The world population is becoming increasingly ethnically diverse, reflecting patterns of global migration. Nursing care requires consideration of nuances of culture to respond sensitively to diversity needs in contemporary demography. The opportunity to nurse within multi-cultural communities makes nursing an interesting and rewarding occupation. Responding sensitively to people from different cultures can be enriching and fulfilling, much appreciated by recipients of care. Sadly, for some black and minority ethnic groups, the experience can be very different. Prejudice and discrimination seen in the wider society can extend into healthcare settings. Nursing staff can feel under-prepared to respond in culturally aware or non-discriminatory ways without adequate training. The concept of ‘otherness’ is a central strand of healthcare debate about individuals who exist outside the boundaries of mainstream society, groups of people who either fail to use health services or are failed by health services. This includes people from different ethnic and cultural backgrounds, travellers, refugees, and asylum seekers. Marginalized, and easily identifiable, people are too often viewed as an aggregate through the prejudicial lens of stigma and stereotype(s). This chapter suggests that the nursing profession is well placed to challenge discrimination and health inequalities as a part of everyday practice.

This chapter focuses on the preparation and care of adult patients undergoing elective surgery, the associated challenges, and supporting evidence in providing safe and effective care. A key principle is the identification of relevant health issues and optimizing comorbidities prior to admission for surgery, which will minimize cancellations on the day of surgery. Informed consent, patient education, and teamworking all contribute to effective care and efficient service delivery. To provide the optimum healthcare, it is essential that the health professional has a full understanding of the patient’s physical and psychological health and social history, allowing tailored care to be shaped and implemented. It is important that the patient understands the associated risks and benefits of planned treatment. Patients often present for elective surgery with comorbidities. In optimizing the treatment, it is possible to prevent negative consequences related to planned care, and to increase the patient’s understanding of these so that he or she they can make an informed choice. Gathering information prior to admission is important because patients are often anxious on the day of surgery, and medication/anaesthetic agents can render them unable to provide clear decisions relating to treatment. This is commonly referred to as the preoperative assessment (POA), but is probably better referred to as patient preparation. Patient preparation is the process by which a patient’s health status is identified and comorbidities made known to the relevant healthcare professionals. The healthcare professional will interpret the information, decide on additional investigations and examinations, and then determine the risk factors associated with the patient’s health and the anticipated anaesthetic and surgical intervention. The patient must be informed of the risk and benefits and be provided with sufficient information to ensure an informed choice. Integral to patient preparation is the anticipation of potential outcomes, including length of hospitalization, ability to complete activities of daily living, and discharge planning. The process will involve the patient and his or her carer(s) and all healthcare professionals appropriate to the individual patient in primary and secondary care. It may be that, at the end of the patient preparation stage, the patient decides not to proceed with surgery.

The key to succeeding in this OSCE is understanding the need for systematic assessment, and timely intervention and escalation will be stressed throughout this chapter. It is likely that you will be allowed approximately half an hour to demonstrate this skill and answer related questions. Revision of key material will enable you to understand why assessment is important and provide you with a systematic framework to use in the OSCE and in clinical practice. Concern over NHS staff ’s management of the deteriorating patient has been highlighted in the literature since the late 1990s. McQuillan et al. (1998) first discussed the concept of suboptimal care suggesting that often deterioration in patients was ignored, misdiagnosed and/or poorly managed in ward environments resulting in increased mortality and morbidity in ward patients. Since McQuillan’s work several other studies have identified similar problems (McGloin et al. 1999; NCEPOD 2005). More recently NICE (2007) has published guidance on recognition and management of deterioration and the Department of Health (2009) has published competencies related to recognition and management of deterioration that all acute staff should achieve. Review of this literature highlights that several issues are clearly important in recognition of acute deterioration and the need to utilize a systematic assessment tool linked to a robust track and trigger scoring system is an important consideration in practice. Smith (2003) was instrumental in developing the ALERT® framework that has been adopted internationally as a robust systematic assessment tool.

The biomedical ethical principles of autonomy, beneficence, non-maleficence, and justice are well established, though they have been challenged by feminist and nursing ethics. Decision-making in practice requires a balance of not only ethical principles, but also legal and professional frameworks, alongside patient and family wishes. Cancer clinical trials raise ethical issues around the balance between risk and potential benefits to patients, and they may need support making the right decision about whether to participate. The rising cost of cancer drug treatments has raised difficult questions about which drugs should be authorized for use within the United Kingdom (UK)'s National Health Service. End-of-life care raises particularly challenging ethical issues. Mental capacity or competence is defined in law in the UK, and treatment decisions may be made on behalf of patients if they are assessed and found to lack capacity. However, patients and families are encouraged to make advance statements and decisions about treatment in the event of losing capacity. Decisions on whether to give, withdraw, or withhold treatment, artificial hydration and nutrition, and cardiopulmonary resuscitation (CPR) are sensitive, and should be based on assessment, consultation with family, and consideration of ethical, legal, and professional principles. Euthanasia and physician-assisted suicide (assisted dying) are highly contentious issues internationally and illegal in most countries. Some countries allow them under certain circumstances. In response to a patient asking about assisted dying, the nurse should listen to their concerns, be prepared to talk about the process of dying, and support them to establish their priorities.

It was in his dealings with the Faculty of Arts and Sciences (FAS) that Conant’s attempt to create a more meritocratic Harvard met its severest test. Out of this often tumultuous relationship came one of Harvard’s most influential academic innovations: a system for the appointment of tenured faculty that became standard practice in American universities. Conant inherited a faculty that was not necessarily the nation’s best. Because of Lowell’s stress on undergraduate instruction, the number and proportion of tutors and instructors steadily increased during the 1920s. At the same time, many of the best known Harvard professors during the Lowell years—Charles Townsend “Copey” Copeland and LeBaron Russell Briggs of the English Department, Roger B. “Frisky” Merriman in History—were not world-class scholars but charismatic classroom performers. Harvard had only one Nobelist, Conant’s chemist father-inlaw, Theodore W. Richards, before 1934; Chicago had three. Nor did its social scientists compare to those at Chicago or Columbia. The rather small stable of Harvard’s scholarly stars included historian Frederick Jackson Turner and philosopher Alfred North Whitehead, whose major accomplishments, done elsewhere, were long behind them. Carnegie Corporation president Frederick Keppel reported the prevailing view in 1934: “Harvard is still princeps but no longer facile princeps; and the story is current that at one of America’s great universities [no doubt Chicago] it is considered the height of academic distinction to receive an invitation from Harvard and to decline it.” Conant warned early on that the growing appeal of other universities and Harvard’s standardized salary, teaching, and research scales made it “increasingly difficult to attract from other universities and research institutes the outstanding men whom we desire.” The dean of the Faculty of Arts and Sciences was English professor Kenneth Murdock. Though he resented Conant for having gotten the Harvard presidency, Murdock was “quite willing” to continue to be dean if Conant wanted him. Conant did not. He appointed the less assertive George D. Birkhoff (among his qualities were exceptional mathematical ability and a keen anti-Semitism), who stayed in the job until 1939, when he was succeeded by the even more unassertive historian William S. Ferguson. Weak deans meant that Conant was in effect his own dean, deeply engaged in curriculum, student recruitment, and above all the selection of faculty.
