This paper investigates cose (pl. of cosa ‘thing’) as a general extender (GE) and marker of non-exhaustivity from Latin to contemporary Italian. The study employs three corpora: CODIT, LIP/VoLIP and KIParla. We show that the frequency of cose-GEs dropped in 16th c., when they started being perceived as colloquial. In Old Italian, cose-GEs already expressed non-exhaustivity in list constructions and, until late 17th c., were frequently specified by a nominal modifier, which however was uninformative to identify the category. Contemporary spoken Italian results confirm the role of spoken language in developing structures encoding non-exhaustivity, also in a dialogical sense. Moreover, recent data show an increase in frequencies of cose-GEs. Finally, we found more variability compared to the structure usually identified for GEs.

Adoption of consensus guidelines has led to better coping and faster recovery. This chapter reviews these initiatives and the problem of assessing fitness for work in those with spinal pain. Emphasis is given to simple, non-specific spinal pain as this is the commonest presentation. Only rarely does the clinician make a more specific diagnosis, but occasionally serious pathology underlies symptoms and different responses are needed. Therefore, this chapter also considers more specific spinal problems and interventions.

Non-specific low-back pain (LBP) is one of the commonest conditions afflicting adults of working age. It represents a leading cause of disability and a major cause of sickness absence. The problem posed in assessing fitness for work in back pain sufferers is one that all occupational physicians frequently face. Neck pain and its associated disability are scarcely less common. Collectively, therefore, axial pains affecting the spine pose a major challenge to the decision-maker. Commonly, a number of placement and fitness questions arise. In assessing the absent worker with a current episode of pain: When will symptoms improve or resolve? Is this a short- or a long-term problem? Are any further investigations required to exclude serious pathology? Who (among the many with pain) should be referred for such an assessment? At what point should the occupational physician intervene to hasten rehabilitation? And how? Has work contributed to symptom onset? Might it worsen or prolong symptoms? Is it appropriate to return the worker to the same job or does the work need to be modified? When is chronic spinal pain serious enough to declare a person permanently unfit for work? Could more be done to avoid or control the demands of work before that point is reached? Following spinal surgery, when will the patient be fit for work? Should special restrictions be considered and if so when? At the pre-employment stage the issues are no less difficult: Are there any specific inquiries (questions, examination findings, and investigations) predictive of future spinal pain leading to serious disability or sickness absence? How should these be utilized in assessing fitness for work? In particular, how should a past history of spinal pain be regarded? Are any characteristics sufficiently predictive to warrant restrictions? And more generally: What steps can be taken to promote fitness for work and to prevent spinal pain? What obligations exist under health and safety legislation and the Equality Act 2010? Do current policies on back pain promote well-being and avoid needless work restrictions? In attempting to answer these questions it is helpful to appreciate the frequency and natural history of spinal pain, the markers of serious pathology, and the evidence on fitness assessment and preventing disability. It is also important, for simple mechanical LBP, to be aware of evidence-based advances in management and rehabilitation. Adoption of consensus guidelines has led to better coping and faster recovery. Specific guidelines have also been developed for the management of workers and these address, in part, some of the questions posed above. In this chapter we review these initiatives and the problem of assessing fitness for work in those with spinal pain. Emphasis is given to simple non-specific axial spinal pain as this is the commonest presentation. Only rarely does the clinician make a more specific diagnosis; but occasionally serious pathology underlies symptoms and different responses are needed. Some account is provided of more specific spinal pathologies including prolapsed intervertebral disc, spinal stenosis, fusion surgery, ankylosing spondylitis, Scheuermann’s disease, fractures, and spinal cord injury.

Abstract Genetic risk factors and pathogenic mechanisms underlying neurodegenerative diseases are being identified at unprecedented resolution, but larger and more diverse studies are needed. Increasing representation of non-European ancestry populations in neurodegenerative illness studies will enhance understanding of disease etiology and ameliorate health disparities. The genetic architecture of the neurodegenerative disorders Alzheimer’s disease (AD), Parkinson’s disease (PD), and Lewy body dementia (LBD) is primarily polygenic or oligogenic. Genetic, neuropathological, and clinical evidence suggests these disorders represent a PD–LBD–AD spectrum. AD, PD, and LBD risk alleles mediate their effects via cell type–specific pathogenic mechanisms. AD genetics converge on microglial or macrophage efferocytosis. PD and LBD genetics implicate endolysosome dysfunction potentially in neurons. Large-scale studies integrating genetics, omics, and the environment in diverse global populations will elucidate neurodegenerative disease pathogenic mechanisms and ultimately yield therapeutic interventions for equitable distribution worldwide.

Photodynamic therapy (PDT) is one of the most innovative therapies currently available with no side effects. Initially introduced for the treatment of dermatological pathologies, it is now used in numerous medical fields, as well as in oral pathology. PDT has antimicrobial effects and can be used in several infections. The mechanism of action of PDT is based on the application of a dye called photosensitizer, which is activated by a precise wavelength. The photosensitizer is activated by specific wavelengths based on the chemical characteristics. Its activation involves the formation of reactive oxygen species that determine the death of the altered cells. Phototherapy is a therapeutic medical treatment based on the use of a special optical unit capable of emitting light, which can be used with or without UV radiation. Phototherapy acts as a natural and non-invasive therapy, capable of transferring energy to the cells of the organism, promoting regenerative capabilities. Ugliness in this chapter is recognizable as the visual aesthetics of underground comix and, as conceptual, nihilistic, and disempowering perspectives outlined as the mechanisms that restrict opting out of stigmatizing political and representational logics. Focusing on the depictions of such restrictions, this chapter explores the limits of allowable and expressible queer identities in the late twentieth century. Examining what affective states such as queer anger in Roberta Gregory’s Bitchy Butch (1991–1999) and ethnic anxiety in Erika Lopez’s Lap Dancing for Mommy (1997) look like, this chapter reads visual discrepancies and identificatory impossibilities as specific issues of depicting nonnormatively gendered and non-white bodies.

In order to understand the effect of particle breakage on the mechanical behaviour of railway ballast, a series of monotonically loaded drained triaxial tests was conducted using the large-scale cylindrical triaxial apparatus. The critical state behaviour of the ballast was investigated. A non-linear critical state envelope is determined in the q-p' and v-lnp' planes. Mathematical expressions for critical state stress ratio and specific volume are proposed to incorporate the evolution of particle breakage during monotonic shearing. An elasto-plastic constitutive model based on the critical state soil mechanics framework is presented capturing the stress-strain behaviour and degradation of ballast. The constitutive model is able to predict the monotonic shear behaviour of ballast observed in the laboratory tests. The model is validated using experimental results available from past independent studies.

Autoimmune diseases described herein include systemic lupus erythematosus (SLE), dermatomyositis/polymyositis (DM/PM), Sjögren syndrome (SS), and mixed connective tissue disease (MCTD). SLE predominantly affects women of reproductive age. Although pleural involvement is the most common thoracic manifestation, other manifestations include pneumonia, diffuse alveolar hemorrhage and lupus pneumonitis. Interstitial lung disease in patients with SLE include non-specific interstitial pneumonia (NSIP) and usual interstitial pneumonia (UIP). DM/PM affects the skeletal muscle and may frequently result in hypoventilation and respiratory failure (respiratory muscle involvement) and aspiration (laryngeal involvement). Interstitial lung disease is also frequent, and NSIP and organizing pneumonia are the most common patterns. SS typically affects women in the 4th to 5th decades of life. Classic symptoms include xerophtalmia and xerostomia. Interstitial lung disease is among the most common thoracic manifestations; and although NSIP, UIP, organizing pneumonia and amyloidoisis can occur, lymphocytic interstitial pneumonia (LIP) is a characteristic form of interstitial lung disease in SS. MCTD combines clinical features of RS, SLE, PSS and PM/DM. Thoracic involvement typically manifests with pulmonary hypertension and interstitial lung disease (NSIP, UIP and LIP). Pulmonary hypertension can occur in any autoimmune disease and is often associated with a worse prognosis. Chest radiography and thin-section chest CT (or HRCT) are the imaging modalities of choice to detect and assess thoracic manifestations of autoimmune disease.
