Full textA la naissance, le controle de la respiration est marque par une grande instabilite qui peut contribuer a l'augmentation de la frequence des apnees, aux malaises graves et a la mort subite du nourrisson. Le syndrome d'hypoventilation alveolaire centrale congenitale (shacc) est la mieux caracterisee de ces anomalies et pourrait etre du a un defaut de developpement de la crete neurale. Notre hypothese est que mash-1, un facteur de transcription participant a la determination neuronale des cellules de la crete neurale, est implique dans le controle respiratoire neonatal. Nous avons developpe des methodes d'exploration non-invasives du controle respiratoire par plethysmographie corps-entier chez le souriceau nouveau-ne, et montre d'importantes modifications de ce controle durant les deux premiers jours de vie. Celles-ci concernent la ventilation de base, la reponse hyperpneique a l'hypercapnie, la reponse biphasique et la reaction d'eveil a l'hypoxie. Nous avons applique ces methodes d'exploration a l'etude du phenotype respiratoire du souriceau transgenique mash-1. Les souriceaux mash-1-/- presentent un faible score d'adaptation a la vie extra-uterine, des anomalies de la frequence respiratoire, et meurent rapidement. Les mash-1+/- survivent mais presentent les memes anomalies respiratoires. La perte de fonction de mash-1 perturbe l'expression du proto-oncogene ret dans des structures noradrenergiques du tronc cerebral qui modulent la frequence respiratoire (noyaux a5 et a6), ainsi que dans des cellules de la partie ventrolaterale du bulbe (hybridation in situ). Le niveau intermediaire d'expression de ret dans le tronc cerebral des mash-1+/- (pcr quantitative) peut expliquer leur phenotype attenue. Ces resultats montrent que mash-1 est implique dans le developpement du controle respiratoire au travers de l'expression de ret dans les structures noradrenergiques du tronc cerebral. Ces donnees qualifient mash-1 comme un des genes candidats du shacc

Full textOur work has highlighted a virulence plasmid pS88 carrying a conserved region (CVP) not only involved in the virulence of strain S88 neonatal meningitis belonging to subgroup highly virulent B21 but also in the virulence of a resistant strain of meningitis (S286) belonging to phylogenetic group A/B1 associated with commensal strains. Epidemiological analysis of a collection of strains A/B1 showed that CVP could be a major genetic determinant of ExPEC strains A/B1. On the other hand, our work on strains of neonatal spesis E. Coli showed the potential involvement of CVP in early-onset infections. Functional analysis of CVP should identify new virulence factors contributing to a better understanding of the pathophysiology of E. Coli neonatal infections and the development of new therapeutic or prophylactic strategy

Full textG protein-coupled receptors (GPCR) are involved in different physiologic processes. The 5-HT4 receptor (R) is a member of Gs PCR family. The role of anti-5-HT4 R autoantibodies (autoAb) in neonatal lupus (NL) symptoms especially the congenital heart block (CHB) has been demonstrated by Eftekhari et al. These results have been contradicted by Buyon et al. We have clarified that this discrepancy was due to technical differences in ELISA test. 16 % of mother's sera whose children have CHB showed reactivity against the second extracellular loop (SEL) of the 5-HT4 R (G21V peptide). Though 5-HT4 R autoAb are present in a minor subset of mothers whose children have CHB, they represent an additional risk factor which may contribute to the pathogenesis of disease. In order to understand the role of 5-HT4 R in NL, we have raised rabbit polyconal Ab against 2 isoforms of the receptor to study their expression during mouse embryonic development by immunofluorescence. The receptor as well as its mRNA has been identified during early development stages. BALB/c females have been immunised with G21V peptide and mated. Abnormalities have been observed among embryos starting from the 12th gestation day. These results suggest the involvement of 5-HT4 R in NL when the presence of anti-R Ab coincides with receptor expression. To study pathogenic paratope structure, we have isolated a monoclonal Ab directed against G21V peptide. The Ab recognised 5-HT4 R by immunoblotting and immunocytofluorescence. Surface plasmon resonance experiments showed that the Ab has pM affinity to G21V peptide. The Ab competitively inhibited [3H]GR113808 binding to 5-HT4 R. It showed an inverse agonist effect on the basal activity of CHO cells expressing 5-HT4 R and inhibited 5-HT effect. The Ab represents a high affinity modulator of 5-HT4 R

Full textIschemic strokes happening in the adulthood or during the perinatal period have important consequences in health care explaining the need to develop therapeutic strategy and the intensity of experimental and clinical studies performed in this field. Although experimental research has allowed the development of numerous therapeutic agents; they all failed to show beneficial effect in clinical trials. Answers need to be found to enable evolution in this area. Experimental design and most of all animal models are suspected to be the causes of the failure of the transposition from the bench to the bedside. The present work supports the fact that arterial hypertension should be integrated in preclinical studies of the physiopathology and treatment of stroke. Arterial hypertension worsens the resultant ischemic lesion and induces early disappearance of the ischemic penumbra, the target of any therapeutic strategy. In newborn, inhibition of caspase-2 was inefficient in severe hypoxic-ischemic lesion but should be more investigated in focal ischemia. Arterial hypertension during pregnancy provides neuroprotection of the immature brain subjected to hypoxic-ischemic lesion. This effect seems to be related to a preconditioning phenomenon caused by repetitive prenatal stress pregnancy hypertension induced

Full textIntra-uterin growth restriction (iugr) and postnatal overfeeding is an important cause of the metabolic syndrome and cardiovascular diseases in adulthood. However, the detailed mechanisms of such programming are still incompletely known. Two experimental approaches in the rat were used in this work of thesis: first, protein depletion during the gestation of rat females has been widely used as a model for human. A transcriptome analysis of control and protein-deprived rat placentas and kidneys was used to better understand the molecular basis of early origin of the diseases in adulthood. Our approach permitted the proposition of hypotheses on a hierarchy of gene inductions/repressions leading to massive transcriptional alterations in the iugr placenta and kidney. Moreover, postnatal growth via renal effects other than glomerular number reduction are likely to contribute to arterial hypertension induced by early postnatal overfeeding. Thus, the comprehension of the mechanisms imply in the early origin of cardiovascular and metabolic diseases in adulthood, currently opens news diagnostic ways by the identification of early markers of this risk and could later on ollow to develop therapeutic, pharmacological or nutritionalways

Full textThe weak immune systems in the elderly, newborns and preterm infants increase their susceptibility to cardiovascular disease and heart failure. Indeed, symptoms of cardiac pathology can be quite unusual and sporadic in this population. This requires continuous monitoring of cardiac activity by electrocardiogram (ECG), which enables early detection of abnormalities. Conventionally, ECG systems are wired, limiting continuous monitoring and patient comfort. Thanks to technology, portable and wireless ECG systems have also been developed, based mainly on efficient radio frequency (RF) technologies, which enable these users to be monitored regularly. However, security and privacy requirements, interference generation, potential harmful effects on patient health, spectrum congestion and energy efficiency are challenges for the massive deployment of RF technologies in healthcare scenarios. Instead of relying on the limitations of RF systems, this thesis focuses on an alternative solution based on optical wireless technologies that enable safe and environmentally-friendly communications. This solution consists of a monitoring system using an infrared link between a transmitter embedded in an ECG sensor placed on the user's body and receivers placed on the ceiling of the environment. In order to study the three monitoring contexts, the corresponding optical channel was simulated using a ray-tracing technique combined with the numerical Monte-Carlo method. Considering the specificities of the monitoring scenario, the population (age and mobility) and the modeled environment (transparent material of premature baby incubators), the static gain characterizing the channel was obtained. This gain was used in a chain developed to simulate ECG transmission in each context. The reliability of the proposed remote monitoring system is thus conditioned by the quality of the transmitted ECG signal. This has been assessed at application level using the Signal Quality Index (SQI) method, which consists in extracting spectral and statistical characteristics from the ECG. In addition, given its usefulness in monitoring the development of premature infants, heart rate variability (HRV) has also been analyzed by computing the temporal parameters that characterize RR intervals in an ECG. The main objective is to assess the impact of time-varying optical transmission performance on the quality of the received ECG signal, in order to design a less intrusive, reliable and cost-effective system for remote ECG signal monitoring. SQIs consist in extracting spectral and statistical characteristics from the ECG. In addition, given its usefulness in monitoring the development of premature infants, heart rate variability (HRV) was also analyzed by computing the temporal parameters that characterize RR intervals. The main objective is to assess the impact of optical transmission performance on ECG quality metrics. For this purpose, a general analysis approach was implemented to jointly study the evolution of (SQIs) and VFC temporal parameters at the application level as a function of classical metrics at the physical level, namely (SNR) and (BER). The results of this research showed the potential of using wireless optics in such medical contexts to provide a reliable and energy-efficient solution. Indeed, in all three cases investigated, ECG signals of sufficient quality for reliable diagnoses were obtained at moderate levels of transmitted optical power, which is important for a portable medical remote monitoring system
