Full textThe current historical phase, characterized by the end of every form of institutionalization of minors in the direction of their integration into the community is marked from a legislative point of view by enactment and implementation of Law 144 of 2001 that to the Article 1 establishes: "Every child has the right to grow in their own family", with the primary objective of respecting the fundamental right of children to maintain their family and community ties. In 1997, the enactment of Law 285, marked a turning point in the understanding of the concept of domiciliarity which, from a mere medical and welfare function, has become connoted with educational functions and the promotion of the minor and his life context (Milani, 2004), recognizing in the figure of the educator the professionalism suitable to carry out this task. The question of home care services to support families of children in difficulty is, in a national scenario described as such, an obvious problem and on which to do research. Home educational interventions are located within these services with the general purpose of maintaining, even in situations of family hardship, the minor in the family of origin (Janssen, 2003). The research focused on how, in the Emilia-Romagna Region, the device of home educational interventions is interpreted and activated by the various institutional subjects (Court for Minors and Social Services), with an in-depth analysis on the context of the Services Social territorial. The subject matter of the thesis was explored through a careful analysis of literature, the structuring and administration of a questionnaire addressed to the Heads of the territorial Social Services - Minor Area of the Emilia-Romagna Region and the realization of some interviews with judges of the Court for Minors of Bologna.

Full textThe fragmentation of services for elderly with multimorbidity and frailty does guarantee neither effective and efficient care, nor the satisfaction of patients and their families. At international level, for decades, organizational models for “intermediate care” have been developing, as solution to emerging problems (aging, chronic disease, multiborbidity and frailty), to avoide inappropriate hospital admission and to delay long-term care. Consistent with national documents, Emilia-Romagna Region has been redefining community services and the acute hospital network with the aim of ensuring continuity of care and integration between hospital and community services, especially for people with chronic diseases, developing intermediate care and activating bed-based intermediate care services (Community Hospitals). The research project aimed to define – through the identification and analysis of specific indicators – a structured method for monitoring health care provided to patients in Community Hospitals in terms of clinical, organizational and patient perspective. The organizational model has been examined comparing three Community Hospital of Emilia-Romagna Region. Indicators have been analyzed to describe patients discharged from the Community Hospital of Castel San Pietro Terme (Imola Local Health Authority). The structured analysis and the identification of emerging clusters of patients admitted in Community Hospital, could provide important input for the best planning of integrated care pathways for patients. The description of clinical pathway of patients admitted in Community Hospitals may help to better define organizational strategies for both acute and community care, both health and social assistance, to enhance responses to patients' needs.

Full textINTRODUCTION Population level exposure to antibiotics is associated with antibiotic resistance, and frequent antibiotic prescribing to children is accepted as an indicator for sub-optimal prescribing to outpatients. There are a number of European studies investigating antibiotic prescribing to children and adolescents that report large variations between countries in choice of therapy and periodic prevalence of prescribing. Aim of this study is to analyse and compare the pattern of antibiotic prescription and use in children 0-5 years in Norway and in Italy (Emilia-Romagna Region). METHODS A retrospective observational study was conducted; data on antibiotic prescribing were obtained from the Norwegian Prescription Database (NorPD) and from Emilia-Romagna Health Authority Database (AFT). All outpatients between 0 and 5 years, with a dispensed prescription on antibiotics (Anatomical Therapeutic Chemical, ATC code J01) were included and a descriptive analysis was made to compare patients, prescriptions of antibiotics and prescribers characteristics among the following 5 age groups: 0-12 months, 13-24 months, 25-36 months, 37-48 months, 49-60 months. RESULTS In Emilia Romagna a greater use of antibiotics was registered (prevalence ranging from 68% to 59% in the study period), while in Norway about 29% of children received at least one prescription of antibiotics. Important differences also emerged in relation to the type of drugs prescribed: amoxicillin, alone and in combination with clavulanic acid, was the most used in Emilia Romagna against phenoxymethylpenicillin in Norway. CONCLUSION The results of the study showed differences in the use of antibiotic therapies in children in Emilia Romagna and Norway. In particular, excessive use of antibiotics has emerged in the Italian pediatric population; this therapeutic approach was often not consistent with national and international guidelines and recommendations. The comparison with a more virtuous reality leads to a reflection on the improvement strategies to be implemented in the field of antibiotic therapy.

Full textThe study that the candidate has worked in the PhD project is part of the complex process of solving the energy problem that necessarily involves different variables: economic, technical, political and social. The aim is to express an opinion on the actual “convenience" of the exploitation of renewable resources. The path chosen was to analyze some systems of exploitation, study their impact on the environment and finally compare them. This made it possible to find objective evidence to assess. In particular, the candidate has discussed the issue of resource "biomass" by analyzing in detail some industries in the Territory of the Emilia-Romagna: plants micro chain, short chain and long chain. With the collaboration of ARPA Emilia-Romagna, CISA Center and Prof. Ciancabilla Association, was made a choice of systems to be analyzed: a micro-industry: wood chip industry in Castel d'Aiano; a short chain: biogas production in industry of agricultural biomass "Mengoli" in Castenaso and finally in long chain: industry of solid biomass "Tampieri Energies" of Faenza. With regard to the methodology used to study has been carried out a study of Life Cycle Assessment (LCA). By using the software "SimaPro 6.0", the results for the impact categories of facilities considering methods "Eco Indicator 99" and "Edip Umip 96". The comparison between the results of the different systems did not lead to general conclusions, but in-depth assessments specific to each industry analyzed, given the multiplicity of variables of all reality, both for the size / scale (micro-industry, short chain and long chain) that as regards the typology biomass used.

To evaluate the prevalence of several HPV genotypes in patients with CIN2/3 in Emilia-Romagna, the genotype-specific HPV DNA persistence and the expression of HPV oncogenes E6/E7 during follow-up after conisation, and their role in the prediction of residual disease; to verify the applicability of new molecular diagnostic tests in cervical cancer screening. Methods. Patients with abnormal screening citology treated by conisation (T0) for histologically confirmed CIN2/3 were included. At T0 and at 6, 12, 18 and 24 months of follow-up, in addition to the Pap smear and colposcopy, research and genotyping of HPV DNA of 28 types were performed. In case of positivity to the DNA of the 5 genotypes 16, 18, 31, 33 and/or 45, we proceeded to detect HPV E6/E7 mRNA. Preliminary results. The 95.8% of 168 selected patients were HPV DNA positive at T0. In 60.9% of cases the infections were single (mostly HPV 16 and 31), in 39.1% were multiple. HPV 16 was the most frequent genotype detected (57%). The 94.3% (117/124) of patients positive for 5 genotypes of HPV DNA were mRNA positive. Of the 168 patients 38 dropped out. At 18 months (95% of patients), the persistence of HPV DNA of any genotype was 46%, of HPV DNA of 5 genotypes was 39%, with mRNA expression in 21%. We found recurrent disease (CIN2 +) in 10.8% (14/130) at 18 months. Citology was negative in 4/14 cases, HPV DNA test was positive in all cases, mRNA testing in 11/12 cases. Conclusions: HR-HPV DNA test is more sensitive than cytology, mRNA testing is more specific in identifying a recurrence. Final data will be available after the follow-up planned.
