We present abstracts of MSc theses defended in 2009 at the Faculty of Medicine, University "Ss Kiril and Metodij", Skopje, Republic of Macedonia. English summaries are published as they are translated by authors and included in the final version of defended MSc. Macedonian Medical Master of Science (MSc) theses are deposited in the Central Medical Library in Skopje.Editorial Borad do not take any responsibility either for the content, nor the quality of the abstracts.At the Faculty of Medicine, 30 MSc theses were defended in 2009. Eight MSc theses are without abstarcts (26.7%), 8 MSc theses are with unstructured abstracts (26.7%), and 14 are with structured abstracts (46.7%) which is significant improvment in comparison with defended MSc in 2008. For one MSc there are not any details about the thesis (Elena Manoleva Nikolovska. Anthropometrical indicators for evaluation of the nutritive status in children between ages of 3 until 36 months), and one MSc was retracted (Ana Strezova. Genotyping of HLA-A, HLA-C and HLA-B loci with Reverse Line Strip hybridization in Macedonian Population and association of HLA class I alleles with spondilitis).We have to repeat that primary responsibility for the quality of the MSc theses belongs to the mentors, to the institutions they are representing, and to the Vice-Dean of science. All of them should be more actively involved in the preparation of Master of Science theses in order international standards to be achieved.

The purpose of the paper is to discover and paint the context of the child and the young girl in the midst of war, trauma to survive, the consequences arising from them, and how these female characters work tirelessly with all this.

Abstract:
We present a total-variation-regularized image segmentation model that uses local regularization parameters to take into account spatial image information. We propose some techniques for defining those parameters, based on the cartoon-texture decomposition of the given image, on the mean and median filters, and on a thresholding technique, with the aim of preventing excessive regularization in piecewise-constant or smooth regions and preserving spatial features in nonsmooth regions. Our model is obtained by modifying a well-known image segmentation model that was developed by T. Chan, S. Esedoḡlu, and M. Nikolova. We solve the modified model by an alternating minimization method using split Bregman iterations. Numerical experiments show the effectiveness of our approach.

The starved-to-fed transition is accompanied by rapid glycogen deposition in skeletal muscles. On the basis of recent findings [Bräu, Ferreira, Nikolovski, Raja, Palmer and Fournier (1997) Biochem. J. 322, 303-308] that during recovery from exercise there is a shift from a glucose 6-phosphate/phosphorylation-based control of glycogen synthesis to a phosphorylation-based control alone, this paper seeks to establish whether a similar shift occurs in muscle during re-feeding after starvation in the rat. Chow re-feeding after 48 h of starvation resulted in glycogen deposition in all muscles examined (white, red and mixed quadriceps, soleus and diaphragm) to levels higher than those in the fed state. Although the early phase of re-feeding was associated with increases in glucose 6-phosphate levels in all muscles, there was no accompanying increase in the fractional velocity of glycogen synthase except in the white quadriceps muscle. This finding, together with the observation that the fractional velocity of glycogen synthase in most muscles was already high in the starved state, suggests that in the initial phase of glycogen deposition the phosphorylation state of the enzyme may be adequate to support net glycogen synthesis. In the later phase of re-feeding, the progressive decrease in the fractional velocity of glycogen synthase in association with a decrease in the rate of glycogen deposition suggests that glycogen synthesis is controlled primarily by changes in the phosphorylation state of glycogen synthase. In conclusion, this study suggests that there is a temporal shift in the site of control of glycogen synthesis as glycogen deposition progresses during re-feeding after starvation.

Abstract:
The characteristics of the glomerular filtration barrier (GFB) are challenging to measure, as macromolecular solutes in blood may be metabolized or transported by various cells in the kidney. Urinary solute concentrations generally reflect the cumulative influence of multiple transport processes rather than the intrinsic behavior of the GFB alone. Synthetic tracer molecules which are not secreted, absorbed, or modified by the kidney are useful tools. Ficoll, a globular polymer of epichlorohydrin and sucrose, is round, physiologically inert, and easily labeled, making it a nearly ideal glomerular probe. Fissell et al. reported filtration data suggesting that Ficoll was not as spherical as had been previously suggested (Fissell WH, Manley S, Dubnisheva A, Glass J, Magistrelli J, Eldridge AN, Fleischman AJ, Zydney AL, Roy S. Am J Physiol Renal Physiol 293: F1209–F1213, 2007). More recently, two investigators published comparisons of neutral and anionic Ficoll clearance that suggest Ficoll may undergo conformational changes when chemically derivatized (Asgeirsson D, Venturoli D, Rippe B, Rippe C. Am J Physiol Renal Physiol 291: F1083–F1089, 2006; Guimaraes MAM, Nikolovski J, Pratt LM, Greive K, Comper WD. Am J Physiol Renal Physiol 285: F1118–F1124, 2003). To investigate Ficoll's characteristics further, we examined two commercial preparations, Ficoll 70 and Ficoll 400, by size-exclusion chromatography using a differential refractive index detector combined with light-scattering and viscosity detectors. A slope of 0.45 was obtained from the plot of the logarithm of molecular mass against the logarithm of root-mean square radius. The Mark-Houwink exponent values of 0.34 and 0.36 were calculated for Ficoll 70 and Ficoll 400, respectively. These results suggest Ficoll's conformation in physiological saline solution is likely intermediate between a solid sphere and a well-solvated linear random coil. The measurements help explain our previous observations and guide interpretation of in vivo experiments.

The glomerular filtration barrier (GFB) is commonly conceived as a negatively charged sieve to proteins. Recent studies, however, indicate that glomerular charge effects are small for anionic, carboxymethylated (CM) dextran vs. neutral dextran. Furthermore, two studies assessing the glomerular sieving coefficients (θ) for negative CM-Ficoll vs. native Ficoll have demonstrated an increased glomerular permeability for CM-Ficoll (Asgeirsson D, Venturoli D, Rippe B, Rippe C. Am J Physiol Renal Physiol 291: F1083–F1089, 2006; Guimarães M, Nikolovski J, Pratt L, Greive K, Comper W. Am Physiol Renal Physiol 285: F1118–F1124, 2003.). The CM-Ficoll used, however, showed a larger Stokes-Einstein radius ( ae) than neutral Ficoll, and it was proposed that the introduction of negative charges in the Ficoll molecule had made it more flexible and permeable. Recently, a negative FITC-labeled CM-Ficoll (CMI-Ficoll) was produced with a conformation identical to that of neutral FITC-Ficoll. Using these probes, we determined their θ:s in anesthetized Wistar rats (259 ± 2.5 g). After blood access had been achieved, the left ureter was cannulated for urine sampling. Either polysaccharide was infused (iv) together with a filtration marker, and urine and plasma were collected. Assessment of θ FITC-Ficoll was achieved by high-performance size-exclusion chromatography (HPSEC). CMI-Ficoll and native Ficoll had identical elugrams on the HPSEC. Diffusion of anionic Ficoll was significantly reduced compared with that of neutral Ficoll across the GFB for molecules of ae ∼20–35 Å, while there were no charge effects for Ficoll of ae = 35–80 Å. The data are consistent with a charge effect present in “small pores,” but not in “large pores,” of the GFB and mimicked those obtained for anionic membranes in vitro for the same probes.

There is little information about the mechanisms by which G-protein–coupled receptors gate ion channels although many ionotropic receptors are well studied. We have investigated gating of the muscarinic cationic channel, which mediates the excitatory effect of acetylcholine in smooth muscles, and proposed a scheme consisting of four pairs of closed and open states. Channel kinetics appeared to be the same in cell-attached or outside-out patches whether the channel was activated by carbachol application or by intracellular dialysis with GTPγS. Since in the latter case G-proteins are permanently active, it is concluded that the cationic channel is the major determinant of its own gating, similarly to the KACh channel (Ivanova-Nikolova, T.T., and G.E. Breitwieser. 1997. J. Gen. Physiol. 109:245–253). Analysis of adjacent-state dwell times revealed connections between the states that showed features conserved among many other ligand-gated ion channels (e.g., nAChR, BKCa channel). Open probability (PO) of the cationic channel was increased by membrane depolarization consistent with the prominent U-shaped I-V relationship of the muscarinic whole-cell current at negative potentials. Membrane potential affected transitions within each closed-open state pair but had little effect on transitions between pairs; thus, the latter are likely to be caused by interactions of the channel with its ligands, e.g., Ca2+ and Gαo-GTP. Channel activity was highly heterogeneous, as was evident from the prominent cycling behavior when PO was measured over 5-s intervals. This was related to the variable frequency of openings (as in the KACh channel) and, especially, to the number of long openings between consecutive long shuttings. Analysis of the underlying Markov chain in terms of probabilities allowed us to evaluate the contribution of each open state to the integral current (from shortest to longest open state: 0.1, 3, 24, and 73%) as PO increased 525-fold in three stages.

Abstract:
Background:Osteoporosis is known to be a risk factor for fragility fractures [4, 5]. On one hand, vertebral body fragility fractures often lead to additional spine deformity [2]. On the other hand, it was found that with the progression of the spinal curvature in osteoporotic patients, the fragility fractures develop more frequently. The increased incidence of these fractures could be explained with a predominance of the mechanical forces on the one side of the already weakened osteoporotic vertebrae [3].Objectives:The aim of this study is to compare the fracture risk (FRAX) for major osteoporotic fractures (MOF) and for hip fractures (HF) in women with and without scoliosis through dual-energy X-ray absorptiomentry (DXA)Methods:In the current study, 59 women underwent DXA scans. Scoliosis was defined as Cobb’s angle ≥ 5◦ according to the Chaklin’s classification [6, 7]. Cobb’s angle was measured from DXA images with DICOM software. We evaluated the following risk factors: previous fractures, parental hip fractures, secondary osteoporosis, rheumatoid arthritis, use of corticosteroids, current smoking and alcohol consumption more than 3 units daily. We estimated FRAX MOF and FRAX HF on the basis of these risk factors and on the basis of the femoral neck bone mineral density (BMD). The calculations were done through FRAX tool published on the website of the University of Sheffield [1].Results:The mean age of the women was 63 years (yrs.) ± 10 yrs. (range 43 yrs. – 89 yrs.). Subjects with scoliosis were significantly older (67 yrs.) than those without scoliosis (59 yrs.), (p = 0.004). Mean weight and height didn’t differ between the groups with- and without scoliosis. Mean lumbar spine BMD and T-score differed significantly between the groups, (p = 0.02). Women with scoliosis had lower mean BMD (0.786 g/cm2) and lower mean T-score (-2.1 standard deviations (SDs)) compared to those without scoliosis (mean BMD: 0.912 g/cm2 and mean T-score: 0.9 SDs). Abstract:
This paper proposes a pure silica photonic crystal fiber (PCF), having its core infiltrated with olive oil, which allows achieving an ultra-flattened normal dispersion regime. As a result, the optimization processes allows us to achieve an ultra-flat normal dispersion in the range of over 682 nm within the wavelength range from 1446 to 2128 nm. Besides, the nonlinear coefficient of the selected PCF structure is extremely high (9.54 x 109 W-1.km-1 at 1550 nm). Abstract:
From year to year the number of investigations on intelligent systems grows rapidly. For example this year 245 papers from 45 countries were sent for the Second International IEEE Conference on Intelligent Systems (www.ieee-is.org; www.fnts-bg.org/is) and this is an increase of more than 50% by all indicators. The presented papers on intelligent systems were marked by big audiences and they provoked a significant interest that ultimately led to the formation of vivid discussions, exchange of ideas and locally provoked the creation of working groups for different applied projects. All this reflects the worldwide tendencies for the leading role of the research on intelligent systems theoretically and practically. The greater part of the presented research dealt with traditional for the intelligent systems problems like artificial intelligence, knowledge engineering, intelligent agents, neural and fuzzy networks, intelligent data processing, intelligent control and decision making systems, and also new interdisciplinary problems like ontology and semantics in Internet, fuzzy intuitionistic logic. The majority of papers from the European and American researchers are dedicated to the theory and the applications of the intelligent systems with machine learning, fuzzy inference or uncertainty. Another big group of papers focuses on the domain of building and integrating ontologies of applications with heterogeneous multiagent systems. A great number of papers on intelligent systems deals with fuzzy sets. The papers of many other researchers underscore the significance of the contemporary perception-oriented methods and also of different applications in the intelligent systems. On the first place this is valid for the paradigm of L. A. Zadeh 'computing with words'. The Guest Editors in the present specialized journal volume would like to introduce a wealth of research with an applied and theoretical character that possesses a common characteristic and it is the conference best papers complemented and updated by the new elaborations of the authors during the last half a year. A short description of the presented in the volume papers follows. In 'Combining Local and Global Access to Ontologies in a Multiagent System' <B>R. Brena and H. Ceballos (Mexico)</B> proposed an original way for operation with ontologies where a part of the ontology is processed by a client's component and the rest is transmitted to the other agents by an ontology agent. The inter-agent communication is improved in this way. In 'Fuzzy Querying of Evolutive Situations: Application to Driving Situations' <B>S. Ould Yahia and S. Loriette-Rougegrez (France)</B> present an approach to analysis of driving situations using multimedia images and fuzzy estimates that will improve the driver's security. In 'Rememberng What You Forget in an Online Shopping Context' <B>M. Halvey and M. Keane (Ireland)</B> presented their approach to constructing online system that predicts the items for future shopping sessions using a novel idea called Memory Zones. In 'Reinforcement Learning for Online Industrial Process Control' the authors <B>J. Govindhasamy et al. (Ireland)</B> use a synthesis of dynamic programming, reinforcement learning and backpropagation for a goal of modeling and controlling an industrial grinding process. The felicitous combination of methods contributes for a greater effectiveness of the applications compared to the existing controllers. In 'Dynamic Visualization of Information: From Database to Dataspace' the authors <B>C. St-Jacques and L. Paquin (Canada)</B> suggested a friendly online access to large multimedia databases. <B>W. Huang (UK)</B> redefines in 'Towards Context-Aware Knowledge Management in e-Enterprises' the concept of context in intelligent systems and proposes a set of meta-information elements for context description in a business environment. His approach is applicable in the E-business, in the Semantic Web and in the Semantic Grid. In 'Block-Based Change Detection in the Presence of Ambient Illuminaion Variations' <B>T. Alexandropoulos et al. (Greece)</B> use a statistic analysis, clustering and pattern recognition algorithms, etc. for the goal of noise extraction and the global illumination correction. In 'Combining Argumentation and Web Search Technology: Towards a Qualitative Approach for Ranking Results' <B>C. Chesñevar (Spain) and A. Maguitman (USA)</B> proposed a recommender system for improving the WEB search. Defeasible argumentation and decision support methods have been used in the system. In 'Modified Axiomatic Basis of Subjective Probability' <B>K. Tenekedjiev et al. (Bulgaria)</B> make a contribution to the axiomatic approach to subjective uncertainty by introducing a modified set of six axioms to subjective probabilities. In 'Fuzzy Rationality in Quantitative Decision Analysis' <B>N. Nikolova et al. (Bulgaria)</B> present a discussion on fuzzy rationality in the elicitation of subjective probabilities and utilities. The possibility to make this special issue was politely offered to the Guest Editors by Prof. Kaoru Hirota, Prof. Toshio Fukuda and we thank them for that. Due to the help of Kenta Uchino and also due to the new elaborations presented by explorers from Europe and America the appearance of this special issue became possible.

Abstract:
The aim of the study was to evaluated the effect of giving water hyacinth and fish oil in quail diet on the performance, egg cholesterol and egg fatty acid profile. This study used 200 unsex day old quail divided into 5 treatments and 4 replications that were reared 5 weeks and then selected male or female. The results of sexing quail at week 5 were 95 females and 86 males. Growth period diets (0-5 weeks) contains 24% crude protein and metabolic energy of 2900 kcal kg-1 and production period diets (5-12 weeks) contains 22% crude protein and 2900 kcal kg-1. The experimental design used a completely randomized design with the treatment of using water hyacinth that was 0%,1%, 2%, 3 % and 4% with fish oil which was 0%, 4%, 5%, 6% and 7% in quail diet. The variables measured were quail performance (0-5 week age and 5-12 week age), egg cholesterol, and egg fatty acid profile. The results showed that using of water hyacinth meal 1% -4% and fish oil 4%-5% in the diet of the growth period resulted in higher feed consumption and body weight gain than diet without water hyacinth and fish oil but feed conversion rasio were same. Quail performance of the production period was not affected by all treatments. Unsaturated fatty acids tend to increased in quail eggs. The lowest cholesterol level of quail eggs was by giving 2% of water hyacinth meal and 5% of fish oil in the diet. It was concluded that water hyacinth can be used as local feed ingredients in quail diet until 4%. Abstract:
V prispevku so predstavljene krajšave, vsesplošno rastoči fenomen, ki je prisoten v vseh jezikih. V slovenskem prostoru so se s krajšavami ukvarjali številni slovničarji, podrobno jih je klasificiral Matej Rode, zadnja obsežna klasifikacija pa je v Slovenskem pravopisu iz leta 2001. Krajšave so zajete v splošnih, specializiranih, eno- in dvojezičnih slovarjih, zaradi hitre dinamike nastajanja so težje ulovljive, slovarji v pisni obliki pa izhajajo preredko, da bi jih ažurno beležili v same slovarje. V zadnjih letih nastaja vse več spletnih zbirk, ki so prosto dostopne in ažurne ter uporabniku omogočajo tudi vnos novih krajšav. Zbirke temeljijo na besedilnih korpusih ali spletnih virih in se s pomočjo pravil in algoritmov lahko oblikujejo tudi samodejno. V nadaljevanju prispevka so predstavljeni pristopi samodejnega prepoznavanja krajšav v zbirki ADAM ter metoda Sateve in Nikolova. Ob pomoči slednjega sledi primer algoritma za prepoznavanje krajšav v slovenskih besedilih.
