rThis case focuses on the numerical pain rating scale by asking the question: What degree of change on a 0-to-10 pain intensity numerical rating scale (PI-NRS) used for the assessment of chronic pain is associated with clinically important improvement? Patient data were taken from 10 completed studies of pregabalin treatment for chronic pain in diabetic neuropathy, postherpetic neuralgia, low back pain, fibromyalgia, and osteoarthritis. All 10 of the double-blind, placebo-controlled, parallel, multicenter chronic pain studies used a common study design with identical pain measures. The study findings suggest that a PI-NRS score decrease of 1.74 and a percentage change of 27.9% correlate with a clinically important improvement in pain symptoms.

Following surgery, pediatric patients suffering from pain warrant consistent and frequent pain assessments. This chapter describes a variety of scales that can be used and that must be tailored to the developmental stage of the child. Examples of commonly used pediatric scales include the Neonatal Pain, Agitation and Sedation Scale (N-PASS); Face, Legs, Activity, Cry, Consolability (FLACC); Numerical Rating Scale (NRS); and the Wong Baker FACES Pain Scale. In the setting of open abdominal surgery, certain interventional techniques (continuous epidural, peripheral truncal blockade) can be used to significantly decrease pain and must be tailored to the individual patient and specific surgical subtype. Separate from interventional techniques, older children may also use patient-controlled analgesia (PCA) devices to deliver systemic opioids. For patients who are too young or are unable to press a PCA device, a variety of nurse-controlled analgesic protocols exist to deliver such systemic analgesic therapies.

This chapter details the integration of a Numerical Rating Scale (NPRSETI) with a portable eye tracker, enabling the assessment of pain in conditions in which verbal communication and use of hands are limited (e.g., advanced Amyotrophic lateral sclerosis, ALS). After detailing the construction of the NPRSETI, we describe its validation in an outpatient pain clinic. More specifically, thirty chronic pain patients performed the NPRSETI and filled a conventional NPRS (order was pseudo-randomized). Eye movements, including gaze direction and additional eye movement measures (e.g., saccade rate), were recorded, while participants rated their pain using the NPRSETI. The study’s findings indicated no significant differences in pain severity ratings of the NPRSETI and conventional NPRS. Notably, ratings using the two scales were highly correlated (r = 0.99). NPRSETI’s ratings were also strongly associated with participants’ currently experienced pain rating using the Brief Pain Inventory (BPI). The findings provide initial proof of concept for integrating conventional pain rating scales with an eye tracker and validate the NPRSETI compared with the well-validated and commonly used NPRS. Enhanced usability and decreasing costs of eye trackers will ease the additional research mandated to validate these preliminary findings and hopefully advance their integration into clinical practice.

Pain is described as being whatever the experiencing person says it is, and their perception of pain is determined by their mood and morale and the meaning of the pain for them. Cancer pain is common, and prevalence is related to the stage of the illness: 59% in patients undergoing treatment, and 64% in those with advanced disease. Pain is multi-causal and multidimensional, and a holistic, interdisciplinary approach to assessment is necessary, encompassing physical, psychological, social, and spiritual aspects of pain (together are ‘total pain’). A narrative approach to pain assessment is helpful, keeping the focus on the patient and their experience. Pain assessment tools can also be used to identify where pain is and how much it hurts. These include: pain body map, visual analogue scales (VAS), verbal rating scales (VRS), numerical rating scales (NRS), and Brief Pain Inventory (BPI). Classifications of pain include acute and chronic pain, nociceptive pain caused by the stimulation of nerve endings, and neuropathic pain caused by nerve dysfunction or compression. Analgesic drugs may be given according to the World Health Organization's pain relief ladder: step 1, non-opioid analgesics such as paracetamol and non-steroidal anti-inflammatory drugs; step 2, mild opioids like codeine with or without non-opioid analgesics; and step 3, strong opioids like morphine with or without non-opioid analgesics. Non-pharmacological interventions for pain management include transcutaneous electrical nerve stimulation (TENS), massage, distraction, relaxation, breathing exercises, comfort measures, and presence of the nurse. Effective communication at all stages of management is essential.

We developed a method of using the Clinically Aligned Pain Assessment (CAPA) measures to reconstruct the Numeric Rating System (NRS). We used an observational retrospective cohort study design with prospective validation using de-identified adult patient data derived from a major health system. Data between 2011-2017 were used for development and 2018-2020 for validation. All included patients had at least one NRS and CAPA measurement at the same time. An ordinal regression model was built with CAPA components to predict NRS scores. We identified 6,414 and 3,543 simultaneous NRS-CAPA pairs in the development and validation dataset, respectively. All CAPA components were significantly related to NRS, with RMSE of 1.938 and Somers’ D of 0.803 on the development dataset, and RMSE of 2.1 and Somers’ D of 0.74 when prospectively validated. Our model was capable of accurately reconstructing NRS based on CAPA and was exact when the NRS was [0,7].

Group B: Ultrasound treatment (Duration -7 min. intensity -0.8 W/cm 2 , intermittent mode) and routine physiotherapy treatment as – Resting in sling, ice application locally, and exercise. Data analysis: Data was analyzed using Numerical Pain Rating scale (NPRS) and ratio of distribution of Frozen shoulder recovery was taken out. Result: Mulligan’s Mobilization + Mulligan’s self exercise technique along with ultrasound treatment was equally effective as regular physiotherapy in the treatment of patients with frozen shoulder. But the pain status in case of Group A is less as compared to Group B. The percentage of male taking treatment in Group A is more than Group B &amp; Vice versa in case of female. Also the result of Oxford shoulder score showed a significant improvement in ADLs. Also according to goniometric results the range of motion of shoulder Flexion, Abduction &amp; rotation has been improved significantly. Abstract:
Background: Worldwide, osteoarthritis predicted to become the fourth leading root cause of disability. The chronic pain associated with the disease consequently results in a disabling state. The prevalence of mental health problems among pre-elderly age group is approaching closer to the level reported in elderly population. Hence the study aimed to determine the relationship of knee osteoarthritis associated joint pain with mental health impairments i.e. anxiety and depression in middle aged knee osteoarthritis population. Methods: To fulfil the purpose of the prospective, observational study, a sample of hundred participants meeting the eligibility criteria were included in the study. After giving the written informed consent, the individuals were asked to autonomously report their Numeric Pain Rating Scale (NPRS) score and Hospital Anxiety and Depression Scale score to the concerned investigators. Results and Conclusion: The mean (SD) of knee pain, anxiety and depression scores were 6.86 (1.042), 14.52 (3.450) and 14.51 (3.166) respectively. The correlation between knee pain and anxiety was 0.216 whereas the correlation between knee pain and depression was -0.036. The study revealed a weak positive correlation between knee pain and anxiety and weak negative correlation between knee pain and depression. During the management of knee osteoarthritis population, special attention should be made to address anxiety issues and the treatment of disease related joint pain should always include relaxation techniques to impart an effective management of the pain.
