ABSTRACT Objective: To present an analysis of the nursing ethos based on the disciplinary foundations of nursing to propose a definition of ethical competences for nursing training. Method: The present proposal is a theoretical reflection, based on a literature review of the specialty, both nursing and ethics. Results: we suggest basing the ethical education of nursing students integrating transversally disciplinary, bioethical and civic ethical competences, considering certain educational dimensions that favor their development. Final considerations: We expect that the definitions of three competences of the moral scope of nursing, contribute to cross-cutting proposals that promote the values and principles of the profession.

ABSTRACT Objective: to investigate nursing professionals' understanding concerning the Code of Ethics; to assess the relevance of the Code of Ethics of the nursing profession and its use in practice; to identify how problem-solving is performed when facing ethical dilemmas in professional practice. Method: exploratory descriptive study, conducted with 34 (thirty-four) nursing professionals from a teaching hospital in João Pessoa, PB - Brazil. Results: four thematic categories emerged: conception of professional ethics in nursing practice; interpretations of ethics in the practice of care; use of the Code of Ethics in the professional practice; strategies for solving ethical issues in the professional practice. Final considerations: some of the nursing professionals comprehend the meaning coherently; others have a limited comprehension, based on jargon. Therefore, a deeper understanding of the text contained in this code is necessary so that it can be applied into practice, aiming to provide a quality care that is, above all, ethical and legal.

ABSTRACT Objective: to evaluate the association of Obstetric Nursing in the best practices of delivery and birth care in maternity hospitals. Method: a cross-sectional study, with 666 women selected for delivery. Parturition obstetric practices performed by professionals were categorized into: clearly useful practices that should be encouraged, practices that are clearly harmful or ineffective and that should be eliminated and practices used inappropriately at the time of parturition. Results: clearly useful practices were used in greater proportions in the hospitals that had Obstetric Nursing working, while clearly harmful practices and those used inappropriately were practiced in smaller proportions in hospitals that had Obstetric Nursing, both with statistical difference. Conclusion: institutions with Obstetric Nursing adopt better practices of delivery and birth care, based on scientific evidence, when compared to those that do not act.

Abstract:
Abstract Introduction Insomnia contributes to fatigue, a common symptom in COPD. Our study aims were: (1) to determine the efficacy of a) cognitive behavioral therapy for insomnia (CBT-I) and b) COPD education (COPD-ED) on insomnia and fatigue, and (2) to define potential mechanistic contributors to pre/post intervention change in insomnia and fatigue in patients with COPD and insomnia. Methods A randomized 2x2 factorial design was used with factors representing CBT-I (yes/no) and COPD-ED (yes/no). Attention control (health videos) were used in the absence of CBT-I or COPD-ED. All patients received 6, 75-minute weekly sessions. Dependent variables included insomnia severity (Sleep Impairment Index (SII), range 0-28) and fatigue (Chronic Respiratory Disease Questionnaire (CRQ) range 1-7) measured at baseline, just post-intervention, and at 3-months post-intervention. Results One hundred nine patients (FEV1% predicted 67 ± 24% (mean ± SD), age 65 ± 8 years, SII 15.9 ± 8, CRQ 3.7 ± 1.1) participated in the study. After 6 sessions, insomnia decreased more in patients who received CBT-I (-5.8) than those who did not (-2.2; p=0.0002). This effect was sustained at the 3-month follow-up (p=0.0003). Fatigue showed no significant differences for CBT-I at 6-weeks (p=.27) but at 3-months patients receiving CBT-I showed marginally better improvement (.75, a clinically important difference) compared to those who did not receive CBT-I (.43; p=.09). COPD-ED showed no effect on insomnia or fatigue. Two main effects suggest mechanisms for the pre-post efficacy of CBT-I: improved sleep beliefs (p=0.0257) and self-efficacy for sleep (p=0.0619) after 6 sessions which were sustained at 3 months (p=0.0184 and p=0.0431 respectively). Conclusion CBT-I produced sustained decreases in insomnia in patients with COPD. Results suggest that changes in beliefs about sleep and improved self-efficacy for managing sleep may mediate CBT-I associated decreases in insomnia. ABSTRACT Objective: to analyze the self-reported knowledge of professors in the use of feedback in the teaching of nursing practice; train professors to use this process; and evaluate the results of this training. Method: a qualitative study of research-action type. The focus group technique was used with seven collaborators of a nursing technical school. For content analysis, the Bardin framework was used and, for intervention, the Problematization Methodology supported by Pendleton’s Rules. Results: most present in their curricula training to teach. It is pointed out that feedback is a simple and essential assessment tool, although universal discourse does not express the exact dimension of feedback potentialities. The intervention was effective to solidify feedback. Final Consideration: this study shows that feedback should be disseminated and consolidated among the teaching staff of nursing technical education.

ABSTRACTObjective: to get to know the meaning of the care provided by nursing staff in the vision of hospitalized elderly patients at the clinic of a public hospital. Methodology: qualitative research, conducted from January to March of 2009 with ten elders of both sexes hospitalized in the medical clinic at a public hospital in the city of Feira de Santana, Bahia. The data collection instrument was a semi-structured recorded interview. Data analysis was made by the method of thematic content analysis. The study was approved by the Ethical Committee of the Faculdade de Tecnologia e Ciências, resolution no. 0589-2008. Results: two themes emerged from the testimonies: the meaning of care in the vision of elderly and the positive and negative aspects of care received by nursing staff. The elderly have a positive view of care received by nursing staff relating to dedication, love and humanization. Regarding the negative aspects of care received, they relate: lack of communication and information about their health state. Conclusion: because it is a fragile stage of life, elderly people require special attention, sensitive care by the nursing staff. Resultados: dos depoimentos emergiram duas categorias temáticas: o significado do cuidado na visão dos idosos e os aspectos positivos e negativos do cuidado recebido pela equipe de enfermagem. ABSTRACTObjective: to identify the possible factors involved in the establishment of an ecological practice by the nursing staff of a public hospital in the town of Santa Cruz, Rio Grande do Norte, Brazil. Methodology: this is a descriptive exploratory research with a qualitative approach carried out with seventeen nursing professionals nurses who work at a public hospital. Data were obtained through semi-structured interviews, within the period from March to April 2010, and analyzed according to Bardin’s content analysis method. The project was approved by the Research Ethics Committee of Universidade do Estado do Rio Grande do Norte (UERN), under the Protocol 059/2009. Results: the content analysis revealed three categories: lack of support at work and its implications for the establishment of an ecological practice by nursing staff; influence of the nursing professional’s excessive workload on the ecological practice; and lack of continuity of work of the nursing professional and its influence on ecological attitudes. Conclusion: some situations in the working conditions of the nursing staff constitute elements that turn the establishment of ecological attitudes more difficult in this category, discouraging it to promote sustainability and environmental preservation initiatives. ABSTRACT Objective: to report the nurses' experience in relation to the training of caregivers of infants with Isolated Robin Sequence (IRS) for maintaining care after hospital discharge from the perspective of Self-Care Theoretical Framework. Method: the following categories were considered in this experience report: self-care action, self-care capacity, therapeutic self-care demand, self-care deficit, and nursing system. The nursing system was wholly compensatory and supportive-educative. Results: caregivers' training by nurses results in the acquisition of technical skills and specific knowledge related to the infant's positioning in elevated ventral decubitus, nasopharyngeal intubation, feeding-facilitating techniques and care with the feeding tube. Final considerations: the continuity of home care is guaranteed from caregivers' training for the therapeutic demand.

Abstract:
60 There is little published evidence for protection of white matter with neuroprotective drugs in animal models of stroke, yet white matter protection may be important in achieving clinical efficacy. We have examined the effects of NXY-059, a nitrone-based free radical trapping agent, on long-term functional disability in a primate model of stroke. We also examined histopathological effects, including analyses of grey and white matter damage. Five minutes after unilateral permanent middle cerebral artery occlusion, marmosets received a 1 ml i.v. infusion of saline (n=5) or NXY-059 (28 mg/kg) (n=6) and osmotic minipumps (model 2001D) were implanted s.c. to provide continuous drug or saline infusion for 48 h. Drug-filled pumps released NXY-059 at a rate of approximately 16 mg/kg/h. The plasma unbound drug concentration at 24 h was 76.3 ± 5.7 μM, a level well tolerated in acute stroke patients. The monkeys had been trained and tested on a variety of behavioral tasks before surgery. NXY-059-treated monkeys were significantly better at reaching with their contralesional arm than were saline-treated monkeys when re-tested 3 (p<0.01) and 10 weeks (p<0.01) after surgery. NXY-059-treatment also significantly reduced spatial neglect measured 3 weeks after surgery (p<0.01) compared with the saline group. After behavioral testing was complete, histopathological analysis showed NXY-059-treated monkeys had significantly smaller infarcts than saline-treated monkeys (F (1, 10)=5.21, p<0.05). NXY-059 reduced overall infarct size by 51%, damage to the cortex was reduced by 54%, white matter by 52%, caudate by 49%, and putamen by 33% compared with saline-treated monkeys. In conclusion, NXY-059 substantially lessened the functional disability in these monkeys. This drug protects not only cortical tissue, but also white matter and subcortical structures against ischemic damage. Together these findings bode well for advancing this drug to further clinical trials for its use in acute stroke.

ABSTRACT Objective: To investigate the Nursing diagnostic accuracy measures and to propose a model to use defining characteristics in order to judge the nursing diagnosis of ineffective breastfeeding. Method: Cross-sectional study with a sample of 73 binomials mom-child hospitalized in a maternity ward of an University Hospital, from July to August of 2014. Results: The diagnostic predominance rate was 58.9%. The characteristics that best meet the needs of logistic regression model were: discontinuance of breast sucking; infant's inability of seizing the areola-nipple region correctly; infant's crying one hour after breastfeeding and inappropriate milk supply perceived. Conclusion: Breastfeeding process is dynamic; diagnostic judgement may suffer some changes according to the time data are collected; the defining characteristics are the best predictors if associated with models and rules of use.

ABSTRACT Objective: To evaluate the accuracy of the defining characteristics of the nursing diagnosis for “fatigue in women with breast cancer under radiotherapy”. Method: Study of diagnostic accuracy, with cross-sectional design, performed in 130 women with breast cancer under radiation treatment. A data collection instrument was used to evaluate clinical socio-demographics and to investigate the presence or absence of defining characteristics for fatigue. The latent class analysis model was applied to assess accuracy measurements of the characteristics identified. Results: Fatigue diagnosis was present in 21.9% of the women. The characteristic which showed the highest sensitivity was “Impaired capacity in maintaining the usual level of physical activity”, while “Impaired capacity in maintaining usual routines” and “Lack of interest about the surrounding environment” presented greater specificity. Conclusion: Accurate diagnostics allow devising an action plan directed to the patients' real needs.

ABSTRACT Introduction: the Journal Club (JC) is a teaching and learning strategy developed by individuals who meet to discuss scientific articles in periodicals. Objective: to describe the experience of the JC strategy at the Group for Studies and Research in Health Services Administration and Nursing Management (Gepag). Method: case studies or scientific research demonstration mode of practical experience for the understanding and justification of facts. Results: Gepag JC emerged in 2008 and, in 2014, was computerized with the Google Drive®, in order to increase its scope and optimize the Group›s meetings. From April to May 2014, the instrument was tested and adjusted, resulting in advancements. Final considerations: the advantages involved optimizing the time of meetings, facilitation of access to publications of interest to the Group and creating the database to support future research.

ABSTRACT Objective: To characterize the socioeconomic and clinical profile of adult cancer patients in palliative therapy. Method: Cross-sectional study in an oncology hospital in Paraná, with 124 adult patients who started palliative therapy in the period from Jan. 2 to June 30, 2015. Results: Of the participating population, 60.5% were women, 68.5% white, 48.4% married, 72.6% catholic and with income of one to two minimum wages. Non-smokers, 45.2%, non-alcoholics 75%, and 92% had Performance Status 1 and 2. The predominant primary diagnosis was breast cancer, with previous chemotherapy and radiotherapy. The sites of metastasis were lung/mediastinum/bronchi and lymph nodes. Conclusion: The socioeconomic and clinical context characterized the profile of adult patients in palliative therapy. The demand arising from the increase in cases of advanced cancer requires nursing care at all stages of treatment.

ABSTRACT Objective: To describe the stages in the cultural adaptation of "The End-Stage Renal Disease Adherence Questionnaire" for use in Brazil. Method: A descriptive, transversal study with a quantitative approach. The cultural adaptation followed the steps of translation, professional committee, back translation, panel of patients and pretest. Results: the translation stage created a version in Portuguese that was analyzed by specialists, who suggested alterations in the title, seven sentences in the questions and three sets of answers. In the panel of patients phase, two questions were altered. The back translation considered the original characteristics of the instrument were maintained, likewise there were no modifications in the pretest phase. The third version written in Portuguese was considered to be the final version of the questionnaire. Conclusion: this instrument should facilitate nursing care management in hemodialysis, enabling the monitoring of adherence among these patients and also contribute to improving the indicators of morbidity and mortality.

SummaryReasons for trends in venous thromboembolism (VTE) incidence are uncertain. It was our objective to determine VTE incidence trends and risk factor prevalence, and estimate population-attributable risk (PAR) trends for each risk factor. In a population-based cohort study of all residents of Olmsted County, Minnesota from 1981–2010, annual incidence rates were calculated using incident VTE cases as the numerator and age- and sex-specific Olmsted County population estimates as the denominator. Poisson regression models were used to assess the relationship of crude incidence rates to year of diagnosis, age at diagnosis, and sex. Trends in annual prevalence of major VTE risk factors were estimated using linear regression. Poisson regression with time-dependent risk factors (person-years approach) was used to model the entire population of Olmsted County and derive the PAR. The age- and sex-adjusted annual VTE incidence, 1981–2010, did not change significantly. Over the time period, 1988–2010, the prevalence of obesity, surgery, active cancer and leg paresis increased. Patient age, hospitalisation, surgery, cancer, trauma, leg paresis and nursing home confinement jointly accounted for 79 % of incident VTE; obesity accounted for 33 % of incident idiopathic VTE. The increasing prevalence of obesity, cancer and surgery accounted in part for the persistent VTE incidence. The PAR of active cancer and surgery, 1981–2010, significantly increased. In conclusion, almost 80 % of incident VTE events are attributable to known major VTE risk factors and one-third of incident idiopathic VTE events are attributable to obesity. Increasing surgery PAR suggests that concurrent efforts to prevent VTE may have been insufficient.Supplementary Material to this article is available online at www.thrombosis-online.com.

Objectives: The study attempts to describethe level of perceived workplace security among the nurses and investigate relationshipbetween theoretically identified predictors and the perceived workplace security of nursesworking in public hospitals in Lahore city. Study Design: Factors such as civility in co-workerbehavior, clarity of norms of communication with co-workers, integrity and trust of the leaders,awareness of law and legal procedure related to harassment at work place and physicalsettings characteristics—including transparency and privacy at the workplace—were modeledthrough a cross-sectional research design as contributing towards workplace security of femalenurses. Study Period: Oct 2015 to May, 2016. Methods: Multi-stage probability sampling wasused to collect data from 317 respondents working in five public sector hospitals in Lahorecity during first quarter of 2016. Zero-order correlation and multiple regression methods wereused to analyze the data. Findings: Results showed that civility (B=.071, p<.000) in co-workerbehavior, integrity (B=.185, p<.000) and trust of leaders (B=.059, p<.000) is the most potentfactor influencing perceived workplace security of female nurses. Moreover, clear norms ofcommunication (B=.169, p<.000) and privacy (B=.133, p<.000) at workplace also significantlyinfluence their perceived workplace security. Transparency (B=.017, p>.05) in physical settingsand awareness (B=-.014, p>.05)of law and legal procedures were found to be insignificant.Conclusion: It was concluded that setting clear work norms and leadership development inthe areas of integrity and trust can play a substantial role in improving perception of workplacesecurity in female nursing staff. Awareness of laws regarding harassment among nurses needto be increased through formal and concerted effort.
