democratic South Africa has endorsed community participation as one of the fundamental pillars of the public Primary Health Care approach in its governance structures. This chapter focuses on the current major health issue in South Africa, the HIV epidemic, which is one of the leading causes of death in the country. Particular attention is paid to members of the LGBT community because of their discrimination in public healthcare facilities on grounds of their sexual orientation. The chapter relies on secondary sources of data collection from extant literature, textbooks, journal articles, and internet sources. Challenges to address LGBT community discrimination in HIV testing, prevention, treatment, care, and support were identified and solutions to uphold their human rights were proffered. These solutions are based on the principles of social justice, inclusion, diversity, and equality.

This chapter traces the history of health policy development in the country in seven phases from the 17th century to the present time. It describes the efforts in transformation made through the promulgation of the National Health Act in 2003 and the eHealth Strategy in 2012. The chapter explores the utility of maturity assessment in assessing whether transformation goals through an analysis of five maturity models: Digital Preservation Capability maturity model, eHealth maturity model, Enterprise Content Management maturity model, Health Normative Standards Framework maturity model, and Records Management Capacity Assessment System. South Africa is already using two of the five models demonstrating that is not just reliant on technology but has developed strategies and principles to guide the transformation process. The chapter argues for more expansive adoption of maturity assessment to cover the full records lifecycle.

Background: The COVID-19 climate has seen a shift in the manner that patients seek care. Lockdown measures and COVID-19 regulations, and the fear of contracting the virus at a health care facility has also changed health seeing behaviour among patients. The COVID-19 climate has seen a significant increase in the utilisation of virtual platforms to consult with providers. Objectives: The objective of this chapter was to conduct the descriptive analysis of telephonic consultations by members of medical schemes who consulted general medical practitioners. Methods: The study entailed a descriptive analysis of medical scheme claims data for the 2020 review period. The inclusion criteria were all National Pharmaceutical Product Interface (NAPPI) codes associated with a telephonic consultation consulting general medical practitioners. The ICD-10 code primary diagnosis was used to describe the diagnosis. The study mainly focused on outpatient patients with service dates between March and December 2020. Results: The analysis covered claims data from a total of 12 medical schemes. The schemes analysed accounted for 1,6 million lives. The total number of telephonic consultations was 17 237. The mean (SD) claimed amount for telephone consultation for a general medical practice consult was R2821 (SD = 20). This was slightly lower than the scheme tariff of R2872 (SD = 19). The study found that most telephonic consults were for Acute bronchitis, unspecified; Acute upper respiratory; Emergency use of U07.1 (Confirmed diagnosis); Emergency use of U07.2 (Suspected Diagnosis); Follow-up examination; Special screening. Conclusion: The study found evidence of patients utilising telephonic consultations for general medical practitioner services. The effect of COVID-19 in this respect was seen in the three main primary diagnoses that were associated with the consult, Acute upper respiratory, Emergency use of U07.1 (confirmed diagnosis) and Emergency use of U07.2 (suspected diagnosis). Even though the average telephonic consult was claimed at just under R3003, few general medical practitioners claimed between R4004 and R5005 which were higher than the industry average. There is a need to develop telephone consult guidelines at industry level, these should also address reimbursement rate differentials.
