Obesity is a modern-day epidemic with increasing prevalence that is directly impacting the global burden of primary total hip arthroplasty. The risk of requiring total hip arthroplasty for osteoarthritis increases incrementally with increasing obesity class. Surgical intervention in obese patients presents a set of unique challenges that should be recognized by the treating medical team. Although predominantly satisfactory outcomes have been reported, perioperative anaesthetic and surgical concerns require thorough patient assessment. There is an increased potential risk of thrombogenic and septic complications, but the body mass index cutoff level beyond which total hip arthroplasty should not be offered in the obese patient remains controversial. Preoperative medical optimization of the patient and appropriate intraoperative interventions are essential to mitigate the risk of complications.

Aim: to identify factors predisposing to perinatal losses, assessment of which is available at the first (only) visit of pregnant woman in antenatal clinic.Materials and Methods. A retrospective analysis of the medical records of 964 women who performed delivery in 2009-2019 in 15 obstetric facilities was carried out. The patients were divided into 2 groups: the main group included 457 women with perinatal losses (stillbirth - 328 children, and 129 children with early neonatal death); the control group consisted of 507 women, whose children survived 7 days during postnatal period. We analyzed parameters routinely determined by an obstetrician-gynecologist at the first visit of woman during within ongoing pregnancy (regardless of gestation age), namely, social status, anamnesis, data of initial examination.Results. The following significant differences were revealed in pregnant women from the main group: a younger age of pregnancy; no registered marriage and permanent job as well as primary and secondary education; smoking, alcohol and drug use; concurrent diabetes mellitus, hypertensive disorders, blood contact infections, obesity; older menarche age and younger sexual debut age; medical history contains infectious genital pathology, more often pregnancies and childbirths, two or more abortions before repeated childbirth, premature births.Conclusion. Thus, the anamnestic indicators noted above can be used to create prognostic statistical systems and models to determine high risk of perinatal losses of any nature.

Abstract:
This article is an overview of a project that aims to study the way in which colloquial Polish is used to verbalise the state of being obese. The innovation of this project is the attempt to comprehensively study the problem by including both how being obese is evaluated from the perspective of slim people and the individual experience of people who are obese. Rather than speaking, the obese are usually spoken of (by slim people). There is a dominant group of slim people that draws up the conditions of discourse, enacts symbolic violence against obese people, and imposes a particular way of verbalising the state of being obese. This project is intended to overcome this dominance, aiming to not only study texts that are authored by slim people, but also give the floor to obese people, with the aim of objectivising the phenomenon. The problem of verbalising obesity is contemporaneous and emotionally biased. The significant progress in elucidating the causes and consequences of obesity that has taken place over the last two decades has not led to a change in the perception of obesity in colloquial language. In the common consciousness of Polish speakers, an obese person disturbs the culturally established sense of aesthetics of a human body. Obese people are perceived as alien, worse, uglier, lazy, less intelligent, and incongruous when compared with the ideal image of people who are healthy, lean, and successful. The project envisions excerpting lexemes and metaphors used to describe obesity from dictionaries, fiction, public discourse (periodicals, TV programmes), and interviews with obese people. The result of the work will be an inventory – a database with a specific template for describing a unit. An important part of each unit will be information on the axiological and semantic features of the lexemes and metaphors presented in the form of a thematic category code. The project is based on the cognitive theory of metaphor of G. Lakoff, the method of removal of metaphors from text of B. Fatyga and P. Zieliński, the theory of cognitive definition of J. Bartmiński, and the method of studying the personal language of values of J. Puzynina.

Background Physical activity is crucial for child obesity prevention and intervention. Narratives embedded in active games can increase children’s physical activity. Objective Little is known about the narrative characteristics that would motivate children to exercise. We attempted to fill the gaps in understanding regarding narrative design for active video games. Methods In this exploratory study, four animated narratives of different genres were professionally generated. Children (N=41) between the ages of 8 and 12 years were interviewed to identify their preferences. Sessions were digitally recorded, transcribed, and analyzed using exploratory thematic analysis. Results Findings revealed that the children rated the dystopian science fiction story as their favorite across all weight, race, and gender groups. The physical activity-friendly narrative strategies included virtuous characters, extraordinary character actions, interesting plots, super powers, and engaging cliffhangers. Alternatively, information not related to physical activity, difficult-to-follow plot lines, passive protagonists, and repetitive narrative tropes were less appealing for physical activity. Conclusions This research provides preliminary evidence that narratives have characteristics that may increase child physical activity when playing active games. Future empirical studies should verify and test these design principles.

As long as health communications have existed in the USA, Americans have faced the task of sorting the agenda of the source from the advice it provides. That task has become more complicated as advances in the science of nutrition and the technology used to present it have heightened the complexity of nutrition communications. Getting consumers to adopt a healthier diet has been a protracted undertaking with limited successes along the way. The obesity epidemic has added urgency to this discourse: not only do we need to eat better, but most of us also need to eat less. This paper reviews the dynamics that have made the communication of accurate and actionable health behaviour information an ongoing challenge, and outlines strategies for moving ahead. It considers the interplay of four sets of factors: the evolutionary nature of the science on which recommendations are based; the many sources of communication about that science; the agendas or motivations of each source; and finally the multifaceted nature of consumers, the recipients of these communications. Communication alone has not been, and will not be, sufficient for consumers to adopt the behavioural changes endorsed by experts. Broad environmental interventions coupled with individual skills development will need to be part of the process. Ultimately, it is the consumer who decides what is for dinner. Media literacy will play a critical role in building consumer efficacy in sorting fact from fiction in order to select food for a healthful diet.

Abstract Objectives Although studies suggested that smartphone addiction increases the risk of obesity, it is unknown whether the duration and content type of smartphone use are independently associated with obesity and obesogenic dietary patterns. Methods Using a nationally representative sample of 53,133 Korean adolescents (age 12–18 years) in the Korea Youth Risk Behavior Web-Based Survey, we conducted a cross-sectional analysis examining the associations of duration and content type of smartphone use with dietary factors and prevalence of overweight. Duration and content type of smartphone use, dietary intakes, weight, and height were self-reported via online-based questionnaire. Overweight was defined as BMI-for-age ≥85th percentile based on the 2017 Korean National Growth Charts. Multivariable logistic regression was performed on 8 individual dietary factors (≥5 d/wk skipping breakfast, ≥7 times/wk fruits, ≥3 times/d vegetables, ≥3 times/wk fast-food, ramen, chips, soda, sugar-sweetened beverage) and the prevalence of overweight to estimate odds ratios (ORs) and 95% confidence intervals (Cls). All analyses accounted for complex survey sampling and adjusted for potential confounders. Results With adjustment for content type of smartphone use, longer duration of smartphone use (≥5 vs. <2 h/d) was significantly associated with higher frequency of skipping breakfast (OR [CI]: 1.83 [1.72, 1.94]), higher intakes of fast-food (2.14 [1.98, 2.30]), ramen (2.12[1.98, 2.27]), chips (1.43 [1.34, 1.52]), soda (2.34 [2.19, 2.50]), sugar-sweetened beverage (1.88 [1.78, 1.99]), lower intakes of fruits (0.61 [0.57, 0.65]) and vegetables (0.69 [0.64, 0.75]), and higher prevalence of overweight (1.23 [1.15, 1.31]) (all p-trend < 0.001). Compared with using smartphone for information search/retrieval, using messenger, game, entertainment, and SNS were significantly associated with higher intakes of soda and sugar-sweetened beverage, adjusting for duration of smartphone use (all P < 0.05). Game, entertainment, and comics/fiction (vs. information search/retrieval) were associated with higher prevalence of overweight (all P < 0.001). Conclusions Our data suggest the duration and content type of smartphone use are independently associated with obesogenic dietary patterns and the prevalence of overweight in a dose-response manner. AbstractObjectiveThe present qualitative study explored health perceptions, diet and the social construction of obesity and how this relates to the initiation and maintenance of a healthier diet in UK Pakistani women.DesignPakistani women in Greater Manchester participated in focus group and one-to-one discussions. Semi-structured interviews employing fictional vignettes and body shape images were used to explore the participants’ beliefs and practices regarding diet, overweight/obesity and the risk of type 2 diabetes. Transcripts were analysed using phenomenological and sociological approaches.SettingInterviews took place either in local community and Pakistani resource centres or in private homes.SubjectsFirst- and second-generation women who were both active in the community and housebound. The women spoke English and/or Urdu.ResultsThe fifty-five participants lacked the motivation to address weight gain and were unsure how to do so. There was a limited awareness of the link between weight gain and type 2 diabetes. Other barriers included the influence of Islam, culture and familial expectations on home cooking, perceptions that weight gain is inevitable (owing to ageing, childbirth or divine predestination) and the prioritisation of family concerns over individual lifestyle changes. As the findings of the present research did not correspond to existing educational and behaviour change models, a new Health Action Transition conceptual model is proposed.ConclusionsHealth education programmes that aim to address obesity and its associated risks in the South Asian community must take into account the complex beliefs and practices and the multiple dimensions of religion, ethnic and social identity within this population. The present study provides further insight into these factors and proposes a novel model for use in designing and implementing education interventions for British Pakistani women.

Over the last centuries, in the context of industrialization, globalization and urbanization, profound dietary changes have occurred. Ubiquitous access to cheap, readily available and highly palatable unhealthy products, together with aggressive marketing that forcefully impacts social norms, have led to overconsumption of energy-dense, nutrient-poor foods. This has not only been associated with a higher prevalence of obesity and diet-related non-communicable diseases, but also with detrimental effects on the world’s natural resources. Broader and braver public health measures favoring the availability and affordability of healthy, minimally processed foods should be implemented in conjunction with educational strategies to re-encourage a healthy and sustainable food consumption.

For most of the world's population, eating is no longer what it was just a few decades ago. The diversification and globalisation of the food supply, the industrialisation of food, and the abundance of information have simplified this everyday activity, but they have also made it more complex. Never before has it been so easy to eat well and to eat poorly. The proof is that obesity and overweight are no longer a problem exclusive to the richest, while nutritional deficiencies do not affect only the most impoverished. The new thing about our time is that both problems, over-nutrition and under-nutrition, coexist not only in countries and cities, but also within the same family and even throughout a person's lifetime. Growing concern for health has fuelled interest in the relationship between diet and disease prevention. But despite the remarkable scientific advances, there are still many unanswered questions, and many evidence-based messages do not reach the population and are lost in a sea of misinformation and half-truths. In this monograph, apart from presenting a brief social history of food, we will delve into the advances in nutritional epidemiology, we will separate facts and fiction regarding current food, we will review what reliable dietary recommendations are and how they are identified, we will explain the evolution of dietary guidelines with an example, we will tackle the problem of ultra-processed food from the point of view of addiction and we will analyse miracle diets and how to detect them.

Abstract:
Fat is, according to Deborah Lupton, a cultural artefact: a bodily substance or body shape that is given meaning by complex and shifting systems of ideas, practices, emotions, material objects and interpersonal relationships (3). Further, as Donna Lee Brien writes in her article for this issue, fat is a, “powerful and most evocative word”, which has the capacity to elicit strong emotional reaction and hostility. As Kulick and Meneley assert in ‘Fat: The Anthropology of an Obsession’, fat, “conjures images of repulsion, disgust, and anxiety for some, but associations of comfort, delight and beauty for others” (1). In choosing this theme for our special edition of the M/C Journal we were cognisant of, and influenced by, the re-appropriation and utilisation of fat as a positive self-identifying and political term by fat activists and academics (Lupton). Our aim in compiling this special issue was/is to provide space for a nuanced discussion of all things fat. Our decision to edit an issue on the topic of fat has been partly motivated by our research: Rachel Jarvie’s on “maternal obesity”, and Julie Parsons’s on "embodied foodways" (107), as well as former patients experiences of bariatric surgery. It was also a consequence of participating in, and (in Parsons case) presenting at, the inaugural Weight Stigma Conference held in the UK in 2013, attended by an interdisciplinary range of scholars and practitioners interested in combatting and/or exploring the impact of weight based stigma. When we put out a call for papers for this issue we were somewhat surprised and dismayed to receive the following response: I expect when they say they want to have viewpoints from the whole "spectrum" of fat they mean people who advocate for an "obesity epidemic" and anti-fat work. Fat Studies work that is accepted will be located with this kind of oppressive work in the journal issue. I'm a fan of multiple viewpoints, but not when it’s about fostering oppression and fat shaming… As editors we wanted to critically engage with the range of discourses pertaining to fat, but in no way to foster oppression. As evidenced in the UK’s Economic and Social Research Council (ESRC) funded, Health At Every Size (HAES) interdisciplinary seminar series, that ran from 2010-2012, there is a clear dissonance between those advocating size acceptance and those medicalising fatness as a major health problem. Indeed, it was suggested at the inaugural Weight Stigma Conference by one speaker that stigma or fat shaming could be useful in fighting the war on “obesity” and herein is the nub of the matter. There is a politics of fatness that challenges “obesity” discourse or the medicalization of fatness and positions this as part of the problem of fat oppression. This therefore raises potential problems for those wanting to draw upon a range of perspectives and open up discussion in this arena. Arguably, enabling viewpoints from across the spectrum to be juxtaposed fosters critical debate. We agree with Marilyn Wann (xv) that: Fat functions as a floating signifier, attaching to individuals based on a power relationship, not a physical measurement [and that] people all along the weight spectrum may experience fat oppression… In other words continued weight based stigma has implications for everyone and should be challenged and contested. This edited collection of articles reflects a gamut of meanings and experiences associated with fat. Mostly they challenge and are critical of “obesity” discourses that medicalise/stigmatise fat/fatness. They also demonstrate on a number of levels the intersectionalities of fat with other socio-cultural, structural factors (in no particular order) such as gender, class, education, ethnicity and sexuality. Indeed, for many, fat and fatness are a cause for celebration and acceptance rather than shame and stigma. We present a number of articles that provide a critical analysis of the reviling of fatness and fat bodies as unhealthy and unattractive in contemporary neo-liberal societies, and the concomitant stigmatisation of people labelled as fat. The prevalent cultural phenomenon of “fat shaming” is an issue discussed in many of the articles. Illustrating Foucault’s maxim, “where there is power, there is resistance” (95). We see in many of the articles a repudiation of fat shaming and refusal to experience fat shame. In some of the articles we find a joyous celebration of, and pride in, fat/fatness and a number of submissions address the tension between these positions. Most of the articles in this collection can therefore be positioned within the realm of fat acceptance, as critical of “obesity” discourses and/or revelatory regarding the power of fat politics. For example, the opening article from Patti Watkins is an exposition of teaching fat positive messages and the impact of her Fat Studies course on students’ perceptions of themselves and “obesity” more generally. This provides a brief background to Fat Studies, Health at Every Size, (HAES), size acceptance movements and the “obesity paradox”. It demonstrates how experiences of fatness intersect with issues such as education, gender and class. James Burford, also locates a critique of “fat shaming” within higher education. In this instance he frames the Twitter/ Tumblr backlash against a Professor critical of fat PhD students, as a celebration of fat embodiment enacted in order to re-negotiate/challenge the hegemonic social discourses of abjection associated with fatness. Thus, Tumblr becomes a significant space for re-imagining new forms of cultural attachments and identifications. Michael Bruner draws upon popular magazines, newspapers, blogs, Web sites, and videos, to compare and discuss the media framing of six political figures from around the world as fat. He documents the diverse ways that fat is woven into public political discourse and explores how women and men are framed as fat for various, mainly discrediting, political purposes. In so doing, he aims to help readers and viewers to take a more critical perspective on fat politics. Deborah Rodan’s work uses online viewer comments about Australian makeover program Bringing Sexy Back to analyse the extent to which audiences considered fat shaming inappropriate as a tool for motivating change. She finds that the majority of those participating on the online forums are supportive of this genre and uncritical of the premise on which these programs are based as they are not critically engaged in fat politics. Cat Pause introduces us to online performances of fatness in the “Fatosphere”, which engender a critique of neo-liberal discourses of health and the will to be healthy. She draws on Goffman’s work on stigma/spoiled identities to discuss an identity management style: “coming out as fat”, and highlights the intersectionalities of fat with race and sexuality. Anna Lavis’s work is also about size acceptance: she analyses videos posted on You Tube by Big Beautiful Women (BBW). Here fatness intersects with sexuality and BBW have agency and a celebrated lived materiality that is visceral and pleasurable. In Gurleen Khandpur’s article she discusses fat sexuality as stigmatised, where fat individuals are culturally viewed as asexual and incapable of arousing sexual desire unless it is fetishistic. This is in contrast to sexual thin privilege: where thin bodies are viewed as beautiful and desirable and thin sex is the norm. Upon closer scrutiny she suggests these fetishistic/normative domains are more similar than distinct and require further critical discussion. In terms of the intersectionalities of fat with other issues, Lily O'Hara, Jane Taylor and Margaret Barnes draw on a critical discourse analysis of Australia’s “Measure Up and Swap It” social marketing campaigns aimed at reducing “obesity” to produce a trenchant critique of the inherent stigmatisation of fat people. They argue that “public health” campaigns such as this are entrenched in “obesity” discourse that can have iatrogenic consequences by inducing shame, blame and fear. Interesting points are made vis-à-vis the targeting of Aboriginal Australians in these campaigns. Similarly, Amita Nijhawan looks at the performance of fat and ethnic identity on American television (The Mindy Project), and examines the bodily mechanisms through which Mindy disrupts these. Nijhawan argues that Mindy uses issues of fat and body image to disrupt stereotypical iterations of race. She identifies the construction of South Asian femininity in American popular culture in order to set up the discussion of fat, gender and race as interrelated performative categories. There are two complementary articles on bariatric surgery, in Aimee Simpson’s work she asserts that shaming and stigmatisation of fat bodies is instrumental in people seeking and accepting drastic weight-loss “solutions” such as bariatric surgery. Drawing on research with people who have undergone bariatric surgery in New Zealand, she critiques the intense policing, monitoring and regulation by self and others that people experience post-operatively, and thus questions the premise that “health” is achieved through “thinness”. Similarly, Zoe Meleo-Erwin conducted semi-structured interviews with 30 American bariatric patients and identifies how the decision to have surgery occurs at the interface of emotion, flesh, space, place and time. This is about negotiating weight-based stigma, which is further shaped by broader neo-liberal discourses of health. There are also two articles on autobiography; Gayle Letherby uses a mixture of auto/biographical writing and fiction, as well as drawing on academic work in order to reflect on the intersectionalities of fat with gender appropriate hetero-normative cultural scripts of “appropriate” femininity. Donna Lee Brien provides us with a preliminary taxonomy of autobiographical writing on fat, whilst this is not all about fat embodiment; it is pro-fat writing as an ingredient in cooking, eating as well as memoirs of fat. Thinking and Writing about Fat… Since the beginning of my academic career I have written auto/biographically. Like others I believe that in including my own experience in my writing I make clear not only the influence my autobiography has on the work that I do but how, in turn, the work that I do influences my autobiography (Stanley; Morgan; Letherby Feminist Research, Interconnected Lives). I began this paper with a list of statements that have been said to me, or about me (and reported to me) by others in the last 18 months since a significant weight loss. As you see the messages ARE mixed and even the ‘compliments’ feel tainted; did I really look so bad before? Jeannine Gailey (16) reminds us that the fat body, especially the female fat body, is marginalised, stigmatised and summarising her study with 74 fat women argues that the women whose voices are represented in this book indicated that they are often hyperinvisible when it comes to their health or actual dealings with health-care practitioners, in addition to frequently feeling invisible with sexual partners, family, friends, colleagues, and strangers. Some of my own (auto/biographical) research has focussed on the experience of ‘infertility’ and ‘involuntary childlessness’ and the above statement also applies to many of my respondents, and similar others, who feel marginalised and stigmatised because of their status as nonmother (e.g. Letherby nonmotherhood). Although not my primary research area I have recently been involved in a number of research projects – either as supervisor or researcher – concerned with weight and/or weight management. One of these focused on the relationship between ‘obesity’ and ‘infertility’ (written, like other phrases in this piece, in quotation marks to highlight the problematic nature of simplistic definitions). Some medical literature suggests that a woman’s body mass index (BMI) is an important determinant of medical outcomes in the treatment of ‘infertility’. However, recent work contests the link between BMI, ‘obesity’ and ‘infertility’. Research from the social sciences shows that medical professionals, media and lay discourse position some individuals as ‘deserving’ and others as ‘undeserving’ of medical treatment (including in/fertility treatment) (Letherby Infertility; Stenhouse and Letherby). Women unable to achieve pregnancy and/or carry a baby to term due to weight related issues (either ‘real’ or assumed) will likely experience multiple stigma in relation to their gender, BMI and fertility status. In addition restricting ‘infertility’ treatment on the grounds of weight can itself cause stigmatization and may lead to depression and low self-esteem. ****I began writing fiction (as an adult) about five years ago and this type of writing has become increasingly important to me both academically and personally and is, I think, another way to tell auto/biographical stories. In my teaching I encourage students to think sociologically about fiction they enjoy and in recent academic writing on reproduction and on bereavement and loss I have included some fictional pieces (e.g. Davidson and Letherby; Letherby Interconnected Lives, Mortality). Taking a traditional view of the relationship between fact and fiction, some might suggest that fiction is the opposite of explicit auto/biographical writing. I disagree. Drawing specifically on respondents’ narratives, or more generally on our research and our own life experiences ‘fiction’ can provide a powerful, accessible narrative (e.g. Frank). What follows is a piece of fiction that is auto/biographical in that it connects to some experiences in my own life (see Letherby Interconnected Lives) and has connections to some of the experience of respondents from various of my research studies. My aim (or rather one of them) in writing this piece was to highlight the stigma and marginalisation that women in these situations sometimes feel. The Mixed Messages, not least with reference to fat, are evident I hope. Mr Sprat and I: A Story He drank three times as much as I did during our first date. I replied ‘yes please,’ when twice he asked if I wanted crisps or nuts with my wine. He suggested a film, followed by more drinks the next time we met. I enjoyed the popcorn in the cinema, the snacks in the pub. He bought us a fish and chip supper on the way home. The cod was fresh and lightly battered, the chips, made from good potatoes, were just the right combination of fat and starch. We ate our meal straight from the paper. He wiped his hands on a tissue but surprised and delighted me by sucking the grease from my fingers one by one. I was lost. I was his. A generous boyfriend he often paid for us to eat out. He never had a pudding but would choose a liqueur, or a shot of whisky, instead. Curious, rather than shocked, I wondered how he could down a pint in just a few seconds. ‘How do you do that, how can you drink it so quickly?’ I asked. ‘I open my throat and it just slips down; only when I'm really thirsty though.' He smacked his lips and wiped his mouth with his hand. He drank the whisky more slowly, ‘to enjoy the hot, fiery kick.’ I always had a taste of his starter and ended my meal with something sweet. Chocolatey creations were my preference but I enjoyed all desserts. He indulged me and reassured me. ‘I love your curves,’ he'd say proving it with his hands and his lips. Many a morning after I’d cook us a big fry-up. ‘Soaks up the booze,’ he said. Amsterdam was his choice for a stag weekend. He travelled with a large group of friends. There weren't any sexual exploits, I'm sure of that, but plenty of drink was taken and some wacky backy smoked. A good time was had by all and it took him a few days to recover from the trip.I choose a country hotel weekend break for my pre-wedding treat. We all had a beauty treatment or two and swam, read and gossiped the two days away. The food was plentiful and beautifully presented. I had to eat leanly between the hen party and the main event to get into my dress.After making such a beautiful speech he deserved to relax a little. But I wish he'd stopped at the champagne. After our first dance he propped up the bar with his mates and my brother and drank more than all of them; mostly beer, a few spirits. I’d been so looking forward to our first night of pleasure as husband and wife but the consummation of our marriage lacked vitality; a waste of the four-poster bed. His breath stank. As soon as it was over he fell asleep, although I was still wide awake. As part of our wedding package there were some goodies waiting for us in the bridal suite including a good sized box of melt-in-the-mouth chocolates. I ate the lot. He made it up to me on the honeymoon. More attentive than ever he hired a boat and took me to secluded beaches. As we sunbathed he lazily stroked my back and my thighs, when we swam we explored each other's bodies undercover of water. ‘I love you, I want you,’ he whispered. ‘I love you so much I want to bite you, to gobble you up.’ My body responded to his touch and to his words. I had never felt so desired, so cherished. The evenings and the nights were the best. We ordered local specialties at dinner and with his bare hands he fed me succulent fish, juicy meats and fruit dripping in syrup. In bed as he licked the excesses off my lips and from my mouth I could taste the wine in his. I drank him in. We were never so in tune again, our senses alive, our individual indulgences merged. We were as one, our bodies replete.Back home he worked hard and played hard keeping up his nights out with the boys and finding new restaurants for us to go to. He became skilled at choosing the correct wine to accompany the dishes I favoured. He drank the pudding wine whilst I ate the pudding. At home he kept beer in the fridge along with a jug of water so he could add a splash to his whisky. For his birthday I treated him to a peaty single malt. Our weekly food bill was a 50/50 split between alcohol and food. I loved to cook. I roasted and baked and chipped and fried. I folded and mixed and whisked. I was adventurous with spices. For my birthday he bought me a cookery book; a best seller from the latest celebrity chef. I experimented some more. My pastry was light and my sauces smooth. He was always appreciative but more often than not he wouldn't finish his food, sometimes leaving as much as he ate. As he carried our glasses (usually his third or fourth alcoholic drink since returning from work, almost always my first) through to the lounge I would take the plates into the kitchen (spooning the remains from his plate into my mouth rather than scraping it into the bin). A hard worker he was promoted, several times. More money led to more expensive tastes and we enjoyed good holidays and ate out even more, sometimes with his colleagues and bosses. A little shy in such company, aware of his status as a working class boy done good, he was always happier after a couple of drinks and would have a quick one before we left the house. In response to my anxious, ‘darling, do you think you should?,’ he would kiss me and say, ‘just a small one to oil the conversation.’I lived for our holidays and the nights we spent alone. We always found something to talk and laugh about and our indulgence of each other's eating and drinking habits was mirrored by a concern for each other's sexual wellbeing. He liked sex with the lights on. I adored it when he quietly sang to me during lovemaking. I hated the corporate entertainment. The women seemed to get thinner each time we met, shrinking as I grew. The way they managed to look as if they were eating the wonderfully cooked and carefully presented food whilst not actually consuming anything was an art form. I couldn't resist the delicious offerings but their snide observation of me turned the food to cardboard in my mouth. His work put him under increasing pressure. Some mornings I could taste alcohol mingled with mint when he kissed me goodbye. I found a bottle of vodka at the back of the cupboard, a cheap brand, that hadn't been in the trolley at our weekly shop. ‘Where did this come from, did you buy it?,’ I asked. ‘I guess I must have, I don't remember,’ he shrugged. The bottle disappeared but he kissed me less and began going straight upstairs when he got home. I'd hear him moving around, opening cupboards, finding hiding places for his not so secret stash.I still shopped and cooked trying new recipes in an attempt to win him back from his liquid mistress. I made meals that in my view were fit for the Gods, rich in flavour and high in calories. But he was less and less interested. He’d push his plate away and re-fill his glass. Eventually I gave up and moved on to cheap two-for-the-price-of-one microwave meals finding their gloopiness strangely comforting. They weren't enough for me though and I’d fill up with extra creamy potatoes or with toast, dripping with butter and topped thickly with cheese or chocolate spread. I ate off and on all day when I was alone and when he was asleep.When I said that I wanted us to have a baby he agreed, clinging, like me, to the hope that a child might make things better. Half-heartedly we tried for a while. The lights were off and there was no singing. Nothing happened. We lied to the GP when asked about our sexual activity, embarrassed and distressed at the lack of passion in our life together. He lied about his drinking too. ‘How much do I drink? Well, a little more than I should I guess, I know I should cut down, but you know how it is?’ He glanced at me, smiled at the male doctor and shrugged. I hated him then. I hated him as he failed to admit that he had a problematic relationship with alcohol, as he duped the GP and won his sympathy rather than rightly causing concern. I could guess what the doctor was thinking. Who wouldn't need a drink when married to a woman like me, a woman who had let food get the better of her spirit and her body? I couldn't lie about my problem. It lay heavy on my bones. I left the surgery with a diet sheet and a red face. When he shook the doctor's hand I turned away in misery and disgust.We drove home with the radio on to cover our silence. Once he tried to take my hand but I pulled away. I went to the kitchen. He went upstairs. I cut some bread and turned on the toaster. He reached into the back of his shirt drawer and pulled out a bottle. One night soon after he took me in his arms, as much of me as he could, holding on tight even as I tried to push him away. ‘Let's do something, anything. I still love you,’ he said. ‘What about a holiday? Please darling. You still love me too don’t you?’ Nodding, I relaxed into him, my bulk against his sharp hips. I packed my optimism along with his tiny shorts and my super-size trousers and dresses but my tentative happiness didn't last long. I couldn't do up the seatbelt in standard class and our upgrade was because of my size rather than our celebrity. For once I wasn't hungry. We tried hard to recreate the more heady days of our relationship but the break was not what either of us wished for. He drank heavily on the return journey, swigging back spirits in the way he once had pints. I closed my eyes to block out the pitying stares.He drank more. He ate even less. He lost his job. I heard him retching in the toilet every morning. He threw his vices up, I kept mine deep inside. As he flushed the toilet I thought of the baby we'd been unable to make I whispered to myself ‘that should be me, the morning sickness should be mine.' Then I went to the kitchen to cook and eat the fried breakfast he couldn’t face anymore. He went out most days, to the pub or the off-license.I went out only to the supermarket. He started to smell. He slept fitfully and snored loudly when he did sleep. He never touched me, unable to make love to me even if either of us had wanted it. When he wasn't sleeping he was drinking. I outgrew my clothes again so I lived in t-shirts and joggers and ordered groceries online. I stuffed the food in as soon as it arrived but it didn't comfort me anymore. He collapsed.I let him go to the hospital alone. He came home. He didn't pour himself a drink. He packed a bag instead. ‘I think I should go, don't you?’ he said.‘Yes’, I said, the tears running down my face. He turned just as he was leaving. ‘Do you think there's a way back for us, we were so good together once?’ ‘I don't know,’ I said. After he left I filled the bin; with dairy and carbohydrates, with fat and sugar… Some Concluding Thoughts… I consider writing as a method of inquiry, a way of finding out about yourself and your topic. Although we usually think about writing as a mode of “telling” about the social world, writing is not just a mopping-up activity at the end of a research project. Writing is also a way of “knowing” – a method of discovery and analysis. By writing in different ways, we discover new aspects of our topic and our relationship to it. Form and content are inseparable (Richardson 515). I agree. Writing – both in the traditional academic style and utilising prose and fiction – enables us, has enabled me, to reflect in detail about issues and topics and that important to me and to others, issues and topics that are often misunderstood and misrepresented. Fat, alongside in/fertility, childlessness and nonmotherhood, is one such issue. Abstract:
BMI, a ratio of weight over height, is a culturally-biased tool imposed upon the scientific, academic and medical communities as an errant measure of obesity across ethnicity. Body Mass Index (BMI) relates mass (g) to a relative fat distribution with regards to height. Its genesis is from the actuarially derived and ethnically exclusive height and weight tables that promote the fictional notion of inter-ethnic ideal weights that would be later adopted by the National Institutes of Health (NIH) as a competent measure of adiposity. Best practice, movement towards individualized medicine and deployment of effective models that impact the diabetes epidemic and its related precursors like insulin resistance and the metabolic syndrome, requires terminal use of BMI, a biologically meaningless and crude indicator of obesity, in favor of effective and culturally competent non-relative body composition evaluation of genetically determined adiposity that untenably compares values among groups. African Americans are among the increasingly affected groups for diabetes and posses unique composition variation requiring proper intra-cultural evaluation independent of inter-ethnic Eurocentric assumptions that over assesses obesity risk. Incorporating use of 4C models to evaluate adiposity and assess risk for diabetic predisposition and onset provides an effective unbiased assessment of the cultural components inherent within body composition variation among ethnicity, age, gender. Obesity and type II diabetes onset and pre-disposition is assessed phenotypically, in creation of a body mass profile among African and African American groups, using 4C model, photography, anthropometry, somatotype and genetic evaluation. Environmental obeseogenic cultural factors are also explored.

To combat rising rates of childhood obesity in the U.S. requires helping parents recognize when their child is overweight or obese. However, parents’ accuracy might be affected by social comparisons, in which parents compare their child to other overweight children, and rationalize that their child is ‘normal’ weight, and therefore, healthy. The aim of the study was to assess whether a photograph of a fictional child impacts a parent’s judgment of their own child’s weight. A nationwide sample of parents (n=517) of children ages 2-12 provided their child’s height and weight, viewed a photograph of an underweight (upward comparison), normal weight (control) or overweight (downward comparison) child, and judged the health of both. Parents inaccurately judged the downward comparison compared to the control and upward comparisons. Further, parents were less accurate in judging their child’s weight when given an upward comparison compared to a control. Intentions to control their children’s weight were unaffected.

Abstract:
As the name suggests, the humanities is largely a study of the human condition, in which history sits as a discipline concerned with the past. Environmental history is a new field that brings together scholars from a range of disciplines to consider the changing relationships between humans and the environment over time. Critiques of anthropocentrism that place humans at the centre of the universe or make assessments through an exclusive human perspective provide a challenge to scholars to rethink our traditional biases against the nonhuman world. The movement towards nonhumanism or posthumanism, however, does not seem to have had much of an impression on history as a discipline. What would a nonhumanist history look like if we re-centred the historical narrative around pigs? There are histories of pigs as food (see for example, The Cambridge History of Food which has a chapter on “Hogs”). There are food histories that feature pork in terms of its relationship to multiethnic identity (such as Donna Gabaccia’s We Are What We Eat) and examples made of pigs to promote ethical eating (Singer). Pigs are central to arguments about dietary rules and what motivates them (Soler; Dolander). Ancient pig DNA has also been employed in studies on human migration and colonisation (Larson et al.; Durham University). Pigs are also widely used in a range of products that would surprise many of us. In 2008, Christien Meindertsma spent three years researching the products made from a single pig. Among some of the more unexpected results were: ammunition, medicine, photographic paper, heart valves, brakes, chewing gum, porcelain, cosmetics, cigarettes, hair conditioner and even bio diesel. Likewise, Fergus Henderson, who coined the term ‘nose to tail eating’, uses a pig on the front cover of the book of that name to suggest the extraordinary and numerous potential of pigs’ bodies. However, my intention here is not to pursue a discussion of how parts of their bodies are used, rather to consider a reorientation of the historical narrative to place pigs at the centre of stories of our co-evolution, in order to see what their history might say about humans and our relationships with them. This is underpinned by recognition of the inter-relationality of humans and animals. The relationships between wild boar and pigs with humans has been long and diverse. In a book exploring 10,000 years of interaction, Anton Ervynck and Peter Rowley-Conwy argue that pigs have been central to complex cultural developments in human societies and they played an important role in human migration patterns. The book is firmly grounded within the disciplines of zoology, anthropology and archaeology and contributes to an understanding of the complex and changing relationship humans have historically shared with wild boar and domestic pigs. Naturalist Lyall Watson also explores human/pig relationships in The Whole Hog. The insights these approaches offer for the discipline of history are valuable (although overlooked) but, more importantly, such scholarship also challenges a humanist perspective that credits humans exclusively with historical change and suggests, moreover, that we did it alone. Pigs occupy a special place in this history because of their likeness to humans, revealed in their use in transplant technology, as well as because of the iconic and paradoxical status they occupy in our lives. As Ervynck and Rowley-Conwy explain, “On the one hand, they are praised for their fecundity, their intelligence, and their ability to eat almost anything, but on the other hand, they are unfairly derided for their apparent slovenliness, unclean ways, and gluttonous behaviour” (1). Scientist Niamh O’Connell was struck by the human parallels in the complex social structures which rule the lives of pigs and people when she began a research project on pig behaviour at the Agricultural Research Institute at Hillsborough in County Down (Cassidy). According to O’Connell, pigs adopt different philosophies and lifestyle strategies to get the most out of their life. “What is interesting from a human perspective is that low-ranking animals tend to adopt one of two strategies,” she says. “You have got the animals who accept their station in life and then you have got the other ones that are continually trying to climb, and as a consequence, their life is very stressed” (qtd. in Cassidy). The closeness of pigs to humans is the justification for their use in numerous experiments. In the so-called ‘pig test’, code named ‘Priscilla’, for instance, over 700 pigs dressed in military uniforms were used to study the effects of nuclear testing at the Nevada (USA) test site in the 1950s. In When Species Meet, Donna Haraway draws attention to the ambiguities and contradictions promoted by the divide between animals and humans, and between nature and culture. There is an ethical and critical dimension to this critique of human exceptionalism—the view that “humanity alone is not [connected to the] spatial and temporal web of interspecies dependencies” (11). There is also that danger that any examination of our interdependencies may just satisfy a humanist preoccupation with self-reflection and self-reproduction. Given that pigs cannot speak, will they just become the raw material to reproduce the world in human’s own image? As Haraway explains: “Productionism is about man the tool-maker and -user, whose highest technical production is himself […] Blinded by the sun, in thrall to the father, reproduced in the sacred image of the same, his rewards is that he is self-born, an auto telic copy. That is the mythos of enlightenment and transcendence” (67). Jared Diamond acknowledges the mutualistic relationship between pigs and humans in Guns, Germs and Steel and the complex co-evolutionary path between humans and domesticated animals but his account is human-centric. Human’s relationships with pigs helped to shape human history and power relations and they spread across the world with human expansion. But questioning their utility as food and their enslavement to this cause was not part of the account. Pigs have no voice in the histories we write of them and so they can appear as passive objects in their own pasts. Traces of their pasts are available in humanity’s use of them in, for example, the sties built for them and the cooking implements used to prepare meals from them. Relics include bones and viruses, DNA sequences and land use patterns. Historians are used to dealing with subjects that cannot speak back, but they have usually left ample evidence of what they have said. In the process of writing, historians attempt to perform the miracle, as Curthoys and Docker have suggested, of restoration; bringing the people and places that existed in the past back to life (7). Writing about pigs should also attempt to bring the animal to life, to understand not just their past but also our own culture. In putting forward the idea of an alternative history that starts with pigs, I am aware of both the limits to such a proposal, and that most people’s only contact with pigs is through the meat they buy at the supermarket. Calls for a ban on intensive pig farming (RSPCA, ABC, AACT) might indeed have shocked people who imagine their dinner comes from the type of family farm featured in the movie Babe. Baby pigs in factory farms would have been killed a long time before the film’s sheep dog show (usually at 3 to 4 months of age). In fact, because baby pigs do grow so fast, 48 different pigs were used to film the role of the central character in Babe. While Babe himself may not have been aware of the relationship pigs generally have to humans, the other animals were very cognisant of their function. People eat pigs, even if they change the name of the form it takes in order to do so:Cat: You know, I probably shouldn’t say this, but I’m not sure if you realize how much the other animals are laughing at you for this sheep dog business. Babe: Why would they do that? Cat: Well, they say that you’ve forgotten that you’re a pig. Isn't that silly? Babe: What do you mean? Cat: You know, why pigs are here. Babe: Why are any of us here? Cat: Well, the cow’s here to be milked, the dogs are here to help the Boss's husband with the sheep, and I’m here to be beautiful and affectionate to the boss. Babe: Yes? Cat: [sighs softly] The fact is that pigs don’t have a purpose, just like ducks don’t have a purpose. Babe: [confused] Uh, I—I don’t, uh ... Cat: Alright, for your own sake, I’ll be blunt. Why do the Bosses keep ducks? To eat them. So why do the Bosses keep a pig? The fact is that animals don’t seem to have a purpose really do have a purpose. The Bosses have to eat. It’s probably the most noble purpose of all, when you come to think about it. Babe: They eat pigs? Cat: Pork, they call it—or bacon. They only call them pigs when they’re alive (Noonan). Babe’s transformation into a working pig to round up the sheep makes him more useful. Ferdinand the duck tried to do the same thing by crowing but was replaced by an alarm clock. This is a common theme in children’s stories, recalling Charlotte’s campaign to praise Wilbur the pig in order to persuade the farmer to let him live in E. B. White’s much loved children’s novel, Charlotte’s Web. Wilbur is “some pig”, “terrific”, “radiant” and “humble”. In 1948, four years before Charlotte’s Web, White had published an essay “Death of a Pig”, in which he fails to save a sick pig that he had bought in order to fatten up and butcher. Babe tried to present an alternative reality from a pig’s perspective, but the little pig was only spared because he was more useful alive than dead. We could all ask the question why are any of us here, but humans do not have to contemplate being eaten to justify their existence. The reputation pigs have for being filthy animals encourages distaste. In another movie, Pulp Fiction, Vincent opts for flavour, but Jules’ denial of pig’s personalities condemns them to insignificance:Vincent: Want some bacon? Jules: No man, I don’t eat pork. Vincent: Are you Jewish? Jules: Nah, I ain’t Jewish, I just don’t dig on swine, that’s all. Vincent: Why not? Jules: Pigs are filthy animals. I don’t eat filthy animals. Vincent: Bacon tastes gooood. Pork chops taste gooood. Jules: Hey, sewer rat may taste like pumpkin pie, but I’d never know ’cause I wouldn’t eat the filthy motherfucker. Pigs sleep and root in shit. That’s a filthy animal. I ain’t eat nothin’ that ain’t got sense enough to disregard its own feces [sic]. Vincent: How about a dog? Dogs eats its own feces. Jules: I don’t eat dog either. Vincent: Yeah, but do you consider a dog to be a filthy animal? Jules: I wouldn’t go so far as to call a dog filthy but they’re definitely dirty. But, a dog’s got personality. Personality goes a long way. Vincent: Ah, so by that rationale, if a pig had a better personality, he would cease to be a filthy animal. Is that true? Jules: Well we’d have to be talkin’ about one charming motherfuckin’ pig. I mean he’d have to be ten times more charmin’ than that Arnold on Green Acres, you know what I’m sayin’? In the 1960s television show Green Acres, Arnold was an exceptional pig who was allowed to do whatever he wanted. He was talented enough to write his own name and play the piano and his attempts at painting earned him the nickname “Porky Picasso”. These talents reflected values that are appreciated, and so he was. The term “pig” is, however, chiefly used a term of abuse, however, embodying traits we abhor—gluttony, obstinence, squealing, foraging, rooting, wallowing. Making a pig of yourself is rarely honoured. Making a pig of the humanities, however, could be a different story. As a historian I love to forage, although I use white gloves rather than a snout. I have rubbed my face and body on tree trunks in the service of forestry history and when the temperature rises I also enjoy wallowing, rolling from side to side rather than drawing a conclusion. More than this, however, pigs provide a valid means of understanding key historical transitions that define modern society. Significant themes in modern history—production, religion, the body, science, power, the national state, colonialism, gender, consumption, migration, memory—can all be understood through a history of our relationships with pigs. Pigs play an important role in everyday life, but their relationship to the economic, social, political and cultural matters discussed in general history texts—industrialisation, the growth of nation states, colonialism, feminism and so on—are generally ignored. However “natural” this place of pigs may seem, culture and tradition profoundly shape their history and their own contribution to those forces has been largely absent in history. What, then, would the contours of such a history that considered the intermeshing of humans and pigs look like? The intermeshing of pigs in early human history Agricultural economies based on domestic animals began independently in different parts of the world, facilitating increases in population and migration. Evidence for long-term genetic continuity between modern and ancient Chinese domestic pigs has been established by DNA sequences. Larson et al. have made an argument for five additional independent domestications of indigenous wild boar populations: in India, South East Asia and Taiwan, which they use to develop a picture of both pig evolution and the development and spread of early farmers in the Far East. Domestication itself involves transformation into something useful to animals. In the process, humans became transformed. The importance of the Fertile Crescent in human history has been well established. The area is attributed as the site for a series of developments that have defined human history—urbanisation, writing, empires, and civilisation. Those developments have been supported by innovations in food production and animal husbandry. Pig, goats, sheep and cows were all domesticated very early in the Fertile Crescent and remain four of the world’s most important domesticated mammals (Diamond 141). Another study of ancient pig DNA has concluded that the earliest domesticated pigs in Europe, believed to be descended from European wild boar, were introduced from the Middle East. The research, by archaeologists at Durham University, sheds new light on the colonisation of Europe by early farmers, who brought their animals with them. Keith Dobney explains:Many archaeologists believe that farming spread through the diffusion of ideas and cultural exchange, not with the direct migration of people. However, the discovery and analysis of ancient Middle Eastern pig remains across Europe reveals that although cultural exchange did happen, Europe was definitely colonised by Middle Eastern farmers. A combination of rising population and possible climate change in the ‘fertile crescent’, which put pressure on land and resources, made them look for new places to settle, plant their crops and breed their animals and so they rapidly spread west into Europe (ctd in ScienceDaily). Middle Eastern farmers colonised Europe with pigs and in the process transformed human history. Identity as a porcine theme Religious restrictions on the consumption of pigs come from the same area. Such restrictions exist in Jewish dietary laws (Kashrut) and in Muslim dietary laws (Halal). The basis of dietary laws has been the subject of much scholarship (Soler). Economic and health and hygiene factors have been used to explain the development of dietary laws historically. The significance of dietary laws, however, and the importance attached to them can be related to other purposes in defining and expressing religious and cultural identity. Dietary laws and their observance may have been an important factor in sustaining Jewish identity despite the dispersal of Jews in foreign lands since biblical times. In those situations, where a person eats in the home of someone who does not keep kosher, the lack of knowledge about your host’s ingredients and the food preparation techniques make it very difficult to keep kosher. Dietary laws require a certain amount of discipline and self-control, and the ability to make distinctions between right and wrong, good and evil, pure and defiled, the sacred and the profane, in everyday life, thus elevating eating into a religious act. Alternatively, people who eat anything are often subject to moral judgments that may also lead to social stigmatisation and discrimination. One of the most powerful and persuasive discourses influencing current thinking about health and bodies is the construction of an ‘obesity epidemic’, critiqued by a range of authors (see for example, Wright & Harwood). As omnivores who appear indiscriminate when it comes to food, pigs provide an image of uncontrolled eating, made visible by the body as a “virtual confessor”, to use Elizabeth Grosz’s term. In Fat Pig, a production by the Sydney Theatre Company in 2006, women are reduced to being either fat pigs or shrieking shallow women. Fatuosity, a blog by PhD student Jackie Wykes drawing on her research on fat and sexual subjectivity, provides a review of the play to describe the misogyny involved: “It leaves no options for women—you can either be a lovely person but a fat pig who will end up alone; or you can be a shrill bitch but beautiful, and end up with an equally obnoxious and shallow male counterpart”. The elision of the divide between women and pigs enacted by such imagery also creates openings for new modes of analysis and new practices of intervention that further challenge humanist histories. Such interventions need to make visible other power relations embedded in assumptions about identity politics. Following the lead of feminists and postcolonial theorists who have challenged the binary oppositions central to western ideology and hierarchical power relations, critical animal theorists have also called into question the essentialist and dualist assumptions underpinning our views of animals (Best). A pig history of the humanities might restore the central role that pigs have played in human history and evolution, beyond their exploitation as food. Humans have constructed their story of the nature of pigs to suit themselves in terms that are specieist, racist, patriarchal and colonialist, and failed to grasp the connections between the oppression of humans and other animals. The past and the ways it is constructed through history reflect and shape contemporary conditions. In this sense, the past has a powerful impact on the present, and the way this is re-told, therefore, also needs to be situated, historicised and problematicised. The examination of history and society from the standpoint of (nonhuman) animals offers new insights on our relationships in the past, but it might also provide an alternative history that restores their agency and contributes to a different kind of future. Abstract:
Introduction: Conceptualizing DisabilityThe two most prominent models for understanding disability are the medical model and the social model (“Disability”). The medical model locates disability in the person and emphasises the possibility of a cure, reinforcing the idea that disability is the fault of the disabled person, their body, their genes, and/or their upbringing. The social model, formulated as a response to the medical model, presents disability as a failure of the surrounding environment to accommodate differently abled bodies and minds. Closely linked to identity politics, the social model argues that disability is not a defect to be fixed but a source of human experience and identity, and that to disregard the needs of people with disability is to discriminate against them by being “ableist.”Both models have limitations. On the one hand, simply being a person with disability or having any other minority identity/-ies does not by itself lead to exclusion and discrimination (Nocella 18); an element of social valuation must be present that goes beyond a mere numbers game. On the other hand, merely focusing on the social aspect neglects “the realities of sickness, suffering, and pain” that many people with disability experience (Mollow 196) and that cannot be substantially alleviated by any degree of social change. The body is irreducible to discourse and representation (Siebers 749). Disability exists only at the confluence of differently abled minds and bodies and unaccommodating social and physical environs. How a body “fits” (my word) its environment is the focus of the “ecosomatic paradigm” (Cella 574-75); one example is how the drastically different environment of Cormac McCarthy’s The Road (2006) reorients the coordinates of ability and impairment (Cella 582–84). I want to examine a novel that, conversely, features a change not in environment but in body.Alien LegsTim Farnsworth, the protagonist of Joshua Ferris’s second novel, The Unnamed (2010), is a high-powered New York lawyer who develops a condition that causes him to walk spontaneously without control over direction or duration. Tim suffers four periods of “walking,” during which his body could without warning stand up and walk at any time up to the point of exhaustion; each period grows increasingly longer with more frequent walks, until the fourth one ends in Tim’s death. As his wife, Jane, understands it, these forced excursions are “a hijacking of some obscure order of the body, the frightened soul inside the runaway train of mindless matter” (24). The direction is not random, for his legs follow roads and traffic lights. When Tim is exhausted, his legs abruptly stop, ceding control back to his conscious will, whence Tim usually calls Jane and then sleeps like a baby wherever he stops. She picks him up at all hours of the day and night.Contemporary critics note shades of Beckett in both the premise and title of the novel (“Young”; Adams), connections confirmed by Ferris (“Involuntary”); Ron Charles mentions the Poe story “The Man of the Crowd” (1845), but it seems only the compulsion to walk is similar. Ferris says he “was interested in writing about disease” (“Involuntary”), and disability is at the core of the novel; Tim more than once thinks bitterly to himself that the smug person without disability in front of him will one day fall ill and die, alluding to the universality of disability. His condition is detrimental to his work and life, and Stuart Murray explores how this reveals the ableist assumptions behind the idea of “productivity” in a post-industrial economy. In one humorous episode, Tim arrives unexpectedly (but volitionally) at a courtroom and has just finished requesting permission to join the proceedings when his legs take him out of the courtroom again; he barely has time to shout over his shoulder, “on second thought, Your Honor” (Ferris Unnamed 103). However, Murray does not discuss what is unique about Tim’s disability: it revolves around walking, the paradigmatic act of ability in popular culture, as connoted in the phrase “to stand up and walk.” This makes it difficult to understand Tim’s predicament solely in terms of either the medical or social model. He is able-bodied—in fact, we might say he is “over-able”—leading one doctor to label his condition “benign idiopathic perambulation” (41; my emphasis); yet the lack of agency in his walking precludes it from becoming a “pedestrian speech act” (de Certeau 98), walking that imbues space with semiotic value. It is difficult to imagine what changes society could make to neutralize Tim’s disability.The novel explores both avenues. At first, Tim adheres to the medical model protocol of seeking a diagnosis to facilitate treatment. He goes to every and any (pseudo)expert in search of “the One Guy” who can diagnose and, possibly, cure him (53), but none can; a paper in The New England Journal of Medicine documents psychiatrists and neurologists, finding nothing, kicking the can between them, “from the mind to body back to the mind” (101). Tim is driven to seek a diagnosis because, under the medical model, a diagnosis facilitates understanding, by others and by oneself. As the Farnsworths experience many times, it is surpassingly difficult to explain to others that one has a disease with no diagnosis or even name. Without a name, the disease may as well not exist, and even their daughter, Becka, doubts Tim at first. Only Jane is able to empathize with him based on her own experience of menopause, incomprehensible to men, gesturing towards the influence of sex on medical hermeneutics (Mollow 188–92). As the last hope of a diagnosis comes up empty, Tim shifts his mentality, attempting to understand his condition through an idiosyncratic idiom: experiencing “brain fog”, feeling “mentally unsticky”, and having “jangly” nerves, “hyperslogged” muscles, a “floaty” left side, and “bunched up” breathing—these, to him, are “the most precise descriptions” of his physical and mental state (126). “Name” something, “revealing nature’s mystery”, and one can “triumph over it”, he thinks at one point (212). But he is never able to eschew the drive toward understanding via naming, and his “deep metaphysical ache” (Burn 45) takes the form of a lament at misfortune, a genre traceable to the Book of Job.Short of crafting a life for Tim in which his family, friends, and work are meaningfully present yet detached enough in scheduling and physical space to accommodate his needs, the social model is insufficient to make sense of, let alone neutralize, his disability. Nonetheless, there are certain aspects of his experience that can be improved with social adjustments. Tim often ends his walks by sleeping wherever he stops, and he would benefit from sensitivity training for police officers and other authority figures; out of all the authority figures who he encounters, only one shows consideration for his safety, comfort, and mental well-being prior to addressing the illegality of his behaviour. And making the general public more aware of “modes of not knowing, unknowing, and failing to know”, in the words of Jack Halberstam (qtd. in McRuer and Johnson 152), would alleviate the plight not just of Tim but of all sufferers of undiagnosed diseases and people with (rare forms of) disability.After Tim leaves home and starts walking cross-country, he has to learn to deal with his disability without any support system. The solution he hits upon illustrates the ecosomatic paradigm: he buys camping gear and treats his walking as an endless hike. Neither “curing” his body nor asking accommodation of society, Tim’s tools mediate a fit between body and environs, and it more or less works. For Tim the involuntary nomad, “everywhere was a wilderness” (Ferris Unnamed 247).The Otherness of the BodyProblems arise when Tim tries to fight his legs. After despairing of a diagnosis, he internalises the struggle against the “somatic noncompliance” of his body (Mollow 197) and refers to it as “the other” (207). One through-line of the novel is a (failed) attempt to overcome cartesian duality (Reiffenrath). Tim divides his experiences along cartesian lines and actively tries to enhance while short-circuiting the body. He recites case law and tries to take up birdwatching to maintain his mind, but his body constantly stymies him, drawing his attention to its own needs. He keeps himself ill-clothed and -fed and spurns needed medical attention, only to find—on the brink of death—that his body has brought him to a hospital, and that he stops walking until he is cured and discharged. Tim’s early impression that his body has “a mind of its own” (44), a situation comparable to the Strange Case of Dr Jekyll and Mr Hyde (1886; Ludwigs 123–24), is borne out when it starts to silently speak to him, monosyllabically at first (“Food!” (207)), then progressing to simple sentences (“Leg is hurting” (213)) and sarcasm (“Deficiency of copper causes anemia, just so you know” (216)) before arriving at full-blown taunting:The other was the interrogator and he the muttering subject […].Q: Are you aware that you can be made to forget words, if certain neurons are suppressed from firing?A: Certain what?Q: And that by suppressing the firing of others, you can be made to forget what words mean entirely? Like the word Jane, for instance.A: Which?Q: And do you know that if I do this—[inaudible]A: Oof!Q: —you will flatline? And if I do this—[inaudible]A: Aaa, aaa…Q: —you will cease flatlining? (223–24; emphases and interpolations in original except for bracketed ellipsis)His Jobean lament turns literal, with his mind on God’s side and his body, “the other”, on the Devil’s in a battle for his eternal soul (Burn 46). Ironically, this “God talk” (Ferris Unnamed 248) finally gets Tim diagnosed with schizophrenia, and he receives medication that silences his body, if not stilling his legs. But when he is not medicated, his body can dominate his mind with multiple-page monologues.Not long after Tim’s mind and body reach a truce thanks to the camping gear and medication, Tim receives word on the west coast that Jane, in New York, has terminal cancer; he resolves to fight his end-of-walk “narcoleptic episodes” (12) to return to her—on foot. His body is not pleased, and it slowly falls apart as Tim fights it eastward cross-country. By the time he is hospitalized “ten miles as the crow flies from his final destination”, his ailments include “conjunctivitis”, “leg cramps”, “myositis”, “kidney failure”, “chafing and blisters”, “shingles”, “back pain”, “bug bites, ticks, fleas and lice”, “sun blisters”, “heatstroke and dehydration”, “rhabdomyolysis”, “excess [blood] potassium”, “splintering [leg] bones”, “burning tongue”, “[ballooning] heels”, “osteal complications”, “acute respiratory distress syndrome”, “excess fluid [in] his peritoneal cavity”, “brain swelling”, and a coma (278–80)—not including the fingers and toes lost to frostbite during an earlier period of walking. Nevertheless, he recovers and reunites with Jane, maintaining a holding pattern by returning to Jane’s hospital bedside after each walk.Jane recovers; the urgency having dissipated, Tim goes back on the road, confident that “he had proven long ago that there was no circumstance under which he could not walk if he put his mind to it” (303). A victory for mind over body? Not quite. The ending, Tim’s death scene, planned by Ferris from the beginning (Ferris “Tracking”), manages to grant victory to both mind and body without uniting them: his mind keeps working after physical death, but its last thought is of a “delicious […] cup of water” (310). Mind and body are two, but indivisible.Cartesian duality has relevance for other significant characters. The chain-smoking Detective Roy, assigned the case Tim is defending, later appears with oxygen tank in tow due to emphysema, yet he cannot quit smoking. What might have been a mere shortcut for characterization here carries physical consequences: the oxygen tank limits Roy’s movement and, one supposes, his investigative ability. After Jane recovers, Tim visits Frank Novovian, the security guard at his old law firm, and finds he has “gone fat [...] His retiring slouch behind the security post said there was no going back”; recognising Tim, Frank “lifted an inch off [his] chair, righting his jellied form, which immediately settled back into place” (297; my emphases). Frank’s physical state reflects the state of his career: settled. The mind-body antagonism is even more stark among Tim’s lawyer colleagues. Lev Wittig cannot become sexually aroused unless there is a “rare and extremely venomous snak[e]” in the room with no lights (145)—in direct contrast to his being a corporate tax specialist and the “dullest person you will ever meet” (141). And Mike Kronish famously once billed a twenty-seven-hour workday by crossing multiple time zones, but his apparent victory of mind over matter is undercut by his other notable achievement, being such a workaholic that his grown kids call him “Uncle Daddy” (148).Jane offers a more vexed case. While serving as Tim’s primary caretaker, she dreads the prospect of sacrificing the rest of her life for him. The pressures of the consciously maintaining her wedding vows directly affects her body. Besides succumbing to and recovering from alcoholism, she is twice tempted by the sexuality of other men; the second time, Tim calls her at the moment of truth to tell her the walking has returned, but instead of offering to pick him up, she says to him, “Come home” (195). As she later admits, asking him to do the impossible is a form of abandonment, and though causality is merely implied, Tim decides a day later not to return. Cartesian duality is similarly blurred in Jane’s fight against cancer. Prior to developing cancer, it is the pretence for Tim’s frequent office absences; she develops cancer; she fights it into remission not by relying on the clinical trial she undergoes, but because Tim’s impossible return inspires her; its remission removes the sense of urgency keeping Tim around, and he leaves; and he later learns that she dies from its recurrence. In multiple senses, Jane’s physical challenges are inextricable from her marriage commitment. Tim’s peripatetic condition affects both of them in homologous ways, gesturing towards the importance of disability studies for understanding the experience both of people with disability and of their caretakers.Becka copes with cartesian duality in the form of her obesity, and the way she does so sets an example for Tim. She gains weight during adolescence, around the time Tim starts walking uncontrollably, and despite her efforts she never loses weight. At first moody and depressed, she later channels her emotions into music, eventually going on tour. After one of her concerts, she tells Tim she has accepted her body, calling it “my one go-around,” freeing her from having to “hate yourself till the bitter end” (262) to instead enjoy her life and music. The idea of acceptance stays with Tim; whereas in previous episodes of walking he ignored the outside world—another example of reconceptualizing walking in the mode of disability—he pays attention to his surroundings on his journey back to New York, which is filled with descriptions of various geographical, meteorological, biological, and sociological phenomena, all while his body slowly breaks down. By the time he leaves home forever, he has acquired the habit of constant observation and the ability to enjoy things moment by moment. “Beauty, surprisingly, was everywhere” (279), he thinks. Invoking the figure of the flâneur, which Ferris had in mind when writing the novel (Ferris “Involuntary”), Peter Ferry argues that “becoming a 21st century incarnation of the flâneur gives Tim a greater sense of selfhood, a belief in the significance of his own existence within the increasingly chaotic and disorientating urban environment” (59). I concur, with two caveats: the chaotic and disorienting environment is not merely urban; and, contrary to Ferry’s claim that this regained selfhood is in contrast to “disintegrating” “conventional understandings of masculinity” (57), it instead incorporates Tim’s new identity as a person with disability.Conclusion: The Experience of DisabilityMore than specific insights into living with disability, the most important contribution of The Unnamed to disability studies is its exploration of the pure experience of disability. Ferris says, “I wanted to strip down this character to the very barest essentials and see what happens when sickness can’t go away and it can’t be answered by all [sic] of the medical technology that the country has at its disposal” (“Tracking”); by making Tim a wealthy lawyer with a caring family—removing common complicating socioeconomic factors of disability—and giving him an unprecedented impairment—removing all medical support and social services—Ferris depicts disability per se, illuminating the importance of disability studies for all people with(out) disability. After undergoing variegated experiences of pure disability, Tim “maintained a sound mind until the end. He was vigilant about periodic checkups and disciplined with his medication. He took care of himself as best he could, eating well however possible, sleeping when his body required it, […] and he persevered in this manner of living until his death” (Ferris Unnamed 306).
