Abstract:
In the paper a static analysis of two-fingered grips is presented. Importance of grasp analysis in human performance engineering and hand therapy is discussed and assessment methods are proposed. Two precision grips were examined in the study: nippers pinch and tip pinch. We have built a grip-measuring device assessing the endpoint forces of two-oppositional grips. The instrument developed is based on a robotic force-wrist sensor and designed to suit human fingers. Through the simultaneous use of an optical measuring system and the grip-measuring device, the finger positions and the grip force acting on the object were obtained. A static model of the hand was also developed. A recursive computational method was used to calculate the joint torques of the fingers from the measured coordinates and the endpoint force vector. The estimated joint torque corresponds to the amount of load on the tendon during isometric muscle contraction. The force output of the two grips considered is presented and compared in two healthy individuals. The calculated joint torques for the index finger and thumb are presented and the maximal values of the finger joint torques acquired in the study are shown.

Zusammenfassung Hintergrund Die Arbeitsgruppe um Kälble stellte 2008 das „Serosa lined and tapered ileum“ („Fulda-Nippel“) als neuen Kontinenzmechanismus des modifizierten MAINZ-Pouch-I vor. In Anlehnung an das Prinzip von Abol-Enein wird ein getapertes Ileumsegment in einen seroserösen Tunnel aus einem zweiten „U“-förmigen Ileumsegment eingebettet. Bei dieser Technik werden Kontinenzmechanismus und Augmentation miteinander kombiniert. Sie kann in allen Pouchformen und auch als kontinente Vesikostomie genutzt werden. Patienten und Methoden In der urologischen Klinik des Universitätsklinikums Bonn wurde diese Technik seit 2008 in bisher 21 Fällen bei unterschiedlichen Indikationen verwendet. In der ersten Analyse von 2011 konnte der Nippel bereits durch geringe Frühkomplikationsraten überzeugen. Ziel dieser Studie ist die Re-Evaluation der Technik insbesondere im Hinblick auf Stenose- und Inkontinenzraten im Langzeitverlauf. Ergebnisse Bei einer mittleren Nachbeobachtungszeit von 37 Monaten traten Stomastenosen in 33 % der Fälle auf. Eine Inkontinenz des Nippels wurde bei 21 % (n = 4) der Patienten beobachtet. Bei zwei der Patienten war die Ursache der Inkontinenz das komplette Ausgleiten des Nippels aus der seroserösen Einbettung. Schlussfolgerung In der Langzeitanalyse zeigen sich ähnliche Stenose- und Inkontinenzraten im Vergleich zu den zwei etabliertesten Techniken, dem Appendixnippel und dem Ileuminvaginationsnippel. Trotz Limitation aufgrund der kleinen Fallzahl ist der Fulda-Nippel zumindest als eine adäquate „Zweitlinientechnik“ bei Versagen der initialen Methode anzusehen. Als Vesikostomie bietet der Fulda-Nippel den Vorteil, Augmentation und Kontinenzmechanismus elegant miteinander zu vereinen und sollte hier auch als Primärtechnik Verwendung finden.

Abstract:
PURPOSE: To analyze the viability of using SITRACC® (single-portal access) to make partial or total nephrectomy in pigs and also to describe the technical difficulties found during these surgical procedures. METHODS: Ten pigs (Landrace specie) with 20kg in average were distributed in two groups: Group A - total right nephrectomy - and Group B - partial left nephrectomy -. The anesthetic procedure was initially done with thiopental (10mg/kg) and maintained with halothane. The surgical procedures were performed inside the Surgical Technique Room from Pontifical Catholic University of Parana (PUCPR). RESULTS: It was analyzed the surgery duration, time to insert SITRACC®, the volemic loss and the size of the organ. In the first surgeries, the surgical time and blood volume loss were higher (between 15 to 43 minutes and 120 to 400 mL, respectively). The more the procedure was performed, the more the surgeons were used to the technique, so the bleeding and the time of surgery was significantly reduced to 15 minutes and 50 mL of blood loss, respectively. The difficulties found were associated with the nippers, however that was not relevant for the surgical technique. It was also found that the intra-body suture caused an expressive volemic loss. CONCLUSION: The total and partial nephrectomy through umbilical single-access by using SITRACC® was feasible and safe in pigs.

Abstract:
This article formally documents an important correction to the provenance attribution of three reclining female figurines from Babylon that reside in the Nippur collection at the University of Pennsylvania Museum and were published with that corpus. Few scholars have noticed the misattribution of these figurines, and the problem has not been formally documented for scholarship. Through historiographical analysis of the late nineteenth century Nippur Expeditions and early twentieth century cataloguing and publication of the Nippur corpus, this article reconstructs how and why these three reclining figurines have been continually misassociated with Nippur, and traces the continued impact of this confusion on scholarship's understanding of the Nippur figurine tradition. Most critically, the publication of these three figurines as Nippur objects lent credence to the testimony of an antiquities dealer who sold an additional eight reclining figurines “from Nippur” to the Harvard Semitic Museum; these figurines continue to be regarded as Nippur objects. This article casts doubt upon that provenance. The figurine tradition of Seleucid-Parthian Nippur is reevaluated in light of the absence of securely-provenanced reclining female figurines at that site. An art historical evaluation of these figurines is undertaken, which links these figurines to the general use of hybrid Greek-Babylonian imagery in Seleucid-Parthian figurines, and connects the specific motif of the reclining figure to Greek banqueting imagery. It is proposed that the Nippur community's lack of interest in reclining female figurines can be correlated with a disinterest in pan-Hellenistic ceramic tablewares; together, these lacunae indicate Nippur's non-participation in negotiated Greek-Babylonian banqueting practices. These differences in cross-cultural interaction between Nippur and the neighboring Babylonian communities have not been fully recognized nor explored, due to scholarship's misunderstanding of the use of reclining female figurines at that site. It is this confusion that this article attempts to resolve.

Abstract:
Present-day terms such as the usufruct – in civil law systems – and its equivalent, the life-right – in common law systems – were foreign to ancient Near Eastern legal texts. Prima facie both terms – usufruct and life-right – direct the “time-limited interest” of the use and enjoyment by a person over the property of another. However, mainstream ancient Near Eastern scholars’ unqualified use of the foreign terms – diverged in time and space – affect the translation and our insight into ancient texts. In addition, differences in land ownership institutions and philosophies in present-day law systems and those of ANE contribute to variances in the meaning and interpretation of the intrinsic aspects of property and as such “time-limited interest” applicable: a usufruct, life-right or even a hybrid form of both. In the article, I focus on the maintenance – a time-limited interest – of the nadītu priestess in the Old Babylonian city-state of Nippur. The application of Stone’s theory on Nippur’s land ownership – the institutions’ economy – prima facie shows that the nadiātu of Nippur held a freestanding life-right, rather than a usufruct which the majority of ANE scholars assigned to the nadiātu’s maintenance. However, I propose a deviation with the superficial overlay of present-day terms on the maintenance of the nadiātu by presenting a time-limited interest framework. The framework serves as a delineation method of identifying the characteristics of the maintenance-construction of the nadiātu from OB Nippur: communicating a “unitary concept” in context of the ancient texts – rather than only assigning coined terms – taking recognition of the influences of Nippur’s land ownership philosophy.

Abstract:
Recent fieldwork and archival sedimentary materials from southern Iraq have revealed new insights into the environment that shaped southern Mesopotamia from the pre-Ubaid (early Holocene) until the early Islamic period. These data have been combined with northern Iraqi speleothem, or stalagmite, data that have revealed relevant palaeoclimate information. The new results are investigated in light of textual sources and satellite remote sensing work. It is evident that areas south of Baghdad, and to the region of Uruk, were already potentially habitable between the eleventh and early eighth millennia B.C., suggesting there were settlements in southern Iraq prior to the Ubaid. Date palms, the earliest recorded for Iraq, are evident before 10,000 B.C., and oak trees are evident south of Baghdad in the early Holocene but disappeared after the mid-sixth millennium B.C. New climate results suggest increased aridity after the end of the fourth millennium B.C. For the third millennium B.C. to first millennium A.D., a negative relationship between grain and date palm cultivation in Nippur is evident, suggesting shifting cultivation emphasising one of these crops at any given time in parts of the city. The Shatt en-Nil was also likely used as a channel for most of Nippur's historical occupation from the third millennium B.C. to the first millennium A.D. In the early to mid-first millennium A.D., around the time of the Sasanian period, a major increase in irrigation is evident in plant remains, likely reflecting large-scale irrigation expansion in the Nippur region. The first millennium B.C. to first millennium A.D. reflects a relatively dry period with periodic increased rainfall. Sedimentary results suggest the Nahrawan, prior to it becoming a well-known canal, formed an ancient branch of the Tigris, while the region just south of Baghdad, around Dalmaj, was near or part of an ancient confluence of the Tigris and Euphrates.

Abstract Background Inverted nipples are prevalent in 3% to 10% of women and can often cause functional, psychological, and aesthetic problems. Objectives The authors sought to treat inverted nipples and restore the aesthetic appearance of the nipple while minimizing ductal disruption, preserving the ability to breast-feed, and reducing recurrence rates. Methods A retrospective review was performed on a total of 161 inverted nipples in 86 consecutive patients who underwent correction of inverted nipples at Oslo Plastic Surgery Clinic. Mean age at operation was 28.7 years and mean follow-up period was 14 months. A novel technique with central tunnelization of the retracted fibers/ducts was employed in 39 patients (45%); partial incision of the center of the inversion through a tunnel in 31 patients (36%); and total cut of the lactiferous ducts in 16 patients (19%). Fat grafting was utilized as support in 14 patients (26 nipples). Postoperatively, nipples were suspended for 4 weeks with a manually constructed device. Results Most patients had moderate (grade 2, 40 patients) or severe (grade 3, 52 patients) nipple inversion. Infection occurred in 4 patients and 2 patients had local irritation. Recurrence was seen in 32 patients (55 nipples) after the first operation, in 6 patients (6 nipples) after the second operation, and in 1 patient (1 nipple) after the third operation. Conclusions The authors propose a treatment algorithm that addresses important therapeutic goals when treating inverted nipples. Clinical examination is crucial to determine the method to be employed. The new central tunnel method and fat grafting to support the nipple are promising, although additional follow-up is necessary. Abstract:
Context.—Breast cancer treatment has greatly evolved from radical mastectomy to more cosmetically acceptable and less-debilitating surgeries. Nipple-sparing mastectomy is increasingly done for both cancer treatment and risk reduction. The frequency of terminal duct lobular units (TDLUs) and occult neoplastic epithelial proliferation in grossly/clinically unremarkable nipples (GUNs) is not well investigated. Objective.—To describe frequency of TDLUs and occult and overt neoplastic nipple involvement. Design.—Nipples from 105 consecutive specimens (90 therapeutic, 15 prophylactic) were studied. Sixty-five nipples were entirely submitted to evaluate frequency of TDLUs; the rest had 1 vertical section submitted. Results.—Terminal duct lobular unit was seen in 17 GUNs (26%). Six had TDLU in the base, 6 had it in the papilla, and 5 in both. Four GUNs showed lobular carcinoma in situ (1), Paget disease (1), and pagetoid extension of underlying malignancy (2). Grossly/clinically abnormal nipples had Paget disease (2), lymphovascular invasion (2), invasive carcinoma (4), and pagetoid extension (5). Involved nipples were closer to tumor (mean, 1.1 versus 3.2 cm, P < .001), had larger underlying tumors (mean, 4.3 versus 2.6 cm, P = .03) and of higher grade (P = .04), and more often had lymph node metastases (91% versus 44%, P = .007). No pathologic abnormalities were found in prophylactic mastectomy nipples. Conclusions.—Terminal duct lobular units were seen in 26% of nipples. They were frequently seen in the nipple papilla. Occult neoplastic epithelial proliferation was seen in 5% of grossly/clinically unremarkable therapeutic mastectomy nipples. Pagetoid extension was the dominant spread of underlying malignancy. Overall, the nipple was more often involved by larger and higher-grade tumors located closer to the nipple. All prophylactic mastectomies had unremarkable nipples. These findings should be considered while selecting patients for nipple-sparing mastectomy.

Abstract:
Purpose This study tested the milk flow rates and variability in flow of currently available nipples used for bottle-feeding infants who are hospitalized. Method Clinicians in 3 countries were surveyed regarding nipples available to them for feeding infants who are hospitalized. Twenty-nine nipple types were identified, and 10 nipples of each type were tested by measuring the amount of infant formula expressed in 1 min using a breast pump. Mean milk flow rate (mL/min) and coefficient of variation were used to compare nipples within brand and within category (i.e., Slow, Standard, Premature). Results Flow rates varied widely between nipples, ranging from 2.10 mL/min for the Enfamil Cross-Cut to 85.34 mL/min for the Dr. Brown's Y-Cut Standard Neck. Variability of flow rates among nipples of the same type ranged from a coefficient of variation of 0.05 for Dr. Brown's Level 1 Standard- and Wide-Neck to 0.42 for the Enfamil Cross-Cut. Mean coefficient of variation by brand ranged from 0.08 for Dr. Brown's to 0.36 for Bionix. Conclusions Milk flow is an easily manipulated variable that may contribute to the degree of physiologic instability experienced by infants who are medically fragile during oral feeding. This study provides clinicians with information to guide appropriate selection of bottle nipples for feeding infants who are hospitalized.

Abstract How enhancers are able to activate promoters located several kilobases away is unknown. Activation by the wing margin enhancer in the cut gene, located 85 kb from the promoter, requires several genes that participate in the Notch receptor pathway in the wing margin, including scalloped, vestigial, mastermind, Chip, and the Nipped locus. Here we show that Nipped mutations disrupt one or more of four essential complementation groups: l(2)41Ae, l(2)41Af, Nipped-A, and Nipped-B. Heterozygous Nipped mutations modify Notch mutant phenotypes in the wing margin and other tissues, and magnify the effects that mutations in the cis regulatory region of cut have on cut expression. Nipped-A and l(2)41Af mutations further diminish activation by a wing margin enhancer partly impaired by a small deletion. In contrast, Nipped-B mutations do not diminish activation by the impaired enhancer, but increase the inhibitory effect of a gypsy transposon insertion between the enhancer and promoter. Nipped-B mutations also magnify the effect of a gypsy insertion in the Ultrabithorax gene. Gypsy binds the Suppressor of Hairy-wing insulator protein [Su(Hw)] that blocks enhancer-promoter communication. Increased insulation by Su(Hw) in Nipped-B mutants suggests that Nipped-B products structurally facilitate enhancer-promoter communication. Compatible with this idea, Nipped-B protein is homologous to a family of chromosomal adherins with broad roles in sister chromatid cohesion, chromosome condensation, and DNA repair.

Taro corm is consumed in Haiti as a sustain against malnutrition, but no study was so far reported about Haitian taro. An ongoing breeding program, aimed at supporting corm production, includes the present investigations on taro. Eleven taro accessions sampled in 9 departments of Haiti were tested at Torbeck from January to August 2018. Seven variables were considered and treated with SPSS. Significant (p < 0.05) differences appeared for three of the seven considered traits. Corm marketable yield varied from 2.97 to 12.79 t/ha (p<0.05), the locations with the largest yield being Coteaux (South department, 12.79 t/ha), Petite- Riviére de Nippes (Nippes department, 7.56 t/ha) and Jacmel (South East department, 7.01 t/ha). Corm marketable yield was significantly (p < 0.001) and directly related with weight of corm per plant (ρ = 0.999), leaf length (ρ = 0.924) and leaf width (ρ = 0.795). Haitian taro cultivars were divided in three separate clusters. The largest differences concerned taro accessions from Grande Rivière du Nord and Coteaux. The results of this study show that accessions from Coteaux, Petite Riviére de Nippes and Jacmel should be recommended in the plant selection program to increase corm production in Haiti.Keywords: Taro, marketable yield, diversity, cultivars, Haiti.

Objective: To describe the consequences of using artificial nipples to exclusive breastfeeding. Materials and method: An integrative review carried out in the Medline®, Cinahl, Lilacs, Web of Science, and Scopus databases to answer the following question: What are the consequences of using artificial nipples to exclusive breastfeeding? Results: 38 articles were analyzed; the analysis gave rise to two categories: neonatal factors and maternal factors influencing exclusive breastfeeding. Breastfeeding interruption was the neonatal factor most associated with the use of artificial nipples, while the level of maternal education stood out as an intervening factor in exclusive breastfeeding. Pacifiers were the dummy type more commonly mentioned, followed by feeding bottles and nipple shields. Conclusions: The consequences of offering artificial nipples to breastfed infants are mostly negative and associated with the newborn, such as early weaning, refusal to breastfeed, impaired suction technique, incorrect latch-on, interference with orofacial development, and interruption of exclusive breastfeeding. Mothers who offered artificial nipples to their infants are more likely to experience pain, nipple fissures, frustration, and reduced interaction with their infants.

Nipple blisters can be caused by trauma to the nipple during breastfeeding, but it can also cause cracks and the formation of a gap - a gap. The incidence of sore nipples caused by several factors including improper feeding techniques and lack of breast care. This study aims to investigate the relationship between breastfeeding and breast care techniques with incidence of sore nipples. This research was conducted in June 2017 and BPS BPS Meiyuni Siti Hotijah Bangkalan. This research uses a checklist approach Sectional Cros. Subjects studied were maternal postpartum primipara 1-40 day total of 36 people. The sampling technique used was simple random sampling. Statistic test used lambda. The results showed breastfeeding technique in the category of pretty 16 respondents (44.4%) and treatment of breast category fairly 18 respondents (50%), while the incidence of sore nipples in the category were 18 respondents (50%). Statistical test results showed the value of breastfeeding technique p Value (0.019) <α (0.05) means that there is a correlation technique of breastfeeding on the incidence of sore nipples, while for the treatment of breast statistical result p value Value (0.047) <α (0.05 ) means that there is a relationship to the incidence rate of breast care sore nipples. This research is expected to provide knowledge to prevent the incidence of sore nipples in a mannerappropriate feeding techniques and treatment of breast good.

Nipped-B-like protein plays a pivotal role as a cohesin loading factor in the segregation of chromosomes when cells divide. Accumulating evidence indicates that alterations of this protein are involved in human carcinogenesis, especially in the regulation of chemotherapeutic drug response. However, the role of Nipped-B-like protein in esophageal squamous cell carcinoma remains unknown. In this study, we investigated the relevance of Nipped-B-like protein in the regulation of cisplatin sensitivity in esophageal squamous cell carcinoma. Ectopic expression of Nipped-B-like protein inhibited the growth of COLO-680N cells with low endogenous expression levels of Nipped-B-like protein, and increased sensitivity to cisplatin, a commonly used chemotherapy drug for patients with esophageal squamous cell carcinoma. In contrast, loss of Nipped-B-like protein stimulated the growth of EC9706 and Eca-109 cells with high levels of the protein, and resulted in resistance to cisplatin. P53-upregulated modulator of apoptosis, which is essential in the modulation of cisplatin sensitivity in a variety of cancers, acts as a downstream effector of Nipped-B-like protein. Restoration of this pro-apoptotic protein in Nipped-B-like protein-overexpressing esophageal squamous cell carcinoma cells effectively increased cisplatin sensitivity. Conversely, the silencing of P53-upregulated modulator of apoptosis in Nipped-B-like protein-depleted esophageal squamous cell carcinoma rendered cells resistant to cisplatin. Moreover, Nipped-B-like protein could bind directly to the promoter region of P53-upregulated modulator of apoptosis. In summary, our study addresses the involvement of Nipped-B-like protein in the development of esophageal squamous cell carcinoma, and the modulation of cisplatin sensitivity via regulation of P53-upregulated modulator of apoptosis.
