Despite an abundance of older and more recent retrospective and considerably fewer prospective-longitudinal studies in bipolar disorders I and II, there are still remarkable deficits with regard to our knowledge about the natural course and burden. The considerable general and diagnosis-specific challenges posed by the nature of bipolar disorders are specified, highlighting in particular problems in diagnostic and symptom assessment, shifts in diagnostic conventions and the broadening of the diagnostic concept by including bipolar spectrum disorders. As a consequence it still remains difficult to agree on several core features of bipolar disorders, such as when they begin, how many remit spontaneously and how many take a chronic course. On the basis of clinical and epidemiological findings this paper summarizes (i) a significant need to extend the study of the natural course of bipolar disorder in clinical samples beyond the snapshot of acute episodes to the study of the mid-term and long-term symptom course, associated comorbidities and the associated burden of the disease. (ii) In terms of epidemiological studies, that are also of key importance for resolving the critical issues of threshold definitions in the context of the bipolar spectrum concept, there is a clear need for identifying the most relevant risk factors for the first onset and those for the further illness progression in early stages. Since there are some indications that these critical processes might start as early as adolescence, such studies might concentrate on young cohorts and clearly before these prospective patients come to clinical attention. (iii) The value of both types of studies might be enhanced, if beyond the use of standardized diagnostic interview, special attempts are made to use prospective life- and episode-charting methods for bipolar illnesses.

Abstract:
Approximately 2-11% of adolescents report chronic insomnia. The study used an archival data set from ADDHealth that assessed adolescent health and health-related behaviors. Adolescents (N = 4102) provided data at baseline (Time 1) and at 1-year follow-up (Time 2). Participants were excluded if no ethnicity, gender, or insomnia data were given at Time 1 or 2. Females were more likely to report insomnia than males at Times 1 and 2. In addition, adolescents with remitted insomnia were significantly younger than adolescents without insomnia at Times 1 and 2. Analyses found a prevalence of 9.6%, a remittance of 6.2%, an incidence of 4.4%, and a chronicity of 2.9%. At Time 1 and 2, AWI were significantly more likely to have depression, suicidal behaviors, and behavioral problems in school than AWOI. At Time 2, incidence and chronic insomnia increased the risk of depression, suicidal behaviors and behavioral problems in school. Risk and protective factors analyses indicated psychological counseling was associated with both remitted and chronic insomnia and depression was associated with incidence insomnia.

Abstract:
Community participation in flood planning has recently emerged as a successful approach to addressing and restricting the traditionally structural methods of flood control. Flooding, the most costly natural hazard worldwide, causes economic damages in spite of flood control efforts throughout the 20th century. To control flooding while allowing development, localities have traditionally used structural controls, such as levees and floodwalls, to physically separate floods from people. In light of the continued failure, high costs, and environmental degradation associated with structural flood controls, localities are now increasingly focusing on non-structural flood mitigation methods to reduce flood risks and losses. Furthermore, communities throughout the country are incorporating innovative flood projects that balance structural and non-structural flood mitigation in an attempt to better address environmental concerns. This approach involves returning previously damaged rivers and floodplains to their natural state. This evolution from structural approaches to environmentally conscious flood planning is illustrated through a case study of Napa, California’s model flood plan. Through an analysis of the flood plan and interviews with government representatives and project engineers, this case study illustrates how localities can design and implement flood plans to provide for environmentally sustainable flood mitigation. Building on a model of best management practices which incorporates the “living river” concept in the Napa River Flood Protection Project, this report suggests how other communities with severe river flooding can develop similar sustainable flood plans. Napa’s flood project represents a paradigm shift in which local residents were the driving force behind designing an environmentally sustainable and locally supported flood plan that would be carried out by the U.S. Army Corps of Engineers. The key lessons learned from Napa’s flood project are that community involvement and consensus building among stakeholders are crucial to developing and implementing an environmentally sustainable flood management project.

SAD reveals considerable persistence with more than half of the years observed since onset spent with symptoms. 56.7% of SAD cases revealed stability with at least symptomatic expressions at follow-up; 15.5% met SAD threshold criteria again. 15.1% were completely remitted (no SAD symptoms and no other mental disorders during follow-up). Several clinical features (early onset, generalized subtype, more anxiety cognitions, severe avoidance and impairment, co-occurring panic) and vulnerability characteristics (parental SAD and depression, behavioural inhibition, harm avoidance) predicted higher SAD persistence and -less impressively- diagnostic stability. Conclusion. A persistent course with a considerable degree of fluctuations in symptom severity is characteristic for SAD. Both, consistently meeting full threshold diagnostic criteria and complete remissions are rare. Vulnerability and clinical severity indicators predict poor prognosis and might be helpful markers for intervention needs.

Abstract The aim of the present work was to elucidate the incidence, associated risk factors and natural course of nephrocalcinosis (NC) in very low birth weight (VLBW) infants, and to evaluate renal function in affected infants during early childhood. The occurrence and course of NC in full-term infants receiving furosemide and in infants with congenital lactase deficiency were also studied. A total of 129 VLBW infants were screened for NC by renal ultrasonography (US) at 2 and 6 weeks and 3 months, and ultrasonic follow-up was performed on the infants with NC at 6, 12, 18 and 24 months, and thereafter annually up to age 5-6 years or until ultrasonic resolution. NC was classified according to its pyramidal localisation and extent. Twenty VLBW children with neonatal NC and 20 control pairs without the condition were examined for renal function at 4.7 (SD 1.1) vs. 4.6 (0.9) years of age. Thirty-six full-term infants who had received furosemide treatment for congestive heart failure for at least 4 weeks and 36 control infants without any diuretic therapy were examined by renal US and by means of a random urine sample taken at a median age of 2.9 vs. 3.4 months. The case records of the 11 infants with congenital lactase deficiency were analysed for NC, and these children were re-evaluated at 2 to 10 years of age. NC was detected in 26 out of the 129 VLBW infants (20%). The infants with NC were sicker and smaller than the unaffected ones and had more often received furosemide, dexamethasone and theophylline treatment. NC was peripheral in 14 cases (54%), scattered in 7 (27%) and extensive in 5 (19%). All the casesof peripheral NC showed resolution at 12 months, but abnormal renal findings were seen in 3 out of the 7 with scattered NC and 3 out of the 4 surviving children with extensive NC at 24 months, in 2 of whom the condition persisted at age 5-6 years. The children with neonatal NC showed increased urinary calcium and μ2-microglobulin excretion as compared with the controls in early childhood, but there was no significant difference in distal tubular acidification capacity, nor in estimated creatinine clearance. Five out of the 36 full-term infants receiving long-term furosemide had NC, but none of the controls. The daily dose of furosemide and the urinary calcium concentration were both higher in the infants with NC. Abnormal renal findings were still visible in two of the cases at 24 months of age. Hypercalcaemia was found in 7 out of 10 infants with congenital lactase deficiency tested at the time of diagnosis, and NC was seen in 5 of the 7 cases examined by renal ultrasonography. No constant dysfunction in calcium homeostasis was seen at re-evaluation, but nephrocalcinotic changes were observable in 3 out of the 11 children. NC may complicate not only the course of VLBW infants, but also that of full-term infants with calciuric medication and diseases that involve hypercalcaemia. Some renal tubular dysfunction may result from NC in former preterm infants, but overall kidney function seems not to be seriously compromised in early childhood.

Natural and technological disasters impact thousands of families in the United States each year. Catastrophic events leave homelessness, unemployment, injury, and death in their wake. The cost to society is usually measured in homes destroyed, jobs lost, casualties, and expected dollar expense of recovery. There are the social, psychological, and family consequences of catastrophic stressors. Anecdotal reports suggest that among these consequences is an increase in family violence, including child abuse. This dissertation tests the hypotheses that reported and confirmed child abuse increases in the wake of natural disasters. Child Protective Services (CPS) records of several jurisdictions that have experienced natural disasters during the past decade were examined. Data were collected from counties in South Carolina impacted by Hurricane Hugo in 1989, counties in California affected by the Loma Prieta Earthquake in 1989, and parishes in Louisiana impacted by Hurricane Andrew in 1992. The numbers of reports and confirmations for a one-year period following each of these events were compared with those for the year prior to the disaster. Analyses of these data indicated statistically significant increases in child abuse reports during the first 6 months following Hurricane Hugo and the Lama Prieta Earthquake, but showed no statistically significant change following Hurricane Andrew. The study concluded that reactions to natural disasters vary for a number of different reasons. The findings from California and South Carolina indicated that there are changes in patterns of reporting and/or confirmation of child abuse following catastrophes. CPS workers in each of the impacted areas were interviewed to obtain their impressions regarding the extent and causes of these changes in reporting and substantiation. Recommendations that governmental and social service agencies dedicate resources and develop programs to address this specific problem following catastrophes were included. Future research that replicates this study and the development of methodologies that do not depend on official reports and investigations were recommended.

Abstract:
The primary objective of this research study was to observe the changes in the physical properties of cement coated secondary aggregates, namely surface roughness and angle of internal friction and how these properties are related to the performance of Open Graded Friction Course asphalt mixtures, OGFC made from these materials under laboratory Uniaxial, deformation and rutting testing simulating the traffic loading and temperature of the materials in service. The secondary aggregates, namely Croxden natural gravel aggregates, which is rejected by road engineers for use in road surfacing was upgraded with a specifically designed cement coating. Asphalt mixtures mechanical properties in terms of stiffness modulus, deformations and rutting were determined and analyzed using state-of-the-art laboratory test equipment and supported by technical literature from different information and resources. Three aggregate types were considered in this study; Arcow rock crushed aggregates( as a control aggregate), Croxden aggregate (as a secondary or low quality aggregates) and cement coated Croxden aggregate (as an upgraded material). One aggregate gradation was designed for all mixture types and the adopted aggregate gradation had a nominal maximum aggregate size (NMAS) of 14mm. Two binder types were selected, 100/150 pen binder and 40/60 pen binder to represent the soft and hard binders respectively that are commonly used by road engineers. This research also documents a survey of literature review that led to the design of a novel machine that is eagerly needed by the pavement industry to boost the knowledge and understanding of real tyre-road interaction. Computer software, SOLIDWORKS, was used to create a 3-dimentional model of the machine that was jointly named by the Highway Agency and Tarmac ltd., as Low Cost Asphalt Tasting Equipment (LoCATE). The machine was designed for use and for the first time to investigate the performance of the said mixtures under real traffic loading and environmental conditions and to correlate its results with those obtained using Repeated Load Indirect Tensile Test (RLIT) and Repeated Load Axial Test (RLAT), which were carried out at 10°C, 20°C, 30°C and 45°C. At an advanced stage of this research work and when the cost of fabricating LoCATE was found impossible to be met by LJMU, wheel tracking tests were done on the materials studied as a replacement of LoCATE and this part of the research program was achieved at Wolverhampton's bituminous testing laboratories in collaboration with Tarmac Ltd. In general, the results of this research investigation indicate that the introduction of cement coated aggregates to OGFC led to an outstanding improvement in the chemical properties of the coated natural aggregates and the stiffness and deformation properties/resistance of the road mixtures containing them. This was in comparison with that made from the uncoated aggregates in all the tests that have been carried out within this research study. Also, the effects of elevated temperature were investigated and it was found to have significant influence on the engineering properties, Indirect Tensile Stiffness Moduli (ITSM) and permanent deformation/rutting characteristic of the mixtures were also investigated. Outstanding new results were achieved, using LJMU cement coating paste for coating the waste natural aggregate and as thus this will hopefully eliminate the restriction imposed by road engineers on the use of natural gravel aggregates in road pavement surfacing layers. This is an outstanding knowledge extension in this field and marks a starting point for more research in this ever growing industry i. e. the use of waste and recycled aggregates in construction industry.

The natural environment is a dynamic context for human development, but current lifestyles and activities are threatening the quality and supply of natural resources, and changing the conditions of the atmosphere. Older adults in the United States have been called upon to contribute their energy to volunteer efforts aimed at improving environmental conditions, but little is known about how or if older adults would be willing to take part. Informed by place attachment and attention restoration theory, and guided by the life course framework of human development, the purpose of this qualitative investigation was to deepen understanding about how a nature trajectory is established and the factors that influence the relationship between humans and nature over time. Interviews were conducted with 34 older rural women who ranged in aged from 71 to 91 years old (mean age 79). Seventeen of the women were living alone and nine lived below the poverty threshold. Using grounded theory coding and analysis techniques, two major findings emerged from the data: the women valued nature for spiritual and psychological reasons, and nature was suffering as a result of a wasteful and destructive economic paradigm. Half of the women's nature trajectories changed over time; trajectories remained positive and stable for the others. As God's creation, the natural environment informed their sense of self and bolstered their psychological well-being. They cared about environmental problems, but were unaware of what they could do to help.<br>Ph. D.

The pulmonary immunopathology of maedi-visna virus (MVV) infection was investigated, firstly performing <I>in vivo</I>, and <I>in vitro</I> studies and secondly, by reproducing the disease experimentally. Peripheral blood (PB) and bronchoalveolar lavage fluid (BALF) cells of naturally MVV-infected and control sheep were phenotypically characterised by using single-colour flow cytometry. BALF from MVV-infected sheep had significantly increased percentages of lymphocytes, neutrophils and CD8<SUP>+</SUP> lymphocytes, decreased percentages of CD4<SUP>+</SUP> and CD5<SUP>+</SUP> lymphocytes (p<0.05), significant (p<0.001) inversion of the CD4<SUP>+</SUP>/CD8<SUP>+</SUP> ratio, increased MHC Class II expression on BALF macrophages and upregulated expression of LFA-1 and LFA-3 on BALF lymphocytes (p<0.05). These data show the phenotypic changes in the cellular components of BALF during the course of natural MVV infection, and suggest a state of activation of BALF lymphocytes, and a pulmonary compartmentalisation of the cellular immune response during the course of the disease. The stage of activation of BALF and PB lymphocytes in MVV-infected and control sheep was investigated by using dual-colour flow cytometry and several parameters of lymphocyte activation. Upregulation of DR and DQ MHC Class II molecules was present in BALF of MVV-infected animals, with no changes in cell size and complexity or increased expression of IL-2R, suggesting an impaired process of lymphocyte activation in BALF lymphocytes of these animals. There was no evidence of lymphocyte activation in PB. To investigate if the impaired lymphocyte activation observed during the course of the natural disease was maintained under <I>in vitro</I> conditions, the response of BALF and PB lymphocytes to <I>in vitro</I> activation by exogenous mitogen was studied.

From the establishment of Harvard in 1636, to the establishment of the first public Junior College in 1901, to the curricular reforms of the 1960's and 1970's, the purpose of higher education and the role of general education have been the subjects for many lively debates. The national debate on issues relating to general education has intensified within the last decade, and community colleges continue to be active participants in these debates. The purpose of this study was to describe the course contents and classroom environments of selected community college general education general biology courses in sufficient detail as to shed light on the contribution each makes to general education. This study offers descriptions, interpretations, and evaluations of community college general education biology classrooms and laboratories. Abstract Modic changes are vertebral bone marrow signal intensity changes adjacent to the endplates of the degenerated intervertebral discs in magnetic resonance imaging (MRI). This study evaluated the prevalence and the determinants of Modic changes and their association with low back pain symptoms in an occupational cohort of middle-aged Finnish men. The prevalence and the natural course of Modic changes were assessed over a 3-year follow-up period among sciatica patients. Finally, in a patient population, the characteristics of bone marrow changes in MRI were compared to the imaging findings in CT. The prevalence of Modic changes was 56% in an occupational cohort of middle-aged males. Besides age, the determinants of Modic changes and disc degeneration were different. Weight-related factors, which add to the load of the lumbar spine, were associated with Modic changes, whereas whole-body vibration was associated with severe disc degeneration. The prevalence of Modic changes among sciatica patients was 65%, type II change being the most frequent. During the 3-year follow-up, 14% of changes converted to another type, while the incidence of new Modic changes was 6%. Among middle-aged working males, Modic changes located at L5–S1 and type I Modic changes were more likely to be associated with pain symptoms than other types of Modic changes or changes located at other lumbar levels. Thirty-eight percent of the endplates with Modic changes had sclerosis in CT. Of specific Modic types, mixed I/II and II/III associated significantly with endplate sclerosis. Endplate sclerosis was not detected in MRI. In conclusion, Modic changes are a common MRI finding both among patients and middle-aged working males. In addition to age, weight-related factors seem to be important in the pathogenesis of Modic changes. Modic changes can convert from one type to another and type II changes may be less stable than previously assumed. A considerable proportion of Modic changes are sclerotic as observed in CT. Modic changes were always found in combination with a degenerative intervertebral disc and thus they are assumed to be a specific phenotype of degenerative disc disease. Finally, Modic changes may be painful – especially when located at L5–S1 and type I changes.

Chronic lateral instability of the ankle (CLI), defined as frequent sprains and recurrent giving way, difficulty in walking and running on uneven surface, is often connected with pain and swollen ankles. It occurs in 10 to 20 percent after acute ankle injuries. Mechanical instability of the talocrural and subtalar joint, peroneal weakness and impaired proprioception has been suggested as etiological factors. Aim. To investigate the natural course in conservatively treated patients with CLI. To assess the mechanical stability in patients with CLI by measuring the three dimensional motions in the talus, the fibula and the calcaneus in relation to the tibia during different testing procedures pre- and postoperatively. To determine if CLI is associated with proprioceptive deficiency. Patients and Methods. This Thesis includes 127 ankles in 78 patients (30 women, 48 men) with CLI. Thirty-seven patients were followed up 20 years after their first contact with the orthopaedic department because of CLI. Forty-six ankles were evaluated radiographically and the result was compared with a gender- and age - matched control-material. The neuromuscular response to a sudden angular displacement of the ankles was studied in 15 ankles in 13 patients using EMG. Thirty-six patients entered a prospective study using roentgen stereophotogrammetric analysis (RSA) in which the ankles were tested at manual adduction, adduction with predetermined torque, with and without external support and at drawer tests (40 N and 160N). Twenty-seven patients were followed five years postoperatively. Result. After 20 years 22 patients, conservatively treated still suffered from instability of the ankle and ten had recurrent giving way symptoms even on plane surface. Six ankles in the patient group and four in the control group displayed osteoarthritic changes Prolonged ipsilateral reaction time (m. per. long, and m. tib. ant.) was found in patients with CLI indicating proprioceptive insufficiency. Increased talar adduction and a tendency toward increased total translation of the talar center was found in ankles with CLI. Concomitant fibular rotations and translations were found but with no conclusive deviation in the ankles with symptoms. The talo-calcaneal adduction reached the same level in the patient and control groups regardless of symptoms. External support (ankle brace) increased the talar stability. The use of predetermined torque and constrained testing procedure did not add information compared with the manual test Twenty-five patients graded the result as excellent or good five years after lateral ligament reconstruction. Talar stability (decreased adduction and translation) was increased two years postoperatively and was improved or remained the same at five years without comprising the range of motion. Conclusion. In more than half the cases symptoms of CLI did not resolve spontaneously. Minor degenerative changes was found after twenty years, but not to a greater extent than in a control group. CLI was associated with proprioceptive insufficiency and talocrural but not subtalar instability. Increased ankle stability can be obtained by the use of an ankle brace and by an anatomical ligament reconstruction.<br><p>Diss. The pulmonary immunopathology of parainfluenza type 3(PIV-3) infection in sheep was investigated firstly by isolating the virus from field cases of sheep pneumonia, secondly by experimentally reproducing the disease with the isolated virus and finally by studying changes in lymphocytes subsets and alveolar macrophages, induced by PIV-3 <I>in vivo </I>and <I>in vitro. </I>Three ovine virus isolates(270-7, 390-10 and 430-7) were obtained and characterised, as PIV-3, according to virus morphology, under transmission electron microscopy(TEM); cytopathic effect(CPE); haemagglutination, of guinea pig erythrocytes; physicochemical properties; serological crossreactivity with antisera raised against PIV-3, isolated from different species; and reactivity with monoclonal antibodies anti PIV-3 structural proteins, crossreacting with human and bovine strains. The ability of the virus to induce respiratory disease was investigated by experimental inoculation of ovine PIV-3 isolate, 270-7, in colostrum deprived lambs. Clinical, pathological, bacteriological and virological studies were carried out on days 2,3,5 and 7 infection(p.i.). This PIV-3 ovine strain was able to induce clinical disease. Histopathological findings were interstitial pneumonia with hyperplasia of bronchiolar associated lymphoid tissue (BALT), degenerative bronchiolar epithelium with lymphocyte infiltration, areas of atelectasis and increased alveolar septa thickness due to proliferation of pneumocytes II, lymphocytes, macrophages and later to fibrosis. The average cell count per normal BALT was 1,307(range 1,205 to 1,480 cells) whereas in infected lungs on PID 7 the average count increased to 4,788 (range 4,390 to 4,930 cells). The large number of lymphocytes, particularly on days 5 and 7 p.i., combined with the minimal to moderate cytolysis of antigen bearing cells, suggests that PIV-3-induced pulmonary disease has an immunopathological component. PIV-3 particles were detected, by immunohistochemistry, more frequently in bronchiolar epithelium cells than in alveolar septal cells and macrophages by day 3 p.i., but the situation was opposite by day 7 p.i.

Natural and human-induced variables influence turfgrasses C fluxes. Species composition, environmental conditions, site characteristics, former land use and agronomic management are the most important factors considered in literature driving C sequestration potential. At the same time different approaches seem to influence C budget estimates. In order to study the effect of different management intensities on turfgrass, we estimated net ecosystem exchange (NEE) through a SC approach in a hole of a golf course in the province of Verona (Italy) for one year. The SC approach presented several advantages but also limits related to the measurement frequency, timing and duration overtime, and to the methodological errors connected to the measuring system. Daily CO2 fluxes changed according to the intensity of maintenance, likely due to different inputs and disturbances affecting biogeochemical cycles, combined also to the different leaf area index (LAI). The annual cumulative NEE decreased with the increase of the intensity of management. NEE was related to the seasonality of turfgrass, following temperatures and physiological activity. Generally on the growing season CO2 fluxes towards atmosphere exceeded C sequestered. The cumulative NEE showed a system near to a steady state for C dynamics. In the final part greenhouse gases (GHGs) emissions due to fossil fuel consumption for turfgrass upkeep were estimated, pinpointing that turfgrass may result a considerable C source. The C potential of trees and shrubs needs to be considered to obtain a complete budget.

O conhecimento sobre sua história natural e fisiopatologia é baseado predominantemente em séries cirúrgicas. A ELTM familiar (ELTMF) é caracterizada pela recorrência em uma mesma família de indivíduos com ELTM com diversos graus de gravidade. Este aspecto, associado ao fato de alguns membros assintomáticos apresentarem sinais de AH na RM, faz com que o estudo prospectivo da ELTMF seja o cenário ideal para o melhor entendimento da história natural e fisiopatologia da ELTM. Materiais e Métodos: Analisamos prospectivamente 103 indivíduos pertencentes a 17 famílias com ELTMF. O tempo de seguimento foi em média 7,6 anos (SD ± 1,8 anos). Os membros das famílias foram divididos em três grupos: ELTMF (n=53), não classificáveis (crises que não preenchiam critérios para ELTM; n=18) e assintomáticos na primeira avaliação (n=32). O grupo com ELTMF foi subdividido em subgrupos: remissão (n=19), benignos (n=17) e refratários (n=17). Posteriormente o grupo com ELTMF foi reclassificado de acordo com sua evolução clínica em favorável e desfavorável. As evoluções clínicas foram correlacionadas com os possíveis fatores prognósticos. Sinais de AH na MRI foram definidos por análise visual. Em um estudo paralelo, foi feita a volumetria manual do hipocampo nas duas imagens de RM quando disponíveis. Resultados: No grupo benigno os pacientes evoluíram ou para refratariedade (17,6%) ou para remissão (23,5%). No grupo inicialmente em remissão, a maioria dos pacientes permaneceu em remissão e 21% foram reclassificados como benignos. Todos os pacientes refratários permaneceram refratários, exceto quando submetidos à cirurgia. Dos assintomáticos 12,5% desenvolveram ELTMF benigna e do grupo não classificáveis 22% evoluíram para o diagnóstico de ELTMF benigna. Foram considerados fatores preditivos relacionados à evolução clínica desfavorável a presença de AH (p=0,0192) e atividade epileptiforme no EEG (p=0,0174). A correlação entre incidentes precipitantes iniciais e evolução clínica não foi significativa, porém, uma clara tendência (p=0,0549) foi observada. Em relação à volumetria observamos progressão da atrofia quando comparamos o volume da RM inicial com a última RM. Discussão: Os resultados observados confirmam o caráter geralmente benigno da ELTMF. Este achado ressalta a existência de pacientes que mesmo na presença de sinais de AH apresentam uma evolução favorável, colocando em questão o conceito tradicional de que a ELTM com AH seria necessariamente refratária. Além disso, a demonstração da existência de fatores preditivos de má evolução clínica corrobora a hipótese de que outros fatores, além da influência genética, também estariam envolvidos na fisiopatologia desta doença. Em relação à progressão da ELTMF, dados de volumetria reforçam a teoria de que a ELTM seria uma doença progressiva, cuja velocidade de seu curso poderia ser influenciada por fatores externos. Este estudo longitudinal trouxe dados adicionais sobre a história natural da ELTMF que reforçam e clareiam algumas das hipóteses previamente formuladas. O melhor entendimento dos mecanismos subjacentes a ELTMF é importante para determinar estratégias terapêuticas visando melhor controle de crises e talvez, assim, prevenindo epileptogênese e progressão de dano neuronal<br>Abstract: Introduction: Mesial temporal lobe epilepsy (MTLE), the main cause of partial epilepsy in adulthood, is frequently associated with hippocampal atrophy (HA). The knowledge regarding its natural history and pathophysiology is based mainly on surgical series. Familial mesial temporal lobe epilepsy (FMTLE) is characterized by the recurrence of MTLE in the same family with different degrees of severity. This aspect, together with the fact that some asymptomatic members have MRI signs of HA, makes the prospective study of FMTLE the perfect scenario for the better understanding of the natural history and pathophysiology of MTLE. Material and Methods: We analyzed prospectively 103 individuals belonging to 17 families with FMTL. The mean duration of follow up was 7.6 years (SD, ± 1.8 years) Family members were divided into 3 groups: FMTLE (n=53), unclassified (seizures that did not fulfill criteria for MTLE, n=18) and asymptomatic at first evaluation (n=32). The FMTLE group was divided into 3 subgroups named: remission (n=19), benign (n=17) and refractory (n=17) and were further reclassified according to their outcome in favorable outcome and poor outcome. Outcome groups were correlated with possible predictor factors. MRI signs of atrophy were defined by visual analysis. In a related study, we performed manual volumetry in both MRI images when they were available. Results: In the benign group patients evolved to either refractoriness (17.6%) or remission (23.5%). In the remission group, most patients remained seizure free and only 21% were further reclassified as benign. All refractory patients remained refractory or underwent surgery. From the asymptomatic group 12.5% developed benign FMTLE and from the unclassified seizure group 22% evolved to a diagnosis of benign FMTLE. Predictive factors related to poor outcome were presence of HA (p=0.0192) and interictal epileptiform discharges on EEGs (p=0.0174). The correlation between initial precipitating injuries and clinical outcome was not significant although a clear tendency was observed (p=0.0549). Regarding volumetric analysis, when comparing hippocampal volumes from initial and last MRIs, we observed progression of HA. Discussion: Our results confirm the benign aspect of FMTLE. This finding emphasizes the existence of patients with good outcome even in the presence of signs of HA, challenging the notion that MTLE with HA is always refractory. Furthermore, the demonstration of predictive factors of poor clinical outcome supports the hypothesis that, in addition to the genetic influence, there should be other factors involved in the pathophysiology of this condition. Volumetric data reinforces the theory that MTLE might be a progressive disorder, and that the rate of progression would be influenced by external factors. This longitudinal study brought additional data regarding the natural history of FMTLE that strengthens and clarifies some of the previously formulated hypotheses. Every year roughly 350,000 women die during childbirth, primarily from preventable causes. The developing world accounts for the majority of this number, and in many regions maternal mortality rates are currently increasing despite the advancements our world has seen in the past decades. Maternal mortality has become a global issue, with international initiatives being launched around the globe. However, this problem hits closer to home than many Americans may know. The United States has the highest GDP in the world, yet it has only the 48th lowest maternal mortality rate. In addition, the past decade has seen our national maternal mortality rate increase rather than decrease. Throughout my study I explore why the United States is not a safer nation for women to give birth in considering our status as a wealthy, developed nation. In doing so I expose the problems inherent in the American medical system and the roots of those problems in larger cultural and social issues. Beneath this is an examination of the history of midwifery in the United States and an argument for increased use of midwives by American mothers and increased collaboration between midwives and the American medical system.

The follow-up incidence, stability as well as selected baseline risk factors (sociodemographics, psychopathology, trauma exposure) for the incidence and stability of somatoform disorders and syndromes are prospectively examined. Diagnostic information was assessed by using the standardized Munich-Composite International Diagnostic Interview (M-CIDI). Results. Over the follow-up period, incidence rate for any of the covered somatoform diagnoses was 25.7%. Stability for the overall group of any somatoform disorder/syndrome was 48%. Female gender, lower social class, the experience of any substance use, anxiety and affective disorder as well as the experience of traumatic sexual and physical threat events predicted new onsets of somatoform conditions, while stability was predicted by being female, prior existing substance use, affective and eating disorders as well as the experience of a serious accident. Conclusions. At least for a substantial proportion of individuals, the overall picture of somatization seems to be relatively stable, but with fluctuation in the symptom picture over time. Being female, the experience of substance use as well as anxiety disorder seem to constitute risk factors for the onset of new somatoform conditions as well as for a stable course over time.

Method: Cumulative incidence and patterns of cannabis use and disorders were examined in a prospective longitudinal design (mean follow-up period=42 months) in a representative sample (N=2446) aged 14–24 years at the outset of the study. Patterns of cannabis use, abuse and dependence (DSM-IV) were assessed with the Composite International Diagnostic Interview (M-CIDI). Results: (1) Cumulative lifetime incidence for cannabis use (at second follow-up): 47%; 5.5% for cannabis abuse, 2.2% for dependence. (2) Men used and abused cannabis more often than women. (3) The majority of the older participants (18–24 years at baseline) had reduced their cannabis use at follow-up, while younger participants (14–17 years at baseline) more often had increased their use and developed abuse or dependence. (4) The younger birth cohort (1977–1981) tended to start earlier with substance (ab)use compared to the older birth cohort (1970–1977). (5) Cannabis use was associated with increasing rates of concomitant use of other licit and illicit drugs. Conclusions: Cannabis use is widespread in our sample, but the probability of developing cannabis abuse or dependence is relatively low (8%). The natural course of cannabis use is quite variable: about half of all cannabis users stopped their use spontaneously in their twenties, others report occasional or more frequent use of cannabis.

Dissertação de Mestrado Integrado em Arquitetura, com a especialização em Arquitetura apresentada na Faculdade de Arquitetura da Universidade de Lisboa para obtenção do grau de Mestre.<br>Assinalando o ponto mais alto de Portugal Continental, a Serra da Estrela destaca-se pelas suas paisagens, repletas de história e aspectos naturais únicos no país, capazes de proporcionar experiências únicas a quem por lá passa. Situado no interior do território nacional, este maciço rochoso funciona como uma espécie de farol em plena beira interior, que atrai visitantes, mas que demonstra incapacidade de os cativar a permanecer. A falta de coordenação e entreajuda entre as entidades reguladoras do território ocultam uma beleza por explorar e um conjunto de actividades que se podem desenvolver ao longo do ano, tais como, escalada, competições de BTT, aulas de Paddle na Lagoa Comprida, caminhadas pelas rotas da transumância e visitas guiadas às instalações militares subterrâneas na Torre. A Serra da Estrela tem muito mais vida para além da neve e as actividades que lhe estão associadas como produto de consumo para turistas. O desequilíbrio que existe entre a época fria e a época quente é um dos factores que mais contribui para a subvalorização da imagem e valores patrimoniais, tanto da própria serra, como das regiões envolventes. Deste modo, o presente trabalho procura contribuir para o reconhecimento da necessidade de se restabelecer uma ligação entre o homem e o território, que se foi perdendo com o tempo. Pretende-se chamar à atenção para a importância da preservação e valorização do património cultural e natural intrínseco na paisagem da Serra da Estrela, que tantas vezes se vê marginalizada. Palco de raros eventos naturais, o Parque Natural da Serra da Estrela, representa uma das maiores atracções turísticas nacionais, em grande medida devido à neve, responsável por maior parte do movimento turístico registado na serra. De forma a evitar a inactividade que se verifica durante as épocas quentes, procura-se elaborar um programa que promova novas actividades no local, mais concretamente, ao nível do planalto superior da Serra. Para isso, o projecto, prevê a delimitação de um percurso que passa pelas lagoas do planalto superior do maciço central, rematando-o na Torre - o principal ponto de atracção turística da região e com maior afluência de pessoas por ano. Aproveita-se a influência deste local, utilizando-o como sede de divulgação de informação e educação para as pessoas, de modo a que estas percebam a importância dos cuidados a ter com a paisagem, visando, recuperar o seu protagonismo através do desenvolvimento de um projecto que interaja de forma directa com o território.<br>ABSTRACT: Marking the highest point in mainland Portugal, Serra da Estrela stands out for its landscapes, full of history and unique natural aspects in the country, capable of providing unique experiences to those who pass by. Situated in the interior of the national territory, this rock mass acts as a kind of lighthouse in the middle, which attracts visitors, but demonstrates inability to captivate them to stay. The lack of coordination and mutual help between the regulators of the territory conceals an untapped beauty and a range of activities that can be developed throughout the year, such as rock climbing, mountain biking competitions, Paddle lessons at Lagoa Comprida walks on the transhumance routes and guided tours to the underground military facilities in the Tower. Serra da Estrela has much more life than snow and the activities associated with it as a consumer product for tourists. The imbalance that exists between the cold season and the hot season is one of the biggest factors to the undervaluation of the image and heritage values, both of the mountains itself and the surrounding regions. Thus, the present work looks to contribute to the recognition of the need to reestablish a connection between man and territory, which has been lost over time. The aim is to draw attention to the importance of preserving and enhancing the intrinsic cultural and natural heritage in the Serra da Estrela landscape, which is often marginalized. Stage of rare natural events, the Serra da Estrela Natural Park, represents one of the largest national tourist attractions, largely due to snow, responsible for most of the recorded tourist movement in the mountains. In order to avoid inactivity that occurs during the hot seasons, it is intended to develop a program that promotes new activities on site, specifically at the level of the upper plateau of the Serra. To accomplish this, the project foresees the delimitation of a route that passes through the lagoons of the upper plateau of the central massif, ending it in the Tower - the main tourist attraction in the region and with the largest influx of people per year. It takes advantage of the influence of this place, using it as an information and education dissemination point for people, so they realize the importance of taking care of the landscape, aiming to recover its role through the development of a project that directly interacts with the territory.<br>N/A

In practical laboratory classes students traditionally receive data by reading from a measurement device (ruler, clock, voltmeter, etc.) or digitally as files in exchange formats such as CSV (comma separated value). In many cases these data have to be processed later using non-linear regression, here referred to as curve fitting. Therefore, analog data first have to be digitalised and imported to a data analysis and visualisation program, which is often commercial and requires installation. In this paper we present an alternative concept fusing open-source community tools into a single page web application facilitating data acquisition, visualisation, analysis via non-linear regression and further post processing usable for error calculations. We demonstrate the e-learning potential of this web application accessible at curvefit.tu-chemnitz.de in the context of acquired data as typically obtained in physical laboratory classes from undergraduate studies. A prototype workflow for the topic 'specific electric resistance determination' is presented along with a technical description of the basic web technology used behind. Restrictions, such as limited portability or cumbersome ways to share results electronically between student and supervisor as occurring in traditional software applications are overcome by enabling export via URL. The discussion is complemented by thorough comparison of curve fitting web applications with focus on their capability to be adaptable to user-specific models (equations) as faced by (undergraduate) students in the context of their education in laboratory classes in natural and life sciences, such as physics, biology and chemistry.

Over the past years, there has been a consistent call from Government and industry for South African tertiary institutions to deliver more graduates in the fields of science and technology. This, however, is no mean feat for universities, as the pool of prospective candidates delivers very few students with the necessary academic literacy abilities, and very few students who passed mathematics and science at the right levels to succeed in science higher education. This puts tertiary institutions under mounting pressure to accept students who are under-prepared and to support these students appropriately. The plight of Open and Distance Learning (ODL) institutions like the University of South Africa (UNISA) is even more desperate, as they are often left with those students who are either unable to gain entrance into, or to afford the study fees of, residential universities. These students are often in greater need for face-to-face interaction than are their counterparts at residential universities, yet they generally receive very little of this. The intervention examined and critiqued in this study is an attempt at raising the academic literacy levels of first-year students at UNISA in the fields of science and technology by means of a 60-hour face-to-face workshop programme. As its foundation, it uses the principles of collaborative learning and authentic material design. It also treats academic literacy abilities as interdependent and holistic. This study starts with a broad overview of the context. This is followed by a review of the literature. This review focuses on concepts such as collaborative learning, academic literacy, English for academic purposes, English for specific purposes and English for science and technology. Thereafter, a needs analysis is done in which students’ Test for Academic Literacy Levels (TALL) pre-test results, as well as a sample of their assignments, are examined. In addition, the workshops in this intervention programme are analysed individually. To determine the effectiveness of the academic literacy intervention, students’ pre- and post-TALL results are scrutinised, and a feedback questionnaire filled in at the end of the year is analysed. Subsequently, recommendations are made as to how the workshop programme could be improved. Findings show that the academic literacy intervention did improve students’ academic literacy levels significantly, though the improvement is not enough to elevate students from being considered at-risk. However, with fine-tuning the existing programme, the possibility exists that students’ academic literacy levels might be further improved. This calls for a careful examination of the areas in which students’ performance did not improve significantly. Student feedback indicated a positive attitude towards the entire intervention programme, as well as a marked preference for collaborative learning and face-to-face interaction. In the redevelopment of the current workshop programme, such preferences would have to receive attention, so as to integrate students’ wants, together with what they lack and what they need, in subsequent interventions. In conclusion, the limitations of this study are discussed, and recommendations are made for future research, as the current study must be seen as only the beginning of a process of action research that could lead to a sustainable intervention programme in future. Abstract:
Zwischen August 1978 und März 1979 wurden in der DDR durch viruskontaminiertes Anti-D-IgG 2867 Frauen mit HCV-Genotyp 1b infiziert, von denen 427 Frauen aus dem Leipziger Raum stammen. Diese Arbeit untersucht nach über 30 Jahren HCV-Infektion den natürlichen HCV-Verlauf von 356 der 427 Frauen aus dem Leipziger Raum. Ziel dieser Untersuchung ist herauszufinden, ob eine HCV-Infektion mit einer erhöhten Leberzirrhoserate bzw. erhöhten Mortalitätsrate einhergeht. Dabei stellen die 356 Frauen in dieser Untersuchung die Gesamtkohorte dar. In die Auswertung der Teilkohorte konnten 181 der 356 Frauen eingeschlossen werden, die im Zeitraum von 2005-2011 mindestens einmal untersucht und von denen aktuelle klinische und laborchemische Parameter erhoben wurden. Patientinnen mit HC-Viruspersistenz wurden z. T. mehrfach untersucht. Von der Gesamtkohorte wurden alle vorhandenen Leberbiopsien, serologische und molekularvirologische Daten wie anti-HCV, HCV-PCR und Viruslast, Leberzirrhose-Eintritt, Todesfälle, Formen der akuten Hepatitis-C-Infektion (ikterisch, anikterisch, asymptomatisch) erfasst. Die bestehenden Daten der Gesamtkohorte waren Grundlage für die Berechnung der Gesamt-Leberzirrhoserate und Gesamtmortalität sowie für die Erstellung der Kaplan-Meier-Kurven und Überlebenstabellen. Dabei wurde in den Überlebenskurven der Eintritt einer Leberzirrhose bzw. der Tod als Ereignis definiert. Für die Auswertung der Teilkohorte (n=181/356) wurden zusätzlich laborchemische Parameter (u. a. Transaminasen), Komorbiditäten (BMI, Diabetes mellitus, Steatosis hepatis), Koinfektionen, FibroScan-Werte, IL28 rs 12979860 Polymorphismus bzw. durch Anamnesebögen bestehender Alkoholkonsum, Medikation, Begleiterkrankungen bzw. subjektive Beschwerden, ermittelt. Bei mehreren Alanin-Aminotransferase-Werten (ALAT-Werten) wurde der Verlauf über fünf Jahre klassifiziert als ständig normwertig, fluktuierend, ständig erhöht und erhöht aufgrund hepatotoxische Einflüsse, die nicht allein HCV-bedingt sind (z. B. durch Alkoholkonsum, Steatosis hepatis, Medikation, Cholestase). In dieser Untersuchung wurden drei Gruppen untereinander verglichen. Patientinnen mit einem akut selbstlimitierenden Verlauf bzw. Patientinnen ohne HCV-Nachweis stellen die Kontrollgruppe (HCV-RNA negative Gruppe) dar, in der Annahme, dass die nur kurz verweilende Hepatitis-C-Infektion bei den akut selbstlimitierenden Verläufen keinen wesentlichen Einfluss auf die Überlebensrate hat. Die HCV-RNA positive Gruppe setzt sich aus therapienaiven HCV-Infizierten und antiviral therapierten HCV-Infizierten zusammen, die keine sustained virological response (SVR) erreicht haben, sogenannte Non-Responder. Diese HCV-RNA positive Gruppe soll das Risiko des weitgehend natürlichen Hepatitis-C-Verlaufes darstellen. Zu der Gruppe mit SVR zählen Patientinnen, die sechs Monate nach einer erfolgreichen antiviralen Therapie weiterhin HCV-PCR negativ sind. Patientinnen mit einer SVR wurden gesondert betrachtet, um einen zu starken Einfluss auf den natürlichen HCV-Verlauf auszuschließen. Dabei war die antivirale Ansprechrate mit Erreichen einer SVR in der Teilkohorte mit 60,1% höher als vorbeschrieben. Eine akute Hepatitis-C konnte bei 279/356 Patientinnen eruiert werden, von denen 82/270 (29,4%) einen ikterischen Verlauf aufwiesen. Dabei wird ein homozygoter CC-Polymorphismus mit einem ikterischen Verlauf und mit einer spontanen Viruselimination assoziiert. Unter Ausschluss der Patientinnen ohne HCV-Nachweis entwickelten 173/329 (53%) eine chronische Hepatitis-C. Das durchschnittliche Alter bei Infektion betrug 24 (16-38) Jahre und zum Zeitpunkt der Studie 54 (46-71) Jahre. Von der Teilkohorte waren 154/181 Frauen (85,1%) weiterhin anti-HCV positiv und bei 59/181 Frauen (32,6%) wurde HCV-RNA detektiert. Die Hälfte der Frauen (92/181; 50,8%) hatten permanent normwertige ALAT-Werte und bei 51/181 Frauen (28,2%) wurden fluktuierende ALAT-Werte ermittelt. Nur 38/181 Frauen (21%) wiesen permanent erhöhte ALAT-Werte auf, die v. a. bei HCV-RNA positiven Patientinnen nachgewiesen wurden, welche mit einer erhöhten Leberzirrhoserate assoziiert werden (p=0,001). Ein weiterer Leberzirrhose-assoziierender-Kofaktor ist ein bestehender Diabetes mellitus (p=0,013). Zusätzlich wird ein Diabetes mellitus mit einem vermehrten Auftreten von Steatosis hepatis (p=0,005) in Verbindung gebracht. Ein Zusammenhang zwischen Steatosis hepatis und erhöhte Leberzirrhoserate besteht hingegen nicht (p=0,839). In allen Gruppen ist mit steigendem Alter ein Zuwachs an Adipositas (32%), Diabetes mellitus (7,7%) und Steatosis hepatis (25,4%) vorhanden. Jedoch konnte bisher kein wesentlicher Einfluss auf die Leberzirrhoserate bzw. Mortalitätsrate bei HCV-RNA positiven Patientinnen nachgewiesen werden. Dennoch sollten Patienten mit einer chronischen HCV-Infektion auf eine Diabetes mellitus Manifestation getestet werden, um ggf. eine antivirale Therapie bei erhöhtem Leberzirrhose-Risiko zu beginnen. In künftigen Studien sollten diese Parameter mehr berücksichtigt werden, um einen möglichen Einfluss auf die Leberzirrhoserate feststellen zu können. Erwartungsgemäß weisen HCV-RNA positive Patientinnen einen höheren intrahepatischen Inflammations- und Fibrosegrad auf als HCV-RNA negative Patientinnen. Vor allem ist ein starker Anstieg an Leberzirrhosen bei den HCV-RNA positiven Frauen ab einem Lebensalter über 50 Jahre zu erkennen. Dennoch ist die Gesamtzirrhoserate 32 Jahre nach HCV-Infektion mit nur 6,45% (22/341) erstaunlich niedrig. Eine Erklärung ist, dass durch den exakt bekannten Infektionszeitpunkt dieser Untersuchung das gesamte HCV-Spektrum (Genotyp 1b), inklusive aller akut selbstlimitierenden Verläufe, Patientinnen ohne HCV-Nachweis und Patientinnen mit einer chronischen Hepatitis-C, erfasst wurde. Somit konnte eine einseitige Patientenselektion zu Gunsten fortgeschrittener Leberfibrosen von chronischen Hepatitis-C-Patientinnen vermieden werden. Ein weiterer Vorteil liegt in dem jungen Infektionsalter mit resultierenden geringen Komorbiditäten, die den natürlichen HCV-Verlauf beeinflussen könnten. Dennoch ist mit zunehmender Studienlänge eine steigende Unterrepräsentation an Patientinnen mit akut selbstlimitierenden Verläufen zu verzeichnen, die im Gegensatz zu den Patientinnen mit chronischer Hepatitis, bei fehlender Symptomatik nicht mehr kontinuierlich den Arzt konsultieren. Dadurch ist in der Teilkohorte der Anteil an fortgeschrittenen Leberzirrhosen mit 12/181 (6,6%) Präzirrhosen und 16/181 (8,8%) Leberzirrhosen erhöht. Dabei sind sieben Frauen mit einer Präzirrhose und 11 Frauen mit einer Leberzirrhose HCV-RNA positiv. Werden die Patientinnen mit einer SVR ausgeschlossen, erhöht sich die Zirrhoserate in der Teilkohorte auf 9,8% und in der Gesamtkohorte auf 6,9%. Die „wahre“ Leberzirrhoserate liegt womöglich zwischen diesen Prozentsätzen. Überraschenderweise besteht bisher keine erhöhte Mortalität bei HCV-RNA positiven Patientinnen im Vergleich zu HCV-RNA negativen Patientinnen, obwohl die Leberzirrhoserate bei den HCV-RNA positiven Patientinnen signifikant erhöht ist (Log-Rang: p=0,001). Insgesamt sind 14 von 349 Frauen (4%) verstorben, von denen acht nicht-HCV-assoziiert waren. Bedingt durch den verstärkten Einsatz von Dualtherapien mit Peg-Interferon und Ribavirin nimmt der Anteil chronisch HCV-Infizierter zu Gunsten der Patientinnen mit SVR kontinuierlich ab. Dennoch sind Leberzirrhosen und Todesfälle in der Gruppe mit SVR zu verzeichnen, die als Spätkomplikation einer durchlebten HCV-Infektion gewertet werden sollten, um eine weitere Patientenselektionen zu verhindern. Durch eine SVR sind normwertige Transaminasen bzw. regrediente Inflammations- bzw. Fibrosegrade zu erreichen. So haben von der Teilkohorte 36/48 (75%) der Patientinnen mit SVR normwertige ALAT-Werte. Die Zirrhoserate von Patientinnen mit SVR liegt 32 Jahre nach HCV-Infektion mit 11,5% signifikant unter der von HCV-RNA positiven Patientinnen mit 30%. Dennoch lässt sich eine erhöhte Überlebenswahrscheinlichkeit von Patientinnen mit SVR gegenüber HCV-RNA positiven Patientinnen momentan nicht belegen. Die nächsten 10 bis 20 Jahre werden eine richtungsweisende Aussage über die Überlebenskurven zeigen. Dafür sind weitere Langzeitstudien nötig. Jedoch wird es zukünftig immer schwieriger werden die Kohorte weiterzuverfolgen. Der Grund liegt in der anhaltenden Abnahme der untersuchten Patientinnen, bedingt durch Umzug, Namens- oder Arztwechsel. Zusammenfassend ist nach über 30 Jahren HCV-Infektion nur ein mäßiger Anstieg an Leberzirrhosen in einer weiblichen Population mit jungem Infektionsalter zu verzeichnen. Jedoch sollte bei zu erwartendem stärkeren Anstieg an Leberzirrhosen, bei HCV-RNA positiven Patientinnen ab einem Alter über 50, eine frühzeitige antivirale Therapie erwogen werden.

Abstract:
Der Langzeitverlauf und die Häufigkeit sogenannter spontaner Remissionen wurde anhand von 77 Fallen mit einer Lifetime-Diagnose einer Angststörung untersucht. Die 77 Angstfälle wurden als Teil der Münchner Follow-up-Studie im Rahmen einer allgemeinen Bevölkerungsuntersuchung im Jahre 1974 identifiziert und wurden über einen Zeitraum von sieben Jahren (bis 1981) weiter untersucht. Die Diagnosen wurden einerseits durch ein standardisiertes diagnostisches Instrument (den DIS), andererseits über eine klinisch-psychiatrische Nachuntersuchung (1981) abgesichert. In Ergänzung hierzu wurde der Verlauf der psychopathologischen Symptome sowie die psychologische und psychosoziale Integration der Versuchspersonen beurteilt. Ergebnisse: Die Lebenszeit-Prävalenz, irgendeine Anststörung zu entwickeln, betrug 13,9%. Einfache und soziale Phobien wiesen eine Prävalenz von 8,0%, Agoraphobie von 5,7%, Zwangsstörungen von 2,0% und Panikstörungen von 2,4% auf. Die Inzidenz, d.h. das Auftreten neuer Fälle im Zeitraum zwischen der Erst- und Zweituntersuchung war niedrig, mit Ausnahme für Panikstörungen (1,2%, bei einer Gesamtprävalenz von 2,4%) und Agoraphobie (1,3% bei einer Gesamtprävalenz von 5,7%). Die Komorbidität war sowohl innerhalb der Angststörungen wie auch bezüglich anderer psychischer Störungen erhöht. 62% hatten mehr als eine Angstdiagnose, Major Depression und Abhängigkeit von Alkohol oder Medikamenten waren die häufigsten komorbiden Störungen, die in der überwiegenden Mehrzahl deutlich nach dem Beginn der Angststörung auftraten. Spontanremissionen wurden auf drei unterschiedlichen Ebenen definiert, von denen ein Kombinationsmaβ als Hauptergebnis interpretiert wurde. Danach war die symptomatische Remission in alien Angstgruppen niedrig, während die psychosoziale Remissionsrate ein günstigeres Bild mit Remissionsraten zwischen 28,6% für Panikstörung und 53,1% für einfache und soziale Phobien ergab. Das kombinierte spontane Remissionsmaβ ergab für keine der Zwangsstörungen, nur 14,3% der Panikstörungen, 19,2% der Agoraphobien und 18,8% der einfachen und sozialen Phobie eine voile Remission. Die Ergebnisse unterstreichen, daβ Angststörungen zumeist in der Kindheit oder frühen Adoleszenz beginnen und dazu neigen, chronisch über den Groβteil des Lebens zu persistieren und nur selten zu remittieren.

In practical laboratory classes students traditionally receive data by reading from a measurement device (ruler, clock, voltmeter, etc.) or digitally as files in exchange formats such as CSV (comma separated value). In many cases these data have to be processed later using non-linear regression, here referred to as curve fitting. Therefore, analog data first have to be digitalised and imported to a data analysis and visualisation program, which is often commercial and requires installation. In this paper we present an alternative concept fusing open-source community tools into a single page web application facilitating data acquisition, visualisation, analysis via non-linear regression and further post processing usable for error calculations. We demonstrate the e-learning potential of this web application accessible at curvefit.tu-chemnitz.de in the context of acquired data as typically obtained in physical laboratory classes from undergraduate studies. A prototype workflow for the topic 'specific electric resistance determination' is presented along with a technical description of the basic web technology used behind. Restrictions, such as limited portability or cumbersome ways to share results electronically between student and supervisor as occurring in traditional software applications are overcome by enabling export via URL. The discussion is complemented by thorough comparison of curve fitting web applications with focus on their capability to be adaptable to user-specific models (equations) as faced by (undergraduate) students in the context of their education in laboratory classes in natural and life sciences, such as physics, biology and chemistry.

Before Namibia's independence in 1990 educational programmes and curricula gave little attention to environmental education which was detrimental to the development of environmental literacy in Namibia. The post independence education reform process paid special attention to the inclusion of environmental education in all spheres of learning. Furthermore, Namibia became a signatory to various environmental conventions thereby raising awareness about the environment and the potential challenges to a sustainable future. The initiatives of various environmental education projects since independence have responded to the growing concern of environmental education programmes, resulting in the establishment of the Namibian Environmental Education Course (NEEC). To date few studies have been conducted examining the relevance of EE programmes in relation to the professional practice of EE providers in the non-formal sector. This study focuses on the work done by two environmental education providers and four assistant environmental education tutors in Rundu, in North-Eastern Namibia. The study adopted a qualitative approach to investigate how the NEEC has shaped and informed the practice of EE providers in the non-formal sector and also how the NEEC has responded to the Education for Sustainable Development (ESD) programme. The study employed three data collection instruments: interviews, field observations and document analysis. The findings indicated that despite the input of the NEEC programme which provided an understanding and knowledge of environmental problems to the EE providers, there are issues that continue to hamper the effective practice of these providers. The study revealed that the NEEC programme, while within the structure of ESD, does not give explicit guidelines for identifying Namibia's environmental challenges and clear strategies of how to respond to these challenges. The findings of the study have provided valuable insights into aspects of an EE programme that need to be addressed to support EE providers in responding to key environmental challenges in their regions and contribute to the decade of Education for Sustainable Development.

Abstract:
To effectively prevent and mitigate the outbreak of natural disasters is a more pressing issue in the twenty-first century than ever before. The frequency and cost of natural disasters is rising globally, most especially in developing countries where the most severe effects of climate change are felt. However, while climate change is indeed a strong force impacting the severity of contemporary catastrophes, it is not directly responsible for the exorbitant cost of the damage and suffering incurred from natural disasters -- both financially and in terms of human life. Rather, the true root causes of natural disasters lie within the power systems at play in any given society when these regions come into contact with a hazard event. Historic processes of isolation, oppression, and exploitation, combined with contemporary international power systems, interact in complex ways to affect different socioeconomic classes distinctly. The result is to create vulnerability and scarcity among the most defenseless communities. These processes affect a society’s ideological orientation and their cultural norms, empowering some while isolating others. When the resulting dynamic socio-political pressures and root causes come into contact with a natural hazard, a disaster is likely to follow due to the high vulnerability of certain groups and their inability to adapt as conditions change. In this light, the following discussion exposes the anthropogenic roots of natural disasters by conducting a detailed case analysis of natural disasters in Haiti, Ethiopia, and Nepal.
